Clinical research is important in establishing the effects of health-care interventions. Vaccine clinical trials are to examine the effectiveness and safety of vaccines for the prevention of diseases. Africa has a high burden of infectious diseases such as malaria, tuberculosis, HIV/AIDS, and Ebola virus disease. Here we report a database surveillance study of vaccine-related clinical trials conducted in Africa. An objective is to address and profile vaccine clinical trials conducted in Africa. Data were extracted from the WHO International Clinical Trials Registry Platform on 22 July 2018 and updated on 05 September 2019. We found that 61% of the 377 clinical trials were registered prospectively and 35% registered retrospectively. About 72% of the trials were single-country studies and within the country, most trials (86%) were single-center studies. The proportion of trials involving multiple African countries was 11% and that of trials involving countries outside of Africa was 16%. The biggest funder of the vaccine trials (34%) was industry, followed by governments (25%) and universities (21%). The most studied diseases were malaria (20%), HIV/AIDS (15%), tuberculosis (7%), and Ebola virus disease (6%). Most of the vaccine trials were conducted in adults (42%). The trials ranged from phase I to phase IV, with most of the trials being in phase I (18%) and phase III (18%). The conduct of vaccine clinical trials in Africa seeks to address the disease epidemics faced by the continent. There is a need for more investments from governmental bodies toward vaccine research in Africa. Further, African country collaborations are needed in efforts to find African solutions to the current infectious disease threats faced by the continent.

Objective: To map the different personal positions of Guinean people regarding vaccination against Ebola. Methods: From January to April 2016, 304 adults in Guinea were presented with 48 vignettes depicting situations in which getting vaccinated would be possible. These situations varied as a function of the constructs of health-protective behavior theories. The participants indicated the likelihood they would get vaccinated in each case. Results: Seven qualitatively different positions were found: Always Vaccinate (38%), Never Vaccinate (25%), Hesitant (19%), Depends on Cost Only (7%), Depends on Neighbors' Attitude and Cost (5%), Mainly Depends on Risk (4%), and Mistrust of Cheap Vaccines (2%). Conclusion: The diversity of Guinean people's positions implies that Ebola vaccination campaigns in Guinea, and probably in other sub-Saharan African countries, must not be “one size fits all,” but must be multifaceted and tailored in design and implementation to match the diversity of these people's needs and views.

The protein composition of an Outer Membrane Vesicle (OMV) preparation that constitutes the active pharmaceutical ingredient of VA-MENGOC-BC®, an effective vaccine against Neisseria meningitidis serogroups B, and C is presented. This preparation has a high lipid content and five abundant membrane proteins (FetA, PorA, PorB, RmpM, and Opc), constituting approximately 70% of the total protein mass. The protein composition was determined by combining the use of the Hexapeptide Ligand Library and an orthogonal tandem fractionation of tryptic peptides by reverse-phase chromatography at alkaline and acid pH. This approach equalizes the concentration of tryptic peptides derived from low- and high-abundance proteins as well as considerably simplifying the number of peptides analyzed by LC-MS/MS, enhancing the possibility of identifying low-abundance species. Fifty-one percent of the proteins originally annotated as membrane proteins in the genome of the MC58 strain were identified. One hundred and sixty-eight low-abundance cytosolic proteins presumably occluded within OMV were also identified. Four (NadA, NUbp, GNA2091, and fHbp), out of the five antigens constituting the Bexsero® vaccine, were detected in this OMV preparation. In particular, fHbp is also the active principle of the Trumenba® vaccine developed by Pfizer. The HpuA and HpuB gene products (not annotated in the MC58 genome) were identified in the CU385 strain, a clinical isolate that is used to produce this OMV. Considering the proteins identified here and previous work done by our group, the protein catalogue of this OMV preparation was extended to 266 different protein species.

To determine the safety and immunogenicity of a novel recombinant adenovirus type 5 vector based Ebola virus disease vaccine (Ad5-EBOV) in Africans in China.A phase 1, dose-escalation, open-label trial was conducted. 61 healthy Africans were sequentially enrolled, with 31 participants receiving one shot intramuscular injection and 30 participants receiving a double-shot regimen. Primary and secondary end points related to safety and immunogenicity were assessed within 28 d after vaccination. This study was registered with ClinicalTrials.gov (NCT02401373).Ad5-EBOV is well tolerated and no adverse reaction of grade 3 or above was observed. 53 (86.89%) participants reported at least one adverse reaction within 28 d of vaccination. The most common reaction was fever and the mild pain at injection site, and there were no significant difference between these 2 groups. Ebola glycoprotein-specific antibodies appeared in all 61 participants and antibodies titers peaked after 28 d of vaccination. The geometric mean titres (GMTs) were similar between these 2 groups (1919.01 vs 1684.70 P = 0.5562). The glycoprotein-specific T-cell responses rapidly peaked after 14 d of vaccination and then decreased, however, the percentage of subjects with responses were much higher in the high-dose group (60.00% vs 9.68%, P = 0.0014). Pre-existing Ad5 neutralizing antibodies could significantly dampen the specific humoral immune response and cellular response to the vaccine.The application of Ad5-EBOV demonstrated safe in Africans in China and a specific GP antibody and T-cell response could occur 14 d after the first immunization. This acceptable safety profile provides a reliable basis to proceed with trials in Africa.

This paper offers a conceptual overview of meta-organizations’ role for environmental sustainability in the under-researched African context. The current paper is one of the few studies to offer a specific differentiation regarding the role and activities of meta-organizations for environmental sustainability in developed vs. emerging economies’ settings. The paper further offers an in-depth assessment of meta-organizations’ role for environmental sustainability in Africa, along with discussing major hurdles in this concern. Our analysis reveals several problems with business only meta-organizations in Africa, which significantly limit their role in ensuring environmental sustainability in this region. These problems include competing interests resulting in failure to accommodate multiple stakeholders, lack of responsible investing and environmental stewardship, along with institutional voids. Finally, the paper presents some potential solutions to overcome these problems.

