Brain differences linked to autism spectrum disorder (ASD) can manifest before observable symptoms. Studying these early neural precursors in larger and more diverse cohorts is crucial for advancing our understanding of developmental pathways and potentially facilitating earlier identification. EEG is an ideal tool for investigating early neural differences in ASD, given its scalability and high tolerability in infant populations. In this context, we integrated EEG into an existing multi-site MRI study of infants with a higher familial likelihood of developing ASD. This paper describes the comprehensive protocol established to collect longitudinal, high-density EEG data from infants across five sites as part of the Infant Brain Imaging Study (IBIS) Network and reports interim feasibility and data quality results. We evaluated feasibility by measuring the percentage of infants from whom we successfully collected each EEG paradigm. The quality of task-free data was assessed based on the duration of EEG recordings remaining after artifact removal. Preliminary analyses revealed low data loss, with average in-session loss rates at 4.16 % and quality control loss rates at 11.66 %. Overall, the task-free data retention rate, accounting for both in-session issues and quality control, was 84.16 %, with high consistency across sites. The insights gained from this preliminary analysis highlight key sources of data attrition and provide practical considerations to guide similar research endeavors.

This study compares how English-speaking adults and children from the United States adapt their speech when talking to a real person and a smart speaker (Amazon Alexa) in a psycholinguistic experiment. Overall, participants produced more effortful speech when talking to a device (longer duration and higher pitch). These differences also varied by age: children produced even higher pitch in device-directed speech, suggesting a stronger expectation to be misunderstood by the system. In support of this, we see that after a staged recognition error by the device, children increased pitch even more. Furthermore, both adults and children displayed the same degree of variation in their responses for whether Alexa seems like a real person or not, further indicating that childrens conceptualization of the systems competence shaped their register adjustments, rather than an increased anthropomorphism response. This work speaks to models on the mechanisms underlying speech production, and human-computer interaction frameworks, providing support for routinized theories of spoken interaction with technology.

This study investigated the extent to which sensory responsivity in infancy contributes to adaptive behavior development among toddlers at high-familial likelihood for autism. Prospective, longitudinal data were analyzed for 218 children, 58 of whom received an autism diagnosis. Results indicated that sensory profiles at age one year (hyperresponsivity, sensory seeking) were negatively associated with later adaptive behavior, particularly for socialization, at age 3 years regardless of diagnostic status. These results suggest that early differences in sensory responsivity may have downstream developmental consequences related to social development among young children with high-familial likelihood for autism.

In response to unwaveringly high attrition from STEM pathways, STEM Intervention Programs (SIPs) support STEM students in effort to increase retention. Using mixed methods (survey and focus groups), we studied students at one university who were either supported or unsupported by SIPs to understand how students may differ in experiences believed to contribute to STEM persistence. We evaluated: sense of belonging, scientific self-efficacy, scientific community values, scientific identity, and STEM involvement. The enrollment status of students two and a half years postsurvey was also tracked. SIP students reported significantly higher science identity and sense of belonging and were more involved in STEM-related activities than counterparts unsupported by SIPs. Differences in these measures were correlated with race/ethnicity, college generation status, and age. Notably, SIP students had higher odds of persisting in STEM than students not supported by SIPs. Focus group data provide additional meaning to the measured survey constructs and revealed nuanced qualitative differences between SIP and non-SIP student experiences. Overall, being involved in a SIP at our institution trends positively with theoretical models that explain STEM student persistence. SIPs have the potential to provide and/or facilitate meaningful and critical support, and students without those intentional supports may be left behind.

James C. McNally, the American Vice Consul in Zürich, Switzerland during the First World War occupied an important position in American-German diplomacy during the conflict. His influence came from his relationship to his son-in-law, the German naval officer, Friedrich Mensing. The American government placed McNally in Switzerland in order to leverage this relationship and report valuable intelligence back to Washington. He also came to be seen as important by the German government in Berlin, owing to his ability and willingness to forward peace negotiations directly to top American officials. McNally worked in an attempt to lay the groundwork for a peace deal, going against his official instructions from the State Department and leading to him being viewed with suspicion by his colleagues and superiors. James McNally engaged in these unauthorized, secret peace talks as an active way of furthering his own importance and in an attempt to advance his career.

