Despite established links among persistent unemployment, low wages, and children's economic well-being, social scientists have yet to document how variability in work hours is linked to child poverty. Our knowledge of the safety net's heterogeneous responses to work-hour instability is also limited. This is of critical importance for scholars and policymakers. Using nationally representative data collected every 4 months, this paper examines how intra-year work-hour volatility is related to child poverty, measured through both the official poverty measure (OPM) and the supplemental poverty measure (SPM). It further assesses varying degrees of buffering effects of cash, in-kind benefits, and tax transfers on income in the context of work-hour volatility. Results indicate that more than one in four households (26%) facing the greatest volatility lived under the poverty line. Black and Hispanic children, as well as those living with unpartnered single mothers, faced substantially higher variability in household market hours worked. Hispanic children experienced not only greater volatility in their caregivers' work hours but also higher poverty level, even after taking government programs into account. In-kind benefits are more effective in buffering household income declines resulting from unstable work hours, followed by tax transfers and cash benefits. The effectiveness of near-cash benefits is particularly salient among Black children and children of single mothers. These results provide new evidence to inform policy discussions surrounding the best ways to help socioeconomically disadvantaged families to retain benefits and smooth their income in the face of frequent variation in work hours and, thus, earnings. This paper presents the rationale for a four-asset growth policy model, which the US economy has executed only on a partial and intermittent basis over the past four decades. The need for such a model is driven by (1) growing global competition, (2) resulting slower gross domestic product and personal income growth, and (3) a consequent rise in income inequality. Without a more effective growth model to both broaden and deepen the role of technology, declining growth and its skewed distribution among income classes will perpetuate current social and political unrest. To achieve both broader and deeper technology-based growth across the economy, the proposed technology element model specifies the four major asset categories that characterize technology-based economic development, which combine to drive more effective growth policy analysis and development. The Preconception Period Analysis of Risks and Exposures Influencing Health and Development (PrePARED) Consortium creates a novel resource for addressing preconception health by merging data from numerous cohort studies. In this paper, we describe our data harmonization methods and results. Individual-level data from 12 prospective studies were pooled. The crosswalk-cataloging-harmonization procedure was used. The index pregnancy was defined as the first postbaseline pregnancy lasting more than 20 weeks. We assessed heterogeneity across studies by comparing preconception characteristics in different types of studies. The pooled data set included 114,762 women, and 25,531 (22%) reported at least 1 pregnancy of more than 20 weeks' gestation during the study period. Babies from the index pregnancies were delivered between 1976 and 2021 (median, 2008), at a mean maternal age of 29.7 (standard deviation, 4.6) years. Before the index pregnancy, 60% of women were nulligravid, 58% had a college degree or more, and 37% were overweight or obese. Other harmonized variables included race/ethnicity, household income, substance use, chronic conditions, and perinatal outcomes. Participants from pregnancy-planning studies had more education and were healthier. The prevalence of preexisting medical conditions did not vary substantially based on whether studies relied on self-reported data. Use of harmonized data presents opportunities to study uncommon preconception risk factors and pregnancy-related events. Summary: This article reports on findings that indicate how First Nations musical activities function as cultural determinants of health. Drawing on early findings from a 3-year Australian Research Council funded project titled The Remedy Project: First Nations Music as a Determinant of Health, we detail Australian and Ni Vanuatu First Nations musicians' reported outcomes of musical activity using a First Nations cultural determinants of health framework. The broader findings indicate that our respondents see musical activity as actively shaping all known domains of cultural health determinants, and some surrounding political and social determinants. However, this paper focusses specifically on the political and economic determinants that emerged in analysis as the most dominant subthemes. We argue that this study provides strong impetus for continued investigation and reconceptualization of the place of music in cultural health determinant models. Lay Summary: This article looks at how making and performing music, recording music and listening to music helps the health of First Nations peoples in Australia and Ni Vanuatu. Music is an important part of the lives of First Nations peoples from these places and so research was done to try to understand why it is meaningful. Music can be used as an outlet for personal feelings, and can also be a way that groups of people can express common concerns. First Nations musicians talked about how music makes them feel, and how music is used to strengthen relationships between people, and between people and their culture. Musicians also talked about how music helps them express their political and economic goals. The findings backed up existing First Nations' models of health that say that health for First Nations People's needs to be thought about in a holistic way. The findings also showed that the relationship between music and health needs to be studied more so that we can better understand how it helps maintain links with the past, gives a guide for the present and opens options for the future. Access to safe drinking water is critical in improving health and well-being. It is estimated that >40% of urban households in Ghana do not have access to safe drinking water. Although the willingness and ability of community members to collectively take local initiatives are essential to curtailing inequities in water access in Ghana, the determinant of collective action is less explored. This paper explores determinants of collective action in water-insecure neighbourhoods and examines how perceived inequities in access to water and trust mediate the relationship between lack of access to water and collective action in urban Ghana. The results show that the urban poor OR = 12.047 (p = 0.000) were more likely to participate in water-related collective action compared to wealthy individuals. Primary decision-makers were 1.696 times more likely to participate in collective (p = 0.02). We also found that perceived inequities OR = 0.381 (p = 0.00) significantly predict participation in collective action to address water insecurity. Parental child-related spending represents an important pathway to promote children's human capital development and material well-being. It is also, however, a pathway that perpetuates social inequalities given the well-known socioeconomic divide in parental spending on children. Although several studies have documented the extent to which current family income is associated with spending on children, we still know little about the causal effects of additional income on child-related spending across the socioeconomic strata. In this paper, I examine the effects of income gains due to universal and unconditional cash transfers from the Alaska Permanent Fund Dividend on child-related spending. Using longitudinal household expenditure data from the 1996–2015 Consumer Expenditure Surveys, I exploit exogenous variation in the generosity of the Alaska Dividend to estimate short- and long-term disparities in parental expenditures resulting from payouts. Results provide a unique opportunity to compare the behavior of high- and low-income parents after an income boost. I find that low- and middle-income parents use cash transfers to catch up with their more affluent counterparts in the short and long