This paper presents a systematic literature review exploring the Family Group Conferencing experiences with racially minoritised families in England. The literature search identified eight qualitative studies that met the inclusion criteria. This review found that racially minoritised families felt that Family Group Conferencing was empowering and met their cultural needs at times, but suggested caution about how culturally adaptable the approach is for this population. We argue that FGC could be presented as an Indigenous model of engaging with family networks with relevance for social workers working with racially minoritised communities in England. We suggest that Family Group Conferencing aligns with aims of decolonisation and cultural competence and argue the need to explore how Indigenous knowledge can be valued more effectively in global social work debates. This paper first reviews how England's middle-class Puritans developed various anxieties regarding their socioeconomic security in the emerging capitalist economy of early modern England. It then investigates three local land practices developed by Puritan colonists in New England to better secure their status in the New World, as revealed in a survey of town proceedings from Watertown, Massachusetts, from 1634 through 1773. These were restricting access to landownership, 'warning out' poor outsiders beyond town bounds, and managing land use. These practices enabled the middle-class, small-scale property owners who dominated local government in colonial Watertown to preserve the utility of land assets for future generations, manage the tax burden they associated with poor relief, and prevent conflicts arising among users of private and shared property. In doing so they maintained their households' status in keeping with their notions of material and spiritual well-being. The paper aligns these practices with later local land practices, including restrictive covenants and zoning, that middle-class communities have used to manage the particular pressures they face in the development of capitalist economies. There is a lack of definition in policy of the term co-design, and yet local authorities and developers are increasingly using it. To avoid that this term becomes meaningless, it is essential to define how to run co-design processes ethically. Building on case studies, professional experience, collaborations with communities, and a Participatory Action Research approach, this paper defines a set of principles on how to run a co-design process ethically and genuinely including communities in decision-making. This research paper examines the significance of the judges in the problem-solving courts of England and Wales's Choices and Consequences (C2) and Prolific Intensive (PI) programmes using the lenses and language of therapeutic jurisprudence. These unique schemes mobilise an intensive combination of strict control measures (with a view to deterring people from reoffending) alongside a personalised package of rehabilitative support overseen by a judge in a problem-solving court. Our findings strongly indicate that the judge-led problem-solving court is the bedrock of the schemes. Acknowledging that this practice relies upon strong leadership from a judge as a community convener with the authority and profile to initiate and sustain the programme, this paper identifies the strengths and barriers that this finding may pose. Our data also points to the difficulties of achieving support for the model at all judicial levels. Readiness (or lack of) within judges in the future could hamper the prospects of both current and new schemes. Moreover, finding a judge with a susceptible personality lowers chances. The authors conclude that the UK's current punitive, rapid results ethos of the justice system is not working. The international problem-solving court movement has shown that long-term success often ensues when practices are embedded into a broader culture of rehabilitative justice supported by visible communities. Although both the analysis of regional culture and urbanisation are long-standing preoccupations in geography, few studies have considered the relationship between the two, the former traditionally being a topic in cultural geography, while the latter is usually interpreted and analysed as a process in economic geography. Taking evidence from the 1851–1911 censuses of England and Wales, this article analyses individual migration paths to identify stable regions of human interaction by applying a sophisticated community-detection algorithm. By accurately mapping the regions within which the majority of migration occurred between 1851 and 1911 and arguing that the stability of these geographies is evidence of more than just mutable communities but rather of persistent regional cultures, this article responds to previous studies that have sought to identify the cultural provinces of England and Wales. Indeed, by demonstrating that the regions bear a striking resemblance to those that have long been hypothesised as being distinct cultural provinces of England and Wales, this article empirically corroborates their existence. In order to further demonstrate that the regions constitute cultural provinces, this paper incorporates these boundaries into a spatial interaction model (SIM). The results of the SIM not only shows that the boundaries between the regions limited the number of migrants that crossed them–over and above that explained by control variables–and therefore represented the boundaries of cultural provinces, demarcating discrete regions of human interaction–but that such boundaries disproportionately restricted rural-urban migrants, thereby slowing the pace at which England and Wales urbanised. This paper therefore demonstrates that urbanisation should not only be interpreted as only an economic phenomenon, but a cultural one also, and that if urbanisation is to be fully understood, individuals' attachment to place as a component of their identity, ought to be formally incorporated into models of migration. Hydraulic fracturing ('fracking') was a controversial issue in the United Kingdom that sparked national and community-led groups to organise protest mobilisations. To date, however, the social science literature has largely focussed upon general anti-fracking discourse rather than on the physical, community-led mobilisations that emerged from the frustrations of people directly affected at a local level by threats to their community. This paper develops and applies a novel conceptualisation of political opportunity structures at the nexus of the national and local levels to more fully explore the usually overlooked role of local-level structures in interaction with the national level in shaping protest. It uses protest event analysis with data derived from two key activist-specific sources. The analysis draws on data from over 1,400 protests occurring across 69 counties from 2011 to 2019. In so doing, this paper observes and accounts for variance in the form and frequency of community-led anti-fracking protest events within and between different areas of England across the life course of the protest episodes. This paper finds that trends in protest frequency and form over time correlate to shifts in opportunity structures, particularly regarding local and national-level interactions, and that this can be usefully conceptualised through a local-national-state-nexus. Placing today's anti-fracking protests amongst previous social struggles in Lancashire, this paper explores how radical environmental movements resist a capitalist drive for cheap Nature that endangers human and extra-human reproduction. Hydraulic fracturing encroaches on parts of nature that in the common sense of people have not yet completely lost their ontology of a commons. Clean water, air, silence and accessibility are not yet ontologically detached from the conditions for healthy lives and a vibrant community. Like the peasants who revolted in early-modern England because enclosures deprived them of access to firewood and land for grazing cattle, today local communities revolt to the new enclosures that deprive them and future generations from a clean environment, necessary for a healthy life. One of the more subtle and underexposed strategies of radical environmental movements is to emphasize and (re-)create an ontological unity between nature and humanity-in-nature as a reaction to the duality of modernity. This paper's approach, which combines the political economic history of enclosures with anthropologically inspired research on changing conceptions of nature, shows how radical environmental movements object to material conditions that result from ontological dualisms. The quest