This article extends topics about linear algebra and operator theoretic linear transformations on complex vector spaces to those on bicomplex spaces. For example, Definition 3 for the first time defines algebraically idempotent vector spaces, which generalizes the standard definition of a vector space and which includes bicomplex vector spaces as a special case, along with its dimension and its basis in terms of a corresponding vectorial idempotent representation. The article also shows how an n×nn\times n bicomplex matrix’s idempotent representation leads to a bicomplex Jordan form and a description of its bicomplex invariant subspace lattice diagram. Similarly, in a new way, the article rigorously defines “bicomplex Banach and Hilbert” spaces, and then it expands, for the first time, the theory of compact operators on complex Banach spaces to those on bicomplex Banach spaces. In these ways, the article indicates that the idempotent representation extends complex linear algebra and operator theory in a surprisingly generalized and straightforward way to vector space results with bicomplex and multicomplex scalars.

In this study, we aimed to develop a theoretical framework for a multimodal, integrative, exercise, anti-inflammatory and dietary counselling (MMIEAD) intervention for patients with renal cachexia with reference to how this addresses the underlying causal pathways for renal cachexia, the outcomes anticipated, and how these will be evaluated. We used a Theory of Change (ToC) approach to guide six steps. Step 1 included inputs from a workshop to obtain key stakeholder views on the potential development of a multimodal intervention for renal cachexia. Step 2 included the findings of a mixed-methods study with Health Care Practitioners (HCPs) caring for individuals with End Stage Kidney Disease (ESKD) and cachexia. Step 3 included the results from our systematic literature review on multimodal interventions for cachexia management. In step 4, we used the body of our research team's cachexia research and wider relevant research to gather evidence on the specific components of the multimodal intervention with reference to how this addresses the underlying causal pathways for renal cachexia. In steps 5 and 6 we developed and refined the ToC map in consultation with the core research team and key stakeholders which illustrates how the intervention components of MMIEAD interact to achieve the intended long-term outcomes and anticipated impact. The results of this study provide a theoretical framework for the forthcoming MMIEAD intervention for those with renal cachexia and in subsequent phases will be used to determine whether this intervention is effective. To the best of our knowledge no other multimodal intervention trials for cachexia management have reported a ToC. Therefore, this research may provide a useful framework and contribute to the ongoing development of interventions for cachexia management.

Renal cachexia is an important consideration in the person-centred care that is needed in end-stage renal disease (ESRD). However, given that clinical guidelines relating to renal cachexia are largely absent, this is an unmet care need. To inform guidelines and future renal service planning, there is an urgency to understand individuals' experiences of renal cachexia and the interrelated experiences of the carers in their lives. We report here the protocol for an interpretative phenomenological study which will explore this lived experience. A purposive sampling strategy will recruit individuals living with ESRD who have cachexia and their carers. A maximum of 30 participants (15 per group) dependent on saturation will be recruited across two nephrology directorates, within two healthcare trusts in the United Kingdom. Individuals with renal cachexia undergoing haemodialysis will be recruited via clinical gatekeepers and their carers will subsequently be invited to participate in the study. Participants will be offered the opportunity to have a face-to-face, virtual or telephone interview. Interviews will be audio-recorded, transcribed verbatim and analysed using interpretative phenomenological analysis. NVivo, will be used for data management. Ethical approval for this study was granted by the Office for Research Ethics Committees Northern Ireland (REC Reference: 22/NI/0107). Scientific evidence tends to focus on measurable psychological, social and quality of life outcomes but there is limited research providing in-depth meaning and understanding of the views of individuals with renal disease who are experiencing renal cachexia. This information is urgently needed to better prepare healthcare providers and in turn support individuals with ESRD and their carers. This study will help healthcare providers understand what challenges individuals with ESRD, and their carers face in relation to cachexia and aims to inform future clinical practice guidelines and develop supportive interventions which recognise and respond to the needs of this population.

