Background: Historical socio-political processes have produced gross inequity of health resource for Aboriginal Australians and New Zealand Māori. Objectives: This paper argues that socio-political factors resulting from the entrenchment of colonialism have produced significant personal and structural barriers to the utilisation of healthcare services and directly impact the health status of these two vulnerable groups. Design: Discussion Paper. Conclusions: Understanding the actual barriers preventing the utilisation of healthcare facilities, as perceived by Indigenous people, is essential in reducing the gross disparity between Indigenous and non-Indigenous morbidity and mortality in Australia and New Zealand. For hospitals and other health-care services, identifying older people experiencing abuse remains a challenge. Social workers at St Vincent's Hospital Melbourne (SVHM) used a clinical data mining approach to retrospectively audit electronic medical records containing 466 episodes of care over a five-year period where elder abuse was identified based on a system of alerts. The findings highlight key characteristics of the vulnerable older persons, the perpetrators, their social contexts, and interventions undertaken, clarifying a range of vulnerability and risk factors in the sample. Half were aged 80 years and over; two thirds lived with the person of concern; two thirds were from culturally and linguistically diverse backgrounds, and a third had cognitive impairment. A high proportion were assessed as being at moderate to high levels of risk, requiring urgent to prompt intervention. Audit findings strengthened knowledge and led to improvements in elder abuse responses by social workers, which included policy development, training, risk management and safety planning processes, resourcing, and research initiatives. This paper describes how social workers engaged in data collection and analysis to inform their practice within a health-care setting and contributed to service evaluation and improvements. IMPLICATIONS This paper demonstrates how social workers can conceptualise and engage in research to better understand a social problem: in this case, it was responding to elder abuse, within a hospital setting. In Australia, like many high-income countries, food insecurity is associated with increased risks of chronic diseases, sub-optimal development outcomes in children, and mental health conditions including depression and anxiety. Food insecure households employ a range of strategies, including the use of food charity, to help alleviate hunger and meet cost of living pressures. The aim of this paper is to investigate the lived experience of food insecurity for welfare-dependent households, and to examine these experiences within a structural violence framework. Structural violence investigations seek to understand the distal causal factors that can help explain poor health patterns and inequities. Semi-structured interviews were conducted with customers (n = 78) of food pantries, soup kitchens, and community development programs (June 2018 to January 2019) in the state of Victoria, Australia. Thematic analysis established evidence of controlling, demeaning and depriving practices in the interactions between the participants and the services and staff at national welfare providers and food charities. The same providers and charities nominally set up to address the exact situations in which participants found themselves. To examine the need, feasibility and acceptability of speech-language pathologists (SLPs) implementing a systematic, routine, unmet social needs identification and referral pathway, as a means of promoting health equity and addressing Sustainable Development Goals (SDGs). Quality Improvement methodologies were used to adapt and pilot an unmet social needs identification and referral pathway for use with parents/carers of children with communication disabilities referred to an urban Australian speech-language pathology service. SLPs were surveyed about the acceptability and feasibility of this practice. The majority of parents/carers, 289 of 293 (99%), agreed to participate in the study, with 31 of the 289 (11%) reporting concerns about unmet social needs. The most common unmet need related to household bills (n = 17, 28%), followed by childcare (n = 12, 20%), employment (n = 10, 16%), food (n = 8, 13%), housing (n = 7, 11%), and parent/carer education (n = 7, 11%). The majority of these families, 26 of 31 (84%), requested referral to, or information about, local community services/resources. SLPs reported high levels of acceptability (93%) and feasibility (98%). This study demonstrates the need, feasibility and acceptability of SLPs implementing an unmet social needs identification and referral pathway, and the potential to scale this initiative across other speech-language pathology services and allied health contexts. An increasing focus in aged care policy is supporting older people to age at home. However, there is little recognition of the health and independence of older private renters living in a space they do not own or control. This paper draws on in-depth interviews and assessments undertaken in the homes of 27 older tenants in the Brisbane environs aiming to understand how renting privately influenced their ability to meet their basic needs. An analysis of the findings highlights the disadvantage experienced by many older private renters. Most older private renters were unable to meet their basic needs given unaffordable rent whilst reliant on the aged