to deliver sustainable development has led to a search for ways to engage all stakeholders in this collective endeavour. Currently, local planners across England and elsewhere use independent design review panels to help raise the design quality of new developments. This paper examines the extent to which such panels can instill the need to adhere to sustainable development principles. We focus on the Cambridgeshire Quality Panel, which has framed its review process around sustainable development principles, named the "4 Cs": community, connectivity, climate, and character. Situating the panel's work within a design governance framework, we scrutinise the value and limitations of this particular governance tool. We conclude that local planners' ability to take forward the panel's recommendations on delivering new developments to high sustainability standards remains problematic, compromised by national priorities and market decisions. Purpose: Dentists may be seen as the most prominent figures within dentistry. Yet, a number of dental care professionals make significant contributions to the profession. The dental health educator (DHE) is one prime example. This teammate is typically a dental nurse who has achieved a post-registration qualification in oral health education (OHE). Through interactions with patients, members of the community and other pertinent stakeholders, the DHE empowers people through promoting the importance of good oral health. The purpose of this paper is to cast light on the unique and invaluable roles that DHEs can play within their organisations. Design/methodology/approach: This paper aims to share some examples of dentist-DHE collaborations, where OHE initiatives were implemented within an ethnically diverse London Borough in England. Findings: It was found that such interprofessional, skill-mix efforts were more productive, when the dentist and DHE worked together, rather than the latter working independently. Originality/value: There exists great potential for DHEs to act as ambassadors for their dental establishments, network and collaborate with other organisations and build worthwhile relations with other healthcare professionals and stakeholders. Background Valuable learning derived from public health practice can be captured through practice-based case studies, also known as practice examples. Practice examples of participatory interventions supplement the evidence base by providing information on the complexities of implementation in communities. This paper reports on a Public Health England project to build a bank of community-centered practice examples based on robust processes of collection and curation. Methods The multidisciplinary project had three phases: (i) development and piloting a process to collect practice examples, (ii) refining review processes and gathering further examples via national and regional teams (iii) maintenance of an accessible collection on the library platform. Results The project resulted in a searchable collection of 55 practice examples illustrating participatory approaches in public health practice. The collection shows diversity in terms of settings, population, focus and type of approach used to work with communities. A secondary outcome was the development of generic guidance and templates for further collections on public health topics. Conclusions This project illustrates how information on the implementation of community-centered approaches in real-life contexts can be gathered and disseminated through a transferable process. Background: Involving communities in research priority setting can increase the relevance and efficiency of research, leading to better health outcomes. However these exercises often lack clarity in how communities are involved and the extent to which priorities are acted upon is unclear. Seldom-heard groups, for example ethnic minorities may experience barriers to participation. We report methods and outcomes of an inclusive co-produced community research priority setting exercise within the multicultural and deprived city of Bradford, UK. The aim was to identify priorities for keeping children happy and healthy and was undertaken by the Born in Bradford (BiB) research programme to inform future research agendas. Methods: A 12 member multi-disciplinary, multi-ethnic community steering group led the process using a modified James Lind Alliance approach between December 2018-March 2020. Research priorities were collected through a widely distributed paper and online survey. Respondents were asked to list three important things to keep children i) happy, ii) healthy and what needs to change to improve either health or happiness. Free text data were coded iteratively by community researchers, and shared priorities were co-produced in a series of workshops and meetings with the community steering group and community members. Results: Five hundred eighty-eight respondents to the survey identified 5748 priorities, which were coded into 22 themes. These covered a range of individual, social and wider socioeconomic, environmental and cultural priorities. Diet/nutrition and exercise were most commonly identified as important for health, including what needs to change to improve health. For happiness, home life and family relationships, listening to children, and education/activities were the most commonly identified. Community assets were identified as important to change for both health and happiness. From the survey response the steering group developed 27 research questions. There were mapped onto existing and planned research agendas within BiB. Conclusions: Communities identified both structural and individual factors as important priorities for health and happiness. We demonstrate how communities can be involved in priority setting using a co-productive approach in the hope this can be used as a model for others. Background: Big data research requires public support. It has been argued that this can be achieved by public involvement and engagement to ensure that public views are at the centre of research projects. Researchers should aim to include diverse communities, including seldom‐heard voices, to ensure that a range of voices are heard and that research is meaningful to them. Objective: We explored how researchers involve and engage seldom‐heard communities around big data research. Methods: This is a qualitative study. Researchers who had experience of involving or engaging seldom‐heard communities in big data research were recruited. They were based in England (n = 5), Scotland (n = 4), Belgium (n = 2) and Canada (n = 1). Twelve semistructured interviews were conducted on Zoom. All interviews were audio‐recorded and transcribed, and we used reflexive thematic analysis to analyse participants' experiences. Results: The analysis highlighted the complexity of involving and engaging seldom‐heard communities around big data research. Four themes were developed to represent participants' experiences: (1) abstraction and complexity of big data, (2) one size does not fit all, (3) working in partnership and (4) empowering the public contribution. Conclusion: The study offers researchers a better understanding of how to involve and engage seldom‐heard communities in a meaningful way around big data research. There is no one right approach, with involvement and engagement activities required to be project‐specific and dependent on the public contributors, researchers' needs, resources and time available. This paper explores how relational approaches to well‐being could support young children's educational achievement in high‐poverty contexts. It draws on findings from a qualitative study involving mothers and early years educators living and/or working in a city characterised as one of the most disadvantaged in England. The findings suggest that children's well‐being, rather than being merely an individual characteristic or aspiration, is interdependent with their social and material environments, as are the institutions that support them. The paper concludes by calling for a recalibration of early childhood policies away from assessing individual children's 'school readiness' to encouraging society's readiness to support everyone's well‐being, and consequently that of young children too. Educational disruptions during COVID-19 in periods of lockdown have redirected attention to homes and parents as key partners in schooling. Educational literature explores multidimensional disadvantages faced by communities in England. COVID-associated school closures changed the relationship between school and home. Parents and family played an important role to provide support for their children's education. There is little reflection on the experiences of parenting during this time of uncertainty. The paper focuses on the parenting experiences of Bangladeshi and Pakistani families with GCSE years children (years 7-11) in supporting their children's education during periods of abrupt school closures. Qualitative data were collected during the third national lockdown from 19-families in England using semi-structured narrative interviews. We find that most of the parents were largely satisfied with the support provided through schools. However, resources were unevenly distributed, and the learning experience was non-interactive for many children. Parental anxiety for their children's futures and education was high. Nevertheless, families adopted diverse strategies to support their children by taking a more active role in providing educational as well as socio-emotional support. Notable in these accounts were significant reflections from parents, who regardless of the adversities, experienced a strengthening of the family as a unit. [ABSTRACT FROM AUTHOR]

