Abstract

This Palgrave Pivot strives to recount and understand Indigenous Law, as set within a remote community in northern Australia. It pays close attention to the realpolitik and high-level political functioning of Indigenous Laws, which inspires a discussion of how this Law models the relational, influences governance and emplaces people in an ordered kincentric lifeworld. The book argues that Indigenous Law can be examined for the ways in which it is a deliberate, stabilizing and powerful force to maintain communal order in relation to Country, a counter framing to popular and ‘soft law or soft power asset’ visions of such Laws often held in the national and international imaginary. It is the latter which too often renders this knowledge esoteric and relinquishes it to a category of lore or folklore. This is an open access book.

THIS REPORT UPDATES PREVIOUS CDC GUIDELINES AND RECOMMENDATIONS ON PREFERRED PREVENTION AND TREATMENT REGIMENS REGARDING NATURALLY OCCURRING ANTHRAX. ALSO PROVIDED ARE A WIDE RANGE OF ALTERNATIVE REGIMENS TO FIRST-LINE ANTIMICROBIAL DRUGS FOR USE IF PATIENTS HAVE CONTRAINDICATIONS OR INTOLERANCES OR AFTER A WIDE-AREA AEROSOL RELEASE OF: Bacillus anthracis spores if resources become limited or a multidrug-resistant B. anthracis strain is used (Hendricks KA, Wright ME, Shadomy SV, et al.; Workgroup on Anthrax Clinical Guidelines. Centers for Disease Control and Prevention expert panel meetings on prevention and treatment of anthrax in adults. Emerg Infect Dis 2014;20:e130687; Meaney-Delman D, Rasmussen SA, Beigi RH, et al. Specifically, this report updates antimicrobial drug and antitoxin use for both postexposure prophylaxis (PEP) and treatment from these previous guidelines best practices and is based on systematic reviews of the literature regarding 1) in vitro antimicrobial drug activity against B. anthracis; 2) in vivo antimicrobial drug efficacy for PEP and treatment; 3) in vivo and human antitoxin efficacy for PEP, treatment, or both; and 4) human survival after antimicrobial drug PEP and treatment of localized anthrax, systemic anthrax, and anthrax meningitis. CHANGES FROM PREVIOUS CDC GUIDELINES AND RECOMMENDATIONS INCLUDE AN EXPANDED LIST OF ALTERNATIVE ANTIMICROBIAL DRUGS TO USE WHEN FIRST-LINE ANTIMICROBIAL DRUGS ARE CONTRAINDICATED OR NOT TOLERATED OR AFTER A BIOTERRORISM EVENT WHEN FIRST-LINE ANTIMICROBIAL DRUGS ARE DEPLETED OR INEFFECTIVE AGAINST A GENETICALLY ENGINEERED RESISTANT: B. anthracis strain. In addition, these updated guidelines include new recommendations regarding special considerations for the diagnosis and treatment of anthrax meningitis, including comorbid, social, and clinical predictors of anthrax meningitis. The previously published CDC guidelines and recommendations described potentially beneficial critical care measures and clinical assessment tools and procedures for persons with anthrax, which have not changed and are not addressed in this update. In addition, no changes were made to the Advisory Committee on Immunization Practices recommendations for use of anthrax vaccine (Bower WA, Schiffer J, Atmar RL, et al. Use of anthrax vaccine in the United States: recommendations of the Advisory Committee on Immunization Practices, 2019. MMWR Recomm Rep 2019;68[No. RR-4]:1-14). The updated guidelines in this report can be used by health care providers to prevent and treat anthrax and guide emergency preparedness officials and planners as they develop and update plans for a wide-area aerosol release of B. anthracis.

Abstract

We present highly sampled photometry of the supernova (SN) 2023ixf, a Type II SN in M101, beginning 2 days before its first known detection. To gather these data, we enlisted the global Unistellar Network of citizen scientists. These 252 observations from 115 telescopes show the SN's rising brightness associated with shock emergence followed by gradual decay. We measure a peak $M_{V}$ = -18.18 $\pm$ 0.09 mag at 2023-05-25 21:37 UTC in agreement with previously published analyses., Comment: 4 pages, 1 figure