This article explores the importance of national governments and national-local relationships for scaling up local climate action to achieve global goals. From the Sustainable Development Goals to the New Urban Agenda to the Paris Climate Change Agreement, the achievement of global sustainability goals will depend on deep changes to national infrastructure and urban systems. Through an analysis of climate action planning and investments in exemplary cases in Africa, the paper highlights the opportunities and challenges that come with integrating national governments into urban-focused priorities and needs, especially for mobilising financial resources. The paper finds that scaling up city climate action in the selected African countries benefits from a constructive multi-level relationship between local and national institutions and stakeholders to shape and improve the legislative, financial and operating frameworks to enable systemic change. Large-scale urban climate action can be enabled by formal multi-level institutional arrangements, links to politically prioritised policy frameworks, and transformative aims that address both climate and socio-economic benefits for communities.

This paper makes the case for a closer study as to how Africa and African people are portrayed within popular culture in the Global North, adding to the discussion surrounding the importance of ‘low politics’ in producing and reproducing global hierarchies. It focuses on representations of African traditional healers in Anglo-American popular culture as a way of highlighting deeply entrenched outsider ‘understandings’ of the continent. The colonial trope of the ‘African witchdoctor’ has remained a fixture in the collective Anglo-American cultural imagination since the days of H. Rider Haggard. Evidence demonstrates that these colonial stereotypes have remained remarkably durable, having seen little change in over one hundred and fifty years. Following Cynthia Enloe (1996), this paper argues that, in the interests of gaining a deeper appreciation of the power structures shaping African ‘development’, the realm of the ‘imaginary’ and the ‘made up’ where popular culture is concerned, demands greater scrutiny.

Knowledge has emerged as a fundamental driver of economic growth and development by inter alia improving the effectiveness and efficiency of economic projects and boosting the process of finding new avenues of addressing developmental policy syndromes. Recent evidence suggests that Africa is on the threshold of significant and sustainable economic growth if its human and material resources can be effectively mobilised to support the process (Asongu & Tchamyou, 2019; Kuada & Mensah, 2018). Consequently, the World Bank’s Knowledge Economy Framework aims to explore and support the extent to which current policies in African countries affect the knowledge development process (and thereby competitiveness) on the continent. A knowledge economy is an economy in which economic prosperity largely depends on the accessibility, quality and quantity of information available, instead of the means of production (Asongu, 2017a, 2017b). This themed issue of Contemporary Social Science-‘Building Knowledge Economies in Africa’–consists of papers that focus on, but are not limited to, the four dimensions of the World Bank’s Knowledge Economy Index. These are: information and communication technology, education, economic incentives and institutional regime, and innovation (Tchamyou, 2017). The themed issue engages with high quality contributions which, taken together, address the drivers towards knowledge-based economies. This introduction provides a context for understanding the importance of building knowledge economies in Africa and summarises the main contributions to the themed issue. The paper ends by advising scholars and policy makers regarding the risks associated with a colonial view of knowledge- notably the importance of proposing knowledge-based policies while avoiding hegemonic paradigms and hierarchical constructs. In summary, the issue consists of a set of theoretically informed, empirically robust, policy-relevant and accessible articles for both specialists and non-specialists.

Research into psychosis in adolescents is important in Africa in view of the high burden of mental disorders and prevalence of risk factors such as child maltreatment implicated in the etiology of mental health problems. In this paper, we synthesized the existing literature on psychosis in adolescents in Africa to understand current developments and chart a pathway for robust research agenda on psychosis. A search was conducted on electronic databases, including Africa Index Medicus and Scopus and selected journals for papers. Google, Google Scholar, reference list of papers selected for inclusion in the review and study authors were contacted for additional papers and grey literature. A total of 11 articles were included in this review and discussed under the following thematic areas:(1) assessment of psychosis; (2) prevalence of psychosis; (3) demographic/psychosocial correlates of psychosis; (4) clinical correlates of psychosis; (5) substance misuse and psychosis; and (6) psychosis literacy and help-seeking behavior. A pathway for research into psychosis centering on the representation of psychosis, risk and protective factors, management/treatment of psychosis and aforementioned thematic areas was proffered.

Unrestricted, reliable and efficient access to accurate, timely, and up-to-date spatial data may be achieved through a spatial data infrastructure. Thus, most developed countries implemented and continue to develop their spatial data infrastructure. In Africa, there is a growing number of governments committed to spatial data infrastructure development. This paper aims to contribute to initialize an spatial data infrastructure in Mozambique. We identified and characterized through a survey the government institutions producing, sharing, and using spatial data in the country to estimate their potential contribution to the development of the Mozambican spatial data infrastructure. We found 12 institutions producing 15 thematic datasets which can constitute the core of the spatial data infrastructure for Mozambique. Two government agencies have the technical skills and policies to make spatial data available to the public. In the last ten years local radio stations have mushroomed in many regions in Africa, as well in Tanzania. Many refer to them as community radio stations. They receive support from international donors, due to their potential to inform the local population about local issues, to broadcast their voices thus to create a democratic space to publicly debate relevant issues of local people. The paper is based on two empirical research projects on rural radio stations in Tanzania in 2014 and 2015; an exploratory case study on the status of rural radio and a structured content analysis of radio news and programmes of 14 stations. The empirical results will be assessed against a model of economic sustainability of radio stations tested earlier in Uganda. The paper shows that the lack of economic viability is major constraint for local radio stations. They cannot generate sufficient revenues from advertisers to sustain the station. This leads to high turn-over of staff due to low pay, results in low quality of content and lack of capacity to do serious programming, which in the end hinders the achievement of the radio stations’ objectives. It needs to be acknowledged that the challenges of local radio are interrelated, and so are the solutions. The paper will argue that the donor community has largely overlooked the need to invest in media sustainability of rural radio stations, as it has invested mainly in journalism capacity but hardly in media management capacity. The paper concludes that a viable economic model in Africa requires to simultaneously support three different fields, (a) the development of content jointly with (b) the development of media management capacities and (c) local media research covering the extent and satisfaction of the local audience in order to develop local advertising markets that serve local media.

Using manufacturing and services firm-level data for 30 sub-Saharan African (SSA) countries, this paper shows that taxation is not a significant driver for the location of foreign firms in SSA, while other investment climate factors, such as infrastructure, human capital, and insitutions, are. By analyzing disaggregate FDI data, the paper establishes that, while there is considerable contrast in behavior between vertical FDI (foreign firms producing for export) and horizontal FDI (foreign firms producing for local markets), taxation is not a key determinant for either type of FDI. Horizontal FDI is attracted to areas with higher trade regulations, highlighting interest in protected markets. Furthermore, horizontal FDI is affected more by financing and human capital constraints, and less by infrastructure and institutional constraints, than is vertical FDI.

