Abstract

Objective To assess the feasibility of the family paediatrician’s (FP) role in identifying the signs of postpartum depression in parents in time to guarantee child well-being.Design, setting and participants Data for this observational prospective study were collected within the NASCITA (NAscere e creSCere in ITAlia) cohort. During the first visit, paediatricians collected sociodemographic data regarding the parents and information about their health status, the pregnancy and the delivery. Whooley questions were administered during the first and second visits (scheduled 60–90 days after childbirth). Moreover, on the third visit (5–7 months after childbirth) the FP was asked to answer ‘yes’ or ‘no’ to a question on the parental postpartum depression, based on his knowledge and on the acquired information.Results In 2203 couples who completed the assessment, 529 mothers (19.9%), 141 fathers (6.3%) and 110 (5%) couples reported any depressive symptomatology. Of these, 141 mothers (5.3% of the total sample) and 18 fathers (0.8% of the total sample) were classified as ‘likely depressed’. An association was found between maternal postnatal depressive symptoms and having a diagnosed psychiatric disorder during pregnancy (OR 9.49, 95% CI: 3.20 to 28.17), not exclusively breastfeeding at hospital discharge (OR 1.76, 95% CI: 1.19 to 2.61) and the presence of child sleeping disorders at 3 (OR 2.46, 95% CI: 1.41 to 4.28) and 6 months (OR 2.18, 95% CI: 1.37 to 3.47). Another significant predictor of postpartum depression was being primiparous (OR 1.99, 95% CI: 1.31 to 3.02). Concerning the fathers, a significant association was reported only between likely depressed fathers and child sleeping disorders at 3 months (OR 7.64, 95% CI: 2.92 to 19.97). Moreover, having a likely depressed partner was strongly associated with depressive symptoms in fathers (OR 85.53, 95% CI 26.83 to 272.69).Conclusions The findings of this study support the feasibility of an active screening programme for parental postnatal depression during well-child visits as an integral part of postpartum care.Trial registration number NCT03894566; Pre-results.

Abstract

Purpose The NASCITA Study, a national-level, population-based, prospective cohort study, was set up to better understand the early health status of Italian children, comprising their physical, cognitive and psychological development, and how it is affected by social and health determinants, including nurturing care. NASCITA will also assess geographical differences and disparities in healthcare.Participants Participating family paediatricians from throughout Italy enrolled infants born during the enrolment period (April 2019–July 2020). The 5054 newborns seen by the 139 paediatricians for at least two visits, including the first well-child visit, and for whom parental consent was given, make up the baseline population.Findings to date Mothers had a mean age at delivery of 33.0 years and tended to have a high or medium level of education (42.5% university and 41.7% high school degrees) and to be employed (69.7%). One-third (36.1%) took folic acid supplementation appropriately, and 6.5% smoked or consumed alcohol (10.0%) during pregnancy. One-third (31.7%) of deliveries were caesarean deliveries. Concerning the newborns, 5.8% had a low birth weight and 6.2% were born prematurely. Abstract

Phthalates and bisphenol A (BPA) are plasticizers used in many industrial products that can act as endocrine disruptors and lead to metabolic diseases. During the LIFE PERSUADED project, we measured the urinary concentrations of BPA and Di(2-ethylhexyl)phthalate (DEHP) metabolites in 900 Italian women representative of the Italian female adult population (living in the north, centre, and south of Italy in both rural and urban areas). The whole cohort was exposed to DEHP and BPA with measurable levels above limit of detection in more than 99% and 95% of the samples, respectively. The exposure patterns differed for the two chemicals in the three macro-areas with the highest urinary levels for DEHP in south compared to central and northern Italy and for BPA in northern compared to central and southern Italy. BPA levels were higher in women living in urban areas, whereas no difference between areas was observed for DEHP. The estimated daily intake of BPA was 0.11 μg/kg per day, about 36-fold below the current temporary tolerable daily intake of 4 μg/kg per day established by the EFSA in 2015. The analysis of cumulative exposure showed a positive correlation between DEHP and BPA. Further, the reduction of exposure to DEHP and BPA, through specific legislative measures, is necessary to limit the harmfulness of these substances.

Abstract

Environment and health We are too slow in health protection actions from air pollution; this is the premise of Legambiente' s annual report Mal'aria di città 2023. This report highlights how air pollution is decreasing too slowly, putting the health of Italian citizens at risk with critical issues for some cities related to the days of exceeding the daily limit for PM10. Considering that attention should also be put to the level of other pollutants like such as those of PM2.5 and NO2. WHO has reported recent scientific evidence on the limits of concentrations not to be exceeded, and the European Air Quality Directive proposes downward limits (20 μg/mc) that should be complied with from January 2030 and that are to be considered an intermediate step from the WHO indications (15μg/mc). So there are 7 years to comply, but despite the mitigation actions put in place and tangible emission reductions, there has been no steady decline in air pollutant concentrations in virtually any city; in fact, the average rate of reduction in concentrations nationwide is only 2% for PM10 and 3% for NO2, too slow to reach the target set by the European directive for 2030. Polluted air is still being breathed for too long. As in previous issues, the main articles published in the monitored journals are summarized here, among which numerous are precisely those related to air pollution. All articles and editorials deemed worthy of attention are listed divided by topic, with a brief commentary. This issue is based on systematic monitoring of the publications of November and December 2022.

Abstract

Environment and health Amid uncertainties, doubts, and lack of courage, COP 27 ended by postponing many decisions to COP 28 next year. Positive is the decision to create a fund that will be made operative by COP28 on Losses and Damages for island states, the most fragile and vulnerable in the face of climate change. Also positive is the appointment of the first youth delegate of the COP chairmanship. In contrast, the goal of mobilizing $100 billion annually for climate finance was not met, and there was no breakthrough on exiting fossil fuels. Only 33 out of 200 states then submitted plans to implement national temperature reduction commitments. The text on human rights in climate change was deleted entirely, the only positive note being the recognition of the right to a healthy, clean and sustainable environment. Little or nothing has been done on biodiversity. That said, we have no choice but to do prevention with daily actions such as those represented by the flyer attached to this issue of Environment&Health News. These actions represent our contribution to system change and mitigation and adaptation processes. As in previous issues, the main articles published in the monitored journals are summarized here, among which numerous are precisely those related to air pollution. All articles and editorials deemed worthy of attention are listed divided by topic, with a brief commentary. Abstract

Environment and health The greatest will the impact of technology on our life be, the greatest will our need for nature be. This is what emerges from this bimonthly review. Some articles put in evidence the importance of soil remediations for the health of people living near polluted sites. Others articles underline children's vulnerability to the heavy metals present in street dust or the risks of pesticides for pregnant women and future offspring. Other articles emphasize the importance of environmental education to increase awareness both in children and teenagers and how activities in natural environments can help cognitive development as well as physical and mental health. Finally, it is highlighted how much work still needs to do be done to train health workers on mitigation and adaptation actions to the impacts of climate change we are experiencing. All articles and editorials deemed worthy of attention are listed divided by topic, with a concise comment. This number is based on the systematic review of the May and June 2022 publications.
