There is little research comparatively assessing prevalence of pain between older people either with or without intellectual disability. This paper explores health and social factors associated with chronic pain in these two groups. A cross-sectional survey was undertaken in New South Wales and Queensland, Australia. Inclusion criteria were adults either with or without intellectual disability, aged 60 years and older, and currently living in community-settings. Univariate and multivariable analyses were undertaken on a sample of 391 adults with intellectual disability and 920 adults without intellectual disability. Key findings included higher prevalence of pain in the intellectual disability group, along with higher rates of osteoarthritis, falls, oral health problems, and mood disorders. Mitigating risk factors for conditions that cause chronic pain in older adults is crucial. As longevity increases, the healthcare sector needs to prioritise chronic pain management for people with intellectual disabilities through appropriate treatment strategies. [ABSTRACT FROM AUTHOR]

By 2050, approximately one quarter of Australia's ageing population will comprise culturally and linguistically diverse individuals. While a number of valid neuropsychological measures are available for use with English-speaking individuals, existing measures may not be appropriate for use with older immigrant populations. The purpose of this review is to highlight existing challenges facing the Australian neuropsychological community in the context of assessing older immigrant populations and offer suggestions to improve clinical practice. By conducting a review of existing research related to assessment of culturally and linguistically diverse populations, this paper provides a) a critical overview relating to factors known to impact on the validity of assessment outcomes, b) examines published Australian studies relating to neuropsychological test adaptation and the development of normative data for use with older immigrant populations, and c) offers recommendations to guide assessment and clinical practice. The use of existing English language test and norms has the potential to underestimate cognitive function in older immigrant populations, leading to erroneous diagnostic outcomes. These issues are related to a) level and quality of education, b) inappropriate test content, c) nonrepresentative normative data, and d) a lack of familiarity with test taking procedures. Research addressing issues relating to cross-cultural assessment is gaining momentum within Australia, resulting in the development of demographically focused normative data and adapted tests to accommodate some cultural groups. Initial results have reported mixed success, indicating the need for further and carefully considered research. The pathway to aging well is not always clear for older migrants living in a foreign country, who must navigate a range of challenges resulting from differences between the cultural expectations of aging in their country of origin and the realities of aging in their country of residence. Transnational migration scholars indicate that digital media are important resources for maintaining relationships and support networks across 'here' and 'there'. They say relatively little, however, about the experiences of maintaining a sense of home, particularly for older migrants. In this paper, we draw on ethnographic interview data with older migrants from Sri Lanka and Burma (Myanmar), who live in Australia, to examine how their practices of 'digital homing' help them to manage the challenges of aging well in a foreign land. Three key findings are proposed. First, older migrants are active and skilled in using digital devices to create spaces of belonging and home. Second, older migrants' access to and uses of digital media are structured by gendered, ethnic and generational roles, expectations and obligations. Third, the practices of digital homing that enhance migrant experiences of aging well tend to simultaneously reinforce and reproduce gendered inequalities within families and communities. We conclude by arguing that it is the very capacity of digital homing practices to reproduce ethnic and generational selves in host societies that simultaneously contributes to the reproduction of unequal gendered obligations and expectations, including in later life. This paper examines the available literature on single older non-home owning women in Australia and their housing issues. Preliminary information suggests that this subset of the population is increasingly at risk of becoming homeless or inadequately housed in later life. In fact, there is a historical dearth of research on women’s housing in general. This invisibility and vulnerability is interrogated within this paper through a feminist standpoint lens. We argue that in order to better understand the situation for single older women who do not own their own homes, it is necessary to revisit the social landscape inhabited by these women in their early years. Firstly, however, the current landscape is explored. It appears that ageing and single status are compounding factors which place non-home owning women at higher risk of homelessness or inappropriate housing. The paper then attempts to assess how the social and economic conditions that were extant in the mid-twentieth century led to the present situation. Rising demand for social housing has induced housing authorities to find ways to increase the utilisation of existing housing stock, including targeting households thought to be living in homes that are larger than they need. This "under-occupation" of social housing is common among older tenants who, as with other under-occupiers, have been subject to unpopular measures designed to encourage downsizing. Yet little attention has been given to the housing needs of under-occupying older tenants and the ability of the social housing sector to meet those needs. In reviewing a new housing initiative for older under-occupying social housing tenants in Brisbane, Australia, this paper explores the housing needs and experience of this cohort. It shows that under-occupancy is a lived experience for some tenants and that downsizing to a smaller property is an attractive, albeit limited option. This suggests that policy prescriptions about the problem of older under-occupiers in the social housing sector should be reframed around the issue of tenants housing needs as they age. Learning is an important aspect of active ageing, yet older people are not often included in discussions of the issue. Older people vary in their need, desire, and ability to learn, and this is evident in the context of technology. The focus of the data analysis for this paper was on determining the place of learning and technology in active ageing. The paper describes results from 2,645 respondents aged from 50 to 74 + years, in Australia, to a 178-item variable postal survey. The survey measured aspects of learning;, work; social, spiritual and emotional status; health; vision; home; life events; and demographics. There was also an open-ended question about being actively engaged in life. Ordinal regression analysis showed that interest in learning, keeping up to date, valuing communication, being younger, and being male are predictors of learning about technology. The results are at variance with an earlier analysis of our data which showed that women