Abstract Human height is strongly influenced by genetics but the contribution of modifiable epigenetic factors is under-explored, particularly in low and middle-income countries (LMIC). We investigate links between blood DNA methylation and child height in four LMIC cohorts (n = 1927) and identify a robust association at three CpGs in the suppressor of cytokine signaling 3 (SOCS3) gene which replicates in a high-income country cohort (n = 879). SOCS3 methylation (SOCS3m)—height associations are independent of genetic effects. Mendelian randomization analysis confirms a causal effect of SOCS3m on height. In longitudinal analysis, SOCS3m explains a maximum 9.5% of height variance in mid-childhood while the variance explained by height polygenic risk score increases from birth to 21 years. Children’s SOCS3m is associated with prenatal maternal folate and socio-economic status. In-vitro characterization confirms a regulatory effect of SOCS3m on gene expression. Our findings suggest epigenetic modifications may play an important role in driving child height in LMIC.

During the COVID-19 pandemic, many children from limited-literacy communities in India did not receive information regarding COVID-19 safety due to the sudden shutdown of schools. Many parents from these communities could not afford virtual learning and lacked the ability to educate the children themselves. In July 2021, the research team developed comic, coloring, and activity books to provide children with fun, yet readable, educational materials. We consulted with teachers to simplify the language and understand the popularity of different cartoon characters. Between August 2021 and January 2022, our community health partners distributed the books to two age groups, ages 6–10 (n = 116, mean age 8.72) and 11–14 (n = 81, mean age 12.05). We conducted surveys with the children during and a week after distribution to assess any change in their knowledge about COVID-19 safety. The average age of children in this study was 10.09 (SD = 2.01) years. All resided in underresourced urban communities with low literacy rates and limited education. All questions were answered more correctly in the postsurvey, with the social distancing question having the greatest and most statistically significant increase (33.6%, p < 0.0001). The average increase in knowledge among children aged 6–10 (16.9%) was greater, though not statistically significantly so, than the average increase among children aged 11–14 (4.7%). These results indicate that child-friendly books can increase health education for children ages 6–14 in low-literacy populations. Additionally, the mechanism of the program is fit to be used in other low-resource populations globally.

IntroductionIn the past, various techniques have been used to improve motor imagery (MI), such as immersive virtual-reality (VR) and kinesthetic rehearsal. While electroencephalography (EEG) has been used to study the differences in brain activity between VR-based action observation and kinesthetic motor imagery (KMI), there has been no investigation into their combined effect. Prior research has demonstrated that VR-based action observation can enhance MI by providing both visual information and embodiment, which is the perception of oneself as part of the observed entity. Additionally, KMI has been found to produce similar brain activity to physically performing a task. Therefore, we hypothesized that utilizing VR to offer an immersive visual scenario for action observation while participants performed kinesthetic motor imagery would significantly improve cortical activity related to MI.MethodsIn this study, 15 participants (9 male, 6 female) performed kinesthetic motor imagery of three hand tasks (drinking, wrist flexion-extension, and grabbing) both with and without VR-based action observation.ResultsOur results indicate that combining VR-based action observation with KMI enhances brain rhythmic patterns and provides better task differentiation compared to KMI without action observation.DiscussionThese findings suggest that using VR-based action observation alongside kinesthetic motor imagery can improve motor imagery performance.