pension. Further, with limited access to community aged care including home modifications, the health and independence of older private renters was found to be at risk. IMPLICATIONS Community aged care is difficult for older private renters to utilise in Australia, with unaffordable services and tenancy law limiting access to community aged care services including home modifications. Introduction: For parents who have experienced childhood maltreatment, parenting an adolescent may trigger memories of abuse or neglect, intensifying parent-adolescent conflict. This paper extends an earlier report of outcomes from a pilot randomised controlled trial (RCT) of Tuning Relationships with Music™, an intervention for parent-adolescent dyads where parents have a trauma history and dyads are experiencing conflict, in order to look systemically at dyads' emotion regulation during their musical representation of nonverbal conflict interaction (NCI). Method: The RCT randomly allocated 26 parent-adolescent dyads into intervention or control conditions, and analysed dyads' self-report and observation measures completed at baseline and 4-month post-baseline follow-up. Aims of the current paper were (1) to examine relationships between parents' trauma history, parent-adolescent conflict, parents' reactivity and responsiveness, and dyads' emotion regulation, consistency and predictability during NCI; and (2) whether a systemic music intervention focused on nonverbal communication (NVC) and emotion regulation would change the dynamics of dyads' relational conflict. Results: Higher parents' childhood betrayal trauma and adolescent-reported conflict scores were correlated with predictable NVC sequences whilst dyads were emotionally dysregulated, and parents' reactivity was correlated with dyads' inconsistent NVC during NCI. Post-intervention dyads were more emotionally regulated, consistent and predictable during NCI, reported lower levels of conflict and showed increased parent emotional responsiveness and reduced reactivity. Discussion: Findings indicate that where parents have experienced childhood maltreatment, conflictual parent-adolescent dyads may benefit from intervention focusing on skills that promote emotionally regulated, predictable and consistent NVC during conflict interactions. This paper reviews the literature on best practices to engage parents in order to equip them to support their children's higher education aspirations. Parents from low socio-economic status (SES) backgrounds, in common with other parents, report that they want "the best" for their children's future. Getting a good education is a part of the aspiration of most parents, regardless of SES. However, unlike parents from mid and high SES backgrounds who usually have "educational cultural capital" to support their children's educational aspiration, parents from low SES backgrounds often are unable to confidently access information they need about possible education pathways within and beyond school. Additionally, they need to know where to find out about financial and other support resources available to facilitate access to higher education. Higher education participation by students from non-traditional backgrounds could be improved by outreach that promotes parents' social capital and "educational cultural" capital. This project has built upon University of Tasmania and University of Wollongong's combined experience, international literature and workshops with parent engagement practitioners to identify features of parent engagement programmes and resources that are efficient and effective in engaging parents from low SES backgrounds to support their children's higher education aspirations. This two-year (2016–2018) study aimed to identify what a good life is for Aboriginal people with disability in remote Central Australia and how service providers can support them to achieve a good life. This paper presents the findings that relate to barriers to delivering services for Aboriginal people with disability. In-depth interviews and focus groups were held with Aboriginal people with disability and their carers aged at least 18 years from the Ngaanyatjarra Pitjantjatjara Yankunytjatjara (NPY) Lands and community organisations providing services there. The data were analysed using thematic analysis. There were 109 participants, of whom 47 were workers in service provider organisations and 62 were Aboriginal people. From the data, barriers to delivering services to support Aboriginal people to live a good life and solutions to overcome the barriers, were identified and described under the headings of environmental barriers and systemic issues. We discuss the policy implications of these findings with regard to addressing Indigenous disadvantage and how governments, service providers, communities, and Aboriginal people with disability and their families can work in partnership to address these barriers. Indigenous people with disability living in remote and very remote communities experience significant access and equity barriers to culturally responsive services that enable them to live a socially and culturally engaged life. Localised government and service provider disability policy approaches in Indigenous communities need to focus on both environmental and systemic issues. Greater investment in local remote communities is required to build the capacity of Indigenous families to support Aboriginal people with a disability to live a culturally and socially included life. This paper examines the experiences of a small group of