Abstract

The development of oral language norms prior to the publication of Theodor Siebs' set of rules "Deutsche Bühnenausprache" (1898) has hardly been systematically studied to date. Ulrich Thilo Hoffmann offers the first comprehensive overview of recognized and controversial pronunciation norms in the 19th century. His analysis of the stage-adequate pronunciation of vowels and diphthongs in the historical specialist literature includes Konrad Duden, Jacob and Wilhelm Grimm, Julius Hey and Wilhelm Viëtor as well as numerous lesser-known authors. Hoffmann thus makes a linguistic contribution to the debate on the development of the German standard language. At the same time, his book is a research impulse for musicology, specifically for the examination of vocal pronunciation in historically informed performance practice. The results of the study are already being used in the projects Wagner-Lesarten and The Wagner Cycles, which are pursuing the historically informed performance of Richard Wagner's Ring des Nibelungen under the direction of Kent Nagano.

Abstract

Das Ulcus hypertonicum Martorell, die „Calciphylaxis bei normaler Nierenfunktion und Nebenschilddrüsenfunktion“ (synonym „Eutrophication“), die Calciphylaxis mit dem peripheren Befallsmuster (inklusive akralen Nekrosen) und die Calciphylaxis mit zentralem Befallsmuster sind vier Diagnosen, welche Klinik, Histopathologie und Pathophysiologie miteinander teilen. Ischämisierende subkutane Arteriolosklerose, bzw. Arteriosklerose der Arteriae digitorum propriae und Penisarterien, mit Wandverdickung der Muscularis, in den meisten Fällen mit Mediakalzinose, Hypozellularität, subendothelialer Hyalinose und stenotischem oder thrombosiertem Lumen sind das pathologisch-anatomische Korrelat, welches zu äusserst schmerzhaften Hautinfarkten und bei der Calciphylaxis auch zu akralen Nekrosen führt. Das Ulcus hypertonicum Martorell betrifft per Definition Personen, die nicht schwer niereninsuffizient oder nierentransplantiert sind. Die gemeinsamen Risikofaktoren sind arterielle Hypertonie, Diabetes mellitus (meistens Typ 2), bei schwer niereninsuffizienten oder nierentransplantierten Betroffenen ein pathologischer Knochenmetabolismus mit erhöhtem Serum-Phosphat (CKD-MBD), sowie die Medikation mit Vitamin K-Antagonisten zur Antikoagulation: Fetuin A und Matrix Gla-Protein sind zwei Vitamin K-abhängige kalziprotektive Faktoren. Die systemische Therapie ersetzt allfällige Vitamin K-Antagonisten indikationsgerecht durch andere Antikoagulanzien, und setzt bei ausgedehnten Nekrosen intravenös verabreichtes Natrium-Thiosulfat ein. Rund die Hälfte der Nekrosen infizieren sich mit gram-negativen und gram-positiven Keimen. Bei Sepsis oder vor wundchirurgischen Interventionen sind systemische Antibiotika indiziert. Das Ulcus hypertonicum Martorell darf nicht mit einem Pyoderma gangraenosum verwechselt werden, denn die Therapie ist grundlegend verschieden. Abstract

Highlights • In acute isolated SVT, the prevalence of cancer is almost 7 %. • Cancer increases the SVT-associated VTE risk at 3 and 12 months. • Cancer patients with isolated SVT may benefit from prolonged anticoagulation. Abstract Background: Despite significant progress in the understanding of paraneoplastic deep vein thrombosis (DVT) and pulmonary embolism (PE), little is known about the outcomes of cancer-associated superficial vein thrombosis (SVT) in daily practice. Methods: INSIGHTS-SVT was a prospective observational study on patients with acute isolated SVT. Primary outcome measure was symptomatic venous thromboembolism (VTE), a composite of DVT, PE, and SVT extension/recurrence, at 3 months. Clinically relevant bleeding was also assessed. Results: Of 1151 patients included, 6.7 % either had active cancer at baseline or were diagnosed with cancer during 12 months of follow-up. At 3 months, symptomatic VTE had occurred in 13.0 % and 5.4 % of cancer and non-cancer patients, respectively (HR 2.6, 95 % CI 1.3–5.0). Regarding secondary outcomes, cancer patients had increased risks of DVT and PE (HR 3.9, 95 % CI 1.3–11.8) and hospitalization due to VTE (HR 11.0, 95 % CI 2.5–49.0). The rate of clinically relevant bleeding was numerically higher in the cancer cohort (3.9 % vs 1.3 %, HR 3.1, 95 % CI 0.9–10.7). At 12 months, the primary composite outcome had occurred in 15.6 % and 11.9 % of cancer and non-cancer patients, respectively (HR 1.9, 95 % CI 1.0–3.5). After adjusting for additional risk factors, including age, history of DVT/PE and cardiovascular risk factors/diseases, the association of cancer with the primary outcome remained statistically significant. Conclusion: Cancer patients with isolated SVT are at significant risk of symptomatic VTE. While most events occur within 3 months, the VTE risk remains elevated up to one year of follow-up. Abstract

We investigated the safety and efficacy of debulking infrainguinal lesions in patients with peripheral artery disease (PAD) undergoing endovascular revascularization (EVR) as part of the RECording Courses of vascular Diseases (RECCORD) registry. Patient and lesion specific characteristics, including the lesion complexity score (LCS) were analyzed. The primary endpoint encompassed: (i) clinical improvement in Rutherford categories, (ii) index limb re-interventions, and (iii) major amputations during follow-up. The secondary endpoint included the need for bail-out stenting. Overall, 2910 patients were analyzed; 2552 without and 358 with debulking-assisted EVR. Patients were 72 (interquartile range (IQR) = 15) years old and 1027 (35.3%) had diabetes. Overall complication rates were similarly low in the debulking vs the non-debulking group (4.7 vs 3.2%, P = .18). However, peripheral embolizations rates were low but more frequent with debulking vs. non-debulking procedures (3.9 vs 1.1%, P < .001). After adjustment for clinical and lesion-specific parameters, including LCS, no differences were noted for the primary endpoint (odds ration (OR) = 0.99, 95%CI = 0.69–1.41, P = .94). Bail-out stenting was less frequently performed in patients with debulking-assisted EVR (OR = 0.5, 95%CI = 0.38–0.65, P < .0001). Debulking-assisted EVR is currently used in ∼12% of EVR with infrainguinal lesions and is associated with lower bail-out stent rates but higher peripheral embolization rates; no differences were found regarding index limb re-intervention and amputation rates.