PurposeThis study aimed to assess the usability of a virtual reality-assisted sensorimotor activation (VRSMA) apparatus for individual digit rehabilitation. The study had two main objectives: Firstly, to collect preliminary data on the expectations and preferences of patients with carpal tunnel syndrome (CTS) regarding virtual reality (VR) and an apparatus-assisted therapy for their affected digits. Secondly, to evaluate the usability of the VRSMA apparatus that was developed.MethodsThe VRSMA system consists of an apparatus that provides sensory and motor stimulation via a vibratory motor and pressure sensor attached to a button, and a virtual reality-based visual cue provided by texts overlaid on top of a 3D model of a hand. The study involved 10 CTS patients who completed five blocks of VRSMA with their affected hand, with each block corresponding to the five digits. The patients were asked to complete a user expectations questionnaire before experiencing the VRSMA, and a user evaluation questionnaire after completing the VRSMA. Expectations for VRSMA were obtained from the questionnaire results using a House of Quality (HoQ) analysis.ResultsIn the survey for expectations, participants rated certain attributes as important for a rehabilitation device for CTS, with mean ratings above 4 for attributes such as ease of use, ease of understanding, motivation, and improvement of hand function based on clinical evidence. The level of immersion and an interesting rehabilitation regime received lower ratings, with mean ratings above 3.5. The survey evaluating VRSMA showed that the current prototype was overall satisfactory with a mean rating of 3.9 out of 5. Based on the HoQ matrix, the highest priority for development of the VRSMA was to enhance device comfort and usage time. This was followed by the need to perform more clinical studies to provide evidence of the efficacy of the VRSMA. Other technical characteristics, such as VRSMA content and device reliability, had lower priority scores.ConclusionThe current study presents a potential for an individual digit sensorimotor rehabilitation device that is well-liked by CTS patients.

Objective: The purpose of this study was to investigate the cortical activity and digit classification performance during tactile imagery (TI) of a vibratory stimulus at the index, middle, and thumb digits within the left hand in healthy individuals. Furthermore, the cortical activities and classification performance of the compound TI were compared with similar compound motor imagery (MI) with the same digits as TI in the same subjects. Methods: Twelve healthy right-handed adults with no history of upper limb injury, musculoskeletal condition, or neurological disorder participated in the study. The study evaluated the event-related desynchronization (ERD) response and brain–computer interface (BCI) classification performance on discriminating between the digits in the left-hand during the imagery of vibrotactile stimuli to either the index, middle, or thumb finger pads for TI and while performing a motor activity with the same digits for MI. A supervised machine learning technique was applied to discriminate between the digits within the same given limb for both imagery conditions. Results: Both TI and MI exhibited similar patterns of ERD in the alpha and beta bands at the index, middle, and thumb digits within the left hand. While TI had significantly lower ERD for all three digits in both bands, the classification performance of TI-based BCI (77.74 ± 6.98%) was found to be similar to the MI-based BCI (78.36 ± 5.38%). Conclusions: The results of this study suggest that compound tactile imagery can be a viable alternative to MI for BCI classification. The study contributes to the growing body of evidence supporting the use of TI in BCI applications, and future research can build on this work to explore the potential of TI-based BCI for motor rehabilitation and the control of external devices.

Background: Reaction time is an important measure of sensorimotor performance and coordination and has been shown to improve with training. Various training methods have been employed in the past to improve reaction time. Tactile imagery (TI) is a method of mentally simulating a tactile sensation and has been used in brain–computer interface applications. However, it is yet unknown whether TI can have a learning effect and improve reaction time. Objective: The purpose of this study was to investigate the effect of TI on reaction time in healthy participants. Methods: We examined the reaction time to vibratory stimuli before and after a TI training session in an experimental group and compared the change in reaction time post-training with pre-training in the experimental group as well as the reaction time in a control group. A follow-up evaluation of reaction time was also conducted. Results: The results showed that TI training significantly improved reaction time after TI compared with before TI by approximately 25% (pre-TI right-hand mean ± SD: 456.62 ± 124.26 ms, pre-TI left-hand mean ± SD: 448.82 ± 124.50 ms, post-TI right-hand mean ± SD: 340.32 ± 65.59 ms, post-TI left-hand mean ± SD: 335.52 ± 59.01 ms). Furthermore, post-training reaction time showed significant reduction compared with the control group and the improved reaction time had a lasting effect even after four weeks post-training. Conclusion: These findings indicate that TI training may serve as an alternate imagery strategy for improving reaction time without the need for physical practice.