Abstract

Antimicrobial resistance increases infection morbidity in both adults and children, necessitating the development of new therapeutic options. Telavancin, an antibiotic approved in the United States for certain bacterial infections in adults, has not been examined in pediatric patients. The objectives of this study were to evaluate the short-term safety and pharmacokinetics (PK) of a single intravenous infusion of telavancin in pediatric patients. Single-dose safety and PK of 10 mg/kg telavancin was investigated in pediatric subjects >12 months to ≤17 years of age with known or suspected bacterial infection. Plasma was collected up to 24-h post-infusion and analyzed for concentrations of telavancin and its metabolite for noncompartmental PK analysis. Safety was monitored by physical exams, vital signs, laboratory values, and adverse events following telavancin administration. Twenty-two subjects were enrolled: 14 subjects in Cohort 1 (12-17 years), 7 subjects in Cohort 2 (6-11 years), and 1 subject in Cohort 3 (2-5 years). A single dose of telavancin was well-tolerated in all pediatric age cohorts without clinically significant effects. All age groups exhibited increased clearance of telavancin and reduced exposure to telavancin compared to adults, with mean peak plasma concentrations of 58.3 µg/mL (Cohort 1), 60.1 µg/mL (Cohort 2), and 53.1 µg/mL (Cohort 3). A 10 mg/kg dose of telavancin was well tolerated in pediatric subjects. Telavancin exposure was lower in pediatric subjects compared to adult subjects. Further studies are needed to determine the dose required in phase 3 clinical trials in pediatrics.

Abstract

Automated segmentation of white matter hyperintensities (WMHs) is an essential step in neuroimaging analysis of Magnetic Resonance Imaging (MRI). Fluid Attenuated Inversion Recovery (FLAIR-weighted) is an MRI contrast that is particularly useful to visualize and quantify WMHs, a hallmark of cerebral small vessel disease and Alzheimer's disease (AD). Clinical MRI protocols migrate to a three-dimensional (3D) FLAIR-weighted acquisition to enable high spatial resolution in all three voxel dimensions. The current study details the deployment of deep learning tools to enable automated WMH segmentation and characterization from 3D FLAIR-weighted images acquired as part of a national AD imaging initiative. Among 441 participants (194 male, mean age: (64.91 +/- 9.32) years) from the DDI study, two in-house networks were trained and validated across five national collection sites. Three models were tested on a held-out subset of the internal data from the 441 participants and an external dataset with 29 cases from an international collaborator. These test sets were evaluated independently. Five established WMH performance metrics were used for comparison against ground truth human-in-the-loop segmentation. Results of the three networks tested, the 3D nnU-Net had the best performance with an average dice similarity coefficient score of 0.76 +/- 0.16, performing better than both the in-house developed 2.5D model and the SOTA Deep Bayesian network. With the increasing use of 3D FLAIR-weighted images in MRI protocols, our results suggest that WMH segmentation models can be trained on 3D data and yield WMH segmentation performance that is comparable to or better than state-of-the-art without the need for including T1-weighted image series., Comment: 20 Pages, 9 Figures, 2 Tables

Abstract

Since the discovery of electron-wave duality, electron scattering instrumentation has developed into a powerful array of techniques for revealing the atomic structure of matter. Beyond detecting local lattice variations in equilibrium structures, recent research efforts have been directed towards the long sought-after dream of visualizing the dynamic evolution of matter in real-time. The atomic behavior at ultrafast timescales carries critical information on phase transition and chemical reaction dynamics, the coupling of electronic and nuclear degrees of freedom in materials and molecules, the correlation between structure, function and previously hidden metastable or nonequilibrium states of matter. Ultrafast electron pulses play an essential role in this scientific endeavor, and their generation has been facilitated by rapid technical advances in both ultrafast laser and particle accelerator technologies. This review presents a summary of the remarkable developments in this field over the last few decades. The physics and technology of ultrafast electron beams is presented with an emphasis on the figures of merit most relevant for ultrafast electron diffraction (UED) experiments. We discuss recent developments in the generation, manipulation and characterization of ultrashort electron beams aimed at improving the combined spatio-temporal resolution of these measurements. The fundamentals of electron scattering from atomic matter and the theoretical frameworks for retrieving dynamic structural information from solid-state and gas-phase samples are described, together with essential experimental techniques and several landmark works. Ultrafast electron probes with ever improving capabilities, combined with other complementary photon-based or spectroscopic approaches, hold tremendous potential for revolutionizing our ability to observe and understand energy and matter at atomic scales., Comment: Manuscript submitted to Review of Modern Physics

Radicals serve as source in dynamic nuclear polarization, but may also act as polarization sink. If the coupling between the electron spins and different nuclear reservoirs is stronger than any of the reservoirs' couplings to the lattice, radicals can mediate hetero-nuclear mixing. Here, we report radical-enhanced $^{13}$C relaxation in pyruvic acid doped with trityl. We find a linear dependence of the carbon $T_1$ on field between 5 mT and 2 T. We extend a model, employed previously for protons, to carbon, and predict efficient proton-carbon mixing via the radical Non-Zeeman reservoir, for fields from 20 mT to beyond 1 T. Discrepancies between the observed carbon relaxation and the model are attributed to enhanced direct hetero-nuclear mixing due to trityl-induced linebroadening, and a field-dependent carbon diffusion from the radical vicinity to the bulk. Abstract

