Introduction: Implementation science (IS) offers methods to systematically achieve the Ending the HIV Epidemic goals in the United States, as well as the global UNAIDS targets. Federal funders such as the National Institutes of Mental Health (NIMH) have invested in implementation research to achieve these goals, including supporting the AIDS Research Centres (ARCs), which focus on high‐impact science in HIV and mental health (MH). To facilitate capacity building for the HIV/MH research workforce in IS, "grey areas," or areas of IS that are confusing, particularly for new investigators, should be addressed in the context of HIV/MH research. Discussion: A group of IS experts affiliated with NIMH‐funded ARCs convened to identify common and challenging grey areas. The group generated a preliminary list of 19 grey areas in HIV/MH‐related IS. From the list, the authors developed a survey which was distributed to all ARCs to prioritize grey areas to address in this paper. ARC members across the United States (N = 60) identified priority grey areas requiring clarification. This commentary discusses topics with 40% or more endorsement. The top grey areas that ARC members identified were: (1) Differentiating implementation strategies from interventions; (2) Determining when an intervention has sufficient evidence for adaptation; (3) Integrating recipient perspectives into HIV/MH implementation research; (4) Evaluating whether an implementation strategy is evidence‐based; (5) Identifying rigorous approaches for evaluating the impact of implementation strategies in the absence of a control group or randomization; and (6) Addressing innovation in HIV/MH IS grants. The commentary addresses each grey area by drawing from the existing literature (when available), providing expert guidance on addressing each in the context of HIV/MH research, and providing domestic and global HIV and HIV/MH case examples that address these grey areas. Conclusions: HIV/MH IS is key to achieving domestic and international goals for ending HIV transmission and mitigating its impact. Suicide is a critical public health issue in the United States, recognized as the tenth leading cause of death across all age groups (Centers for Disease Control and Prevention, 2020). Despite the Islamic prohibition on suicide, suicidal ideation and suicide mortality persist among Muslim populations. Recent data suggest that U.S. Muslim adults are particularly vulnerable, with a higher attempt history compared to respondents from other faith traditions. While the underlying reasons for this vulnerability are unclear, it is evident that culturally and religiously congruent mental health services can be utilized to steer suicide prevention, intervention, and postvention in Muslim communities across the United States. However, the development of Suicide Response toolkits specific to Muslim populations is currently limited. As a result, Muslim communities lack a detailed framework to appropriately respond in the event of a suicide tragedy. This paper aims to fill this gap in the literature by providing structured guidelines for the formation of a Crisis Response Team (CRT) through an Islamic lens. The CRT comprises of a group of individuals who are strategically positioned to respond to a suicide tragedy. Ideally, the team will include religious leaders, mental health professionals, healthcare providers, social workers, and community leaders. The proposed guidelines are designed to be culturally and religiously congruent and take into account the unique cultural and religious factors that influence Muslim communities' responses to suicide. By equipping key personnel in Muslim communities with the resources to intervene in an emergent situation, provide support to those affected, and mobilize community members to assist in prevention efforts, this model can help save lives and prevent future suicide tragedies in Muslim communities across the United States. Suicide among American Muslims is understudied, despite recent research highlighting increased suicide attempts among this population. While suicide is forbidden in Islam, formal guidelines for addressing and responding to suicide within Muslim communities did not exist until recently. The Stanford Muslim Mental Health and Islamic Psychology Lab has responded to a number of suicides in Muslim communities across North America and implemented an original model for suicide response and community healing. This approach incorporates Islamic principles and values to create a culturally and religiously congruent response to suicide that can support loss survivors and steer impacted communities toward healing. The Muslim Postvention Community Healing session described in this paper aims to provide a safe space for individuals impacted by suicide to come together and process their emotions, while also using Islamic teachings to guide the healing process. This unique model has the potential to serve as a valuable resource for Muslim communities across North America, and beyond, in addressing and responding to suicide. [ABSTRACT FROM AUTHOR]