The COVID-19 pandemic has had high mortality rates in the elderly and frail worldwide, particularly in care homes. This is driven by the difficulty of isolating care homes from the wider community, the large population sizes within care facilities (relative to typical households), and the age/frailty of the residents. To quantify the mortality risk posed by disease, the case fatality risk (CFR) is an important tool. This quantifies the proportion of cases that result in death. Throughout the pandemic, CFR amongst care home residents in England has been monitored closely. To estimate CFR, we apply both novel and existing methods to data on deaths in care homes, collected by Public Health England and the Care Quality Commission. We compare these different methods, evaluating their relative strengths and weaknesses. Using these methods, we estimate temporal trends in the instantaneous CFR (at both daily and weekly resolutions) and the overall CFR across the whole of England, and dis-aggregated at regional level. We also investigate how the CFR varies based on age and on the type of care required, dis-aggregating by whether care homes include nursing staff and by age of residents. This work has contributed to the summary of measures used for monitoring the UK epidemic. Author summary: During an epidemic, the case fatality risk (CFR), i.e. the probability that an individual dies after testing positive for a disease, is a key parameter informing the public health response. However, calculating the CFR is not trivial, since there are cases who may die in the future but have not died yet. Therefore, statistical methods are required to correct for the distribution of times between testing positive and dying. In this paper, we derive multiple methods, some existing and some novel, within a consistent methodological framework. This allows us to understand how these different approaches are related and their relative strengths and weaknesses. During the COVID-19 pandemic, care homes have been particularly affected, due to the high risk of COVID-19-associated mortality in the frail and elderly. How humans interact with nature affects the health of both people and ecosystems. Yet, long‐term data on nature engagement are scarce because traditional survey methods are expensive, time consuming and require commitment over multiple years. Digital data sources (e.g. aggregated data from online searches) have major potential as a supplementary source of information and, in the absence of available data, as a proxy for more direct measurement of nature engagement.Using Google Trends, we created a list of refined and relevant search terms relating to diverse outdoor spaces and activities. We then compared trends in Google search volumes in England across both a 1‐year and 10‐year period to those from Google Community Mobility Reports, and from nationally representative survey data (Natural England's People and Nature Survey and the Monitor of Engagement with the Natural Environment).Search, survey and mobility data all support a general increase in public engagement with nature since 2009, and a more substantial increase during, or following, the initial national 'lockdown' period of the COVID‐19 pandemic in England. Search volumes increased for many urban and rural outdoor green spaces (e.g. woodlands), blue spaces (e.g. reservoirs), exercise activities (e.g. walking, running and hiking) and explicitly nature‐based activities (e.g. fishing, wild swimming and encouraging wildlife).Overall, volumes of Google searches were more closely related with longer‐term (10‐year) trends from survey data, than shorter‐term changes during the COVID‐19 pandemic. There were statistically significant relationships between search volumes, survey data (self‐reported past behaviour) and mobility data (movement trends) for around half of comparisons. Of these, an average of 13–44% of variation in the data was explained.The findings show that Google Trends provides valuable information about public engagement with nature, which can help to supplement existing survey data by providing new insights about behavioural trends. Coastal communities have received little attention in the public health literature, perhaps because our mental maps tend to associate socio-economic deprivation and health inequalities with inner cities. Mapping a range of key health indicators at small area level, this paper reveals a distinct core-periphery pattern in disease prevalence, with coastal communities experiencing a high burden of ill health across almost all conditions included in the Quality and Outcomes Framework dataset. Other sources suggest poor outcomes for children and young people living in coastal areas. Low rates of participation in higher education contrast with high rates of hospitalisation for self-harm, alcohol and substance use. Reflecting a shift in the distribution of children living in poverty since the 1990s, this may be an early indicator of a future public health crisis in these communities. Exploring reasons for the health challenges facing the periphery, this perspective piece calls for more public health research that can accommodate the complex and interlinked problems facing coastal communities and a more concerted effort to align public health with economic, education, local government and transport policies at the national level. This paper presents two remarkably similar characterological figures who are stereotyped embodiments of working-class personas: Haagse Harry in The Hague and chavs in England. The two figures have similar attires, class positions, attitudes, and associated attributes. We compare and contextualize the indexical links between their linguistic features and their social characteristics. Firstly, while chavs can be both men and women, the fictional persona Haagse Harry represents an all-male lower-working-class subculture. Secondly, while Haagse Harry consistently speaks Broad Haags, the language of chavs is not rooted in any single regional dialect but invariably indexes working-class features. Thirdly, Haagse Harry, and his sociolect, has a higher social status compared to the language and persona of chavs, who embody British class prejudice. We demonstrate that the repertoire of linguistic features deployed in the stylisation of characterological figures is strongly dependent on patterns of variation and ideas that are prevalent in the local speech community. To dispose permanently of its higher activity nuclear waste England and Wales have chosen deep geological disposal as the most appropriate solution currently available. The purpose of this paper is to describe the main geological features, events and processes relevant to England and Wales that will need to be considered to demonstrate that a site is suitable for a geological disposal facility (GDF). England and Wales are in the early stages of a GDF siting process in which areas of interest are being evaluated using mainly existing data from surface mapping and hydrocarbon exploration and production. Sites are evaluated consistently under six overarching headings, three of which are impacted by their geological setting – safety, engineering feasibility and value for money. "Suitable" geology is that which is safe during the operational and long-term post-closure period, which could have a GDF and its accessways constructed within it, and which delivers value for money. A GDF needs to fulfil dual safety functions wherever it is located: long-term containment of radionuclides, and isolation of the waste from human actions and from natural processes such as glaciations and earthquakes. The role of the geosphere in delivering these safety functions is to provide a low-flux groundwater environment