3. The lived experience of the home educator

Abstract

This qualitative study examines the experiences of twelve home educators from the North Carolina, USA. The primary data collection method was semi-structured interviews, supplemented by visual collages which were completed by participants. Data were coded and analysed using NVivo 12 software. The research resulted in nine major themes which emerged out of the experiences shared by the participants: 1) Structure, 2) Curriculum, 3) Personal Fulfilment, 4) Personal Struggles, 5) Sought Advantages), 6) Reasons for Home Educating, 7) Reflection, 8) Support, and 9) The Decision to Homeschool. I analysed these themes from the participants’ experiences through the lens of inductive thematic analysis. The experiences associated with the home educators in this study serve as a framework for discussion of the way in which home education is experienced by its practitioners and the perceived value home education has in their lives. The research revealed that control is a fundamental necessity for home educators. Additionally, the findings reveal that home educators craft the lifestyle and practice of home education to match their own capacity, desire and lifestyle. Recommendations are made for home educators, traditional educators and further study.

6. An Initial Validation of a New Quality of Life Measure for Adults with Intellectual Disability: The Mini-MANS-LD

Abstract

There is an appropriate increasing focus on the need to ensure the voices of people with intellectual disability are captured as part of assessing individuals' quality of life; however, there remains a lack of a consensus on ways to achieve this. This article describes the development of a self-report measure of quality of life for people with intellectual disability, the 'Mini-MANS-LD', based on the concepts of Maslow's hierarchy of needs. Following use with 33 individuals with intellectual disability, the Mini-MANS-LD was found to have acceptable psychometric properties, including moderate congruent validity and acceptable internal consistency. Administrators' feedback suggested good acceptability and feasibility, and the measure was relatively quick to administer, easy to use and acceptable to service users. Despite a small sample size, this initial study suggests that the Mini-MANS-LD may present a conceptually relevant, feasible and acceptable self-report measure of quality of life for people with intellectual disability.

7. Effect of valenced stimuli on hippocampus-dependent spatial memory in depression

Abstract

This thesis focused on neurobiological changes in depression. Part 1 reviewed research investigating the relationship between trauma in childhood, depression in adulthood and dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis. The reviewed studies provided some evidence for associated changes to the HPA axis, however, there was inconsistency in findings as to whether this was linked to childhood trauma or depression in adulthood independently, or an interaction of the two. The studies provided tentative evidence for a specific sub-type of depression, associated with a history of childhood trauma, but study heterogeneity and lack of replication mean further research is needed. Part 2 reported an investigation into the effects on hippocampus-dependent spatial memory of using valenced stimuli (neutral, negative) for both people with depression and healthy controls. No influence of valence on memory performance was found, counter to the hypotheses. However, some interesting effects of sex were found, warranting further investigation. Potential reasons for the null results were explored, along with suggestions for further research to allow these reasons to be examined. This investigation formed part of a joint study with Line Sagfors (trainee clinical psychologist, UCL; Sagfors, 2017). Part 3 reflected on some of the methodological issues and learnings from the major research project, including adapting an existing spatial memory test and working with a mental health service and clinical population. It explored the advantages of conducting joint DClinPsy research, both practical and emotional, and ended with some brief reflections on the challenges of the literature review.

Abstract

Approximately 30% of triple-negative breast cancers (TNBC) harbor molecular alterations in PI3K/mTOR signaling, but therapeutic inhibition of this pathway has not been effective. We hypothesized that intrinsic resistance to TORC1/2 inhibition is driven by cancer stem cell (CSC)-like populations that could be targeted to enhance the antitumor action of these drugs. Therefore, we investigated the molecular mechanisms by which PI3K/mTOR inhibitors affect the stem-like properties of TNBC cells. Treatment of established TNBC cell lines with a PI3K/mTOR inhibitor or a TORC1/2 inhibitor increased the expression of CSC markers and mammosphere formation. A CSC-specific PCR array revealed that inhibition of TORC1/2 increased FGF1 and Notch1 expression. Notch1 activity was also induced in TNBC cells treated with TORC1/2 inhibitors and associated with increased mitochondrial metabolism and FGFR1 signaling. Notably, genetic and pharmacologic blockade of Notch1 abrogated the increase in CSC markers, mammosphere formation, and in vivo tumor-initiating capacity induced by TORC1/2 inhibition. These results suggest that targeting the FGFR-mitochondrial metabolism-Notch1 axis prevents resistance to TORC1/2 inhibitors by eradicating drug-resistant CSCs in TNBC, and may thus represent an attractive therapeutic strategy to improve drug responsiveness and efficacy.

