1. Constraints on $\tau$ electromagnetic moments via tau pair production at the Muon colliders

Abstract

The anomalous magnetic ($\tilde{a}_\tau$) and electric dipole ($\tilde{d}_\tau$) moment of tau lepton described in $\tau\bar{\tau}\gamma$ vertices are studied via $\mu^{+}\mu^{-} \rightarrow \tau^+\bar{\tau}^-$ process at the Muon colliders designed with the proposed center-of-mass-energy / integrated luminosity configurations of 3 TeV/ 1 ab$^{-1}$ and 10 TeV/ 10 ab$^{-1}$.We obtained the $95\%$ confidence level limits on the $\tilde{a}_\tau$ and $\tilde{d}_\tau$ parameters without and with systematic uncertainty of $10\%$ and compared with the experimental results.The most stringent limits on the anomalous couplings without systematic uncertainty are $-2.63\times10^{-4}< \tilde{a}_\tau <2. 65\times10^{-4}$ and $\mid \tilde{d}_\tau \mid \leq 1.47\times10^{-18} e cm$ at $\sqrt s =10$ TeV and $L_{int}$=10 ab$^{-1}$ option of the Muon collider. Our results show that the Muon colliders lead to a remarkable improvement on the current experimental limits of the anomalous magnetic and electric dipole moments of the tau lepton., Comment: 12 pages, 3 figures

Abstract

In the post-LHC era, the muon collider represents a frontier project capable of providing high-energy and high-luminosity leptonic collisions among future lepton-lepton particle accelerators. In addition, it provides significantly cleaner final states than those produced in hadron collisions. With this expectation in mind, in this article, we research the sensitivity of the anomalous neutral gauge boson couplings $Z\gamma\gamma\gamma$, $ZZ\gamma \gamma$, and $ZZZ\gamma$ defined by dimension-8 operators, through the $\mu^+\mu^- \to \mu^+\mu^-Z\gamma$ signal, with the $Z$-boson decaying to neutrino pair. The projections of new physics at the future muon collider with the center-of-mass energy of $\sqrt{s}=10$ TeV, integrated luminosity of ${\cal L}=10$ $\rm ab^{-1}$, and systematic uncertainties of $\delta_{sys}=0\%, 3\%, 5\%$, for extraction of expected sensitivity on the anomalous $f_ {T,j}/\Lambda^4$ couplings at $95\%$ confidence level, are of the order of ${\cal O}(10^{-4}- 10^{-3})$. Compared with the research of the ATLAS and CMS Collaborations on the anomalous quartic gauge couplings, we find that the high-luminosity future muon collider could have better sensitivity.

3. Probing the electromagnetic properties of the neutrinos at future lepton colliders

Abstract

In this study, we explore the non-standard $\nu\bar{\nu}\gamma \gamma$ couplings parametrized by dimension-seven operators via $e^{+}e^{-} \to \nu\bar{\nu}\gamma$ process at the FCC-ee/CEPC and $\mu^{+}\mu^{-}\to\nu\bar{\nu}\gamma$ process at the Muon Colliders. For the detailed Monte Carlo simulation, all signal and relevant background events are produced within the framework of Madgraph where non-standard $\nu\bar{\nu}\gamma \gamma$ couplings are implemented. After passing through Pythia for parton showering and hadronization, detector effects are included via tuned corresponding detector cards for each collider in Delphes. Projected sensitivities on $\nu\bar{\nu}\gamma \gamma$ couplings are obtained at a 5$\sigma$ confidence level without and with $5\%$ systematic uncertainties for the FCC-ee/CEPC and the Muon Colliders, showcasing the complementarity between lepton colliders. Our best limit on the anomalous $\nu\bar{\nu}\gamma \gamma$ couplings even with 5\% systematic uncertainties for muon collider with $\sqrt{s}=10$ TeV and $L_{int}=3$ ab$^{-1}$ are found to be thirteen orders of magnitude stronger than the upper bound obtained from rare decay $Z\to\gamma\gamma\nu\bar{\nu}$ analysis using LEP data., Comment: 17 pages, 9 figures