term. Low-income parents are, however, unable to keep up with the long-term increases in child-related spending made by their middle-income counterparts. Objectives Previous research in the United States suggests contextual income mobility may play a role in explaining the disparities between life expectancy in the United States and peer countries. This article aims to extend previous research by estimating the consequences of average individual exposure to mobility regimes during childhood and adolescence on adult health. Methods This study draws its data from two longitudinal datasets that track the county of residence of respondents during childhood and adolescence, the Panel Study of Income Dynamics and the National Longitudinal Survey of Youth 1997. We implement marginal structural models to assess the association of the average exposure to county income mobility on five health outcomes and behaviors. Results The results are only partially consistent with a systematic association between exposure to income mobility and health outcomes. Evidence obtained from the National Longitudinal Survey of Youth suggests less income mobility might increase the probability of smoking by age 30. Discussion The paper provides a precise assessment of the hypothesis that childhood exposure to income mobility regimes may influence health status through behavior later in life and contribute to longevity gaps. Only partial evidence on smoking suggests an association between income mobility and health, so we discuss potential reasons for the disparities in results with previous research. In our recent paper,[1] we examined associations between family income during childhood and adolescence on subsequent risks of developing psychiatric disorders, having substance misuse problems and being arrested for a violent crime. In fact, the point estimates for the sibling-comparison estimates were all close to 1, indicating no difference between the siblings regardless of their family income exposure, and the confidence intervals were narrow, indicating a high level of precision of the estimates. Background Lesbian, gay, bisexual, trans and intersex (LGBTI) people experience significant health inequalities. Located within a European Commission funded pilot project, this paper presents a review of the health inequalities faced by LGBTI people and the barriers health professionals encounter when providing care. Methods A narrative synthesis of 57 papers including systematic reviews, narrative reviews, meta-analyses and primary research. Literature was searched in Cochrane, Campbell Collaboration, Web of Science, CINAHL, PsychINFO and Medline. The review was undertaken to promote understanding of the causes and range of inequalities, as well as how to reduce inequalities. Results LGBTI people are more likely to experience health inequalities due to heteronormativity or heterosexism, minority stress, experiences of victimization and discrimination, compounded by stigma. Inequalities pertaining to LGBTI health(care) vary depending on gender, age, income and disability as well as between LGBTI groupings. Gaps in the literature remain around how these factors intersect to influence health, with further large-scale research needed particularly regarding trans and intersex people. Conclusion Health inequalities can be addressed via changes in policy, research and in practice through health services that accommodate the needs of LGBTI people. With improved training to address gaps in their knowledge of LGBTI health and healthcare, health professionals should work in collaboration with LGBTI people to address a range of barriers that prevent access to care. Introduction Unintentional injuries represent a substantial public health burden among children and adolescents, and previous evidence suggests that there are disparities in injury by socioeconomic status (SES). This paper reports on a systematic review of literature on injury rates among children and adolescents by measures of SES. Methods A systematic literature search was conducted using six electronic databases: MEDLINE, PsycINFO, CINAHL, HealthSTAR, EMBASE, and SportsDiscus. This review considered children ages 19 years and under and publications between 1997 and 2017—representing an update since the last systematic review examined this specific question. Fifty-four articles were summarized based on study and participant descriptions, outcome and exposure, statistical tests used, effect estimates, and overall significance. Results Most articles addressed risk factors across all injury mechanisms; however, some focused particularly on burns/scalds, road traffic injuries, falls/drowning cases, and playground/sports injuries. Other studies reported on specific injury types including traumatic dental injuries, traumatic brain injuries, and fractures. The studies were of moderate quality, with a median of 15.5 (95% confidence interval [CI]: 15.34 to 15.66) out of 19. Thirty-two studies found an inverse association between SES and childhood unintentional injury, three found a positive association while twenty were not significant or failed to report effect measures. Conclusion Given the variability in definition of the exposure (SES) and outcome (injury), the results of this review were mixed; however, the majority of studies supported a relationship between low SES and increased injury risk. This paper considers the impact of variables at three different levels—city, community and individual—on the depression of adults aged 45 years and above in China. Evidence shows that community factors, such as infrastructure and elderly centres, are critical to reduce depression but the effect of city-level factors such as lowering income inequality and improving public health investment is different for the segments of poor and non-poor as well as the rural and urban residents. This highlights the need to consider targeted policy mix options to avoid resource misallocation. Lastly, Chinese females' depression has worsened over time with ageing, particularly those who drink alcohol or are less educated are prone to depression prompting the need for specialist women health centres. As input costs increase, producers need to be mindful of management practices to maintain profit margins and it is important to understand current management practices. Therefore, the objective of this study was to investigate current beef marketing strategies and beliefs of beef producers in the state of Mississippi in 2019. This survey was distributed to cattle producers in Mississippi in the June/July issue of "Cattle Business in Mississippi" magazine with return postage. There also was a link included in the "Monday Memo" email sent to members of the Mississippi Cattlemen's Association. The survey consisted of 22 questions that were divided into three sections: current practices, current cow-calf marketing practice, and general information. Responses from the survey were anonymous and confidential and were received via paper mail or electronic responses. Responses were stored and analyzed using Qualtrics. Data collected from this study were compared with 1999 data from a similar study conducted by Little et al. (2000). Out of 148 respondents, there was a 95% completion rate of surveys taken. Most respondents were 51 years and older with 54% of the respondents being part of the cattle industry for over 31 years. Ninety percent of respondents were members of the Mississippi Cattlemen's Association and almost 60% were certified in Beef Quality Assurance. The majority, 122 of 148 producers, stated their cattle operation was not the primary source of income for their household. The average number of cows, bulls, stockers/yearling, replacement heifers and cattle on-feed on an operation was greater in the 2019 data compared with 1999. On average, the number of cows per operation increased from 40 to 68 animals, bulls increased from 2 to 5 animals, and replacement heifers increased from 7 to 17 animals from 1999 to 2019. There was also an average increase from 30 to76 animals of stocker/yearling and an increase from 2 to 27 animals of on-feed cattle. Most producers vaccinate (81%) and deworm (85%) according to recommendations and indicated a lack of labor and time as reasons for not implementing some common management practices such as preconditioning. Respondents mainly purchased calves directly from a producer (61%) or from special