Introduction Renal disease is commonly suspected in patients with proteinuria. Renal biopsy might be considered based on the patient's clinical history and the results of diagnostic tests. However, as orthostatic proteinuria is benign and requires no treatment, it is important to obtain a diagnosis without renal biopsy whenever possible. Therefore, up to now, for the diagnosis of orthostatic proteinuria, in addition to resting urinalysis evaluation (disappearance of proteinuria), we have performed the lordosis load test and urine biochemistry of the samples showing peak proteinuria in a lordosis load test. Method We retrospectively enrolled all patients who visited the pediatric department and underwent the lordosis load test at Kanazawa Medical Center between 2011 and 2020. In the present study, samples with the highest concentrations of protein after the lordosis load test were subjected to general urinary biochemistry and urinary sediment analysis. Patients were followed up with the lordosis load test for several years. Results Enrolled in the study were 68 patients with OP. The mean age at the time of diagnosis was 11.5 years (range, six to 16 years). General urinary tests, urinary sediment, and urinary biochemistry including N-acetyl-beta-D-glucosaminidase (NAG), alpha1-microglobulin(α1MG), and beta 2-microglobulin (β2MG) were normal in all patients with orthostatic proteinuria except one case who was a premature baby. Conclusion If proteinuria disappears after two hours of rest, and urinary biochemistry of the samples showing peak proteinuria in the lordosis load test is normal, orthostatic proteinuria can be diagnosed more accurately., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. Background: Urinary levels of N-acetyl-β-D-glucosaminidase (NAG), α1-microglobulin (α1-MG), and β2-microglobulin (β2-MG) are measured as markers of renal tubular damage. We previously determined normal values for these urine biochemical examinations in healthy children over 3 years old. However, the values are not applicable to children younger than 2 years old, and children less than 1 year old, in particular, seem to show very high levels for all these markers. Hence, as normal values for children below 2 years old remain unclear, we determined the normal values for urinary biochemical markers in this age group., Material and Methods: Fresh urine samples were obtained from 293 healthy children (from newborns to 2-year-old children). All the samples were subjected to normal urinalysis. NAG, α1-MG, β2-MG, and creatinine (Cr) levels in extracted samples were measured immediately in the central laboratory at Kanazawa Medical Center., Results: The normal values for each biomarker in children below 2 years of age were determined. Additionally, urinary α1-MG levels were observed to decrease most rapidly with age, almost reaching the level at ≥ 3 years by 6 months after birth., Conclusion: Renal tubular function can be evaluated in children < 3 years old using the normal values. Many female mammals have recurring cycles of ovulation and sexual behaviors that are regulated by reproductive hormones and confer reproductive success. In addition to sexual behaviors, circadian behavioral rhythms of locomotor activity also fluctuate across the estrous cycle in rodents. Moreover, there is a bidirectional relationship between circadian rhythms and estrous cyclicity since mice with disrupted circadian rhythms also have compromised estrous cycles resulting in fewer pregnancies. In the present study, we assessed whether extending day length, which alters circadian rhythms, normalizes estrous cyclicity in mice. We found that Period ( Per ) 1 / 2 / 3 triple knockout (KO) mice, that have disabled canonical molecular circadian clocks, have markedly disrupted estrous cycles. Surprisingly, extending the day length by only 2 h per day restored regular 4- or 5-day estrous cycles to Per1 / 2 / 3 KO mice. Longer days also induced consistent 4-day, rather than 5-day, estrous cycles in wild-type C57BL/6J mice. Interstitial microdeletions in the long arm of chromosome 3 are rare. In this study, we identified two patients with approximately 5-Mb overlapping deletions in the 3q26.2q26.31 region. Both patients showed neurodevelopmental delays, congenital heart defects, and distinctive facial features. One of them showed growth deficiency and brain abnormalities, as shown on a magnetic resonance imaging scan. Haploinsufficiency of NLGN1 and FNDC3B present in the common deletion region was considered to be responsible for neurodevelopmental delay and the distinctive features, respectively. The possibility of unmasked variants in PLD1 was considered and analyzed, but no possible pathogenic variant was found, and the mechanism of the congenital heart defects observed in the patients is unknown. Because 3q26.2q26.31 deletions are rare, more information is required to establish genotype-phenotype correlations associated with microdeletions in this region., (© 2022 Wiley Periodicals LLC.)
