1. Tilted Light, Giant Currents: Engineering Floquet Moir\'e Patterns for Scalable Photocurrents

Abstract

While intense laser irradiation and moir\'e engineering have independently proven powerful for tuning material properties on demand in condensed matter physics, their combination remains unexplored. Here we exploit tilted laser illumination to create spatially modulated light-matter interactions, leading to two striking phenomena in graphene. First, using two lasers tilted along the same axis, we create a quasi-1D supercell hosting a network of Floquet topological states that generate controllable and scalable photocurrents spanning the entire irradiated region. Second, by tilting lasers along orthogonal axes, we establish a 2D polarization moir\'e pattern giving rise to closed orbital propagation of Floquet states, reminiscent of bulk Landau states. These features, imprinted in the bulk of the irradiated region and controlled through laser wavelength and tilt angles, establish a new way for engineering quantum states through spatially modulated light-matter coupling., Comment: 13 pages, 9 figures

2. Hysteresis and effective reciprocity breaking due to current-induced forces

Abstract

Directed transport is a key concept for many ongoing applications including nanoscale heat management, current rectification, source protection, and energy harvesting. Within the context of quantum transport, we here explore the use of nonlinear effects introduced by current-induced forces (CIFs) as a practical way to effectively break charge and heat transport reciprocities. In particular, we consider a simple model consisting of a mobile quantum dot (QD) coupled to two leads, where the charge (or heat) current develops an asymmetric behavior under inversion of voltage (or temperature) bias, thereby turning the system into a quantum diode (or quantum thermal diode). Furthermore, we find multiple stable positions for the QD and we show how the extraction of useful work is possible by modulating the nonequilibrium sources along well-established hysteresis loops. Finally, we explore a particular case where the nonlinearity of the CIFs can be exploited to pump heat or charge, even for systems that preserve inversion symmetry. This counterintuitive result is attributed to a spontaneous breaking of the inversion symmetry due to the intrinsic system's dynamics., Comment: 14 pages, 9 figures

Abstract

Graphene nanoribbons (GNRs) are thin strips of graphene with unique properties due to their structure and nanometric dimensions. They stand out as basic components for the construction of different types of nanoelectromechanical systems (NEMS), including some very promising sensors and pumps. However, various phenomena, such as unintended mechanical vibrations, can induce undesired electrical currents in these devices. Here, we take a quantum mechanical approach to analyze how currents induced by fluctuations (either thermal or of some other kind) in suspended GNRs contribute to the electric current. In particular, we study the pumping current induced by the adiabatic variation of the Hamiltonian of the system when a transverse vibration (flexural mode) of a GNR suspended over a gate is excited. Our theoretical approach and results provide useful tools and rules of thumb to understand and control the charge current induced by fluctuations in GNR-based NEMS, which is important for their applications in nanoscale sensors, pumps, and energy harvesting devices.

5. A dynamical model for Brownian molecular motors driven by inelastic electron tunneling

Abstract

In recent years, several artificial molecular motors driven and controlled by electric currents have been proposed. Similar to Brownian machines, these systems work by turning random inelastic tunneling events into a directional rotation of the molecule. Despite their importance as the ultimate component of future molecular machines, their modeling has not been sufficiently studied. Here, we develop a dynamical model to describe these systems. We illustrate the validity and usefulness of our model by applying it to a well-known molecular motor, showing that the obtained results are consistent with the available experimental data. Moreover, we demonstrate how to use our model to extract some difficult-to-access microscopic parameters. Finally, we include an analysis of the expected effects of current-induced forces (CIFs). Our analysis suggests that, although nonconservative contributions of the CIFs can be important in some scenarios, they do not seem important in the analyzed case. Despite this, the conservative contributions of CIFs could be strong enough to significantly alter the system's dynamics.

Abstract

BACKGROUND AND AIMS: Alagille syndrome (ALGS) is characterized by chronic cholestasis with associated pruritus and extrahepatic anomalies. Maralixibat, an ileal bile acid transporter inhibitor, is an approved pharmacologic therapy for cholestatic pruritus in ALGS. Since long-term placebo-controlled studies are not feasible or ethical in children with rare diseases, a novel approach was taken comparing 6-year outcomes from maralixibat trials with an aligned and harmonized natural history cohort from the G lobal AL agille A lliance (GALA) study. APPROACH AND RESULTS: Maralixibat trials comprise 84 patients with ALGS with up to 6 years of treatment. GALA contains retrospective data from 1438 participants. GALA was filtered to align with key maralixibat eligibility criteria, yielding 469 participants. Serum bile acids could not be included in the GALA filtering criteria as these are not routinely performed in clinical practice. Index time was determined through maximum likelihood estimation in an effort to align the disease severity between the two cohorts with the initiation of maralixibat. Event-free survival, defined as the time to first event of manifestations of portal hypertension (variceal bleeding, ascites requiring therapy), surgical biliary diversion, liver transplant, or death, was analyzed by Cox proportional hazards methods. Sensitivity analyses and adjustments for covariates were applied. Age, total bilirubin, gamma-glutamyl transferase, and alanine aminotransferase were balanced between groups with no statistical differences. Event-free survival in the maralixibat cohort was significantly better than the GALA cohort (HR, 0.305; 95% CI, 0.189-0.491; p <0.0001). Multiple sensitivity and subgroup analyses (including serum bile acid availability) showed similar findings. Abstract

