Background The advantages of mobile smart healthcare in screening, intervention, treatment, and management of cardiovascular diseases have become increasingly prominent in recent years. The application value of wearable single lead electrocardiogram equipment in the early screening and diagnosis of arrhythmia patients out of the hospital has been recognized by scholars and clinical workers. However, its application in risk screening and long-term follow-up management of chronic disease patients in primary care is limited by the single function of the cloud platform led and matched by it. Objective To explore the application value of mobile smart healthcare in the prevention and control of cardiovascular disease risk in elderly patients with chronic disease in primary care from the perspective of chronic disease prevention and control and using wearable single lead electrocardiogram equipment as carriers, based on hierarchical medical system. Methods A total of 3 000 patients with chronic disease aged above 65 years admitted to primary care institutions in Ningxia from January 2022 to August 2022 were selected as research subjects, including 1 202 males and 1 798 females with an average age of (71.3±5.0) years. The baseline data were recorded by primary care physicians through mobile phone APP and cloud platform of patient management, 72-hour ECG data collected by wearable single lead electrocardiogram equipment were also uploaded to cloud platform of patient management. Analysis of data and risk stratification for arrhythmia, heart rate variability (HRV) and obstructive sleep apnea hypopnea syndrome (OSAHS) were performed by professional electrocardiographers, and the corresponding marks on the cloud platform for the data with low, medium and high risk of cardiovascular disease were ticked according to the stratification results of the three analysis methods. The cloud platform notifies primary care physicians of high and medium risk data in the form of SMS for the management of different processes of patients. The number of detected cases with different risk stratification, the number of detected cases and rates of management according to the process among patients with low, medium and high risk of cardiovascular disease were counted. Results Arrhythmias were detected in 1 526 (50.87%), 1 349 (44.97%), and 125 (4.17%) cases consistent with negativity, positivity, and significant positivity, respectively; HRV was mildly, moderately, and severely reduced in 2 330 (78.50%), 630 (21.23%), and 8 (0.27%) cases, respectively; OSAHS was consistent with mild, moderate, and severe abnormalities in 1 769 (59.60%), 573 (19.31%), and 626 (21.09%) cases, respectively. The comprehensive risk of cardiovascular disease was low, moderate, and high in 744 (24.80%), 1 640 (54.67%), and 616 (20.53%) patients, respectively. The rates of management according to the process in patients with high, medium and low comprehensive risk of cardiovascular disease were 94.49% (703 cases), 88.10% (1 445 cases) and 100% (616 cases), respectively. Conclusion The elderly patients with chronic disease in primary care with combined application of mobile smart healthcare technology and arrhythmia, HRV, OSAHS analysis methods to the prevention and control of cardiovascular diseases have higher detection rate of cardiovascular disease risk and management rate, which is conducive to the establishment of a cardiovascular disease prevention and control system for them.

A common complication of stroke patients is post-stroke cognitive impairment (PSCI) , which significantly reduces their quality of life. There are no effective targeted treatment measures currently available for PSCI in clinical practice. A large number of studies have already indicated that the activation of NLRP3 inflammasome plays a crucial role in PSCI, and many inhibitory treatments have been shown to improve cognitive impairment. The purpose of this study was to summarized the activation and modulating factors of NLRP3 inflammatory bodies and the relationship with PSCI. Some studies have been demonstrated that inhibiting NLRP3 or its associated inflammatory body components reduces the inflammatory response, promoting cognitive function recovery in cell and animal models of PSCI. Consequently, targeting NLRP3 inflammatory bodies may be a new trends of dealing with PSCI treatment. Despite the fact that numerous drugs and therapeutic measures have been proved to suppress the activation of NLRP3 inflammatory bodies, their clinical efficacy and safety have not yet been confirmed.

Despite the availability of various drugs for the treatment of diabetes worldwide, many patients still fail to achieve the recommended target blood sugar levels. Therefore, it is urgent to develop and apply new hypoglycemic drugs. In May 2022, FDA approved tirzepatide, a dual receptor agonist for GIP and GLP-1, administrated once a week to improve blood glucose control in adults with type 2 diabetes by assisting diet and exercise. Numerous clinical studies have proven its effectiveness and safety in type 2 diabetes, and its potential applications in obesity, cardiovascular diseases, non-alcoholic steatohepatitis and so on. In this paper, the mechanism of action, efficacy and safety of tirzepatide were reviewed to provide a reference for its clinical application after marketed in China.

