PURPOSE: To investigate the association between papillary thyroid cancer (PTC) and Hashimoto’s thyroiditis (HT). DESIGN: This study is a retrospective study that conducted during 7 consecutive years with a median 119.5 months follow-up. SUBJECTS AND METHOD: Patients who underwent thyroidectomy in Dokuz Eylül University Hospital during 7 consecutive years were included. Patients’ demographics, biochemical, radiological, and pathological results were retrospectively assessed. RESULTS: Four hundred sixty nine patients were evaluated. Among 469 patients who underwent thyroidectomy, 132 (28.1%) were malignant, while 182 patients were diagnosed with HT (38.8%). PTC was ranked first at 92.4% (n: 122). 9. THE USE OF LOW DOSE PREDNISOLONE IN PATIENTS WITH SUBACUTE THYROIDITIS AND ITS EFFECT ON IMPAIRED LIFE AND SLEEP QUALITY

CONTEXT: Subacute thyroiditis is an inflammatory thyroid disease, which is treated by nonsteroidal anti-inflammatory drugs (NSAIDs) or steroids. OBJECTIVE: Defining characteristics of patients with subacute thyroiditis at diagnosis and during follow-up. Investigating the efficacies of NSAID and different doses of steroids and their effects on rates of relapse, recurrence, development of hypothyroidism and on quality of life and sleep parameters. DESIGN: A 3-year observational study in a tertiary referral center. SUBJECTS AND METHODS: A total of 63 patients with subacute thyroiditis were included. Clinical outcomes of patients treated with NSAIDs and NSAID unresponsive patients treated with prednisolone with initial doses of 0.5 mg/kg/day and 15 mg/day were evaluated. RESULTS: White blood cell count at diagnosis was an independent predictor of NSAID unresponsiveness. No relapse or recurrence was observed in patients receiving low dose of steroids. Long symptom duration until diagnosis and treatment with NSAIDs were associated with development of hypothyroidism. Subacute thyroiditis caused significant deterioration in quality of life and sleep of patients and low dose of steroid was as effective as higher doses in improving these parameters. CONCLUSIONS: For patients with no response to NSAID therapy, an initial low dose of prednisolone (15 mg/day) is determined as a safe treatment method when dose reduction is performed with appropriate timing.

- Abstract
Milk samples were collected in early lactation (8–14 days in milk; DIM) and late lactation (199–326 DIM) from 11 cows to assess the differences in bovine milk fatty acid (FA) and triacylglycerol (TAG) composition, FA positional distribution in the TAG structure, and milk solid fat content (SFC). Cows in early lactation that were in negative energy balance, mobilised C16:0, C18:0, and C18:1cis9 from their body fat stores, increasing the concentrations of these FA in milk fat. These high concentrations of C18:0 and C18:1cis9 enhanced the synthesis of high molecular weight TAG and decreased milk fat SFC in early lactation. For both lactation stages, alterations in the total FA concentrations in the TAG structures resulted in changes in the abundances and the proportions of the FA at the sn-2 and sn-1 (3) positions in the TAG structure, yet without affecting the main esterification preferences of the FA.