Recent interdisciplinary scholarship has drawn increased critical attention to issues related to race, gender, class, and culture within public physical activity (PA) spaces, foregrounding the importance of rethinking dominant narratives of PA and promoting diverse and more inclusive practices within these contexts to address PA disparities. This paper addresses issues related to diversity and inclusion within community fitness spaces by examining the experiences of stakeholders and members involved in 'The Collective' Free Fitness Group (CFFG), a free, open-to-the-public fitness community based in Oakland, Ca (the original name has been changed for purposes of anonymity). This organization has made intentional efforts towards addressing diversity and inclusion across its membership and is located in one of the most ethnoracially diverse regions of the country. Drawing on McGee's (1980) concept of ideographs, we explore the potential limitations and consequences of how the CFFG Oakland community members communicated discourses and lived experiences of and to one another and the broader Oakland community. Specifically, we argue that by emphasizing certain expressions of and while deemphasizing alternative forms, participant responses reflected a 'paradox' of diversity, despite their shared in-group identification around as an important individual moral virtue and their open and community. Background: The concept of stigma has been widely used to understand patterns of discrimination and negative ideas surrounding people with mental health problems, yet we know little of the specific nuances of how this might operate beyond the 'Global North'. Aim: This paper aims to explore the notion of stigma in an Indian context by considering the lived experience of patients, carers and community members. Methods: A sample of 204 participants, representing mental health patients, informal carers and community members was recruited from urban and rural areas in Kerala, India. Participants took part in interviews where they were encouraged to talk about their experiences of mental ill health, attitudes towards these problems, barriers encountered and sources of support. Results: Experiences akin to the experience of stigma in Europe and the United States were elicited but there were important local dimensions specific to the Indian context. The difficulties faced by people with diagnoses of mental disorders in finding marriage partners was seen as an important problem, leading to marriage proposals being refused in some cases, and secrecy on the part of those with mental health problems. Rather than the 'self-stigma' identified in the US, participants were more likely to see this as a collective problem in that it could reflect badly on the family group as a whole rather than just the sufferer. Conclusions: In the Indian context, the idioms of stigma emphasised impairments in marriage eligibility and the implications for the family group rather than just the self. [ABSTRACT FROM AUTHOR]

Background: The increased contamination of illicit drugs with fentanyl in the United States drug market has contributed to escalating mortality from drug overdose. Leisure and hospitality service industry workers are encountering opioid-triggered overdoses in their workplaces, such as restaurants and bars. Consequently, this increases the need for overdose education and naloxone distribution (OEND) training, which has been limited. We aimed to describe the experiences among service industry workers encountering an overdose in their workplace. Methods: We conducted in-depth qualitative interviews with service industry workers in Little Five Points (L5P), Atlanta, between October 2019 and April 2020 and triangulated methods with participant observations and fieldwork. Purposive criterion sampling methods were applied to recruit from different establishments in the L5P commercial district, which comprised restaurants, bars, retail shops, and theaters. After an initial seed sample was identified by engaging key stakeholders during fieldwork (business owners, managers, and the business association), a snowball sample followed for a final sample of N = 15. To contextualize the local population of harm reduction workers, people who use drugs and/or obtain safer drug consumption supplies in L5P (sterile syringes, safer using kits, naloxone), and service industry workers and their customers in L5P, the first author volunteered with an Atlanta syringe services program from October 2019 to April 2020. The first author conducted participant observations during the syringe exchange program and field notes were taken during observation (44 hours). This engagement ensured a rich, thick description. We used a pragmatic approach to thematic data analysis for this study. Data were analyzed iteratively and inductively from interviews and observations. Two independent researchers reviewed transcripts to identify passages in the data related to the question of interest. The passages were contextualized within the full data set independently to understand the relationships in developing a theory of what was commonly occurring across participants' experiences, and these relationships led to emerging salient themes regarding encountering an opioid overdose at work. Results: One salient theme related to overdose response emerged with the service industry workers included fear of negative consequences of overdose response, specifically, fear of disease transmission from artifacts of drug use and overdose response, including the spread of blood-borne disease, violence, and exposure to unintentional overdose. When discussing drug use, participants' beliefs about the potential for personal danger from drug use artifacts (syringes and discarded drugs) and violence were identified as