The provenance of GEMS (glass with embedded metal and sulfides) in cometary type interplanetary dust particles is investigated using analytical scanning transmission electron microscopy and secondary ion mass spectrometry. We review the current state of knowledge and closely examine the densities, elemental compositions and distributions, iron oxidation states, and isotopic compositions of a subset of GEMS in chondritic porous interplanetary dust. We find that GEMS are underdense with estimated densities that are 35–65% of compositionally equivalent crystalline aggregates. GEMS low densities result in a lower contribution to the bulk compositions of IDPs than has been assumed based on their volume fraction. We also find that element/Si ratios, assumed to be primary (indigenous), are instead perturbed by contamination and secondary alteration, including pulse heating during atmospheric entry. Fe in pyrrhotite inclusions was oxidized and Mg, S, Ca, and Fe were depleted relative to lithophile Al and Si, resulting in reduction in element/Si ratios. Because they trap outgassing elements, Fe-rich oxide rims that formed on the surface of GEMS are serendipitous “witness plates” to the changes in composition that accompany atmospheric entry. As a result of alteration, GEMS elemental compositions cannot reliably inform about their provenance. Except for highly anomalous oxygen isotope ratios measured in some large GEMS grains that indicate a contribution from circumstellar dust, oxygen isotope compositions are generally poor indicators of provenance. Prior work indicates that most GEMS fall close to the terrestrial oxygen isotope composition, which, however, does not exclude a presolar interstellar origin. Abstract

Guidelines for management of Melody transcatheter pulmonary valve (TPV) infective endocarditis (IE) are lacking. We aimed to identify factors associated with surgical valve removal versus antimicrobial therapy in Melody TPV IE. Multicenter retrospective analysis of all patients receiving Melody TPV from 10/2010 to 3/2019 was performed to identify cases of IE. Surgical explants versus non-surgical cases were compared. Of the 663 Melody TPV implants, there were 66 cases of IE in 59 patients (59/663, 8.8%). 39/66 (59%) were treated with IV antimicrobials and 27/66(41%) underwent valve explantation. 26/59 patients (44%) were treated medically without explantation or recurrence with average follow-up time of 3.5 years (range:1-9). 32% of Streptococcus cases, 53% of MSSA, and all MRSA cases were explanted. 2 of the 4 deaths had MSSA. CART analysis demonstrated two important parameters associated with explantation: a peak echo gradient ≥ 47 mmHg at IE diagnosis(OR 10.6, p  24 mmHg compared to baseline (OR 6.7, p = 0.01). Rates of explantation varied by institution (27 to 64%). In our multicenter experience, 44% of patients with Melody IE were successfully medically treated without valve explantation or recurrence. The degree of valve stenosis at time of IE diagnosis was strongly associated with explantation. Rates of explantation varied significantly among the institutions.

Abstract

Increasing prevalence of infections caused by antimicrobial-resistant gram-negative bacteria represents a global health crisis, and while several novel therapies that target various aspects of antimicrobial resistance have been introduced in recent years, few are currently approved for children. Ceftazidime-avibactam is a novel β-lactam β-lactamase inhibitor combination approved for adults and children 3 months and older with complicated intra-abdominal infection, and complicated urinary tract infection or hospital-acquired ventilator-associated pneumonia (adults only in the United States) caused by susceptible gram-negative bacteria. Extensive population pharmacokinetic (PK) data sets for ceftazidime and avibactam obtained during the adult clinical development program were used to iteratively select, modify, and validate the approved adult dosage regimen (2,000-500 mg by 2-hour intravenous (IV) infusion every 8 hours (q8h), with adjustments for renal function). Following the completion of one phase I (NCT01893346) and two phase II ceftazidime-avibactam studies (NCT02475733 and NCT02497781) in children, adult PK data sets were updated with pediatric PK data. This paper describes the development of updated combined adult and pediatric population PK models and their application in characterizing the population PK of ceftazidime and avibactam in children, and in dose selection for further pediatric evaluation. The updated models supported the approval of ceftazidime-avibactam pediatric dosage regimens (all by 2-hour IV infusion) of 50-12.5 mg/kg (maximum 2,000-500 mg) q8h for those ≥6 months to 18 years old, and 40-10 mg/kg q8h for those ≥3 to 6 months old with creatinine clearance > 50 mL/min/1.73 m2 .