Background There have been increasing calls for collaboration between Indigenous health practitioners (IHPs) and allopathic health practitioners (AHPs) in Africa. Despite this, very few successful systems exist to facilitate formal collaboration. Direct relationships between providers, and at a health systems level are crucial to successful collaboration, but the nature and extent of these relationships have yet to be adequately explored. Objective To explore the relationship between IHPs and AHPs in Africa, and to discuss the implications of this for future collaboration. Methods An interpretive qualitative synthesis approach, combining elements of thematic analysis, meta-ethnography, and grounded theory, was used to systematically bring together findings of qualitative studies addressing the topic of collaboration between Indigenous and allopathic health practitioners in Africa. Results A total of 1,765 papers were initially identified, 1,748 were excluded after abstract, full text and duplicate screening. Five additional studies were identified through references. Thus, 22 papers were included in the final analysis. We found that the relationship between Indigenous and allopathic health practitioners is defined by a power struggle which gives rise to lack of mutual understanding, rivalry, distrust, and disrespect. Conclusion The power struggle which defines the relationship between IHPs and AHPs in Africa is a hindrance to their collaboration and as such could partly account for the limited success of efforts to foster collaboration to date. Future efforts to foster collaboration between IHPs and AHPs in Africa must aim to balance the power disparity between them if collaboration is to be successful. Since this would be a novel approach, decision-makers and organisations who trial this power balancing approach to facilitate collaboration should evaluate resultant policies and interventions to ascertain their feasibility and efficacy in fostering collaboration, and the lessons learnt should be shared.

In a recent intervention, Alejandro Haber and I have questioned the extent to which discussions under the heading of ethics continue to provide a useful framework within which to conceptualise the complex and politically entangled contexts in which archaeologists work. Notwithstanding these concerns, the papers collected here show why discussions under the heading of ethics in archaeology do still matter. I briefly discuss the argument presented by each paper. In concluding, I ask: what would it mean for African archaeologists to articulate a theory of practice? And does the language of ethics best enable us to do this?

As a result of massive scale-up efforts in developing countries, millions of people living with HIV are now receiving antiretroviral therapy (ART). However, countries have been uneven in their scale-up efforts with ART coverage rates exceeding expectations in some places and lagging behind expectation in others. This paper develops a model that explains ART scale-up as a function of the responsiveness of political parties to their primary constituents. Specifically, the paper argues that, faced with a perilous 'threat to the nation', countries responded in one of two ways, both of which were designed to appeal to their primary constituents--either adopting a 'Geneva Consensus' response, or depicting the epidemic as a Western disease and adopting a 'pan-African' response. The article tests this theory using Afrobarometer data for eleven countries. The paper finds that HIV/AIDS is generally a non-partisan issue in most countries. However, the analysis does uncover some differences in partisan support for HIV/AIDS responses in both countries that have adopted Geneva Consensus and pan-African responses, though not in the direction hypothesised. The lack of congruence in policy preferences between the public and their governments suggests a democratic deficit in that these governments have acted independently of the preferences of core constituents.

This paper contextualises counselling within a broader historical formation that links disclosure to healing and deploys confessional technologies to incite disclosure and awareness of the mysterious substance of the self. Foucault's argument that sexuality was the privileged arena for using confessional technologies to ‘produce’ the truth of the self is particularly relevant in light of the diffusion of counselling practices in Africa in the wake of the HIV epidemic, particularly with their emphasis on inciting appropriate sexual behaviour. Examination of the historical assemblage of counselling practices shows how they articulate what the self is, the nature of truth and a politics of language. This paper focuses on the genealogy of four key assumptions that express this confessional reason. These are that: (1) people can be ‘empowered’ to have control on their own lives by working on themselves, (2) secrets untold become pathogens, (3) the ability to heal requires that one first overcome personal illness, (4) the experience of sharing secrets is cathartic and healing. The genealogy intertwines four strands: that of the Unconscious as revealed by Freud and his followers, attempts to treat shell-shocked veterans of World War I, group psychotherapy and participatory research after World War II.El presente artículo contextualiza el asesoramiento dentro de una formación histórica más amplia, que vincula la transparencia con la sanación, utilizando tecnologías confesionales que alientan la transparencia y la conciencia en torno a la misteriosa sustancia del sí mismo. Es particularmente pertinente el argumento de Foucault acerca de que la sexualidad es el espacio privilegiado para el uso de tecnologías confesionales que "producen" la verdad del sí mismo, especialmente a la luz de la difusión de prácticas de asesoramiento en África tras la epidemia del vih y considerando la importancia que las mismas atribuyen al hecho de fomentar un comportamiento sexual apropiado. Foreign investment in agricultural land acquisition in sub-Saharan Africa has been viewed primarily as driven by a set of linked 'crises': in financial capital markets, in security of energy and food supply, and in global environmental governance. This paper argues that a focus on the 'buyers' of land risks overlooking the dynamics that operate on the side of the land 'sellers'. Accordingly, the first part of the paper argues that it is important to view the current 'land grab' as the latest stage in a longer historical process of competition for control of land and other natural resources by different 'domestic' economic and political actors within African countries. While such struggles are often characterised as the 'state versus the peasantry', with the state acting on behalf of 'urban elites', the paper argues that processes of accumulation and associated enclosure of natural resources need to be examined more critically in specific contexts if the role and impact of foreign capital investment are to be understood. The second part of the paper seeks to identify the ways in which questions of scale (in the sense of greater capital intensity) can be considered to be constraints to the development of African agriculture. Particularly, it considers the extent to which the production models most frequently mentioned in connection with foreign investment (large-scale mechanised farms and small-scale outgrower contract farming) respond to current productivity constraints. The paper argues that current debates about foreign investment in agricultural land underplay the importance of water resources needed to overcome production risks associated with irregular rainfall. Bringing the water dimension of land deals more clearly into focus is necessary if the scope for positive and negative impacts of new investment on existing land users is to be fully understood. This paper challenges both public and private African institutions to tap opportunities residing in the diversity of all forms for the development of the continent using the grounded theory approach. African countries are often confronted by challenging socio-economic and demographic situations and set apart by the diversity of their social mechanisms. The paper argues that, for Africa’s development, there is little doubt that diversity management is indispensable presenting a positive scope for innovative “made in Africa” policies. The limitations of this research relate largely to its dependence on success examples that have been noted outside Africa with cultures and origins of non-black descent.