are generally more interested in learning. The open statements contained mentions of learning about technology for the purposes of communication, learning, family links, keeping up to date, enjoyment, staying mentally alert, and just using the computer. These results are discussed in terms of the subtle but important differences between needing and wanting to learn about technology and the opportunities for such learning by older people. In an increasingly urbanised and ageing world the spatial distribution of an older population is a matter of growing scholarly and policy interest. Much of the research on this topic has tended to draw on one of two key measures: structural ageing, or the ratio of older cohorts to the rest of the population, or numeric ageing, which simply counts the number of older people without reference to the rest of the population. This paper argues that, on their own, these measures have limitations, and that considerable value lies in assessing the interplay between numeric and structural ageing measures. The population ageing matrix, a theoretical framework that classifies the demographic dynamics of population ageing across metropolitan areas, is presented and examined through the case study of Perth, Western Australia. The value of the matrix to reveal patterns and trends missed by analyses of single measures is investigated and areas that are experiencing changes in the composition and size of the ageing demographic are identified. Some older people describe learning to stay fit and healthy and exercise as a regular part of their lives. There is evidence that exercise is beneficial in terms of physical, psychological and social functioning in older adults. This paper is unique in that it is an analysis of the spontaneous discussion of exercise by older people as a priority in their lives. It also provides information about the kinds of exercise they choose. An online written data forum in Australia with responses from 103 people aged 50 to 92 years (F52, M 51) was analysed for all mention of exercise. The respondents were prompted to comment on sustainable lifestyles as they aged. Thirty per cent mentioned exercise as a priority in their lives. Activities included irregular exercise (11%) routine organised exercise such as golf or tennis (8%) and purposeful exercise such as regular gym attendance (11%), where the respondents explained why they needed to exercise. A few (7%) talked about the need to stay healthy without explaining how. Some (2%) described barriers and conditions that made exercise difficult. These figures for engaging in exercise are similar to those identified in 2018 by the Australian Institute for Health and Welfare. As evidenced by the literature in the area, lack of awareness of the specific benefits of exercise and barriers to exercise for older adults are not new problems. However, given the increase in the percentage of older people in the population and the demonstrated beneficial effects of exercise, it is worthwhile to document the motivation for exercise and engagement in older adults and to suggest strategies to increase their activity. There has not been ample recent research on teaching older adults. However, as described below, there is mounting research in how and why older adults want to learn. This brief discussion is derived from work undertaken for a workshop in Hong Kong on issues in teaching older adults. It combines recent research on learning and teaching in higher education as it applies to older adults. The paper addresses: learning and ageing, learning choices, an information processing view of learning, deep and surface learning, teaching and learning approaches, formal, non-formal and informal learning, teaching roles, evaluation, and technology. This paper considers the lived experience and meaning of care for women born between 1946 and 1951, who are living alone, and are participants in the Australian Longitudinal Study of Women’s Health (ALSWH). The discussion is based on free-text survey comments (n = 150 women) and in-depth interview data (n = 15 women) exploring women’s experiences of ageing and their future expectations of care. It delves into tensions within relevant themes relating to care (unpaid informal care, volunteer work, and reciprocity), and the caring ethos, as described by women who are ageing in Australian communities. It highlights dilemmas faced by women who have grown old with the expectation of government support and care in later life, but who are facing the possibility this might not eventuate. These dilemmas are discussed with reference to governmental ethical-moral responsibilities for care within the context of user-pays welfare systems and profiteering within marketised aged-care services. The aim of this paper is to highlight emergent occupational health issues associated with an ageing workforce and sedentary working life, their implications for injury prevention, work ability, and worker well-being. Older workers have a higher rate of injury and disease and their injuries are more costly. It is also known that physical capacity declines with age which is a concern for workers engaged in physically demanding work. In addition to physiological deterioration and the development of occult disease associated with ageing, lower physical capacity may in part be due to lower participation rates in physical activity, and the increasing tendency for sedentariness in working life. Indeed, physical inactivity and sedentary time are increasing in working life and are known to be independently associated with a higher risk of chronic disease-related morbidity and mortality, events which are largely preventable. While ageing and injury and disease are associated with declining work ability, in future more workers will be required to remain in employment beyond current retirement age to meet future workforce demands. This paper will suggest that work ability and worker well-being in an ageing workforce may be optimized by adjusting organizational work practices, by ergonomic re-design and by increasing physical activity in working life. Applied research examples will demonstrate how targeted exercise, incidental physical activity and reduced sitting time can be facilitated in the workplace to promote well-being and work ability. Within the context of Australia's rapidly ageing population, this paper argues that there is a need to consider the positive contributions made by older people through their volunteer and civic engagement activities. There is a strong civil society tradition in Australia, and a growing body of evidence that demonstrates that older Australians contribute both as volunteers and more informally to their families, their peers, and their communities. Older Australians are active volunteers across a range of organizations, and contribute more time than other age group. Yet there are few policies and very few programs in Australia that promote and support volunteering amongst older people, despite the associated health benefits. There are also significant barriers to volunteering that require attention if these benefits are to be made available to a broader and more diverse group of older Australians. In Australia we have become preoccupied with the potential adverse impact of our ageing population on our health and social systems. The projected cost of having increasing proportions of our population in the over 70s, retired, chronically