socio-economically disadvantaged people and their attempts to access disability welfare under Australia's National Disability Insurance Scheme (NDIS). We interviewed 32 individuals with some form of disability who were potentially eligible to apply for the NDIS. Our research revealed that only a small proportion of a sample successfully applied to the NDIS, with our interviewees characterised as 'hard-to-reach' because of their socio-economic disadvantage, social isolation, and lack of adequate support. Reflecting on the many and varied experiences of this group, we consider our interviewees to be at risk of further marginalisation in the absence of effective measures to engage and support them. We recommend ways to bring the hard-to-reach group within the ambit of the NDIS, as well as possibilities for future research. This article reports on the experiences of people who have attempted to engage with Australia's national disability support scheme. Only a few people within our sample had successfully applied for support under the national scheme, with others either unaware of it or unsuccessful in their applications Many in our sample were effectively excluded from the scheme due to the complexities of disability, social exclusion, and financial hardship. Further research in this area is critical so that the problems faced by people in the hard-to-reach group, with respect to accessing the national disability support scheme, can be addressed. This paper begins by identifying the growth in female-headed households in Australia. Despite this increase, it is argued that females still lag males in terms of home ownership rates (HORs) with women in Australia facing similar ownership challenges as those experienced in the UK and the USA. In seeking to establish any dynamic change, the study analyses female home ownership rates in Australia for two points in time, 1998 and 2008. Logit models are used to predict HORs. The results indicate that factors such as household type, income and source of income have an impact on rates of ownership. Significant variations in educational outcomes across both the spatial and socioeconomic spectra in Australia have been widely debated by policymakers in recent years. This paper examines these variations and decomposes educational outcomes into 3 major input factors: availability of school resources, socioeconomic background, and a latent factor that links to the specificities of the local education system such as efficiencies. The proposed method respects the multi-dimensional nature of educational outcomes by estimating structural parameters of an extended education production function with multiple outcome measures. The results indicate that all factors contribute to the variations. Nevertheless, socioeconomic factors dominate non-school-based measures such as the tertiary education enrolment rate. The study also reveals significant differences in education system efficiencies across areas, suggesting compounding factors are often responsible for poor educational performance. Current international targets aim for 90% of people diagnosed with HIV to be on antiretroviral treatment (ART). This paper aims to identify sociodemographic and attitudinal factors associated with ART non-use over time in three samples of Australian people living with HIV (PLHIV). Data for this paper were derived from an Australian cross-sectional survey of PLHIV that was repeated at three different time points: 1997, 2003, and 2012. There were approximately 1000 respondents to each survey (n = 3042 in total). The survey included approximately 250 items related broadly to health and well-being, ART use, and attitudes towards ART use. Univariate and multivariate logistic regression analyses were used. While the proportion of participants using ART increased between 1997 and 2012 (78.8–87.6%,p < .001), there was a decrease between 1997 and 2003 to 70.6% (p < .001). Factors linked to ART non-use remained steady over those 15 years. In all cohorts, people less likely to be using ART were younger and had a more recent diagnosis of HIV. In 2003 and 2012, people in full-time employment were less likely to be using ART, while those whose main source of income was a pension or social security were more likely to be using ART. Multivariate models showed that, at each time point, a belief in the health benefits of delayed ART uptake was associated with non-use. These findings suggest that there may be barriers to ART uptake that have persisted over time despite changes to clinical guidelines that now encourage early uptake. [ABSTRACT FROM PUBLISHER]

Using specially generated tabulations of Muslim and non-Muslim Australians from the 2006 Census, this paper examines the social and economic position of Muslims in Australia and implications for their social inclusion. Although Australian Muslims come from more than 130 countries, the largest number, 38%, are Australian-born and almost 40% are younger than 20 years of age. Educationally they are high-achievers but on all indicators of socioeconomic well-being they fall into a very disadvantaged category. Twenty-one% of adult Muslim men have a university degree compared with 15% of non-Muslim Australians, yet their age-specific unemployment rates are two to four times higher than those of non-Muslim Australian; their rate of home ownership is half the national average; 40% of Muslim children are living in poverty and only 25% of Muslim households have above-average household. Thus, a significant proportion of Muslim Australians occupy a relatively