- Abstract
Triacylglycerols (TAG) composition and structure in milk fat determine the physical, nutritional and functional characteristics in milk fat. TAG are composed of three fatty acids (FA) bound to a glycerol molecule. Therefore, changes in milk FA composition are expected to influence milk fat TAG composition and structure. Feed and animal related factors are known to affect milk FA composition. Feed related factors refer to alteration in the dairy cows’ feeding regimes, whereas animal related factors refer to factors such as lactation stage, breed, and genetics. This thesis aimed to assess the effect of feed and animal related factors on the variation in FA and TAG composition and structure and to determine its influence on the milk fat solid fat content (SFC). Milk fat SFC is one of the physical properties that define the suitability of a fat for a specific food product application. Therefore, it was selected as the physical property of study in this thesis to highlight the effect of milk FA and TAG variations.In chapters 2 and 3, the effect of the seasonal feeding regimes on the FA and TAG composition and structure were assessed in a non-seasonal milking system. Both chapters were based on weekly-pooled milk samples collected from fourteen dairy factories in the Netherlands, during one year, from May 2017 to April 2018. Seasonal FA and TAG composition variation were presented in chapter 2, where the largest FA variations were found between summer and winter, while spring and autumn were considered transition periods. The FA variations included a smaller proportion of de novo synthesized FA and an increased proportion of biohydrogenation products and conjugated linoleic acid (CLA) isomer FA in spring and summer. Moreover, the proportion of linolenic acid (ALA) in milk fat was greatest during late spring and summer, reflecting the increased intake of fresh pasture in these periods. - Abstract
The oilseed crops Crambe abyssinica and Camelina sativa produce oils rich in erucic acid and n-3 polyunsaturated fatty acids (PUFA), respectively. After pressing the oil, a seed cake remains as a protein-rich by-product. Edible insects may convert this seed press cake and the defatted seed meal produced from it into insect biomass suitable for animal feed. Black soldier fly larvae (BSF, Hermetia illucens) can grow on a wide range of organic waste types, but may be hindered by excess protein or the plant toxins characteristic for these two oilseed crops, i.e. glucosinolates and their breakdown products. We tested the effects of 25, 50 and 100% oilseed by-product inclusion in the diet on survival, development, biomass production and fatty acid composition of BSF larvae. Larval performance on diets with up to 50% camelina by-product or 25% crambe by-product was similar to performance on control diet (chicken feed), and decreased with higher inclusion percentages. Larval fatty acid profiles differed significantly among diets, with larvae fed press cake more distinct from control than larvae fed seed meal. Larvae fed camelina press cake had more α-linolenic acid, whereas larvae fed crambe contained most oleic acid. The n-6:n-3 PUFA ratio decreased with increasing proportion of by-product, especially on camelina diets. Lauric acid content was highest in larvae fed 100% camelina meal or 50% crambe meal. These results indicate that BSF larvae can be successfully grown on diets with camelina or crambe oilseed by-products, and that the resulting larval n-6:n-3 PUFA ratio is favourable for animal feed. However, the fate of glucosinolates and their derivatives remains to be determined, to guarantee chemical safety of camelinaor crambe-fed BSF larvae for animal feed.

- Abstract
Trees that we see every day stay firm in the same place for several years. They are the symbol of nature and one of the most important living beings which are a bridge between the past, the present and the future. According to Andres [1], the monumental trees differ from others of their species regarding their unusual size, their age, or their unusual shape, which means that they are of special natural, historical, cultural, or scenic interest. Characteristics such as their height, trunk diameter, crown shape, or their having witnessed historical events or appearing in myths and legends make these trees of public interest and therefore protected by law. The veteran trees are also different from other trees with their ages, unusual size, unusual shape etc. They can be seen as a candidate for monumental trees. Istanbul, the biggest metropolitan city of Turkey, is particularly rich in monumental and veteran trees. In the study of IMM, the monumental trees and veteran trees of Istanbul city have been identificated and their locations have been stated. According to this study the monumental and veteran plane trees in the urban areas of the city are of special interest while they are of scenic value (throughout the history?!) but also have unwanted features like being nontolerant to arid conditions and also to various diseases [2]. Firstly, in this study the spatial distributions of the two monumental and veteran plane species (Platanus acerifolia, Platanus orientalis) in Istanbul have been revealed. - Abstract
Background: Obesity is the main obstacle for metabolic control in patients with type 2 diabetes. Turkey has the highest prevalence of obesity and type 2 diabetes in Europe. The effect of obesity on the metabolic control, and the macro-and microvascular complications of patients are not apparent. Objectives: This nationwide survey aimed to investigate the prevalence of overweight and obesity among patients with type 2 diabetes and to search for the impact of obesity on the metabolic control of these patients. We also investigated the independent associates of obesity in patients with type 2 diabetes. Methods: We consecutively enrolled patients who were under follow-up for at least 1 year in 69 tertiary healthcare units in 37 cities. The demographic, anthropometric, and clinical data including medications were recorded. Patients were excluded if they were pregnant, younger than 18 years, had decompensated liver disease, psychiatric disorders interfering with cognition or compliance, had bariatric surgery, or were undergoing renal replacement therapy. Results: Only 10% of patients with type 2 diabetes (n = 4,648) had normal body mass indexes (BMI), while the others were affected by overweight (31%) or obesity (59%). Women had a significantly higher prevalence of obesity (53.4 vs. 40%) and severe obesity (16.6 vs. 3.3%). Significant associations were present between high BMI levels and lower education levels, intake of insulin, antihypertensives and statins, poor metabolic control, or the presence of microvascular complications. Age, gender, level of education, smoking, and physical inactivity were the independent associates of obesity in patients with type 2 diabetes. Conclusion: The TEMD Obesity Study shows that obesity is a major determinant of the poor metabolic control in patients with type 2 diabetes. These results underline the importance of prevention and management of obesity to improve health care in patients with type 2 diabetes. Also, the results point out the independent sociodemographic and clinical associates of obesity, which should be the prior targets to overcome, in the national fight with obesity. BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. Aims: Turkey has the highest prevalence of diabetes in Europe. It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. Methods: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c < 7%, home arterial blood pressure (ABP) < 135/85 mmHg, or LDL-C < 100 mg/dL. Achieving all parameters indicated triple metabolic control. Results: HbA1c levels of patients (n = 5211) were 8.6 ± 1.9% (71 ± 22 mmol/mol) and 7.7 ± 1.7% (61 ± 19 mmol/mol), in Type 1 and Type 2 diabetes, respectively. Glycemic control was achieved in 15.3% and 40.2%, and triple metabolic control was achieved in 5.5% and 10.1%, respectively. Only 1.5% of patients met all the criteria of being non-obese, non-smoker, exercising, and under triple metabolic control. Low education level was a significant predictor of poor glycemic control in both groups. Conclusions: Few patients with Type 2, and even fewer with Type 1 diabetes have optimal metabolic control in Turkey. TEMD study will provide evidence-based information to policy makers to focus more on the quality and sustainability of diabetes care in order to reduce the national burden of the disease. © 2018 Elsevier B.V.