sales (59%). In 1999, over one-half of respondents purchased from an auction. When it came to selling cattle, 58% of respondents sold at auction regularly in 2019. In 1999, 73% of respondents sold at auctions. In 2019, 72% of respondents stated they did not retain ownership while in 1999 only 58% of respondents did not retain ownership. In conclusion, our results suggest there have been key changes within the cattle industry regarding herd size increases and a shift in selling and purchasing habits since 1999. For many households, energy consumption represents a non-discretionary portion of their budget and directly relates to quality of life. As researchers continue to study the environmental impacts of energy behavior, it is important to explore how energy consumption relates to socio-economic wellbeing. This paper examines the economic impacts of being energy-burdened in the United States, defined as spending at least 10% of household income on heating and electricity services; energy burdens are partially, but not entirely, driven by income, since energy needs and costs can vary substantially due to housing characteristics, utility rates, and other factors. Using panel data of US household income and energy expenditures during 1999–2017, this analysis demonstrates that energy-burdened households were at about 150%–200% greater risk of transitioning into or extending the duration of economic poverty over a two-year timeframe relative to non-burdened households. This analysis indicates that dedicating inordinate amounts of income to energy services can threaten a household's economic well-being over time, possibly by preventing a household from engaging in other economic activities or compounding existing economic hardship. These results emphasize the importance of energy assistance and energy efficiency for low-income households, drawing attention to how structures of energy consumption, the welfare state, and social stratification intertwine. Aims The objective of this paper was to examine 20-year trends of the socioeconomic status (SES) measures income and employment and their association with current alcohol use behaviors in Danish adults aged 60–70. Methods Data from The Danish National Health Survey 2013 and the baseline assessment of the Elderly Study (2013–2016) were combined to form four groups from the general population with various drinking patterns, but with no recent treatment for alcohol use disorder (AUD), and one group seeking treatment: Abstinent (n  = 691), low- (n  = 1978), moderate- (n  = 602), and high-risk (HR) drinkers (n  = 467), and DSM-5 AUD seeking treatment (n  = 262). For all groups, Danish national register data were linked at the individual level obtain find annual information on income and employment during the 20 years prior to interview. Mixed effects models were utilized to model trajectories of income and employment for the five groups. Results Lower income and employment status was observed from middle-aged adulthood when comparing 12-month abstinence or AUD to individuals with low or moderate alcohol consumption. At the end of the study period, moderate-risk drinkers experienced an increase, and HR drinkers a decrease, in income and rate of employment relative to the low-risk drinkers. Conclusions Alcohol use behaviors observed in older adults are related to distinct long-term trajectories regarding income and employment status, which are observable already in middle-aged adulthood. Classical and recent accounts of education posit that education legitimately, and authoritatively, classifies individuals to positions of lower or higher status. However, despite these general theoretical claims, empirical evidence that provides an in-depth picture of the relationship between educational attainment and social status remains scarce. In this paper, based on a dataset of 31 countries (International Social Survey Programme), we investigate the extent to which education is related to subjective social status, the degree to which this is seen as legitimate, and how this relationship varies between countries. We contextualize this relationship with the influence of the centrality of education in countries (operationalized as the share of higher educated). Results showed that education is an important source of subjective social status for individuals across all countries, and is seen as relatively legitimate and uncontroversial among all educational groups. Moreover, among those who perceive education to be more important for status, subjective status differences between educational groups are larger. Additionally, in countries with larger shares of higher educated, educational differences in subjective social status correlate more strongly with whether or not people obtained a degree of higher (tertiary) education. Lastly, the relationship between education and subjective social status in these countries is more independent from other sources of status, such as income and gender. It therefore seems to be that as higher education becomes more central and widely shared in a society, rather than leveling social differences, ironically it also becomes more distinctive and diagnostic in distinguishing people along group lines. This study examines the predictors of job satisfaction among public sector social workers in occupied Palestinian territories. Using a quantitative design, data were collected in summer of 2016 through a paper-based, self-administered questionnaire (N = 237). Using hierarchical multiple regression, the three models explained 15 percent to 32 percent of the variance in job satisfaction. In the final model, coefficient indicated that monthly income, contract work status, service orientation, and supervisory support are related to job satisfaction. Respondents with more children and lower salaries showed lower levels of job satisfaction. Respondents who are working as temporary contract workers, who have positive attitudes toward the social work profession, and who have positive attitudes toward supervisory support are more likely to be satisfied with their job. Background Frailty is a common, multi-factorial, age-related syndrome commonly observed in people with diabetes. Although older diabetics are prone to adverse healthcare outcomes and diabetes increases the risk of developing frailty, little is known about the effects of frailty on diabetes. This paper examines the association between diabetes, frailty, and mortality in Europeans aged ≥50 years. Methods Data were included from The Survey of Health, Ageing and Retirement in Europe (SHARE) waves one and six. A participant's first interview was taken as the baseline and subsequent waves were used for mortality follow-up (time and cause). Frailty and pre-frailty were measured using established cut-offs using the Physical Phenotype (SHARE-FI) and a 55-item Frailty Index (FI-55). Kaplan-Meier survival analysis was used to assess the relationship between frailty and mortality in people with diabetes and significance tested using log-rank test. Cox regression was used to adjust for potential confounders (age, sex, education, income, employment, alcohol use, smoking, hypertension, hypercholesterolaemia, myocardial infarction, stroke, metastatic cancer, chronic lung disease, polypharmacy, self-perceived health and depression). Results Data from 8,954 diabetics aged 50-99 years were included with 1,598 deaths (17.8%). According to the SHARE-FI, 1,971 (22.0%) were frail, 4,183 (46.7%) pre-frail and 2,800 (31.3%) robust. According to the Kaplan-Meier log-rank test survival varied significantly across frailty strata according to both indexes (p<0.001). At 10-year follow-up, adjusting for confounders SHARE-FI frailty and pre-frailty were significantly associated with mortality, adjusted Hazard Ratio (HR) 2.19, (95% CI:1.66-2.89), and 1.38 (95% CI:1.09-1.74), respectively. Results were similar using the FI-55, HR for frailty 1.66 (95% CI:1.09-2.54). Causes of mortality were significantly different according to frailty status (p<0.05). Conclusion Frailty and pre-frailty are independent risk factors for mortality in diabetics. The identification of frailty is important for the risk-stratification and management of middle aged and older patients with diabetes and should be included in the routine assessment of these high-risk individuals. Background: Welfare regimes in Europe modify individuals' socioeconomic trajectories