barriers to opioid overdose responses. Conclusions/Implications for Occupational Health Practice: Our findings provide valuable insights for tailoring OEND training for service industry workers to confront fears associated with opioid overdose response in their places of work to decrease mortality from the opioid epidemic. Harm reduction approaches need to be sensitive to the places in which overdose occurs and who the overdose responder is likely to be, which requires appropriately tailoring OEND training for service industry workers. Background: Racism contributes to health disparities and is a serious threat to public health. Teaching physicians about racism, how to address it in medical practice, and developing high quality and sustainable curricula are essential to combating racism. Objective: This study aimed to (1) describe the experience of racism and anti-racism teaching in residency programs, and elicit recommendations from key informants, and (2) use these data and formative research to develop recommendations for other residencies creating, implementing, and evaluating anti-racism curricula in their own programs. Methods: From May to July 2023, 20 faculty and residents were recruited via convenience sampling for key informant interviews conducted via Microsoft Teams. Interviews were audio recorded, transcribed, and coded. An initial list of themes was developed using theoretical frameworks, and then refined using a grounded-theory approach. A brief online optional anonymous demographic survey was sent to participants in August of 2023. Results: Eighty percent (20/25) of participants approached were interviewed. Seventy-five percent (15/20) answered a brief optional demographic survey. Seven themes emerged: (1) Racism in medicine is ubiquitous; (2) Anti-racism teaching in medicine varies widely; (3) Sustainability strategies should be multifaceted and include recruitment, resource allocation, and outcome measures; (4) Resources are widely available and accessible if one knows where to look; (5) Outcomes and metrics of success should include resident- faculty-, patient- community-, and system-focused outcomes; (6) Curricular strategies should be multilayered, longitudinal, and woven into the curriculum; and (7) Self-reflection and discomfort are necessary parts of the process. Conclusions: This study is one of the first to qualitatively examine perspectives of key stakeholders invested in anti-racism teaching for residents. The Support - Pipeline - Outcomes - Community (SPOC) Model, that was developed using information collected during this study, can be used in the future as a guide for others working to design and implement sustainable and high quality anti-racism curricula for residents. There is a debate in the literature about whether one can address food system problems with market-based approaches while seeking food justice or food sovereignty. However, as part of a team of researchers and community leaders, we have found that this debate is less relevant in practice. The concepts are interrelated within real-world food systems. As such, we were motivated to ask, how do social enterprises (SEs) interact with food justice and food sovereignty movements and their visions in order to realize more democratic and equitable local food systems in communities? To answer this question, we conducted a systematic review at the intersection of SE, food sovereignty, and food justice literature. Analyzing nine articles, which included 17 food-related SEs, we found evidence of potential interactions between food SEs, food justice, and food sovereignty that are compatible (e.g., create employment) and incompatible (e.g., limited ability to address issues like community employability and green gentrification). The literature includes at least three important characteristics that inform how food-related SEs may interact with food justice and sovereignty, including employee and ownership demographics, the enterprise business model, and aspects of the food system targeted by the enterprise via market activities. If we consider a systems perspective, we can envision the ways in which the aspects are embedded and interdependent in a neoliberal society. Despite significant and increasing numbers of students studying internationally, there are few data about the experiences of study abroad for various marginalized students, including transgender and gender expansive (TGE) students. Therefore, 15 TGE adults from the United States were interviewed about navigating gender and culture during undergraduate study abroad programs. Interviews were analyzed using consensual qualitative research (CQR). Participants shared how they benefited from international study and navigated intersecting social identities, including gender, which was complex and nuanced. They discussed how they actively managed issues of disclosure and its consequences, explored their identities and the influence of their social setting, and dealt with anticipated, deliberate, and unintentional harm from others. Relationships and community were priorities for participants when studying internationally; they explained how they determined whether to invest in relationships or not. Participants also clarified what their relationships looked like during study abroad, as well as unique considerations that arose from their marginalized identities. These findings can help inform the development of more inclusive, safe, and satisfying