International audience; In this study, we demonstrated the suitability of direct injection mass spectrometry headspace analysis for rapid non-invasive quality control of semi-finished dairy ingredients, such as skim milk powder (SMP), whole milk powder (WMP), whey powder (WP) and anhydrous milk fat (AMF), which are widely used as ingredients in the food industry. In this work, for the first time, we applied proton transfer reaction-mass spectrometry (PTR-MS) with a time-of-flight (ToF) analyzer for the rapid and non-invasive analysis of volatile compounds in different samples of SMP, WMP, WP and AMF. We selected different dairy ingredients in various concrete situations (e.g. same producer and different expiration times, different producers and same days of storage, different producers) based on their sensory evaluation. PTR-ToF-MS allowed the separation and characterization of different samples based on the volatile organic compound (VOC) profiles. Statistically significant differences in VOC content were generally coherent with differences in sensory evaluation, particularly for SMP, WMP and WP. The good separation of SMP samples from WMP samples suggested the possible application of PTR-ToF-MS to detect possible cases of adulteration of dairy ingredients for the food industry. Our findings demonstrate the efficient and rapid differentiation of dairy ingredients on the basis of the released VOCs via PTR-ToF-MS analysis and suggest this method as a versatile tool (1) for the facilitation/optimization of the selection of dairy ingredients in the food industry and (2) and for the prompt innovation in the production of dairy ingredients

In this work, a new memristor-based low-pass filter topology is suggested. Memristor-based filters are nonlinear circuits and many of them cannot be solved analytically. To find an approximate analytical solution of the filter equations, MacLaurin series is used for the first time. Time and frequency domain behavior of the filter are obtained. Also, the frequency domain gain and total harmonic distortion characteristics of the filter are given. Results obtained by the small signal model have been compared with the dynamic model results. It is shown that the small signal model is able to predict the filter behavior very well.