4. Expected sensitivity on the anomalous quartic neutral gauge couplings in $\gamma\gamma$ collisions at the CLIC

Abstract

The presence of multi-boson self-interactions is implied by the non-Abelian gauge structure of the Standard Model (SM). Precise measurements of these interactions allow not only testing the nature of the SM but also new physics contribution arising from the beyond SM. The investigation of these interactions can be approached in a model-independent manner using an effective theory approach, which forms the main motivation of this study. In this paper, we examine the anomalous neutral quartic gauge couplings through the process $\gamma \gamma \rightarrow Z Z$ at the Compact Linear Collider (CLIC) with the center-of-mass energy of $\sqrt{s}=3$ TeV, integrated luminosities of ${\cal L}=5$ $\rm ab^{-1}$. The anomalous neutral quartic gauge couplings is implemented into FeynRules to generate a UFO module inserted into Madgraph to generate both background and signal events. These events are then passed through Pythia 8 for parton showering and Delphes to include realistic detector effects. We obtain that the sensitivities on the anomalous quartic neutral gauge couplings with $95\%$ Confidence Level are given as: $f_{T0}/\Lambda^{4}=[-1.06; 1.08]\times 10^{-3}$ ${\rm TeV^{-4}}$, $f_{T1}/\Lambda^{4}=[-1.06; 1.08]\times 10^{-3}$ ${\rm TeV^{-4}}$,$f_{T2}/\Lambda^{4}=[-1.06; 1.08]\times 10^{-3}$ ${\rm TeV^{-4}}$,$f_{T0}/\Lambda^{4}=[-1.06; 1.08]\times 10^{-3}$ ${\rm TeV^{-4}}$, $f_{T5}/\Lambda^{4}=[-4.08; 4.08]\times 10^{-4}$ ${\rm TeV^{-4}}$ and $f_{T8}/\Lambda^{4}=[-1.10; 1.10]\times10^{-4}$ ${\rm TeV^{-4}}$. 5. Search for the anomalous quartic gauge couplings through $Z\gamma$ production at $e^{-} e^{+}$ colliders

Abstract

Spontaneous breaking of the $SU(2)_{L}\times U(1)_{Y}$ electroweak symmetry of the Standard Model (SM) sets the constraints on triple gauge couplings and quartic gauge couplings. Therefore, the measurement of multiboson production in $e^{-} e^{+}$ collisions allows us to directly examine the SM predictions and perform indirect investigations of new physics beyond the SM. In this paper, we concentrate the process $e^{-} e^{+} \to e^{-} Z\gamma e^{+}$ with $Z$ boson decaying to neutrinos to investigate the anomalous quartic gauge couplings using the effective Lagrangian approach at the Compact Linear Collider (CLIC). We obtain the sensitivities on the anomalous $ f_ {Ti}/\Lambda^4$ ($i=0,2,5,6,7,8,9$) couplings taking into account the systematic uncertainties of $3, 5 \%$ at $95\%$ Confidence Level for the CLIC with $\sqrt{s}=3$ TeV. Our results show that the sensitivities on some anomalous couplings without systematic errors are up to two orders of magnitude better than the current experimental limits. Considering a realistic systematic uncertainty such as $5 \%$ from possible experimental sources, the sensitivity of all anomalous quartic couplings gets worse by about $10\%$ compared to those without systematic uncertainty for the CLIC., Comment: 23 Pages, 10 Figures, 2 Tables

Abstract

Abstract

This work offers the constraints on the anomalous neutral triple gauge couplings for the process $e^+e^- \to ZZ $ at the CLIC with $\sqrt{s}=3$ TeV. The realistic CLIC detector environments and their effects are considered in our analysis. The study is planned for the decays of produced $Z$ bosons to a pair of charged leptons (electrons or muons) and neutrino pairs. The bounds on the anomalous neutral triple gauge couplings defining $CP$-violating $C_{\widetilde{B}W}/{\Lambda^4}$ coupling and three $CP$-conserving $C_{WW}/{\Lambda^4}$, $C_{BW}/{\Lambda^4}$, and $C_{BB}/{\Lambda^4}$ couplings are obtained. Abstract

In this study, we investigate the potential of $\mu^{-} \mu^{+}\to Z\gamma \to \nu\bar{\nu}\gamma$ process at the future muon collider with a center-of-mass energy of 3 TeV to examine the anomalous $ZZ\gamma$ and $Z\gamma\gamma$ neutral triple gauge couplings defining $CP$-conserving $C_{\widetilde{B}W}/{\Lambda^4}$ coupling and three $CP$-violating $C_{BB}/{\Lambda^4}$, $C_{BW}/{\Lambda^4}$, $C_{WW}/{\Lambda^4}$ couplings. All signal and relevant background events are generated in MadGraph and passed through Pythia for parton showering and hadronization. Detector effects are also considered via tuned muon detector cards in Delphes. The effects of systematic uncertainties of $0\%$, $3\%$ and $5\%$ on the sensitivities are studied. The best sensitivities obtained from the process $\mu^{-} \mu^{+}\to Z\gamma \to \nu\bar{\nu}\gamma$ are $[-6.53;6.64]\times10^{-2}$ TeV$^{-4}$ on $CP$-conserving $C_{\widetilde{B}W}/{\Lambda^4}$ coupling and $[-2.47;2.47]\times10^{-2}$ TeV$^{-4}$, $[-8.46;8.46]\times10^{-2}$ TeV$^{-4}$ and $[-2.20;2.20]\times10^{-1}$ TeV$^{-4}$ on $CP$-conserving $C_{BB}/{\Lambda^4}$, $C_{BW}/{\Lambda^4}$, $C_{WW}/{\Lambda^4}$ couplings , respectively. Our obtained results on the anomalous neutral gauge couplings set more stringent sensitivity, ranging between 5 and 15 times than the current experimental results while slightly better than the phenomenological studies at future pp colliders such as the HL-LHC, the HE-LHC and the FCC-hh, respectively. On the other hand, we can see that the bounds on the anomalous neutral gauge couplings expected to be obtained for the future $e^{-}e^{+}$ colliders such as the CLIC are roughly 2 times better than our results., Comment: 21 pages, 9 figures

