Oral reading fluency probes are essential for reading assessment, intervention, and progress monitoring. Due to the limited options for choosing oral reading fluency probes, it is important to utilize all available resources such as generative artificial intelligence (AI) like ChatGPT to create oral reading fluency probes. The purpose of this article is to describe how to use AI through ChatGPT to create customizable reading passages comparable with that of oral reading fluency probes. Using readability estimates, the ChatGPT-generated passages can be tailored to suit for specific grade levels, similar to how current publishers design oral reading fluency probes for the market. The implication of ChatGPT-generated passages is that researchers and practitioners alike could use ChatGPT to be able to create a seemingly unlimited amount of reading passages tailored to the skill level and interests of the learner for intervention material and potentially assessment material, while reducing cost and time investment.

BACKGROUND: Patients with ulcerative colitis (UC) who are hospitalised for acute severe flares represent a high-risk orphan population. AIM: To provide guidance for clinical trial design methodology in these patients. METHODS: We created a multi-centre consortium to design and conduct a clinical trial for a novel therapeutic intervention (hyperbaric oxygen therapy) in patients with UC hospitalised for moderate-severe flares. During planning, we identified and addressed specific gaps for inclusion/exclusion criteria; disease activity measures; pragmatic trial design considerations within care pathways for hospitalised patients; standardisation of care delivery; primary and secondary outcomes; and sample size and statistical analysis approaches. RESULTS: The Truelove-Witt criteria should not be used in isolation. Endoscopy is critical for defining eligible populations. Patient-reported outcomes should include rectal bleeding and stool frequency, with secondary measurement of urgency and nocturnal bowel movements. Trial design needs to be tailored to care pathways, with early intervention focused on replacing and/or optimising responsiveness to steroids and later interventions focused on testing novel rescue agents or strategies. The PRECIS-2 framework offers a means of tailoring to local populations. We provide standardisation of baseline testing, venous thromboprophylaxis, steroid dosing, discharge criteria and post-discharge follow-up to avoid confounding by usual care variability. Statistical considerations are provided given the small clinical trial nature of this population. CONCLUSION: We provide an outline for framework decisions made for the hyperbaric oxygen trial in patients hospitalised for UC flares. Future research should focus on the remaining gaps identified.

Autistic youth are more likely to experience depression than their non-autistic peers, yet research on risk and protective factors to depression in this population is limited. Behavioral activation (i.e., prioritizing and engaging in meaningful activities), including social activities, is an important mechanism in the pathway to depression in non-autistic youth that is understudied in autism. Ratings of youth depressive symptoms and behavioral and social activation at one timepoint from 100 autistic youth without intellectual disability and 100 of their caregivers were analyzed. The study aims were to examine caregiver and youth ratings of youth internalizing symptoms and behavioral and social activation, inter-rater reliability on study variables, and associations between depressive symptoms and behavioral and social activation in autistic youth by rater. Results revealed significant differences in youth and caregiver ratings on all variables and inter-rater reliability ranged from poor to moderate. Across both raters, more severe anxiety symptoms and lower behavioral activation were associated with more severe depressive symptoms; social activation, specifically the number of friends youth have, was significant in caregiver ratings only. Findings can be leveraged to enhance risk stratification and intervention efforts for autistic youth experiencing depression.

This essay examines strategies for thoughtfully integrating generative AI (Gen-AI) into management curricula to enhance student learning while mitigating risks like overreliance. We make the case that outright resistance is counterproductive; instead, management educators should embrace Gen-AI's potential to create more engaging, experiential learning aligned with andragogical principles. We provide a conceptual framework mapping nine Gen-AI objectives to the principles of andragogy. A semester-long course example illustrates this framework in action through AI activities fostering autonomy, competence, and real-world application. Student surveys revealed overwhelmingly positive perceptions of Gen-AI integration and improved exam scores. However, dependence risks remain. The essay discusses strategies to enhance critical thinking, personal growth, and academic integrity. Overall, we propose that prudent Gen-AI adoption can enrich management education, but long-term vigilance regarding overreliance is vital.