23. Impact of the COVID-19 Pandemic on Stroke Subtype Presentation in Patients Without COVID-19 Infection

Abstract

ABSTRACT:Background & Aims:It is unknown if the COVID-19 pandemic and public health measures had an immediate impact on stroke subtypes and etiologies in patients not infected with COVID-19. We aimed to evaluate if the proportion of non-COVID-19-related stroke subtypes (ischemic vs. hemorrhagic) and etiologies (cardioembolic, atherosclerosis, small vessel disease, and others) during the pandemic’s first wave were different from prepandemic.Methods:For this retrospective cohort study, we included patients without COVID-19 with ischemic or hemorrhagic stroke at two large Canadian stroke centers between March–May 2019 (prepandemic cohort) and March–May 2020 (pandemic cohort). Proportions of stroke subtypes and etiologies were compared between cohorts using chi-square tests.Results:The prepandemic cohort consisted of 234 stroke patients and the pandemic cohort of 207 stroke patients. There were no major differences in baseline characteristics. The proportions of ischemic versus hemorrhagic stroke were similar (ischemic stroke: 77% prepandemic vs. 75% pandemic; hemorrhagic stroke:12% prepandemic vs. 14% pandemic; p> 0.05). There were no differences in etiologies, except for a decreased proportion of ischemic stroke due to atherosclerosis in the pandemic cohort (26% prepandemic vs. 15% pandemic; difference: 10.6%, 95%CI: 1.4-19.7; p= 0.03). Notably, during the pandemic, the cause of ischemic stroke was more often unknown because of incomplete work-up (13.3% prepandemic vs. 28.2% pandemic, difference: 14.9%, 95%-CI: 5.7–24.2; p= <0.01).Conclusions:In this study, the pandemic had no clear effect on stroke subtypes and etiologies suggesting a limited impact of the pandemic on stroke triggers. However, the shift from atherosclerosis toward other causes warrants further exploration.

"February 2006".Project Number: Development of a Conceptual Model for the Development and Maintenance of Blue-Green Algal Blooms in Lake Hume Including the Preparation of a Draft Monitoring Plan for the Dam - M/BUS/94.MDFRC item.42 pages.Between 2002 and 2004 there were a number of high alert blue-green algal (cyanobacterial) blooms in Lake Hume (i.e. >15,000 cells/ml). This report examines the factors that might be controlling blue-green algal blooms in the Lake, with an intent to develop a conceptual model that links the incidence of blue-green algal blooms to lake or catchment condition. Although many of the factors that are thought to promote blue-green algal blooms occur in Lake Hume, actual blue-green algal blooms aren’t very common. There have been a few recorded incidences of blooms in the lake – e.g. immediately following commissioning of the dam, in 1973, 1981-2 and 2002-04. The last three periods coincided with severe droughts in southeastern Australia. The available data are insufficient to be able to unequivocally assign causation of blue-green algal blooms in Lake Hume. However we propose a model that links sediment chemistry in the lake to bioavailable N:P ratios in the surface waters. Under non-drought conditions we suggest that the surface N:P ratios favour algae other than blue-green algae. Under drought conditions the N:P ratio decreases, leading to bluegreen algal blooms. We propose a monitoring and research strategy to test this model for Lake Hume. Specifically: - We recommend fortnightly depth-integrated samples be taken within the mixed layer for at least two fixed sites in the dam using the methodology and data reporting outlined in Matveev and Matveeva (2005); We recommend that the loads of phosphorus (total and filterable reactive), nitrogen (total, ammonia and oxides of nitrogen) and carbon (dissolved organic carbon and fine particulate organic matter) entering and leaving the lake are determined; We recommend that the phosphorus and nitrogen species (see above) and dissolved oxygen are monitored at least 4 sites within the lake (Murray, Mitta Mitta and Bowna arms and near the wall) both above and below any thermocline, on a fortnightly basis; We recommend that a three dimensional hydrodynamic model of Lake Hume should be developed In addition, we also make a number of suggestions regarding future research to fill identified knowledge gaps.