Obtained sensitivities on the anomalous neutral triple gauge couplings with $95\%$ Confidence Level are given with systematic uncertainties of $0\%$, $5\%$ and $10\%$ for unpolarized, $-80\%$ and $80\%$ polarized electron beams with integrated luminosities of ${\cal L}_{\text{int}}=5$ $\rm ab^{-1}$, ${\cal L}_{\text{int}}=4$ $\rm ab^{-1}$ and ${\cal L}_{\text{int}}=1$ $\rm ab^{-1}$, respectively. Abstract

The Large Hadron–Electron Collider (LHeC) is designed to move the field of deep inelastic scattering (DIS) to the energy and intensity frontier of particle physics. Exploiting energy-recovery technology, it collides a novel, intense electron beam with a proton or ion beam from the High-Luminosity Large Hadron Collider (HL-LHC). The accelerator and interaction region are designed for concurrent electron–proton and proton–proton operations. This report represents an update to the LHeC’s conceptual design report (CDR), published in 2012. It comprises new results on the parton structure of the proton and heavier nuclei, QCD dynamics, and electroweak and top-quark physics. It is shown how the LHeC will open a new chapter of nuclear particle physics by extending the accessible kinematic range of lepton–nucleus scattering by several orders of magnitude. Due to its enhanced luminosity and large energy and the cleanliness of the final hadronic states, the LHeC has a strong Higgs physics programme and its own discovery potential for new physics. Building on the 2012 CDR, this report contains a detailed updated design for the energy-recovery electron linac (ERL), including a new lattice, magnet and superconducting radio-frequency technology, and further components. Challenges of energy recovery are described, and the lower-energy, high-current, three-turn ERL facility, PERLE at Orsay, is presented, which uses the LHeC characteristics serving as a development facility for the design and operation of the LHeC. An updated detector design is presented corresponding to the acceptance, resolution, and calibration goals that arise from the Higgs and parton-density-function physics programmes. This paper also presents novel results for the Future Circular Collider in electron–hadron (FCC-eh) mode, which utilises the same ERL technology to further extend the reach of DIS to even higher centre-of-mass energies.

Abstract

This work offers the constraints on the anomalous neutral triple gauge couplings for the process e + e - → Z Z at the CLIC with s = 3 TeV. The realistic CLIC detector environments and their effects are considered in our analysis. The study is planned for the decays of produced Z bosons to a pair of charged leptons (electrons or muons) and neutrino pairs. The bounds on the anomalous neutral triple gauge couplings defining CP-violating C B ~ W / Λ 4 coupling and three CP-conserving C WW / Λ 4 , C BW / Λ 4 , and C BB / Λ 4 couplings are obtained. Abstract

Spontaneous breaking of the SU 2 L × U 1 Y electroweak symmetry of the standard model (SM) sets the constraints on triple and quartic gauge couplings. Therefore, the measurement of multiboson production in e − e + collisions allows us to directly test the predictions of the SM and to indirectly probe new physics beyond the SM. In this paper, we focus on the process e − e + → e − Z γ e + with Z boson decaying into neutrinos to study the anomalous quartic gauge couplings using the effective Lagrangian approach at the compact linear collider (CLIC). We obtain the sensitivities on the anomalous f T, i /Λ4 (i = 0, 2, 5, 6, 7, 8, 9) couplings taking into account the systematic uncertainties of 3, 5% at 95% Confidence Level for the CLIC with s = 3 TeV. Our results show that the sensitivities on some anomalous couplings without systematic errors are up to two orders of magnitude better than the current experimental limits. Considering a realistic systematic uncertainty such as 5% from possible experimental sources, the sensitivity of all anomalous quartic couplings becomes worse by about 10% compared to those without systematic uncertainty for the CLIC. Abstract