Abstract Background Pregnancy complications, such as gestational diabetes mellitus (GDM) and hypertensive disorders of pregnancy (HDP), affect a significant proportion of women in Australia, with long-term implications for cardiovascular disease (CVD) risk. Despite existing preventive measures, participation in ongoing health monitoring remains low. This study aims to explore women’s preferences and experiences regarding preventive healthcare after GDM and HDP, and to identify their unanswered questions about the association between these conditions and future CVD risk. Methods A participatory, qualitative approach was adopted, involving a Lived Experience Expert Group (LEE Group) to plan, conduct, and interpret focus groups with women who had experienced either GDM or HDP. Participants were recruited through health consumer and community organisations and took part in two focus groups conducted via Zoom. The focus groups involved a stimulus presentation about CVD and GDM or HDP, facilitated group discussion about participants’ health and healthcare since their pregnancy, and Nominal Group Technique to prioritise participants’ questions about their CVD risk. Focus groups were audio recorded and transcripts from each group were analysed thematically. Synthesised Member Checking was used to verify the trustworthiness of findings. Results Twelve women participated in the focus groups, with distinct themes emerging from the GDM and HDP focus groups. Participants were previously unaware of the association between their pregnancy complication and increased risk of future CVD and wished to know more. Three themes were generated from the GDM focus groups: ‘a distressing diagnosis’; ‘degrees of diabetes’; and ‘balancing motherhood and self-care’. Two themes were generated from the HDP focus groups: ‘women’s concerns were dismissed’ and ‘wanting follow up at the right time and with the right person’. The ‘top ten’ questions from each group focussed on improving maternity care, preventing CVD, and (for the HDP group) concerns beyond CVD. Conclusions Women’s capacity to engage in preventive health after GDM and HDP is influenced by their maternity care experiences and the accessibility of primary care pathways. Future interventions should focus on improving woman-centred maternity care, ensuring seamless transitions to primary care, and addressing the social determinants of health for new mothers.

Abstract Background The Health Literacy Questionnaire (HLQ) is an increasingly used health literacy instrument that has been translated into many languages. The HLQ has 44 items and comprises 9 scales assessing the multidimensional construct of health literacy. This study reports the HLQ reliability and construct validity tested in people with chronic diseases living in Vietnam. Methods Adults (n = 600) hospitalized with chronic disease in Vietnam completed the HLQ. Floor and ceiling effects, item, and scale difficulty levels were assessed. Generalized linear models with backward modeling techniques were performed to test key variables associated with each HL domain. Confirmatory factor analyses (CFA) testing nine one-factor models were fitted to test the structure of each scale, and a nine-factor model tested the hypothesized structure of the HLQ, followed by the calculation of scale reliability using Cronbach’s alpha. Results No item had floor effects, and only eight items showed ceiling effects. Two scales that had the most difficult tasks to complete (highest difficulty level) were 8 “Ability to find good information” and 9 “Understanding enough to know what to do”. Variables associated with health literacy were education, income, age, residential area, main support persons and comorbidity index (associated with 7, 7, 4, 3, 2 and 2 out of 9 scales, respectively). Each HLQ scale demonstrated a robust unidimensional construct with all CFI ≥ 0.95, RMSEA varied from 0 to 0.07. Large Language Models (LLMs) are a powerful type of Artificial Intelligence (AI) that simulates how humans organize language and are able to interpret, predict, and generate text. This allows for contextual understanding of natural human language which enables the LLM to understand conversational human input and respond in a natural manner. Recent examples of this, such as the Generative Pre-Trained Transformer (GPT) model, popularized by OpenAI's web application, ChatGPT, are able to complete an astounding variety of tasks when provided with simple language input. For education, LLMs can alleviate teacher curriculum and grading workloads and even perform specific tasks such as generating creative ideas for activities. Specifically for language learning, LLMs can draw on their immense corpus of language content to generate learner-centric materials to aid teachers in delivering targeted, personalized language instruction. The aim of this paper is to provide the reader with examples of how LLMs can be utilized for materials development, classroom activities, and providing feedback. After giving specific examples and explanations, the paper will conclude with a discussion of how this technology can provide teachers with new innovative ways to streamline the teaching process to focus on learner needs.

