BACKGROUND: Malaria in the first trimester of pregnancy is associated with adverse pregnancy outcomes. Artemisinin-based combination therapies (ACTs) are a highly effective, first-line treatment for uncomplicated Plasmodium falciparum malaria, except in the first trimester of pregnancy, when quinine with clindamycin is recommended due to concerns about the potential embryotoxicity of artemisinins. We compared adverse pregnancy outcomes after artemisinin-based treatment (ABT) versus non-ABTs in the first trimester of pregnancy. METHODS: For this systematic review and individual patient data (IPD) meta-analysis, we searched MEDLINE, Embase, and the Malaria in Pregnancy Library for prospective cohort studies published between Nov 1, 2015, and Dec 21, 2021, containing data on outcomes of pregnancies exposed to ABT and non-ABT in the first trimester. The results of this search were added to those of a previous systematic review that included publications published up until November, 2015. We included pregnancies enrolled before the pregnancy outcome was known. We excluded pregnancies with missing estimated gestational age or exposure information, multiple gestation pregnancies, and if the fetus was confirmed to be unviable before antimalarial treatment. The primary endpoint was adverse pregnancy outcome, defined as a composite of either miscarriage, stillbirth, or major congenital anomalies. A one-stage IPD meta-analysis was done by use of shared-frailty Cox models. This study is registered with PROSPERO, number CRD42015032371. FINDINGS: We identified seven eligible studies that included 12 cohorts. All 12 cohorts contributed IPD, including 34 178 pregnancies, 737 with confirmed first-trimester exposure to ABTs and 1076 with confirmed first-trimester exposure to non-ABTs. Adverse pregnancy outcomes occurred in 42 (5·7%) of 736 ABT-exposed pregnancies compared with 96 (8·9%) of 1074 non-ABT-exposed pregnancies in the first trimester (adjusted hazard ratio [aHR] 0·71, 95% CI 0·49-1·03). Similar results were seen for the individual components of miscarriage (aHR=0·74, 0·47-1·17), stillbirth (aHR=0·71, 0·32-1·57), and major congenital anomalies (aHR=0·60, 0·13-2·87). The risk of adverse pregnancy outcomes was lower with artemether-lumefantrine than with oral quinine in the first trimester of pregnancy (25 [4·8%] of 524 vs 84 [9·2%] of 915; aHR 0·58, 0·36-0·92). INTERPRETATION: We found no evidence of embryotoxicity or teratogenicity based on the risk of miscarriage, stillbirth, or major congenital anomalies associated with ABT during the first trimester of pregnancy. Given that treatment with artemether-lumefantrine was associated with fewer adverse pregnancy outcomes than quinine, and because of the known superior tolerability and antimalarial effectiveness of ACTs, artemether-lumefantrine should be considered the preferred treatment for uncomplicated P falciparum malaria in the first trimester. If artemether-lumefantrine is unavailable, other ACTs (except artesunate-sulfadoxine-pyrimethamine) should be preferred to quinine. Continued active pharmacovigilance is warranted. : Introduction: The delivery of daily, oral HIV pre‐exposure prophylaxis (PrEP) at private pharmacies may overcome barriers to PrEP delivery at public healthcare facilities, including HIV‐associated stigma, long wait times and overcrowding. Methods: At five private, community‐based pharmacies in Kenya, a care pathway for PrEP delivery (ClinicalTrials.gov: NCT04558554) was piloted—the first of its kind in Africa. Pharmacy providers screened clients interested in PrEP for HIV risk, then used a prescribing checklist to identify clients without medical conditions that might contraindicate PrEP safety, counsel them on PrEP use and safety, conduct provider‐assisted HIV self‐testing and dispense PrEP. For complex clinical cases, a remote clinician was available for consultation. Clients who did not meet the checklist criteria were referred to public facilities for free services delivered by clinicians. Pharmacy providers dispensed a 1‐month PrEP supply at initiation and a 3‐month supply thereafter at a client fee of 300 KES (∼$3 USD) per visit. Results: From November 2020 to October 2021, pharmacy providers screened 575 clients, identified 476 who met the prescribing checklist criteria and initiated 287 (60%) on PrEP. Among pharmacy PrEP clients, the median age was 26 years (IQR 22–33) and 57% (163/287) were male. The prevalence of behaviours associated with HIV risk among clients was high; 84% (240/287) reported sexual partners with unknown HIV status and 53% (151/287) reported multiple sexual partners (past 6 months). PrEP continuation among clients was 53% (153/287) at 1 month, 36% (103/287) at 4 months and 21% (51/242) at 7 months. During the pilot observation period, 21% (61/287) of clients stopped and restarted PrEP and overall pill coverage was 40% (IQR 10%–70%). Nearly, all pharmacy PrEP clients (≥96%) agreed or strongly agreed with statements regarding the acceptability and appropriateness of pharmacy‐delivered PrEP services. Conclusions: Findings from this pilot suggest that populations at HIV risk frequently visit private pharmacies and PrEP initiation and continuation at pharmacies is similar to or exceeds that at public healthcare facilities. Introduction: Oral pre‐exposure prophylaxis (PrEP) is highly effective, but coverage remains low in high HIV prevalence settings. Initiating and continuing PrEP remotely via online pharmacies is a promising strategy to expand PrEP uptake, but little is known about potential users' preferences. Methods: We conducted a discrete choice experiment (DCE) to assess preferences for online pharmacy PrEP services. We partnered with MYDAWA, an online pharmacy in Nairobi, Kenya. Eligibility criteria were: ≥18 years, not known HIV positive, interested in PrEP. The DCE contained four attributes: PrEP eligibility assessment (online self‐assessed, guided), HIV test type (provider administered, oral HIV self‐test [HIVST], blood‐based HIVST), clinical consultation (remote, in‐person) and user support options (text messages, phone/video call, email). Additionally, participants indicated whether they were willing to uptake their