Exercise-induced bronchoconstriction (EIB) is a common clinical entity in people with asthma. EIB is characterized by postexercise airway obstruction that results in symptoms such as coughing, dyspnea, wheezing, chest tightness, and increased fatigue. The underlying mechanism of EIB is not completely understood. "Osmotic theory" and "thermal or vascular theory" have been proposed. Initial assessment must include a specific work-up to exclude alternative diagnoses like exercise-induced laryngeal obstruction (EILO), cardiac disease, or physical deconditioning. Detailed medical history and clinical examination must be followed by basal spirometry and exercise challenge test. The standardized treadmill running (TR) test, a controlled and standardized method to assess bronchial response to exercise, is the most adopted exercise challenge test for children aged at least 8 years. In the TR test, the goal is to reach the target heart rate in a short period and maintain it for at least 6 min. The test is then followed by spirometry at specific time points (5, 10, 15, and 30 min after exercise). In addition, bronchoprovocation tests like dry air hyperpnea (exercise and eucapnic voluntary hyperpnea) or osmotic aerosols (inhaled mannitol) can be considered when the diagnosis is uncertain. Treatment options include both pharmacological and behavioral approaches. Considering medications, the use of short-acting beta-agonists (SABA) just before exercise is the commonest option strategy, but daily inhaled corticosteroids (ICS) can also be considered, especially when EIB is not controlled with SABA only or when the patients practice physical activity very often. Among the behavioral approaches, warm-up before exercise, breathing through the nose or face mask, and avoiding polluted environments are all recommended strategies to reduce EIB risk. This review summarizes the latest evidence published over the last 10 years on the pathogenesis, diagnosis using spirometry and indirect bronchoprovocation tests, and treatment strategies, including SABA and ICS, of EIB. A specific focus has been placed on EIB management in young athletes, since this condition can not only prevent them from practicing regular physical activity but also competitive sports. Athletes often experience lower airway dysfunction, such as asthma and exercise‐induced bronchoconstriction (EIB), which affects more than half the athletes in some sports, not least in endurance sports. Symptoms include coughing, wheezing, and breathlessness, alongside airway narrowing, hyperresponsiveness, and inflammation. Early diagnosis and management are essential. Not only because untreated or poorly managed asthma and EIB potentially affects competition performance and training, but also because untreated airway inflammation can result in airway epithelial damage, remodeling, and fibrosis. Asthma and EIB do not hinder performance, as advancements in treatment strategies have made it possible for affected athletes to compete at the highest level. However, practitioners and athletes must ensure that the treatment complies with general guidelines and anti‐doping regulations to prevent the risk of a doping sanction because of inadvertently exceeding specified dosing limits. In this review, we describe considerations and challenges in diagnosing and managing athletes with asthma and EIB. Exercise-induced bronchoconstriction (EIB) is the most common chronic disease among elite athletes and when left untreated, can impact both respiratory health and sports performance. In recent years, there has been an increase in the awareness and detection of EIB in elite athletes. This narrative review aims to evaluate the risk, prevention, diagnosis, medication, and anti-doping policies of EIB in elite athletes, and to provide more references for athletes with EIB. The results showed that athletes of endurance, winter, and water sports generally have a higher prevalence of EIB than athletes of other sports. Adaptive warm-up before formal exercise and using heat exchange masks at low temperatures are effective ways for athletes to prevent EIB. For physicians, the exercise challenge test and eucapnic voluntary hyperpnea are the recommended diagnostic methods for EIB in athletes. The treatment of athletes with EIB is medication-based, such as inhaled corticosteroids and beta-2 agonists, but current anti-doping policies should be considered when used. [ABSTRACT FROM AUTHOR]

To determine the discriminatory value of various impulse oscillometry (IOS) parameters, and to find the cutoff value of the appropriate parameter for identifying exercise-induced bronchoconstriction (EIB) in children with asthma. This cross-sectional study was conducted in India from October 2016 to March 2018 in children with asthma who were 6–15 years of age. One hundred and five children were enrolled and subjected to pre-exercise IOS and spirometry followed by free running treadmill test as an exercise challenge. All children could achieve minute ventilation >17.5–21 times of FEV1 during the exercise challenge test. Then, IOS and spirometry were performed at 10 ± 2, 20 ± 2, and 30 ± 2 min post-exercise challenge. EIB was defined as reduction of FEV1 ≥10% within 30 min of exercise. For purposes of analysis, the children were grouped into two categories: "EIB Present" or "EIB Absent". The prevalence of EIB in our study was 20.95% (n = 22). ΔR5max percentage within 30 min post-exercise (AUC 0.74; 95% CI: 0.64, 0.84) had the best discriminating capacity among all IOS parameters for identifying EIB. A cutoff value of 14.1% increase in R5 within 30 min post-exercise was obtained for detection of EIB (sensitivity—95.45%, specificity—50.6%, PPV—33.87% and NPV—97.67%). A percentage change in R5 with a cutoff value of 14.1% increase post-exercise had the best discriminatory capacity among all IOS parameters for detection of EIB in children with asthma. However, low positive predictive value (PPV) with high negative predictive value (NPV) made this cutoff value more apt to rule out EIB. [ABSTRACT FROM AUTHOR]