The ShadowCam instrument on the Danuri spacecraft provides high-resolution views of shadowed portions of the Moon, which are illuminated by naturally scattered light from nearby sunlit terrain. The sensitive time-delay integration detector captures high signal-to-noise observations within the permanently shadowed regions and areas in shadow for part of the year. We characterized the geometric properties of the images, enabling accurate placement of observations within the lunar cartographic framework. This work describes the internal and external orientation parameters using laboratory observations and images collected during the cruise and commissioning phase of the mission. We identified a radial distortion in the cross-track direction from these observations, which is correctable during our standard calibration pipeline procedures. We also calculated the pointing of the camera relative to the spacecraft bus within ~0.001°. Using these models, corrections, and the initial ephemeris provided by the Korea Aerospace Research Institute, images can be aligned within 60 m on the surface (95% confidence interval). This calibration and a precise radiometric model will enable reliable interpretation of ShadowCam images and the development of future derived products, including precisely mapped mosaics and meter-scale digital elevation models.

Background: Academic emergency medicine (EM) communities have viewed anonymous online communities (AOC) such as Reddit or specialty-specific “applicant spreadsheets” as poor advising resources. Despite this, robust EM AOCs exist, with large user bases and heavy readership. Insights about applicants’ authentic experiences can be critical for applicants and program leadership decision-making. To date, there are no EM studies to qualitatively assess EM AOC narratives during the application cycle. Our goal was to perform a qualitative analysis of students’ EM program experiences through a publicly available AOC.Methods: This was a qualitative analysis of a publicly available, time-stamped, user-locked AOC dataset: “Official 2020–2021 Emergency Medicine Applicant Spreadsheet.” We extracted and then de-identified all data from selected sub-sheets entitled “Virtual Interview Impressions” and “Rotation Impressions.” Four investigators used constant comparative method to analyze the data inductively, and they subsequently met to generate common themes discussed by students. Preliminary thematic analysis was conducted on a random sample of 37/183 (20%) independent narratives to create the initial codebook. This was used and updated iteratively to analyze the entire narrative set consisting of 841 discrete statements. Finally, two unique codes were created to distinguish whether the identified sub-themes, or program attributes, were likely “modifiable” or “non-modifiable.”Results: We identified six major themes: living and working conditions; interpersonal relationships; learning experiences, postgraduate readiness, and online/virtual supplements. Common sub-themes included patient population (13%); resident personality (7%); program leadership personality (7%); relationship with faculty/leadership (6%); geography (4%); practice setting (4%); program reputation (4%), and postgraduate year-3 experiences (4%). Modifiable sub-themes outnumbered non-modifiable sub-themes, 60.7% to 39.3%.Conclusion: In this analysis of selected medical students’ narratives in an AOC, the majority of identified themes represented topics that may serve as external feedback for EM residency programs and their clerkships. Selective use of AOCs may set a precedent for future program assessments by applicants and inform program leadership of important programmatic elements in the eyes of applicants. It elucidates important themes in their interactions or learning experiences with programs and creates opportunities for learner-centric program improvement.

BACKGROUND: It is essential that medical education (MedEd) fellows achieve desired outcomes prior to graduation. Despite the increase in postgraduate MedEd fellowships in emergency medicine (EM), there is no consistently applied competency framework. We sought to develop entrustable professional activities (EPAs) for EM MedEd fellows. METHODS: From 2021 to 2022, we used a modified Delphi method to achieve consensus for EPAs. EM education experts generated an initial list of 173 EPAs after literature review. In each Delphi round, panelists were asked to make a binary choice of whether to include the EPA. We determined an inclusion threshold of 70% agreement a priori. After the first round, given the large number of EPAs meeting inclusion threshold, panelists were instructed to vote whether each EPA should be included in the 20 most important EPAs for a MedEd fellowship. Modifications were made between rounds based on expert feedback. We calculated descriptive statistics. RESULTS: Seventeen experts completed four Delphi rounds each with 100% response. After Round 1, 87 EPAs were eliminated and two were combined. Following Round 2, 46 EPAs were eliminated, seven were combined, and three were included in the final list. After the third round, one EPA was eliminated and 13 were included. After the fourth round, 11 EPAs were eliminated. The final list consisted of 16 EPAs in domains of career development, education theory and methods, research and scholarship, and educational program administration. CONCLUSIONS: We developed a list of 16 EPAs for EM MedEd fellowships, the first step in implementing competency-based MedEd.

