Zusammenfassung Hintergrund Die Strahlentherapie ist ein integraler Bestandteil in den meisten modernen multimodalen Tumortherapiekonzepten sowohl in kurativen als auch in palliativen Therapiesituationen. Dies gilt auch für viele Tumorentitäten im allgemein- und viszeralchirurgischen Bereich. Dabei kann es zu neuen Herausforderungen im Rahmen des klinischen Alltags und der interdisziplinären Tumorkonferenzen kommen. Ziel Praxisrelevanter Überblick, basierend auf selektiven Referenzen der aktuellen medizinisch-wissenschaftlichen Literatur und gewonnenen klinischen Alltagserfahrungen, für den onkologisch tätigen Chirurgen über strahlentherapeutische Therapieoptionen bei viszeralmedizinischen Tumoren mit dem Fokus auf die viszeralonkologischen Tumoren wie Rektumkarzinom, Ösophaguskarzinom, Analkarzinom und Lebermetastasen Methode Es wird eine narrative Übersicht präsentiert. Ergebnisse (selektive Eckpunkte) In ausgewählten Fällen ist es beispielsweise möglich, dass im Rahmen neuer Konzepte beim Rektumkarzinom ein derart gutes Ansprechen erreicht wird, dass unter engmaschiger Kontrolle eine Resektion vermieden werden kann. Beim Ösophaguskarzinom gilt die neoadjuvante Radiochemotherapie mit anschließender Resektion bei allen geeigneten Patienten als Therapieregime der Wahl. Sollte eine Operation nicht infrage kommen, so steht mit der definitiven Radiochemotherapie, insbesondere bei einem Plattenepithelkarzinom, eine gute Alternative zur Verfügung. Beim Analkarzinom bleibt auch im Licht neuster Erkenntnisse die primär definitive Radiochemotherapie als strahlentherapeutische Therapie der Wahl. Lebertumoren können mithilfe der stereotaktischen Strahlentherapie lokal abladiert werden. Mit der Leberstereotaxie ist ein hoch wirksames Mittel mit geringer Nebenwirkungsrate verfügbar. Schlussfolgerung Auch vor dem Hintergrund der jüngsten Studien bleibt die enge Zusammenarbeit der Disziplinen im Rahmen der Tumortherapie essenziell zur bestmöglichen Therapie der betroffenen Patienten.

Methode Narratives Review Ergebnisse Schlussfolgerung Das NA als relevante Versorgungsgröße ist inzwischen längst mit in den Fokus eines basalen gefäßchirurgischen/-interventionellen Betreuungsprofils gerückt, dem sich der Gefäßchirurg/-interventionalist aufgrund der bestehenden Herausforderung hinsichtlich des anspruchsvollen diagnostischen und therapeutischen Managements mit seiner ganzen fachspezifischen Kompetenz widmen muss.

The core concepts of physiology, as first published in this journal in 2011, not only provide a noteworthy teaching approach but also encourage reflection on the fundamentals of physiology. Unfortunately, a fundamental flaw has crept into the core concept of flow down gradients. Fluids do not generally flow from high to low pressure, as claimed, but only because of a specific pressure difference, that is, the perfusion pressure. This is related to a problem that is widespread in physiology, from which even the core concepts are not free, namely, the description of mean arterial pressure (MAP) solely by means of Ohm's law of circulation, although this law actually describes perfusion pressure. Both pressures can be numerically approximately equal in the physiological case, but conceptually they remain different in principle. We solved this problem using the extended Bernoulli equation (a combination of Ohm's law and the simple Bernoulli equation). Thereafter, MAP depends on the following pressure components, all of which are essential for a basic understanding of circulation: perfusion, central venous, gravitational, and dynamic pressures. These pressures also have great pathophysiological and clinical importance, which we exemplify here. Towards the end of this paper, we provide recommendations that should be considered in teaching, whether it is a beginner or advanced course. We address physiology teachers who are open to critical constructive improvements in their teaching, especially in hemodynamics. In particular, we encourage the authors of the flow down gradients core concept to improve and refine its "unpacking."

