- Abstract
Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. Results confirm the presence of language production deficits already at the first episode of the illness, being such impairment not related to other cognitive domains. The high accuracy obtained by the linguistic set of features in classifying groups support the use of machine learning methods in neuroscience investigations.

The need to satisfy the world's ever-increasing demand for a low carbon, secure and affordable electricity and energy source is one of the greatest challenges of this century[1]. Nuclear fusion technology is aiming to join the family of non-greenhouse gas emitting electricity sources, currently consisting of renewables and nuclear fission power plants. For the realization of the first commercial fusion reactor, multiple challenges are yet to be solved, but the technological transfer from 60+ years operational experience of fission power plants, including materials research, cooling circuit optimisation and corrosion mitigation strategies, can be key to ensure the success of fusion reactors. This is especially the case as fusion reactors are expected to use similar coolants and structural materials as the current light water reactors fleet (LWRs). The challenges faced by materials in nuclear power plant cooling circuits are very unique. Materials need to maintain their mechanical properties intact for 40+ years (and even 60+ or 80+ years for current fission power plants) while operating at temperature, around 300°C for the current LWR fleet, up to 530°C for the UK's Advanced Gas cooled Reactors (AGRs) and up to 1000°C for future designs of Very High Temperature Reactors (VHTR), under neutron irradiation (up to 100 dpa) and in a corrosive environment. In the past, nuclear fission plants have exploited various coolants for research purposes or for commercial purposes (energy and electricity production): gases, liquid metals and molten salts, although water is by far the most common in current designs. The Generation IV roadmap for advanced reactors anticipates the use of liquid metals (lead & lead/bismuth eutectic or sodium), gases (helium), water and molten salts (mainly fluorides) as coolants [2]. Potential fusion reactor coolants are quite similar, with water, gases (helium) and liquid metals (lithium, lead/lithium) [3] under consideration. Notably, two types of innovative fission reactors are already in operation in the world: the high temperature gas (helium) cooled reactor-pebble bed module (HTR-PM) in China and the sodium fast reactors Beloyarsk Nuclear Power Station (BN-600 and BN-800) in Russia....

