Abstract

Owing to the geometrical frustration, Mn$_3 $Sn exhibits a 120$^{\circ}$ in-plane triangular antiferromagnetic (AFM) order with a large anomalous Hall effect (AHE). Here, we present a combined theoretical and experimental study to demonstrate that the in-plane AFM structure in Mn$_3 $Sn can be significantly modified to a tunable noncoplanar magnetic state by suitable electron doping. With the help of Density Functional Theory calculations we show that the presence of higher-order exchange interactions in the system leads to the stabilization of the noncoplanar magnetic ground state, which is further established by neutron diffraction study in the Fe-doped Mn$_3 $Sn sample. Interestingly, we find a large scalar spin chirality (SSC) induced topological AHE that can be significantly tuned with the degree of non-coplanarity. We carry out 60~T magnetic and Hall resistivity measurements to demonstrate the contribution of SSC to the observed AHE. We also illustrate a simultaneous manipulation of dual order in the system, where the AHE arising from the in-plane 120$^{\circ}$ triangular AFM order can switch its sign without affecting the AHE generated by the SSC. The present study opens up a new direction to explore novel quantum phenomena associated with the coexistence of multiple orders.

Abstract

Owing to geometrical frustration in the kagome lattice, Mn3Sn displays a 120° in-plane triangular antiferromagnetic order, a manifestation of exchange interaction within the Heisenberg model. Here, we show the formation of a tunable noncoplanar magnetic ground state stabilized by higher-order exchange interactions in electron-doped Mn3Sn samples. Our density Functional Theory calculations reveal that the higher-order exchange induces a partial out-of-plane alignment of the Mn moments, resulting in a canted magnetic state, further experimentally confirmed by neutron diffraction study along with 60 T magnetic and Hall resistivity measurements. Interestingly, we find a large scalar spin chirality-induced Hall signal depending on the degree of non-coplanarity of the Mn moments. Additionally, we demonstrate simultaneous manipulation of two-component order-parameter in the system, where the two Hall signals can be independently manipulated. 27. The frequency and clinical outcome of mono-hit and multi-hit TP53aberrations in newly diagnosed multiple myeloma

Abstract

We investigated the frequency and outcome of mono-hit and multi-hit TP53aberrations [biallelic or ≥1 TP53mutations (TP53mut) or TP53mutwith variant allele frequency (VAF) ≥55%] in an Indian cohort of newly diagnosed multiple myeloma (NDMM) patients. We employed fluorescence insituhybridisation (FISH; n=457) and targeted next-generation sequencing (NGS; n=244) on plasma cell-enriched samples. We also studied the impact of TP53mutin cases with and without TP53deletions (TP53del). In our cohort with a median age of 60 years, TP53deland TP53mutwere seen in 12.9% (n=59/457; 14–95% cells) and 10.2% (n=25/244; 30 variants; VAF 3.4–98.2%; median 38.2%) respectively. Mono-hit and multi-hit-TP53aberrations were observed in 10.2% and 7.8%, respectively. Compared to TP53-wild-type (TP53wt), mono-hit and multi-hit TP53aberrations were associated with significantly poorer progression-free survival (PFS) (22.6 vs 12.1 vs 9.5 months; p=0.004) and overall survival (OS) [not reached (NR) vs 13.1 vs 15.6 months respectively; p=0.024]. However, multi-hit TP53did not significantly differ in OS/PFS compared to mono-hit cases. Compared to TP53wt, PFS and OS were significantly poorer in patients with TP53mutonly (9.5 vs 22.6 months and 12.1 months vs NR, respectively; p=0.020/0.004). TP53mutretained its significance even in the presence of any Revised International Staging System (HR 2.1; 95% CI 1.1–3.8; p=0.015) for OS. The detection of additional cases with TP53aberrations, as well as poor survival associated with the presence of mutation alone, supports TP53muttesting in NDMM at least in patients without TP53deland other high-risk cytogenetic abnormalities.

Abstract

Summary: Arsenic trioxide (ATO) and all‐trans retinoic acid (ATRA) form the backbone of the treatment of acute promyelocytic leukaemia (APL), with the addition of chemotherapy for high‐risk patients. We describe our experience of treating patients with APL of all risk classes with ATO and ATRA without chemotherapeutic agents. Patients received induction with ATO and ATRA followed by three cycles of consolidation with ATO and ATRA (each 1 month apart) after achieving morphological remission. Patients with intermediate‐ and high‐risk disease received a further 2 years of maintenance with ATRA, 6‐mercaptopurine and methotrexate. A total of 206 patients were included in the study. The majority of the patients were intermediate risk (51.9%), followed by high risk (43.2%). Differentiation syndrome was seen in 41 patients (19.9%). Overall, 25 patients (12.1%) died within 7 days of initiating therapy. Seven patients relapsed during follow‐up. The mean (SD) estimated 5‐year event‐free survival (EFS) and overall survival (OS) in the entire cohort was 79% [5.8%] and 80% [5.8%] respectively. After excluding patients who died within 7 days of therapy initiation, the mean (SD) estimated 5‐year EFS and OS was 90% [5.8%] and 93% [3.9%] respectively. Our study shows that treatment of all risk classes of APL with ATO and ATRA without chemotherapy is associated with excellent long‐term outcomes in the real‐world setting. [ABSTRACT FROM AUTHOR]

Abstract

Anaplastic lymphoma kinase (ALK) positive, anaplastic large cell lymphoma involving the non-mammary implant is an extremely rare presentation. Irrespective of the type or site, the implant-associated primary ALCL is morphologically and immunophenotypically similar to ALK-negative ALCLs. Herein, we present the case of a 42-year-old male who developed a lytic lesion after an implant for a right femur fracture. The lytic lesion biopsy revealed anaplastic large cell lymphoma with ALK protein expression. Imaging findings showed the widespread dissemination of disease all over the body, entrapping the implant too. ALCL involving the bone implant is a very unusual and rare presentation that needs to be documented.