In 1971, Leon Chua theoretically postulated that the memristor is the fourth fundamental circuit element, besides the three well-known circuit elements; namely, resistor, capacitor and inductor. For a long time, because of lack of a simple and practical realization, memristor remained just a theoretical element and rarely appeared in the literature. In 2008, a research team from HP laboratories declared that they had found a physical implementation based on thin films, behaving as a memristor. Memristor can offer new opportunities in circuit design due to its nonlinear behavior and memory. Nevertheless, since a cheap and reliable practical implementation of memristor is yet unavailable on the market, the design of such a realization, which mimics memristor behavior, is vital from the point of view of real-world circuit design. In this paper, a new microcontroller-based memristive chaotic circuit is proposed. Presented design has been implemented using an Ardunio Mega board, which solves numerically the dynamics of the memristor-based chaotic system using Runge-Kutta method. It sends the chaotic signals to the outputs of the circuit, using digital-to-analog converters. Chaotic dynamics and the strange attractors are obtained from the circuit using both, the computer simulations and the lab experiments. Considering both simulation and experimental results, it is shown that the proposed circuit mimics well the dynamics of the memristive chaotic system. [ABSTRACT FROM AUTHOR]

Our environment is full of radio frequency (RF) electromagnetic radiation due to rapid progress of wireless communication technologies, which have many applications, such as base stations of mobile communication networks and wireless internet antennas. In the context of Electromagnetic Compatibility (EMC) discipline, the word shield is used to describe any sort of metal sheet or material capable of reflecting, refracting or absorbing electromagnetic radiation (EMR), which is custom manufactured for the target electronic device casing, in a way to intervene any radiation passing to and from the device. Examining of electromagnetic shielding effectiveness (SE) is an important part of not only ensuring electromagnetic compatibility and of protecting electrical and electronic equipment but also protection of human beings against electromagnetic energy exposure as well. In this study, after a brief introduction and review on electromagnetic shielding, material selection and their applications for an indoor environment protection against electromagnetic radiation are explained. Potential electromagnetic shielding values of fabrics found in the market have been analyzed and compared by considering various criteria and requirements. Experimental results show that around 20 dB shielding effectiveness values can be obtained using test fabrics for a typical indoor environment. [ABSTRACT FROM AUTHOR]

38. Malignant triton tumor of the gluteal region in a patient unaffected by neurofibromatosis: A case report

Malignant triton tumor (MTT) is a rare variant of malignant peripheral nerve sheath tumor (MPNST) made up of both malignant schwannoma cells and malignant rhabdomyoblasts.A 26-years-old male patient was admitted with an asymptomatic gluteal mass. Magnetic resonance imaging showed heterogeneous soft tissue mass and he underwent open biopsy. Malignant peripheral nerve sheath tumor was diagnosed. He was given adjuvant chemotherapy following the removal of the tumor with hip disarticulation. The tumor was diagnosed as “malignant triton tumor” based on pathological examination including immunohistochemical studies. There were no signs of metastasis but recurrence was observed at 9 months follow up.MTT is usually associated with Neurofibromatosis 1 and located in head, neck region. In this case sporadic involvement of gluteal region and aggressive behavior of the lesion despite radical surgery was demonstrated. 39. Parosteal lipoma as a rare cause of peripheral neuropathy and local irritation: A report of 12 cases