The aim of this study was to create an economic decision-making tool to allow farmers and stakeholders to make informed decisions around costs, benefits, and risks involved in production of sandfish, Holothuria scahru. and Babylon snail, Babylonia areolata, in different coculture arrangements based on a sound economic framework. A small-scale (0.16 ha) farm operation in Vietnam was modelled using production data obtained in previous studies to represent the costs and returns of rearing sandfish in tank-based nursery systems and in pond-based coculture grow-out systems. Sandfish and Babylonia monoculture was compared with three coculture models where the two species were separated, and the amount of pond area allocated to each ranged from 25% to 75%. A fourth coculture model (Co-4; 'comingled') was assessed where the two species were not separated and shared 100% of the substrate. The sandfish monoculture system produced 487 kg of sandfish at an annual cost of USD 5,616. providing an annual return of USD -3.521. The largest annual cost associated with sandfish monoculture was labor (USD 8.04 per kg). Pond monoculture of Babylon snail returned USD 1.876 annually, with an annual production cost of USD 19,858. Feeding represented the largest production cost associated with Babylonia monoculture of USD 14,340 per annum, or USD 5.11 per kg. Sandfish monoculture and Babylonia monoculture returned benefit cost ratios of 0.37 and 1.09, respectively. The coculture model with pond area divided 50:50 between sandfish and Babylonia during grow-out, produced 258 kg of sandfish and 2,584 kg of Babylonia over a 359-day production cycle. This equates to USD 1,108 and USD 20,001 annual production of sandfish and Babylonia, respectively. After production costs, this model provided a return of USD 2.309 annually at a benefit cost ratio of 1.12. The comingled culture model. Co-4. provided the greatest annual return of USD 6,019 and a benefit-cost ratio of 1.24. Results of this study build on previous research into practical aspects of sandfish/Babylonia coculture and provide a more complete picture of the potential application of this research to improve livelihoods and income of small-scale farmers in Vietnam. Purpose: To present prosthetic complications observed in the medium and long term in patients treated with metal-acrylic resin implant-supported fixed complete dentures (MAIFCDs). Materials and Methods: A total of 87 patients who were treated with MAIFCDs were included. Acrylic resin (denture teeth and base) fracture, screw loosening (prosthetic or abutment), screw (prosthetic or abutment), and framework fracture were analyzed for descriptive statistics. Product-limit survival estimates were used for the cumulative no-complication rate (CNCR) of prostheses. All results were assessed using a = .05. Results: The average observation time was 42.9 months (range: 19 to 153). Of 597 implants, 8 mandibular (44.4%) and 10 maxillary implants (55.6%) failed. In total, 29 patients (33.3%) experienced no complications in an average of 7 years, and 58 patients (66.6%) experienced at least one complication. The average time for the first complication to occur was 23 months (range: 1 to 97 months). The average number of complications was 5.9/patient. Acrylic resin complications were the most frequent, followed by screw loosening, screw fracture, and framework fracture. Of those patients with complications, 9 (16%) had maxillary MAIFCDs with an average of 4 complications. In total, 28 patients (48%) had only mandibular MAIFCDs with an average of 5 complications, and 21 patients (36%) had both arches treated with MAIFCDs with an average of 9 complications. Mandibular MAIFCDPs in women had a higher CNCR compared to maxillary MAIFCDs of both women and men and when both arches of women were treated with MAIFCDs (P < .05). Conclusions: The CNCR of MAIFCDs differed depending on the arch treated and sex. Mandibular MAIFCDs in women had a lower complication rate than MAIFCDs in other arch-sex pairs. Background Methods Results Conclusions The oral microbiome‐dependent nitrate (NO3−)–nitrite (NO2−)–nitric oxide (NO) pathway may help regulate blood pressure. NO2−‐producing bacteria in subgingival plaque are reduced in relative abundance in patients with untreated periodontitis compared with periodontally healthy patients. In periodontitis patients, the NO2−‐producing bacteria increase several months after periodontal treatment. The early effects of periodontal treatment on NO2−‐producing bacteria and the NO3−–NO2−–NO pathway remain unknown. The aim of this study was to determine how periodontal treatment affects the oral NO2−‐producing microbiome and salivary NO3− and NO2− levels over time.The subgingival microbiota of 38 periodontitis patients was analysed before (baseline [BL]) and 1, 7 and 90 days after periodontal treatment. Changes in NO2−‐producing bacteria and periodontitis‐associated bacteria were determined by 16s rRNA Illumina sequencing. Saliva samples were collected at all‐time points to determine NO3− and NO2− levels using gas‐phase chemiluminescence.A significant increase was observed in the relative abundance of NO2−‐producing species between BL and all subsequent timepoints (all p < 0.001). Periodontitis‐associated species decreased at all timepoints, relative to BL (all p < 0.02). NO2−‐producing species negatively correlated with periodontitis‐associated species at all timepoints, with this relationship strongest 90 days post‐treatment (ρ = −0.792, p < 0.001). Despite these findings, no significant changes were found in salivary NO3− and NO2− over time (all p > 0.05).Periodontal treatment induced an immediate increase in the relative abundance of health‐associated NO2−‐producing bacteria. This increase persisted throughout periodontal healing. Future studies should test the effect of periodontal treatment combined with NO3− intake on periodontal and cardiovascular health.