ill category of the demographic profile is emerging as a major challenge for governments and private insurers: so much so in fact that the government is now urging older people to stay at work longer. In America, new approaches to the management and self-management of chronic diseases have been invoked to encourage and support older people to improve their quality of life and reduce their recourse to and dependence upon health care technologies, clinical interventions and health care management systems. Unless this is achieved, it is argued, the cost of looking after this emerging ‘bubble’ of elderly people will become increasingly unsustainable as fewer and fewer (proportionately) younger people work to pay the taxes that support ageing, retired, sick and dependent populations. This paper argues that we are at real risk of having our economic wealth and productivity impeded and truncated by the financial burden of looking after high demand and high cost dependants at the aged end of the social demographic. This paper offers an alternative view of our ageing population, as well as highlighting some of the assets we have in our elderly populations, and providing suggestions as to an alternative view of the phenomenon of ageing that incorporates elements such as flexible working arrangements and the application of new, enabling technologies. This approach to our ageing population dilemma is predicated on a concept of lifelong learning and social participation along with better preventive and early intervention systems of health care. This paper presents a perspective on the development of knowledge in the spiritual dimension of aging, with particular emphasis on nursing and pastoral studies. Research that provides important understandings for these two disciplines but conducted by other disciplines will also be included. Issues of definitions of spirituality and the reasons for choosing to use the term "spirituality" rather than "religion" in the practice settings of nursing and pastoral care in a secular society are discussed. Study from the specialties of mental health, palliative care and issues of spirituality and chronic illnesses are examined. Finally, the state of knowledge in these two disciplines is summarized and future directions in research of aging and spirituality arc suggested. The paper is written from an Australian context. This paper presents data from a small study exploring the impacts of homophobia on the lives of older lesbian and gay Australians. Eleven in-depth interviews were conducted with older lesbians (6) and gay men (5) ranging in age from 65 to 79 years. The study found that participants’ sense of self was shaped by the dominant medical, legal and religious institutions of their youth that defined them as sick, immoral or criminal. Participants described enforced “cure” therapies, being imprisoned, having employment terminated and being disowned and disinherited by family. In this context, intimate relationships and social networks provided refuge where trust was rebuilt and sexuality affirmed. Many created safe spaces for themselves. This equilibrium was threatened with increasing age, disability and the reliance on health and social services. Participants feared a return to institutional control and a need to “straighten up” or hide their sexuality. In response, partners stepped into the role of caregiver, at times beyond their capacity and at a cost to their relationship. The study describes the importance of understanding social connections in the lives of older lesbians and gay men. It highlights the need for inclusive services to ensure that social networks are supported and that health and well-being are promoted. [ABSTRACT FROM PUBLISHER]

As the global senior population expands, there is increasing scholarly interest in the intersection between tourism and the aging population. After nearly four decades of scholarly inquiry, there is the need for a comprehensive review of emerging studies. Therefore, this study was initiated to investigate publications on senior tourists' motivations from 1980 to 2017. Specific objectives include the identification of publication trends over the years, the identification of research methodologies, and an analysis of the nature of the motivations that have emerged over the past three decades. To accomplish these, a systematic and snow-balling process was employed to identify relevant publications on travel motivations. Furthermore, a classification of senior tourists' motivations into domains is provided. The findings suggest a potential increase in publications in the coming decades, domination of certain markets, including the USA, Australia, and China, the emergence of four broad senior travel motivation domains divided into 13 sub-domains, and some distinctiveness and similarities in senior travel motivations in comparison with other motivation schemes. These motives vary across different generations of seniors. The variables identified in this study provide a useful theoretical understanding of the senior travel phenomenon and for future studies. This paper sets out to explore the Australian instance of a significant international problem: Intergenerational solidarity and the willingness of younger generations to support the future care of older people. It draws on Bengston's intergenerational solidarity theory, in particular his conception of normative solidarity relative to filial obligations, to analyse data from the Australian Survey of Social Attitudes. This data demonstrates evidence of intergenerational solidarity at the policy level and a need for a continued role for government in the provision of residential care, insurance schemes, and the payment of income to full time and occasional carers. At the family level there is less support for the role of adult children in the payment of formal care or the provision of informal care. There is also a significant difference between men and women concerning the direct provision of informal care to ageing parents. Suggestions for future research are highlighted. The Australian Imaging Biomarkers and Lifestyle (AIBL) study is a longitudinal study of 1,112 volunteers from healthy, mild cognitive impairment (MCI) and Alzheimer's disease (AD) populations who are assessed at 18-month intervals in order to enable prospective research into ageing and AD. Using a multidisciplinary battery, AIBL assessments comprise the extensive study of clinical factors and cognitive function, collection of blood and cerebrospinal fluid (CSF) samples for biomarker discovery, structural and β -amyloid (Aβ) neuroimaging, and obtaining information on diet and physical activity patterns of the cohort. Now in its seventh year, AIBL is part of a substantial international effort to prospectively study the relationships between clinical characteristics and putative AD biomarkers in groups who carry different risk factors for AD. The identification of biomarkers would provide a window of opportunity to assess AD risk in individuals prior to the onset of advanced clinical symptoms, in addition to facilitating testing of therapeutic and lifestyle interventions likely to emerge within the next decade that prevent or delay symptom emergence in those at high risk for developing AD. Productive ageing policy and programs emerged in Australia in the early 1990s, influenced both by international debate and proactive federal government leadership