Abstract

Background: Total pharyngolaryngectomy (TPL) is standard treatment for hypopharyngeal cancer. However, extensive thyroidectomy and paratracheal nodal dissection (PTND) can cause hypoparathyroidism. We sought to determine the optimum extent of resection. Methods: We analyzed the clinicopathological information of 161 pyriform sinus cancer patients undergoing TPL from 25 Japanese institutions. Rates of recurrence and risk factors for hypoparathyroidism, as well as incidence of pathological contralateral level VI nodal metastasis and stomal recurrence, were investigated. Results: The extent of thyroidectomy and nodal dissection were not independent risk factors for recurrence. Incidences of contralateral level VI nodal involvement and stomal recurrence were 1.8% and 1.2%, respectively. Patients undergoing hemithyroidectomy/ipsilateral PTND did not develop stomal recurrence and had the lowest incidence of hypoparathyroidism. Prognosis in patients without tracheostomy prior to hemithyroidectomy/ipsilateral PTND was comparable to that with more extensive resections. Conclusions: Hemithyroidectomy/ipsilateral PTND may be sufficient for pyriform sinus cancer cases without tracheostomy. [ABSTRACT FROM AUTHOR]

This paper proposes a recommendation method considering users' time series contexts which are situations that have occurred / will occur in the past/future. There are some recommendation methods that provide information suitable for users' action patterns as the recommendation methods considering them. These methods provide information referring to the other users that have a similar action pattern to that of an active user. However, since a user's action pattern changes depending on the user's contexts, the methods need to refer to the other users' action patterns related to the current user's contexts. Abstract

We propose a ranking method using a Support Vector Machine for information recommendation. By using the SVM, a recommendation method can determine suitable items for a user from enormous item sets. However, it can decide based on just two classes: whether the user likes a thing or not. When there is a large number of recommended items, it is not easy for the user to find the best item by herself. To resolve this issue, it is desirable to rank the items based on the user's preferences. Moreover, the user's preferences change depending on the context. Based on the above problem, we propose a context-aware ranking method for information recommendation. Our method considers a user's context when ranking items. Our method consists of the following two steps: (1) Predicting important feature parameters for the user. (2) Calculating a ranking score of each item in recommendation candidates. Abstract

Arytenoid cartilage necrosis (ACN) is a life-threatening laryngeal complication that develops after prolonged endotracheal intubation (PEI). We describe a case of ACN due to PEI that was treated conservatively with close endoscopic follow-up. A 46-year-old man required 4 days of postoperative endotracheal intubation after surgery for Stanford A acute aortic dissection. Five days post-extubation, he presented with hoarseness and wheezing. Endoscopy and computed tomography findings confirmed ACN. We used conservative treatment to avoid a tracheostomy because the patient had no dyspnoea. During healing, vocal cord mobility changed per the extent of cicatrisation and adhesion, and glottic closure due to inter arytenoid adhesion was possible. The bilateral vocal cords were eventually fixed in a paramedian position. The patient could phonate via ventricular band mobility without aspiration or dyspnoea. Given the possibility of variable laryngeal findings, careful endoscopic evaluations may be needed until wound healing completes to avoid sudden suffocation after PEI. [ABSTRACT FROM AUTHOR]

Abstract

ObjectiveBasal information of head and neck small-cell carcinoma (HNSmCC) including epidemiology, primary site, treatment, and prognosis remains sparse due to its rarity. We report here a multicenter retrospective study on the diagnosis, treatment, and outcomes of patients with HNSmCC.Materials and methodsThis study involved 47 patients with HNSmCC from 10 participating institutions. Eight patients were excluded for whom no pathological specimens were available (n = 2) and for discrepant central pathological judgements (n = 6). The remaining 39 patients were processed for data analysis.ResultsAs pretreatment examinations, computed tomography (CT) was performed for the brain (n = 8), neck (n = 39), and chest (n = 32), magnetic resonance imaging (MRI) for the brain (n = 4) and neck (n = 23), positron emission tomography-CT (PET-CT) in 23 patients, bone scintigraphy in 4, neck ultrasonography in 9, and tumor markers in 25. Primary sites were oral cavity (n = 1), nasal cavity/paranasal sinuses (n = 16), nasopharynx (n = 2), oropharynx (n = 4), hypopharynx (n = 2), larynx (n = 6), salivary gland (n = 3), thyroid (n = 2), and others (n = 3). Stages were II/III/IV-A/IV-B/IV-C/Not determined = 3/5/16/6/5/4; stage IV comprised 69%. No patient had brain metastases. First-line treatments were divided into 3 groups: the chemoradiotherapy (CRT) group (n = 27), non-CRT group (n = 8), and best supportive care group (n = 4). The CRT group included concurrent CRT (CCRT) (n = 17), chemotherapy (Chemo) followed by radiotherapy (RT) (n = 5), and surgery (Surg) followed by CCRT (n = 5). The non-CRT group included Surg followed by RT (n = 2), Surg followed by Chemo (n = 1), RT alone (n = 2), and Chemo alone (n = 3). The 1-year/2-year overall survival (OS) of all 39 patients was 65.3/53.3%. The 1-year OS of the CRT group (77.6%) was significantly better compared with the non-CRT group (31.3%). There were no significant differences in adverse events between the CCRT group (n = 22) and the Chemo without concurrent RT group (n = 9).ConclusionNeck and chest CT, neck MRI, and PET-CT would be necessary and sufficient examinations in the diagnostic set up for HNSmCC. CCRT may be recommended as the first-line treatment. The 1-year/2-year OS was 65.3%/53.3%. This study would provide basal data for a proposing the diagnostic and treatment algorithms for HNSmCC.

