Based on the most prominent underlying pathophysiological mechanisms, CRPS can be classified into different subtypes, namely inflammatory, nociplastic/neuropathic, vasomotor, and motor. Depending on the subtype, personalized treatment can be applied. If conservative treatments are insufficient or ineffective, more invasive treatments may be recommended. This article provides an overview of the most recent insights into CRPS and discusses the most common invasive treatments.The literature regarding interventional treatments for CRPS has been systematically reviewed and summarized.Bisphosphonates are effective in treating the inflammatory subtype, while ketamine can provide pain relief for the nociplastic/neuropathic subtype. Sympathetic blocks are effective in addressing vasomotor disturbances. For patients with refractory symptoms, neurostimulation is a viable option due to its multimechanistic properties for all subtypes. End‐of‐line motor disturbances may benefit from intrathecal baclofen.CRPS is a debilitating condition with an unpredictable course. The effectiveness of treatment varies from patient to patient. Objective: This study aims to compare the efficacy, safety, and tolerability of intravenous ferric carboxymaltose to oral ferrous sulfate in pregnant women (gestation weeks 14–21) with antepartum anemia. Material and Methods: A retrospective cohort study was conducted in a tertiary hospital comparing intravenous 1000 mg ferric carboxymaltose treatment during pregnancy (120 patients) to oral ferrous sulfate (100 mg) 2x1 treatment until delivery (120 patients) for the treatment of iron deficiency anemia in pregnancy. The patients’ responses to treatment were assessed by measuring hemoglobin, hematocrit, and ferritin levels on the 60th day, 120th day, and postpartum 1st day following the initiation of the therapeutic intervention. Results: There were no significant differences between the groups in terms of gestational age, parity, delivery patterns, or antepartum hemoglobin and hematocrit levels. On the 60th and postpartum first day of treatment, the IV ferric carboxymaltose group had significantly higher hemoglobin and hematocrit levels than the oral ferrous sulfate group (p<0.05). Ferritin levels improved rapidly on the 60th day of IV treatment. However, there was no significant difference in hemoglobin, hematocrit, or ferritin levels on the 120th day. Conclusion: Intravenous ferric carboxymaltose proves to be safe and well-tolerated in the management of antepartum iron deficiency anemia. While short-term intravenous iron therapy leads to a quicker elevation of hemoglobin, hematocrit, and ferritin levels in women with antepartum anemia compared to oral ferrous sulfate therapy, over the long term, the levels tend to equalize. This retrospective study aimed to assess and compare guideline adherence and treatment costs in the management of urinary tract infections (UTIs) and bloodstream infections (BSIs) in German tertiary hospitals from January 2019 to December 2020. The study analyzed 586 patient records, with 65% diagnosed with UTIs and 35% with BSIs. Antibiotic treatment was given to 98% of patients, but only 65% received microbiological diagnostics. Bacterial growth was observed in 86% of patients with cultures taken, with Escherichia coli being the leading pathogen. The treatment was intravenous in 63% of cases, with Ceftriaxone as the leading antibiotic agent. The guideline adherence was found to be low, at 33%. Multivariable logistic regression analysis revealed that patients with urogenital risk factors (OR = 1.589; p < 0.001) and increasing age (OR = 1.01; p = 0.007) were significantly more likely to receive guideline-concordant treatment for UTIs and BSIs. Additionally, complicating factors such as diabetes and renal dysfunction were associated with higher adherence rates, underscoring the importance of targeted antibiotic stewardship interventions. Accidents related to the administration of intravenous (IV) medication, such as drug overdose/underdose, drug/patient mis-identification, and delayed bag exchange, occur consistently in clinical fields. Several previous studies have suggested various contact-sensing and image-processing methodologies; however, most of them can increase the workload of nursing staffs during the long-term, continuous monitoring. In this study, we proposed a smart IV pole that can monitor the infusion status of up to four IV medications (patient/drug identification, and liquid residue) with various sizes and hanging positions to reduce IV-related accidents and improve patient safety with the least additional workload; the system consists of 12 cameras, one code