We evaluated the effect of compression leggings with micromassage in association with physical activity on women with lipedema, not previously treated and without recent changes in body weight. The treatment resulted in an improvement in all subjective parameters, in spontaneous and evoked pain, in the volume of the limbs, in the absence of significant changes in body weight, and regardless of the duration of use, age, years of illness, the clinical stage, and body mass index (BMI). Evoked pain did not improve in areas affected by untreated lipedema; foot circumferences did not increase. We found a significant reduction in the thickness of subcutaneous adipose tissue (SAT) and skin evaluated in multiple points of the lower limb. Micromassage compression leggings are proposed as an integral part of conservative treatment. A method is proposed for the clinical evaluation of evoked pain, called the Progressive Pain Check (PPC), which allows for the calculation of a numerical score called the Ricolfi–Patton Score (RPS) and for the ultrasound evaluation of tissues. The method is simple and repeatable and allows for completion of the clinical evaluation of the patient at diagnosis and for an evaluation of the effects of various treatments, even applied to just one side of the body. Background and purpose: Poor clinical outcomes are still common in successfully reperfused acute ischemic stroke patients. The aim of our study was to assess the impact of sarcopenia and myosteatosis on neurological outcomes and mortality in successfully reperfused acute ischemic stroke patients. Materials and methods: We included in our retrospective observational study 166 consecutive patients who underwent technically successful mechanical thrombectomy for anterior circulation acute ischemic stroke between Jan 2016 and Dec 2019. ASPECTS and collateral score were assessed on pre-operative CT/CTA. Masseter muscles area and attenuation were measured on CTA images. Clinical and radiological variables were tested in multivariate logistic models to predict the probability of death and, among survivors, of incurring poor outcome. Results: At admission, mean NIHSS was 19 (SD = 6.5), mean body mass index 25.5 (SD = 4.4) kg/m2, and mean ASPECTS 8.0 (SD = 1.9). Of all, 48.2% patients showed good collaterals, 38.5% intermediate collaterals, and 13.3% poor collaterals. Overall, 90 days mRS was ≤2 in 48.2% of the patients, 3–5 in 30.7%, and 6 in 21.1%. At multivariate logistic regression, age (OR = 1.08, p = 0.036), ASPECTS (OR = 0.59, p = 0.013), and masseter muscles attenuation (OR = 0.93, p = 0.010) were independent predictors of mortality, whereas sex (OR = 7.15, p = 0.043), age (OR = 1.05, p = 0.042), body mass index (OR = 1.35, p = 0.013), NIHSS (OR = 1.12, p = 0.012), and ASPECTS (OR = 0.64, p = 0.024) were independent predictors of poor neurological outcome (mRS 3–5). Conclusion: Beyond other well-known variables, low masseter attenuation, indicating myosteatosis, represents an independent negative prognostic factor for 90 days mortality in patients successfully reperfused after anterior circulation stroke. Background Because of its ability to help assess the presence of subtle morphologic changes in bone and bone marrow edema, dual-energy CT (DECT) could be an alternative to MRI in the diagnosis of bone stress injury that includes a stress fracture (SF) and stress reaction (SR). Purpose To determine the diagnostic accuracy of DECT in identifying bone stress injury of the lower limb using MRI as the reference standard. Materials and Methods This prospective study, conducted between June 2021 and January 2024, included consecutive patients clinically suspected of having stress injury (SF or SR) of the lower limb (leg, ankle, or foot). Imaging diagnosis was based on the absence or presence of cortical or periosteal changes, bone marrow edema, or a fracture line. The diagnostic performance of four blinded independent readers was determined for the entire cohort and for the subset of participants without fracture lines. Sensitivity, specificity, and area under the receiver operating characteristic curve (AUC) were calculated. Interobserver agreement was evaluated with Kendall coefficient of concordance (W) . Results A total of 184 patients (mean age, 58 years ± 17 [SD]; 97 male) were enrolled. At MRI, 107 of 184 participants (58%) had positive diagnoses, including 70 with SF and 37 with SR. The mean overall sensitivity and specificity were 91% (390 of 428; 95% CI: 0.85, 0.95) and 93% (287 of 308; 95% CI: 0.87, 0.97), respectively, with an AUC of 0.94 (95% CI: 0.91, 0.97). Among patients without fracture lines ( n = 114), the mean overall sensitivity and specificity of DECT were 79% (117 of 148; 95% CI: 0.65, 0.88) and 93% (287 of 308; 95% CI: 0.87, 0.97), respectively, with an AUC of 0.87 (95% CI: 0.81, 0.94). The interobserver agreement was very good for diagnosis of SF and SR combined (Kendall W = 0.90) and SR alone (Kendall W = 0.84). Conclusion DECT helped to accurately diagnose bone stress injury of the lower limb by identifying fracture lines and osseous stress reactions. © RSNA, 2024 Supplemental material is available for this article.