In Africa, research and studies have adversely relegated the effects of inadequate service provision on the poor to the background. Yet, quagmires resulting from severe strike actions, agitations and revolutions are grasped throughout the continent. This paper tends to ponder on the effect of inadequate service delivery on the quality of life of the poor with a focus on education and health in Nigeria. A mixed method was used for data collective; (quasi) quantitative and qualitative medium were exploited to gather data for this paper. A descriptive method based on empirical data and narrative literature analysis was used to demarcate the paper into themes. It found among others that over 70% of inmates in Nigerian prisons are on awaiting trial. It also demonstrated that the tripartite alliance of institutional failure, corruption and poor leadership (self-serving leaders) inhibits the provision of basic services in the country. In conclusion, cronyism, cabalism, nepotism and sycophancy, including inadequate interaction of systems, institutions and structures are decried as the bane of Nigeria’s underdevelopment, which have had adverse negative effects on the educational and judicial sectors thereby undermining the well-being of the poor.

The paper examines the way in which power was routinely exercised in colonial German East Africa in everyday life. In order to achieve domination rather than ruling just by brute force, the German administration set up district councils called Schauri, in which members of the local African élite were invited to act as advisors to the District Officer. The highly formalized meetings of the council were held in public at least once a week. The district officer had the final say in all matters. The council members deliberated the most mundane aspects of colonial rule. Yet the meetings also dealt with more substantive issues, such as the administration of law in the district, local political affairs or the latest public decree from the Governor in Dar es Salaam. The paper argues that the Schauri meetings constituted the most important field of political interaciton and engagement between the German rulers and their African subjects. These meetings were thus an indispensible part of what one might call the 'colonial public sphere'.

Metadata only record The paper analysis five African case studies to answer some questions relating gender, livelihood security and sustainable environments. Men and women face different opportunities and constraints. The paper also identifies the conditions that allow effective involvement in natural resource management, and the development of indicators to clarify progress in terms of impact, process, and sustainability. Results suggest that if policy, programs, and projects are to encourage sustainable, effective, and equitable management of resources, they must focus on the concerns of locals and how they relate to the state, the economy, and the resource base. What is needed is to develop projects that are gender centered and focusing in capacity-building. The data supports the concept that women must play a major role in resource management decisions in order to achieve livelihood security and sustainable environments in Africa. The case studies highlight the importance of: 1. continuous collection, monitoring and evaluation of gender-disaggregated data. 2. Integrations of new technologies combined with gender-aware extension services and training. 3. Capacity building at the local level using local participation and organization, which helps strengthen projects and leads to their sustainability. 4. Impact on livelihood security. 5. Linkages and partnerships related to infrastructure, research, policy, training, and institution building can build project success.

This text introduces the themed issue “Imperialism, Internationalism and Education in Africa: Connected Histories”. It provides an overview of the history of education in twentieth-century Africa, highlighting its major problems, themes, and actors. Arguing for a transnational and connected approach to this field of research, it also explains how the four papers included in the issue allow to address the multiple entanglements between educational internationalism and imperialism in the late colonial and early postcolonial period

African countries seek to intensify their industrialization while also increasingly pursuing green growth and, more recently, circular economy strategies. The competitive implications of this are, however, little researched empirically. We seek to investigate how African countries, examplified in this paper by Kenya, are experiencing a green and circular structural change of their economies. On the basis of early findings from a survey among 27 mixed manufacturing companies in Ruaraka industrial area in Nairobi, we highlight green and circular innovation trends, conditions and dynamics for different types of industries and firms (size). We apply a strong business perspective in framing the questions. We conclude that the companies are reaching a moderate stage of greening,although with a high degree of variability. Circular innovations are relatively widespread and ensuring resource supply seems to be a more important incentive than cutting costs. There seems overall to be quite strong business incentives to go circular among Kenyan companies, although these incentives are not necessarily realized by the companies, a factor that could be utilized in policymaking. We suggest a strong business-oriented survey methodology as a way forward to expand insights into the greening of industries in Africa and similar economies.

To understand the impact of the COVID-19 public health response on families of children with disabilities in Central Uganda we conducted phone interviews with parents and children during the first 5 months of the outbreak (March - July 2020). Most parents and children were well informed about COVID-19 and were keen to adhere to government prevention measures. The majority said lock-down measures had a negative effect on their mental and physical health, social life, finances, education and food security. Access to medical services and medication for chronic illness had been limited or absent due to restrictions in travel, some facilities restricting access, and limited financial resources. The majority of parents reported loss of work which resulted in difficulties in finding enough food and paying rent. Parents worried about children missing education and friends. We suggest greater attention to children with disabilities and their families when implementing mitigating and long-term responses.Points of interest This paper reports a study with families of children with disabilities in Uganda during the Coronavirus pandemic in 2020, known as COVID-19. Families of children with disabilities in Uganda are well informed about COVID-19 and try to follow prevention measures. Families of children with disabilities have difficulties meeting daily basic needs as they were unable to work and had no income during the COVID-19 related lock down. The COVID-19 response affects access to health and rehabilitation services for children with disabilities in Uganda. Parents of children with disabilities struggle with home education and learning due to lack of access to accessible learning materials and learning support in Uganda. The COVID-19 response affects the peer support networks and social support for parents of children with disabilities in Uganda. Children with disabilities and their families should be involved and considered in the development and implementation of the COVID-19 response.

Traditional approaches to development programming with fixed targets and outcomes do not fit complex problems where the pathway to achieve results differs in each context and evolves constantly. Adaptive programming improves responses to complex problems by identifying which solutions bring change. This paper reviews the theory behind adaptive programming approaches and introduces the 'Pathways of Change' tool for achieving sustainability results, developed for the multi-country Women's Integrated Sexual Health programme. Qualitative data, using semi-structured interviews and group discussions from teams in over 17 countries in Africa and South Asia, are presented which examine the application of the Pathways of Change (PoC) tool focusing on successes and challenges across different intervention areas. The PoC responds to the need for a more practical adaptive programming tool that can be tailored to support flexibility in global health programme implementation while meeting donor requirements. Findings suggest that the PoC tool provides a flexible yet robust alternative to traditional monitoring frameworks and is able to facilitate adaptive, contextualised planning and monitoring for multi-country programmes. The PoC tool offers a solution to realise the ambitions of implementing adaptive programming within global health programmes and potentially beyond.