particularly in the area of economic and health planning for an ageing population. These economic and health frames have remained the dominant discourses surrounding productive ageing initiatives, driven both by the need to maintain growth and productivity in an ageing society and to restrain burgeoning health costs through more active preventative strategies. Social framings of productive ageing, from both the left and the right, have also been evident around ideas of mutual obligation, the building of social capital, and more recently, with a new government, around the concept of social inclusion. This paper explores the development of productive ageing policy and programs in Australia across the areas of employment/retirement, volunteering/civic engagement, care-giving, and lifelong learning. Background Australian government policy regards people with intellectual disability (ID) as citizens with equal rights, which means that they should have access to the same opportunities as the wider community. Ageing in place is central to aged care policy in Australia for the general population. Method This paper reviews policy to support the provision of similar opportunities to age in place for people with ID, and the reasons for its slow development. Results Due to lifestyle patterns earlier in the life course, many people with ID experience a mid-life disruption to their accommodation, and may live in a group home as they age or may move prematurely to residential aged care. The absence of mechanisms to adjust disability funding as needs change, and the existence of policy that denies residents in group homes access to community-based aged care, forces disability services to "go it alone" to support ageing in place. Conclusion Despite a national priority to improve the interface between the disability and aged care sectors, administrative and funding characteristics continue to obstruct the development of implementation strategies to support ageing in place for people with ID, which remain at the stage of an exploration of the issues. Learning is an important aspect of aging productively. This paper describes results from 2645 respondents (aged from 50 to 74 + years) to a 165-variable postal survey in Australia. The focus is on learning and its relation to work; social, spiritual, and emotional status; health; vision; home; life events; and demographic details. Clustering analysis showed that learning and health were most important. The relationship between variables was then investigated using the Gamma test by age, and factor and regression analyses. This paper discusses a study that examined why older people volunteer for a research registry based at the University of Queensland, Australia. A mailed questionnaire was utilized to explore a list of reported motives developed from an in-depth qualitative phase. An exploratory factor analysis of the findings was conducted, which showed that there were four main motivations for respondents to volunteer in this context. These motives were to make a contribution to society and to research; to be involved in research; to meet others; and to be informed about the university environment. These findings show that older research volunteers have a strong interest in university research outcomes and demonstrate a commitment to aging productively. There are lessons here for researchers and policy-makers, who need to develop additional ways to involve older people in the research that affects them. [ABSTRACT FROM AUTHOR]

Ageism is an ongoing pervasive issue that can be difficult to challenge. The aims of this study were to co-design alternative frames about aging in a workshop and to explore the process participants went through using these to challenge ageism in their communities over a three-month period. Participatory action research combined with co-design were used. Full-day workshops involving 27 participants across urban and regional Australia introduced participants to the concept of ageism, framing, engaging with community, and co-designing alternative frames of aging. An online group allowed participants to support each other and continue exploring ideas as they challenged ageism in their communities. After 3 months, follow-up debrief sessions involving activities and semi-structured interviews were conducted. Data from workshops (activities, recordings), online posts, and debrief sessions were analyzed. Participants found the reframing process useful for challenging aging stereotypes in the community. Over the three months they developed an increased awareness of ageism (in self, others, society) and challenged these in different ways, including changing their internal dialogue, challenging others' views on aging, and actively defying stereotypes. Five alternative frames emerged: diversity and human rights; empowerment and support; value/contributions; lifelong learning; intergenerational. The aim of this study was to explore older adults' experiences of and approaches to managing their long-term health and mobility after traumatic injury. A longitudinal qualitative study was undertaken with older adults following traumatic injury in Victoria, Australia. Fifteen participants (≥65 years) were interviewed at three years post-injury (n = 15), and re-interviewed at four (n = 14) and five years (n = 12) post-injury. Using a framework approach, a longitudinal thematic analysis was performed. Older age at the time of injury was identified by participants as a key factor influencing their recovery. Many participants reported actively attempting to regain their strength and fitness in the first five years following injury. However, their age, injury impacts, other health conditions, and weight gain made it difficult to achieve recovery goals. Many older adults reported a decline in their physical function over time. While these experiences and persistent disability constrained or changed the quality of social relationships, community participation, and independence, several participants described adapting to their functional limitations, and managing their secondary conditions over time. In our cohort, the intertwined combination of ageing, injury, and comorbid conditions negatively affected health and mobility, reinforcing the need for preventative strategies. Older adults recovering from traumatic injury may benefit from specialised care pathways that offer long-term and tailored therapies, with programs and services specific to their needs and goals. An integrated service approach by injury insurers, health care, primary care, disability, and aged care could more clearly identify and effectively address the individual needs and goals of older adults with complex conditions. Health and social services that work with people with injuries to develop personalised coping strategies can reduce anxiety related to uncertainty about the future, promote well-being, and support participation in valued activities. Occupational therapists have skills to work with people considering or experiencing the transition from paid work to retirement. The aims were to explore: (1) occupational therapists' experiences and perceptions in working with people transitioning to retirement; (2) current scope of practice of occupational therapy in the work-to-retirement transition; and (3) factors influencing current scope of practice of occupational therapy in the work-to-retirement transition. Qualitative semi-structured interviews