Objectif : Décrire les lésions tomodensitométriques des cancers du côlon et montrer l’apport de la tomodensitométrie dans la prise en charge des patients atteint de cette affection à Bouaké.Matériels et méthodes: Cette étude rétrospective concernait 32 dossiers de patients atteints de cancer du côlon. Il s’agissait de 21 hommes et de 11 femmes, explorés par une tomodensitométrie abdominopelvienne du 1er Janvier 2014 au 30 Avril 2017 à Bouaké.Résultats : Les principaux signes cliniques étaient les douleurs (30 cas) et les masses abdominales (21 cas).La tomodensitométrie abdomino-pelvienne réalisée objectivait un épaississement pariétal irrégulier court asymétrique colique voir sténosant siégeant au colon gauche (19 cas) puis au colon droit (10 cas) et au transverse (3 cas). Elle retrouvait en plus des infiltrations tumorales locorégionales (23 cas), des adénopathies abdominales profondes (12 cas) et des métastases (6 cas) ; ce qui avaient permis de stadifier ces lésions selon la classification TNM ( T = tumeur primaire ,N = atteinte éventuelle des ganglions lymphatiques « Nodes » en anglais, M = présence éventuelle de métastases). Ainsi on notait 10 cas au stade I / II ,16 cas au stade III et 6 cas au stade IV.Un cancer du côlon avait été diagnostiqué chez tous les patients après un examen anatomo-pathologique des pièces opératoires ou des biopsies au cours des coloscopies réalisées hors de Bouaké.Conclusion : Le scanner constitue un moyen d’imagerie incontournable pour l’exploration des cancers coliques, en absence de la coloscopie. Il permet de localiser les lésions coliques et leur extension pour établir le stade évolutif afin faire un choix thérapeutique. Objective: To describe the lesions of colon cancers and to show the contribution of CT scans in the management of patients in Bouake.Materials and methods: This retrospective study is based on 32 cases of patients with colic carcinoma. They were 21 men and 11 women, subjected to Tummy CT scan from January 1, 2014, to April 30, 2017, in Bouake.Results: The main signs were pain (30 cases) and abdominal masses (21 cases). The abdominal-pelvic CT scan revealed a short asymmetrical or irregular colic parietal thickening, even stenosant residing in the left (19 cases), right (10 cases) colons and the transverse (3 cases). In addition, locoregional tumor infiltrations (23 cases), deep abdominal adenopathy (12 cases) and metastases (6 cases) were found. These findings allowed the identification of the lesions according to the TNM classification (T = primary tumor, N =possible involvement of the lymph nodes, M = possible presence of metastases). Thus, there were 10 cases in stage I / II, 16 cases in stage III, and 6 cases in stage IV.Carcinoma had been confirmed in all patients after an anatomo-pathological examination of the operating parts or biopsies during colonoscopies performed outside of Bouake.Conclusion: In the absence of colonoscopy, the scanner is an excellent imaging tool for the exploration of colic cancers. It can detect and locate colic and extra colic lesions and then determine their stage in order to make an appropriate therapeutic decision.

Les auteurs rapportent le cas d’un corps étranger introduit accidentellement par une élève de 16 ans par voie vaginale lors d’une chute de sa hauteur.Les multiples tentatives de retrait du corps étranger par la patiente à domicile puis par le personnel médical auraient favorisées sa disparition totale dans l’organisme.L’imagerie médicale (ASP, ECHOGRAPHIE et TDM ABDOMINALES) a confirmé le diagnostic évoqué par la clinique, et la topographie pelvienne du corps étranger dans le pelvis.Une laparotomie réalisée d’urgence a permis l’extraction du corps étranger qui était un stylo bille avec son capuchon situé dans sa longueur en para-vésicale droite.Les suites post-opératoires étaient simples après 3 jours d’hospitalisation avec antalgique, antibiothérapie et drainage.L’évolution fut favorable.L’intérêt de cette observation réside dans le caractère insolite du corps étranger, sa gravité, sa survenue chez un sujet jeune et le rôle de l’imagerie dans sa prise en charge. The authors report the case of a foreign body accidentally introduced by a 16-year-old vaginal student when she falls from her height. Multiple attempts to be removed by the patient and then by medical personnel would have contributed to the total disappearance of the foreign body in the body. Medical imaging (ASP, ECHOGRAPHIE and ABDOMINAL CT) confirmed the diagnosis evoked by the clinic, and the pelvic topography of the foreign body.An emergency laparotomy allowed the extraction of the foreign body which was a pen with its cap located in its length in straight para-vesical.Post-operative suites were simple after 3 days of hospitalization with painkiller, antibiotic and drainage. The evolution was favorable. The interest of this observation lies in the unusual temper of the foreign body, its gravity, its occurrence in a young subject and the role of imaging in its management.