ZusammenfassungUnser „Bauchgehirn“ besteht nicht nur aus einem eigenständig tätigen enterischen Nervensystem, sondern auch aus bidirektionalen Verbindungen mit Parasympathikus und Sympathikus sowie zu Gehirn und Rückenmark. Neue Studien zeigen, dass diese Verbindungen schnell Informationen über die aufgenommene Nahrung an das Gehirn weiterleiten können, um so das Hungergefühl und komplexes Verhalten wie Belohnungslernen zu steuern. Aber auch unser emotionales Erleben, insbesondere Stress, hat einen starken Einfluss auf das gastrointestinale System. Das Immunsystem, die Motilität und die Barrierefunktion des Gastrointestinaltrakts werden außerdem stark vom intestinalen Mikrobiom moduliert. Lokale Bakterien können über freigesetzte Stoffwechselprodukte und Neuropeptide direkt die neuronale Kommunikation beeinflussen und Entzündungsfaktoren kontrollieren. Intensive Forschung in den letzten 10 Jahren konnte außerdem belegen, dass das intestinale Mikrobiom emotionale und kognitive Aspekte unseres Verhaltens beeinflusst und somit im Fokus zahlreicher neuropsychiatrischer Erkrankungen wie z. B. Depressionen und Angststörungen steht.Dieser Übersichtsartikel soll einen kurzen Überblick über die anatomischen Grundlagen der sogenannten Darm-Gehirn-Achse geben und Modi der bidirektionalen Regulierung vorstellen. Über indirekte Verbindungen zum limbischen System kann die Darm-Gehirn-Achse wesentlich Stress und Angst, aber auch die Schmerzverarbeitung beeinflussen. Außerdem wird die Rolle des Mikrobioms erläutert und werden zukünftige Wege dargestellt, wie die (Mikrobiom-)Darm-Gehirn-Achse emotionales Erleben, Schmerzverarbeitung und Darmfunktion beeinflussen kann. Diese Zusammenhänge sind für die Weiterentwicklung der Viszeralmedizin und damit auch für den Viszeralchirurgen relevant, um interdisziplinär orientierte zukünftige Behandlungskonzepte zu entwickeln.

ZusammenfassungEin persistierendes Problem im klinisch-operativen Alltag ist die operationsassoziierte venöse Thrombembolie mit ihren möglichen Komplikationen. Zu den ureigenen und elementaren Aufgaben des Chirurgen, sowohl des Operateurs als auch des klinisch tätigen Mediziners, gehören daher die kompetente und verlässlich realisierte Thrombembolieprophylaxe. Zahlreiche präventive Ansätze wurden entwickelt und im täglichen Management etabliert, ein Restrisiko für eine Thromboseentwicklung bleibt bestehen. Unter diesem Aspekt wurde eine Recherche insbesondere zur wissenschaftlichen Literatur mit Übersichts- und Leitliniencharakter zum Thema Risikostratifizierung, Prophylaxeverfahren allgemein und bei spezifischen Indikationen durchgeführt.

The demographic development in Germany, especially in Saxony-Anhalt (SA), also poses challenges for vascular surgery, as the incidence of vascular diseases has increased following demographic change. For example, the prevalence of peripheral arterial occlusive disease (PAOD) in industrialised countries is estimated at around 10-20% in people over 60 years of age; thus, the number of people affected will also increase here with demographic change. Especially in rural areas, it seems to be more difficult for patients to reach appropriate specialist treatment.A compact narrative brief review, based on selective references from the current medical-scientific literature and our own experiences from daily practice in setting up a vascular surgery department in a rural area.In 2020, the population in the rural district of Jerichower Land (SA) was approximately 89,403 (male: 44,489; female: 44,914). The age distribution in the age groups relevant for PAOD is as follows: 65-74 years-total, 12.38%; 75 years and older-total, 13.85%; average age, 48.36 years (population density, 56.4/kmThe high proportion of residents at risk of and suffering from PAOD in a rural area with low population and doctor density allows investment in the establishment of a new vascular surgery department to ensure local care in this patient group with limited mobility and self-help, thus, ultimately from an appropriate health policy perspective but also from the perspective of a relevant revenue outlook.