over their life-course, and, ultimately, the link between socioeconomic circumstances (SECs) and health. This paper aimed to assess whether the associations between life-course SECs (early-life, young adult-life, middle-age and old-age) and risk of poor self-rated health (SRH) trajectories in old age are modified by welfare regimes (Scandinavian [SC], Bismarckian [BM], Southern European [SE], Eastern European [EE]).Methods: We used data from the longitudinal SHARE survey. Early-life SECs consisted of four indicators of living conditions at age 10. Young adult-life, middle-age, and old-age SECs indicators were education, main occupation and satisfaction with household income, respectively. The association of life-course SECs with poor SRH trajectories was analysed by confounder-adjusted multilevel logistic regression models stratified by welfare regime. We included 24 011 participants (3626 in SC, 10 256 in BM, 6891 in SE, 3238 in EE) aged 50 to 96 years from 13 European countries.Results: The risk of poor SRH increased gradually with early-life SECs from most advantaged to most disadvantaged. The addition of adult-life SECs differentially attenuated the association of early-life SECs and SRH at older age across regimes: education attenuated the association only in SC and SE regimes and occupation only in SC and BM regimes; satisfaction with household income attenuated the association across regimes.Conclusions: Early-life SECs have a long-lasting effect on SRH in all welfare regimes. We read with interest the recent paper by Sariaslan I et al i .[1] investigating the associations between childhood family income and three outcomes observed later in life: psychiatric disorders, substance misuse and violent crime arrests. To illustrate our point, assume that childhood family income has a causal effect on the neighbourhood in which a family resides while the children are growing up: the higher the income, the more affluent the neighbourhood. Even though the sibling groups carry the entire causal conclusion of the article, we do not find a single table describing the distribution of within-family income differences, or outcome differences. Income differences are most likely smaller in Finland than in most other countries, and the causes and consequences of such income differences may be very different under different societal models and/or in vulnerable groups. Background and Objectives Reverse mortgages are loans that allow older homeowners to borrow from their home equity with no repayment due until the borrower dies or moves out of the home. We currently know very little about how homeowners evaluate and experience reverse mortgages as solutions to their financial and housing needs in later life. Furthermore, despite an increasingly diverse population of reverse mortgage borrowers, we know little about how social inequalities may contribute to reverse mortgage decisions and their outcomes. In this paper, we examine reverse mortgage decision-making and experiences in a racially and economically diverse sample of older US homeowners. Research Design and Methods We conducted 44 in-depth interviews with older homeowners who were considering or who had obtained a reverse mortgage loan. We inductively and iteratively developed a thematic coding scheme that was applied to all interview transcripts. Results Our analysis produced a dichotomous schema of reverse mortgage decision making that was shaped by social and economic opportunities and constraints. For some participants, reverse mortgages represented strategic tools used to maximize home equity and its benefits. For others, it was an option of last resort to which participants turned when faced with the imminent loss of their home. Discussion and Implications Focusing on reverse mortgages, our analysis suggests way that social inequalities may be reproduced through financial decisions and the unequal landscapes of opportunity in which they are made. Stratification and inequality are among the most central concepts in sociology, and although related, they are fundamentally distinct: inequality refers to the extent to which resources are distributed unevenly across individuals or between population subgroups, whereas stratification refers to the extent to which population subgroups occupy distinct hierarchical layers within an overall resource distribution. Despite the centrality of stratification in theories of class structure, prior empirical studies have focused exclusively on measures of inequality, which do not accurately capture the degree of class stratification and suffer from a variety of methodological limitations. In this paper, we employ a novel rank-based index of stratification to measure the degree to which occupational classes inhabit distinct, non-overlapping, and hierarchically arranged layers in the distribution of personal market income. The stratification index is nonparametric, both scale and translation invariant, and independent of the level of inequality. Based on this index, our results show that the US income distribution is highly stratified by occupational class and that the degree of class stratification increased substantially from 1980 to 2016. Moreover, we find that this trend is almost entirely due to growing stratification among aggregate occupational classes rather than among the disaggregate occupations nested within them. Finally, a set of counterfactual analyses indicate that the rise of occupational class stratification is driven by increases in the income returns to education, deunionization, and deindustrialization, although the relative importance of these factors varies by gender. Risk assessment in cardiac surgery traditionally consists of medical and physiological parameters. However, non-physiological factors have also been found to be predictive of poor outcomes following cardiac surgery. Therefore, the isolated focus on physiological parameters is questionable. This paper describes the emotional, behavioural, social and functional factors that have been established to play a role in outcomes following cardiac surgery. This forms a basis for future research, testing the value of these factors above and beyond the physiological parameters. Many surveys ask respondents to consult financial records in order to improve data accuracy. However, the assumption that record use reduces measurement error has not been tested with a large-scale comparison to administrative data. This paper compares interest, dividend, and rental income in the Survey of Income and Program Participation (SIPP) to administrative IRS 1040 tax data. In a novel estimation strategy, we use various measures of respondent motivation and precision to account for nonrandom selection. Our results show that record use is associated with reducing the discrepancy between survey and administrative data by approximately 21 to 43 percent. In terms of potential costs from encouraging record use, record users spend an extra 3.5 seconds for each asset question, on average, after controlling for their behavior in other parts of the SIPP interview. The extra time per question translates to a 2.2 percent increase in the total duration of the interview. Thus, while record use may be an effective tool for improving data accuracy, it may come at the cost of higher interviewer compensation and increased respondent burden. [ABSTRACT FROM AUTHOR]