with Australian occupational therapists were thematically analysed. Fourteen Australian occupational therapists were interviewed. Three overarching themes (12 sub themes) emerged: (1) occupational therapists' application of a contemporary occupational paradigm; (2) current approaches adopted by occupational therapists are add on, stylistic and talk based; and (3) contextual challenges and opportunities encountered by occupational therapists. Occupational therapists use enablement skills (e.g. coach; collaborate) and consider various professional reasons (e.g. engagement in meaningful occupation; health responsibility) when addressing the transition from work to retirement. Overcoming barriers to service provision (e.g. funding; lack of role clarity) will facilitate a more comprehensive service provision to enable health and well-being of older adults. Divorce is a life-changing event with financial implications for women. Although large-scale studies have examined the factors associated with financial coping after divorce, little attention has been paid to the lived experience of women over time. In this study, we used mixed methods to examine the financial well-being of divorced women over 20 years from 1996 to 2016. Using data from the Australian Longitudinal Study on Women's Health [ALSWH], we analyzed women's ratings of their ability to manage on available income, and their narrative comments about financial coping over eight waves, beginning in midlife (ages 46–51). The ratings improved over time, particularly as women reached peak career in their 50s or 60s or entered retirement. Despite this upward trajectory, financial strain persisted for ~40% of the cohort who faced poor health or diminishing job prospects. We conclude that, although financial hardship often eases over time, women's early ratings of financial coping predict levels of income security in older age. [ABSTRACT FROM AUTHOR]

Addressing midlife hearing loss could prevent up to 9% of new cases of dementia, the highest of any potentially modifiable risk factor identified in the 2017 commissioned report in The Lancet. In Australia, hearing loss is the second-most common chronic health condition in older people, affecting 74% of people aged over 70. Estimates indicate that people with severe hearing loss are up to 5-times more likely to develop dementia, but these estimates vary between studies due to methodological limitations. Using data from the Sydney Memory and Aging Study, in which 1,037 Australian men and women aged between 70 and 90 years were enrolled and completed biennial assessments from 2005-2017, investigations between hearing loss and baseline cognitive performance as well as longitudinal risk of neurocognitive disorder were undertaken. Individuals who reported moderate-to-severe hearing difficulties had poorer cognitive performances in the domains of Attention/Processing Speed and Visuospatial Ability, and on an overall index of Global Cognition, and had a 1.5-times greater risk for the neurocognitive disorder during 6-years' follow-up. Hearing loss independently predicted risk for MCI but not dementia. The presence of hearing loss is an important consideration for neuropsychological case formulation in older adults with cognitive impairment. Hearing loss may increase cognitive load, resulting in observable cognitive impairment on neuropsychological testing. Individuals with hearing loss who demonstrate impairment in non-amnestic domains may experience benefits from the provision of hearing devices; This study provides support for a randomized control trial of hearing devices for improvement of cognitive function in this group. This study investigated the stability of propositional density (PD) in written texts, as this aspect of language shows promise as an indicator and as a predictor of language decline with ageing. This descriptive longitudinal study analysed written texts obtained from the Australian Longitudinal Study of Women's Health in which participants were invited to respond to an open-ended question about their health. The 635 texts used for this study were taken from 127 middle-aged women who responded to this question on each of the five surveys conducted at 3-year intervals over a 16-year period. The study made use of an automated PD rater (CPIDR-3) for the analysis. PD was found to be a stable measure over time when comparing the grouped data, but there was between- and within-subject variation over time. This article seeks to assess the "goodness of fit" between the social science literature on the impact of older voters on electoral outcomes and the Australian experience of the politics of aging. While the literature suggests that the notion of senior power is a flawed one, Australia's 2004 federal election campaign indicated that this is not quite so. This article offers a possible explanation for the difference between the literature and the Australian experience in terms of the calculus that underpinned the election campaign, namely, capturing the votes of swing voters in marginal seats including "silver" swing voters in seats with a disproportionate number of older voters. The preliminary findings of a small exploratory study of campaign strategists suggest that there may be a real basis to this explanation. Government budgetary pressures and demographic trends have made retirement income policy a priority in developed economies. One option for policy reform is to increase private saving. In Australia, legislation requiring compulsory employer payments for the purposes of retirement savings addresses this option. This system poses particular difficulties for women who have broken patterns of paid employment and relatively low wages. When simulations that project likely employment participation and retirement outcomes incorporate a gendered approach and focus on the “baby boomer” cohort, the results highlight the low probability that women will accumulate adequate independent private retirement income. Over their lifetimes, Australian women baby boomers will spend around 35 percent less time in paid employment than their male counterparts. The projected average gender gap in compulsory accumulations is of a similarly large magnitude. This article discusses aged care to gay men living in Inner City Sydney, Australia. The stories of two men interviewed for The Aging Men's Health Project are highlighted in the article. There are many vulneraries for gay aged men as there are many barriers to accessing services for this group, such as the fact that many conceal their identity or avoid using the services. The past research done on this subject has led to a recent surge in recognizing the aged gay and lesbians more extensively. There are other minority groups that are starting to be recognized for studies. The aims of the article are outlined such as the presenting of data over a three year ethnographic study on the subject and to show the disadvantages that aged men in Sydney have compared to the rest of the community. The inner city services for gay aged men is explained. There are many problems with the negligence of older men in gerontology as there are areas of general concern for aging gay men which are similar to most adults: loneliness, health and income. Older gay men may be at greater risk of entering old age with additional problems, such as poor mental health, due to stigmatization and hostility towards homosexual identity.

