Abstract

We investigate the level of accuracy and precision of cluster weak-lensing (WL) masses measured with the \Euclid data processing pipeline. We use the DEMNUni-Cov $N$-body simulations to assess how well the WL mass probes the true halo mass, and, then, how well WL masses can be recovered in the presence of measurement uncertainties. We consider different halo mass density models, priors, and mass point estimates. WL mass differs from true mass due to, e.g., the intrinsic ellipticity of sources, correlated or uncorrelated matter and large-scale structure, halo triaxiality and orientation, and merging or irregular morphology. In an ideal scenario without observational or measurement errors, the maximum likelihood estimator is the most accurate, with WL masses biased low by $\langle b_M \rangle = -14.6 \pm 1.7 \, \%$ on average over the full range $M_\text{200c} > 5 \times 10^{13} \, M_\odot$ and $z < 1$. Due to the stabilising effect of the prior, the biweight, mean, and median estimates are more precise. The scatter decreases with increasing mass and informative priors significantly reduce the scatter. Halo mass density profiles with a truncation provide better fits to the lensing signal, while the accuracy and precision are not significantly affected. We further investigate the impact of additional sources of systematic uncertainty on the WL mass, namely the impact of photometric redshift uncertainties and source selection, the expected performance of \Euclid cluster detection algorithms, and the presence of masks. Taken in isolation, we find that the largest effect is induced by non-conservative source selection. This effect can be mostly removed with a robust selection. As a final \Euclid-like test, we combine systematic effects in a realistic observational setting and find results similar to the ideal case, $\langle b_M \rangle = - 15.5 \pm 2.4 \, \%$, under a robust selection.

Abstract

Precise and accurate mass calibration is required to exploit galaxy clusters as astrophysical and cosmological probes in the Euclid era. Systematic errors in lensing signals by galaxy clusters can be empirically estimated by comparing different surveys with independent and uncorrelated systematics. To assess the robustness of the lensing results to systematic errors, we carried out end-to-end tests across different data sets. We performed a unified analysis at the catalogue level by leveraging the Euclid combined cluster and weak-lensing pipeline (COMB-CL). COMB-CL will measure weak lensing cluster masses for the Euclid Survey. Heterogeneous data sets from five independent, recent, lensing surveys (CHFTLenS, DES~SV1, HSC-SSP~S16a, KiDS~DR4, and RCSLenS), which exploited different shear and photometric redshift estimation algorithms, were analysed with a consistent pipeline under the same model assumptions. We performed a comparison of the amplitude of the reduced excess surface density and of the mass estimates using lenses from the Planck PSZ2 and SDSS redMaPPer cluster samples. Mass estimates agree with literature results collected in the LC2 catalogues. Mass accuracy was further investigated considering the AMICO detected clusters in the HSC-SSP XXL North field. The consistency of the data sets was tested using our unified analysis framework. We found agreement between independent surveys, at the level of systematic noise in Stage-III surveys or precursors. This indicates successful control over systematics. Abstract

We derived galaxy colour selections from Euclid and ground-based photometry, aiming to accurately define background galaxy samples in cluster weak-lensing analyses. Given any set of photometric bands, we developed a method for the calibration of optimal galaxy colour selections that maximises the selection completeness, given a threshold on purity. We calibrated galaxy selections using simulated ground-based $griz$ and Euclid $Y_{\rm E}J_{\rm E}H_{\rm E}$ photometry. Both selections produce a purity higher than 97%. The $griz$ selection completeness ranges from 30% to 84% in the lens redshift range $z_{\rm l}\in[0.2,0.8]$. With the full $grizY_{\rm E}J_{\rm E}H_{\rm E}$ selection, the completeness improves by up to $25$ percentage points, and the $z_{\rm l}$ range extends up to $z_{\rm l}=1.5$. The calibrated colour selections are stable to changes in the sample limiting magnitudes and redshift, and the selection based on $griz$ bands provides excellent results on real external datasets. The $griz$ selection is also purer at high redshift and more complete at low redshift compared to colour selections found in the literature. We find excellent agreement in terms of purity and completeness between the analysis of an independent, simulated Euclid galaxy catalogue and our calibration sample, except for galaxies at high redshifts, for which we obtain up to 50 percent points higher completeness. The combination of colour and photo-$z$ selections applied to simulated Euclid data yields up to 95% completeness, while the purity decreases down to 92% at high $z_{\rm l}$. We show that the calibrated colour selections provide robust results even when observations from a single band are missing from the ground-based data. Abstract