The non-Abelian gauge structure of the Standard Model (SM) implies the presence of multi-boson self-interactions. Precise measurements of these interactions allow testing the nature of the SM and new physics contributions arising from the beyond SM. The investigation of these interactions can be approached in a model-independent manner using an effective theory approach, which forms the main motivation of this study. In this paper, we examine the anomalous neutral quartic gauge couplings through the process γ γ → Z Z at the Compact Linear Collider with the center-of-mass energy of s = 3 TeV and integrated luminosity of L = 5 a b - 1 . The anomalous neutral quartic gauge couplings are implemented into FeynRules to generate a UFO module inserted into Madgraph to create background and signal events. These events are passed through Pythia 8 for parton showering and Delphes to include realistic detector effects. We obtain that the sensitivities on the anomalous quartic neutral gauge couplings with 95 % Confidence Level are given as: f T 0 / Λ 4 = [ - 1.06 ; 1.08 ] × 10 - 3 TeV - 4 , f T 1 / Λ 4 = [ - 1.06 ; 1.08 ] × 10 - 3 TeV - 4 , f T 2 / Λ 4 = [ - 1.06 ; 1.08 ] × 10 - 3 TeV - 4 , f T 0 / Λ 4 = [ - 1.06 ; 1.08 ] × 10 - 3 TeV - 4 , f T 5 / Λ 4 = [ - 4.08 ; 4.08 ] × 10 - 4 TeV - 4 and f T 8 / Λ 4 = [ - 1.10 ; 1.10 ] × 10 - 4 TeV - 4 . Abstract

These interactions can be examined using a model-independent way in effective theory approach that composes the motivation part of this study. In this paper, we examine the anomalous ZZγ and Zγγ neutral triple gauge couplings via the process e−e+→Zγ for the neutrino-antineutrino pair decay of Z boson. It has performed with both unpolarized and polarized electron beams at the Compact Linear Collider with s=3 TeV. The study focused on CP-conserving CB˜W/Λ4 and CP-violating CBB/Λ4, CBW/Λ4, CWW/Λ4 couplings. Obtained sensitivities on the anomalous neutral triple gauge couplings with 95% Confidence Level are given with systematic uncertainties of 0%, 5% and 10% for unpolarized, −80% and 80% polarized electron beams with integrated luminosities of Lint=5 ab−1, Lint=4 ab−1 and Lint=1 ab−1, respectively. Comparing the latest experimental limits and related phenomenological studies, our results on the anomalous neutral gauge couplings are set more stringent sensitivity between 10-30 times of magnitude.

33. Model-independent study on the anomalous ZZγ and Zγγ couplings at the future muon collider.

Abstract

In this study, we investigate the potential of μ - μ + → Z γ → ν ν ¯ γ process at the future muon collider with a center-of-mass energy of 3 TeV to examine the anomalous ZZ γ and Z γ γ neutral triple gauge couplings defining CP-conserving C B ~ W / Λ 4 coupling and three CP-violating C BB / Λ 4 , C BW / Λ 4 , C WW / Λ 4 couplings. All signal and relevant background events are generated in MadGraph and passed through Pythia for parton showering and hadronization. Detector effects are also considered via tuned muon detector cards in Delphes. The effects of systematic uncertainties of 0 % , 3 % and 5 % on the sensitivities are studied. The best sensitivities obtained from the process μ - μ + → Z γ → ν ν ¯ γ are [ - 6.53 ; 6.64 ] × 10 - 2 TeV - 4 on CP-conserving C B ~ W / Λ 4 coupling and [ - 2.47 ; 2.47 ] × 10 - 2 TeV - 4 , [ - 8.46 ; 8.46 ] × 10 - 2 TeV - 4 and [ - 2.20 ; 2.20 ] × 10 - 1 TeV - 4 on CP-conserving C BB / Λ 4 , C BW / Λ 4 , C WW / Λ 4 couplings , respectively. Our obtained results on the anomalous neutral gauge couplings set more stringent sensitivity, ranging between 5 and 15 times than the current experimental results while slightly better than the phenomenological studies at future pp colliders such as the HL-LHC, the HE-LHC and the FCC-hh, respectively. On the other hand, we can see that the bounds on the anomalous neutral gauge couplings expected to be obtained for the future e - e + colliders such as the CLIC are roughly 2 times better than our results. Abstract

In this study, we explore the non-standard ν ν ¯ γ γ couplings parametrized by dimension-seven operators via e + e − → ν ν ¯ γ process at the FCC-ee/CEPC and μ + μ − → ν ν ¯ γ process at the Muon Colliders. For the detailed Monte Carlo simulation, all signal and relevant background events are produced within the framework of Madgraph where non-standard ν ν ¯ γ γ couplings are implemented. After passing through Pythia for parton showering and hadronization, detector effects are included via tuned corresponding detector cards for each collider in Delphes. Projected sensitivities on ν ν ¯ γ γ couplings are obtained at a 5 σ confidence level without and with 5% systematic uncertainties for the FCC-ee/CEPC and the Muon Colliders, showcasing the complementarity between lepton colliders. Our best limit on the anomalous ν ν ¯ γ γ couplings even with 5% systematic uncertainties for muon collider with s = 10 TeV and L i n t = 10 ab−1 are found to be fourteen orders of magnitude stronger than the upper bound obtained from rare decay Z → γ γ ν ν ¯ analysis using LEP data. 39. Can invasive interventions be avoided with a holistic swallowing therapy program in older patients in intensive care units: percutaneous endoscopic gastrostomy tubes or oral intake?