Integral to the mission of academic institutions, trainees are among the most vulnerable to negative workplace experiences, calling for a need to understand factors that contribute to psychological safety in this population. Our objective is to empirically explore trainee psychological safety to inform best practices for training environments.Design: A survey was used to capture organizational, interpersonal, and individual factors, as well as demographic information in graduate, postdoctoral, medical, resident, and fellow trainees. Descriptive statistics, multivariate ordinal logistic regression, and dominance analyses were used to understand psychological safety and the predictors that explained the majority of the variance in its statistical model.Findings: Gender minorities and those who suppressed their race information were less likely to feel psychologically safe. Psychological safety was predominately explained by senses of belonging, recognition, and respect. Notably, trust and confidence in supervisor emerged as a pivotal factor influencing belonging and respect, whereas organizational support played a crucial role in fostering recognition and belonging. Intriguingly, clarity in role expectations and autonomy were positively correlated with recognition.Originality: Our findings highlight the interplay between organizational, interpersonal, and individual dynamics shaping psychological safety. Importantly, those who suppress their race as well as female or gender minorities are disproportionally prone to feeling unsafe. We further elucidate role clarity and autonomy as important factors in achieving a sense of recognition. We suggest programs prioritize development beyond technical competencies, recognizing trainees as key stakeholders in the cultivation of positive culture within academic environments.Keywords: psychological safety, trainee, belonging, recognition, respect, autonomy, role clarity

All students have the right to be safe at school. Regulations mandate that schools must establish procedures to ensure the safety of all students during emergency situations. Twenty years ago, a Midwestern school district created Individualized Emergency Evacuation Plans (IEEPs) to address unique safety needs of students with disabilities. A mixed methods design program evaluation was conducted to study its effectiveness. It examined the extent to which these practices align with the needs of students with disabilities, and what changes might be implemented to improve the program. Participants were multiple levels of District personnel. Data were collected using a quantitative online survey and qualitative individual semi-structured interviews and two focus groups. Most survey participants felt that current plans meet needs of students with disabilities; however, the plans could be updated to better support student safety. Interview analyses produced three emergent themes: We will do whatever it takes; There is always room for improvement; and "It might be helpful to … ." District Q continues to implement some practices from the original framework of the Individualized Emergency Evacuation Plans. The program evaluation produced suggestions for more effectively and efficiently meeting the safety needs of students with disabilities in emergency situations.

Background: Narrative discourse, or storytelling, is used in daily conversation and requires higher-level language and social communication skills that are not always captured by standardised assessments of language. Many autistic individuals and individuals with fetal alcohol spectrum disorders (FASD) have difficulties with both social communication and language skills, and narrative discourse analysis offers an ecologically relevant approach to assessing those challenges. Aims: This study investigated narrative discourse in individuals with autism and FASD, as well as an age- and sex-matched comparison group. Methods and Procedures: Narratives from 45 adolescents and adults, 11 with autism, 11 with FASD and 23 age- and sex-matched comparison participants were elicited using a wordless storybook. They were then transcribed orthographically, formatted to the Systematic Analyses of Language Transcript (SALT) convention and scored based on the SALT Narrative Scoring Scheme (NSS), a standardised language analysis protocol. In addition to the NSS total score, which assesses the overall structure and cohesion of the narratives produced, local and global measures of language ability were also employed. The local language measures included the number of mental state and temporal relation terms produced, while the global language measures included mean length of utterance, total different words, total words, total utterances, rate of speech, the number of mazes (e.g., repetitions, 'um', 'uh' or self-corrections) per total word and the NSS total score. Outcomes and Results: Using the SALT "Language Sample Analysis" tool, our results revealed that on global language measures, group differences were found on rate of speech, number of mazes per total words and the description of conflict/resolution in the narratives produced. The autism group produced significantly more mazes per total word and scored higher on the NSS conflict/resolution category score compared to the FASD and comparison groups. Both the autism and FASD groups spoke at a lower rate than the comparison group. On local language measures of narrative production, all groups were comparable, on average. Conclusions and Implications: While many aspects of narrative discourse in the autism and FASD groups were similar to each other and to the comparison group, we observed group differences on global measures of narrative production and significant individual variability within groups, suggesting that narrative abilities considered at an individual level may provide important clinical information for intervention planning. Future research should also consider additional variables that influence narrative discourse, such as motivation, distractibility or decision-making of individual participants.

