Resume Contexte Le suivi endoscopique apres sleeve gastrectomie (SG) revele parfois des resultats inquietants comme la presence de lesions digestives majeures telles que l’œsophagite, l’œsophage de Barrett (OB), ainsi que le cancer de l’œsophage. Objectifs Le but de cette etude etait d’etudier la frequence et la severite des lesions œsophagiennes et gastriques longtemps apres une SG. Methodes Notre equipe a recueilli les resultats des endoscopies œsogastriques realisees sur des patients ayant eu une SG, cinq ans et plus, auparavant, dans notre institution ; c’est-a-dire d’avril 2010 a aout 2014. Nous avons envoye une prescription d’endoscopie œsogastrique par la poste a tous les patients operes entre ces dates. Les resultats ont ete recueillis de janvier 2018 a juin 2019. Resultats Sur les 765 patients qui ont beneficie d’une SG cinq ans et plus auparavant, seulement 78 (10,2 %) ont accepte d’avoir une endoscopie œsogastrique. L’âge moyen avant la SG etait de 51 ans (extremes : 25–70 ans). L’indice de masse corporelle (IMC) moyen etait de 44,2 ± 4,6 kg/m2. Pour 12 de ces 78 patients (15,4 %), il s’agissait d’une chirurgie de revision : 9 SG apres anneau gastrique, et 3 SG de revision (Re-SG). Avec un suivi moyen de 6,3 ± 0,8 ans, les resultats endoscopiques des 78 patients se sont averes normaux dans 31 cas (39,7 %). Une gastrite a ete trouvee chez 28 patients (35,9 %), une œsophagite de grade C ou D dans 15 cas (19,2 %), une hernie hiatale dans 18 cas (23,1 %), un polype gastrique benin dans 2 cas (2,6 %). Parmi les 28 patients atteints de gastrite, l’Helicobacter Pylori a ete detecte dans 8 cas (28,6 %). Aucun cancer n’a ete diagnostique, un OB apres SG a ete trouve dans un seul cas (1,3 %) (endoscopie normale avant la SG). Six des 15 patients atteints d’œsophagite (40 %) avaient des symptomes de reflux. Quatorze de ces patients (93,3 %) ont developpe une œsophagite de novo. Conclusion Seulement 10,2 % des patients operes ont accepte d’avoir une endoscopie œsogastrique de controle, a long terme, apres SG. La frequence et la severite des anomalies observees, avec un suivi moyen de plus de 6 ans, plaident pour une strategie d’endoscopies œsogastriques systematiques de controle, et a long terme, apres SG.

Gastric sleeve stenosis (GSS) is described in 1%-4% of patients.To evaluate the role of endoscopy in the management of stenosis after laparoscopic sleeve gastrectomy using a standardized approach according to the characteristic of stenosis.Retrospective, observational, single-center study on patients referred from several bariatric surgery departments to an endoscopic referral center.We enrolled 202 patients. All patients underwent endoscopy in a fluoroscopy setting, and a systematic classification of the type, site, and length of the GSS was performed. According to the characteristics of the stenosis, patients underwent pneumatic dilatation or placement of a self-expandable metal stent or a lumen-apposed metal stent. Failure of endoscopic treatment was considered an indication for redo surgery, whereas patients with partial or complete response were followed up for 2 years. In the event of a recurrence, a different endoscopic approach was used.We found inflammatory strictures in 4.5% of patients, pure narrowing in 11%, and functional stenosis in 84.5%. Stenosis was in the upper tract of the stomach in 53 patients, whereas medium and distal stenosis was detected in 138 and 11 patients, respectively, and short stenosis in 194 patients. A total of 126 patients underwent pneumatic dilatation, 8 self-expandable metal stent placement, 64 lumen-apposed metal stent positioning, and 36 combined therapy. The overall rate of endoscopy success was 69%.GSS should be considered to be a chronic disease, and the endoscopic approach seems to be the most successful treatment, with a prolonged positive outcome of 69%.