Innovations in light technologies (i.e. Light Emitting Diodes; LED) and cover films with specific optical features (e.g. photo-selective, light-extracting) have revolutionized crop production in both protected environments and open fields. The possibility to modulate the light spectra, thereby enriching/depleting cultivated plants with targeted wavebands has attracted increasing interest from both basic and applicative research. Indeed, the light environment not only influences plant biomass production but is also a pivotal factor in shaping plant size, development and metabolism. In the last decade, the strict interdependence between specific wavebands and the accumulation of targeted secondary metabolites has been exploited to improve the quality of horticultural products. Innovation in LED lighting has also marked the improvement of streetlamp illumination, thereby posing new questions about the possible influence of light pollution on urban tree metabolism. In this case, it is urgent and challenging to propose new, less-impacting solutions by modulating streetlamp spectra in order to preserve the ecosystem services provided by urban trees. The present review critically summarizes the main recent findings related to the morpho-anatomical, physiological, and biochemical changes induced by light spectra management via different techniques in crops as well as in non-cultivated species. This review explores the following topics: (1) plant growth in monochromatic environments, (2) the use of greenhouse light supplementation, (3) the application of covering films with different properties, and (4) the drawbacks of streetlamp illumination on urban trees. When plants are transferred from nursery to urban environments, they often face drought stress due to inadequate maintenance, such as insufficient irrigation. Using drought tolerant species may help mitigate the adverse impact of drought stress in urban settings. Additionally, utilizing novel technologies for water status monitoring may help optimize irrigation schedules to prevent transplanting failures. This study investigated the physiological and biochemical responses of two ornamental shrubs, Photinia x fraseri and Viburnum tinus, subjected to water stress of increasing severity and rewatering. Water relations, gas exchanges, chlorophyll fluorescence and biochemical analyses were conducted alongside real-time monitoring of water status using leaf-water-meter sensors (LWM). The progression of water stress had a notable negative impact on leaf gas exchanges and water relations in both species. Notably, P. fraseri avoided photoinhibition by reducing chlorophyll content and actual efficiency of PSII. Adjustments in leaf phenolic compounds played a significant role in enhancing drought tolerance of both species due to their antioxidant and photoprotective properties. Upon rewatering, both species exhibited complete recovery in their physiological functions, underscoring their remarkable tolerance and resilience to drought stress. Additionally, LWM sensors efficiently tracked the dehydration levels, exhibiting a rising trend during the water stress progression and a subsequent decline after rewatering for both species. Objectives: This TRIPLE study was aimed to evaluate the efficacy of polycarbophil vaginal gel (PCV) in treating symptoms of vaginal atrophy (VA) of peri- and post-menopausal women., Materials and Methods: Sexually active women in peri- (n = 29) and post-menopause (n = 54) suffering from VA, were progressively enrolled and treated for 30 days with PCV. Those wishing to continue (n = 73) were treated for additional 180 days. PCV was administered as one application twice a week. The vaginal health index (VHI; range 5 to 25) and the visual analogue score (VAS range for 0 to 100 mm for each item) for vaginal dryness, irritation, and pain at intercourse, along with the global symptoms score (GSS; range 1 to 15) and treatment safety, were evaluated at baseline and after 30 days. In those continuing the treatment an evaluation was performed after additional 180 days., Results: Women in peri and post-menopause were of 48.7 ± 3.3 years and 57.5 ± 5.7 years old., respectively. At baseline all outcomes were significantly worse (p<0.002) in postmenopausal group, except the VHI (p < 0.056). After 30 days VHI increased (p < 0.001) of 4.1 ± 0.5 (mean ± SE), and 5.1 ± 0.4 in peri- and post-menopausal women respectively. VAS of vaginal dryness decreased (p < 0.001) of -24.4 ± 3.6, and -52.7 ± 2.6 (p < 0.001), VAS of irritation decreased (p<0.001) of -18.6 ± 4.4 and -47.8 ± 3.2, VAS of pain decreased (p < 0.001) of -26.2 ± 4.3 and -55.6 ± 3.1 and the GSS decreased (p < 0.001) of -3.9 ± 0.3, and -4.9 ± 0.2, in peri and post-menopausal women, respectively. All the modifications were significantly greater (p < 0.001)(p < 0.032 for GSS) in postmenopausal women, and after 30 days all outcomes were similar in the two groups of women. In comparison to baseline, after 210 days of treatment VHI increased of 7.7 ± 0.3 (p < 0.001), VAS of vaginal dryness decreased of -53.6 ± 1.9 (p < 0.001) VAS of irritation of -42.6 ± 1.4 (p < 0.001) VAS of pain of -46.7 ± 2.3 (p < 0.001) and the GSS of -6.5 ± 0.2 ± 0.2 (p < 0.001). All outcomes improved (p < 0.001) over the values observed after 30 days of treatment (p < 0.001). Objectives: Nausea and vomiting in pregnancy is a common and invalidating condition in early pregnancy. However, no data are available on its prevalence in Italy. This survey aims to evaluate the prevalence and impact of nausea and vomiting during pregnancy on the quality of life of Italian women., Study Design: The survey was performed in three Italian public University Hospitals in two distinct periods: a first interview took place between the 18th and 22nd week of pregnancy, using the Questionnaire for Pregnancy Period (14 questions regarding demographic data and 30 questions about nausea and vomiting in pregnancy, including Pregnancy-Unique Quantification of Emesis questionnaire), and a follow-up interview, by telephone call, took place immediately after giving birth and in any case within 14 days of delivery, using the Questionnaire for Post-Pregnancy (9 questions). Included women were Caucasian, in physiological pregnancy and between the 18th and 22nd week (time of morphological ultrasound), able to communicate adequately with the interviewer, understand the questionnaires and able to provide valid informed consent. Twin pregnancies and women who recurred to medically assisted procreation were excluded. This is an interim report on data collected from 232 of the planned 600 women., Results: Mean age of the recruited subjects was 32.6 ± 4.6 years, with approximately 60% primiparous. The prevalence of nausea and vomiting in pregnancy in the sample examined was 65.5% overall (152 out of 232 subjects). Of these 152 women, 63 (41.4%) experienced only nausea, 6 (3.9%) only vomiting, and 83 (54.6%) reported both. Symptoms were reported to begin at 7.2 ± 2.7 weeks, lasted 10.2 ± 5.6 weeks, and persisted at the time of the interview in 32.2% of cases. Overall, over 50% of the women interviewed experienced a negative impact of nausea and vomiting in pregnancy on social relationships and work activity., Conclusions: A high prevalence of nausea and vomiting in pregnancy, 65.5% overall, was found in this interim analysis. These symptoms appeared capable of negatively influencing women quality of life. - Abstract
The paper presents the skeletal remains of an adult male of 30-40 years with bone lesions and deformity on the left hip, recovered during the archaeological excavation below the former Basilica of San Dionigi, dated to Late Antiquity (3
rd - 5th century AD) Milan. Biological profile and paleopathological analysis were performed following standard references and the bones underwent radiological examination. Differential diagnosis included congenital anomaly, active rickets, infectious diseases, femoral neck trauma, Legg-Carvé-Perthes disease, Slipped Capital Femoral Epiphysis (SFCE), osteogenesis imperfecta and osteoporosis. While the lesions were highly consistent with Legg-Calvé-Perthes disease, they were typical of SFCE, complicated by an avascular necrosis and secondary osteoarthrosis. The alteration of the femoral head led to a 7.8% leg-length discrepancy, causing gait alteration with partial compensation though increased muscular activity on the right leg, reduced mobility of the joint and potentially hip pain, difficulty in walking and running and even limping. This paper explores a case of physical disability from the Roman era found near a Christian place of worship and represents a rare case of SFCE in the paleopathological record.- Published

19. Biochar as a soil amendment in the tree establishment phase: What are the consequences for tree physiology, soil quality and carbon sequestration?

