Previous studies have demonstrated changes in lumbar multifidus muscle (LM) contractility after motor control exercises (MCE), and it has been hypothesized that adding neuromuscular electrical stimulation (NMES) may help to re-activate motor units. To present the effects of combined NMES and MCE on LM contractility, spatial and temporal motor unit recruitment, and movement control in a patient with recurrent low back pain (rLBP). Motion tracking system was used to measure quality of movement (smoothness) during an active forward bend, while ultrasound imaging and decomposition electromyography were used to measure the LM contractility and motor unit recruitment during the Sorensen test. These data were collected pre and post intervention. Perceived improvement was also recorded. Improved movement smoothness post intervention was found, with increases in LM contractility from 68.1% to 97.7%, and from 74.2% to 86.7% on the right and left sides, respectively. Number of motor unit increased from 14 to 18 units, while mean firing rate decreased from 10.9 to 7.1 pulses/second post intervention. The patient also reported a perceived improvement of +2 on the Global Rating of Change (GROC). However, this change was not greater than +3 to be considered as minimal clinically important difference. These findings indicate improvements in movement control, LM contractility, and changes in spatial and temporal motor unit recruitment in the study patient, suggesting the potential clinical utility and the need for further research on combined NMES and MCE in the treatment of patients with rLBP. Background and Objectives: Practitioners of martial arts such as Taekwondo are likelier to experience back pain during training or competition. As the back pain of taekwondo athletes shows various symptoms depending on the athlete's characteristics, such as technique and movement, a case study was conducted to verify the intervention effect suitable for individual traits. We examined the effects of a complex pain control program on pain, mechanosensitivity, and physical function in a Taekwondo athlete with recurrent low back pain (LBP). Materials and Methods: A Taekwondo athlete with LBP was recruited from D University, Busan. The intervention program was performed for 45 min twice a week for 3 weeks, and the patient was followed up with after 2 weeks. The numerical rating pain scale (NRPS), pain pressure threshold, mechanosensitivity, and Oswestry Disability Index (ODI) scores were measured before and after the intervention. Therapeutic massage and nerve stimulation therapy were performed. Lumbar flexion, extension, and rotation were performed in the movement control exercise group, whereas the sliding technique, a neurodynamic technique of the tibial nerve, was applied in the neurodynamic technique group. This effect was verified by comparing the average measured values before and after the intervention. Results: Pain (NRPS) and mechanosensitivity reduced, range of motion and tactile discrimination abilities improved, and physical function (ODI) improved. The effect of the improved intervention lasted 2 weeks. Conclusions: These results indicate that application of complex pain control programs considering the four aspects of pain mechanisms for 3 weeks can be an effective intervention in Taekwondo athletes with recurrent LBP. Although postural control measures were reported to identify neuromuscular impairments, postural steadiness and stabilization time were not carefully investigated between subjects with and without recurrent low back pain (LBP). Research QuestionAre there group differences in the stabilization time, direction of sway, and dynamic postural steadiness index (DPSI) during one-leg standing? Thirty-four control subjects and 29 subjects with recurrent LBP participated in the study. Each subject stood upright on a single leg with and without visual input. The outcomes were measured for standing duration (sec), direction of sway, and the DPSI, which included the vertical steadiness index (VSI). The VSI assesses fluctuations to standardize the vertical ground reaction forces on the force plate. The control group demonstrated significantly longer standing duration compared to the LBP group during the eyes-open condition (t = 3.55, p = 0.001). The LBP group demonstrated significantly faster stabilization time (t = 2.53, p = 0.01) in the sagittal plane. The DPSI demonstrated an excellent relationship with the VSI without visual input in the control group (r = 0.98, p = 0.001). The directions of sway demonstrated a significant interaction between groups (F = 9.29, p = 0.004). Although standing duration in the eyes-open condition decreased in the LBP group, a faster stabilization time in the sagittal plane was evident compared to the control group to adapt postural stability. These results indicated that vertical dynamic steadiness with visual input might be important to enhance compensatory postural control. • The low back pain group demonstrated faster stabilization in the sagittal plane. • The groups demonstrated a significant interaction with sway directions. • Vertical standing steadiness was enhanced with a longer standing duration. Background: Recurrent low back pain (RLBP) affects different structures in the lumbar spine. Exercise therapy is highly recommended as one of the first-line treatments. One crucial variable introduced to enhance RLBP is the external focus. The present study aimed to investigate the effects of external focus training on pain, the thickness of transverse abdominis (TrA), internal oblique (IO), external oblique (EO), and lumbar multifidus (LM) muscles, kinesiophobia, fear-avoidance beliefs, and disability of people with RLBP., Methods: This randomized-controlled trial consisted of 38 RLBP patients. Patients were randomly divided into two groups the treatment group (n=19) that received external focus training in addition to motor control training and the control group (n=19) that received motor control training alone. The primary outcome was pain intensity, and secondary outcomes were the thickness of TrA, IO, EO, LM muscles, kinesiophobia, fear-avoidance beliefs, and disability that were measured at the baseline and after 16 sessions of interventions. The interventions were performed three sessions weekly., Results: Reduction in pain intensity was more significant in the intervention group than in the control group ( P<0.001 , Cohen's d =-1.47). The thickness of TrA muscle in the contraction condition of the intervention group was significantly more on the left side ( P < 0.001 , Cohen's d =1.05) than on the right side ( P=0.03 , Cohen's d =0.44). Other outcomes showed no significant differences. However, the Cohen's d effect size for the left IO (Cohen's d =0.57) and TKS (Cohen's d =-0.53) were moderate., Conclusion: In RLBP patients, external focus and motor control training could effectively reduce the pain. Although this intervention could increase the thickness of the TrA muscle of RLBP, it has no significant effect on the thickness of IO, EO, and LM muscles. In addition, the obtained results indicated that this intervention has no significant effect on kinesiophobia, fear-avoidance beliefs, and disability . ., Competing Interests: None.
