1. Empirical Evidence That There Is No Such Thing As A Validated Prediction Model

Abstract

Background: External validations are essential to assess clinical prediction models (CPMs) before deployment. Apart from model misspecification, differences in patient population and other factors influence a model's AUC (c-statistic). We aimed to quantify variation in AUCs across external validation studies and adjust expectations of a model's performance in a new setting. Methods: The Tufts-PACE CPM Registry contains CPMs for cardiovascular disease prognosis. We analyzed the AUCs of 469 CPMs with a total of 1,603 external validations. For each CPM, we performed a random effects meta-analysis to estimate the between-study standard deviation $\tau$ among the AUCs. Since the majority of these meta-analyses has only a handful of validations, this leads to very poor estimates of $\tau$. So, we estimated a log normal distribution of $\tau$ across all CPMs and used this as an empirical prior. We compared this empirical Bayesian approach with frequentist meta-analyses using cross-validation. Results: The 469 CPMs had a median of 2 external validations (IQR: [1-3]). The estimated distribution of $\tau$ had a mean of 0.055 and a standard deviation of 0.015. If $\tau$ = 0.05, the 95% prediction interval for the AUC in a new setting is at least +/- 0.1, regardless of the number of validations. Frequentist methods underestimate the uncertainty about the AUC in a new setting. Accounting for $\tau$ in a Bayesian approach achieved near nominal coverage. Conclusion: Due to large heterogeneity among the validated AUC values of a CPM, there is great irreducible uncertainty in predicting the AUC in a new setting. This uncertainty is underestimated by existing methods. The proposed empirical Bayes approach addresses this problem which merits wide application in judging the validity of prediction models.

Abstract

G-formula is a popular approach for estimating treatment or exposure effects from longitudinal data that are subject to time-varying confounding. G-formula estimation is typically performed by Monte-Carlo simulation, with non-parametric bootstrapping used for inference. We show that G-formula can be implemented by exploiting existing methods for multiple imputation (MI) for synthetic data. This involves using an existing modified version of Rubin's variance estimator. In practice missing data is ubiquitous in longitudinal datasets. We show that such missing data can be readily accommodated as part of the MI procedure when using G-formula, and describe how MI software can be used to implement the approach. Abstract

While shrinkage is essential in high-dimensional settings, its use for low-dimensional regression-based prediction has been debated. It reduces variance, often leading to improved prediction accuracy. However, it also inevitably introduces bias, which may harm two other measures of predictive performance: calibration and coverage of confidence intervals. Much of the criticism stems from the usage of standard shrinkage methods, such as lasso and ridge with a single, cross-validated penalty. Our aim is to show that readily available alternatives can strongly improve predictive performance, in terms of accuracy, calibration or coverage. For linear regression, we use small sample splits of a large, fairly typical epidemiological data set to illustrate this. We show that usage of differential ridge penalties for covariate groups may enhance prediction accuracy, while calibration and coverage benefit from additional shrinkage of the penalties. In the logistic setting, we apply an external simulation to demonstrate that local shrinkage improves calibration with respect to global shrinkage, while providing better prediction accuracy than other solutions, like Firth's correction. The benefits of the alternative shrinkage methods are easily accessible via example implementations using \texttt{mgcv} and \texttt{r-stan}, including the estimation of multiple penalties. Abstract

Benchmarking is commonly used in many healthcare settings to monitor clinical performance, with the aim of increasing cost-effectiveness and safe care of patients. The funnel plot is a popular tool in visualizing the performance of a healthcare center in relation to other centers and to a target, taking into account statistical uncertainty. In this paper we develop methodology for constructing funnel plots for survival data. The method takes into account censoring and can deal with differences in censoring distributions across centers. Practical issues in implementing the methodology are discussed, particularly in the setting of benchmarking clinical outcomes for hematopoietic stem cell transplantation. A simulation study is performed to assess the performance of the funnel plots under several scenarios. 24. Simultaneous Confidence Intervals for Ranks With Application to Ranking Institutions