Aims. We derived galaxy colour selections from Euclid and ground-based photometry, aiming to accurately define background galaxy samples in cluster weak-lensing analyses. These selections have been implemented in the Euclid data analysis pipelines for galaxy clusters. Methods. Given any set of photometric bands, we developed a method for the calibration of optimal galaxy colour selections that maximises the selection completeness, given a threshold on purity. Such colour selections are expressed as a function of the lens redshift. Results. We calibrated galaxy selections using simulated ground-based griz and Euclid YEJEHE photometry. Both selections produce a purity higher than 97%. The griz selection completeness ranges from 30% to 84% in the lens redshift range zl ∈ [0.2, 0.8]. With the full grizYEJEHE selection, the completeness improves by up to 25 percentage points, and the zl range extends up to zl = 1.5. The calibrated colour selections are stable to changes in the sample limiting magnitudes and redshift, and the selection based on griz bands provides excellent results on real external datasets. Furthermore, the calibrated selections provide stable results using alternative photometric aperture definitions obtained from different ground-based telescopes. The griz selection is also purer at high redshift and more complete at low redshift compared to colour selections found in the literature. We find excellent agreement in terms of purity and completeness between the analysis of an independent, simulated Euclid galaxy catalogue and our calibration sample, except for galaxies at high redshifts, for which we obtain up to 50 percentage points higher completeness. The combination of colour and photo-z selections applied to simulated Euclid data yields up to 95% completeness, while the purity decreases down to 92% at high zl. Abstract

Precise and accurate mass calibration is required to exploit galaxy clusters as astrophysical and cosmological probes in the Euclid era. Systematic errors in lensing signals by galaxy clusters can be empirically estimated by comparing dierent surveys with independent and uncorrelated systematics. To assess the robustness of the lensing results to systematic errors, we carried out end-to-end tests across dierent data sets. We performed a unified analysis at the catalogue level by leveraging the Euclid combined cluster and weak-lensing pipeline (COMB-CL). Notably, COMB-CL will measure weak lensing cluster masses for the Euclid Survey. Heterogeneous data sets from five recent, independent lensing surveys (CHFTLenS, DES SV1, HSC-SSP S16a, KiDS DR4, and RCSLenS), which exploited dierent shear and photometric redshift estimation algorithms, were analysed with a consistent pipeline under the same model assumptions. We performed a comparison of the amplitude of the reduced excess surface density and of the mass estimates using lenses from the Planck PSZ2 and SDSS redMaPPer cluster samples. Mass estimates agree with the results in the literature collected in the LC2 catalogues. Mass accuracy was further investigated considering the AMICO-detected clusters in the HSC-SSP XXL-North field. The consistency of the data sets was tested using our unified analysis framework.We found agreement between independent surveys at the level of systematic noise in Stage-III surveys or precursors. This indicates successful control over systematics. If this control continues into Stage IV, Euclid will be able to measure the weak lensing masses of around 13 000 (considering shot noise only) or 3000 (noise from shape and large-scale-structure) massive clusters with a signal-to-noise ratio greater than three.

Abstract

Aims. We derived galaxy colour selections from Euclid and ground-based photometry, aiming to accurately define background galaxy samples in cluster weak-lensing analyses. These selections have been implemented in the Euclid data analysis pipelines for galaxy clusters. Methods. Given any set of photometric bands, we developed a method for the calibration of optimal galaxy colour selections that maximises the selection completeness, given a threshold on purity. Such colour selections are expressed as a function of the lens redshift. Results. We calibrated galaxy selections using simulated ground-based griz and EuclidYEJEHE photometry. Both selections produce a purity higher than 97%. The griz selection completeness ranges from 30% to 84% in the lens redshift range zl ∈ [0.2, 0.8]. With the full grizYEJEHE selection, the completeness improves by up to 25 percentage points, and the zl range extends up to zl = 1.5. The calibrated colour selections are stable to changes in the sample limiting magnitudes and redshift, and the selection based on griz bands provides excellent results on real external datasets. Furthermore, the calibrated selections provide stable results using alternative photometric aperture definitions obtained from different ground-based telescopes. The griz selection is also purer at high redshift and more complete at low redshift compared to colour selections found in the literature. We find excellent agreement in terms of purity and completeness between the analysis of an independent, simulated Euclid galaxy catalogue and our calibration sample, except for galaxies at high redshifts, for which we obtain up to 50 percentage points higher completeness. Abstract

