- Abstract
We read with great interest the article published recently in CHEST (October 2019) by Pauley et al1 in which their goal was to evaluate national trends in admission rates, discharge disposition, and length of stay in patients hospitalized with pulmonary embolism (PE) by assessing patient demographic and hospital characteristics. They found a continued increase in admissions for PE between 2000 and 2015. Elderly patients have been shown to be affected disproportionally and experience higher mortality rates compared with the other age groups. Increased age and comorbidity burden, including congestive heart failure, paralysis, and metastatic cancer, have been found to be independently associated with poor outcomes. The authors also noted that targeted clinical trials designed to improve survival and quality of life in all age brackets are needed. WOS:000500923700038 31812196 Q1

Right aortic arch anomaly (RAAA) is a seldomly encountered condition. As it may present with other vascular abnormalities, it may also present alone. It usually becomes symptomatic at childhood, and symptoms like dyspnea and dsyphagia, if there is compression on trachea or esophagus, may be encountered. Sometimes it is coincidentally diagnosed with radiological methods in asymptomatic individuals who were admitted to the hospital for other reasons. RAAA was diagnosed with by chest x-ray and dynamic thorax computed tomography for asymptomatic a 47 year-old woman who was admitted to the hospital due to a traffic accident. This patient was presented as a case report since it is a rarely seen syndrome diagnosed coincidentally in patients who remained asymptomatic for several years. Key words: Right aortic arch anomaly, Chest x-ray, Thorax computed tomography., Sağ aortik ark anomalisi (SAAA) nadir görülen bir durumdur. SAAA başka damarsal anomalilerle birlikte olabileceği gibi tek başına da olabilir. Genellikle çocuklukta semptomatik hale gelir, trakea veya özafagusa bası durumunda nefes darlığı, disfaji gibi şikayetlere yol açabilir. Bazen SAAA tanısı, semptom vermeksizin başka bir nedenle hastaneye başvuran kişilerde istenen radyolojik görüntüleme yöntemleri ile tesadüfen konulur. 47 yaşına kadar herhangi bir şikayeti olmayan bayan hastaya travma sonrası çekilen akciğer grafisi ve dinamik toraks tomografisindeki bulgulara dayanarak SAAA tanısı konuldu. SAAA az görüldüğü ve hastamız uzun yıllar semptom vermediği için olgu sunusu olarak sunulmuştur. Anahtar kelimeler: Sağ aortik ark anomalisi, Akciğer grafisi, Toraks bilgisayarlı tomografisi.

Hepatodiaphragmatic interposition of small or large intestine is known as Chilaiditi syndrome. This sendrome is a rare disease and diagnosed incidentally. Chilaiditi syndrome is typically asymptomatic, but it can be associated with symptoms ranging from intermittent, mild abdominal pain to acute intestinal obstruction, constipation, chest pain and breathlessness. We report a 54 years old women previously misdiagnosed as pulmonary infection and gastric ulcer. Key words: Chilaiditi Syndrome, Hepatodiaphragmatic Interposition, Chest Pain, Hepatodiafragmatik interpozisyon olarak bilinen Chilaiditi sendromu nadir rastlanan bir sendromdur. Barsak ansları diafram ve karaciğer arasında yerleşmiştir. Olguların çoğuna tesadüfen tanı konur, hafif abdominal ağrı, intestinal obstrüksiyon, göğüs ağrısı, nefes darlığı gibi semptomlarla ortaya çıkabilir. Bizim olgumuz 54 yaşında bayan hasta, daha önce solunum yolu enfeksiyonu veya peptik ülser olarak tedavi edilmiş bir olgu. Poland syndrome is a rarely encountered genetical disorder that is characterized with unilateral absence of pectoralis major muscle. Upper extremity deformities and anomalies in the breast, nipple and hand may accompany the syndrome. It is generally right sided and is more frequent in males. A 36 year-old male patient's PA chest X-ray who was admitted with the complaint of haemoptysis, showed rightsided hyperlucency. His chest computed tomography demonstrated Poland syndrome. This patient was presented as case report because of rarity of Poland syndrome. Key words: Poland syndrome, Absence of pectoralis major, Unilateral hyperlusensy. Poland sendromu tek taraflı pektoralis major kasının yokluğu ile karakterize konjenital, Poland sendromu pektoralis major kasının tek taraflı yokluğu ile karakterize konjenital bir sendromdur. Bu sendroma meme, el ve diğer organ anomalileri de eşlik edebilir. Genellikle sağ tarafı tutar ve erkeklerde daha sıktır. Otuzaltı yaşında hemoptizi ile başvuran erkek hastanın çekilen akciğer grafisinde sağ hemitoraksta saydamlık artışı saptandı. Bu nedenle çekilen toraks bilgisayarlı tomografisinde Poland sendromu saptandı. Bu olgu az görüldüğü ve radyolojik özellikleri nedeniyle sunulmuştur. 9. Acil Serviste Pulmoner Tromboemboli Tanısı Alan Hastaların Retrospektif Olarak Değerlendirilmesi