Background Chronic obstructive pulmonary disease (COPD) is a common respiratory disease that is associated with high risk of disability and mortality. Although evidence suggests that fatigue may induce acute exacerbation in COPD, it needs to be further tested. Objective To explore the relationship between fatigue and the acute exacerbation in COPD. Methods By use of convenient sampling, a total of 597 COPD patients were selected from the First Affiliated Hospital of Bengbu Medical College, the Second Affiliated Hospital of Bengbu Medical College and the Third People's Hospital of Bengbu to establish a cohort study population between March 2019 and October 2020. During the hospitalization, general information such as gender, age, educational level, marital status, living style, comorbidities, smoking history, and monthly family income were collected from patients. Fatigue Scale-14 was used to measure the fatigue status, BODE index was used to predict the severity and prognosis, Anxiety and depression were assessed by the Anxiety Scale (HADS-A) and the Depression Scale (HADS-D) in the Hospital Anxiety and Depression Scale (HADS) . The patients were followed up for 1 year after discharge, recorded the occurrence of acute exacerbation in COPD, and assessed the quality of life with COPD Assessment Test (CAT) . Univariate analysis and multivariate Cox regression analysis were performed to identity factors possibly associated with the acute exacerbation in COPD. ROC curve analysis was used to measure the performance of fatigue in predicting acute exacerbation in COPD. Results The lost cases were screened and eliminated according to the corresponding inclusion criteria and exclusion criteria, five hundred and fifty cases were finally included for analysis, 416 of them had fatigue, and other 134 did not. Background and purpose: The treatment-related cardiovascular disease in patients with breast cancer poses a greater mortality threat than cancer itself. To assist clinical oncologist, we introduced a scale to help clinicians by screening out patients with breast cancer who were at risk of cardiovascular diseases. The breast cancer patients in Zhongshan Hospital, Fudan University were analyzed retrospectively by the scale. Methods: This was a single center, retrospective trial. A total of 760 patients who met the inclusion and exclusion criteria were enrolled in this trial from January 1, 2017 to December 31, 2018. The incidence rate of cardiovascular risk was collected from patients eligible for admission. Secondly, follow-up information for each patient was evaluated by the scale and the clinician. The repeatability, sensitivity, specificity and consistency of the scale were evaluated. Results: The average follow-up duration was (746.55±309.94) d, and the average population age was (56.60±12.62) years. A total of 36 patients developed severe arrhythmias during or after the treatment, 8 patients with emerging myocardial injury and 6 patients with emerging cardiac insufficiency. The follow-up rates of patients with test for NT-proBNP from half a year to one year and more than one year after baseline were 34.31% and 29.62%, while 35.48% and 32.48% for those who took echocardiography. The sensitivity, specificity and overall accuracy of medium-and high-risk patient tests were 0.828, 0.934 and 0.921, respectively. The Eastern Kunlun Orogenic Belt (EKOB), located north of the Qinghai-Tibetan Plateau, recorded voluminous late Palaeozoic – early Mesozoic intermediate-acid volcanic rocks. The tectonic evolution of the Palaeo-Tethys Ocean from initial subduction and final closure and post-collision extension has been hotly debated. This study examines the petrogenesis of Palaeozoic to Early Mesozoic volcanic rocks at six locations: Xiahe, Elashankou, Jirimai, Harizha, Nagengkangqieer, and Yazigounan. The investigation involves detailed field observations, petrology, zircon U-Pb geochronology, Lu-Hf isotopes, and whole-rock geochemical analysis. The rocks analysed represent three episodic volcanic events. The ca. 257–245 Ma volcanic rocks were derived from magma mixing response to the subduction. The ca. 227–220 Ma highly fractionated I-type volcanics were derived from partial melting of the ancient lower crust, with varying contribution proportions of mantle materials. The ca. 216 Ma A-type volcanic rocks derived from partial melting of the juvenile crust. In this study, using existing regional geological data and spatiotemporal distribution of the magmatic region, combined with our new data, we attempt to propose more rational models to reveal the tectonic evolution of the Palaeo-Tethys Ocean, and we recognize three significant time nodes: (1) Ca. 240 Ma marks the final closure time of the Palaeo-Tethys Ocean. (2) Ca. 230 Ma indicates the vital