A fully-automated unsaturated triaxial device is developed at the Advanced Soil Mechanic Laboratory of the Sharif University of Technology (SUT) to investigate the hydromechanical behavior of unsaturated soils under any complicated path. The main improvements of the developed device are (1) the ability to continuously measure accurately different stress/strain variables during long duration of unsaturated tests. This is of great importance in capturing the sudden deformations of the specimen, under wetting (2) providing a user-friendly controller software enabling the definition of any desired stress/strain variable and stress/strain paths under either stress-controlled or strain-controlled conditions. Finally, to evaluate the performance of the developed apparatus, two wetting tests were carried out to study the effect of initial shear stress on the hydromechanical behavior of unsaturated reconstituted specimens of Gorgan loess. In these wetting tests, the specimens were wetted under isotropic and anisotropic stress states by stepwise reduction of suction to approach to the saturated condition. The obtained results showed the excellent performance of the developed device to accurately follow the defined stress path along with the continuous measurement of stress/strain variables. The tested collapsible soil specimen that was wetted under the effect of initial shear stress exhibited a larger volume decrease accompanied by a larger degree of saturation compared to those of the specimen wetted under the isotropic stress state with the same initial mean net stress.

There has been limited research conducted to date on the role and significance of soil parameters in an unsaturated state on the amount of deformation and safety of reinforced deep urban excavations. This study investigates stress–deformations and static/pseudostatic safety factors against the general failure of an anchored deep excavated wall in an unsaturated soil deposit through two-dimensional finite-element modeling and limit equilibrium analysis, respectively. A suction-dependent elastic–plastic Mohr-Coulomb model is used in the analyses considering the effective stress approach in unsaturated soils. The results obtained from numerical modeling are validated against field monitoring data, including wall deformations and tensile forces in the anchors. A series of parametric studies are then performed assuming different groundwater levels, surcharge loads, and surcharge load distances from the wall crest to investigate their effects on the stability and deformation of the unsaturated soil excavation. Results are compared with those obtained from the corresponding routine analyses, in which the unsaturated state of the soil is not considered. The parametric study shows that the depth of the groundwater table is more influential on the results compared with the intensity and location of the surcharge load. The study demonstrates that unsaturated soil conditions result in a reduction of up to 37% in the maximum horizontal deformations of the excavation and increase the static and pseudostatic safety factors against general failure by 28% and 19%, respectively. In addition, taking unsaturated soil conditions into account during analysis leads to a decrease of up to 31% in the estimated tensile forces in the anchors. More importantly, it is shown that a more cost-effective stabilization plan can be developed for deep urban excavations by considering soil unsaturation effects, as demonstrated by comparing the results of the numerical analyses with the field data. Practical Applications: Presenting a safe and economically feasible plan for stabilizing deep urban excavations is a significant challenge for geotechnical engineers. Traditionally, engineers have relied on classical methods that consider soil parameters under dry or saturated conditions. However, in practice, the soil above the water table is unsaturated, and its mechanical properties are greatly influenced by the saturation level. Contrary to common belief, rainfall or pipe leakage near an excavation site does not fully saturate the soil and eliminate suction effects. Previous studies have shown that percolation from rainfall or other factors does not completely infiltrate clay-rich soils, mainly affecting moisture content and suction at shallow depths. This study evaluates the stress–strain behavior and the factor of safety against sliding in a deep excavated wall in Tehran, Iran, by considering unsaturated soil parameters. Comparing the outcomes with conventional methodologies, the results demonstrate that incorporating unsaturated soil parameters provides more accurate results aligned with field observations. This approach also results in smaller displacements of the excavation wall and higher safety factors against wall sliding. Accordingly, incorporating unsaturated soil parameters ensures accurate safety assessments for urban excavation walls and enables the creation of cost-effective and optimized design suggestions. Purpose of review: Vitamin B12 (B12, cobalamin) deficiency has been associated with neuropsychiatric symptoms, suggesting a role for B12 supplementation both as a treatment for psychiatric symptoms due to B12 deficiency and as an augmentation strategy for pharmacological treatments of psychiatric disorders. This critical review discusses the major causes of B12 deficiency, the range of psychiatric and non-psychiatric manifestations of B12 deficiency, the indications for testing B12 levels, and the evidence for B12 supplementation for major psychiatric disorders. Recent findings: We find that high-quality evidence shows no benefit to routine B12 supplementation for mild depressive symptoms or to prevent depression. There is very limited evidence on the role of B12 supplementation to augment antidepressants. No high-quality evidence to date suggests a role for routine B12 supplementation in any other major psychiatric disorder. No formal guidelines indicate when clinicians should test B12 levels for common psychiatric symptoms, in the absence of major risk factors for deficiency or cardinal symptoms of deficiency. Summary: No robust evidence currently supports routine B12 supplementation for major psychiatric disorders. However, psychiatrists should be aware of the important risk factors for B12 deficiency