This qualitative evaluation is based on data gathered during field trips to Uganda between 1981 and 1988. The first part of the paper describes the official view on prevalence (in 1988 Uganda, with 5455 reported cases, had the third highest prevalence of AIDS in Africa with the disease transmitted largely through heterosexual relations), the official view on the disease origin (the index case of AIDS occurred in 1983 in Kasensero village in the Rakai district), and the official control efforts (acknowledgement and publication of the magnitude of the epidemic and a 5-year prevention and control program with educational, medical, and epidemiological measures). The second part of the paper discusses public perceptions towards AIDS and its victims and highlights beliefs that assign a supernatural etiology to AIDS, social discrimination of AIDS victims, the consequences of the disease on the family and on sexuality and nuptiality. The next section looks at the impact of AIDS on health seeking behavior and the delivery of health care and notes that the private health delivery system has probably contributed to the spread of AIDS because of the use of unsterilized instruments, that unscrupulous practitioners are claiming cures, and that some people avoid health facilities for fear of infection. The fourth topic considered is the risk of HIV transmission because of sexual harassment perpetrated against women. This is followed by an assessment of 1) the affect of AIDS on women's reproductive rights and the controversy about transmission of the virus in breast milk, 2) the situation faced by women in high-risk professions, and 3) the acceptability of the condom. The paper ends by summarizing the policy and research implications revealed by this qualitative study.

This synthesis brings together findings on cause-specific mortality documented by means of verbal autopsies applied to over 110,000 deaths across Africa and Asia, within INDEPTH Network sites.Methods: Developments in computerised methods to assign causes of death on the basis of data from verbal autopsy (VA) interviews have made possible these standardised analyses of over 110,000 deaths from 22 African and Asian Health and Demographic Surveillance System sites in the INDEPTH Network. In addition to previous validations of the InterVA-4 probabilistic model, these wide-ranging analyses provide further evidence of the applicability of this approach to assigning the cause of death. Plausible comparisons with existing knowledge of disease patterns, as well as substantial correlations with out-of-model parameters such as time period, country, and other independent data sources were observed.Findings: Substantial variations in mortality between sites, and in some cases within countries, were observed. A number of the mortality burdens revealed clearly constitute grounds for public health actions. At an overall level, these included high maternal and neonatal mortality rates. More specific examples were childhood drowning in Bangladesh and homicide among adult males in eastern and southern Africa. Mortality from non-communicable diseases, particularly in younger adulthood, is an emerging cause for concern. INDEPTH's approach of documenting all deaths in particular populations, and successfully assigning causes to the majority, is important for formulating health policies.Future directions: The pooled dataset underlying these analyses is available at the INDEPTH Data Repository for further analysis. INDEPTH will continue to fill cause-specific mortality knowledge gaps across Africa and Asia, which will also serve as a baseline for post-2015 development goals. Background : The MRC/Wits University Agincourt research centre, part of the INDEPTH Network, has documented mortality in a defined population in the rural northeast of South Africa for 20 years (1992–2011) using long-term health and socio-demographic surveillance. Detail on the unfolding, at times unpredicted, mortality pattern has been published. This experience is reviewed here and updated using more recent data. Objective : To present a review and summary of mortality patterns across all age-sex groups in the Agincourt sub-district population for the period 1992–2011 as a comprehensive basis for public health action. Design : Vital events in the Agincourt population have been updated in annual surveys undertaken since 1992. All deaths have been rigorously recorded and followed by verbal autopsy interviews. Responses to questions from these interviews have been processed retrospectively using the WHO 2012 verbal autopsy standard and the InterVA-4 model for assigning causes of death in a standardised manner. Results : Between 1992 and 2011, a total of 12,209 deaths were registered over 1,436,195 person-years of follow-up, giving a crude mortality rate of 8.5 per 1,000 person-years. During the 20-year period, the population experienced a major HIV epidemic, which resulted in more than doubling of overall mortality for an extended period. Recent years show signs of declining mortality, but levels remain above the 1992 baseline recorded using the surveillance system. Conclusions : The Agincourt population has experienced a major mortality shock over the past two decades from which it will take time to recover. The basic epidemic patterns are consistent with generalised mortality patterns observed in South Africa as a whole, but the detailed individual surveillance behind these analyses allows finer-grained analyses of specific causes, age-related risks, and trends over time. These demonstrate the complex, somewhat unpredicted course of mortality transition over the years since the dawn of South Africa's democratic era in 1994. Africa is still struggling to mitigate its electricity insecurity issues. This situation renders foreign direct investment in the renewable electricity industry (FDIREI) and renewable electricity production (REP) to become simultaneously important to Africa. Using a novel dataset of FDIREI, this paper examines the existence and nature of cointegration and causality nexuses among FDIREI, REP, and economic growth (GDP) in 32 African countries over 2003–2019. For methodological robustness purposes; GDP is added. By applying the panel vector autoregression model based-Granger causality test and a static panel data model, which are followed by robustness tests, more informative results are reported. Importantly, we find evidence of the growth hypothesis between REP and GDP, as a unidirectional Granger causality is seen from REP to GDP. Further, the neutrality hypothesis is confirmed among the remaining variables. This left us with the importance of REP in revitalizing African countries’ economic growth. All facets of REP thus should be enhanced.

African countries are among the prime destinations of electronic waste (e-waste) also called Waste of Electrical and Electronic Equipment (WEEE), and have been challenged with the management of its environmental and health impacts. This paper was carried out to understand the e-waste sector and policy responses in selected African countries. Data for the study were generated from sources; such as policy documents, legislations and literature. Findings show that the import of WEEE is on rising in Africa while landfill and incineration continued to be widely used handling approaches. Countries studied lack WEEE specific national policies and stringent policy instruments to enforce proper collection and recycling systems. Despite the start-ups in emerging recycling operations, a major gap is that informal e-waste actors dominate the e-waste chain from collection to material extraction and refurbish activities through rudimentary tools that cannot detect toxic elements. Tackling the problem demands integrated multi-actor interventions with multiple stakeholders to reduce WEEE inflow on one hand, and ramping up safe recycling capacity on the other hand.The article attempts to explain the electronic waste problem in African countries, the nature of existing policy responses and limitations, and ways forward to address policy gaps. Electronic waste is a global problem but with local impacts with the hazardous substances it contains. Because E-waste is still not well recognized health and environment threat, less attention is given for the problem especially in African countries making the uneducated youth more vulnerable to toxic elements. The epistemic community, hence, is supposed to write about it and develop knowledge so that evidences for policy decision making would grow. The focus is on Africa because the problem needs special attention. E-waste has been dumped in Africa for long time and people who work with such waste are mostly uneducated and vulnerable to toxic substances. This problem requires certain attention in the scholarly and policy community at the international level.