study abroad experiences for all students, especially TGE individuals; implications for future research and study abroad interventions are provided. Eviction filing rates have declined in many large cities in the United States. Existing scholarship on eviction, which focuses on discrete tenant-landlord relationships, has few explanations for this decline. I consider whether community organizing by nonprofit organizations shapes the social organization of communities and causes landlords to file fewer eviction filings. In cities where tenant and anti-poverty organizing has become common, community-oriented nonprofit organizations advocate for disadvantaged communities and help residents avoid poverty. Community organizing has rarely been studied as a predictor of housing security among low-income tenants, despite studies of how community organizing shapes the use of property in wealthy neighborhoods. I estimate the causal effect of community organizations on eviction filing rates between 2000 and 2016 using longitudinal data and a strategy to account for the endogeneity of nonprofits and eviction. Evidence from year-to-year models in 75 large cities spanning sixteen years estimate that an addition of ten community nonprofits in a city of 100,000 residents is associated with a ten percent reduction in eviction filing. This effect is comparable to the effect of community organizations on murder and is roughly a third of the association between eviction and concentrated disadvantage. [ABSTRACT FROM AUTHOR]

Background: Although a large body of research has identified challenges faced by women in STEM fields and strategies to improve the experience for women in STEM, little of this research has examined which strategies undergraduate women would recommend to their peers. In the current study, undergraduate women in STEM fields (N = 89) wrote letters to younger women in STEM about their experiences. The participants were recruited from a small public liberal arts college and a large public research institution in the United States. Participants were juniors and seniors majoring in engineering, mathematics, computer science, physics, biological or biomedical sciences, and chemistry. Results: Using thematic analysis, we identified seven types of advice. The participants shared advice about improving academically, forming communities, finding family support, and seeking out women role models. They also provided general words of encouragement and reassured women that everyone struggles, and failure is not indicative of their potential. In some cases, the letters were consistent with themes from prior research; however, other influences that have been studied by quantitative research were not prominent in women's own advice to their fellow students. For example, although the letters focused on communal themes such as building community, they did not focus on the communal goal of helping others through their careers. Additionally, they highlighted the role of family, which has been relatively neglected in prior work. Conclusions: The present research highlights which empirically supported theories about retention and success in STEM are reflected in students' advice to others. These letters also provide insight into which obstacles and solutions were most salient for women students looking back on their undergraduate STEM careers. The women's letters provide a rich understanding of how women navigate STEM fields and what they would tell future students about persisting in those fields. [ABSTRACT FROM AUTHOR]

Understanding patterns of species coexistence is a fundamental challenge in ecology. The physical environment is believed to play an important role, influencing patterns of dispersal and biotic interactions across space and time. Floodplain forest species are presumed to interact strongly with their environment, as evidenced by pronounced spatial variation in forest composition associated with flood‐driven abiotic constraints. Questions of how, and to what degree, fine‐scale heterogeneity interacts with broad‐scale hydrogeomorphology to influence patterns of functional composition remain unresolved. We examined how functional diversity (i.e. richness and range of functional traits) and ecological strategies (i.e. functional trait combinations) of floodplain forest communities varied across regional and local gradients of flooding in a northern temperate region of the United States of America. We found functional diversity of woody overstory species varied across hydrogeomorphic settings and that different settings altered associations between functional diversity and both the relative elevation above and proximity to rivers. Ecological strategies shifted over local gradients of relative elevation and distance to channel with different magnitudes and directions depending on the broader hydrogeomorphic context. We found evidence that interactions among flood regimes and landform position impose different levels of functional constraint. These results indicate patterns of community assembly are not easily discerned from landform type or position alone, but rather from filtering that operates and interacts with biota over multiple spatial scales. Our results imply that it is important to characterize flood dynamics in ways that can be clearly linked to ecological processes and that treatment of floodplain landforms as transferable units across river‐valley segments is problematic, even