At present, deep learning has achieved great success in the field of computer vision, but small object detection is still a challenging problem in the field of object detection. Aiming at the problems of low resolution of small objects, blurred images, and less information carried, one object detector that introduces residual learning and multi-scale feature enhancement is proposed. Firstly, an enhanced feature mapping block based on residual learning is introduced into the backbone network. Through channel averaging and normalization, the model more focuses on the object area instead of the background, and it provides additional semantics information for the effective feature layer while taking into account the detection speed. Then the feature map increases the receptive field of the effective feature map through feature fusion block sensitive to context information, and fuses the shallow feature layer and the deep feature layer used for prediction to improve the detection performance at low resolution. Finally, a dual attention block is used to suppress background noise, and key features are embedded in attention. While preserving spatial information, it strengthens the information association between channels, thereby enhancing the expressive ability of features. In order to better detect small objects, the number of a priori boxes for shallow feature mapping is also adjusted. Experimental results show that on the dataset of PASCAL VOC2007, the detection accuracy (mAP) of the algorithm for 300×300 input scale is 79.9%, which is 2.7 percentage points higher than that of SSD, and the detection accuracy of small objects bird, bottle, chair, and plant is improved 5.1 percentage points, 7.5 percentage points, 3.9 percentage points, 7.2 percentage points，respectively. The detection accuracy (mAP) on the OAP self-made aerial dataset is 82.7%.

For the observation of a picture, people may instinctly pay more attention to the eye-catching objects in the picture. Usually such objects tend to occupy a larger proportion in the picture, which leads to small targets being ignored. Because the area where the small target is located is often a weak detection area, and the features that can be extracted in the process of extracting features by the detector are few and are easily lost in the process of feature information transmission after the feature is extracted, the effect of small target detection is not good. Therefore, on the basis of the single-order detector, this paper adds a cross-channel interaction mechanism to ensure the integrity of the information between layers, adopts target enhancement of training samples and designs a general loss function. Apart from this, this paper improves the sample weighting on the basis of the loss function to predict weight of samples. The mAP of this paper framework UWN (unified weighted network) on the VOC public dataset is 81.2% and the mAP on the self-made small target aerial photography dataset is 82.3%. Compared with the FSSD algorithm, some speed is sacrificed, and the accuracy is greatly improved.

Background: Recognizing the predictors of poor short-term prognosis after first-line immunotherapy in patients with anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is essential for individualized treatment strategy. The objective of this study was to ascertain the factors that forecast short-term prognosis in patients with anti-NMDAR encephalitis, develop a prognostic prediction model, and authenticate its efficacy in an external validation cohort. Further, all patients were followed-up long-term to assess the factors of long-term outcome and relapses. Methods: A prospective enrollment of patients diagnosed with anti-NMDAR encephalitis was conducted across five clinical centers in China from June 2014 to Mar 2022. The enrolled patients were divided into the derivation and validation sets based on enrollment time. The short-term prognostic model was visualized using a nomogram. Further, all patients were followed-up long-term to assess the factors of long-term outcome. Results: This study found that poor short-term prognosis was a risk factor for poor long-term outcome (6-month prognosis, OR 29.792, 95%CI 6.507-136.398, p < 0.001; 12-month prognosis, OR 15.756, 95%CI 3.384–73.075, p < 0.001; 24-month prognosis, OR 5.500, 95%CI 1.045–28.955, p = 0.044). Abnormal behavior or cognitive dysfunction (OR 8.57, 95%CI 1.48–49.79, p = 0.017), consciousness impairment (OR19.32, 95%CI 3.03-123.09, p = 0.002), autonomic dysfunction or central hypoventilation (OR 5.66, 95%CI 1.25–25.75, p = 0.025), CSF pleocytosis (OR 4.33, 95%CI 1.48–12.65, p = 0.007), abnormal EEG (OR 5.48, 95% CI 1.09–27.54, p = 0.039) were independent predictors for a poor short-term prognosis after first-line immunotherapy. A nomogram that incorporated those factors showed good discrimination and calibration abilities. The area under the curve (AUC) for the prognostic model were 0.866 (95%CI: 0.798–0.934) with a sensitivity of 0.761 and specificity of 0.869. Conclusion: We established and validated a prognostic model that can provide individual prediction of short-term prognosis after first-line immunotherapy for patients with anti-NMDAR encephalitis. Magnesium is one of the most abundant essential minerals in the body. Magnesium supplements mostly have low bioavailability, except magnesium L-threonate. In 2010, a novel magnesium compound, magnesium L-threonate (Magtein®) was identified and was shown to raise the magnesium levels in the brain and neurons effectively. In this double-blind, placebo-controlled study, Magtein®PS, a magnesium L-threonate (Magtein®)- and phosphatidylserine-based formulation additionally containing vitamins C and D, was tested for its cognitive benefits in 109 healthy Chinese adults aged 18–65 years. Subjects were randomly assigned to receive either Magtein®PS or placebo (starch) capsules, at a dose of 2 g/day. "The Clinical Memory Test", the standard test commonly used in Chinese hospitals and academic institutes for cognitive evaluation, was administered before and 30 days after subjects received the supplement. Subjects receiving Magtein®PS showed significant improvements over the control group in all five subcategories of "The Clinical Memory Test" as well as the overall memory quotient scores. The older participants showed more improvement than younger participants. Results indicated significant benefits of Magtein®PS in improving memory and cognition in healthy Chinese adults. Objective: To explore the effect of comprehensive nursing measures, based on the risk factors of dementia, in liver cancer patients, who accepted intervention therapy., Methods: 86 patients met the inclusion criteria, and exclusion criteria were divided into an experimental group (EG group) and a control group (CG group). The general baseline data of age, sex, and education level of groups were evaluated before intervention. CG group adopted routine nursing measures, while EG group gave comprehensive nursing measures on the basis of routine nursing measures. Cognitive function, severity, Barthel index (BI), and quality of life were evaluated on admission day, discharge day, and three months after discharge., Results: The scores of cognitive function, mental state, and self-care ability of daily life in the EG group were significantly higher than those in the CG group on the day of discharge and 3 months after discharge. Three months after discharge, the quality of life total score of EG group was significantly higher than that in CG group, and the scores in the family role, language, activity, self-care ability, social role, thinking and social function were also higher than those in CG group (P < .05). Three months after discharge, the readmission rate and retraining rate in the EG group were significantly lower than those in the CG group (P < .05), but there was no significant difference in mortality between the two groups(P > .05)., Conclusion: Comprehensive nursing measures based on prognostic factors in patients undergoing interventional therapy for hepatocellular carcinoma can more effectively enhance patient recovery.