International audience; Objectives: Our aim was to assess if switching from a protease inhibitors (PI)-based regimen to a PI-free one is associated with an increased risk of Kaposi Sarcoma (KS) relapse among patients living with HIV (PLHIV) with history of KS and controlled HIV replication.Methods: In a retrospective analysis of the prospectively collected Dat'AIDS database we selected patients who both had a past KS history and a HIV-1 viral load below 200 copies/mL while being PI-treated. We searched for KS relapses while persistent virological success was maintained for at least 6 months, whether patients kept taking the PI, or switched to PI-free regimen.Results: Among the 216 patients with past KS event and a history of HIV-1 infection efficiently treated by a PI-based regimen, 148 patients (68.5%) later switched to a PI-sparing regimen. Their baseline characteristics were not different from non-switching patients. We described 7 cases of relapse (3.2% of the 216 patients). Five cases of relapse occurred in switching patients (3.4%). The remaining two relapses occurred in PI-treated patients (2.9%). At KS relapse, CD4 cell count was 459 cells/μL (range 225-560) for switching patients, compared with 362 and 136 cells/μL for the other two patients.Conclusions: In this large cohort of PLHIV with a history of KS and ART-controlled HIV replication, KS relapses were described in 3.2% of the patients, and were not more frequent when a PI-containing ART regimen has been switched to a PI-free regimen. Our results do not support a specific effect of PI on KS.

Abstract

Background: Emergency Medical Communication Centres (EMCCs) play a crucial role in emergency care by ensuring timely responses through telephone triage. However, extended communication times can impede accessibility, patient triage, and decision-making. Identifying the factors influencing communication duration is essential for improving EMCC efficiency., Objective: This study aims to identify temporal, human, and contextual factors associated with prolonged communication times in an EMCC where decision-making is conducted by physicians., Methods: We conducted a retrospective observational study of all calls received at a French EMCC between March 1 and December 31, 2019. A total of 108,548 patient medical files were analyzed, excluding calls from medical personnel or hospitals. We examined the total communication time (from call initiation to decision) and the medical communication time (physician involvement). Bivariate and multivariate logistic regressions were used to identify factors associated with prolonged communication times., Results: The median total communication time was 7 min [IQR 5-11], and the median medical communication time was 3 min [IQR 2-4]. Psychiatric reasons for calling (OR = 1.75) and elderly patients (OR = 1.58) were associated with longer communication times. Calls leading to medical advice (OR = 1.48) and calls during weekends or nighttime were also significant factors. Conversely, calls for trauma or from nursing homes, and those handled by emergency physicians, were associated with shorter durations., Conclusion: Several factors influence communication times in EMCCs, including patient demographics, reason for the call, and time of day., Competing Interests: Declarations. 16. Effect of noise on the performance of arthroscopic simulator.

Abstract

Background: Noise is omnipresent in the operating room. The average noise in the operating room generally ranges between 60 and 65 dB and can sometimes exceed 100 dB, despite the ARS (Agence Régionale de Santé) and WHO (World Health Organization) recommending levels of 35 dB(A). This study aimed to evaluate the effect of different kinds of background auditory stimuli on the performance of surgeons during an arthroscopic simulation task., Methods: Forty-seven surgeons with varying experience in arthroscopic surgery undertook different exercises under four different conditions: quiet, classical music, hard rock, and sustained chatter. All background auditory stimuli were set at 65 dB(A). Each participant underwent double randomization for the four sound stimuli and the four exercises to be performed. A musical questionnaire was also completed by each participant. Data related to each exercise included operating time in seconds, distance from the camera or instruments in centimeters, and an overall score automatically calculated by the simulator based on safety, economy of movement, and speed (scale: 0-20 points)., Results: Operative time in an environment with classical music was significantly lower than in an environment with hard rock (95.9 s vs. 128.7 s, p  = 0.0003). The overall rating in an environment with chatter was significantly lower than in a silent environment (11.7 vs. 15.7, p  < 0.0001). The overall rating in an environment with hard rock was significantly lower than in an environment with classical music (14.3 vs. 17.5, p  = 0.0008).Surgeons who preferred listening to music in the operating room performed differently than those who did not. The mean operative time for surgeons who preferred music was 99.52 s (SD = 47.20), compared to 117.16 s (SD = 61.06) for those who did not prefer music, though this difference was not statistically significant ( p  = 0.082). The mean overall score for surgeons who preferred music was significantly higher at 17.46 (SD = 2.29) compared to 15.57 (SD = 3.49) for those who did not prefer music ( p  = 0.001)., Conclusions: Our study suggests that exposure to classical music and silence may confer greater benefits to the surgeon compared to the impact of hard rock and chatter. These conclusions are grounded in significant differences observed in operative time and overall evaluations, highlighting the potential advantages of an environment characterized by acoustic tranquility for surgical professionals. Abstract