Background: Filgotinib, an oral Janus kinase 1 preferential inhibitor, has been shown to be an effective treatment for ulcerative colitis (UC) in pre-registration studies. We aimed to describe the treatment population, effectiveness and safety of filgotinib in a real-world cohort of patients with UC. Methods: A retrospective observational cohort evaluation was conducted across nine UK inflammatory bowel disease centres. Baseline demographic and clinical data, clinical disease activity scores, endoscopic activity indices, and biomarkers (C-reactive protein and faecal calprotectin) were collected at baseline, at 8–12 weeks after initiation (post-induction) and during maintenance (the most recent review) where available. Effectiveness outcomes were assessed in patients with combined clinical disease activity and objective evidence of inflammation at filgotinib initiation. Results: Data were analysed for a total of 286 patients with a median follow-up time of 229 (IQR 113–324) days. The median age at filgotinib initiation was 38 (IQR 27–51) years, 64% were men and median disease duration was 5.1 (IQR 1.9–10.5) years; 56% had previous exposure to advanced therapies (biologics and small molecule) and 6% previously received tofacitinib. At the post-induction review, clinical response and remission were achieved in 65% and 51% of patients, respectively. There was a reduction in biomarkers and 78% of patients using corticosteroids at baseline were steroid-free. Persistence on filgotinib at 12 months was 66%. Adverse events were recorded in 30 patients with 8 patients discontinuing filgotinib as a result of an adverse event. Conclusions: In a large real-world cohort of patients with UC, filgotinib appears to be effective and welltolerated.

Introduction: A large proportion of postoperative mortality after pediatric surgery occurs among neonates with specific high-risk diagnoses. The extent to which there is hospital-level mortality variation among patients with these diagnoses and whether this variation is associated with differences in failure to rescue (FTR) is unclear. Methods: The Pediatric Health Information System® database (2012–2020) was used to identify patients who underwent surgery for eight high-risk neonatal diagnoses: gastroschisis; volvulus; necrotizing enterocolitis; intestinal atresia; meconium peritonitis; tracheoesophageal fistula; congenital diaphragmatic hernia; and perinatal intestinal perforation. Hospitals were stratified into tertiles of reliability-adjusted inpatient mortality rates (lower than average mortality – tertile 1 [T1]; higher than average mortality – tertile 3 [T3]). Multivariable hierarchical regression was used to evaluate the association between hospital-level, reliability-adjusted mortality and FTR. Results: Overall, 20,838 infants were identified across 48 academic, pediatric hospitals. Adjusted hospital mortality rates ranged from 4.0% (95% CI, 0.0–8.2) to 16.3% (12.2–20.4). Median case volume (range, 80–1,238) and number of NICU beds (range, 24–126) were not significantly different across hospital tertiles. Compared to the hospitals with the lowest postoperative mortality (T1), the odds of FTR were significantly higher in hospitals with the highest (T3) postoperative mortality (odds ratio 1.97 [1.50–2.59]). Conclusions: Significant variation in neonatal hospital mortality for high-risk diagnoses does not appear to be explained by hospital structural characteristics. Rather, difference in FTR suggests quality improvement interventions targeting early recognition and management of postoperative complications could improve surgical quality and safety for high-risk neonatal care. The investigation of heat transmission and entropy optimization in the thin-film flow of electromagnetic micropolar nanofluids has important implications in industrial lubrications, surgical instruments, and improved cooling systems. The Maxwell–Bruggeman and Krieger–Dougherty models are employed in this study to provide the nanoparticle aggregation/without aggregation attributes affecting the fluid characteristics. The Krieger-Dougherty model examines the modified viscosity caused by concentration of nanoparticle and accumulation, whereas the Maxwell-Bruggeman model assesses the effective thermal conductivity. The flow of micropolar nanofluid is assumed unsteady and laminar. The fundamental equations that govern the flow model are coupled into differential equations by applying appropriate similarity variables. The modeled problem have been solved through the implementation of the Runge–Kutta numerical technique. The findings elucidate that the micropolar effects such as Eringen parameter, spin gradient viscosity, and microrotation have a significant impact on the thin film thermodynamic behavior and flow kinetics. The electromagnetic fields alters the flow and thermal behavior significantly. It has been established that uniform dispersion of nanoparticles is crucial for optimizing thermal efficiency and reducing problems associated with aggregation and without aggregation. It is observed that when agglomeration is considered, the skin friction, entropy and Nusselt number increases significantly.