Abstract

When a ranking of institutions such as medical centers or universities is based on an indicator provided with a standard error, confidence intervals should be calculated to assess the quality of these ranks. We consider the problem of constructing simultaneous confidence intervals for the ranks of means based on an observed sample. For this aim, the only available method from the literature uses Monte-Carlo simulations and is highly anticonservative especially when the means are close to each other or have ties. We present a novel method based on Tukey's honest significant difference test (HSD). Our new method is on the contrary conservative when there are no ties. By properly rescaling these two methods to the nominal confidence level, they surprisingly perform very similarly. The Monte-Carlo method is however unscalable when the number of institutions is large than 30 to 50 and stays thus anticonservative. We provide extensive simulations to support our claims and the two methods are compared in terms of their simultaneous coverage and their efficiency. We provide a data analysis for 64 hospitals in the Netherlands and compare both methods. Software for our new methods is available online in package ICRanks downloadable from CRAN. 25. Adaptive Critical Value for Constrained Likelihood Ratio Testing

Abstract

We present a new way of testing ordered hypotheses against all alternatives which overpowers the classical approach both in simplicity and statistical power. Our new method tests the constrained likelihood ratio statistic against the quantile of one and only one chi-squared random variable with a data-dependent degrees of freedom instead of a mixture of chi-squares. Our new test is proved to have a valid finite-sample significance level $\alpha$ and provides more power especially for sparse alternatives (those with a few or moderate number of null constraints violations) in comparison to the classical approach. Our method is also easier to use than the classical approach which requires to calculate or simulate a set of complicated weights. Two special cases are considered with more details, namely the case of testing orthants $\mu_1<0, \cdots, \mu_n<0$ and the isotonic case of testing $\mu_1<\mu_2<\mu_3$ against all alternatives. Abstract

Background Women with anterior circulation large vessel occlusion (LVO) have been reported to have worse outcomes after endovascular treatment (EVT) than men. Whether these disparities also exist in LVO of the posterior circulation is yet uncertain. We assessed sex differences in clinical, technical, and safety outcomes of EVT in posterior circulation LVO. Methods We used data of patients with posterior circulation LVO included in the Multicenter Randomized Clinical Trial of Endovascular Treatment for Acute Ischemic Stroke in the Netherlands (MR CLEAN) Registry (2014-2018). Primary outcome was the modified Rankin Scale (mRS) score at 90 days assessed with multivariable ordinal regression analysis. Secondary outcomes included favorable functional outcome (mRS ≤3), functional independence (mRS ≤2), death within 90 days, National Institutes of Health Stroke Scale (NIHSS) score 24-48 hours postintervention, complications, successful reperfusion (extended Thrombolysis in Cerebral Ischemia 2B-3), and procedure duration analyzed with multivariable logistic and linear regression analyses. Results We included 264 patients (42% women). Compared with men, women were older (median age 68 vs 63 years), more often had prestroke disability (mRS ≥1: 37% vs 30%), and received intravenous thrombolytics less often (45% vs 56%). Clinical outcomes were similar between sexes (adjusted (common) OR (aOR) 0.82, 95% CI 0.51 to 1.34; favorable functional outcome 50% vs 43%, aOR 1.31, 95% CI 0.77 to 2.25; death 32% vs 29%, aOR 0.98, 95% CI 0.52 to 1.84). In addition, NIHSS score after 24-48 hours (median 7 vs 9), successful reperfusion (77% vs 73%), and complications did not differ between men and women. Conclusions Outcomes in women treated with EVT for posterior circulation LVO were similar compared with men despite less favorable baseline characteristics in women. Abstract

Ranking institutions such as medical centers or universities is based on an indicator accompanied with an uncertainty measure such as a standard deviation, and confidence intervals should be calculated to assess the quality of these ranks. We consider the problem of constructing simultaneous confidence intervals for the ranks of centers based on an observed sample. We present in this paper a novel method based on multiple testing which uses the partitioning principle and employs the likelihood ratio (LR) test on the partitions. The complexity of the algorithm is super exponential. We present several ways and shortcuts to reduce this complexity. We provide also a polynomial algorithm which produces a very good bracketing for the multiple testing by linearizing the critical value of the LR test. We show that Tukey's Honest Significant Difference (HSD) test can be written as a partitioning procedure. The new methodology has promising properties in the sens that it opens the door in a simple and easy way to construct new methods which may trade the exponential complexity with power of the test or vice versa. In comparison to Tukey's HSD test, the LR test seems to give better results when the centers are close to each others or the uncertainty in the data is high which is confirmed during a simulation study., Comment: Technical report. Abstract