Abstract

Purpose : This study aimed to evaluate the effectiveness of a diabetes oral nursing intervention program for individuals with diabetes. Methods : Fifty-six participants with diabetes underwent a diabetes oral nursing intervention program. The program’s effect was evaluated through questionnaires and small interviews. The modified diabetes oral health assessment tool (M-DiOHAT©) was used to assess and educate four factors ; oral conditions, behaviors, perceptions and knowledge about diabetes and periodontal disease, and health information-sharing, among participants at baseline, 3, 6, and 12 months later. Primary outcomes included changes in the M-DiOHAT© total scores. Secondary outcomes included scores on the motivation stage of changes in oral health behaviors’ scales, dental visits, number of present teeth, hemoglobin A1c (HbA1c), and participants’ comments. Results : The M-DiOHAT© total score and the motivation stage score significantly improved with the narrative comment of “being motivated to practice oral health behaviors” between the baseline and 12 months later. Eight participants visited the dentist, whereas no differences were observed in the number of present teeth or HbA1c. Conclusions : This program improved participants’ M-DiOHAT© total score, motivation stage score, and dental visits. These results suggest the program improved oral health perceptions and behaviors among individuals with diabetes.

25. Association Between Nutrient Patterns and Fatty Liver Index : Baseline Survey of the Japan Multi-Institutional Collaborative Cohort Study in Tokushima, Japan

Abstract

Backgrounds: The fatty liver index (FLI) is a good non-invasive approach for fatty liver disease diagnosis. The objective of this study was to examine the associations of nutrient patterns with non-alcoholic fatty liver disease (NAFLD) in a Japanese population. Methods: A total of 1,588 subjects (789 men and 799 women) aged 35–69 years were recruited in the baseline survey of the Japan Multi-Institutional Collaborative Cohort (J-MICC) Study in Tokushima Prefecture. Factor analysis was applied to energy-adjusted intake of 21 nutrients, and nutrient patterns were extracted. Multiple logistic regression analysis was used to analyze the relationships between nutrient patterns and the high FLI category (≥60). Results: Four nutrient patterns were extracted: Factor 1, vitamins, dietary fiber, iron and potassium pattern; Factor 2, fats and fat-soluble vitamins pattern; Factor 3, saturated fat, calcium, vitamin B2 and low carbohydrate pattern; and Factor 4, sodium, protein and vitamin D pattern. After adjustment for sex, age, and other potential confounding variables, higher Factor 1 scores were significantly associated with lower odds ratios of NAFLD (P for trend <0.05). Analysis of each component of FLI showed that there were significant inverse associations between Factor 1 scores and high body mass index and large waist circumference. Conclusion: The present findings suggest that a nutrient pattern rich in vitamins, fiber, iron, and potassium was associated with lower prevalence of NAFLD in a Japanese population. Obesity and abdominal obesity may be intermediate variables for the association between this nutrient pattern and NAFLD.

Abstract

Multiple endocrine neoplasia (MEN) with medullary thyroid carcinoma (MTC) is associated with rearranged during transfection (RET) mutations. The authors encountered four cases of MTC-related MEN type 2B (MEN2B) with RET codon M918T mutation in one family. Case 1 included a 19 year-old male diagnosed with MTC with lung metastases. Genetic testing revealed an RET codon M918T mutation, which indicated MEN2B. The patient responded partially to vandetanib and the disease has shown no progression in 25 months. Case 2 involved the mother of the patient in Case 1. She underwent total thyroidectomy (TT) when diagnosed with MTC-related MEN2B at 12 years of age, but was not counseled adequately. Cases 3 and 4 involved the sisters of the Case 1 patient and were assessed after Case 1 was diagnosed. Genetic testing revealed the same mutation. Case 3 was diagnosed with MTC and underwent TT. Case 4 was asymptomatic but underwent prophylactic TT; histopathologic examination revealed MTC tissue. Prophylactic TT prevented MTC from being detected at an advanced state. Genetic counseling is essential in treating MEN2B. The mother was uninformed about the genetic characteristics of MEN2B, delaying the detection of MTC in her children.