with geochemical conditions that minimise degradation of the engineered components of the GDF, to promote retention of mobilised radionuclides, and to protect the waste from the impacts of humans and natural processes. The containment function of a GDF is provided by a combination of rock and engineering generally referred to as the multibarrier system. It comprises the engineered barriers – solid wasteforms, canisters, buffers, backfill materials, plugs and seals – that work together with the rock to ensure long-term containment. The GDF Programme in England and Wales seeks to identify suitable geological environments for which bespoke engineered barriers can be tailored to optimize the performance of the multibarrier system. The post-closure period over which independent regulators will require a safety case to demonstrate the long-term containment and isolation capabilities of a GDF is up to 1 million years. The long timescales make post-closure safety assessments a unique feature of deep geological disposal programmes. A comprehensive site characterization programme will use information mostly from seismic surveying and deep investigation boreholes to establish adequate rock availability (host rock depth, thickness, areal extent and compartmentalisation), suitable properties and behaviour of the deep geological environment, and the constructability and operability of a potential GDF site including its surface to subsurface access ways. Nuclear Waste Services, the organisation tasked with developing a GDF in England and Wales, is currently engaged with four Community Partnerships through a volunteer siting process: three in west Cumbria, and one on the English east coast in Theddlethorpe, Lincolnshire. In all of these areas Mesozoic claystones have been provisionally identified as potentially suitable GDF host rocks and are being investigated further, with a dedicated 3D seismic survey acquired off the coast of Cumbria in 2022. The main conclusion to be drawn from this paper is that a GDF could be sited in a large number of geological settings in England and Wales, and that the success of the current siting process will largely depend on engaging effectively with willing communities and building enduring relationships with them. Professional sports clubs (PSCs) are potentially effective settings for health promotion; however, their role within policy is unclear. Potential reasons include lack of awareness about existing provision of health and wellbeing (H&W) programmes and adequacy of monitoring and evaluation (M&E) practices. This review aimed to: (i) map the provision of H&W programmes delivered by PSCs in the United Kingdom (UK), and (ii) explore current M&E practices of PSCs and consider the policy implications of this. Websites from eight professional sport leagues were hand-searched for programmes and impact reports. Suitable programmes were quantified, whilst impact reports were analysed via inductive documentary content analysis. Results identified 176 H&W programmes and 36 impact reports, as well as 43 H&W impact statements, but only 14 of these were aligned to specific H&W outcomes. The H&W aims of programmes were typically vague, measurement tools were rarely used, and evaluations were usually anecdotal case studies and based on engagement figures, which may not only limit the potential uptake of these programmes but also the relevance of PSCs to public policy. Further research is thus needed to build a stronger evidence base for the use of PSCs as vehicles of public health promotion and policy, and to better address the challenges faced when seeking to monitor and evaluate PSC programmes effectively. Purpose: This study aims to reflect upon the first wave of training of Education Mental Health Practitioners (EMHPs), a new National Health Service role to provide support for Children and Young People (CYP) with low and moderate mental health needs in education settings in England. The study specifically focusses on the training for EMHPs in relation to their support for CYP who identify themselves as Black, Asian or Minority Ethnic (BAME), refugees or from the traveller community. Design/methodology/approach: A brief review of the policy and literature on the role and remit of EMHPs was undertaken, including an exploration of the current status of BAME, refugee and traveller community CYP in schools in England. The review was then related to the specific experience of the author within the context of teaching EMHP trainees in a higher educational setting and evaluated as developing outcomes in low-intensity school-based practice. Findings: There are benefits for trainee EMHPs to have an understanding of the minority groups of CYP attending schools in the area their Mental Health Support Team (MHST) covers. There are benefits for the MHST to form relationships with minority groups at an early stage in the MHST formation. Adapting the EMHP curriculum at a local level to include specific training on the needs of minority groups supports the development of relationships between schools and the communities they engage with locally. Ongoing training should be provided by services focussing on the specific needs of minority groups in their MHST area. Originality/value: Involving minority groups in education in the formation of MHST and the training of EMHPs may improve outcomes in developing therapeutic relationships with CYP. Developing engagement practices in MHSTs with higher education providers, begins the process early in the experience of EMHP trainees, providing a safe environment in which to develop engagement skills. Respiratory viruses that were suppressed through previous lockdowns during the COVID-19 pandemic have recently started to co-circulate with SARS-CoV-2. Understanding the clinical characteristics and symptomatology of different respiratory viral infections can help address the challenges related to the identification of cases and the understanding of SARS-CoV-2 variants' evolutionary patterns. Flu Watch (2006–2011) and Virus Watch (2020–2022) are household community cohort studies monitoring the epidemiology of influenza, respiratory syncytial virus, rhinovirus, seasonal coronavirus, and SARS-CoV-2, in England and Wales. This study describes and compares the proportion of symptoms reported during illnesses infected by common respiratory viruses. The SARS-CoV-2 symptom profile increasingly resembles that of other respiratory viruses as new strains emerge. Increased cough, sore throat, runny nose, and sneezing are associated with the emergence of the Omicron strains. As SARS-CoV-2 becomes endemic, monitoring the evolution of its symptomatology associated with new variants will be critical for clinical surveillance. [ABSTRACT FROM AUTHOR]

Adopting a qualitative case study design, this article draws upon the concept of ableism to analyse the extent to which mainstreaming policy in the UK leads to inclusive sport practice at the community level. In-depth qualitative data were collected from 31 semi-structured interviews with stakeholders in the inclusion process in England including sports organisations, officials in community sports clubs and disabled people. Data were thematically analysed to explore how stakeholders understood inclusion and what the role of ableism might be in formulating this understanding. The findings illustrate that ableism appears to play a key role in the understanding of inclusion and how it is operationalised in different clubs and sports organisations. This in turn impacts whether disabled people feel able to participate within that environment. The research identified three outcomes of inclusion (parallel inclusion, full inclusion and choice) and four approaches used or necessary to achieve the three outcomes by stakeholders (able-inclusion, barrier removal, creating opportunities and mutual identity). This article identifies that, irrespective of policy intent, the way inclusion policy is understood by those that have to operationalise it is often underpinned by an ableist view of disability, meaning that the desired increases in participation may not materialise. Based on the findings, it is suggested that sport organisations should strategically embed disability provision and should actively rather than passively engage with disabled people. Background: UK local authorities that experienced sustained high levels of COVID-19 between 1st March 2020 and 28th February 2021 were described by the UK Scientific Advisory Group for Emergencies as areas of enduring prevalence. This research was carried out in order to examine the views of local authority Directors of Public Health, who played a crucial role in the local response to COVID-19, on reasons for sustained high levels of