marginal position both socially and economically in Australian society. This marginalisation fosters intergenerational transfer of disadvantage and may also contribute to their alienation from Australian society and its values which makes them vulnerable to religious and non-religious radicalism. The paper will argue that socioeconomic marginalisation and a sense of relative deprivation are often breeding grounds for religious and non-religious radicalisation. Our choice for care at the end of our lives is constrained by many factors, including the options available to us, our capacity to choose and the social structures that constrain our options and therefore our choices. Working with the interaction between personal agency and social constraints is a core public health activity. An intentional public health approach to palliative and end-of-life care can elucidate the direct relationship between our social circumstances and the quality of our end of life and uncover the implications of structural inequity for end-of-life choice. The approach reorients systems and settings to achieve accessible and equitable palliative and end of-life care for all, and identifies contributions that all jurisdictions, settings, organisations, sectors and communities can make to improving end-of-life care outcomes. Frameworks that support this shift in practice and policy are however in their infancy. Implementation frameworks that can structure and guide 'how' to translate public health palliative care concepts into sustainable practice are needed. Purpose: To examine whether differences in access to specialist disability services by people from culturally and linguistically diverse (CALD) backgrounds when compared with those born in Australia represent a service gap or the healthy migrant effect.Method: To use the latest disability statistics to measure the different rates of people with disability, and the rates of people with profound and severe disability, of people born in Australia and those born abroad; to compare the difference between those who mainly speak English with those who primarily speak a language other than English at home (LOTE); and examine the age-specific and standardised disability rates of these subgroups.Results: The rate of access of specialist disability services by people with disability who were from CALD backgrounds is highly disproportionate to their presence in the community. As a whole, people from CALD backgrounds have a similar level of disability as Australia-born people. They have a greater rate of profound and severe disability and a higher level of need for assistance in undertaking core activities. For younger age cohorts targeted by specialist disability services, there is little difference in the level of need for assistance between people from CALD backgrounds and the rest of community. Those people who mainly speak LOTE at home have a relatively higher level of need for assistance than those who speak mainly English at home.Conclusions: The paper reveals a substantial gap in specialist disability services between people from CALD and the broader community. This cannot be explained by the difference in the level of need for assistance between Australia-born and overseas-born populations, therefore raises some policy questions as to the barriers to the use of such services and how to effectively narrow the service access gap and improve utilisation rates.Implications for RehabilitationThe paper reveals a substantial accessibility gap in specialist disability services between people from culturally and linguistically diverse (CALD) backgrounds and the broader community in Australia.Rehabilitation is a large component of disability services. Over the past two decades, migration from Mainland China (MC) to Australia has become increasingly significant. In contrast to previous migrant waves, post-2000 MC-born migrants in Australia are more likely to be highly skilled and/or financially independent, and often migrate under skilled, business and investment visa programs. Though scholarship has explored various facets of MC-born migrants' settlement in Australian cities, the spatial dynamics of settlement, and change over time associated with these shifts in socio-economic profile, have remained unexplored. This paper draws on three census periods, and employs a local indicator of spatial autocorrelation together with a geographically weighted regression. We conclude that the settlement patterns of MC-born migrants in Australia do not follow the 'straight-line' assumptions of spatial assimilation theory, nor are they tied explicitly to socio-economic factors in the majority of capital cities. Instead, the settlement patterns of MC-born migrants vary by city: Sydney and Melbourne display the most spatially segmented settlement patterns; while Adelaide and Perth exhibit increasing levels of concentration as the number of MC-born migrants grows. Results indicate that while MC-born migrants are somewhat likely to cluster at the early stage of group formation, within-group spatial patterns are articulated by social attributes as the group grows in size and significance in a metropolitan context. Children and young people in rural Australia experience disadvantage compared with metropolitan counterparts, with low educational attainment and disengagement from schooling being linked to poorer health outcomes. Schools are an existing contact point between individuals and health services. However, these health services are often overburdened and have limited scope to address broader