Toxic waste is chemical compounds that, when ingested or inhaled, can cause physiological impairment and, in extreme cases, death. It is also known for its detrimental effect on the environment when disposed of in an unsafe manner. Yet, countries in the Gulf of Guinea continue to be targeted by Western waste-brokers notwithstanding the existence of laws prohibiting the transboundary disposal of such materials. There is also a rise in the export of electronic waste (e-waste) from developed countries to countries in the region, purportedly as reusable electronics, much of which ends up in landfills. The primitive recycling techniques of this e-waste undermines the health of the local populations and their environment due to inadequate care of the heavy metal and toxin content. Drawing on examples from Côte d’Ivoire, Nigeria and Ghana, this paper argues that toxic waste dumping in the Gulf of Guinea amounts to environmental racism. The article makes recommendations relating to the challenges of toxic waste dumping in the Gulf of Guinea, including the need for countries to implement the provisions of the Basel and Bamako conventions in their entirety, recognise acts of environmental racism as violations of human rights, and for young people to rise to the occasion as ‘agents of change’. A wide range of evidence shows systematic differences in health status among social groups, which are associated with unequal exposure to and distribution of the social determinants of health (SDH). However, the role of these SDH has not been studied extensively in low-income countries, where most studies focus on access to medical care. In this paper, we undertake a retrospective cross-sectional analysis of the SDH in Mozambique for the period 2002-2014 based on a set of household budget surveys, covering 152,259 observations. We run logistic regression models, comparing various sets of determinants of health status proxied by two measures: self-assessed overall health and number of days ill. The results, which include models stratified by sex, age and year, consistently show that neither better access to healthcare nor material conditions are related to better health. Rather, macro factors, proxied by place of residence, are the dominant predictor of health inequalities. In the stratified models, women and elders appear more vulnerable to poor health. The policy implication is that a narrow focus of health policy on selected services ignores the underlying economic and social context and their role in producing well-being or poor health. More integrated approaches are required to address the health equity gap in low-income countries.

During the past decade, sizable investments have been made to strengthen maize (Zea mays L.) seed production in Eastern and Southern Africa by private seed companies. However, efforts have generally overlooked downstream issues, such as how seed companies market their products and position their business in a competitive market. This paper assesses competition and customer preferences in Kenya at the retail level for varieties from the parastatal, private domestic and international companies. Data were collected from agro-dealer surveys (n = 80) and farmer intercept interviews (n = 377). Compared to the market leader, the parastatal Kenya Seed Company, private domestic and international companies provide greater value to farmers by selling varieties that are, on average, 10 years younger. However, these companies offer few late-maturing varieties, thus giving the parastatal a near monopoly in that market segment. Kenya Seed Company also excels at being present at all sales locations with their varieties. If private domestic companies focus more on smallholders with lower budgets (including travel budget), they should consider the most cost-effective ways to extend their networks to remote areas as well as compete with the lower prices of Kenya Seed Company. Modern breeding programs should explicitly consider these marketing challenges when designing strategies for seed production and engagement with seed companies.

Preparation and coordination have come to dominate discourse on operational excellence in post disaster supply chain management. In this paper we explore the critical success factors (CSF) of operations excellence in post-disaster operations management in Africa, using in-depth interviews with disaster management practitioners. Available studies have often focused on the commercial supply chains, but rarely on the critical success factors in service excellence in post disaster management operations in Africa. Using in-depth semi-structured interviews and desk-top data collection techniques from disaster management experts in Ghana a number of factors were identified. Employing thematic data analysis technique, these factors were categorised into eight (8) themes: management and administration, resources, political, governance structure, socio-cultural, education and training, infrastructure, and stakeholder involvement and co-operation.

For the past 30 years, many communities and countries in sub-Saharan Africa have suffered from the ravages of the HIV epidemic. During this time the mortality rates in HIV-infected people have been 10–20 times higher than in HIV-uninfected people . Since the advent of anti-retroviral therapy (ART), mortality and morbidity have decreased considerably . However, it has been difficult to get reliable estimates of the impact of ART in population-based studies. The measurement of the impact of ART on mortality requires long-term follow-up in communities where regular HIV testing allows the estimation of mortality by HIV status, such as the population cohorts in the network for Analysing Longitudinal Population-based HIV data in Africa (ALPHA) . An alternative estimation from verbal autopsy (VA) requires reliable tools and consistent interpretation of causes of death, which have been taken forward by recent standards from the World Health Organization (WHO) and the development of the corresponding InterVA-4 model for cause of death assignment . Background : Despite indications that infection-related mortality in sub-Saharan Africa may be decreasing and the burden of non-communicable diseases increasing, the overwhelming reality is that health information systems across most of sub-Saharan Africa remain too weak to track epidemiological transition in a meaningful and effective way. Proposals : We propose a minimum dataset as the basis of a functional health information system in countries where health information is lacking. This would involve continuous monitoring of cause-specific mortality through routine civil registration, regular documentation of exposure to leading risk factors, and monitoring effective coverage of key preventive and curative interventions in the health sector. Consideration must be given as to how these minimum data requirements can be effectively integrated within national health information systems, what methods and tools are needed, and ensuring that ethical and political issues are addressed. A more strategic approach to health information systems in sub-Saharan African countries, along these lines, is essential if epidemiological changes are to be tracked effectively for the benefit of local health planners and policy makers. Conclusion : African countries have a unique opportunity to capitalize on modern information and communications technology in order to achieve this. Methodological standards need to be established and political momentum fostered so that the African continent's health status can be reliably tracked. This will greatly strengthen the evidence base for health policies and facilitate the effective delivery of services. Comprehensive sexuality education (CSE) is a key component of efforts to improve sexual and reproductive outcomes for young people. While many governments have established policies and curricula for CSE, there are no quantitative measures of the quality of their implementation in schools. We describe the construction of a school-level index to measure CSE implementation quality using data from Peru, Guatemala, Kenya and Ghana for validation. The composite index represents seven key components of a successful programme: range of topics, values imparted, teaching methods, teacher training, resources available, monitoring and evaluation, and the school environment. These components are combined into two sub-indices (curriculum content and implementation context), which together provide a summary index of implementation quality. We present scores for the seven components, the two sub-indices and the summary index for each country. Ghana scored highest on the summary index, while Guatemala scored lowest, but country rankings varied within the different components. The paper discusses the implications of the findings for improving school-based CSE in these countries. An assessment of the validity and reliability of the index indicates it can be used in other settings and could be a useful tool for educationalists, researchers and programme implementers to measure CSE implementation quality.