AbstractElectrostatic analyzers are key instruments for understanding magnetospheric plasma. Their measurementsalso contribute to the determination of the surface and interior composition of moons and planets. Theymeasure properties in particle charge, energy and 3D velocity vectors of electrons and ions. Calibration ofthese instruments is challenging. Ground facilities can not reproduce the electromagnetic and radiationbackground of Jupiter. Also the particle flows and, their full energy- and 3D flow spectrum over thecomplete field of view of a spherical analyzers can not be produced by our facilities. Instead beamslarger than the field of view of at least one detector channel reproduce step-vise parts of the parameterspaces of the expected spectrum. By combining simulations, ground based calibration and in-flightcalibration, the reliability of the calibration can be increased. The calibration of the flight model ofthe The Jupiter Electron and Ion Spectrometer (JEI) showed good agreement with the simulations, exceptfor a significant discrepancy in azimuth resolution and the geometric factor at high pole inclinations.After a final upgrade of the test bench, we will allow for better comparison of results betweenThe Jupiter Icy Moons Explorer (JUICE) and the BepiColombo missions by cross-calibrating JEI andthe The Planetary Ion Camera (PICAM). Plain Language Summary Charged particles interact with magnetic field, planets and moons. Measuring the properties of these par-ticles helps to understand the interaction and allows to analyze the surfaces and interiors of planets andmoons as a result of their interaction with their surrounding magnetic field. Unfortunately, calibration ofthese instruments is complicated because accelerators only produce a small spectrum of the target particlestreams at once. In order to improve the results, calibration measurements on the ground are combinedwith simulations and later with calibrations during the mission using known plasma, like the solar wind. Thecalibration for JEI agreed with the simulations, except for high polar angles(Fig. 1.) This is due to the different beams used for calibration and simulation. 1 Introduction JUICE will explore the Jovian magnetosphere and three of the four Galilean moons. After several fly-bys of Europa and Callisto it will eventually enter orbit around Ganymede. JEI is part of The Particle Environ-ment Package (PEP) on JUICE. JEI will measure electron and ion distributions in the range from a few eV to a fewMeV with almost full hemispheric coverage.PICAM on BepiColombo has overlaps with JEI in its energy range from 1 eV to 3 keV. While mea-suring ions only, PICAM has additionally a time-of-flight unit, which allows to resolve incoming particlesat a m/δm ratio of 100. There are links between Mercury and Ganymede. Besides being similarsize, they are the only known solid objects in the solar system other than Earth that have intrinsic mag-netic fields. While the magnetic moments are similar in strength, Mercury is exposed to a supersonic plasma of the solar wind while Ganymede is only surrounded by the subsonic plasma of Jupiter’s magnetosphere. 2. Results For all JEI models, including the qualification model, the polar-, energy- and azimuth resolution could bemeasured in agreement with the accompanying simulations and within the specifications of the originalproposal. The resolutions and deflector response are plotted in Fig. 2. Only the azimuth resolution deviatesat higher polar inclinations from the simulations. The deviation directly affects the geometric factor, whichdiffers over the polar inclinations in the same range.The deviation is suspected to be reasoned by the different exposure of the sensor in the particle flow be-tween the simulations and the beam in the test facility. Simulations and measurements with an off-centerbeam are currently conducted to confirm the cause. A slight coaxial misalignment between the inner and outer spheres could be measured due to the deviationsof the individual energy peaks of the channels.Currently the positing system is under replacement to allow joint measurements of JEI and PICAM. Objective Development of an effective therapeutic algorithm for interventional (minimally invasive) approach to infected walled-off necrosis (WON) in patients with necrotising pancreatitis reflecting real-word data. Material/methods All consecutive patients who underwent endoscopic necrosectomy for necrotising pancreatitis through a defined study period were enclosed in this retrospective observational case study. The therapeutic approach was analysed for clinical success rate, complication spectrum and rate as well as mortality and compared with data from the literature. Finally, a therapeutic algorithm was derived. Results From 2004 to 2019, 126 patients with necrotising pancreatitis (median of APACHE II score, 10.5 points) were treated. In 92.9 % of cases (n=117), an infected WON with microbial pathogen detection was found. After a median of 18 days from symptom onset, first intervention was performed (53.2 % as percutaneous drainage with programmed rinsing, 29.4 % as EUS-guided internal drainage). From 2004 to 2010, double pigtail stents were used. Later, lumen-apposing metal stent (LAMS) such as AXIOSTM stent (Boston Scientific, Ratingen, Germany) was preferred. The combined percutaneous and internal drainage was performed in approximately 50 % of subjects.Endoscopic transluminal necrosectomy was performed in 123 patients (97.6 %) at a median of 33 days from symptom onset. Endoscopic percutaneous necrosectomy was conducted in 11.1 % of the individuals. A median number of two endoscopic necrosectomy sessions per patient was necessary for the therapy. The clinical success rate (discharge without surgical intervention) was 82.5 %. The complication rate (bleeding and perforation) and the need for surgery were both 9.5 %. The overall mortality was 8.7 %. Conclusion Therapy of necrotising pancreatitis with infected WON consists of early calculated antibiotic therapy with adequate drainage. Combined external and internal drainages with programmed rinsing seem to improve prognosis, as well as minimise i) the need for forced necrosectomies (mainly via a transluminal access site) and ii) complication rate as well as, thus, improve outcome.