within a single basin. Small-scale fisheries make important socioeconomic, cultural, and environmental contributions to society, yet are often poorly characterized. In the United States, there is not a formal definition of the term "small-scale" in fisheries, making it difficult to monitor the status of the sector, discern drivers of change, or design and implement targeted policies that ensure its long-term viability. In this paper, we aim to bring attention to the sector by: (1) developing a set of preliminary definitions of small-scale fisheries; (2) describing the diversity, catch composition, and economic value of landings harvested by the sector; and (3) summarizing key drivers of change. We identify 1019 small-scale fishing "units" in the United States, which can be understood as semi-discrete fisheries based on five characteristics. Those who participate in these fisheries collectively harvest 350 commercially reported taxa. We estimate that these fisheries represented between 1 % and 25 % of the total commercial landings in the United States by weight and as much as 68 % of total value. Climate change and loss of social capital are viewed as key threats to small-scale fisheries, as is the decline of working waterfront infrastructure, nearshore habitat loss and degradation, and the privatization of access privileges. We identify a range of impediments to studying the sector in the United States and offer recommendations for improving data collection, monitoring, evaluation long-term. Background: With the ongoing global COVID-19 pandemic and the recent political divide in the United States (US), there is an urgent need to address the soaring mental well-being problems and promote positive well-being. The Warwick-Edinburgh Mental Well-Being Scale (WEMWBS) measures the positive aspects of mental health. Previous studies confirmed its construct validity, reliability, and unidimensionality with confirmatory factor analysis. Six studies have performed a Rasch analysis on the WEMWBS, and only one evaluated young adults in the US. The goal of our study is to use Rasch analysis to validate the WEMBS in a wider age group of community-dwelling adults in the US. Methods: We used the Rasch unidimensional measurement model 2030 software to evaluate item and person fit, targeting, person separation reliability (PSR), and differential item functioning (DIF) for sample sizes of at least 200 persons in each subgroup. Results: After deleting two items, the WEMBS analyzed in our 553 community-dwelling adults (average age 51.22 ± 17.18 years; 358 women) showed an excellent PSR = 0.91 as well as person and item fit, but the items are too easy for this population (person mean location = 2.17 ± 2.00). There was no DIF for sex, mental health, or practicing breathing exercises. Conclusions: The WEMWBS had good item and person fit but the targeting is off when used in community-dwelling adults in the US. Immigration and forced migration have been on the rise globally because of a variety of political, social and economic factors. The association between forced migration and poor mental health is well documented, with many families and youth experiencing violence, loss of identity or status, racism, and discrimination. Despite these adversities, families and youth have leveraged individual and collective resources that have assisted in their integration, and overall well-being as they have acculturated to the United States (US). To explore the phenomenon of what refugee youth viewed as a healthy community during their transition to the US, a community based participatory research photovoice study was conducted with youth between 13-18 years of age. Through the photovoice process three main themes were identified by refugee youth that they felt were critical in making a healthy, thriving community, with thematic analysis revealing additional subthemes. Participants described how these themes were not only intertwined, but necessary for their social, emotional, and mental stability and the stability of members of their community. Community paramedic roles are expanding internationally, and no review of the literature could be found to guide services in the formation of community paramedicine programmes. For this reason, the aim of this restricted review was to explore and better understand the successes and learnings of community paramedic programmes across five domains being; education requirements, models of delivery, clinical governance and supervision, scope of roles and outcomes. This restricted review was conducted by searching four databases (CENTRAL, ERIC, EMBASE, MEDLINE and Google Scholar) as well as grey literature search from 2001 until 28/12/2021. After screening, 98 articles were included in the narrative synthesis. Most studies were from the USA (n = 37), followed by Canada (n = 29). Most studies reported on outcomes of community paramedicine programmes (n = 50), followed by models of delivery (n = 28). The findings of this review demonstrate a lack of research and understanding in the areas of education and scope of the role for community paramedics. The findings highlight a need to develop common approaches to education and scope of role while maintaining flexibility in addressing community needs. There was an observable lack of standardisation in the implementation of governance and supervision models, which may prevent community paramedicine from realising its full potential. The outcome measures reported show that there is evidence to support the implementation of community paramedicine into healthcare system design. Community paramedicine programmes result in a net reduction in acute healthcare utilisation, appear to be economically viable and result in positive patient outcomes with high patient satisfaction with care. There is a developing pool of evidence to many aspects of community paramedicine programmes. However, at this time, gaps in the literature prevent a definitive recommendation on the impact of community paramedicine programmes on healthcare system functionality. [ABSTRACT FROM AUTHOR]