Background and Purpose: Data on health‐related quality of life (HRQoL) and mood in cerebral amyloid angiopathy (CAA), a disease characterized by stroke and cognitive decline, are limited. We aimed to investigate the impacted domains of life, value‐based HRQoL and the prevalence of depression and anxiety in patients with CAA. Methods: We conducted a cross‐sectional study of patients with sporadic (s)CAA, lobar dominant mixed CAA and hypertensive arteriopathy (mixed CAA‐HTA), or Dutch‐type hereditary (D‐)CAA, from prospective outpatient clinic cohorts. Participants completed four questionnaires: the EuroQoL 5 dimensions 5‐level questionnaire (EQ‐5D‐5L; EQ‐VAS for visual analogue scale; EQ‐Index for index rating), the Short‐Form 36 questionnaire (SF‐36), the Center for Epidemiologic Studies—Depression scale (CES‐D), and the Hospital Anxiety and Depression Scale (HADS; ‐D for depression and ‐A for anxiety subscales). The EQ‐5D‐5L assesses the domains mobility, self‐care, usual activities, pain/discomfort and anxiety/depression. The SF‐36 domains are physical functioning, social functioning, physical role limitations, emotional role limitations, mental health, vitality, bodily pain, and general health perceptions. We compared age‐ and sex‐ adjusted HRQoL (SF‐36 domain scores; EQ‐VAS; EQ‐Index) to the Dutch normative population, and estimated the prevalences of current depression (either: history of depression or current use of antidepressants, with high score on CES‐D [≥16] and/or HADS‐D [≥8]; or high score on both depression questionnaires) and anxiety (HADS‐A ≥ 8). Results: We included 179 patients: 77 with sCAA (mean age: 72 years, women: 36%), 31 with mixed CAA‐HTA (68 years, women: 29%), and 71 with D‐CAA (56 years, women: 52%, symptomatic: 35 [49%]). The SF‐36 profiles of all patient groups were similar, negatively differing from the norm in emotional role functioning, social functioning and vitality. The EQ‐VAS score of patients (mean [SD] sCAA: 76 [16], D‐CAA: 77 [15]) was similar to the norm, as was the EQ‐Index score. Fifteen patients with sCAA (23%; 95% confidence interval [CI] 13%–33%), seven with mixed CAA‐HTA (27%; 95% CI 10%–44%) and eight with D‐CAA (14%; 95% CI 5%–22%) were noted as having depression. The prevalences of anxiety and depression were equivalent. Conclusions: We found that CAA influenced emotional role functioning and aspects linked to social engagement consistently across its subtypes. One quarter of patients exhibited depressive or anxiety symptoms. Recognizing these impacted domains could enhance overall well‐being. [ABSTRACT FROM AUTHOR]

Abstract

Genome-wide association studies have identified hundreds of genetic variants associated with blood pressure (BP), but sequence variation accounts for a small fraction of the phenotypic variance. Epigenetic changes may alter the expression of genes involved in BP regulation and explain part of the missing heritability. We therefore conducted a two-stage meta-analysis of the cross-sectional associations of systolic and diastolic BP with blood-derived genome-wide DNA methylation measured on the Infinium HumanMethylation450 BeadChip in 17,010 individuals of European, African American, and Hispanic ancestry. Of 31 discovery-stage cytosine-phosphate-guanine (CpG) dinucleotides, 13 replicated after Bonferroni correction (discovery: N = 9,828, p < 1.0 × 10-7; replication: N = 7,182, p  30%) and independent of known BP genetic variants, explaining an additional 1.4% and 2.0% of the interindividual variation in systolic and diastolic BP, respectively. Bidirectional Mendelian randomization among up to 4,513 individuals of European ancestry from 4 cohorts suggested that methylation at cg08035323 (TAF1B-YWHAQ) influences BP, while BP influences methylation at cg00533891 (ZMIZ1), cg00574958 (CPT1A), and cg02711608 (SLC1A5). Gene expression analyses further identified six genes (TSPAN2, SLC7A11, UNC93B1, CPT1A, PTMS, and LPCAT3) with evidence of triangular associations between methylation, gene expression, and BP. Additional integrative Mendelian randomization analyses of gene expression and DNA methylation suggested that the expression of TSPAN2 is a putative mediator of association between DNA methylation at cg23999170 and BP. These findings suggest that heritable DNA methylation plays a role in regulating BP independently of previously known genetic variants.