Parents play a vital role in shaping the ways young children understand the world around them. This paper reports on how the Community Mathematics Project (CMP)--a mathematics tutoring program for parents of diverse young children--affected the ways parents engaged in mathematical sensemaking while using virtual manipulatives and questioning to develop mathematical thinking. Further, we describe how these parents engaged with their children in daily activities and funds of knowledge in the form of at-home activities to contextualize mathematics word problems. We share stories on how parents and young children engaged in conversations around mathematics, and we provide examples on how these parents' lives were already mathematically rich and the CMP simply helped them to connect to mathematics more explicitly.

OBJECTIVE: ACTN2, encoding alpha-actinin-2, is essential for cardiac and skeletal muscle sarcomeric function. ACTN2 variants are a known cause of cardiomyopathy without skeletal muscle involvement. Recently, specific dominant monoallelic variants were reported as a rare cause of core myopathy of variable clinical onset, although the pathomechanism remains to be elucidated. The possibility of a recessively inherited ACTN2-myopathy has also been proposed in a single series. METHODS: We provide clinical, imaging, and histological characterization of a series of patients with a novel biallelic ACTN2 variant. RESULTS: We report seven patients from five families with a recurring biallelic variant in ACTN2: c.1516A>G (p.Arg506Gly), all manifesting with a consistent phenotype of asymmetric, progressive, proximal, and distal lower extremity predominant muscle weakness. None of the patients have cardiomyopathy or respiratory insufficiency. Notably, all patients report Palestinian ethnicity, suggesting a possible founder ACTN2 variant, which was confirmed through haplotype analysis in two families. Muscle biopsies reveal an underlying myopathic process with disruption of the intermyofibrillar architecture, Type I fiber predominance and atrophy. MRI of the lower extremities demonstrate a distinct pattern of asymmetric muscle involvement with selective involvement of the hamstrings and adductors in the thigh, and anterior tibial group and soleus in the lower leg. Using an in vitro splicing assay, we show that c.1516A>G ACTN2 does not impair normal splicing. INTERPRETATION: This series further establishes ACTN2 as a muscle disease gene, now also including variants with a recessive inheritance mode, and expands the clinical spectrum of actinopathies to adult-onset progressive muscle disease.

Abstract Hypoxia is a common feature in various pathophysiological contexts, including tumor microenvironment, and IFN-γ is instrumental for anti-tumor immunity. HIF1α has long been known as a primary regulator of cellular adaptive responses to hypoxia, but its role in IFN-γ induction in hypoxic T cells is unknown. Here, we show that the HIF1α-glycolysis axis controls IFN-γ induction in both human and mouse T cells, activated under hypoxia. Specific deletion of HIF1α in T cells (Hif1α –/–) and glycolytic inhibition suppresses IFN-γ induction. Conversely, HIF1α stabilization by hypoxia and VHL deletion in T cells (Vhl –/–) increases IFN-γ production. Hypoxic Hif1α –/– T cells are less able to kill tumor cells in vitro, and tumor-bearing Hif1α –/– mice are not responsive to immune checkpoint blockade (ICB) therapy in vivo. Mechanistically, loss of HIF1α greatly diminishes glycolytic activity in hypoxic T cells, resulting in depleted intracellular acetyl-CoA and attenuated activation-induced cell death (AICD). Restoration of intracellular acetyl-CoA by acetate supplementation re-engages AICD, rescuing IFN-γ production in hypoxic Hif1α –/– T cells and re-sensitizing Hif1α –/– tumor-bearing mice to ICB. In summary, we identify HIF1α-regulated glycolysis as a key metabolic control of IFN-γ production in hypoxic T cells and ICB response.