- Abstract
Trees play a pivotal role in the urban environment alleviating the negative impacts of urbanization, and for this reason, local governments have promoted strongly tree planting policies. However, poor soil quality and neglect tree maintenance (e.g., irrigation and fertilization) can seriously mine the plant health status during the tree establishment phase. The use of biochar to provide long-lasting C to the soil and, at the same time, improving soil properties (e.g., improved water holding capacity), soil enzymes activities and NPK concentrations, is a promising research field. Therefore, with a two-step experiment, the study aimed to assay the physiological responses of a commonly used urban tree species (Tilia × europaea L.) to 1.5 % (w/w) biochar amendment (B), and secondly, to assess the ability of trees, grown in biochar amended soil, to tolerate a period of drought. Biochar amendment increased P and K availability in the soil, resulting in higher P and K concentrations in B than control leaves, according to the leaf stage. This induced B trees, higher values in both total biomass than controls (+22 %) in well-watered plants. Moreover, the higher water availability in soil amended with biochar helped B trees to tolerate water stress, with better leaf photosynthetic performances and a faster recovery than stressed controls after the re-watering. This study highlights the dual function of the biochar, improving CO
2 sequestration and soil properties, and at the same time, enhancing plant physiological responses to environmental constraints. - Abstract
Purpose: To evaluate the feasibility and early outcomes of below the ankle orbital atherectomy in chronic limb-threatening ischemia in patients with calcified foot arteries., Methods: 12 patients (mean age 69.4 ± 14.7; range 57 to 85 years) who were affected by diabetes underwent orbital atherectomy below the knee and ankle arterial segments. Technical success was defined as orbital atherectomy passage and debulked the calcified lesion, delivery of adjunctive therapy, and < 30% residual stenosis at final angiogram. The procedural outcome included complications, amputation-free survival, and freedom from clinically-driven target lesion revascularization at 30-days and 6-months of follow-up., Results: Orbital atherectomy was performed in 3 cases in Anterior tibial (AT) and dorsalis pedis (Ped) arteries + Posterior tibial (PT) and Lateral Plantar (Lat Plan), 5 cases in PT and Lat Plan arteries, 1 case of PT and Medial Plantar, 1 case of Peroneal and Plan Lat, and 2 cases of AT and Ped. After atherectomy, we used a drug-coated balloon (DCB) angioplasty. Technical success was achieved in 11 (91.6%) cases. No deaths were registered during the follow-up. The limb salvage rate was 100%, and no major amputations were registered. Amputation-free survival was 50%. Freedom from CD-TLR was 100% at 30 days and 91.7% at 6-months. One patient underwent a TLR at three months. No major cardiovascular events, limb events, or significant procedure-related complications were registered., Conclusions: CSI orbital atherectomy and DCB angioplasty appear a feasible and promissor treatment options in diabetic CLTI patients., Competing Interests: Declaration of competing interest Dr. Palena is consultant for CSA and BD. Dysregulations of lipid metabolism in the liver may trigger steatosis progression, leading to potentially severe clinical consequences such as nonalcoholic fatty liver diseases (NAFLDs). Molecular mechanisms underlying liver lipogenesis are very complex and fine-tuned by chromatin dynamics and multiple key transcription factors. Here, we demonstrate that the nuclear factor HMGB1 acts as a strong repressor of liver lipogenesis. Mice with liver-specific Hmgb1 deficiency display exacerbated liver steatosis, while Hmgb1 -overexpressing mice exhibited a protection from fatty liver progression when subjected to nutritional stress. Global transcriptome and functional analysis revealed that the deletion of Hmgb1 gene enhances LXRα and PPARγ activity. HMGB1 repression is not mediated through nucleosome landscape reorganization but rather via a preferential DNA occupation in a region carrying genes regulated by LXRα and PPARγ. Together, these findings suggest that hepatocellular HMGB1 protects from liver steatosis development. HMGB1 may constitute a new attractive option to therapeutically target the LXRα-PPARγ axis during NAFLD.

- Abstract
Background: COVID-19 pandemic has impacted professional, economic and social activities. In the surgical field, it has brought changes to operating activities, the organization of workforces, the protection measures for patients and personnel against possible intraoperative transmissions as well as training young surgeons. This study intends to assess the extent of this impact in our institution., Methods: The patients operated on in nine Operating Units (OUs) in the period February 1 - March 31, 2020, with follow-ups on April 30, 2020, were evaluated both retrospectively and prospectively. Organizational, clinical and impact parameters on staff were evaluated., Results: Of the 833 consecutive admitted patients, 742 were operated on, 705 of whom were recruited for the study. Compared to the same period in 2019 there was a decrease in the number of operations (742 compared to 1187), similar use of intensive care unit (ICU), a diagnostic activity only for symptomatic patients, heterogeneity in organizational behaviors, an impact on staff who highlighted concerns about getting sick or passing the infection on to others (87.64%) or their family members (75.14%)., Conclusions: The present study made it possible to detect the need to make significant changes in the clinical, organizational and teaching fields, for which some operational proposals are suggested.