In this article, I discuss compulsory genital examinations in Swedish African, mainly Somali, girls. The discussion is based on data from 122 police files, including criminal investigations regarding suspected “female genital mutilation” (FGM). A growing body of research in European countries indicates that processes of cultural change are occurring among immigrant communities from areas where traditionally girls are subjected to what is construed as “circumcision”. Many studies show growing opposition to these practices among people who have migrated to Europe, and there is little evidence to support the assertion that large-scale illegal activities are prevalent. Yet there is a dominant discourse stating that FGM is secretively practised on a large scale among some immigrant groups in Europe, and policies encourage the detection of cases to charge in criminal court. I describe the current situation in Sweden and highlight some of the drawbacks of a very harsh, although well-intended, policy to check for FGM in Europe. While the ultimate aim is to protect girls at risk for FGM, current policies have ramifications that are invasive and sometimes even traumatising for the girls involved. This paper offers an empirical example of how politics in western multicultural societies may negatively influence the sexual health and rights of a target group, in this case, girls and young women whose families originate from countries where circumcision of girls is practiced.

Since 2005, international civil society support has faced increasing resistance around the world. Ethiopia is widely recognized as a key example of this so-called Closing Space phenomenon. With the 2009 Charities and Societies Proclamation (CSP) Ethiopia has established strict regulations on civil society organizations that, in particular, restrict the ability of local associations to make use of foreign funding and the range of activities allowed for foreign (funded) organizations. This paper traces the process of international negotiations that has accompanied the drafting of the CSP and identifies the consequences of these negotiations for international civil society support in the country. Focusing on the interaction between foreign “donors” and the Ethiopian government, it analyzes (a) what precisely these negotiations have been about, (b) to what extent these negotiations have actually influenced the content of the CSP, and (c) how the CSP as finally adopted has actually affected international civil society support in Ethiopia.

Despite having convincingly linked colonial extractive institutions to African current poverty, the literature remains unclear about which exact institutions are to blame. To address this research question, in this paper I identify trade policies as one of the main components of colonial extraction by showing their long-term effects on African economic growth. By using the gap between prices paid to African producers in the French colonies and competitive prices as a measure of rent extraction via trade monopsonies, I find a negative correlation between such price gaps and current development. This correlation is not driven by differences in geographic characteristics or national institutions. Moreover, it cannot be explained by the selection of initially poorer places into higher colonial extraction. The evidence suggests that trade monopsonies affected subsequent growth by reducing development in rural areas and that these effects persisted for a long time after independence.

This paper examines the role the digital economy plays in international trade impacts on Africa’s economic growth based on 53 countries’ sample from 2000–2018. We further divided the sample into five sub-regions, and the results are estimated by POLS, random and fixed effects, and the GMM models. The findings showed that (1) trade only has positive effects on economic growth when interacted with the digital economy in the POLS estimations, (2) Trade has a significantly positive impact on economic prosperity without and with the interactive term in the RE, FE, and the sys-GMM estimations, (3) the output elasticities of capital and labor have positive and negative impacts on economic growth, respectively, (4) the regressions for the sub-sample yielded statistically significant differences in the output elasticities for the indicators. The study recommends that concentrated efforts be directed towards developing the digital economy to ensure international trade’s full economic effect in Africa.

Genomic research and biobanking present several ethical, social and cultural challenges, particularly when conducted in settings with limited scientific research capacity. One of these challenges is determining the model of consent that should support the sharing of human biological samples and data in the context of international collaborative research. In this paper, we report on the views of key research stakeholders in Ghana on what should count as good ethical practice when seeking consent for genomic research and biobanking in Africa. This study was part of a multi-country qualitative case study conducted in three African countries: Ghana, Uganda and Zambia under the auspices of the Human Heredity and Health in Africa initiative (H3Africa). Our study suggests that while participants are willing to give consent for their samples and associated data to be used for future research purposes, they expect to receive feedback about the progress of the research and about the kinds of research being undertaken on their samples and data. These expectations need to be anticipated and discussed during the consent process which should be seen as part of an ongoing communication process throughout the research process.

Introduction: Mentoring programs are frequently recommended as innovative and low-cost solutions, and these have been implemented in many healthcare institutions to tackle multiple human resource-related challenges. This review sought to locate, appraise and describe the literature reporting on mentorship programs that were designed to improve healthcare worker competence and institutional performance in Africa. Methods: This review searched and synthesized reports from studies that assessed the effectiveness of mentorship programs among healthcare workers in Africa. We searched for studies reported in the English language in EMBASE, CINAHL, COCHRANE and MEDLINE. Additional search was conducted in Google Scholar. Results: We included 30 papers reporting on 24 studies. Diverse approaches of mentorship were reported: a) placing a mentor in health facility for a period of time (embedded mentor), b) visits by a mobile mentor, c) a mentoring approach involving a team of mobile multidisciplinary mentors, d) facility twinning, and e) within-facility mentorship by a focal person or a manager. Implication for practice: Mentoring interventions were effective in improving the clinical management of infectious diseases, maternal, neonatal and childhood illnesses. Mentoring interventions were also found to improve managerial performance (accounting, human resources, monitoring and evaluation, and transportation management) of health institutions. Additionally, mentoring had improved laboratory accreditation scores. Mentoring interventions may be used to increase adherence of health professionals to guidelines, standards, and protocols. While different types of interventions (embedded mentoring, visits by mobile mentors, facility twinning and within-facility mentorship by a focal person) were reported to be effective, there is no evidence to recommend one model of mentoring over other types of mentoring. Implications for research: Further research-experimental methods measuring the impact of different mentoring formats and longitudinal studies establishing their long-term effectiveness-is required to compare the effectiveness and cost-effectiveness of different models of mentoring. Further studies are needed to explore why and how different mentoring programs succeed and the meaningfulness of mentoring programs for the different stakeholders are also required.