prevalence in some areas, alongside an investigation of the mitigation strategies that they implemented during the course of the pandemic. Methods: Interviews were conducted with Directors of Public Health in 19 local authority areas across England, between July and November 2021. This included nine areas identified as areas of enduring prevalence and ten 'comparison' areas. Results: The outcomes of this study suggests that the geographical differences in prevalence rates are strongly influenced by health inequalities. Structural factors including deprivation, employment, and housing, due to their disproportionate impact on specific groups, converged with demographic factors, including ethnicity and age, and vaccination rates, and were identified as the main drivers of enduring prevalence. There are key differences in these drivers both within and, to a lesser extent, between local authorities. Other than these structural barriers, no major differences in facilitators or barriers to COVID-19 mitigation were identified between areas of varying prevalence. The main features of successful mitigation strategies were a locally tailored approach and partnership working involving local authority departments working with local health, community, voluntary and business organisations. Conclusions: This study is the first to add the voices of Directors of Public Health, who played a crucial role in the local COVID-19 response. Areas of enduring prevalence existed during the pandemic which were caused by a complex mix of structural factors related to inequalities. Participants advised that more research is needed on the effectiveness of mitigation strategies and other measures to reduce the impact of structural inequalities, to better understand the factors that drive prevalence. Background: The UK was the first country to launch a national pandemic COVID-19 vaccination programme, which was implemented swiftly despite significant vaccine supply constraints. The delivery strategy used a combination of mass vaccination sites operated by NHS secondary care providers and local sites led by Primary Care Networks, and local pharmacies. Despite nation-wide rollout, persistent gaps in coverage continued to affect particular populations, including ethnic minority and marginalised social groups. Aim: The study examined sub-national immunisation commissioners and providers' perspectives on how the COVID-19 vaccine programme was operationalised, and how delivery strategies impacted inequalities in access to vaccination services and uptake. The study aimed to inform national programme implementation, sustainability and future pandemic preparedness. Methods: Qualitative research was conducted in eight local NHS areas in 4 regions of England. Semi-structured interviews were performed with 82 sub-national NHS and public health vaccine providers and commissioners. Results: England's COVID-19 vaccination programme was described as top down, centralised and highly political. The programme gradually morphed from a predominantly mass vaccination strategy into more locally driven and tailored approaches able to respond more effectively to inequalities in uptake. Over time more flexibility was introduced, as providers adapted services by "working around" the national systems for vaccine supply and appointment booking. The constant change faced by providers and commissioners was mitigated by high staff motivation and resilience, local collaboration and pragmatism. Opportunities for efficient implementation were missed because priority was given to achieving national performance targets at the expense of a more flexible sub-national tailored delivery. Conclusion: Pandemic vaccination delivery models need to be adapted for underserved and hesitant groups, working in collaboration with local actors. Learnings from the initial COVID-19 vaccine roll-out in England and elsewhere is important to inform future pandemic responses, in tailoring strategies to local communities, and improve large-scale vaccination programmes. Background Social participation is linked to better health and well-being. However, there is limited research on the individual and area-level predictors of participation. This study aims to determine the characteristics associated with participation, particularly the impact of community asset availability. Methods We used data from 34 582 adult respondents to the nationally representative Community Life Survey from 2013 to 2018. We measured social participation by reported participation in 15 types of groups. We used probit and negative binomial regression models and included a wide range of individual, household and area characteristics, and availability of 14 types of community assets. Results The following characteristics were associated with higher levels of participation: being female (+3.0 percentage points (p.p.) (95% CI 1.8 to 4.1p.p.), Black, Asian or Minority Ethnicity (+3.7p.p. (1.9 to 5.5p.p.)), homeownership (+4.1 p.p. (2.7 to 5.6p.p.)) and living in a rural area (+2.1p.p. (0.5 to 3.6p.p)). Respondents from the most deprived areas were less likely to participate than those in average deprivation areas (−3.9p.p. (−5.9 to −1.99p.p.)). Higher availability of community assets was associated with increased participation in groups. The effect of availability on participation varied by type of asset. Using Gorz's writing on cities and time as a starting point, this sensory ethnographic study uses pinhole photography to explore how time feels for 'unemployed' volunteers at a community garden in the north-east of England. It upholds that the garden's ability to fill time meaningfully is grounded in the food-growing and composting cycle but is also anchored to the mature trees, structures and artworks – made, grown or maintained by the volunteers themselves, that persist in the space for many years. We argue that urban community gardens offer their denizens an 'elongated present' that is fulfilling to the individual while also sustaining community and nature. Emphasising the need for enduring, rather than temporary or pop-up, growing spaces in helping us transition to a sustainable, post-work society, the study thus adds temporal insight to existing scholarship on the importance of community gardens. Despite increasing attention on how community enterprises support community well-being and public services, we know little about how durable they are to sustain their activities. Previous research has studied individual conditions for durability, ignoring the intricate interplay between these conditions. Based on literature on community enterprises, we identified five key conditions for durability. Using semi-structured interviews with representatives of 19 community enterprises, this article aims to better understand the interplay between these five conditions affects community enterprises' durability. A fuzzy-set Qualitative Comparative Analysis revealed 'aligned entrepreneurial culture' as a necessary condition and identifies two configurations of conditions sufficient for durability. Objective: To explore the views of female genital mutilation (FGM) survivors, men and healthcare professionals (HCPs) on the timing of deinfibulation surgery and NHS service provision. Design: Qualitative study informed by the sound of silence framework. Setting: Survivors and men were recruited from three FGM prevalent areas of England. HCPs and stakeholders were from across the UK. Sample Forty‐four survivors, 13 men and 44 HCPs. Ten participants at two community workshops and 30 stakeholders at a national workshop. Methods: Hybrid framework analysis of 101 interviews and three workshops. Results: There was no consensus across groups on the optimal timing of deinfibulation for survivors who wished to be deinfibulated. Within group, survivors expressed a preference for deinfibulation pre‐pregnancy and HCPs antenatal deinfibulation. There was no consensus for men. Participants reported that deinfibulation should take place in a hospital setting and be undertaken by a suitable HCP. Decision making around deinfibulation was complex but for those who underwent surgery it helped to mitigate FGM impacts. Although there were examples of good practice, in general, FGM service provision was suboptimal. Conclusion: Deinfibulation services need to be widely advertised. Information should highlight that the procedure can be carried out at different time points, according to preference, and in a hospital by suitable HCPs. Future services should ideally be developed with survivors, to ensure that they are clinically and culturally appropriate. This article contributes to our understanding of the complex role of civil society organisations (CSOs) in tackling precariousness through