Exercise-induced bronchoconstriction (EIB) is a dysfunction of the respiratory tract consisting of transient airflow obstruction. This study is a retrospective analysis of two prospective studies concerning EIB symptoms in two adolescent populations. Our study group included 400 non-athletes and 101 athletes. Due to the similarity of indoor exercise conditions, an analysis was performed on the basis of where training took place. The study aims to assess the EIB prevalence in the following groups of adolescent children: non-athletes and athletes. In "indoor" athletes, the EIB prevalence was 22.4%. Among non-athletes, EIB was diagnosed in 10.2% (p = 0.007). A history of asthma was found in 6.5% of non-athletes and 29.3% of indoor athletes (p < 0.001). The incidence of EIB without asthma was higher in indoor athletes (14.6%) than in non-athletes (9.9%). Athletes achieved higher mean values in forced expiratory volume in one second (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF), and maximum expiratory flow rate at 25% (MEF25) parameters. In the group of non-athletes, higher results were observed in forced expiratory volume in one second % of vital capacity (FEV1%VC), MEF50, and MEF75. Background: Early of initiation breastfeeding (EIB) can reduce the Infant Mortality Rate (IMR) and Maternal Mortality Rate (MMR). The COVID-19 pandemic reduced early initiation of breastfeeding activities. As a result of the COVID-19 pandemic, access to essential services such as breastfeeding counseling in hospitals has been disrupted. Purpose: The purpose of this study is that researchers want to examine the implementation of the EIB by health workers in the maternity room of "Aisyiyah Ponorogo" General Hospital during the COVID-19 pandemic. Methods: The research used was qualitative with a case study design. The social situation in this study was delivery support health workers in the Maternity Room and Perinatology Room with informants consisting of five implementing midwives and six implementing nurses. The data collection process was carried out until the data collected experienced data saturation. Data validity was tested using source triangulation, namely patients who gave birth at "Aisyiyah Ponorogo" General Hospital. Results: EIB at 'Aisyiyah Ponorogo General Hospital has always been applied to newborns since 2010 and underwent adjustments during COVID-19 pandemic. The EIB was not implemented due to several conditions which made it impossible. Supporting factors for the implementation of early initiation of breastfeeding are the number and ability of health workers and the facilities available from both the hospital and the patient's family. The obstacle in this study is the perinatology room which is located far away so that the nurses in the perinatology department have difficulty carrying out several tasks at once which will cause a discrepancy in the duration of EIB implementation. Conclusion: Collaboration between the hospital, health workers, and the community is needed in an effort to increase the implementation of EIB. Starting from the existence of policies from the Hospital, the ability of health workers and the willingness of the community to improve the implementation of EIB.

Purpose: Exercise‐induced bronchoconstriction (EIB) and exercise‐induced laryngeal obstruction (EILO) are the two disorders commonly considered when athletes complain of exertional dyspnea. They are highly different but often confused. We aimed to address this diagnostic challenge and its consequences in elite athletes. Methods: We included all athletes competing at national or international level, referred to our institution for workup for EILO during 2013–2016. We diagnosed EILO from video‐recorded laryngoscopy performed during maximal cardiopulmonary treadmill exercise (CLE test). Symptoms and previous diagnostic evaluations were obtained from referral letters and chart reviews. Results: Exercise‐induced laryngeal obstruction was diagnosed in 73/101 referred athletes, of whom 70/73 had moderate/severe supraglottic obstruction and 3/73 had primarily glottic obstruction with only minor supraglottic involvement. Of the 73 athletes with EILO, we were able to identify objective tests for asthma in 55 participants, of whom 22 had findings supporting asthma. However, 58/73 had used asthma therapy at some time previously, with current use in 28. Only three reported that asthma medication had improved their exercise‐related breathing problems, two of whom with tests confirming asthma. Treatment for EILO improved breathing problems in all but four. Conclusions: Objective testing verified EILO in most of the referred athletes. EILO coexisting with asthma was common, and large proportions had used asthma medication; however, few reported effect on exercise‐related breathing problems. Unexplained persistent exertional dyspnea must not lead to indiscriminate escalation of asthma treatment, but instead incite investigation for EILO, either as a co‐morbidity or as a differential diagnosis. The impact of EIB in asthmatic children's daily lives is immense, considering the effects on both physical and psychosocial development. Monitoring childhood asthma by ECT's can provide insight into daily life disease burden and the control of asthma. Current guidelines for bronchoprovocation tests restrict both the use of reliever and maintenance asthma medication before an exercise challenge to prevent false-negative testing, as both have significant acute bronchoprotective properties. However, restricting maintenance medication before an ECT may be less appropiate to evaluate EIB symptoms in daily life when a diagnosis of asthma is well established. Rigorous of maintenance medication before an ECT according to guidelines may lead to overestimation of the real, daily life asthma burden and lead to an inappropiate step-up in therapy. The protection against EIB offered by the combined acute and chronic bronchoprotective effects of maintenance medication can be properly assessed whilst maintaining them. This may aid in achieving the goal of unrestricted participation of children in daily play and sports activities with their peers without escalation of therapy. When considering a step down in medication, a strategic wash-out of maintenance medication before an ECT aids in providing objective support of potential discontinuation of maintenance medication.