The study of boundary layer flow (BLF) with heat-mass transfer of binary chemical processes and nanofluids (NF) over a wedge is essential for improving heat transfer and reaction kinetics in applications including processing of material technologies, chemical reactors, and energy-efficient cooling mechanisms. This paper examines the entropy optimized BLF of silver Ag− water based nanofluid with binary chemical species over a wedge surface. The Tiwari-Das model is executed in this model which account the load of Ag− nanomaterials. The flow of NF over a moving wedge subject to favorable and adverse pressure differential is addressed by Naiver-Stokes equation. This model accounts the homogeneous heat reaction, viscous dissipation, joule heating and thermal radiations. The dimensionless equations for flow, for heat, and concentration are formulated and solved numerically using the fourth ordered Rung-Kutta approach. The findings suggest that fluid concentration is lowered with a rise in Schmidt number and homogenous chemical reaction. Thermal distribution improve with heterogonous reaction, magnetic parameter and deteriorate with wedge parameter. The skin friction rises from 25.277 % to 26.455 % with a material load of 3 % and magnetic parameter. The Nusselt decline with a radiative parameter from 10.984 % to 2.9748 % when particle load of 3 % is accounted.

This work investigates the heat transmission and boundary layer (BL) slip flow of a power-law fluid induced by stretching surfaces. The power-law fluid, induced magnetic field, and finite thermal relaxation have significant applications in a variety of industrial settings, including thin-layer growth, cooling systems, the extrusion process, and coating and shaping operations in a heat transfer mechanism. With these significant industrial applications, the current study explores BL flow and heat transfer in the context of the of the Cattaneo-Christov theory against the backdrop of an induced magnetic field over a stretching surface. The shear-thinning and thickening features are captured by the power-law fluid model, while the Cattaneo-Christov theory solves the deficiencies of Fourier's law. The BVP4c technique is used to construct and solve BL equations numerically. The surface drag and heat transmission rates at the wall are studied using statistical multi-regression analysis. The findings clarify that the magnetic and power law indices have a favorable effect on the Nusselt number, while their influence on skin friction is conflicting. A lower heat transmission is predicted with thermal relaxation as compared to Fourier's law in the energy equation. The accumulated thermal relaxation parameter produces a non-equilibrium state, delaying the internal thermal processes.

Heat-mass transfer in a liquid flow over a stretching surface at a stagnation regime is significant in optimal manufacturing and quality assurance depending on the rheological behavior of viscoelastic fluids in process machinery. Stretching sheet flow at a stagnation point is a standard procedure used in the metallurgical industries for manufacturing of different equipment and cooling systems. Due to its various applications in industrial operations, the current study aims to explore the heat-mass transfer of buoyancy-driven flow of cross liquid over a stretching sheet at a stagnation point. The heat and mass transfer under the framework of Dufour and Soret effects, radiant heat, and chemical reactions are assessed. Furthermore, the analysis also takes into account the linear and nonlinear convection-diffusion phenomena. The model findings are obtained by merging the Bvp4c in MATLAB using a local similarity technique up to the fourth truncation threshold. The findings illustrate that the action of magnetic field can be used to control the flow across the sheet. Temperature increases with thermal radiation, Biot number and Dufour number. The action of buoyancy ratio parameter result in improvement in fluid flow. With increasing trend of magnetic and Weissenberg number, the flow field and skin friction coefficient drops.