Objective: The aim of this study was to evaluate the clinical features and functional results of patients with parosteal lipomas. Methods: A total of 12 patients (8 females and 4 males; mean age: 45 (10–62) years) with parosteal lipomas who were treated between April 1986 and April 2014, were included into the study. The medical records of the patients were reviewed to analyze the clinical features and functional results of the patients. Results: Of the 12 lipomas, 5 were localized in the proximal arm, 4 in the forearm, 1 in the distal arm, 1 in the distal thigh and 1 in the distal tibia. All patients presented with a progressive, slow-growing mass that was associated with thumb extension weakness in 1 case, and brachialgia-like symptoms in 1 case. Plain radiographs showed a juxtacortical mass in all cases and irregular ossification in 3 cases. In all cases, marginal excision was performed and no clinical recurrence was observed after a mean follow-up of 16 months. Conclusion: Parosteal lipomas are uncommon tumors that can be diagnosed with their characteristic radiological features. Parosteal lipomas occurring in the proximal radius may easily cause paralysis of the posterior interosseous nerve or muscle weakness. Objective: In this study, we sought to review the clinical and histopathological features and the chemotherapy regimens in osteogenic sarcoma in patients over 40 years of age, and we aimed at identifying the possible prognostic factors in this particular group of patients. Methods: We reviewed 287 patients with osteosarcoma treated between the year 1986 and 2010. Patients from this group who met the following criteria were considered eligible for our study; presence of primary OS, had typical histological and radiographic features of OS, no prior history of cancer or any treatment elsewhere and no prior history of preexisting bone abnormalities. Results: The Kaplan–Meier survival curve for the entire group, with a 95% confidence interval, at two and five years showed the survival rates as 76.2% and 72.8% respectively. The surgical margin was a significant factor affecting the survival. Presence of a pathological fracture also had a significant effect on the survival rate. Conclusion: Osteogenic sarcoma remains a challenging disease to treat. Despite the expectation that elderly patients may not tolerate aggressive modern chemotherapy as the younger patients, we believe that patients with primary OS over the age of 40 should be treated aggressively with effective chemotherapy and complete surgical excision whenever possible. Purpose. To investigate the association between papillary thyroid cancer (PTC) and Hashimoto's thyroiditis (HT). Design. This study is a retrospective study that conducted during 7 consecutive years with a median 119.5 months follow-up. Subjects and Method. Patients who underwent thyroidectomy in Dokuz Eylül University Hospital during 7 consecutive years were included. Patients' demographics, biochemical, radiological, and pathological results were retrospectively assessed. Results. Four hundred sixty nine patients were evaluated. Among 469 patients who underwent thyroidectomy, 132 (28.1%) were malignant, while 182 patients were diagnosed with HT (38.8%). PTC was ranked first at 92.4% (n: 122). The prevalence of HT was 54.9% in patients with PTC and 33.1% in patients without PTC diagnosis (p<0.001). Younger age and the presence of HT were independently associated with PTC. The presence of HT was associated with increased risk of development of PTC (OR: 2.2, %95 CI: 1.4-3.5, p<0.001) but not with TNM stage or recurrence. Lymph node metastasis at presentation was the strongest predictor of recurrence (OR: 13.9, CI: 3.5-54.6, p<0.001) Conclusions. HT was an independent risk factor for development of PTC. According to our findings, HT patients (particularly with nodular HT) should be observed carefully and thyroid fine needle aspiration biopsy (TFNAB) should be encouraged if necessary. Context. Immunoassays are vulnerable to occasional analytical errors despite their sensitivity and specificity. Adrenocorticotropic hormone (ACTH) is among hormones which are vulnerable to assay interference. Objective. In this case report, we aimed to present a case of ACTH interference in a patient with subclinical hypercortisolism. Case report. We present a case of subclinical hypercortisolism with unexpectedly elevated levels of ACTH in whom interference was shown by different biochemical methods. It is important for the clinician to keep in mind the possibility of interference when clinical and laboratory results are discordant. The communication between the clinician and laboratory staff is important while interpreting results. This case report shows that ACTH interference should be considered in patients with subclinical hypercortisolism who have unexpectedly high levels of adrenocorticotropic hormone. Objective. Mean platelet volume (MPV) and platelet function analysis have been studied before in acromegaly, but the effect of treatment on both parameters has not been evaluated. We aimed to investigate MPV and platelet function analysis in acromegalic patients after sixmonths of treatment. Methods. Forty patients with active acromegaly and 36 healthy subjects were included in the study. Plasma glucose and lipids, fibrinogen, GH, IGF-1 levels, MPV and platelet function analysis were measured. All patients with acromegaly were re-evaluated six months after treatment. Results. Fasting blood glucose (FBG), GH, IGF- 1, fibrinogen levels and MPV values were significantly higher in acromegalic group compared with the control. Platelet function was enhanced significantly (pcol-ADP: 0.002, pcolepinephrine: 0.002). After 6 months of treatment FBG, serum GH, IGF-1, fibrinogen and MPV decreased and collagen/ADP- and collagen/epinephrine-closure times (CT) were increased. Acromegalic patients that were in remission with long-acting SSA after surgery had significantly higher fibrinogen levels and MPV and decreased collagen/ epinephrine-CT with respect to the controls (pfibrinogen: 0.001, pMPV: 0.026, pcol-epinephrine: 0.037). Conclusion. Acromegaly was associated with increased MPV and enhanced platelet activity. Although growth hormone hypersecretion was controlled by surgery and medical treatment, these parameters did not improve - indicating a still increased risk for cardiovascular events. The study employed regression models to systematically test hypotheses, providing nuanced insights into the complex dynamics of the variables under investigation., Methods: Regression models were utilized to test the hypotheses systematically, considering the moderating role of career adapt abilities. The study included a sample of 576 nurses from 10 urban hospitals in Turkey, distributed across five distinct cities. This approach allowed for a detailed examination of the intricate relationships between career stress, work stress, career regret, job satisfaction and career adapt abilities within the nursing profession., Results: The findings underscore the delicate balance between the positive gains of job satisfaction and the challenges posed by career stress, work stress and career regret in the multifaceted landscape of nursing., Conclusion: In nursing, the relationship between career stress, work stress, career regret and job satisfaction include progress, resilience and obstacles. Developing strategies requires a cooperative effort from health care organizations, nursing leaders and individual nurses., Impact: The research provides valuable insights into the interplay of these factors and emphasizes the moderating role played by career adapt abilities. 46. Evaluation of bladder capacity in pediatric patients with nocturnal enuresis using voiding diary and uroflowmetry: A retrospective study.