Wildfire regimes are changing rapidly with widespread increase in the intensity, frequency, and duration of fire activity, especially in the western United States. Limited studies explore the impacts of wildfires on aquatic taxa and few focus on lentic habitats that are essential for amphibians, many of which are of conservation concern. We capitalized on existing pre-fire surveys for anuran species and resurveyed a random subset of wetlands across a gradient of soil burn severity to investigate the short-term effects of wildfire on a relict population of wood frogs in the southern Rocky Mountains. We also investigated whether maps created to support rapid post-fire emergency response activities (i.e., United States Forest Service Burned Area Emergency Response program) accurately characterize soil burn severity around small habitat features (i.e., ponds) that serve as important amphibian breeding and rearing habitat. Soil burn severity reflects fire impacts on soil and surface organic layers, including vegetation loss and changes in soil structure and function. We found that wood frog (Lithobates sylvaticus) breeding persistence following fires was negatively influenced by the percentage of their terrestrial habitat (100 m buffer surrounding breeding ponds) that was burned. Wood frog colonization probability of previously unoccupied ponds was low (∼ 0.10) and unaffected by soil burn severity. Importantly, we found that remotely sensed data typically produced to predict flooding and erosion at broad (catchment) scales is a poor representation of the amount and variation in soil burn severity surrounding small habitat features, suggesting that additional field sampling is necessary to understand wildfire responses for species that rely on these small habitat features. Understanding short-term geographic- and species-specific variation in response to wildfires provides the basis to explore time to recovery (e.g., when wood frogs return to burned breeding sites) or to determine if declines in breeding distributions intensify over time.

Background: Diastolic dysfunction (DD) is known to be a predictor of mortality. However, the impact of DD on the risk for myocardial infarction (MI) is not well defined. We sought to examine whether DD is an independent predictor of risk of MI in patients with a preserved ejection fraction. Methods: This was an observational study of consecutive patients who underwent an echocardiogram that showed normal systolic function and had ≥ 3 months of follow-up. DD was graded using the contemporaneous guidelines at the time of the echocardiogram. Subsequent MI was determined by an inpatient encounter with a primary diagnosis of MI. Results: 129,476 patients were included (mean age 56 years; 58 % women). DD was present in 17.6 % of patients (13.6 % Grade I, 3.6 % Grade II, 0.4 % Grade III). Patients with DD were more likely to be older and have cardiovascular comorbidities. Survival free from MI was significantly lower as DD severity increased. Multivariate Cox proportional hazards modeling demonstrated that DD was an independent predictor of MI (hazard ratios [CI]: Grade I: 1.48 [1.33–1.66]; Grade II: 1.84 [1.57–2.16]; Grade III: 2.90 [1.98–4.25]). Conclusion: Our data demonstrate that the risk of MI is significantly increased in the presence of DD, with higher risk at higher grades of DD. The increased risk associated with grade III DD is comparable to that from a prior history of percutaneous coronary intervention. These findings suggest that the severity of DD may be a useful tool in stratifying patients for risk of MI.