Research suggests that lesbian and gay people's disclosure of their sexual orientation to parents is associated with better mental health and identity adjustment. However, adolescents and younger adults have been the main focus with little known about the experiences of older people. The following study focused on older lesbian and gay adults, and examined whether believing that their parents knew about their sexual orientation is linked to better current mental health and identity adjustment. A survey of 548 lesbian and gay adults aged 60 years and older in Australia measured psychological distress, positive mental health, internalised homonegativity, sexual identity affirmation, and whether participants believed their parents knew about their sexual orientation After controlling for age of first disclosure, whether their parents were alive, and socio-demographic variables, women who reported at least one parent definitely knowing of their sexual orientation were significantly lower on psychological distress and higher on positive mental health and identity affirmation than those who reported neither parent knowing or were uncertain of their parents' knowledge. No significant effects were found for the men. Believing that at least one parent definitely knew about their sexual orientation was linked to better mental health outcomes among lesbian women, but not among older gay men. These findings reveal a potential risk factor for poorer mental health among older lesbian women, as well as important gender differences, and may be useful in understanding and supporting the well-being of older lesbian and gay adults. Appropriately skilled staff are required to meet the health and care needs of aging populations yet, shared competencies for the workforce are lacking. This study aimed to develop multidisciplinary core competencies for health and aged care workers in Australia through a scoping review and Delphi survey. The scoping review identified 28 records which were synthesized through thematic analysis into draft domains and measurable competencies. Consensus was sought from experts over two Delphi rounds (n = 111 invited; n = 59 round one; n = 42 round two). Ten domains with 66 core competencies, to be interpreted and applied according to the worker's scope of practice were finalized. Consensus on multidisciplinary core competencies which are inclusive of a broad range of registered health professionals and unregistered aged care workers was achieved. Shared knowledge, attitudes, and skills across the workforce may improve the standard and coordination of person-centered, integrated care for older Australians from diverse backgrounds. This qualitative study explored the barriers and facilitators to sexual communication between older adults and friends. Fifty-three women and men aged 58 and older were interviewed about their intimate relationships and sexual behaviours and attitudes. Findings indicated that talking about sex with friends played an important role in providing support and sharing information. The privacy of the topic meant that trust and confidentiality had to be in place before sexual conversations occurred, and that discretion was required for those married or in a relationship due to potential breaches of privacy. Stereotypes associated with older age made talking about sex 'risky' as participants were vulnerable to scrutiny. Growing-up during a time when sex was taboo influenced willingness and comfort in talking about sex today. Among those who did talk with friends, women tended to talk to women and men to men. These findings are significant in the context of an increasing global population of older adults and silence around sex and ageing. By exploring sexual communication outside of the healthcare context, where previous research has focused, the findings indicate novel ways to support the sexual health and well-being of older adults. There is a dearth of research exploring the unique contribution of social work in residential aged care, particularly in Australia. This scoping review addresses this gap by documenting the international literature on the role of social work. Ten peer-reviewed databases were utilised, resulting in 77 relevant articles. The results demonstrated that social work has a key role in aged-care facilities by contributing to an integrated, person-centred, and holistic model of service delivery. IMPLICATIONS Contributing to the dignity, worth, wellbeing, and quality of life of all people who live in aged care requires a holistic and integrated model of care that involves the inclusion of social work. Australia falls behind other countries in providing holistic, person-centred, psychosocial support. Objectives: To ascertain the trajectories of mental health among women in Australia assessed in repeat waves from their early 70 s to the end of their lives or their mid 80 s. Method: Secondary analysis of data contributed by the 1921–26 cohort of the Australian Longitudinal Study of Women's Health Waves 1–6. Primary outcome was the 4-item SF-36 Vitality Subscale, which assesses mental health as life satisfaction, social participation, energy and enthusiasm. Structural, individual and intermediary factors were assessed using study-specific and standardised measures. Trajectories were identified using Growth Mixture Modelling and associations with baseline characteristics with Structural Equation Modelling. Results: 12,432 women completed Survey One. Three mental health trajectories: stable high (77%); stable low (18.2%) and declining from high to low (4.8%) were identified. Compared to the stable high group, women in the stable low group were significantly less physically active, had more nutritional risks, more recent adverse life events, fewer social interactions and less social support, reported more stress and were more likely to have a serious illness or disability at Survey One. The declining group had similar characteristics to the stable high group, but were significantly more likely to report at baseline that they had experienced recent financial, physical and emotional elder abuse. These interact, but not directly with socioeconomic position and marital status. Conclusion: Mental health among older women is related to social relationships, general health, access to physical activity and healthy nutrition, coincidental adverse life events and experiences of interpersonal violence, in particular elder abuse. Objective: To present normative performance data on the Modified Mini-Mental State (3MS) examination for healthy community-dwelling older individuals according to gender, age, education level, and ethno-racial group. Method: More than 19,000 generally healthy older men and women without a diagnosis of dementia were recruited from the general population in Australia and