A group of women in the rural Eastern Cape Province of South Africa earns a living collecting scrap metal and paying to transport it for sale in Durban. The sustainability of their livelihood is in question because of an ever-shrinking supply of recyclable metals, fluctuating commodity prices, and the logistical difficulties of transporting the materials to buyers further along the recycling value chain. Using the sustainable livelihoods framework, this paper highlights the opportunities and challenges facing these informal recyclers. It shows how the women have fashioned a livelihood practice in the context of widespread rural poverty, isolation from recycling markets, and limited community assets. Implications for community development include the need for a participatory sustainable livelihood analysis to evaluate ways to counterbalance the instability and uncertainty involved in informal recycling and to raise the incomes of the women involved in this activity. Objective: Pall Mall gained significant brand share in the cigarette market between 2002 and 2013. We sought to determine whether demographic shifts occurred among the participants reporting Pall Mall as their usual brand during this time span.Method: We examined National Survey of Drug Use and Health (NSDUH) data from 2002-2014. Demographic characteristics included age, education, ethnicity, income, and cigarette use (cigarettes per day, daily/non-daily smoking, and nicotine dependence). We also examined RJ Reynolds investor reports and shareholder documents to determine the impact of tobacco marketing on the growth of Pall Mall.Results: Over 2002-2014, Pall Mall has gained among smokers 26 to 34 years of age. More Pall Mall smokers in 2014 report higher incomes (over $75000), and also report lower scores on measures of cigarette dependence, compared to 2002. Pall Mall smokers over time seem to share characteristics of premium cigarette brands smokers.Conclusion: The profile of the typical Pall Mall smoker has changed as the brand has gained market share. An association exists between brand positioning and economic forces, which has contributed to an increase in the market share for Pall Mall.Implications: It is well known that cigarette marketing drives the sale of tobacco products. The growth in the market share of Pall Mall serves as an excellent example to demonstrate how economic uncertainty paired with brand positioning and advertising worked together to serve as a catalyst for the rapid growth observed for this brand. This paper also looked at various demographic changes that occurred among Pall Mall smokers over a 12 year period and compared them to smokers of all other cigarette brands. Background: Up to now, research has focused on the effects of urban renewal programs and their impacts on health. While some of this research points to potential negative health effects due to gentrification, evidence that addresses the complexity associated with this relation is much needed. This paper seeks to better understand when, why and how health inequities arise from urban renewal interventions resulting in gentrification. Methods: A realist review, a qualitative systematic review method, aimed to better explain the relation between context, mechanism and outcomes, was used. A literature search was done to identify theoretical models of how urban renewal programs can result in gentrification, which in turn could have negative impacts on health. A systematic approach was then used to identify peer-reviewed studies that provided evidence to support or refute the initial assumptions. Results: Urban renewal programs that resulted in gentrification tended to have negative health effects primarily in residents that were low-income. Urban renewal policies that were inclusive of populations that are vulnerable, from the beginning were less likely to result in gentrification and more likely to positively impact health through physical and social improvements. Conclusions: Research has shown urban renewal policies have significant impacts on populations that are vulnerable and those that result in gentrification can result in negative health consequences for this population. A better understanding of this is needed to impact future policies and advocate for a community-participatory model that includes such populations in the early planning stages. Background: Consistent evidence shows the importance of preventing smoking at young ages, when health behaviours are formed, with long-term consequences on health and survival. Although tobacco control policies and programmes targeting adolescents are widely promoted, the cost-effectiveness of such interventions has not been systematically documented. We performed a systematic review on the cost-effectiveness of policies and programmes preventing tobacco consumption targeting adolescents. Methods: We systematically reviewed literature on the (i) cost and effectiveness of (ii) prevention policies targeting (iii) smoking by (iv) adolescents. PubMed, Web of Science, Cochrane, CEA-TUFTS, Health Economic Evaluations, Wiley Online Library, Centre for Reviews and Dissemination Database, the National Institute for Health and Care Excellence and Google Scholar databases were used, and Google search engine was used for other grey literature review. Results: We obtained 793 full-text papers and 19 grey literature documents, from which 16 studies fulfilled the inclusion criteria. Of these, only one was published in the last 5 years, and 15 were performed in high-income countries. Eight analyzed the cost-effectiveness of school-based programmes, five focused on media campaigns and three on legal bans. Policies and programmes were found to be cost-effective in all studies, and both effective and cost-saving in about half of the studies. Conclusions: Evidence is scarce and relatively obsolete, and rarely focused on the evaluation of legal bans. Moreover, no comparisons have been made between different interventions or across different contexts and implementation levels. However, all studies conclude that smoking prevention policies and programmes amongst adolescents are greatly worth their costs. [ABSTRACT FROM AUTHOR]

Background: In 2015, new disability weights (DWs) for infectious diseases were constructed based on data from four European countries. In this paper, we evaluated if country, age, sex, disease experience status, income and educational levels have an impact on these DWs. Methods: We analyzed paired comparison responses of the European DW study by participants' characteristics with separate probit regression models. To evaluate the effect of participants' characteristics, we performed correlation analyses between countries and within country by respondent characteristics and constructed seven probit regression models, including a null model and six models containing participants' characteristics. We compared these seven models using Akaike Information Criterion (AIC). Results: According to AIC, the probit model including country as covariate was the best model. We found a lower correlation of the probit coefficients between countries and income levels (range rs: 0.97-0.99, P < 0.01) than between age groups (range rs: 0.98-0.99, P < 0.01), educational level (range rs: 0.98-0.99, P < 0.01), sex (rs = 0.99, P < 0.01) and disease status (rs = 0.99, P < 0.01). Within country the lowest correlations of the probit coefficients were between low and high income level (range rs = 0.89-0.94, P < 0.01). Conclusions: We observed variations in health valuation across countries and within country between income levels. These observations should be further explored in a systematic way, also in non-European countries. Much emphasis has been placed on designing development projects to include community participation (CP), which is normally seen as a way of ensuring project effectiveness, but less attention has been directed to identifying households that exclude themselves and their reasons for doing so. This paper uses household-level survey data from twenty-nine African and Latin American communities to show the extent to which some socioeconomic characteristics of a household influence the members' willingness to participate in community development (CD). It provides answers to the question, ‘in what ways do income, education, security of tenure, household type and household size correlate with variables that are measures of CP and management?’ From the findings, the paper concludes that income levels and housing status have positive correlation with factors influencing CP and management. On the other hand, educational level, household size and family type have no significant correlation with CP and management. Two lessons are drawn from the study. First, efforts to promote CP as a useful approach to CD require a sociological study of household characteristics. Prior information about a community's household set-up can help project organizers and outsiders to develop realistic expectations about CP. Second, the findings that certain groups of households do not participate while others are more likely to play a leading role have implications in terms of the distribution of the benefits of community intervention programmes. In this paper, we examine patterns of self-reported diagnosis of noncommunicable diseases (NCDs) and prevalences of algorithm/measured test-based, undiagnosed, and untreated NCDs in China, Ghana, India, Mexico, Russia, and South Africa. Nationally representative samples of older adults aged ≥50 years were analyzed from wave 1 of the World Health Organization's Study on Global Ageing and Adult Health (2007-2010; n = 34,149). Analyses focused on 6 conditions: angina, arthritis, asthma, chronic lung disease, depression, and hypertension. Outcomes for these NCDs were: 1) self-reported disease, 2) algorithm/measured test-based disease, 3) undiagnosed disease, and 4) untreated disease. Algorithm/measured test-based prevalence of NCDs was much higher than self-reported prevalence in all 6 countries, indicating underestimation of NCD prevalence in low- and middle-income countries. Undiagnosed prevalence of NCDs was highest for hypertension, ranging from 19.7% (95% confidence interval (CI): 18.1, 21.3) in India to 49.6% (95% CI: 46.2, 53.0) in South Africa. The proportion untreated among all diseases was highest for depression, ranging from 69.5% (95% CI: 57.1, 81.9) in South Africa to 93.2% (95% CI: 90.1, 95.7) in India. Higher levels of education and wealth significantly reduced the odds of an undiagnosed condition and untreated morbidity. A high prevalence of undiagnosed NCDs and an even higher proportion of untreated NCDs highlights the inadequacies in diagnosis and management of NCDs in local health-care systems. Food insecurity is steadily increasing in Canada. The objective of this paper is to determine if food capacities and satisfaction of recently enrolled participants in food security interventions are associated with the intervention having either a traditional or an alternative type of approach. Participants having recently accessed traditional (n = 711) or alternative community interventions (n= 113) in the Montreal metropolitan area, Canada, were interviewed with a questionnaire. The categorizing variable was participation in a community organization providing either traditional interventions, aimed to help people cope with the urgent need of food, or alternative interventions, aimed at first assistance, in addition to the creation of long-term solutions such as social integration and skills development. Participants' food and nutrition-related capacities and food satisfaction are studied. Multilevel regression models were used to assess whether participants took part in a traditional or alternative interventions. These interventions do not reach the same population. Relative to participants in alternative food security interventions, participants in traditional interventions demonstrated less capacity for accessing information about food safety and healthiness, and perceived their diet as less healthy. Traditional food security participants also paid less attention to the nutritional properties of food and reported less satisfaction with quantity, variety and taste of the food they accessed. The reasons why individuals who may benefit the most from alternative interventions were unlikely to participate should be investigated. This paper offers an exploratory analysis of the subnational dimension of income inequality, using data from the Luxembourg Income Study. The paper undertakes two basic tasks. First, it describes the results of calculations on household-level income data that produce indicators of intra- and inter-household inequality for 191 regions in 12 developed countries for the late 1980s and early 1990s, and for 149 regions in 8 countries for the mid-1990s. Second, the paper demonstrates the value of regional analysis by reexamining the relationship between electoral turnout and income inequality, an important substantive issue which has heretofore been explored almost entirely at the national or individual level but upon which regional-level analysis can shed valuable light. This study aimed to validate a Turkish version of the Short Test of Functional Health Literacy (S-TOFHLA) (Baker et al., Development of a brief test to measure functional health literacy. Patient Educ Counsel 1999;38:33-42) and a Turkish version of the Chew self-report scale (Chew et al., Brief questions to identify patients with inadequate health literacy. Family Med, 2004;36:588-94) for measuring functional health literacy. The original English version of the S-TOFHLA and the Chew items were translated by applying standardized translation methods and cultural adaptations, and both were administered to a sample of diabetes patients (N=302) in two diabetes clinics in one of the major cities in Turkey. Self-administered paper-pencil questionnaires were distributed to eligible outpatients who had a clinic appointment. In addition to the S-TOFHLA measurement and the Chew screening questions, gender, age, educational attainment, income, marital status and diabetes knowledge were obtained. The Turkish version of S-TOFHLA showed high internal consistency. Both S-TOFHLA and the Chew screening scale correlated significantly with known predictors of health literacy: age, education and income. The Chew scale was also related weakly but significantly with general diabetes knowledge. It is expected that the Turkish versions of S-TOFHLA and the Chew scale will be used in Turkey as well as in other countries with large Turkish communities. [ABSTRACT FROM AUTHOR]

This paper examines what is known about error in the U.S. Consumer Expenditure Survey (CE). It reviews likely sources of error in consumer expenditure surveys and describes the various approaches that have been used to evaluate estimates from the CE in the United States and from similar surveys around the world. In the United States, many efforts to assess the quality of the CE have compared CE estimates to estimates from other surveys or from the National Income and Product Accounts (NIPA). In addition, other studies have used data internal to the CE in an attempt to assess the level of error. We review studies that take each of these approaches and assess their strengths and weaknesses. Our review shows that, as with other national and international surveys on household spending, the direction and magnitude of measurement errors in the CE are not well understood. The general impression that the CE expenditure estimates are dominated by underreporting is based largely on comparisons of the CE with NIPA estimates. However, other results suggest that the magnitude and direction of errors in the CE depend on the expenditure category examined, characteristics of the respondent and the survey protocol, and the method used to assess these errors. Single mothers' relatively high levels of poverty are well documented, but the role that intergenerational coresidence may play in mitigating this disadvantage is not well understood. In this paper, we use multiple rounds of a large national survey between 1986 and 2007 (n = 67,252) to evaluate the extent to which income sharing via intergenerational coresidence limits poverty among single mothers in Japan. Results indicate that conventional poverty rates based on single-mother households overstate the prevalence of poverty among single mothers by 12–20 percent as a result of excluding those who are coresiding with parents. We find no evidence that the prevalence of intergenerational coresidence has changed in a way that would offset the poverty-increasing effect of growth in single parenthood. Finally, we show not only that shared income is the most important factor in limiting poverty among single mothers living with parents, but also that many single mothers are coresiding with parents who are themselves economically disadvantaged. We discuss the implications of our findings for understanding relationships between rapid family change and poverty in countries like Japan, where public income support for single mothers is limited and where family support via intergenerational coresidence is both common and normative. Although low-income youth are likely to have low or less frequent fruit and vegetable intake, current understanding of the influences on intake in youth is limited. A systematic review of quantitative research on determinants of fruit and vegetable intake among low-income youth (i.e., persons aged <20 years) was conducted. The aims were to identify which determinants have been studied and which are consistently associated with intake. Fifty-eight papers published between 2003 and August 2013 were included. Across studies, 85 unique determinants were identified. Those best supported by evidence were race/ethnicity (with intake consistently higher