Abstract

Introduction: The use of percutaneous endoscopic gastrostomy (PEG) tubes in older patients did not show any benefits in terms of survival, improvement in quality of life, or reduction in aspiration pneumonia. Significant gaps exist regarding the evidence for the evaluation and management of dysphagia in older patients. This study aimed to diagnose swallowing disorders and highlight the importance of swallowing therapy in older patients in intensive care units (ICUs)., Materials and Methods: Twenty-five older patients (12 men, 13 women, mean age 67.22 ± 24.03 years) hospitalized in the ICUs with complaints of dysphagia were analyzed prospectively. The 12 weeks (14-16 sessions) of swallowing therapy were administered to patients with dysphagia who signed the (voluntary) consent form. The bedside water swallowing test (BWSS), Functional Oral Intake Scale (FOIS) Score, Clinical Swallowing Evaluation, Mini Nutritional Assessment Test (MNAT), Eating Assessment Tool (EAT-10), the Turkish version of the World Health Organisation Quality of Life Scale Elderly Module, and the Swallowing Therapy Programme Protocol were applied. Pretherapy stage (T1) and post-therapy stage (T2) results were compared with videofluoroscopy swallowing study (VFSS) recordings with thin liquids, moderately thick liquids, extremely thick liquids, and crackers (International Dysphagia Diet Standardization Initiative [IDDSI] Levels 0, 3, 4, and 7, respectively)., Results: The World Health Organisation Quality of Life Scale Elderly Module (WHOQOL-OLD) raw scores significantly improved from T1 (38.63 ± 7.05) to T2 (73.07 ± 4.82). The bedside water swallowing test results demonstrated statistically significant differences in therapy timings among older patients ( p  < 0.001). There were significant improvements in swallowing physiology, as represented by the improved oral and pharyngeal composite scores of the Modified Barium Swallow Impairment Profile (MBSImP) and Penetration-Aspiration Scala (PAS) levels. Before therapy, all patients exhibited high rates of oropharyngeal residue with thin liquids and spoon-thick pudding viscosities (MNA ≤ 17). The results reported in the present study show that malnutrition risk is linked to a poorer QoL in older patients on admission to ICUs. Statistical analyses revealed the dominant effects of functional status and eating-related factors on QoL in this group., Discussion: Early dysphagia diagnosis of older patients and subsequent application of exercise-based swallowing therapy increase the quality of life of patients. In this study, exercise-based swallowing therapy was developed in Turkey and can be used in older patients as part of a holistic cognitive-communication-swallowing intervention program. Results prove the effectiveness of the developed exercise-based swallowing therapy on the cognitive-communication-swallowing skills of older patients. The present findings reinforce the role of nutrition as a priority for improving patients' perceptions of QoL. Further studies are required to investigate and identify the interventions that improve QoL in older patients. 40. The Relationship of Carotid and Brachial Artery Wall Thickness Measurement with Cardiovascular Risk Scoring in Individuals Living with HIV.

Abstract

Objective: This study aimed to investigate whether human immunodeficiency virus (HIV) infection affects carotid and brachial artery wall thickness and whether measurement of this thickness contributes to traditional cardiovascular risk scoring in individuals living with HIV., Materials and Methods: The patient group included people living with HIV who were followed up in the infectious disease clinic, and the control group included patients without HIV. In both groups, carotid artery intima-media thickness (cIMT) was measured with B-mode ultrasonography (B-USG). cIMT 0.9 mm and above was considered subclinical atherosclerosis., Results: The patient group consisted of 66, and the control group consisted of 40 participants. The median cIMT of the patient and control groups was 0.92 (0.45-1.45) mm and 0.55 (0.35-1.25) mm, respectively ( p <0.001). Brachial artery IMT was significantly higher in the patient group with 0.45 (0.30-0.76) mm, while it was 0.35 (0.17-0.50) mm in the control group ( p <0.001). Although the difference between the cIMT and brachial artery IMT results of the patient and control groups was significant between 18-59 years of age, this difference disappeared in older ages. In the patient group, subclinical atherosclerosis was detected with cIMT in individuals under 30 years of age for whom FRS (Framingham risk score) could not be calculated and in low-risk groups according to FRS (20% and 62.9%, respectively)., Conclusion: cIMT and brachial artery IMT were found to be significantly higher in people living with HIV. Abstract