the U.S. for the ASPirin in Reducing Events in the Elderly (ASPREE) study. The 3MS exam was administered as part of the baseline screening and individuals scoring above 77 were eligible to participate. Results: The sample comprised 16,360 Australian whites, 1080 U.S. whites, 895 African-Americans and 316 Hispanic/Latinos. The median age of participants was 74 years (range 65–98), with an average of 12 years of education and 56% were female. Increasing age and fewer years of completed education were associated with lower scores on the 3MS. Women scored higher than men in most age and education categories. Differences across ethno-racial groups were found. With factor analysis, four factors were identified which accounted for 35% of the between-person variance in 3MS scores for white Australians. Conclusions: This large cohort of older individuals provides some of the most comprehensive 3MS normative data to be generated for whites (Australian and U.S.), Hispanic/Latinos and African-Americans, by age, gender, and educational attainment. These findings will serve as important reference standards for monitoring cognitive function in generally healthy older individuals, becoming increasingly important as this fraction of the population increases. Accurate detection of mild cognitive impairment (MCI) is important to stratify and address risk. Yet, few short cognitive screening instruments are validated for this. . In Australia, all clients referred to an Aged Care Assessment Team (ACAT) receive comprehensive geriatric assessment (CGA) including the Standardized Mini-Mental State Examination (SMMSE). We compared the accuracy of the quick mild cognitive impairment (Qmci) screen to the SMMSE in 283 participants: 195 with dementia, 47 with MCI, and 41 with subjective cognitive decline (SCD) in an Australian community-based ACAT. Both had similar accuracy in identifying dementia, AUC of 0.86 for the Qmci versus 0.93 for the SMMSE (p = 0.10), but the Qmci was more accurate than the SMMSE in differentiating MCI from SCD, AUC of 0.84 versus 0.71, respectively, p = 0.046. Background: Clinical deterioration and adverse events in hospitals is an increasing cause for concern. Rapid response systems have been widely implemented to identify deteriorating patients. Aim: We aimed to examine the literature highlighting major historical trends leading to the widespread adoption of rapid response systems, focussing on Australian issues and identifying future focus areas. Method: Integrative literature review including published and grey literature. Results: Seventy-eight sources including journal articles and Australian government matierlas resulted. Pertinent themes were the increasing acuity and aging of the population, importance of hospital cultures, the emerging role of the consumer, and proliferation, evolution and standardisation of rapid response systems. Discussion: Translating evidence to usual care practice is challenging and strongly driven by local factors and political imperatives. Conclusion: Rapid response systems are complex interventions requiring consideration of contextual factors at all levels. Objectives: This study explored unique and personal domains associated with quality of life (QOL) in 153 mid-aged and older Australian adults over age 50, using an innovative measure of individualized QOL, the Schedule for the Evaluation of Individual Quality of Life (SEIQOL-DW). Methods: Demographic measures as well as the SEIQOL-DW were administered. The qualitative responses on this scale were analyzed by Leximancer, a text analysis program that extracts semantic meaning and relational information automatically from text. Results: For the sample as a whole, the most important QOL domain was found to be Relationships, followed by Family, Health, Activities, Community, Security, Beliefs, Independence and finally Wellbeing. When the sample was broken down into age bands for analysis, the most important domain theme was Family (50-59 year olds), Relationships (60-69 year olds), Relationships (70-79 year olds), and Health (80+ year olds). Conclusions: The wide range of concepts elicited in each domain via Leximancer underscores the argument for analyzing QOL in an individualized way, as it was clear from these concepts that different domains had unique meanings to each person. Clinical Implications: Understanding QOL at the level of which domains in an individual are important to QOL in later life is critical to providing services to both healthy aged as well as those with health and mental health needs. The SEIQOL-DW affords clinicians a unique tool with which to describe an individual's unique and relevant quality of life areas, and affords a way to measure change in these areas as a result of interventions. Policies and programs relating to successful aging in place are premised on the fact that older people's housing is stable and suitable. Home-based aged care delivery models such as individual budgets aim to deliver increased service flexibility, choice, and control for consumers. Little attention has been paid to the consequences of such models for older renters, who often have less stable housing and restricted rights to modify their accommodation. Objectives: This article examines relationships between experiences of ageism and four specific mental health outcomes among older Australian adults, including whether these relationships vary depending on age, gender, and sexual orientation. Methods: A survey was conducted nationwide involving 2137 participants aged 60 years and older. Mental health variables included depressive symptoms, anxious symptoms, general stress, and positive mental health or flourishing. Results: Recent experiences of ageism were found to be strongly related to poorer mental health on all four mental health variables. However, experiences of ageism appeared to have a greater effect on the mental health of those who were younger in age (specifically depression), of men more so than women (specifically depression), and of those who identified as heterosexual as opposed to other sexual orientations (specifically general stress). Conclusion: These findings suggest that experiences of ageism may be an important factor in the health and well-being of older adults, especially for those who are younger, male, and heterosexual, and may need to be taken into account when devising strategies for supporting healthier and happier ageing. This article is an overview and summary of the findings from a General Research Fund project funded (2012–2014) by the Research Grants Council of the University Grants Committee of Hong Kong to investigate cross cultural understanding of ageing and learning by seniors in Hong Kong and Australia. Results have been published separately in Boulton-Lewis, & Buys (2015), Boulton-Lewis, Aird, & Buys (2016), Boulton-Lewis, Tam, Buys, & Chui (2016), Tam (2016), Tam & Chui (2016) and Tam, Aird, Boulton-Lewis & Buys (2016). These articles have dealt separately and variously with either a qualitative or quantitative analysis of learning choices in Australia, structural barriers to learning in Australia, a comparison of views of and ageing and learning in both cultures, Chinese views of later life learning, the meaning of ageing and learning to Chinese elders and an analysis and comparison of results in both places. The focus of this article is a summative discussion based mainly on the quantitative and qualitative analyses of the main themes of ageing and learning. Extensive research examines senses of home in aging, focusing on built and care environments. In parallel, a body of work examines housing governance, reviewing degrees of resident satisfaction. However, there is little crossover between this work, with governance research only rarely considering senses of home. This article argues for attention to the interconnections between provider-scale housing governance and senses of home in older age, arguing that governance mechanisms structure the housing context and provide the framework through which home is experienced and lived. Through a case study of a small, affordable housing community in Sydney, Australia, it shows that housing governance can profoundly shape senses of home in aging, both contributing to and diminishing senses of home. Further, it points to a connection between housing governance and housing design with different housing typologies associated with different aging bodies and forms of management practice, with profound implications for residents' senses of home. Objectives: This study assesses nonintentional injury deaths of older farmers and the causal agents associated with these fatalities in Australia (2001–2015). It also explores approaches based on the hierarchy of risk controls to reduce fatalities and injuries in this cohort. Methods: Data on farm-related incidents were sourced from the National Coroners Information System (NCIS) for the study period and coded. Rates were calculated and regression analyses completed to assess trends over time. Results: Those in the 50+ years category (n= 610) accounted for 49.8% of all on-farm nonintentional injury deaths, with males (n= 566) dominating the cases. The annual mean was 40.66 cases (SD= 8.08) and the average rate 36/100,000. Trend analysis revealed a nonstatistically significant (P< 0.05) increase in cases over the period. Farm vehicles and machinery were responsible for almost two thirds of the fatal cases. Conclusion: Nonintentional fatality rates for older farmers have remained relatively unchanged for a significant period in Australia. There is a need to examine additional approaches that not only maintain the benefits of work activity for older farmers but also balance this against a safety perspective. [ABSTRACT FROM AUTHOR]

In this article, the discussion of loss and its relationship to learning is based on the analysis of interview data from 39 older adults in Hong Kong and 40 in Australia. The focus of the research was on ageing and learning. The phenomenon of life changes, specifically losses, and their relationship to learning was frequently mentioned, and this theme is described here. Three different kinds of loss were identified in the data as motivators for organized or nonorganized learning or for negatively affecting learning. These included loss of aspects of health, of job, or of partner. The results are potentially useful to policy makers and learning providers in encouraging older adults to engage in learning as one way of coping with loss. [ABSTRACT FROM PUBLISHER]

Australia, like all developed Western countries, is experiencing a demographic shift resulting in an increasing proportion of the population being over the age of 65 years. Contrary to stereotypes, the vast majority of older people live independently in communities. This article explores the potential of social work practice informed by community development principles to enable socially disadvantaged older women to live in vibrant and supportive communities, in which they feel safe and are able to access the support services they need. It argues that participation in social action not only builds older women’s well-being but also enables them to become (or continue to be) agents for social change in local communities. Adopting a community-based research methodology, this article draws on a decade of community development practice with the Concerned Older Women’s (COW) Group. This data suggests that community development practice based on participation, empowerment, and social action founded on respectful relationships may accrue significant benefits to individuals and the broader community. Aims: Handwriting processes deteriorate with age and following neurological conditions. Improving handwriting is often a focus of rehabilitation. Yet knowledge of handwriting legibility in the elderly is limited. This study describes the distribution of handwriting legibility scores in healthy older adults, relationships between handwriting legibility, age and writing task, and reliability of rating procedures. Methods: A cross-sectional study design was used involving 120 healthy older Australians. Tasks included writing sentences, shopping lists and transcribing a telephone message. Legibility was scored using the modified Four Point Scale—version 2. Results: Legibility differed between tasks but was not related to increasing age. Conclusions: Impaired handwriting legibility in the elderly is less likely due to the effects of aging than the required task or medical conditions. The author of this conceptual article was awarded a Churchill Fellowship to travel to the United States, Ireland, and United Kingdom to meet with researchers, practitioners, and community agencies and review how support for people with intellectual disability is provided. A specific focus was upon provision of care for individuals who were either ageing or requiring palliative care. The project took place from June to August 2013 and involved face-to-face interviews and site visits with key academics, government representatives, and community providers in Los Angeles, Chicago, New York, New Jersey, Dublin, Edinburgh, London, Oxford, Cambridge, Bath, Bristol, Birmingham, and Stoke-on-Trent. A priority was placed upon examining how the key features of leading practice models could be implemented within the Australian environment featuring a National Disability Insurance Scheme (NDIS). On the basis of the observations of practice, key points are discussed regarding the potential for improvements to the support of people with intellectual disability within the Australian context of the NDIS.