advice, advocacy and activism. It draws on qualitative data gathered primarily from two local CSOs in the north of England that help clients navigate a highly flexible labour market and an increasingly punitive welfare system. The findings reveal that in marginalised communities, CSOs compensate for retreating state services by providing clients with individual advice and advocacy, but there is little evidence of the grassroots activism observed in labour CSOs in North America. We argue that the uneven tradition of community organising across cities in the UK combined with the complex dependencies of service‐oriented CSOs on state resources has restricted their role to that of labour market intermediaries that serve primarily to integrate clients into low‐wage jobs. This article recognizes the significance of commercial entertainment producers in early modern England operating outside of London. In doing so, it offers fresh methodological approaches for understanding pre-modern social status. I explore the geographical and social places of independent bearwards – individuals who kept bears for the commercial sport of baiting. Regional figures involved in entertainment production have been little explored and left behind frustratingly few biographical details. Yet three generations of one family – the Whitestones of Ormskirk in Lancashire (1610s through the 1630s) – do leave substantial surviving documentary evidence about their activities, assets, and networks. I use the Whitestones's probate inventories and wills and their and their neighbours' court depositions and petitions to offer for the first time a holistic appraisal of the material, economic, and cultural circumstances of the bearward. By stepping inside the households and communities of several generations of independent entertainment producers, we can appreciate their complex and variable social status and the role of commercial recreation in social mobility. I finish by considering the human–animal relationships that underpinned the bearward's place in early modern England, offering fresh evidence of bears' living arrangements and a theoretical framework for discussing their exploitation in the blood sport industry. As part of an NHS England pilot scheme, Specialist Community Forensic Teams were developed aiming to support service users' transition of care from secure forensic inpatient settings to living in the community. This study explored the experiences of service users, carers, health professionals and specialist residential accommodation providers in relation to one of these teams. The Good Lives Model was adapted by the team to inform formulation and intervention. Nineteen participants completed a mixed-methods survey. Data were processed using descriptive statistics and thematic analysis. Findings indicate that the use of a strengths-based model within a forensic setting was perceived by stakeholders as providing an experience of relational security and hopefulness. Most stakeholders surveyed endorsed survey items indicative of a positive experience of the service. The findings from this study suggest that GLM is an acceptable model to stakeholders for formulating the needs and strengths of service users in planning their transition from residing in secure settings to living in the community. Objectives: Falls are the most common cause of injury related deaths in people over 75 years. The aim of this study was to explore the experience of providers (instructors) and service users (clients) of a fall's prevention exercise programme and the impact of the COVID-19 pandemic in Derbyshire, UK. Methods: Ten one-to-one interviews with class instructors and five focus groups with clients (n=41). Transcripts were analysed using inductive thematic analysis. Results: Most clients were initially motivated to attend the programme to improve their physical health. All clients reported improvements in their physical health as a result of attending the classes; additional benefits to social cohesion were also widely discussed. Clients referred to the support provided by instructors during the pandemic (online classes and telephone calls) as a 'life-line'. Clients and instructors thought more could be done to advertise the programme, especially linking in with community and healthcare services. Conclusions: The benefits of attending exercise classes went beyond the intended purpose of improving fitness and reducing the risk of falls, extending into improved mental and social wellbeing. During the pandemic the programme also prevented feelings of isolation. Participants felt more could be done to advertise the service and increase referrals from healthcare settings. • City's pain treatment in a 13-month intercity longitudinal study estimated using WBE. • WBE triangulated with per postcode National Health Service (NHS) prescription data. • Strong seasonal patterns of short pain medications vs chronic pain treatment observed. • Topical (non-metabolic) application/down-the-drain disposal of pharmaceuticals observed. • Population size is the main driver of pharma loads in wastewater. This paper explores Wastewater-Based Epidemiology (WBE) as a tool enabling understanding of city's pain treatment in an intercity longitudinal study. An intensive 13-month monitoring programme was undertaken in two adjacent urban areas in South-West England: a small commuter town Keynsham and the city of Bath (>180 samples collected). The study has shown a great potential of using triangulated WBE and National health Service (NHS) prescription data in understanding pain treatment in two contrasting communities with strong apparent seasonal patterns of short pain medications vs chronic pain treatment as well as the type of treatment used (e.g. oral vs topical). Community-wide usage of Non-Steroidal Anti-inflammatory Drugs (NSAIDs) and paracetamol in the intercity study is population size and season driven with the highest usage recorded in winter months. This contrasts with other pain pharmaceuticals, especially those used for chronic pain, where no/limited seasonal usage was recorded. Unmetabolized NSAIDs are, to a large extent, directly disposed of into the sewerage system bypassing metabolism due to their topical application. This is particularly apparent in winter months with naproxen showing the highest seasonal variability. Pharma/met (ratio of pharmaceutical and its metabolite concentration) analysis allows for tracking topical (non-metabolic) application/down-the-drain disposal of pharmaceuticals with frequent instances of direct disposal of NSAIDs into the sewerage system observed. Normalisation of pharma markers to population size shows comparable estimates of pharma usage in the two cities confirming population as the main driver of pharma loads in wastewater. Variable application patterns of pain pharmaceuticals make back-calculation of intake more convoluted. Intake calculated using percentage excretion of parent NSAIDs will likely lead to overestimation, as it is assumed that NSAIDs are subject to extensive metabolism (this is not the case for topical applications). Intake calculated using percentage excretion of metabolites (or parent compound) as consumption markers leads to underestimation of NSAIDs usage due to contributions from topical application not being accounted for. Prescription data indicates cumulative internal and topical usage, but the data ignores large proportion of over-the-counter usage. Therefore, we have proposed a combined approach allowing for estimation of total usage including, and differentiating between, topical application and oral administration. [ABSTRACT FROM AUTHOR]

This paper reports findings from a qualitative study of girl and young women skateboarders in and around two cities in England. We consider what it means to have a skateboarder identity, and how this is related to commitment, skill level, and participation in local skateboarder communities. We found that girls and young women frequently need more support to take up skateboarding than do boys and young men. Crucial enablers are both structural and personal, and include: initial and ongoing support, often from young men skateboarders; women role models in skate spaces; and official women and girl only sessions. Factors inhibiting young women skateboarders include: the public perception of skateboarding; harassment from members of the public; feeling uncomfortable in skate spaces; and hassle from men and boy skateboarders. We conclude that, while skateboarding, both officially and locally, is working to become more inclusive, it remains embedded in wider gendered power/knowledge relations, which privilege and support its domination by men. Photovoltaic solar farms are utility-level ground-mounted arrays of interconnected panels that convert sunlight into electricity. Little is known about the extent of any disamenity impact from these farms, despite numerous communities objecting to their construction. This study uses a property fixed-effects model to examine the disamenity impact of photovoltaic solar farms on households in England and Wales, as revealed by changes in property prices. Properties located ≤ 750 m south of an operational solar farm greater than 5 megawatts in capacity suffer a 5.4% reduction in relative prices. The effect of solar farms ≤ 5 megawatts in capacity or located more than 750 m south of properties is statistically insignificant. [ABSTRACT FROM AUTHOR]