Background: Acute gastrointestinal bleeding (AGIB) is common in older patients but the use of iron in this context remains understudied., Aims: This study aimed to evaluate prospectively the efficacy of ferric carboxymaltose to treat anaemia in older patients after AGIB., Methods: This randomised double-blinded placebo-controlled clinical trial was conducted in 10 French centres. Eligible patients were 65 years or more, had controlled upper or lower gastrointestinal bleeding and a haemoglobin level of 9-11 g/dl. Patients were randomly assigned, in a 1:1 ratio, to receive either one intravenous iron injection of ferric carboxymaltose or one injection of saline solution. The primary endpoint was the difference in haemoglobin level between day 0 and day 42. Secondary endpoints were treatment-emergent adverse events, serious adverse events, rehospitalisation and improvement of quality of life (QOL) at day 180., Results: From January 2013 to January 2017, 59 patients were included. The median age of patients was 81.9 [75.8, 87.3] years. At day 42, a significant difference in haemoglobin level increase was observed (2.49 g/dl in the ferric carboxymaltose group vs. 1.56 g/dl in the placebo group, P = 0.02). At day 180, QOL, measured on European Organisation for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30, improved by 10.5 points in the ferric carboxymaltose group and by 8.2 points in the placebo group (P = 0.56). Rates of adverse events and rehospitalisation were similar in the two groups., Conclusions: Intravenous iron seems safe and effective to treat anaemia in older patients after AGIB and should be considered as a standard-of-care treatment. 18. Short-Course Versus Prolonged-Course Antimicrobial Therapy in Adults With Catheter-Related Septic Thrombosis: A Propensity-Weighted Retrospective Study.

Abstract

Background: Optimal duration of antimicrobial therapy (AT) for catheter-related septic deep venous thrombosis (DVT) is unknown. We aimed to compare the outcomes of patients receiving short-course AT (≤21 days) versus prolonged-course AT (>21 days)., Methods: This was a monocentric retrospective study comparing adults with catheter-related septic DVT from 2015 to 2020 treated with short- or prolonged-course AT. A propensity score-weighted analysis was used to mitigate potential bias. The primary outcome was a composite of all-cause mortality or recurrent bloodstream infection 30 days after AT discontinuation., Results: Of 172 patients with catheter-related septic DVT, 104 were treated with prolonged-course AT and 68 with short-course AT. In the propensity score analysis, we found no significant difference in 30-day all-cause mortality or relapse between the 2 groups (inverse probability of treatment weighted hazard ratio [wHR], 2.16 [95% confidence interval {CI}, .68-6.88]; P = .192). No differences in 90-day all-cause mortality and 90-day relapse were observed between the treatment groups (wHR, 1.01 [95% CI, .49-2.05], P = .987 and 1.13 [95% CI, .08-15.62], P = .928, respectively)., Conclusions: A 21-day AT could be an effective and safe option to treat catheter-related septic DVT. Abstract

Objectives: Adequate pain management after thoracoscopic surgery is a major issue in the prevention of respiratory complications. The combination of the paravertebral block (PVB) with the serratus anterior plane block (SAPB) may decrease postoperative pain. The objective of this study was to evaluate the impact of the combination of PVB and SAPB on the consumption of morphine and pain after video- or robot-assisted thoracic surgery., Methods: The main objective of this randomized controlled trial was to compare the cumulative postoperative morphine consumption at 24 h between a group having PVB (PVB group) and a group having PVB and SAPB (PV-SAPB group). Postoperative pain at 6 and 24 h and morphine-related complications were also assessed., Results: A total of 112 patients were included with 56 in each group. There was no difference in median cumulative morphine consumption at 24 h between the 2 groups (P = 0.1640). At 6 h, the median postoperative pain was higher in the PVB group compared to the PV-SAPB group (3 [0; 4] vs 2 [0; 3], P = 0.0231). There were no differences between the 2 groups for pain at 24 h and morphine-related complications., Conclusions: We did not find any difference in morphine consumption between the 2 groups. Our results suggest that the combination of PVB and SAPB for video-assisted thoracic surgery or robot-assisted thoracic surgery is safe effective and reliable and could be an alternative to PVB alone in certain indications., (© The Author(s) 2023. 20. Development of metaverse for intelligent healthcare.