En el presente trabajo de investigación, se realiza una descripción semántica del uso de eufemismos en los discursos orales de Educación Intercultural Bilingüe (EIB). El objetivo de este estudio es explicar las expresiones eufemísticas utilizadas en los discursos de EIB en el Perú sobre la base de las metáforas conceptuales que implican eufemismos. Para ello, se emplea una metodología relacionada con la lingüística cognitiva, que permite explicar los significados vinculados a estas expresiones. El análisis es cualitativo y se ha realizado a partir de los datos que fueron extraídos de videos sobre EIB en el Perú publicados en la plataforma de YouTube. A partir de la muestra extraída, se logró confirmar que los eufemismos acrecientan la idea de pertenencia a una comunidad nativa, al énfasis propio de su naturaleza.

Besowo Village is one of the focus locations to resolve stunting in Indonesia. To understand the nutrition fulfillment of under three children in the location, a case control study design was applied. The socio-economic characteristic of Besowo village society tends to be homogeneous. The incidence of stunting in the area is influenced by birth length and history of early initiation of breastfeeding (EIB). Stunted children with EIB did not continue exclusive breastfeeding. Parents' characteristic, history of micronutrient adequacy from supplements during pregnancy, nutrients adequacy of children, nutrition knowledge of mothers between two groups did not show significant differences. Emotion-induced blindness is known as the impairment in the identification of targets that follow shortly after emotional distractors in the rapid serial visual presentation (RSVP) paradigm. Besides negative or erotic stimuli, food distractors have also been found to elicit a similar effect. This indicates an attentional bias for food stimuli in the temporal dimension of visual attention, which is highly relevant in the context of eating behaviour. So far, the neural mechanisms of this food-induced blindness are widely unknown. In the present study (N = 53), we investigated event-related potentials (ERPs) in response to food distractors and non-food targets in a single-target RSVP task. Automatic capture of visual attention by task-irrelevant food distractors was evident in enhanced Distractor Negativity (N2) and Early Food Positivity (P1). However, food distractors did not elicit a P3b. Thus, apparently, not the encoding of food distractors in working memory but alternative attentional processes are responsible for the food-induced blindness. Reduced target P3b reflected errors in target identification but was unaffected by the preceding distractor. In contrast, target N2 was specifically reduced for unidentified targets preceded by food distractors. Presumably, the attentional capture of food distractors may have caused a reduction of target N2, thereby contributing to the impairment in target identification. However, future research is needed to confirm this assumption and bridge the gap between the food-induced blindness on the behavioural level and the attentional capture of food stimuli reflected in the distractor ERPs. Exercise may trigger bronchoconstriction, especially in a group of athletes in whom bronchospasm during exercise is reported to occur more frequently than in nonathletes. The aim of this study was to determine the prevalence and environmental risk factors contributing to exercise-induced bronchoconstriction (EIB) in adolescent athletes. A prospective study was conducted among a group of 101 adolescent athletes who underwent spirometry, exercise challenge, fractional exhaled nitric oxide (FeNO) measurements, and allergy assessment. The study group was divided into three subgroups of athletes based on the most common sports environments: swimmers, "indoor" athletes, and "outdoor" athletes. The clinical evaluation demonstrated a high frequency of EIB in the study group. Moreover, a large proportion of the athletes in whom EIB was observed reported no pre-existing symptoms suggestive of bronchospasm or asthma. Among patients without a previous diagnosis of asthma, clinical evaluation confirmed 22% with positive exercise challenges, compared with 77% of adolescents with negative test results. Moreover, among the athletes with a history of asthma, 39% had positive exercise challenges. Both EIB and asthma are common conditions that affect adolescent athletes. Physicians should pay particular attention to this group, as the symptoms can lead to under- and overdiagnosis.