selected service. The survey was advertised on MYDAWA's website; interested participants met staff in‐person at a convenient location to complete the survey from 1 June to 20 November 2022. We used conditional logit modelling with an interaction by current PrEP use to estimate overall preferences and latent class analysis (LCA) to assess preference heterogeneity. Results: Overall, 772 participants completed the DCE; the mean age was 25 years and 54% were female. Most participants indicated a willingness to acquire online PrEP services, with particularly high demand among PrEP‐naive individuals. Overall, participants preferred remote clinical consultation, HIV self‐testing, online self‐assessment and phone call user support. The LCA identified three subgroups: the "prefer online PrEP with remote components" group (60.3% of the sample) whose preferences aligned with the main analysis, the "prefer online PrEP with in‐person components" group (20.7%), who preferred in‐person consultation, provider‐administered HIV testing, and guided assessment, and the "prefer remote PrEP (18.9%)" group who preferred online PrEP services only if they were remote. Conclusions: Online pharmacy PrEP is highly acceptable and may expand PrEP coverage to those interested in PrEP but not accessing services. Most participants valued privacy and autonomy, preferring HIVST and remote provider interactions. However, when needing support for questions regarding PrEP, participants preferred phone/SMS contact with a provider. Newly introduced vaccines and drugs hold the promise of reducing morbidity and mortality among pregnant women and infants living in LMICs. However, since pregnant women are actively excluded from most clinical trials, a sufficient level of pregnancy safety information is rarely available at the time of a medical product’s licensure or approval. Additionally, most pharmacovigilance studies are conducted in high-income countries’ setting and therefore, patterns and levels of exposure in LMICs are under-investigated and are not captured in these studies (Kiguba et al., 2021). Consequently, advancing new interventions to improve the health of pregnant women and their infants must be accompanied by systems that monitor both the risks and benefits of those interventions throughout a product’s life cycle. The thalidomide disaster of the 1960s demonstrated all too well the importance of post-licensure monitoring of the safety of medical products used during pregnancy. More recently, concerns about the neurodevelopmental effects of the antiepileptic sodium valproate have highlighted the need for ongoing vigilance of medicinal products as diagnostic science evolves. Maternal immunizations (MI) (i.e., immunizations during pregnancy) are an important intervention, protecting the woman during pregnancy, protecting the fetus from infection-related adverse effects, and protecting the newborn after birth through passive transfer of maternal vaccine-derived antibodies to the fetus (Munoz & Jamieson, 2019). MI will increase due to the growing use of vaccines for COVID-19 and the anticipated introduction of new vaccines specific for pregnancy, e.g., respiratory syncytial virus (RSV) and group B streptococcus (GBS) (Engmann et al., 2020). The safety of vaccines administered during pregnancy must be evaluated throughout a vaccine’s life cycle, particularly through active surveillance approaches during the post-licensure or post-authorization phase. Pregnancy exposure registries (PERs), particularly in many high-income countries (HICs), are used throughout the post-marketing phase to monitor the safety of vaccines and medicines used during pregnancy (Gelperin et al., 2017). While the primary objective of most PERs is the safety of specific types or classes of products used during pregnancy, they may provide an opportunity to evaluate their use and effectiveness as well as the safety of other medical products used during pregnancy. In HICs, PERs have been successful in both generating signals of potential teratogenicity that require further investigation and providing reassurance that certain products are not major teratogens. PERs are an important source of safety data for drugs and vaccines used in pregnancy to evaluate their risks and benefits. PERs have been used infrequently in LMICs, where there are unique challenges with respect to the availability of data on background rates of obstetric/neonatal complications and outcomes, access to interventions, and availability of data resources and infrastructure for capturing information on (a) use of vaccines and other medicines in pregnancy, (b) obstetric/perinatal complications, and outcomes, and (c) being able to link these data sources together (Berrueta et al., 2021). Moreover, there are types of medicines more commonly used in many LMICs, such as antimalarials (Dellicour et al., 2008) and antiretrovirals (Mehta, et al. 2019). Nevertheless, studies that follow and systematically record pregnancy exposure outcomes are scarce, especially in LMICs. Previous research has focused on identifying existing maternal, newborn, and child health (MNCH) data collection systems in LMICs and their capacity to be leveraged for active safety surveillance of novel vaccines used during pregnancy (Berrueta et al., 2021). In contrast, PERs are established based on actively collecting information on exposures to medical products during pregnancy and associated pregnancy and infant outcomes. Better knowledge of PERs in addition to MNCH systems can better inform how they can aid in a larger maternal immunization introduction strategy in LMICs. PATH in collaboration with the Gates Foundation, WHO, and many stakeholders including the University of Washington will conduct a scoping review, to 1) identify existing pregnancy exposure registries and other related resources in LMICs, and 2) describe their characteristics, comparability, and ability to monitor health outcomes during pregnancy and after birth. We will follow the Joanna Briggs Institute manual for scoping reviews to conduct this study (JBI 2017). Findings will be reported in accordance with PRISMA Extension on Scoping Reviews reporting guidelines (PRISMA-ScR).