Objective: This retrospective study was conducted in a tertiary hospital in Turkiye, it was aimed to evaluate the results of kidney and bladder ultrasonography, uroflowmetric study and volume frequency chart in children diagnosed with bedwetting at night., Methods: This study focused on patients aged 5-17 years who were treated for nocturnal enuresis in the single-center pediatric urology clinic of a tertiary hospital between 2017-2021. Patients' comorbidities, surgical history, urinary ultrasonography findings, uroflowmetry examination results, and two-day voiding diary were requested., Results: The number of pediatric patients diagnosed with enuresis nocturna was 956. Abnormal ultrasonography findings were observed in 13.9% of patients. Almost half of the patients had a positive family history, and 13.4% had a history of previous surgical procedures. While the voiding diary indicated that 54.2% of patients had bladder volumes equivalent to the expected bladder capacity, uroflowmetry examination revealed low bladder capacity in 65% of patients., Conclusion: This study underscores the significance of comprehensive evaluations, including detailed ultrasonography and voiding diary measurements, in pediatric patients with monosymptomatic enuresis nocturna. Our study suggests that bladder volume measured through a two-day weekend voiding diary may provide more effective information for estimating optimal bladder capacity compared to measurements obtained through uroflowmetry., Competing Interests: No conflict of interest was declared by the authors.

47. Assessment of aortomesenteric distance and mesenteric and retroperitoneal adipose tissue thickness in genetic forms of lipodystrophy.

Introduction: Lipodystrophy is a rare disease characterized by the loss of adipose tissue. Visceral adipose tissue loss in certain forms of lipodystrophy may affect the amount of mesenteric fat., Method: We studied visceral adipose tissue by measuring the thickness of mesenteric and retroperitoneal adipose tissue and the aortomesenteric (AOM) distance in patients with genetic forms of lipodystrophy (n = 48; 7 males; 41 females; mean age 39.1 ± 11.9 years; 19 with congenital generalized lipodystrophy [CGL], and 29 with familial partial lipodystrophy [FPLD]). An age- and gender-matched control group with a ratio of 1:2 was generated., Results: Patients with CGL had severely depleted mesenteric adipose tissue (2.0 [IQR: 1.5-3.5] mm vs. 18.8 [IQR: 4.4-42.2] mm in FPLD, P < .001; 30.3 [IQR: 13.9-46.6] mm in controls, P < .001) and retroperitoneal adipose tissue (1.3 [IQR: 0.0-5.3] mm vs. 33.7 [IQR: 21.6-42.1] mm in FPLD, P < .001; 29.7 [IQR: 23.1-36.7] mm in controls, P < .001). The AOM distance was shorter in patients with CGL (8.1 [IQR: 6.0-10.8] mm) compared to patients with FPLD (vs. 13.0 [IQR: 8.8-18.1] mm; P = .023) and controls (vs. 11.3 [IQR: 8.4-15.5] mm, P = .016). Leptin levels were positively correlated with AOM distance in lipodystrophy (r = .513, P < .001). Multivariate linear regression analysis identified body mass index as a significant predictor of AOM distance (data controlled for age and sex; beta = 0.537, 95% CI: 0.277-0.798, P < .001). Twelve of 19 patients (63%) with CGL had an AOM distance of < 10 mm, a risk factor that may predispose patients to developing superior mesenteric artery syndrome., Conclusion: CGL is associated with a severe loss of mesenteric adipose tissue, which leads to a narrowing of the space between the superior mesenteric artery and the aorta., (© 2024. 48. Primitive myxoid mesenchymal tumor of infancy presenting with an abdominal and thoracic mass misdiagnosed as neuroblastoma: A case report.