The aim of this research was to identify the extent of, and barriers, to pro-environmental activity within community-based, association football clubs in England. Semi-structured interviews were undertaken with volunteers at community football clubs in the FA county of "North Country". The ownership and management of football facilities provides for a low level of independent decision making by clubs and negates the encouragement of pro-environmental activity. Strategies for increased environmental activity in football need to look beyond individual clubs and volunteers to the way the sport is organised. Community Protection Notices (CPNs) were introduced in England and Wales by the Anti-Social Behaviour, Crime and Policing Act (2014) to tackle anti-social behaviour (ASB) which 'spoils the community's quality of life'. The notices can prevent and/or require specific actions by the recipient where the behaviour in question has a 'detrimental impact on the quality of life of those in the locality'. Breach of the notice incurs a £100 fine through a Fixed Penalty Notice or a possible criminal conviction. CPNs are issued by individual officers without the need to go to court and a low evidential threshold is employed. Almost any behaviour can be sanctioned by a CPN and they are being used to address ASB attributed to people experiencing street homelessness by prohibiting behaviours such as begging, the creation of sleeping structures, and street drinking. This article draws on empirical evidence generated through a qualitative study conducted across four case study areas in England between 2019 and 2021. We use Lipsky's (2010) work on street-level bureaucracy and Tyler's (2006) theory of distributive fairness as lenses to explore how frontline practitioners use CPNs within a homelessness context. The findings demonstrate divergent uses of the notices, specifically in relation to the CPN issuing process, the incremental intervention approach, and enforcement for breach. This provides a unique insight into how ASB policy is being used to manage a vulnerable population and we offer three research-informed recommendations for policy and practice. Background: Many male prisoners have significant mental health problems, including anxiety and depression. High proportions struggle with homelessness and substance misuse. Aims: This study aims to evaluate whether the Engager intervention improves mental health outcomes following release. Method: The design is a parallel randomised superiority trial that was conducted in the North West and South West of England (ISRCTN11707331). Men serving a prison sentence of 2 years or less were individually allocated 1:1 to either the intervention (Engager plus usual care) or usual care alone. Engager included psychological and practical support in prison, on release and for 3–5 months in the community. The primary outcome was the Clinical Outcomes in Routine Evaluation Outcome Measure (CORE-OM), 6 months after release. Primary analysis compared groups based on intention-to-treat (ITT). Results: In total, 280 men were randomised out of the 396 who were potentially eligible and agreed to participate; 105 did not meet the mental health inclusion criteria. There was no mean difference in the ITT complete case analysis between groups (92 in each arm) for change in the CORE-OM score (1.1, 95% CI –1.1 to 3.2, P = 0.325) or secondary analyses. There were no consistent clinically significant between-group differences for secondary outcomes. Full delivery was not achieved, with 77% (108/140) receiving community-based contact. Conclusions: Engager is the first trial of a collaborative care intervention adapted for prison leavers. The intervention was not shown to be effective using standard outcome measures. This article reports research exploring students’ perceptions of higher education in traditional and local universities in England and Cuba. The study has explored parallels identified in the course of researchinformed teaching that harvested qualitative questionnaire data from students at two English universities. One a Traditional (high-status and elite) institution, the other a Local (post-92 widening participation) university. This data was analysed in relation to pre-existing qualitative data from two Cuban universities with similar profiles. Thematic analysis extrapolates parallels in relation to students’ experiences and motivations, and considers the relative impact of political culture, systems of access and choice, and the community character of each of the institutions. Key findings suggest that the marketisation of the system in England provides choice that is dependent on (and therefore reinforces) socio-economic status. They further suggest that both a participatory political culture and local modes of study can be effective in developing perceptions of higher education that are more closely aligned with social contribution and the collective good. Background: Minoritised ethnic perinatal women can experience judgemental and stigmatising care due to systemic racism. Discriminatory care contributes to increased risks of poor maternal and infant outcomes, including higher rates of mental ill-health. This study aimed to explore minoritised ethnic women's experiences of maternity services, including maternity care and mental health support, within a North-West England locality. Here we use an equity lens to report the findings that describe if and how women's personal, cultural, and spiritual needs were met, their experiences of discriminatory and prejudicial care, and to identify recommendations for service provision. Methods: A mixed-methods study was undertaken comprising an online survey, interviews, and community consultations. Questions explored access to and experiences of antenatal care and education; information, communication, and choice; experiences of (dis)respect and judgement; mental health needs and support; cultural/religious needs and support; and overall experiences of maternity care. Eligibility criteria were: women, 18+ years, from self-reported minoritised ethnic backgrounds, who had given birth in the previous 2 years and received maternity care in the locality. Surveys were available in seven languages and distributed via social media, mother-baby groups, and community locations. English-speaking survey participants were invited to take part in a follow-up interview. Community staff were approached to collect data on behalf of the study team. Quantitative data were analysed descriptively (n, %) and merged with qualitative data into descriptive themes. Results: Overall, 104 women provided data; most self-identified as Asian (65.0%) or Black (10.7%) and were aged between 30–34 (32.0%) or 25–29 years (23.3%). Four descriptive themes are reported: 'accessing care' details variations and barriers in accessing maternity care; 'communication needs, and resources' describes views on adaptions and resources for specific communication needs; 'meeting religious and cultural needs' outlines how various religious and cultural needs were met by maternity providers; 'discriminatory or stigmatising care' reports on experiences of pejorative and inequitable care. Conclusions: An equity lens helped identify areas of discriminatory and inequitable care. Key recommendations include cultural safety training for staff; service-user engagement and co-production of research and resources, and appropriate facilities and recording systems to facilitate individualised, needs-based maternity care. Rewilding is an increasingly common conservation approach, aiming to restore ecosystem processes and minimise human intervention. Rewilding has the potential to profoundly change landscapes and people–nature relations. These issues prompt an ongoing debate about how and if rewilding should be done.Farmers are key stakeholders in this debate; they stand to be both affected by and influence the trajectory of rewilding initiatives developing in the United Kingdom and globally. Despite this, a comprehensive understanding of farmers' perceptions