Abstract

INTRODUCTION: Cerebral amyloid angiopathy (CAA) is a main cause of cognitive dysfunction in the elderly. We investigated specific cognitive profiles, cognitive function in the stage before intracerebral hemorrhage (ICH), and the association between magnetic resonance imaging (MRI) based cerebral small vessel disease (cSVD) burden in CAA because data on these topics are limited. METHODS: We included Dutch‐type hereditary CAA (D‐CAA) mutation carriers with and without ICH, patients with sporadic CAA (sCAA), and age‐matched controls. Cognition was measured with a standardized test battery. Linear regression was performed to assess the association between MRI‐cSVD burden and cognition. RESULTS: D‐CAA ICH− mutation carriers exhibited poorer global cognition and executive function compared to age‐matched controls. Patients with sCAA performed worse across all cognitive domains compared to D‐CAA ICH+ mutation carriers and age‐matched controls. MRI‐cSVD burden is associated with decreased processing speed. DISCUSSION: CAA is associated with dysfunction in multiple cognitive domains, even before ICH, with increased MRI‐cSVD burden being associated with slower processing speed. Abstract

Peak width of skeletonized mean diffusivity (PSMD) is an emerging diffusion-MRI based marker to study subtle early alterations to white matter microstructure. We assessed PSMD over the clinical continuum in Dutch-type hereditary CAA (D-CAA) and its association with other CAA-related MRI-markers and cognitive symptoms. We included (pre)symptomatic D-CAA mutation-carriers and calculated PSMD from diffusion-MRI data. Associations between PSMD-levels, cognitive performance and CAA-related MRI-markers were assessed with linear regression models. We included 59 participants (25/34 presymptomatic/symptomatic; mean age 39/58 y). PSMD-levels increased with disease severity and were higher in symptomatic D-CAA mutation-carriers (median [range] 4.90 [2.77–9.50]mm2/s × 10−4) compared with presymptomatic mutation-carriers (2.62 [1.96–3.43]mm2/s × 10−4) p = <0.001. PSMD was positively correlated with age, CAA-SVD burden on MRI (adj.B [confidence interval] = 0.42 [0.16–0.67], p = 0.002), with number of cerebral microbleeds (adj.B = 0.30 [0.08–0.53], p = 0.009), and with both deep (adj.B = 0.46 [0.22–0.69], p = <0.001) and periventricular (adj.B = 0.38 [0.13–0.62], p = 0.004) white matter hyperintensities. Increasing PSMD was associated with decreasing Trail Making Test (TMT)-A performance (B = −0.42 [−0.69–0.14], p = 0.04. In D-CAA mutation-carriers microstructural white matter damage is associated with disease phase, CAA burden on MRI and cognitive impairment as reflected by a decrease in information processing speed. Abstract

Background: The optimal treatment for odontoid fractures in older people remains debated. Odontoid fractures are increasingly relevant to clinical practice due to ageing of the population.Methods: An international prospective comparative study was conducted in fifteen European centres, involving patients aged ≥55 years with type II/III odontoid fractures. The surgeon and patient jointly decided on the applied treatment. Surgical and conservative treatments were compared. Primary outcomes were Neck Disability Index (NDI) improvement, fracture union and stability at 52 weeks. Secondary outcomes were Visual Analogue Scale neck pain, Likert patient-perceived recovery and EuroQol-5D-3L at 52 weeks. Subgroup analyses considered age, type II and displaced fractures. Multivariable regression analyses adjusted for age, gender and fracture characteristics. Results: The study included 276 patients, of which 144 (52%) were treated surgically and 132 (48%) conservatively (mean (SD) age 77.3 (9.1) vs. 76.6 (9.7), P = 0.56). NDI improvement was largely similar between surgical and conservative treatments (mean (SE) −11 (2.4) vs. −14 (1.8), P = 0.08), as were union (86% vs. 78%, aOR 2.3, 95% CI 0.97–5.7) and stability (99% vs. 98%, aOR NA). NDI improvement did not differ between patients with union and persistent non-union (mean (SE) −13 (2.0) vs. −12 (2.8), P = 0.78). There was no difference for any of the secondary outcomes or subgroups. Conclusions: Clinical outcome and fracture healing at 52 weeks were similar between treatments. Clinical outcome and fracture union were not associated. Treatments should prioritize favourable clinical over radiological outcomes.