Background: Older adults are the least active population in the U.S. Low-income communities have fewer physical activity (PA) resources, contributing to less PA and increased chronic disease risk. This study assessed the effect of the multilevel, peer-led, Peer Empowerment Program 4 Physical Activity (PEP4PA) on moderate-to-vigorous PA (MVPA) and health outcomes, over 2 years of follow up. Methods: In a cluster-randomized controlled trial, 12 senior or community centers serving low-income older adults were assigned to a PA intervention (n = 6) or usual programming (n = 6) condition. PEP4PA included self-monitoring, health coaching, group walks, social support, and community advocacy to improve walking conditions. The primary outcome was daily minutes of MVPA (7-day accelerometer). Secondary outcomes included Perceived Quality of Life (PQoL), 6-Minute Walk Test (6-MWT), blood pressure (BP), and depressive symptoms at baseline, 6, 12, 18 and 24 months. Mixed effects regression models estimated the effects on outcomes between groups over time and included random effects for repeated measures and center clustering. Effect modification by sex and income status was assessed. We calculated the incremental cost per daily minute of MVPA gained in the intervention group relative to the control group to assess cost effectiveness. Results: We enrolled 476 older adults (50 + years). Participants were on average 71 years old, 76% female, 60% low income, and 38% identified as racial or ethnic minorities. Compared to the control group, intervention participants sustained roughly a 10 min/day increase in MVPA from baseline at all time points and increased mean PQoL scores from unsatisfied at baseline to satisfied at 12, 18 and 24 months. Males and higher-income groups had greater improvements in MVPA. No significant effects were observed for 6-MWT or depressive symptoms, and BP results were mixed. The incremental cost per minute MVPA gained per person was $0.25, $0.09, $0.06, and $0.05 at 6, 12, 18 and 24 months, respectively. Conclusions: PEP4PA achieved increases in MVPA and PQoL in low-income older adults, over 2 years of follow up. The peer-led, community-based intervention provides a sustainable and cost-effective model to improve health behaviors in underserved, aging populations. Children and youth exposed to toxic stress are at a greater risk for negative outcomes on health, well‐being and occupational performance. Focus groups were conducted to gain understanding of the experiences and perspectives of youth at‐risk who were currently receiving services from a community‐based agency in a small city in the Western United States. Participants discussed acquiring life skills through agency programming including social skills and coping/self‐regulation skills. Participants also reported a positive culture of the agency, expressing feelings of safety, trust and supportive relationships. Community‐based programming may be in a pivotal position to provide services and supports that act as buffering protection against the effects of toxic stress. [ABSTRACT FROM AUTHOR]

Communities with high levels of social capital enjoy an array of positive economic and community development outcomes. We assess the role of several key community characteristics, including the strength of government institutions, in explaining local social capital variation. The analysis draws on data from United States counties and includes regression modelling and a Blinder–Oaxaca decomposition to explore differences in social capital across an area's metropolitan status and region. The data show social capital determinants vary by place both due to the endowment levels of these determinants and the productive value of their coefficients. For example, the coefficient productive values of government capacity explain some differences in social capital levels across metropolitan status (but not across region). This article discusses heightened curriculum consciousness in a time of crisis due to Covid-19. A teacher, a principal, and a university professor in the United States offer intertwined perspectives on sharpened understandings that call for transforming our daily practice of education and everyday curriculum. The article includes four interrelated threads that underlie everyday curriculum but take on fresh meanings and urgent callings in a time of crisis: life and death, interdependence and collaboration, inequality and equity, and community beyond separation. Throughout discussions, storytelling and examples of how teachers and school leaders have responded to the difficult situation are also provided. Covid-19 is taken as a wake-up call to deepened understandings, heightened awareness, and relational interventions, so that the landscape of curriculum can be reimagined and recreated.