Rationale Firefighters are at risk for cardiovascular disease due to occupational-related inflammation, oxidative stress, and lifestyle practices. Astaxanthin (AX) possesses anti-inflammatory/antioxidant and purported ergogenic properties. This study examined the impact of supplementing the diet with 12 mg/d AX for four weeks on markers of inflammation, oxidative stress, cardiometabolic health, exercise capacity, and occupation-related performance in career firefighters.Methods In a randomized, double-blinded, placebo-controlled, crossover fashion, 15 male career firefighters (34.5 ± 7.4 years; 177.7 ± 7.0 cm; 95.6 ± 12.0 kg; 30.1 ± 2.9 kg/m2; 11.03 ± 6.85 years of service) ingested 12 mg/d of AX (AstaReal®, AstaReal AB, Nacka, SWE) or placebo (PLA) for four weeks while following a standardized resistance training program. After each treatment, testing sessions were completed to assess inflammatory markers, oxidative stress markers, cardiopulmonary exercise capacity, and performance to a fire ground test (FGT) consisting of nine fire suppressive activities. Data were analyzed using general linear model (GLM) analysis with repeated measures. Clinical significance was assessed via mean changes from baseline with 95% confidence intervals.Results Analysis of mean percent changes from baseline revealed that AX supplementation lessened the inflammatory response to to performing an incremental maximal exercise test and attenuated increases in interleukin-1β, cortisol, and uric acid in response to performing fire suppressive activities compared to when they ingested PLA. However, most of these effects were within groups rather than between groups. Additionally, there was evidence that AX ingestion increased the ventilatory anaerobic threshold. Four weeks of AX supplementation did not significantly affect fasting markers of oxidative stress, blood lipids, performance during the FGT, general clinical chemistry panels, or self-reported side effects.Conclusions Results provide some evidence that AX supplementation may help mediate occupation-related inflammation in response to high-intensity, short-duration exercise in firefighters. More research is warranted to determine if long-term supplementation can improve cardiometabolic risk in this population.Clinical trial registration ISRCTN10901752.

Background: Coccidioidomycosis is a fungal infection endemic to the southwestern United States. Musculoskeletal manifestations are uncommon and seen in disseminated disease. While the involvement of the axial skeleton has been well described, the literature is limited on diseases involving the lower extremity. Methods: We identified three patients, at two regional academic medical centers in southern Arizona, who demonstrated different manifestations of osteoarticular coccidioidomycosis involving the lower extremity. Results: Case 1 is a 41-year-old male, with a history of HIV/AIDS and vertebral coccidioidomycosis, who presented with abscesses in the left hemipelvis and left proximal femoral osteomyelitis. He was treated with staged surgical debridement, including the use of amphotericin B impregnated beads. He remains on indefinite oral posaconazole suppression. Case 2 is a 46-year-old female, who presented with suspected right knee osteoarthritis. An MRI revealed septic arthritis and osteomyelitis. Necrotic bone was debrided, and synovial fluid cultures were positive for Coccidioides. She underwent a resection of the native knee joint with the insertion of an amphotericin B and voriconazole impregnated spacer. She continues oral itraconazole and awaits a total knee arthroplasty. Case 3 is a 76-year-old male, who presented with a draining right heel ulcer. Radiographs revealed bony destruction consistent with Charcot arthropathy. Irrigation and debridement revealed the gelatinous destruction of the talus and calcaneus, and cultures confirmed Coccidioides infection. A polymethyl methacrylate voriconazole spacer was placed. He subsequently underwent arthrodesis and remains on lifelong fluconazole. Conclusion: Lower-extremity osteoarticular coccidioidomycosis has various debilitating presentations that frequently mimic non-infectious etiologies. Treatment warrants surgical debridement, and prolonged antifungal therapy should be considered.

The introduction of Large Language Models (LLM) to the chatbot landscape has opened intriguing possibilities for academic libraries to offer more responsive and institutionally contextualized support to users, especially outside of regular service hours. While a few academic libraries currently employ AI-based chatbots on their websites, this service has not yet become the norm and there are no best practices in place for how academic libraries should launch, train, and assess the usefulness of a chatbot. In summer 2023, staff from the University of Delaware's Morris Library information technology (IT) and reference departments came together in a unique partnership to pilot a low-cost AI-powered chatbot called UDStax. The goals of the pilot were to learn more about the campus community's interest in engaging with this tool and to better understand the labor required on the staff side to maintain the bot. After researching six different options, the team selected Chatbase, a subscription-model product based on ChatGPT 3.5 that provides user-friendly training methods for an AI model using website URLs and uploaded source material. Chatbase removed the need to utilize the OpenAI API directly to code processes for submitting information to the AI engine to train the model, cutting down the amount of work for library information technology and making it possible to leverage the expertise of reference librarians and other public-facing staff, including student workers, to distribute the work of developing, refining, and reviewing training materials. This article will discuss the development of prompts, leveraging of existing data sources for training materials, and workflows involved in the pilot. It will argue that, when implementing AI-based tools in the academic library, involving staff from across the organization is essential to ensure buy-in and success.