tectonic regime transition from compression to extension response to slab break-off and continued lithospheric delamination. To provide some references for researchers engaged in infrared multi gas detection, this study introduced the infrared multi gas detection system thoroughly from infrared light source, infrared detector, optical multiplexing structure, and detection method. Currently, the coherent source represented by quantum cascade laser has replaced the traditional incoherent source like thermal radiant infrared light source and became the dominant light source in infrared multi gas detection. Accordingly, the infrared photodetector is widely used. The optical multiplexing structure based on the "multiplexing thought" is the core of infrared multi gas detection system. It mainly includes the single-source multiplex detection structure and multi-source multiplexing detection structure. Nondispersive infrared spectroscopy, long optical distance spectroscopy, wavelength/frequency modulation spectroscopy, cavity enhancement spectroscopy, and photoacoustic spectroscopy are major detection methods used in the infrared multi gas detection. Rhizobia preferentially enter legume root hairs via infection threads, after which root hairs undergo tip swelling, branching, and curling. However, the mechanisms underlying such root hair deformation are poorly understood. Here, we showed that a type II small GTPase, ROP10, of Medicago truncatula is localized at the plasma membrane (PM) of root hair tips to regulate root hair tip growth. Overexpression of ROP10 and a constitutively active mutant (ROP10CA) generated depolarized growth of root hairs, whereas a dominant negative mutant (ROP10DN) inhibited root hair elongation. Inoculated with Sinorhizobium meliloti , the depolarized swollen and ballooning root hairs exhibited extensive root hair deformation and aberrant infection symptoms. Upon treatment with rhizobia-secreted nodulation factors (NFs), ROP10 was transiently upregulated in root hairs, and ROP10 fused to green fluorescent protein was ectopically localized at the PM of NF -induced outgrowths and curls around rhizobia. ROP10 interacted with the kinase domain of the NF receptor NFP in a GTP-dependent manner. Moreover, NF -induced expression of the early nodulin gene ENOD11 was enhanced by the overexpression of ROP10 and ROP10CA. These data suggest that NFs spatiotemporally regulate ROP10 localization and activity at the PM of root hair tips and that interactions between ROP10 and NF receptors are required for root hair deformation and continuous curling during rhizobial infection. With analysis of the features of decision-making information fusion and actual emergency decision making, this paper presents a decision-making information fusion method based on distance entropy according to the demand for objectivity and scientificity. Firstly, the fusion set is acquired and stored based on knowledge unit model. Secondly, the information units are formed through instantiating knowledge units, which can provide information support for the fusion. Afterwards, the concept of distance entropy is proposed with an element of distance added into the traditional entropy weight method. Thus, we can obtain both local and global fusion weight, finally to gain the global fusion results using the way of linear weighting. Furthermore, the application value and effectiveness of this method are illustrated by empirical analysis. Experimental analysis show that the proposed method can solve problems of the hugeness of knowledge base and improve the objectivity and scientificity of fusion results in decision-making information fusion. In single machine scheduling with deteriorating processing time, we study the problem of dealing with the arrival of multiple unexpected orders. We build up the bi-objective model where original objective is based on system operational cost, while the deviation objective is based on the delay of job's completion time with respect to its original completion time. In order to effectively solve the model, we combine simulated annealing-based multi-objective optimization algorithm, which is good at jumping out of local optimality, with non-dominated sorting genetic algorithm, which is good at fast converging to Pareto front. And we design a hybrid algorithm to balance between exploration and exploitation. By analyzing the Pareto optimal property, we could further effectively narrow the searching space of hybrid algorithm, speeding up convergence and improving Pareto front quality. Finally, by randomly generating and solving numerical problem instances, we show that our hybrid algorithm is effective for the disruption management problem, and Pareto optimal property could significantly improve the performance of hybrid algorithm.
