3. Ethics and the use of generative AI in professional editing

Abstract

Generative artificial intelligence (GnAI) has garnered significant attention worldwide across diverse industries, including in book publishing. To date, more attention has been paid to its potential in creative collaboration and less to the editorial possibilities of its application. Interest has accelerated since the breakthrough of a new Large Language Model in late 2022. This paper engages with the ethical and industrial implications of using GnAI in a creative context, namely literary publishing. It raises crucial questions about intellectual property, trust, the author–editor relationship and publishing professionals’ evolving roles in shaping quality literature. Using a published story as a test case, we compare edits using GnAI with those by professional editors over multiple drafts and at different stages of editorial development. We consider the potential ethical implications of the use of GnAI in literary fiction editing, highlighting the principles and practices that underpin professional editing to consider how these may or may not translate in the use of GnAI. This is followed by a discussion of the risks and opportunities in using GnAI in editing literary texts in the trade publishing context.

Abstract

Objectives Magnetic resonance venography (MRV) is underutilized in the evaluation of thrombus properties prior to endovascular treatment but may improve procedural outcomes. We therefore investigated the clinical impact of using a dedicated MRV scoring system to assess thrombus characteristics prior to endovascular intervention for iliofemoral deep vein thrombosis (DVT). Methods This is a post hoc analysis of data from the CAVA trial (Clinicaltrials.gov:NCT00970619). MRV studies of patients receiving ultrasound-accelerated catheter-directed thrombolysis (CDT) for iliofemoral DVT were reviewed. Thrombus age-related imaging characteristics were scored and translated into an overall score (acute, subacute, or old). MRV scores were compared to patient-reported complaints. MRV-scored groups were compared for CDT duration and success rate. Results Fifty-six patients (29 men; age 50.8 ± 16.4 years) were included. Using MRV, 27 thrombi were classified acute, 17 subacute, and 12 old. Based on patient-reported complaints, 11 (91.7%) of these old thrombi would have been categorized acute or subacute, and one (3.7%) of the acute thrombi as old. Average duration of CDT to > 90% restored patency differed significantly between groups (p < 0.0001): average duration was 23 h for acute thromboses (range: 19–25), 43 h for subacute (range: 41–62), and 85 h for old thromboses (range: 74–96). CDT was almost eleven times more successful in thromboses characterized as acute and subacute compared to old thromboses (OR: 10.7; 95% CI 2.1–55.5). Conclusion A dedicated MRV scoring system can safely discriminate between acute, subacute, and old thromboses. MRV-based selection is predictive of procedural duration and success rate and can help avoid unnecessary complications. Key Points • Thrombus age, characterized by MRV as acute, subacute, and old, can predict CDT duration and probability of success. • Accurate pre-interventional MRV-based thrombus aging has the potential to facilitate identification of eligible patients and may thus prevent CDT-related complications.

Introducción: El desarrollo y el impacto social generado en los parques de la ciudad de Barranquilla ha despertado gran interés, pues ha sido un mecanismo para propiciar el desarrollo social y el cambio en la dinámica de la economía local. Por ende, Objetivos: analizar las políticas publicas aplicadas a los parques en la ciudad de Barranquilla que influyen en el desarrollo económico y social. Metodología: Para llevar a cabo este análisis, se empleó una metodología cuyo enfoque hipotético-deductivo de naturaleza transversal, bajo la técnica del análisis documental y la entrevista no estructurada, Resultados: se evidencio que las políticas actuales han permitido generar mas empleo y reducir la delincuencia en la ciudad, aspectos fundamentales para el progreso social. Conclusiones: la inversión continua en el desarrollo y mejora de los parques, representa un medio efectivo para fomentar el crecimiento sostenible y el progreso social en Barranquilla.

Abstract

Background: Nutrition and nutritional care are essential for optimal outcomes, and, therefore of importance for patients with chronic limb threatening ischemia (CLTI) given their high risk of complications. However, insight is lacking in how healthcare professionals directly involved in the care of patients with CLTI perceive nutritional care, as well as in the perceived barriers and facilitators regarding optimal nutritional care., Methods: In this qualitative study with a phenomenological approach, 3 online focus groups were conducted with various healthcare professionals directly involved in the care of patients with CLTI. Sample size was guided by information power. Focus group recordings were transcribed verbatim, and reflexive thematic analysis was performed., Results: Seventeen healthcare professionals participated, including vascular surgeons, fellows in vascular surgery, a medical doctor and researcher, nurse specialized in wound care, general nurse, physical therapists, dietitians, and nutrition assistants. Four themes were generated: (1) nutritional care is crucial for optimal clinical outcomes and a healthy life, (2) insufficient attention to undernutrition and nutritional care by healthcare professionals, (3) patient-related factors challenge healthcare professionals in providing nutritional care, and (4) need for optimizing the organizational process related to nutritional care. Perceived barriers regarding nutritional care included knowledge deficits, nutritional care not being part of the healthcare professionals' routine, missing tools to identify undernutrition, patient-related factors, and time constraints. Facilitators regarding nutritional care included more scientific evidence regarding the effect of nutritional care on clinical outcomes and optimization of organizational processes related to nutritional care., Conclusions: Healthcare professionals perceive nutritional care as important for optimal outcomes, but nutritional care is not routinely implemented in the care of patients with CLTI. This lack of implementation of nutritional care may be due to the barriers perceived in various domains.