Purpose: To assess the value of radiation therapy (RT) with helical tomotherapy (HT) in the management of locally advanced malignant pleural mesothelioma (MPM) receiving no or lung-sparing surgery., Methods: Consecutive MPM cases not undergoing extrapleural pneumonectomy and receiving intensity-modulated (IM) HT were retrospectively evaluated for local control, distant control, progression-free survival (PFS), and overall survival (OS). Impact of age, systemic treatment, RT dose, and recurrence patterns was analyzed by univariate and multivariate analysis. As a secondary endpoint, reported toxicity was assessed., Results: A total of 34 localized MPM cases undergoing IMHT were identified, of which follow-up data were available for 31 patients. Grade 3 side effects were experienced by 26.7% of patients and there were no grade 4 or 5 events observed. Median PFS was 19 months. Median OS was 20 months and the rates for 1‑ and 2‑year OS were 86.2 and 41.4%, respectively. OS was significantly superior for patients receiving adjuvant chemotherapy (p = 0.008)., Conclusion: IMHT of locally advanced MPM after lung-sparing surgery is safe and feasible, resulting in satisfactory local control and survival. Adjuvant chemotherapy significantly improves OS. Randomized clinical trials incorporating modern RT techniques as a component of trimodal treatment are warranted to establish an evidence-based standard of care pattern for locally advanced MPM., (© 2023. 42. Image Guided Intraoperative Radiation Therapy After Surgical Resection of Brain Metastases: A First In-Human Feasibility Report.

Abstract

Purpose: A correct placement of the applicator during intraoperative radiation therapy for brain metastasis is of paramount importance, to deliver a precise and safe treatment. The applicator-to-surface contact assessment cannot be performed under direct observation because the applicator itself limits the visual range. No image guided verification is currently performed intracranially. We hypothesize that image guided intraoperative radiation therapy would assure a more precise delivery in the target area. We describe our workflow in a first in-human experience., Methods and Materials: Phantom-based measurements were performed to reach the best cone beam computed tomography imaging quality possible. Once defined, a clinical feasibility study was initiated. An in-room cone beam computed tomography device is used to acquire intraoperative images after placing the applicator. Repositioning the applicator is thereafter discussed with the surgeon, according to the imaging outcomes, if required., Results: An optimal image quality was achieved with 120-kV voltage, 20-mA current, and a tube current time product of 150 mAs. An additional 0.51 mSv patient exposure was calculated for the entire procedure. The wide dynamic range (-600 HU to +600 HU) of cone beam computed tomography and a 27 HU mean computed tomography values difference between brain tissue and spherical applicator allows distinguishing both structures. In this first in-human experience, the applicator was repositioned after evidencing air gaps, assuring full applicator-to-surface contact., Conclusions: This first in-human procedure confirmed the feasibility of kilovoltage image guided intraoperative radiation therapy in a neurosurgical setting. Abstract

We address the lack of research regarding aerodynamic events behind respiratory distress at COVID-19. The use of chest CT enables quantification of pneumonia extent; however, there is a paucity of data regarding the impact of airflow changes. We reviewed 31 COVID-19 patients who were admitted in March 2020 with varying severity of pulmonary disease. Lung volumes were segmented and measured on CT images and patient-specific models of the lungs were created. Incompressible, laminar, and three-dimensional Navier-Stokes equations were used for the fluid dynamics (CFD) analyses of ten patients (five mild, five pneumonia). Of 31 patients, 17 were female, 18 had pneumonia, and 2 were deceased. Effective lung volume decreased in the general group, but the involvement of the right lung was prominent in dyspnea patients. CFD analyses revealed that the mass flow distribution was significantly distorted in pneumonia cases with diminished flow rate towards the right lung. In addition, the distribution of flow parameters showed mild group had less airway resistance with higher velocity (1.228 m/s vs 1.572 m/s) and higher static pressure values at airway branches (1.5112 Pa vs 1.3024 Pa). Therefore, we conclude that airway resistance and mass flow rate distribution are as important as the radiological involvement degree in defining the disease severity., (© 2023. 44. Optimized Conformal Total Body Irradiation with VMAT Using a Linear-Accelerator-Based Radiosurgery Treatment System in Comparison to the Golden Standard Helical TomoTherapy.

Abstract

Modern irradiation techniques for optimized conformal TBI can be realized by Helical Tomotherapy (HT) or Volumetric Modulated Arc Therapy (VMAT), depending on the availability of suitable specialized equipment. In this dosimetric planning study, we compared both modalities and addressed the question of whether VMAT with small field sizes is also suitable as a backup in case of HT equipment malfunctions. For this purpose, we retrospectively used planning computed tomography (CT) data from 10 patients treated with HT with a total dose of 8 Gy (n = 5) or 12 Gy (n = 5) for treatment planning for VMAT with a small field size (36 × 22 cm). The target volume coverage, dose homogeneity at target volume, and dose reduction in organs at risk (OAR) (lungs, kidneys, lenses) were analyzed and compared. One patient was irradiated with both modalities due to a device failure of the HT equipment during the study, which facilitated a comparison in a real clinical setting. The findings indicate that in addition to a higher mean dose to the lenses in the 12 Gy group for VMAT and a better dose homogeneity in the target volume for HT, comparably good and adequate target dose coverage and dose reduction in the other OAR could be achieved for both modalities, with significantly longer treatment times for VMAT. In conclusion, after appropriate optimization of the treatment times, VMAT using linear accelerator radiosurgery technology can be used both as a backup in addition to HT and in clinical routines to perform optimized conformal TBI.