and should be able to identify symptoms of B12 deficiency, which requires prompt testing, medical workup, and treatment. Background: Although not approved for the treatment of anxiety (except trifluoperazine) there is ongoing off-label, unapproved use of first-generation antipsychotics (FGAs) and second-generation antipsychotics (SGAs) for anxiety. There have been systematic reviews and meta-analyses on the use of antipsychotics in anxiety disorders, most of which focused on SGAs. Aims and objectives: The specific aims of this umbrella review are to: (1) Evaluate the evidence of efficacy of FGAs and SGAs in anxiety disorders as an adjunctive treatment to traditional antidepressant treatments and other non- antipsychotic medications; (2) Compare monotherapy with antipsychotics to first-line treatments for anxiety disorders in terms of effectiveness, risks, and side effects. The review protocol is registered on PROSPERO. (CRD42021237436). Methods: An initial search was undertaken to identify systematic reviews and meta-analyses from inception until 2020, with an updated search completed August 2021 and January 2023. The searches were conducted in PubMed, MEDLINE (Ovid), EMBASE (Ovid), APA PsycInfo (Ovid), CINAHL Complete (EBSCOhost), and the Cochrane Library through hand searches of references of included articles. Review quality was measured using the AMSTAR-2 (A MeaSurement Tool to Assess Systematic Reviews) scale. Results: The original and updated searches yielded 1796 and 3744 articles respectively, of which 45 were eligible. After final review, 25 systematic reviews and meta- analyses were included in the analysis. Most of the systematic reviews and meta-analyses were deemed low-quality through AMSTAR-2 with only one review being deemed high-quality. In evaluating the monotherapies with antipsychotics compared to first-line treatments for anxiety disorder there was insufficient evidence due to experimental designs. There was limited evidence suggesting efficacy of antipsychotic agents in anxiety disorders other than quetiapine in generalized anxiety disorder (GAD) and monotherapy or augmentation with olanzapine in meta-analyses. Discussion and conclusions: This umbrella review indicates a lack of high-quality studies of antipsychotics in anxiety disorders outside of the use of quetiapine in GAD. Although potentially effective for anxiety, FGAs and SGAs may have risks and side effects that outweigh their efficacy. Introduction: The purpose of the study was to investigate factors that influence vocational rehabilitation program enrollment and employment at discharge of veterans with psychiatric and co-occurring alcohol and other substance use disorders enrolled at a veteran health administration (VHA) medical center. Methods: A sample of 2,550 veteran patients referred for VHA vocational rehabilitation between 2016 and 2021 were examined for the current study. The current study was classified as quality improvement/assurance, thus resulting in exempt research by the U.S. Department of Veteran Affairs Institutional Review Board. Results: Veterans with active alcohol use disorders (AUDs) and co-occurring depression, anxiety, post-traumatic stress disorder, or bipolar disorders were less likely to be enrolled for vocational rehabilitation program services compared to those without these co-occurring diagnoses. Veterans with AUD (active & inremission status combined into one category) and a diagnosis of anxiety were less likely to be employed at discharge compared to veterans with AUDs and no anxiety diagnosis (anxiety diagnosis - 3.5% vs. no anxiety diagnosis - 5.8%). Discussion: VHA vocational rehabilitation can be an effective intervention to assist veterans in reintegrating back into the community. Yet, there appears to be some disparities in the program enrollment and employment at discharge, dependingon the nature of the psychiatric diagnosis. Investigating the factors contributing (mediating or moderating) to these discrepancies are needed. • Micro energy Piles' load-bearing and cooling function in transformers is examined. • Various soils and micro pile configurations are examined numerically. • Numerical simulations are validated and used for the parametric studies. • Safety, cooling demand, and economic evaluations are conducted. • Optimal micro pile configurations are recommended for different soil types. The numerical assessment of Micro Energy Piles (MEPs) to enhance foundation bearing capacity (Q u) and cooling efficiency of 400-kV transformers is followed by economic evaluations. Findings show that increasing temperature-differential, MEP length, grout cohesion, and especially MEP diameter can increase Q u by 6–29 %, 25 %, 22–26 %, and 96–123 %, respectively. Optimal MEP configurations are recommended based on economic viability across different soils, with higher heat-exchange rates and grout cohesion yielding cost-effective solutions. Exploring viable options to improve Q u and cooling power demonstrates that utilizing MEPs is 26 % and 31 % more cost-effective than energy piles and helical energy piles, respectively, under comparable conditions. Approximately 1.1 million veterans meet criteria for substance use disorders (SUDs) with 1 in 4 struggling with illicit drugs, 4 in 5 struggling with alcohol use, and 1 in 13 struggling with both. The purpose of this study was to examine the impact of SUDs on closure status (exiting with employment, did not exit with employment) for veterans served in a Department of Veterans Affairs' Veterans Health Administration (VHA) vocational rehabilitation (VR) program. Data ( N = 2,620) from a VHA VR program in the Veterans Integrated Service Network 12 network were obtained for the purpose of the present study and consisted of veterans enrolled from 2012 to 2018. Findings showed that veterans without SUDs were more likely to exit with employment, and veterans enrolled in transitional work and community-based employment were more likely to exit with employment compared to those within supported employment (SE). Given that SE helps to serve veterans with the most severe psychological or medical conditions (e.g., active psychosis, schizophrenia, bipolar disorder, spinal cord injury, traumatic brain injury), findings suggest veterans are more successful with less serious mental health conditions.