among Hispanic as compared with African American and white youth), fruit and vegetable preferences, and maternal fruit and vegetable intake. For many potential determinants, the consistency of evidence could not be examined because of a lack of studies. Findings highlight racial/ethnic differences in fruit and vegetable intake and influences on intake that should be considered when designing dietary interventions for low-income youth. Background: Factors underlying socioeconomic inequalities in mortality are not well understood. This study contributes to our understanding of potential pathways to result in socioeconomic inequalities, by examining alcohol consumption as one potential explanation via comparing socioeconomic inequalities in alcohol-attributable mortality and all-cause mortality.Methods: Web of Science, MEDLINE, PsycINFO and ETOH were searched systematically from their inception to second week of February 2013 for articles reporting alcohol-attributable mortality by socioeconomic status, operationalized by using information on education, occupation, employment status or income. The sex-specific ratios of relative risks (RRRs) of alcohol-attributable mortality to all-cause mortality were pooled for different operationalizations of socioeconomic status using inverse-variance weighted random effects models. These RRRs were then combined to a single estimate.Results: We identified 15 unique papers suitable for a meta-analysis; capturing about 133 million people, 3 741 334 deaths from all causes and 167 652 alcohol-attributable deaths. The overall RRRs amounted to RRR = 1.78 (95% confidence interval (CI) 1.43 to 2.22) and RRR = 1.66 (95% CI 1.20 to 2.31), for women and men, respectively. In other words: lower socioeconomic status leads to 1.5-2-fold higher mortality for alcohol-attributable causes compared with all causes.Conclusions: Alcohol was identified as a factor underlying higher mortality risks in more disadvantaged populations. Most research on the effects of international migration on democratic institutions in sending countries focuses on how emigration changes the civic and democratic values of those left behind. Little attention has been given to how the additional income provided by migrant remittances alters the incentive structure of the political actors involved and how this will affect political outcomes. This paper develops a voting model that accounts for the effect of higher income through remittances and shows that its expected effects on voter turnout patterns differ in important ways from those of improved civic values. Taking these predictions to the data, it is shown that, for the case of Mexican municipal elections over the year 2000-2002 period, the empirical evidence strongly supports the notion that international remittances had a positive effect on electoral competitiveness in Mexico by reducing the clientelistic power of the formerly dominant state party ( Institutional Revolutionary Party). Proponents often recommend high labor standards as a means of reducing inequality between and within countries. Opponents suggest that labor standards exacerbate international and domestic inequalities. In this paper, we forward a simple argument whereby the impact of higher labor standards on domestic inequality depends on a country's labor endowment. We hypothesize that where labor is abundant, higher standards will exacerbate inequality. Where labor is scarce, higher labor standards might lower inequality. In both cases, the impact of labor standards on inequality work through an employment and wage effect. Using newly available data on labor standards around the world from 1981 to 2000, we provide evidence largely consistent with our hypotheses. Higher labor standards do, indeed, exacerbate inequality in labor-abundant economies. On the other hand, higher labor standards lower inequality in labor-scarce economies. Skin color has been proposed to contribute to race-based health disparities in the United States because of differences in ultraviolet (UV) light–induced vitamin D synthesis. The prediction of this hypothesis, herein named the UVD hypothesis, is that racial disparities in health outcomes are correlated with UV light availability. This paper investigates whether UV light availability is associated with disparities in the rates of low birth weight (LBW) and preterm birth (PTB) between whites and blacks, because these outcomes are thought to be influenced by vitamin D status and to shape disease risk in later life. Data on LBW and PTB from 2007 (n = 2,825,620 births) were compared with data on UV light exposure across the United States. Contrary to the predictions of the UVD hypothesis, LBW and PTB rate disparities were greatest in states with the highest UV light exposure. Notably, income inequality was positively and significantly related to LBW and PTB disparities, even after controlling for UV light availability. The results of this analysis demonstrate that there is a significant environmental gradient in racial disparities in birth outcomes in the United States, but other social or environmental factors associated with living in the southern United States are likely stronger contributors to disparities in birth outcomes than UV light–induced vitamin D status. Although there is strong evidence that short-term exposure to particulate matter is associated with health risks, less is known about whether some subpopulations face higher risks. We identified 108 papers published after 1995 and summarized the scientific evidence regarding effect modification of associations between short-term exposure to particulate matter and the risk of death or hospitalization. We performed a meta-analysis of estimated mortality associations by age and sex. We found strong, consistent evidence that the elderly experience higher risk of particular matter-associated hospitalization and death, weak evidence that women have higher risks of hospitalization and death, and suggestive evidence that those with lower education, income, or employment status have higher risk of death. Meta-analysis showed a statistically higher risk of death of 0.64% (95% confidence interval (Cl): 0.50, 0.78) for older populations compared with 0.34% (95% Cl: 0.25, 0.42) for younger populations per 10 ng/m³ increase of particulate matter with aerodynamic diameter ≤10 µm. Women had a slightly higher risk of death of 0.55% (95% Cl: 0.41,0.70) compared with 0.50% (95% Cl: 0.34, 0.54) for men, but these 2 risks were not statistically different. Our synthesis on modifiers for risks associated with particulate matter can aid the design of air quality policies and suggest directions for future research. This paper reports on a complex environmental approach to addressing ‘wicked’ health promotion problems devised to inform policy for enhancing food security and physical activity among Māori, Pacific and low-income people in New Zealand. This multi-phase research utilized literature reviews, focus groups, stakeholder workshops and key informant interviews. Participants included members of affected communities, policy-makers and academics. Results suggest that food security and physical activity ‘emerge’ from complex systems. Key areas for intervention include availability of money within households; the cost of food; improvements in urban design and culturally specific physical activity programmes. Seventeen prioritized intervention areas were explored in-depth and recommendations for action identified. These include healthy food subsidies, increasing the statutory minimum wage rate and enhancing open space and connectivity in communities. This approach has moved away from seeking individual solutions to complex social problems. In doing so, it has enabled the mapping of the relevant systems and the identification of a range of interventions while taking account of the views of affected communities and the concerns of policy-makers. The complex environmental approach used in this research provides a method to identify how to intervene in complex systems that may be relevant to other ‘wicked’ health promotion problems. This paper has contributed to confirming the link between education and health in developed countries. The analysis is based on 11 European Union countries. We estimate country-specific health functions, where the dependent variable is self-reported health status and the education attainment is one of