Abstract

Background: Major demographical changes in Germany commenced in the 1960s. Ongoing humanitarian crises in the Ukraine with subsequent immigration will have also long-range effects on national provision of cancer treatment. Ensuring the best possible outcomes for each cancer patient undergoing radiotherapy requires the prediction and prevention of unfavorable side effects. Given that recent research has primarily focused on clinical outcome indicators solely, less is known regarding sociodemographic predictors of therapeutic outcomes, such as patient nationality. Here, we investigated whether the severity of early side effects after radiotherapy are associated with patient nationality and other sociodemographic and clinical characteristics., Methods: Out of 9187 patients treated at a German university medical center between 2017 and 2021, 178 German and 178 non-German patients were selected for matched-pair analysis based on diagnostic and demographic criteria. For all 356 patients, data on side effects from follow-up care after radiotherapy were collected., Results: Non-German patients were more likely to have severe side effects than German patients. Side effect severity was also associated with tumor entity, concomitant therapy, body mass index, and age., Conclusion: Foreign cancer patients are at higher risk of experiencing severe side effects of radiotherapy, suggesting a need to develop and implement targeted preventive measures for these patients. Further research investigating factors predicting the occurrence of radiotherapy side effects, including other sociodemographic characteristics, is needed to better personalize therapy regimens for cancer., (© 2022. Abstract

Purpose: Intraoperative radiation therapy (IORT) is an emerging alternative to adjuvant stereotactic external beam radiation therapy (EBRT) following resection of brain metastases (BM). Advantages of IORT include an instant prevention of tumor regrowth, optimized dose-sparing of adjacent healthy brain tissue and immediate completion of BM treatment, allowing an earlier admission to subsequent systemic treatments. However, prospective outcome data are limited. We sought to assess long-term outcome of IORT in comparison to EBRT., Methods: A total of 35 consecutive patients, prospectively recruited within a study registry, who received IORT following BM resection at a single neuro-oncological center were evaluated for radiation necrosis (RN) incidence rates, local control rates (LCR), distant brain progression (DBP) and overall survival (OS) as long-term outcome parameters. The 1 year-estimated OS and survival rates were compared in a balanced comparative matched-pair analysis to those of our institutional database, encompassing 388 consecutive patients who underwent adjuvant EBRT after BM resection., Results: The median IORT dose was 30 Gy prescribed to the applicator surface. A 2.9% RN rate was observed. The estimated 1 year-LCR was 97.1% and the 1 year-DBP-free survival 73.5%. Median time to DBP was 6.4 (range 1.7-24) months in the subgroup of patients experiencing intracerebral progression. The median OS was 17.5 (0.5-not reached) months with a 1 year-survival rate of 61.3%, which did not not significantly differ from the comparative cohort (p = 0.55 and p = 0.82, respectively)., Conclusion: IORT is a safe and effective fast-track approach following BM resection, with comparable long-term outcomes as adjuvant EBRT., (© 2023. Abstract

Background: Intensity-modulated helical tomotherapy (HT) is a promising technique in preparation for bone marrow transplantation. Nevertheless, radiation-sensitive organs can be substantially compromised due to suboptimal delivery techniques of total body irradiation (TBI). To reduce the potential burden of radiation toxicity to organs at risk (OAR), high-quality coverage and homogeneity are essential. We investigated dosimetric data from kidney, lung and thorax, liver, and spleen in relation to peripheral blood kinetics. To further advance intensity-modulated total body irradiation (TBI), the potential for dose reduction to lung and kidney was considered in the analysis., Patients and Methods: 46 patients undergoing TBI were included in this analysis, partially divided into dose groups (2, 4, 8, and 12 Gy). HT was performed using a rotating gantry to ensuring optimal reduction of radiation to the lungs and kidneys and to provide optimal coverage of other OAR. Common dosimetric parameters, such as D05, D95, and D50, were calculated and analysed. Leukocytes, neutrophils, platelets, creatinine, GFR, haemoglobin, overall survival, and graft-versus-host disease were related to the dosimetric evaluation using statistical tests., Results: The mean D95 of the lung is 48.23%, less than half the prescribed and unreduced dose. The D95 of the chest is almost twice as high at 84.95%. Overall liver coverage values ranged from 96.79% for D95 to 107% for D05. The average dose sparing of all patients analysed resulted in an average D95 of 68.64% in the right kidney and 69.31% in the left kidney. Average D95 in the spleen was 94.28% and D05 was 107.05%. Homogeneity indexes ranged from 1.12 for liver to 2.28 for lung. The additional significance analyses conducted on these blood kinetics showed a significant difference between the 2 Gray group and the other three groups for leukocyte counts. Further statistical comparisons of the dose groups showed no significant differences. However, there were significant changes in the dose of OAR prescribed with dose sparing (e.g., lung vs. rib and kidney)., Conclusion: Using intensity-modulated helical tomotherapy to deliver TBI is a feasible method in preparation for haematopoietic stem cell transplantation. Significant dose sparing in radiosensitive organs such as the lungs and kidneys is achievable with good overall quality of coverage. Abstract