Morocco is a country with one of the highest migration rates in the world. With 29% of Moroccan films dealing with migration as the main topic, the objectives of this paper are to establish the most recurrent migratory thematic lines and to determine how migratory topics have evolved from 1958 - the year in which the first postcolonial film was produced- to 2015. A sample of 96 migratory films included in the Moroccan Cinematographic Center´s catalogue has been explored by way of content and interpretative discourse analysis. The methodology has been complemented with in-depth interviews with ten Moroccan directors. The most active production period occurred from 2008 to 2011. The largest amount of films belongs to the category ‘migrations abroad’ (18 films, 18,7%). Secondly, the most talked about topic has been the ‘return home’, (16 films, 16.6%), and thirdly, that of ‘life abroad’. Furthermore, Moroccan cinema has covered all the different phases of migration (rural exodus, migrations abroad, life abroad and returning home) in a very balanced way, showing coherent cinematographic reaction mechanisms towards social reality.

Botswana has long been praised for its financial and political achievements. High economic growth rates and uninterrupted democratic governance since independence in 1966 have led to Botswana's labeling as the ‘African Miracle’. Long before Botswana's emergence as a darling of Western development agencies however, Tswana elites and colonial officials also saw Botswana as exceptional: surrounded by states divided along racial lines, these individuals sought to construct a nation organized around principles of racial and tribal unity. Aspirations of non-racialism were to be exemplified in Botswana's newly constructed capital city, Gaborone. At the same time, underlying the planning vision for Gaborone was a competing set of narratives, practices and aspirations that undercut these lofty ideals and resulted in the creation of a city highly stratified by racial segregation. This essay identifies three complementary urban planning rationales that produced urban exclusion in Gaborone: the desire to build Gaborone as an administrative capital, borrowing from both colonial and indigenous Tswana traditions that privileged spatial divisions related to status and race, and the goal to build a ‘modern’ urban center to lead Botswana into the future. These tensions divided the city in ways both familiar and unexpected and set the parameters determining who counts as a legitimate resident of the city. The paper, therefore, seeks to explore how a city founded on an ideal of racial unity instead became a site of stark division(s).

Background: Dietary patterns are often considered as one of the main causes of non-communicable diseases worldwide. It is of utmost importance to study dietary habits in developing countries since this work is scarce. Objective: To summarize the most recent research conducted in this field in African countries, namely the most used methodologies and tools. Methods: A systematic review was conducted on MEDLINE/PubMed, aiming to identify scientific publications focused on studies of dietary intake of different African populations, in a ten-year period. Papers not written in English/Portuguese/Spanish, studies developed among African people but not developed in African countries, studies aiming to assess a particular nutrient/specific food/food toxin and studies that assessed dietary intake among children were excluded. Findings: Out of 99 included studies, the 24-hour recall and the food-frequency questionnaire were the most used dietary intake assessment tools, used to assess diet at an individual level. It was also observed that often country-unspecific food composition databases are used, and the methodologies employed are poorly validated and standardized. Conclusions: There is an emergent need to improve the existing food databases by updating food data and to develop suitable country-specific databases for those that do not have their own food composition table.

The paper reports on a participatory action research study conducted in six rural primary schools in Uganda in 2013 to establish why children taught in the local language had difficulties in reading and writing. Findings through interviews, focus group discussions, reviews of exercise books and lesson observations indicated that though it was easier for pupils to learn the concepts in the local language; challenges ranging from poor translation, inadequate teachers' language proficiency, lack of instructional materials, high pupils' enrolment, lack of administrative support and teacher-centred approach of teaching, affected pupils' learning to read and write. Participants recommended adopting the child-centred pedagogy, incorporating instructional materials, conducting continuous assessment and recording pupils' competencies attained in reading and writing. Teachers need to engage more in Participatory action research in order to reflect on their practices and pupils' learning, and collaboratively decide what works best and what needs improvement in their classrooms.

The current knowledge of the diversity and distribution of tardigrade species in northern Africa is scarce and far from satisfactory. In this paper we review old and provide new faunistic records for various regions of Tunisia. Among the newly recorded taxa, we found a new heterotardigrade Bryodelphax maculatus sp. nov. The new species belongs to the weglarskae group, characterised by the presence of ventral plates, but differs from its congeners by a different arrangement of ventral plates and by conspicuous groups of cuticular tubercles on the ventral cuticle. The analysis of larval, juvenile and mature instars of the new species revealed, for the first time in heterotardigrades, ontogenetic variability in the development of ventral armature.Keywords: Africa, biodiversity, ontogenetic variability, Palearctic, Tardigrada, ventral plates

Driven by long-standing ties with India, and intertwined with global market interests, a growing influx of materia medica from South Asia is currently emerging in East Africa. To compete at a global level and framed within the language of 'South-South cooperation for mutual benefit', India presents itself as an appropriate exporter of medical products suitable for African clientele. The expansion of India's medical sector into Africa is particularly relevant for Ayurvedic pharmaceuticals. Ayurveda's diaspora into Africa exemplifies the emergence of new health interventions and medical assemblages in a transnationally interconnected world and implies innovative coalitions involved in the complex promotion of local/Global Health industries. By tracking the flow of Ayurvedic pharmaceuticals to East Africa, this paper aims to contribute to the understanding of medical practices as they are configured by the dynamics of global mobility. It interrogates the extent to which the industrialisation of Ayurveda occupies a strategic position in Indo-African discursive practices and the creation of new market opportunities. Moreover, it explores the ways in which South-South alliances between Indian and African actors produce (new) solidarities, but also hierarchies and power imbalances. Special attention will be directed to the political-economic implications of the expansion of transnational markets.

However, studies assessing the levels and changes of these services at national level are rare. In this paper, we estimate past and future changes in multiple ES and biodiversity, as a consequence of land cover change (LCC) in Mozambique. Water yield, water quality, erosion regulation, climate regulation, and biodiversity were modeled using a spatially explicit approach. Changes in ES and biodiversity were mapped at province level between 2005 and 2009. Through the use of a land change model, land cover was projected for 2025, and the resulting impacts on ES and biodiversity were estimated. We found a moderate increase in climate regulating service between 2005 and 2009. However, water quality (nutrient retention) and biodiversity decreased. LCC for 2025 is expected to have a similar impact on these ES. This study contributes with a methodology that can be useful for monitoring ES and assist decision policies affecting ES provision and trade-offs. publishersversion published