the main inputs. All eight waves (1994–2001) of the European Community Household Panel are deployed. A random effects ordered probit is estimated in order to control, to a given extent, for unobserved heterogeneity. Explanatory variables are both time invariant (education attainment and gender) and time varying (gross wages, hours of work, age and living alone). Results confirm the positive impact of secondary education on health in most cases and tertiary education in all cases, even after controlling for other inputs in the health function and taking unobserved heterogeneity into account. Secondary education has an impact on health in all countries in the sample except for The Netherlands and UK. The effect does not differ between secondary and tertiary education in France, Ireland and Greece. The correlation between education and health is interpreted in different but complementary ways by diverse approaches and we may not disentangle the precise mechanism that connects health with education from our results. Anyway, it seems clear that better coordination is needed between education and health policies to effectively improve health literacy. Background: The aim of this paper was to measure socio-economic inequalities in mortality over a 27-year period, and estimate the simultaneous effects of education and income adjusted for changing proportions and potential confounders. Methods: Census data in 1970, 1980 and 1990 with information about income, educational level and household size were linked to mortality records at the individual level and analysed with absolute mortality rates, Poisson regression (univariate and multivariate) and Relative Index of Inequality (RII). Results: Mortality differences increased between income quartiles and educational groups as well as between singles and non-singles. RII between income quartiles increased from 1.6 to 3.6 for men and 1.7 to 2.7 for women while RII between the educational groups increased from 1.6 to 2.8 for men and 1.5 to 2.1 for women. However, RII derived from the adjusted multivariate regression analysis was 1.8 (men) and 1.9 (women) between the income groups in 1990 and 2.2 (men) and 1.7 (women) between the educational groups. In the 1990s current income was more important than previous income, and the rate ratios (RR) increased for groups dropping into the lowest income quartile between two censuses. Conclusion: Low education and single status have become relatively more important risk factors for mortality over time. Confounder effects of education and household size could partly explain the seemingly large increase in inequalities between income groups. In their normative role in shaping the basic structure of a health care system, liberty and equality are often thought to conflict so sharply that health policy is condemned to remain an ideological battleground. In this paper, I will articulate my own view of why much of the apparently fundamental conflict between individual liberty and responsibility, on the one hand, and equality and equality's related concern for cost-efficiency, on the other hand, is less intractable than it is usually assumed to be. The result will be to break the rigid and stereotypical association of liberty-emphasizing social philosophies with the pluralistic market paradigm for a health care system and egalitarian, equity-emphasizing social philosophies with the unitary public system paradigm. Understanding better the moral ingredients of liberty and equitable distribution as well as the complexity of how liberty and equality actually intersect in a health care system opens the door to seeing the possibility of significant reconciliation. I will conclude, among other things, that even semi-libertarian views of distributive justice should strongly embrace compulsory, universal coverage of health care for some significant level of care, and that egalitarian views ought not to regard different levels of coverage for people of different income levels as necessarily unjust. Background: Non-communicable diseases (NCDs) remain a major challenge in the 21st century. High-income countries (HICs) populations are ethnically and culturally diverse due to international migration. Evidence suggests that NCDs rates differ between migrants and the host populations in HICs. This paper presents a review of NCDs burden among migrant groups in HICs in Europe, North America and Australia with a major focus on cardiovascular diseases (CVDs), cancer and diabetes.Methods: We performed a narrative review consisting of scholarly papers published between 1960 until 2018.Results: CVD risk differs by country of origin, country of destination and duration of residence. For example, stroke is more common in sub-Sahara African and South-Asian migrants, but lower in North African and Chinese migrants. Chinese migrants, however, have a higher risk of haemorrhagic stroke despite the lower rate of overall stroke. Coronary heart disease (CHD) is more common in South-Asian migrants, but less common in sub-Saharan and north African migrants although the lower risk of CHD in these population is waning. Diabetes risk is higher in all migrants and migrants seem to develop diabetes at an earlier age than the host populations. Migrants in general have lower rates of overall cancer morbidity and mortality than the host populations in Europe. However, migrants have a higher infectious disease-related cancers than the host populations in Europe. In North America, the picture is more complex. Data from cross-national comparisons indicate that migration-related lifestyle changes associated with the lifestyle of the host population in the country of settlement may influence NCDs risk among migrants in a very significant way.Conclusion: With exception of diabetes, which is consistently higher in all migrant groups than in the host populations, the burden of NCDs among migrants seems to depend on the migrant group, country of settlement and NCD type. This suggests that more work is needed to disentangle the key migration-related lifestyle changes and contextual factors that may be driving the differential risk of NCDs among migrants in order to assist prevention and clinical management of NCDs in these populations. Abstract Most previous cross-national research on income inequality has been insensitive to issues concerning the quality of the data. This paper builds on recent work by Menard (1986) in examining the comparability of various types of income inequality data. Specifically, I suggest that comparability of the data will be enhanced (and research findings will be more trustworthy) if we are more stringent concerning the definition and measurement of the key concept: income. Abstract This paper gives a cross-national comparison, involving West German, Dutch, and United States data, on the use of social ties in the job-search process. Data for West Germany and the Netherlands are compared with the Lin et al. research on Albany-Schenectady and Troy. The findings show that (1) there is more use of informal sources in the U.S. than in West Germany and the Netherlands; (2) informal sources do not generally lead to higher occupational prestige and income; (3) for the Dutch data (no West German data are available for this question) greater social resources, that is, a contact person with relatively high prestige, do lead to a job with a higher prestige, though effects are less pronounced than found by Lin et al.; and (4) an extension of the Lin et al. analysis shows that these social resources do not have a significant impact on income. This last finding is consistent with the recent results of Marsden and Hurlbert's analysis of Detroit Area Study data. Abstract The relation between sociodemographic factors and racial group identification is explored in this paper. Previous research suffers from important limitations which have led to much confusion in this area of research. For this and other reasons, the sociodemographic determinants of racial group identification have been little studied. Using data from a national sample of black adults, the findings of this research indicate that racial group identification is strongest among older blacks and the least-educated blacks who live in urban areas. Additionally, significant interactions between education and region are found; highly educated blacks living outside the West have strong levels of racial group identification.