Safety signals or adverse event signals alert pharmacovigilance (PV) systems to possible adverse events (AE) that were not identified by premarket safety assessments of medicines and vaccines. This study explores the extent to which World Health Organization-detected AE signals are being contextualized (i.e., reviewed for validation, confirmation, analysis, prioritization, assessment and recommendation for action) in LMICs and the extent to which they use this data for decision-making. Concepts describing countries’ use of these AE signals will be explored and defined via a scoping review. Primary question: - To what extent do LMICs detect or use adverse event signals for country-level decision making? Background Using comprehensive estimates of cause of death, life expectancy (LE-70), healthy life expectancy (HALE-70) and burden of disease in Australia from the Global Burden of Disease Study 2019, we examine patterns of health loss in the Australian elderly population over 70 to provide guidance for policy makers. Methods Using the standardised methodology framework and analytical strategies from GBD 2019, we estimated mortality, causes of death, years of life lost (YLLs), years lived with disability (YLDs), disability-adjusted life-years (DALYs), life expectancy (LE-70) at age 70 years, and healthy life expectancy (HALE-70) for Australia from 1990 to 2019 and compared to global and SDI groups estimates. Findings Health of Australians over 70 has steadily improved over the past 30 years as measured by DALY rates. The decrease in DALY rates were primarily driven by a decrease in YLLs attributable to cardiovascular (60%) and chronic respiratory (30.2%) diseases, and transport injuries (56.9%), while the non-fatal burden, including attributable to mental health, remained stable from 1990 to 2019. The five top causes of DALY rates were ischemic heart disease, Alzheimer’s disease, COPD, stroke, and falls, where falls showed the largest increase from 1990. Interpretation This study provides an in-depth report on the main causes of mortality and disability, HALE-70 and LE-70 in the population aged 70 and older in Australia, highlighting the changes in burden over the past 30 years. These results can be treated as pre-COVID baseline estimates of mortality and disease burden for the Australian elderly population over 70.

- Abstract
OBJECTIVES: To use data from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019 (GBD 2019) to estimate mortality and disability trends for the population aged ≥70 and evaluate patterns in causes of death, disability, and risk factors. DESIGN: Systematic analysis. SETTING: Participants were aged ≥70 from 204 countries and territories, 1990-2019. MAIN OUTCOMES MEASURES: Years of life lost, years lived with disability, disability adjusted life years, life expectancy at age 70 (LE-70), healthy life expectancy at age 70 (HALE-70), proportion of years in ill health at age 70 (PYIH-70), risk factors, and data coverage index were estimated based on standardised GBD methods. RESULTS: Globally the population of older adults has increased since 1990 and all cause death rates have decreased for men and women. However, mortality rates due to falls increased between 1990 and 2019. The probability of death among people aged 70-90 decreased, mainly because of reductions in non-communicable diseases. Globally disability burden was largely driven by functional decline, vision and hearing loss, and symptoms of pain. LE-70 and HALE-70 showed continuous increases since 1990 globally, with certain regional disparities. Globally higher LE-70 resulted in higher HALE-70 and slightly increased PYIH-70. Sociodemographic and healthcare access and quality indices were positively correlated with HALE-70 and LE-70. For high exposure risk factors, data coverage was moderate, while limited data were available for various dietary, environmental or occupational, and metabolic risks. CONCLUSIONS: Life expectancy at age 70 has continued to rise globally, mostly because of decreases in chronic diseases. Adults aged ≥70 living in high income countries and regions with better healthcare access and quality were found to experience the highest life expectancy and healthy life expectancy.