Background: Total body irradiation (TBI) is often a component of the conditioning regimen prior to hematopoietic stem cell transplantation in patients with hematological malignancies. However, total marrow irradiation (TMI) could be an alternative method for reducing radiation therapy-associated toxicity, as it specifically targets the skeleton and thus could better protect organs at risk. Here, we compared dosimetric changes in irradiation received by the target volume and organs at risk between TBI and TMI plans., Materials and Methods: Theoretical TMI plans were calculated for 35 patients with various hematological malignancies who had already received TBI in our clinic. We then statistically compared irradiation doses between the new TMI plans and existing TBI plans. We examined whether TMI provides greater protection of organs at risk while maintaining the prescribed dose in the targeted skeletal area. We also compared beam-on times between TBI and TMI., Results: TMI planning achieved significant reductions in the mean, minimum, and maximum irradiation doses in the lungs, kidneys, liver, spleen, and body (i.e., remaining tissue except organs and skeleton). In particular, the mean dose was reduced by 49% in the liver and spleen and by 55-59% in the kidneys. Moreover, TMI planning reduced the corpus beam-on time by an average of 217 s., Conclusion: TMI planning achieved significant dose reduction in organs at risk while still achieving the prescribed dose in the target volume. Abstract

In 2019, the FLOT4 protocol was established as the new standard for perioperative therapy in patients with locally advanced gastroesophageal and gastric cancer. Whether this protocol is beneficial in a real-world setting remains a question with limited answers to date. In our study, a large cohort of unselected patients treated with FLOT4 was analyzed and compared to protocols based on 5-FU/platinum derivative. This retrospective analysis included patients with locally advanced gastroesophageal and gastric cancer treated with perioperative FLOT or 5-FU/platinum derivative at University Hospital, Bonn between 2010 and 2022 in a curative setting (n = 99). Overall survival, disease-free survival, therapy response and therapy complications were analyzed. Patients treated with FLOT showed a statistically significant longer median overall survival of 57.8 vs 28.9 months (HR: 0.554, 95% CI: 0.317-0.969, P = .036). Moreover, pathological tumor regression (pTR) was significantly higher in the FLOT group compared to the 5-FU/platinum group (P = .001). Subgroup analysis showed a favorable survival benefit for the FLOT vs 5-FU/platinum derivate in patients with AEG and non-signet cell carcinoma. Overall, FLOT was tolerated well but CTCAE ≥3 grade neutropenia and diarrhea occurred more often within the FLOT group. Similar to the prospective phase II/III trials, FLOT4 was the best protocol for patients with locally advanced gastroesophageal and gastric cancer as perioperative therapy in terms of overall survival and pathological response rate compared to 5-FU/platinum derivative protocols. Interestingly, patients with gastroesophageal cancer benefitted more from this therapy. Abstract

Purpose: Fluorouracil, leucovorin, oxaliplatin and docetaxel (FLOT) regimen has shown strong efficacy as perioperative therapy for patients with locally advanced gastric (GC) and gastroesophageal (AEG) carcinoma. In the palliative situation, FLOT is recommended only for young fit patients. Data of efficacy and tolerability of FLOT in elderly patients are scarce and controversial. Thus, this study aimed to provide real-life experience of elderly patients with GC and AEG treated with FLOT as first-line palliative chemotherapy., Methods: Patients with advanced or metastatic GC or AEG and treated with FLOT as first-line palliative therapy between 2010 and 2021 were analyzed. Patients were grouped into < 65 years old (n = 35) and ≥ 65 years old (n = 22) groups. Overall survival (OS), progression-free survival (PFS), feasibility and toxicity were analyzed., Results: The median OS was 10.4 months with no significant difference between both groups (HR 0.86; 95% CI 0.48, 1.57; p = 0.632). The ECOG performance status showed powerful influence on OS in the subgroup analysis with median OS of 12.3 months for ECOG = 0 compared to 5.0 months for ECOG ≥ 1 (p = 0.015) as well as in multivariate analysis (HR 2.62; 95% CI 1.36, 5.04; p = 0.004)., Conclusion: In the present study the ECOG performance status showed a stronger prognostic value than patient age in FLOT as first- line therapy in a real-life cohort with advanced and metastatic GC and AEG. The performance status should therefore be considered in the therapeutic decision making of elderly patients with GC and AEG., (© 2022.