Background and Aims: Alagille syndrome (ALGS) is a multisystem disorder, characterized by cholestasis. Existing outcome data are largely derived from tertiary centers, and real-world data are lacking. This study aimed to elucidate the natural history of liver disease in a contemporary, international cohort of children with ALGS. Approach and Results: This was a multicenter retrospective study of children with a clinically and/or genetically confirmed ALGS diagnosis, born between January 1997 and August 2019. Native liver survival (NLS) and event-free survival rates were assessed. Cox models were constructed to identify early biochemical predictors of clinically evident portal hypertension (CEPH) and NLS. In total, 1433 children (57% male) from 67 centers in 29 countries were included. The 10 and 18-year NLS rates were 54.4% and 40.3%. By 10 and 18 years, 51.5% and 66.0% of children with ALGS experienced ≥1 adverse liver-related event (CEPH, transplant, or death). Children (>6 and ≤12 months) with median total bilirubin (TB) levels between ≥5.0 and <10.0 mg/dl had a 4.1-fold (95% confidence interval [CI], 1.6-10.8), and those ≥10.0 mg/dl had an 8.0-fold (95% CI, 3.4-18.4) increased risk of developing CEPH compared with those <5.0 mg/dl. Median TB levels between ≥5.0 and <10.0 mg/dl and >10.0 mg/dl were associated with a 4.8 (95% CI, 2.4-9.7) and 15.6 (95% CI, 8.7-28.2) increased risk of transplantation relative to <5.0 mg/dl. Median TB <5.0 mg/dl were associated with higher NLS rates relative to ≥5.0 mg/dl, with 79% reaching adulthood with native liver (p < 0.001). Conclusions: In this large international cohort of ALGS, only 40.3% of children reach adulthood with their native liver. A TB <5.0 mg/dl between 6 and 12 months of age is associated with better hepatic outcomes. These thresholds provide clinicians with an objective tool to assist with clinical decision-making and in the evaluation of therapies.

Abstract

Different approaches for establishing shear fracture appearance (SFA) on tested Charpy specimens were assessed and compared, namely (in order of increasing expected accuracy) estimates based on characteristic forces from instrumented Charpy tests, optical measurements using image analysis software, and measurements performed with the aid of a scanning electron microscope. Comparisons between different SFA values for 129 instrumented Charpy tests currently included in the National Institute of Standards and Technology database allowed us to establish the relationships between the different available approaches and formulate recommendations for the revision of the standards ASTM E2298-18, Standard Test Method for Instrumented Impact Testing of Metallic Materials, and ISO 14556:2015, Metallic Materials -- Charpy V-Notch Pendulum Impact Test -- Instrumented Test Method, which currently provide four empirical correlations that can be used to estimate SFA. Abstract

Neonatal and infantile cholestasis (NIC) can represent the onset of a surgically correctable disease and of a genetic or metabolic disorder worthy of medical treatment. Timely recognition of NIC and identification of the underlying etiology are paramount to improve outcomes. Upon invitation by the Italian National Institute of Health (ISS), an expert working grouped was formed to formulate evidence-based positions on current knowledge about the diagnosis of NIC. A systematic literature search was conducted to collect evidence about epidemiology, etiology, clinical aspects and accuracy of available diagnostic tests in NIC. Evidence was scored using the GRADE system. All recommendations were approved by a panel of experts upon agreement of at least 75% of the members. The final document was approved by all the panel components. This position document summarizes the collected statements and defines the best-evidence diagnostic approach to cholestasis in the first year of life.

Abstract

Introduction: The present study aimed at evaluating Italian epidemiological trends of pediatric inflammatory bowel diseases (IBD) over the period 2009–2018. Materials and methods: Data from 1969 patients enrolled in the Italian Society of Pediatric Gastroenterology, Hepatology and Nutrition Registry, by 49 pediatric IBD centers throughout the country, were analyzed, comparing three different time intervals (2009–2012, 2013–2015, 2016–2018). Results: The number of new IBD diagnoses ranged from 175 to 219 per year, evenly distributed over the examined period of time. From 2009 to 2018, the minimal incidence ranged from 1.59 to 2.04 /105 inhabitants aged < 18 years, with an overall slight predominance of ulcerative colitis (UC) over Crohn's disease (CD) (ratio: 1.1). Mean diagnostic delay was 6.8 months for CD and 4.1 months for UC, with a significant reduction for CD when comparing the three-time intervals (p =0.008). The most frequent disease locations according to the Paris classification were ileocolonic for CD (41.3%) and pancolitis for UC (54.6%). Conclusions: The minimal incidence rate in Italy seems to have stabilized over the last two decades, even if it has increased when compared to previous reports. UC is still slightly more prevalent than CD in our country. Diagnostic delay significantly decreased for CD, reflecting an improved diagnostic capacity.

21. Cyclic Vomiting Syndrome in Patients Affected by Jansen-de Vries Syndrome: Results From an International Survey.

Abstract

Jansen-de Vries syndrome (JdVS) is an autosomal dominant neurodevelopmental disorder with intellectual disability and gastrointestinal (GI) abnormalities, including recurrent vomiting. This study aimed to understand the frequency and severity of GI symptoms in JdVS patients and to investigate the potential association with cyclic vomiting syndrome (CVS), which has not been previously reported. An international online survey assessed the prevalence and features of CVS and GI disorders in JdVS patients using Rome IV Criteria. The anonymous survey was conducted via Google Forms in April 2021. A total of 21 patients/guardians responded to the survey. The average age at JdVS diagnosis was 8.22 years (range: 1-42). Of the respondents, 6 (28.5%) had a CVS diagnosis, 5 (23.8%) had migraine, and 2 (9.5%) had abdominal migraine. Additionally, 8 (38%) had gastroesophageal reflux disease (GERD) and 8 (38%) had functional constipation. An analysis targeted questions showed that 7 (33%) met the Rome IV Criteria for CVS but were undiagnosed, leading to a CVS prevalence of 61% in this cohort. This study highlights a high prevalence of CVS in JdVS patients and underscores the need for increased awareness and accurate diagnosis to address misdiagnosis., (© 2024 Wiley Periodicals LLC.)