"December 2005".Project Number: Broken River Monitoring Review - M/BUS/88.MDFRC item.120 pages.The mission of the Goulburn Broken Catchment Management Authority (GBCMA) is the cost-effective protection and enhancement of the Catchment's land and water resources to improve social well-being, environmental quality and the sustainable productive capacity of the catchment. Specifically, the GBCMA seeks to protect and enhance natural environmental assets and their ecosystem processes and functions in a way that provides benefits for native biodiversity and the social and economic aspects of the catchment. As part of this goal the GBCMA has commissioned MDFRC to undertake a review of monitoring and research programs on the Broken River, which will contribute to the “‘Mending the Broken: Improving Flows and Habitat in the Broken River” and “Our Water –Our Future“ initiatives. This report reviews existing monitoring activities in the Broken River and Broken Creek catchments, identifies the strengths and weaknesses of these monitoring activities and their capacity to assess changes to stream health resulting from management activities. This information will provide a basis for the design of a comprehensive monitoring program of the Broken River and Broken Creek catchments. Currently, a variety of parameters are measured in monitoring programs within the Broken River and Broken Creek catchments by a number of different agencies. These parameters can be summarised into the following broad groups: Water Quality; Water Quantity; Biodiversity; Algal /Bacterial Populations; Habitat Variables; Climate Variables; The agencies collecting monitoring data operate under various primary objectives. These objectives influence the parameters measured, sites chosen, monitoring frequency, monitoring duration, the resolution of measurements, the statistical robustness of the project design and the comparability of the data with that from other projects. The GBCMA shares some of the objectives of each monitoring agency, but not all. In turn, this influences the applicability of the data to GBCMA’s monitoring objectives. There is great variation in the data sets in terms of spatial and temporal distribution, continuity, quality assurance and duration of data collection. Some data provides an indication of the site health for a discrete time period (snap shots) while other data is more useful for determining changes over time and space (long term monitoring). Analytical methods and data analysis also differ between agencies. The standard of quality assurance imposed by a monitoring program is of great importance when data from a variety of sources is to be used to assess ecological condition, particularly when it is to be put to a use other than that for which it was initially intended.When the available data is evaluated in terms of the data required for the assessment of future GBCMA management works, it becomes obvious that there are several areas where the data are inadequate. These knowledge gaps are most prominent in the areas of wetland biodiversity and water quality, stream geomorphology and hydraulics and in-stream and riparian habitat assessment. Population and habitat data for icon species and pest species in the Broken catchment are also lacking. The applicability of the existing data to GBCMA’s monitoring objectives will ultimately depend on GBCMA’s specific monitoring requirements, which will vary depending on the reach, management works and expected outcomes. Once specific monitoring requirements are determined the applicability of the existing data may also be determined. GBCMA may benefit from supporting and/or implementing a suite of monitoring programs/projects that are aimed at continuing the collection of long term data, improving the spatial distribution of data, assessing the efficacy of rehabilitation works, and filling knowledge gaps. This suite of monitoring data should include the following types for specific purposes: Descriptive Monitoring – detect change, predict problems, characterise sites; Analytical Monitoring - determine whether management interventions are achieving their objectives and improving knowledge of system response. Where possible, design should be based on conceptual models and hypotheses of how the works influence the system; and Scientific Research (e.g. PhD projects) to fill knowledge gaps. Several conceptual models based on the objectives of the GBCMA for environmental health in the Broken River and Broken Creek catchments conclude this report. Conceptual models are provided as a means of designing monitoring programs and assessing the environmental variables of most direct relevance to restoration works. They can also be instrumental in detecting knowledge gaps, which is information that will be critical for the second phase of the ‘Mending the Broken’ initiative - design of an appropriate monitoring program.

Murray-Darling Freshwater Research CentreMDFRC item.The blackwater model was developed to predict adverse water quality associated with flooding of the Barmah-Millewa Forests on the River Murray. Specifically, the model examines the likelihood and severity of blackwater events-high dissolved organic carbon associated with low dissolved oxygen. The Barmah-Millewa Forests are dominated by an overstorey of River Red Gum (Eucalyptus camaldulensis) and the litter from these trees contributes a substantial proportion of the pulse of dissolved organic matter released from the floodplain during flooding. This model examines rates of litter accumulation and decay on the floodplain (prior to and during flooding), rates of carbon leaching, microbial degradation, oxygen consumption, reaeration processes and the effects of flow on the concentrations of dissolved organic carbon and dissolved oxygen in the water column (both on the floodplain and in the river channel downstream). The model has been calibrated with data from two blackwater events that have taken place in these forests within the last 5 years. Scenario testing with the model highlights the particularly important roles of flow and temperature in the development of anoxia. Pooled floods and those in the warmest months of the year are substantially more likely to result in blackwater events than floods in cooler times of the year and involving more water exchange between the river channel and the floodplain.

Abstract

Background: Nurse practitioner (NP) faculty assess student acquisition of knowledge through examinations, simulation, and clinical performance. Problem: Developing appropriately leveled curriculum, assessments, and clinical expectations that accurately capture student maturation presents a challenge. Approach: The Reporter, Interpreter, Manager, Educator (RIME) provided the framework for doctor of nursing practice NP curriculum redesign to enhance student performance and content mastery. Faculty used a gap analysis approach, iteratively leveling specialty content, course competencies, examination questions, simulation cases, and clinical expectations using the building blocks of RIME. Outcomes: Objective scores on student evaluations for clinical courses exceeded the threshold established, including 83% of simulation encounters. Faculty implemented targeted methods to remediate areas of underperformance. Conclusions: Structuring the course competencies and preceptor feedback around RIME made it easier to pinpoint specific deficiencies and target remediation. It also helped guide discussions about the minimum acceptable standard for student performance. [ABSTRACT FROM AUTHOR]