Background: The Volar Locking Plate Fixation (VLPF) is getting more preference among orthopedic surgeons for the treatment of distal radius fractures. Its management is overall very controversial. Objective: To compare two generally used surgical methods for Dorsally Displaced Fracture of Distal Radius in adults i.e. Percutaneous Fixation (using Kirschner Wires) and VLPF. Study type, settings & duration: This was a randomized control trial study was conducted in Lady Reading Hospital, Peshawar from April 2018 to January 2019. Methodology: A total of 78 adults were distributed into two groups of 39 each. Patients in Group A fractures were fixed by percutaneous k-wire fixation while in Group B fractures were fixed by VLPF. After surgery, patients were followed up in the outpatient department as per protocol. Patients were assessed by using the Disability of Arm, shoulder, and Hand (DASH) score. Data were analyzed using SPSS version 21. Results: Out of 78 patients with distal radius fractures, there were 27 (70%) males and 12 (30%) females in Group A, while there were 28 (72%) males and 11 (28%) females in Group B. The mean DASH score among the two groups was found to be 11.86±5.96 in group A and 18.24±8.02 in group B. This study examines the entropy generation and heat transfer performance in a Jeffrey-Hamel flow of biviscosity Bingham fluid with the addition of Al2O3 and Cu nanomaterials in the presence of a thermal Marangoni convective process. An emerging Jeffrey-Hamel problem is extended with the implementation of the Bingham fluid stress tensor in a Naiver Stokes equation. The governing equations with Marangoni boundary conditions due to surface tension are solved computationally utilizing the Keller-Box methodology. The findings demonstrate the complex interplay of entropy production processes, fluid-solid interfaces, and thermal Marangoni convection. Findings demonstrated that increasing Marangoni, Bingham parameters, and thermal radiation enhances the rate of heat transmission. The influence of the Marangoni and Bingham parameters on skin friction is conflicting in a narrow channel. System entropy and flow field uplift with a higher Marangoni convection parameter. Increasing the Reynolds number significantly increases the drag force, whereas the effect of the magnetic variable is the reverse. Velocity is an increasing function of the Reynolds and Bingham parameters and deteriorates with the load of nanomaterials. Temperature is a rising function of nanomaterial load, Eckert, and Bingham parameter. The results of this investigation have important ramifications for improving heat transfer, coolant systems, and nozzles design.

The ever-increasing electricity demand, its dependency on fossil fuels, and the consequent environmental degradation are major concerns of this era. The worldwide domination of fossil fuels in bulk electricity generation is rapidly increasing the emissions of CO 2 and other environmentally dangerous gases that are contributing to climate change. The economic and emission dispatch are two important problems in thermal power generation whose combination produces a complex highly constrained nonlinear optimization problem known as combined economic and emission dispatch. The optimization of combined economic and emission dispatch aims to allocate the generation of committed units to minimize fuel cost and emissions, simultaneously while honoring all equality and inequality constraints. Therefore, in this article, we investigate a solution of the combined economic and emission dispatch problem using quantum particle swarm optimization and its two modified versions, that is, enhanced quantum particle swarm optimization and quantum particle swarm optimization integrated with weighted mean personal best and adaptive local attractor. The enhanced quantum particle swarm optimization algorithm achieves particles’ diversification at early stages and shows good performance in local search at later stages. The quantum particle swarm optimization integrated with weighted mean personal best and adaptive local attractor boosts search performance of quantum particle swarm optimization and attains better global optimality. The suggested methods are employed to achieve solution for the combined economic and emission dispatch in four distinct systems, encompassing two scenarios with 6 units each, one with a 10-unit configuration, and another with an 11-unit setup. A comparative analysis with methodologies documented in existing literature reveals that the proposed approach outperforms others, demonstrating superior computational performance and robust efficiency.