scanner, and four controllers. Two types of deep learning models for automated camera selection (CNN-1) and liquid residue monitoring (CNN-2), and three drug residue estimation equations were implemented. The experimental results demonstrated that the accuracy of identification code-checking (60 tests) was 100%. The classification accuracy and the mean inference time of CNN-1 (1200 tests) were 100% and 140 ms. The mean average precision and the mean inference time of CNN-2 (300 tests) were 0.94 and 144 ms. The average error rates between the alarm setting (20, 30, and 40 mL) and the actual drug residue when the alarm first generated were 4.00%, 7.33%, and 4.50% for a 1,000 mL bag; 6.00%, 4.67%, and 2.50% for a 500 mL bag; and 3.00%, 6.00%, and 3.50% for a 100 mL bag, respectively. Our results suggest that the implemented AI-based prototype IV pole is a potential tool for reducing IV-related accidents and improving in-hospital patient safety. [ABSTRACT FROM AUTHOR]

Severe bronchopulmonary dysplasia (BPD) is a chronic lung disorder that primarily affects premature babies with extremely low birth weight and involves in multiple organ system; no effective pharmacotherapy for this disease exists, and mortality remains high. Based on the evidence from previous preclinical studies and phase I clinical trials, this study aims to test the safety of intravenous application of a single dose of human umbilical cord-derived mesenchymal stem cells (hUC-MSCs) in patients with severe BPD. The Mesenchymal Stem cells for Bronchopulmonary Dysplasia Treatment (MSBDT) trial is a single center, open-label, dose-escalation phase I clinical trial. Severe BPD patients were enrolled in Children Hospital of Chongqing Medical University, Chongqing, China. The first six patients were treated with low-dose hUC-MSCs (1 × 106 cells/kg) and the next seven patients were treated with high-dose hUC-MSCs (5 × 106 cells/kg). This study is registered with ClinicalTrials.gov, number NCT03558334. No prespecified infusion-associated adverse events, immediate complication, respiratory or cardiovascular compromise were observed during infusion and 24 h after infusion. No significant changes in safety laboratory values were observed. One death event occurred in the low-dose group on study day 10, and one death event occurred in the high-dose group on study day 24, while, after review in detail, the two cases are not believed to be infusion-associated events. In conclusion, intravenous application of a single dose of hUC-MSCs was tolerated in thirteen patients with severe BPD.

Glans ischemia is an extremely infrequent complication characterized by a total or partial compromise in the penile arterial perfusion. A 15-year-old male patient suffered an episode of ischemia in the glans penis post-circumcision 24 h after surgery. Intravenous treatment with continuous perfusion of pentoxifylline was started for 4 days, with favorable evolution. Complete resolution was observed with no sequelae. There is no consensus on the best therapeutic management. The favorable evolution reported in most of the cases despite different therapeutic approaches leads us to think that the role of the treatments proposed so far is probably less than we believe. Additionally, we present a proposal for a diagnostic and therapeutic guide for this entity. Although the evidence in the literature is scarce and this guideline should be interpreted with caution, we believe that it can constitute a support resource for cases similar to ours. Background: Scant evidence is available on the use of intravenous pain treatment in acute exacerbations of trigeminal neuralgia. The aim of this descriptive study was to evaluate the effectiveness and security of intravenous lacosamide and phenytoin in the treatment of acute trigeminal neuralgia pain. Methods: We reviewed patients who attended the emergency department of a tertiary hospital between 2012 and 2020 for exacerbations of trigeminal neuralgia pain and were treated with either intravenous phenytoin or lacosamide for the first time. Primary endpoints were pain relief and adverse effects during the hospital stay. A comparative analysis between both treatment groups was performed. Results: We studied 144 episodes in 121 patients (median age 61 years, 66.1% women). Trigeminal neuralgia etiology was secondary in 9.9%. Pain relief was observed in 77.8% of 63 patients receiving lacosamide infusions, and adverse effects in 1.6%. Pain relief was observed in 72.8% of 81 phenytoin infusions and adverse effects in 12.3%, all mild. No difference was observed in pain relief between groups, but the proportion of adverse effects was significantly different (p = 0.023). Statistically significant differences were also detected in readmissions within six months, time to readmission, and pain relief status at first follow-up visit. Conclusion: Intravenous lacosamide and phenytoin can be effective and safe treatments for acute pain in trigeminal neuralgia. Introduction: Iron deficiency anemia (IDA) is the most common hematological disease among infants and children. Although most of the children with IDA are treated with oral iron preparations, intravenous (IV) iron is an alternative for children with severe IDA who have difficulty in adhering to or absorbing oral iron. In this study, the reasons of IDA in patients treated with IV iron and the effectivity and safety of iron sucrose preparations in pediatric patients were evaluated. Materials and Methods: The medical records of children received IV iron sucrose at a pheripheral hospital between June 2014 and June 2017 were reviewed retrospectively. The IDA patients who are unresponsive to oral iron or whose hemoglobin (Hb) <7 g/dL and received IV iron treatment were evaluated for the reasons of anemia, the efficacy and safety of IV iron treatment. Before and after iron sucrose infusion laboratory results were compared. All records were reviewed for serious side effects and allergic reactions. Results: A total of 33 patients, aged between 3-18 years were enrolled into the study. While 58% of the patients who received IV iron treatment had inadequate consumption of iron, 24% had malabsorption. In the initial evaluation Hb levels of the patients were 3.2-7.8 (6.54±1.06) g/dL, (mean corpuscular volume) MCV was 59.18±6.66 fL and ferritin was 1.87±1.34 µg/L, the mean post-treatment Hb was 11.39±1.51 g/dL, MCV was 76.06±7.59 fL and ferritin was 54.79±15.64 µg/L. No serious side effects were seen. Conclusion: The use of IV iron sucrose in pediatric patients with IDA leads to significant increase in Hb and reduces erythrocyte suspension transfusion and is an effective and safe method for iron treatment. OBJECTIVE: Osteomyelitis is a relatively understudied disease with no standardized and evidence-based approach to treatment. We aimed to evaluate a cohort of patients with osteomyelitis, comparing the outcomes between intravenous (IV) and oral treatment. PATIENTS AND METHODS: We performed an observational retrospective study on osteomyelitis cases in adult patients seen for care between 2017 and 2019. We collected information on patient characteristics, microbiological etiology, infection treatment, and outcome. In addition, we divided osteomyelitis cases by antibiotic regimens [oral (OTG) vs. intravenous±oral (ITG)] and therapy durations to evaluate outcomes differences. RESULTS: A total of 235 episodes of osteomyelitis were evaluated, with a higher prevalence in male gender. Staphylococci, especially S. aureus, were the most common strains. Out of the 235 evaluated episodes, we selected 142 cases. Of these, 75 were treated with OTG and 67 with ITG. Gram-positive bacteria were the most frequent aetiological agents, with 81 isolates (61.8%). Full recovery was observed in 79 (55.6%) cases; of these, 36 (53.7%) were in the ITG and 43 (57.3%) in the OTG (p = 0.666). At the logistic regression, a polymicrobial infection [OR 4.16 (95%CI 1.28-13.4), p = 0.017] and a less than six weeks treatment duration [OR 4.24 (95%CI 1.38-5.43) p = 0.004] were significantly associated with a higher risk of treatment failure. CONCLUSIONS: Our study suggests that oral treatment efficacy is comparable to ITG therapy for osteomyelitis, confirming the most recent evidence suggesting that oral therapy is non-inferior to intravenous therapy to treat osteomyelitis. Objective: This research aims to examine the effect of playing games with toys made of medical materials in children with cancer on pain that occurs during intravenous (IV) treatment.Methods: The randomized controlled clinical trial was conducted between May 2016 and February 2018. The study sample comprised 110 children (experimental group 55; control group 55), determined using power analysis from the study population. The data were collected by the researcher, using face-to-face interview techniques, the Information Form, and Wong-Baker FACES Pain Rating Scale (WBS).Results: The pre- and post-test pain mean scores of patients in the experimental group were 2.27 ± 0.91 and 0.43 ± 0.66, respectively (p = 0.0001). The pre- and post-test pain mean scores of patients in the control group were 1.72 ± 0.82 and 3.34 ± 0.77, respectively (p = 0.0001).Significance Of Results: The experience of playing with toys made from materials used for invasive procedures relieves pain the during IV treatment.