Aim: Pulmonary thromboembolism (PTE) is a major health problem. The aim of this study was the retrospective evaluation of patients diagnosed with pulmonary thromboembolism in the emergency department with regard to underlying risk factors, diagnostic procedures, treatment modalities, mortality rates and mortality-related factors. Materials and method: Two hundred and five patients, that were treated between 2004 and 2007, were retrospectively evaluated. Demographic characteristics of the patients were analyzed together with underlying risk factors, diagnostic procedures, treatment modalities, mortality rates and mortality-related factors. Results: With regard to risk factors, gynecological surgical interventions were present in 17 patients, orthopedical surgical interventions in 58, abdominal surgical interventions in 20, intracranial surgical interventions in 12, comorbid disorders in 25, history of immobility in 20, antiphospolipid antibody positivity in five, protein C and protein S defficiency in five and oral contraceptive use in three, whereas no risk factors could be observed in 35 patients. Sixteen patients had a mortal disease course. Conclusion: PTE is a life threatening disorder and early recognition is important. In the postoperative period, appropriate medical measures and the prophylactic administration of low molecular weight heparin might prevent the development of deep vein thrombosis and, hence, PTE. Bu çalışmada, acil serviste PTE tanısı alan hastaların altta yatan risk faktörleri, tanı yöntemleri, tedavi yöntemleri, mortalite oranları ve mortaliteye neden olan faktörler açısından retrospektif olarak değerlendirilmesi amaçlanmıştır. Materyal ve metot: 2004 ile 2007 yılları arasında tedavi uygulanan 205 hasta retrospektif olarak değerlendirildi. Hastaların demografik özellikleri ile altta yatan risk faktörleri, uygulanan tanı yöntemleri, uygulanan tedavi yöntemleri, mortalite oranları ve mortaliteye neden olan faktörler incelendi. Bulgular: Risk faktörü olarak hastaların 18'inde jinekolojik cerrahi girişim, 59'unda ortopedik cerrahi girişim, 20'sinde abdominal cerrahi girişim, 12'sinde intrakranial cerrahi girişim, 25'inde komorbid hastalıklar, 20'sinde immobilite öyküsü, beşinde antifosfolipid antikor pozitifliği, beşinde protein C ve protein S eksikliği ve üçünde oral kontraseptif kullanım öyküsü mevcutken 38 hastada herhangi bir risk faktörüne rastlanmamıştı. Onaltı hastada hastalık mortal seyretmişti. Sonuç: PTE yaşamı tehdit eden bir hastalıktır ve erken tanınması önemlidir. Postoperatif dönemde gerekli medikal önlemlerin alınması ve düşük molekül ağırlıklı heparin profilaksisi ile derin venöz trombüs ve PTE oluşumunun önüne geçilebilir. - Abstract
Introduction: In this study, changes in energy metabolism before and after treatment were compared in obstructive sleep apnea syndrome (OSAS) patients who received positive airway pressure therapy., Materials and Methods: Thirty-nine patients (22 male, 17 female) were admitted to study. Patients for PAP therapy who had moderate to severe in polysomnography were included. Values of energy metabolism were recorded during three days via metaboic holter device, before and after PAP therapy., Result: The mean age of the patients was 51.53 ± 11.16 year. In 15 (38.46%) of the patients BPAP, and in 24 (61.54%) of the patients CPAP treatment started. Three days after using metabolic holter device: the total daily energy consumption of the patients was found to be 482,4 ± 296.1 kcal/day before treatment and 524.5 ± 343.1 kcal/day after treatment (p<0.0001); patients' daily physical activity was 7867 ± 3319 steps/day before treatment and 12.416 ± 1451 steps/day after treatment, which was considered statistically significant (p<0001); the total daily resting period of the patients was 7.90 ± 1.36 hours/day before treatment and 7.44 ± 1.42 hours/day after treatment, considered statistically significant (p<0001); the total sleep duration of the patients was 5.50 ± 1.88 hours/day before treatment and 5.87 ± 1.20 hours/ day after treatment, considered statistically significant (p<0001)., Conclusions: In our study, we found that daily physical activity and energy consumption increased with PAP treatment. With PAP treatment, obesity, diabetes and hypertension can be controlled. In our study, since PAP treatment was effective with effective sleep, the immobilization time was decreased and therefore the duration of daytime physical activity was prolonged. With these effects, patients' compliance with treatment increases. In conclusion, PAP treatment should be recommended in patients with moderate and severe OSAS.

- Abstract
Background: COPD is preventable and treatable and is characterized by completely nonreversible airflow obstruction. In this study, we aimed to investigate the effect of long-term oxygen therapy on patients with stage 4 COPD who were followed up and treated at the polyclinic or clinic service. We evaluated the effects of oxygen therapy on energy metabolism and physical activity in patients with COPD., Methods: Nineteen patients with COPD (16 male/3 female), treated with oxygen therapy for the first time, were included in this study. Analysis of arterial blood gases and pulmonary function test was performed. Metabolic Holter device (SenseWear
® Armband) was placed pre- and post-oxygen therapy on the patients' arm for at least 3 days. This device captures Holter data in a digitized electronic system, and the daily average value was calculated from the data., Results: Post-oxygen treatment showed a significant increase in energy expenditure by patients with COPD (pretreatment, 1,497±596 joule; posttreatment, 2,977±5,985 joule; P =0.044). Moreover, number of steps during walking (pretreatment, 2,056±256; posttreatment, 2,120±195; P =0.03), resting (pretreatment, 6.36±3.31 hours; posttreatment, 3.47±2.19 hours; P <0.03), and sleeping (pretreatment, 4.23±2.13 hours; posttreatment, 2.33±1.42 hours; P <0.00) showed significant differences. Increased daily energy expenditure in patients with respiratory failure was detected with long-term oxygen therapy. In addition, the immobility of patients decreased and duration of physical activity increased in patients with COPD., Conclusion: In this study, positive effects of long-term oxygen therapy have been demonstrated with respect to energy metabolism and physical activity of patients with COPD.