Objective: To describe the frequency and patterns of postoperative complications and FTR after inpatient pediatric surgical procedures and to evaluate the association between number of complications and FTR. Summary and Background: FTR, or a postoperative death after a complication, is currently a nationally endorsed quality measure for adults. Although it is a contributing factor to variation in mortality, relatively little is known about FTR after pediatric surgery. Methods: Cohort study of 200,554 patients within the National Surgical Quality Improvement Program-Pediatric database (2012–2016) who underwent a high (≥ 1%) or low (< 1%) mortality risk inpatient surgical procedures. Patients were stratified based on number of postoperative complications (0, 1, 2, or ≥3) and further categorized as having undergone either a low- or high-risk procedure. The association between the number of postoperative complications and FTR was evaluated with multivariable logistic regression. Results: Among patients who underwent a low- (89.4%) or high-risk (10.6%) procedures, 14.0% and 12.5% had at least 1 postoperative complication, respectively. FTR rates after low- and high-risk procedures demonstrated step-wise increases as the number of complications accrued (eg, low-risk— 9.2% in patients with ≥3 complications; high-risk—36.9% in patients with ≥ 3 complications). Relative to patients who had no complications, there was a dose-response relationship between mortality and the number of complications after low-risk [1 complication – odds ratio (OR) 3.34 (95% CI 2.62–4.27); 2 – OR 10.15 (95% CI 7.40–13.92); ≥3-27.48 (95% CI 19.06-39.62)] and high-risk operations [1 – OR 3.29 (2.61–4.16); 2–7.24 (5.14–10.19); ≥3–20.73 (12.62–34.04)]. Conclusions: There is a dose-response relationship between the number of postoperative complications after inpatient surgery and FTR, ever after common, "minor" surgical procedures. Introduction:  The Nuss procedure is the most common and preferred operative correction of pectus excavatum. Surgeon preference and patient factors can result in variations in Nuss procedure technique. We hypothesize that certain techniques are associated with increased risk of complications.Materials and Methods:  We performed a single-center retrospective review of Nuss operations from 2016 to 2020. Variations in intraoperative techniques included sternal elevator (SE) use, number of bars placed, and usage of bilateral stabilizing sutures. Patient demographics, intraoperative data, and postoperative outcomes were reported as median with interquartile ranges or percentages. Statistical significance (p < 0.05) was determined with Wilcoxon's rank-sum and chi-square tests. Multivariate analysis was performed to control for introduction of intercostal nerve cryoablation and surgeon volume, and reported as odds ratio with 95% confidence interval.Results:  Two hundred and sixty-five patients were identified. Patients repaired with two bars were older with a larger Haller index (HI). Patient demographics were not significantly different for SE or stabilizing suture use. Placement of two bars was associated with significantly increased risk of readmission. Similarly, SE use was associated with increased risk of pleural effusion and readmission. Finally, the use of bilateral stabilizing sutures resulted in less frequent slipped bars without statistical significance.Conclusion:  Older patients with a larger HI were more likely to need two bars placed to repair pectus excavatum. A wide range of industrial applications where effective heat transport, fluid dynamics, and material adaptability are vital and can be addressed by the deployment of ternary hybrid nanofluids across an extensible cylinder. This work deals with entropy optimized flow and heat transport features of partially ionized and magnetically affected Prandtl fluid loaded with three nanomaterials drifting over a convectively heated cylinder. Three distinct nanomaterials (Al2O3,CuO,TiO2) are dispersed in order to enhance the heat performance rate of functional fluid. The ion and Hall slip effects, Joule heating, non-uniform heat source, and frictional heating are among the initial hypotheses in this computational analysis. The problem is presented quantitatively by taking the Tiwari-Das model to the basic equations. The Keller-Box framework is used to solve the nonlinear equations computationally. The findings reveal that enhancement in the Hall and ion slip effects diminish a rapid growth in entropy, while an opposite trend emerges for the Brinkman and magnetic parameter. The fluid motion is depressed by magnetic field intensity. The flow field and thermal transfer rate are enhanced by uplifting Prandtl first and curvature parameters. The system's irreversibility detracts with Brinkman number. A higher curvature parameter β results an elevated shear stresses and skin friction.