Abstract: Primitive myxoid mesenchymal tumor of infancy (PMMTI) is a rare soft tissue tumor that has been recently described in only a few cases. A 10-month-old boy was admitted with a mass filling the abdomen and right hemothorax. Gross examination revealed a fragile, and occasionally gelatinous and rubbery mass. On microscopic examination, there were fibrillary off-white appearance with occasionally myxoid microcystic areas. Immunohistochemical examination results were as follows: BCOR (+), and SATB2 (+) with diffuse nuclear expression, Desmin (-), CD34 (-), S-100 (-), MyoD1 (-), Myogenin (-). The next-generation sequencing showed internal tandem duplication of the BCOR exon 15. In conclusion, PMMTI is a distinctive entity with unique characteristics. However, it can be clinically confused with other childhood malignancies such as neuroblastoma. 49. Analysis of Various Laparoscopic Pediatric Urology Surgeries: Five-Year Experience of a Single Institution.

- Abstract
Background In this study, we aim to report our single-center experience with laparoscopic pediatric urological surgeries. We aim to determine the feasibility of various urological and urogenital laparoscopic procedures and the tricks that increase surgical success. Methodology Data from 98 patients who underwent laparoscopic urological and/or urogenital procedures for diagnostic and therapeutic purposes in our clinic between June 2018 and February 2023 were retrospectively analyzed. All surgeries were performed by the same surgical team. Laparoscopic procedures included orchidopexy, gonadectomy, vaginoplasty, hysterectomy, pyeloplasty, nephrectomy/partial nephrectomy, ureteroneocystostomy, bladder diverticulum excision, renal cyst excision, proximal ureter stone removal, oophorectomy, ovarian detorsion, oophoropexy, and lymph node excision for diagnostic purposes. The surgical planning of the patients was based on the decisions of the pediatric nephrology, pediatric endocrinology, and pediatric oncology departments and the multidisciplinary council. Demographic characteristics of the patients, surgical indications, and intraoperative data, as well as postoperative pathological diagnoses and complications, were recorded. All patients underwent a transperitoneal approach. The duration of the operation was obtained from anesthesia records and defined as the time from the beginning of the surgical incision to the closure of the skin incision. Results Of the patients, 54 were males and 44 were females. The median age was 7.8 years. No complications other than grade 1 according to the Clavien-Dindo classification were observed in our patients. As different types of surgeries were analyzed, the mean operative duration was estimated. Conclusions The laparoscopic method should be performed by surgeons experienced in advanced surgeries in pediatric urology. It is critical to consider the difference in the size of pediatric patients in preparation for laparoscopic surgery to minimize technical and ergonomic problems. We believe that each surgery has its specific tricks and that these should be a part of laparoscopy training. 50. Multiple immunoassay interference in a patient with falsely elevated calcitonin.

- Abstract
Calcitonin (CT) is a diagnostic and follow-up marker of medullary thyroid carcinoma. Heterophile antibodies (HAbs) may interfere during immunometric assay measurements and result in falsely high CT levels and different markers. A 50-year-old female patient was referred to our institution for elevated CT levels (3,199 pg/mL [0-11,5]). Physical examination and thyroid ultrasonography show no thyroid nodules. Because of the discrepancy between the clinical picture and the laboratory results, various markers and hormones were examined to determine whether there was any interference in the immunometric assay. Thyroglobulin (Tg) and Adrenocorticotropic hormone (ACTH) levels were also found inaccurately elevated. After precipitation with polyethylene glycol, CT, Tg, and ACTH levels markedly decreased, showing macro-aggregates. Also, serial dilutions showed non-linearity in plasma concentrations. Additionally, CT samples were pretreated with a heterophilic blocking tube before measuring, and the CT level decreased to < 0.1 pg/mL, suggesting a HAb presence. Immunoassay interference should be considered when conflicting laboratory data are observed. This may help reduce the amount of unnecessary laboratory and imaging studies and prevent patients from complex diagnostic procedures.