Background: Comprehensive sex and sexuality education (CSE) is an evidence-based intervention associated with improved sexual and reproductive health outcomes. Currently, there are no standardized requirements for sex and sexuality education in the United States, despite expert recommendations. Contributions to Theory: In the United States, a Whole School, Whole Community, Whole Child theoretical model proposed by the Centers for Disease Control, and current examples of school sex education policy is used to make recommendations for the standardization of comprehensive sexual health education in K-12 schools. This article describes the necessary components to adopt CSE equitably, and provides an example of the process implemented to improve CSE in 1 school district in Pittsburgh, PA. Implications for School Health Policy, Practice, and Equity: Understanding the key components of legislation that align with recommended standards, and the process of advocating for school policy change, allows local advocacy groups and education policymakers to create and pass feasible legislation that will ensure appropriate instruction. There is additional room for improvement in states and local districts that have current CSE legislation, to adopt approaches grounded in frameworks that critically evaluate social determinants of health and amend legislation to further improve health equity. Conclusions: Several states and local districts have passed CSE regulations, and are implementing and evaluating their efficacy, providing support and examples for other regions interested in adopting similar policies and processes. Pittsburgh Public Schools can be used as an example adapting current legislation and adopting more comprehensive language.

Throughout history, Historically Black Colleges and Universities (HBCUs) have overcome countless challenges to achieve their goals of maintaining cultural traditions, providing key leadership and role models, assuring economic functions, addressing issues between minority and majority populations, and producing Black agents for research, institutional training, and information dissemination within the Black and other minority communities. Using a Scholarly Personal Narrative (SPN), this article focuses on the function, legacy, and relevance of current HBCUs. Using W. E. B. Du Bois's "double consciousness" as a theoretical framework, each of the 12 contributing scholars address these questions: How have you reconciled your individual strivings? Has the HBCU placed a role in your reconciliation process? What SPN is emblematic of your reconciliation process? These questions are addressed through vivid narrative accounts that speak to the critical constructs of belonging--Black identity; gifted education, selfhood, spirituality, and theoretical frameworks. Each of these constructs represents an identity vector that points inward to the core--the HBCU.

This paper seeks to explain the migration decisions of minority language speakers by investigating motivating factors. Viewed through a language planning lens, the study pushes the parameters of some of the discipline's more recent agency concepts within the context of migration. Semi-structured interviews were conducted with 60 Welsh speakers aged 18-40 who have stayed, left, or returned to the Welsh language 'heartlands' to create a typology representing the diversity in speakers' priorities when deliberating migration decisions. The data shows that the Welsh language is a significant consideration in the migration decisions of some typology groups, however most groups prioritised other factors. It was found that, in some cases, employment was a means of sustaining speakers within the linguistic community or attracting them back, offering much-needed evidence to support key tenets of the Welsh Government's current language strategy. Furthermore, given the likelihood of minority language speakers' decision to stay, leave or return to a language 'stronghold' to increase or limit opportunities to use the language, we argue that migration is an important context for probing the use of agency by minority language speakers at a micro level. Consequently, we contend that migration should receive greater attention from language planning scholars.

Partnerships between two- and four-year Hispanic Serving Institutions (HSI) can provide high-quality, in-depth learning experiences for traditionally underserved students. Blurring the lines between these institutions allows for more seamless transitions for prospective teachers (PTs), and maximum collaboration and program impact. This article contributes to the growing literature on such partnerships and collaborations. We describe how our partnership between Northwest Vista College (2-year institution) and University of Texas at San Antonio (4-year institution) supported PTs and their development of mathematics pedagogical content knowledge and skills to facilitate in diverse settings (i.e., mathematics sensemaking, questioning, teaching through problem solving, soft teaching skills). To describe the nature of our partnership, in this article we (1) explain some of the main elements of the partnership between the two- and four-year campuses, (2) review the unique, collaborative nature of the Community Mathematics Project, and (3) discuss outcomes that have resulted among Mathematics Fellows who have gone through the program.