towards rewilding is lacking.Here, we focus on how members of the farming community in England perceive common rewilding scenarios (beaver release, farm‐level rewilding and landscape‐scale rewilding), and how these perceptions shape farmers' attitudinal support for rewilding practices.Using thematic analysis of semi‐structured interviews with 36 farmers and farming representatives, we show that the diversity of famers' attitudes can be understood through the prism of perceptions on five core issues: (a) the perceived need for restoration action, (b) the ecological effectiveness of rewilding, (c) rewilding's compatibility with ensuring food security, (d) rewilding's compatibility with rural lifestyles, livelihoods and economies and (e) multidimensional justice of rewilding initiatives. These issues are rooted in collective farming values, and farmers' perceptions of these issues are influenced by mental models, perceived social impacts and perceived ecological outcomes of rewilding initiatives. Diverse perceptions result in a range of attitudes, from enthusiastic support to strong opposition to different rewilding practices.We argue that the scope to increase support for rewilding varies depending on the type of underlying negative perceptions. Where the negative perceptions are based on objectively verifiable causal beliefs (mental models), opponents' minds may be changed through the provision of positive experiences, social learning, and adhering to good governance principles. However, where negative perceptions are based on values, for example, a preference for traditional rural landscapes, they are unlikely to change easily. Pursuing rewilding ambitions that clash with the values of local farmers may risk social conflicts, but accommodating these values too much may compromise rewilding's own goals. A cultural revival in funeral floriography – the secret language of flowers – is a novel feature of modernity neglected by historiography. A 'Russian flu' pandemic which swept across Britain in the 1890s was a commercial catalyst for the floral industry. An excess in mortality of 125,000 people, reflected how 60% of the population were infected with influenza between 1889 and 1895. The floristry business flourished, by following the fashion to line graves with flowers and display them at crematoriums. In standard historical accounts, consumers were supposed to be less willing to buy into this elaborate funeral flora. Seldom have historical studies appreciated the popularity of floral tributes for the dead in provincial England. In a Midlands' case study , the experiences across provincial central England, touching on northern communities, highlight how florists were at the forefront of reviving floral death customs, some of which had not been in vogue since the eighteenth century. Background: In response to anticipated increased health needs as a consequence of aging populations, and associated rising co-morbidity and medicines use, in both Japan and the UK potential extended roles for community pharmacy are part of health policy agendas. It is widely perceived that community pharmacists do not fully utilize their expertise, which may limit their contribution to health care in their communities and also impact negatively on their own job satisfaction. Purpose: The aim of this qualitative study was to identify operations that are perceived as opportunities to demonstrate expertise (ODE) and/or sources of job satisfaction by pharmacists in Japan and England, and explore associated system and contextual factors. Methods: Data were gathered with purposively selected pharmacists in Japan and England. An initial questionnaire was forwarded which guided subsequent face-to-face semi-structured interviews. These were audio-recoded to enable qualitative analytical procedures. Results: There were 18 participants in England and 13 in Japan. There was a notable association between ODE and job satisfaction. However, ODE was not a sufficient condition for job satisfaction. Conclusion: In order for pharmacists to be satisfied with their jobs, not only ODE but also additional contextual factors, i.e. recognition of professionalism, autonomy and positive collaboration were also needed. The findings can inform initiatives in the development of pharmacy services that would be positive for pharmacists whilst potentially contributing to wider public health objectives. This article examines the gap between legal theory and practice regarding bestiality in late medieval England. It focuses on a microhistorical analysis of a 1520 trial heard in Chichester consistory court, integrating other archival documents, legislation and legal commentaries to illustrate local and national contexts. I argue first that personal enmity rather than a moralizing agenda was the primary motivating factor in this case, and second that this trial illustrates the broader limitations of English church courts and the system of canon law in affecting popular attitudes to 'unnatural' intercourse and in the suppression of such acts. Background: In England, the emergence the more transmissible SARS-CoV-2 variant Alpha (B.1.1.7) led to a third national lockdown from December 2020, including restricted attendance at schools. Nurseries, however, remained fully open. COVID-19 outbreaks (≥ 2 laboratory-confirmed cases within 14 days) in nurseries were investigated to assess the risk of SARS-CoV-2 infection and cumulative incidence in staff and children over a three-month period when community SARS-CoV-2 infections rates were high and the Alpha variant was spreading rapidly across England.Methods: This was a cross-sectional national investigation of COVID-19 outbreaks in nurseries across England. Nurseries reporting a COVID-19 outbreak to PHE between November 2020 and January 2021 were requested to complete a questionnaire about their outbreak.Results: Three hundred and twenty-four nurseries, comprising 1% (324/32,852) of nurseries in England, reported a COVID-19 outbreak. Of the 315 (97%) nurseries contacted, 173 (55%) reported 1,657 SARS-CoV-2 cases, including 510 (31%) children and 1,147 (69%) staff. A child was the index case in 45 outbreaks (26%) and staff in 125 (72%) outbreaks. Overall, children had an incidence rate of 3.50% (95%CI, 3.21-3.81%) and was similar irrespective of whether the index case was a child (3.55%; 95%CI, 3.01-4.19%) or staff (3.44%; 95%CI, 3.10-3.82%). Among staff, cumulative incidence was lower if the index case was a child (26.28%; 95%CI, 23.54-29.21%%) compared to a staff member (32.98%; 95%CI, 31.19-34.82%), with the highest cumulative incidence when the index case was also a staff member (37.52%; 95%CI, 35.39-39.70%). Compared to November 2020, outbreak sizes and cumulative incidence was higher in January 2021, when the Alpha variant predominated. Nationally, SARS-CoV-2 infection rates in < 5 year-olds remained low and followed trends in older age-groups, increasing during December 2020 and declining thereafter.Conclusions: In this cross-sectional study of COVID-19 outbreaks in nurseries, one in three staff were affected compared to one in thirty children. There was some evidence of increased transmissibility and higher cumulative incidence associated with the Alpha variant, highlighting the importance of maintaining a low level of community infections. Background: The NHS Community Pharmacist Consultation Service (CPCS) offers patients requiring urgent care a consultation with a community pharmacist, following referral from general practice or urgent care. The study explored the impact of undertaking a Centre for Pharmacy Postgraduate Education (CPPE) CPCS learning programme, and barriers and enablers to CPCS delivery. Methods: CPPE distributed an online survey to those who had undertaken their CPCS learning. The survey explored participants' knowledge, confidence and application of taught skills/tools, including clinical history-taking, clinical assessment, record keeping, transfer of care, and Calgary-Cambridge, L(ICE)F and SBARD communication tools. Details on barriers and enablers to CPCS delivery were also included. Results: One-hundred-and-fifty-nine responses were received (response rate 5.6%). Knowledge of, and confidence in, taught skills were high and respondents reported applying skills in CPCS consultations and wider practice. Barriers to CPCS included a lack of general practice referrals, staffing levels, workload, and GP attitudes. Enablers included a clear understanding of what was expected, minimal concerns over indemnity cover and privacy, and positive patient attitudes towards pharmacy. Conclusion: This study demonstrates that community pharmacists can extend their practice and contribute to the enhanced provision of urgent care in England.