The growing influence of qualitative visualisations that support qualitative and mixed methods approaches in population geography specifically open new perspectives for theoretical and methodological developments. Whereas qualitative research using visualisation remains rare when compared to quantitative approaches, several cases of innovation can be identified in population geography. Furthermore, we identified three favourable conditions to potentially inspire new methods of visualisation: the growing relevance of art-based research, an increasing number of transdisciplinary experiences, and the contemporary critical debate on communicating using maps. Based on this diagnosis, we call for systematic experiments and exchange that can inform an intense transdisciplinary academic debate on key epistemological questions, on different approaches to visualisation, and on new tools and techniques to visualise in population geography and beyond.

The complex changes in vascular surgical teaching and commonalities are to be outlined and discussed, in particular, similarities/differences to general surgery, specifics of teaching/teaching content and its university requirements.Compact narrative overview RESULTS: Vascular surgical teaching component at Magdeburg University Hospital comprises 10 academic teaching hours and includes the following topics: PAD, embolism/thrombosis, vascular injury, compartment syndrome, mesenteric ischemia, aortic aneurysm and venous surgery. This puts vascularsurgical teaching here well above the average of 6.1 academic teaching hours in Germany. The strength of (vascular)surgical training lies in the fact that the knowledge gained at the bedside can be linked to visual impressions and simultaneous explanations in the operating theatre. Close integration of the student into the team is the high art of promoting acceptance of surgery as a profession and vocation. The prerequisites for successful teaching are a didactically competent teacher with specialist expertise, a proactive teaching attitude, knowledge of the specific learning objectives, the curriculum, modern teaching methods and awareness of the special role model function for students. Die Stärke der (gefäß‑)chirurgischen Ausbildung liegt insbesondere darin, dass der Erkenntnisgewinn am Krankenbett anschließend im Operationssaal insbesondere an visuelle Eindrücke mit simultaner Erklärung gekoppelt werden kann. Eine enge Einbindung der/s Studierenden ins Team ist die hohe Kunst, um die Akzeptanz der (Gefäß‑)Chirurgie als Beruf und Berufung zu fördern. Voraussetzung für eine erfolgreiche Lehre ist ein didaktisch kompetenter Lehrender mit fachlicher Expertise, proaktiver Lehreinstellung, Kenntnissen der speziellen Lernziele, des Curriculums, modernen Lehrmethoden und Bewusstsein für die besondere Vorbildfunktion für Studierende. Die berechtigte und zu stärkende klassische Lehre mittels Vorlesung, Seminar, Praktikum und Lehrbuch wird zunehmend durch Nutzung von internetbasierten Lernplattformen, Bibliotheken und Videoportalen ergänzt.Die gefäßchirurgische Lehre wird sich in den nächsten Jahren multimodal/-medial mit mehr praxisorientierten Anteilen und intensiver Integration der Studierenden in den Klinikalltag verlagern (müssen).