Abstract

Amorphous silicates in meteoritic samples are significant as potential surviving interstellar dust from the formation of the Solar System. Amorphous silicate-rich grains called GEMS, glass with embedded metal and sulfides, are abundant in anhydrous interplanetary dust particles and some ultracarbonaceous Antarctic micrometeorites that have high presolar grain abundances. Some GEMS within these objects have been confirmed to be presolar. GEMS-like material, consisting of amorphous silicate with opaque inclusions, has been identified in the matrices of some primitive meteorites. We use specialized thin specimen preparation and transmission electron microscopy methods to compare the GEMS-like material in Paris (CM) and Acfer 094 (ungrouped) chondrites with GEMS in anhydrous, chondritic interplanetary dust particles. Specifically, we compared the amorphous silicate morphology, degree of partial ordering, elemental composition and dominant iron oxidation state. We find that the amorphous silicates in the GEMS-like materials in Paris and Acfer 094 show incipient ordering and are Fe-rich and highly oxidized, while those in GEMS are fully amorphous, Fe-poor, and anhydrous. From examination of various formation routes for the amorphous silicates, we conclude that the GEMS-like material in Paris and Acfer 094 is not GEMS and that it is unlikely that GEMS-like material accreted as primary amorphous material that retained its original amorphous structure since accretion. We also conclude that the presence of GEMS-like amorphous silicates in chondrite matrix is not a reliable indicator for high degrees of primitivity or pristinity since even mild, complex alteration processes may significantly alter silicate structure. Finally, we propose searches for presolar amorphous silicates in chondrite matrix samples that have been precharacterized by transmission electron microscopy methods to enable direct comparisons of presolar amorphous silicate abundances.

Abstract

Infections remain a leading cause of death in neonates. The sparse antibiotic development pipeline and challenges in conducting neonatal research have resulted in few effective antibiotics being adequately studied to treat multidrug-resistant (MDR) infections in neonates, despite the increasing global mortality burden caused by antimicrobial resistance. Of 40 antibiotics approved for use in adults since 2000, only four have included dosing information for neonates in their labelling. Currently, 43 adult antibiotic clinical trials are recruiting patients, compared with only six trials recruiting neonates. We review the World Health Organization (WHO) priority pathogens list relevant to neonatal sepsis and propose a WHO multiexpert stakeholder meeting to promote the development of a neonatal priority antibiotic development list. The goal is to develop international, interdisciplinary consensus for an accelerated neonatal antibiotic development programme. This programme would enable focused research on identified priority antibiotics for neonates to reduce the excess morbidity and mortality caused by MDR infections in this vulnerable population. Les infections demeurent l'une des principales causes de deces chez les nouveau-nes. Les rares projets de developpement d'antibiotiques et les defis poses par la recherche neonatale ont entraine une penurie d'antibiotiques efficaces specialement etudies pour traiter les infections multiresistantes (MR) chez les nouveau-nes, en depit d'une mortalite galopante due a une resistance accrue aux antimicrobiens. Sur 40 antibiotiques autorises pour les adultes depuis 2000, quatre a peine sont munis d'un etiquetage indiquant la posologie adaptee aux nouveaunes. Actuellement, 43 essais cliniques portant sur des antibiotiques recrutent des patients du cote des adultes, contre six seulement du cote des nouveau-nes. Dans le present document, nous passons en revue la liste prioritaire d'agents pathogenes etablie par l'Organisation mondiale de la Sante (OMS) pour soigner la septicemie neonatale et proposons de reunir, sous l'egide de l'OMS, des parties prenantes issues de plusieurs domaines d'expertise afin de promouvoir la creation d'une liste prioritaire de developpement d'antibiotiques destines aux nouveaunes. Objectif: parvenir a un consensus international et interdisciplinaire visant a accelerer le programme de mise au point d'antibiotiques a usage neonatal. Ce programme permettrait d'orienter les recherches vers des antibiotiques identifies comme prioritaires pour les nouveau-nes, en vue de faire baisser les taux de morbidite et de mortalite excessifs qu'engendrent les infections MR au sein de cette population vulnerable. Las infecciones siguen siendo una de las principales causas de muerte en los recien nacidos. Debido al escaso desarrollo de los antibioticos y a las dificultades para llevar a cabo la investigacion neonatal, son pocos los antibioticos eficaces que se estudian de manera adecuada para tratar las infecciones multirresistentes (MR) en los recien nacidos, a pesar de la creciente carga de mortalidad mundial causada por la resistencia a los antimicrobianos. De los 40 antibioticos aprobados para su uso en adultos desde el 2000, solo cuatro han incluido informacion sobre la dosis para recien nacidos en su etiquetado. En la actualidad, 43 ensayos clinicos con antibioticos para adultos estan reclutando pacientes, en comparacion con solo seis ensayos que reclutan recien nacidos. Se revisa la lista de patogenos prioritarios de la Organizacion Mundial de la Salud (OMS) relevantes para la sepsis neonatal y se propone una reunion de la OMS con multiples expertos para promover el desarrollo de una lista de antibioticos prioritarios para los recien nacidos. El objetivo es desarrollar un consenso internacional e interdisciplinario para establecer un programa acelerado de desarrollo de antibioticos neonatales.