Objective: A small percentage of universities and colleges conducted mass SARS-CoV-2 testing. However, universal testing is resource-intensive, strains national testing capacity, and false negative tests can encourage unsafe behaviors. Participants: A large urban university campus. Methods: Virus control centered on three pillars: mitigation, containment, and communication, with testing of symptomatic and a random subset of asymptomatic students. Results: Random surveillance testing demonstrated a prevalence among asymptomatic students of 0.4% throughout the term. There were two surges in cases that were contained by enhanced mitigation and communication combined with targeted testing. Cumulative cases totaled 445 for the term, most resulting from unsafe undergraduate student behavior and among students living off-campus. A case rate of 232/10,000 undergraduates equaled or surpassed several peer institutions that conducted mass testing. Conclusions: An emphasis on behavioral mitigation and communication can control virus transmission on a large urban campus combined with a limited and targeted testing strategy.

Objective: To assess the frequency of preventative COVID-19 behaviors and vaccination willingness among United States (US) college and university students during the COVID-19 pandemic. Participants: Participants (N = 653) were [greater than or equal to]18 years old and students at institutions for higher education in the US in March 2020. Methods: Students self-reported preventative behaviors, willingness to be vaccinated, and social contact patterns during four waves of online surveys from May-August 2020. Results: Student engagement in preventative behaviors was generally high. The majority of students intended to be vaccinated (81.5%). Overall, there were no significant differences in the proportion adopting preventative behaviors or in willingness to be vaccinated by sex or geographic location. The most common reason for willingness to get vaccinated was wanting to contribute to ending COVID-19 outbreaks (44.7%). Conclusions: Early in the pandemic, college students primarily reported willingness to vaccinate and adherence to preventative behaviors. Outreach strategies are needed to continue this momentum.

Individuals who were active before and during pregnancy face significant barriers to return postpartum, and participation in exercise and sport often decreases significantly during this period. Evidence is beginning to emerge regarding elite athletes' experiences with returning to sport postpartum. Understanding the experiences of recreational participants is essential to improve wider participation postpartum. This qualitative study aimed to describe the experiences of those returning to recreational exercise or sport postpartum, and to identify actions to support such a return. 14 mothers from the UK and Ireland, participated in one-to-one interviews. Interviews were transcribed verbatim and analysed using contemporary reflexive thematic analysis. Six themes were generated: (1) The challenging transition to motherhood, (2) Regaining, reclaiming and role modelling: motivators for returning, (3) Changed bodies and reframed minds: the experience and benefits of returning, (4) The mental load: adaptation, priorities, judgement and guilt, (5) Lack of appropriate knowledge and professional support and (6) Support, environment, culture and provision. Participants gave detailed accounts of the experience of returning to recreational exercise or sport postpartum. A list of actionable steps to support the return to recreational exercise and sport postpartum was generated from the data. These include the training of relevant professionals, the delivery of guidance during antenatal and postpartum care, the provision of opportunities to return postpartum and the embedding of policy to support such a return. The study provides actionable recommendations and future research directions to support postpartum mothers in resuming recreational exercise and sport.

Pathology of the long head of the biceps tendon can be treated surgically with a multitude of tenodesis techniques; however, there is a lack of consensus on which technique provides the most optimal outcomes. Commonly used methods include inlay tenodesis with a bone tunnel and interference screw construct and onlay tenodesis with anchors or unicortical buttons. Although current methods typically provide satisfactory outcomes, many surgeons believe complications and failure rates remain suboptimal across techniques. In this article, we present an arthroscopic suprapectoral biceps tenodesis technique using an anchor device that was developed to address the shortcomings of current techniques, optimize outcomes, and minimize risk.