Abstract

The metaverse integrates physical and virtual realities, enabling humans and their avatars to interact in an environment supported by technologies such as high-speed internet, virtual reality, augmented reality, mixed and extended reality, blockchain, digital twins and artificial intelligence (AI), all enriched by effectively unlimited data. The metaverse recently emerged as social media and entertainment platforms, but extension to healthcare could have a profound impact on clinical practice and human health. As a group of academic, industrial, clinical and regulatory researchers, we identify unique opportunities for metaverse approaches in the healthcare domain. A metaverse of 'medical technology and AI' (MeTAI) can facilitate the development, prototyping, evaluation, regulation, translation and refinement of AI-based medical practice, especially medical imaging-guided diagnosis and therapy. Here, we present metaverse use cases, including virtual comparative scanning, raw data sharing, augmented regulatory science and metaversed medical intervention. We discuss relevant issues on the ecosystem of the MeTAI metaverse including privacy, security and disparity. Abstract

Aim: Sacral nerve modulation (SNM) is recommended as a first-line surgical therapy for patients with faecal incontinence (FI). During patient follow-up, it is recommended that stimulation settings be reprogrammed to optimize patient outcomes. The aim of the present study was to evaluate the efficacy of stimulator reprogramming in patients with an implanted SNM device to treat FI., Method: The data from patients who received a permanent SNM implant in a single centre from January 2008 to December 2019 were retrospectively analysed. Symptoms that occurred after implantation, the stimulator settings of the SNM device and changes made at each follow-up visit were noted. The efficacy of reprogramming was determined by assessing patient satisfaction., Results: Of the 117 patients (male/female 4/113; mean age 59.5 ± 11.8 years) with a SNM implant for FI, 84 (72%) had at least one symptom requiring reprogramming of the stimulator, most often during the first year after implantation (p = 0.05). The most frequently reported symptoms were loss of efficacy (68.5%; p = 1 × 10 -3 ) and pain (20.5%; p = 1 × 10 -3 ). Reprogramming was effective 53% of the time when treating loss of efficacy and 76% of the time when treating pain. When the stimulation parameters were reprogrammed at least four consecutive times to correct a symptom, the reprogramming was less effective in treating the symptom (p = 0.02)., Conclusion: Regular follow-up of patients with SNM device implants associated with reprogramming of stimulation parameters to improve the treatment of reported symptoms would optimize the efficacy of SNM., (© 2022 The Association of Coloproctology of Great Britain and Ireland.)

Abstract

Background and Purpose: MRA assessment of parent artery patency after flow-diverter placement is complicated by imaging artifacts produced by these devices. The purpose of this study was to assess the accuracy of liver acquisition with volume acceleration-flex technique (LAVA-Flex) MRA in combination with 3D-TOF with HyperSense MRA for the evaluation of parent vessel status after intracranial flow-diverter placement., Materials and Methods: Fifty-six patients treated by flow diversion and followed with both DSA and 3T MRA between November 2020 and August 2021 were included. All patients were evaluated for parent artery patency using the same imaging protocol (DSA, noncontrast MRA including 3D-TOF with HyperSense and LAVA-Flex, and contrast-enhanced MRA, including time-resolved imaging of contrast kinetics MRA and delayed contrast-enhanced MRA)., Results: With DSA as a criterion standard to evaluate the patency of the parent vessel, noncontrast MRA had a good specificity (0.83) and positive predictive value (0.65), better than contrast-enhanced MRA (0.55 and 0.41, respectively). Both had excellent sensitivity and negative predictive value: noncontrast MRA, 0.93 and 0.97, respectively; contrast-enhanced MRA, 0.93 and 0.96, respectively. Specificity and positive predictive value tended to be lower for patients treated with additional devices than for those treated with flow diverters exclusively and for patients treated with a specific type of flow diverter., Conclusions: Noncontrast MRA can be used for noninvasive follow-up of intracranial aneurysms treated by flow diverters.