Objectif : Décrire les aspects épidémio-cliniques et tomodensitométriques des lésions crânio-encéphaliques des accidentés de la voie publique par engins à deux roues à Bouaké.Patients et méthodes : Etude transversale descriptive réalisée du 1er Janvier 2015 au 31 Octobre 2016 dans deux services d’imagerie médicale de Bouaké. Elle a inclus des traumatisés crâniens documentés par un scanner crânio-encéphalique sans injection de produit de contraste. Les variables étudiées portaient sur les aspects épidémio-cliniques et tomodensitométriques.Résultats: Cent cinquante patients étaient inclus dont 139 hommes. L’âge moyen était de 30.8 ans (2 et 83 ans). Dans 32% des cas l’âge était compris entre 21 et 30 ans. L’accident impliquait une moto dans 95,3% des cas dans un mécanisme moto contre moto. La victime était conductrice et passager de la moto respectivement dans 74 et 14% des cas. La victime portait un casque de sécurité dans 6,7% des cas. La tomodensitométrie a révélé des lésions cérébrales parenchymateuses cérébrales et extra-parenchymateuses (67%), osseuses (25%), du scalp (8%). Les lésions cérébrales parenchymateuses et extra-parenchymateuses étaient dominées respectivement par la contusion oedémato-hémorragique (68,4%) et l’hématome sous-dural frontal (37,6%). Le siège de la fracture était temporo-pariétal gauche dans 34,6%.Conclusion : Les accidentés de la voie publique impliquant les engins à deux roues motrices sont en majorité des hommes jeunes portant rarement un casque de sécurité. Le mécanisme moto-moto prédomine et la contusion oedémato-hémorragique était la lésion la plus fréquente. La prévention parr le port de casque de sécurité et le respect du code de sécurité routière doit être renforcée. Objective: To describe the epidemiological, clinical and computed tomography aspects of cranio-encephalic lesions of road accident victims by two-wheeled vehicles in BouakéPatients and methods: Cross-sectional descriptive study carried out from January 1, 2015 to October 31, 2016 in two medical imaging departments in Bouaké. It included traumatic brain injuries proved on non-enhanced ct-scan. The studied variables was epidemiological, clinical and ct-scan lesions.Results: One hundred and fifty patients were included, among which 139 men. The mean age was 30.8 years (extremes: 2 to 83 years); 32% of patients was aged between 21 and 30 years. The accident involved a motorcycle in 95.3% of the cases in a motor cycle against motor cycle mechanism. The victim was the driver or passenger of the motor cycle in 74 and 14% of the cases, respectively. The victim was wearing a safety helmet in 6.7% of the cases. Computed tomography revealed cerebral and extra-parenchymal (67%), bony (25%), scalp (8%) brain lesions. Parenchymal and extra-parenchymal brain lesions were dominated by hemorrhagic contusion (68.4%) and frontal subdural hematoma (37.6%), respectively. The site of the fracture was left-temporo-parietal in 34.6%.Conclusion: Road traffic accident victims involving two-wheel drive vehicles are mostly young men who rarely wear a safety helmet. The motorbike-motorbike mechanism accident and the hemorrhagic brain contusion was mostly observed. Prevention based on wearing a safety helmet and respecting the road safety code should be observed.