social issues and teaching staff are focused on achieving curriculum outcomes. Embedding social workers within schools may provide an avenue for supporting students, yet social workers are rarely present in New South Wales public schools. This paper describes a study to determine teachers' understanding of the socioeconomic issues faced by school students, and the role of the social work profession in addressing such issues as part of the early stages of establishing a Social Work in Schools (SWiS) project. Eighteen semistructured interviews were undertaken with teachers across four rural Australian schools. Participants identified their awareness of potential risk factors that could lead to educational disengagement, reflected on their own limitations in dealing with these broader health presentations given their teaching focus, and provided insights into their understanding of the potential role of social workers. Rural and remote school children in Australia are at risk of reduced educational attainment and disengagement due to low socioeconomic status (SES), geographical location, parental education and employment, and family dysfunction. Social workers should advocate for the role they can play within rural school communities to assist in ameliorating the impact of socioeconomic factors on a student's capacity to engage in learning. Workplace health promotion (WHP) is advocated to progress the health and well-being of employees. However, research findings on its uptake and impacts are equivocal, particularly in smaller workplaces. This paper describes managers' and workers' responses to a WHP programme in the Australian Capital Territory. Informed by a cultural economy framework, in-depth interviews were conducted with 44 workers and managers from 10 small to medium sized enterprises (SMEs). Examining their availability and acceptability to workers and managers, we found a limited array of health promotion activities were adopted; a caring environment, provision of healthy foods, occasional health checks and health advice. Physical activity programmes during work hours were unlikely to be accepted by managers due to time costs, and workers were reluctant to spend their non-paid time on them. Casual workers were often excluded from WHP activities because their work times did not synchronize with other employees' hours. This study illuminates how WHP is shaped by a complex of employment regulations that stress individual performance, associated limits on employer and worker time and resources, and organizational, cultural norms and practices regarding healthy work environments. We conclude that SMEs are implementing a limited array of behaviour change initiatives reflecting a particular view of health promotion. While organizational change may expand adoption of health practices during the workday, there are impediments to workers adopting wholesale changes in their health practices given a national culture of long hours, and intense job demands embedded in Australia's neoliberal employment system. Australia and its Indigenous peoples have experienced what one commentator has called “a juxtaposition of first and third worlds”, manifest, among other aspects, in lower levels of education achievement compared with other Australians. Specifically, profound levels of educational inequality have been observed for Aboriginal Australians because of the colonial experience, and this has resulted in on-going challenges in terms of their collective histories. This paper examines the interrelationship between Indigeneity and socioeconomic status (SES) in the primary school years, and analyzes the association between a school’s location, levels of segregation, school SES and literacy and numeracy achievement. I argue that relatively high levels of school segregation and clustering structure and maintain Indigenous children’s inequality in schooling outcomes in socioeconomically disadvantaged communities. Characteristics of low SES neighbourhoods and schools are examined, using 2011 Population Census and National Assessment Program - Literacy and Numeracy (NAPLAN) data to assess the effect on reading and numeracy mean achievement levels of Year 5 students in schools across several states. Analyzes of the data demonstrate relationships between school socioeconomic composition and school segregation, and have important implications for Indigenous people in a majority of low SES communities. Children have been made a priority in Australian government obesity interventions, with programmes tending to focus on promoting behaviour change in food and exercise practices. This paper reports findings from ethnographic research with a group of Australian children aged 10–14 years living in a low socio-economic suburban setting. We propose that central to these children’s experiences of food and eating is the negotiation and management of hunger. Historically, disadvantage has been embodied and inscribed on children’s bodies in very visible ways, and the coexistence of hunger and obesity is part of this continuum. Despite considerable evidence of food insecurity in our research site, issues of hunger were absent in an obesity intervention underway at the time. We examine how hunger was felt in children’s lives, how it was managed and the ways in which messages about eating less, while eating more ‘healthy foods’, were at odds with the children’s experiences of hunger. Moreover, we argue that hunger and obesity in this context were doubly stigmatised, as children were very aware of how their hunger and their bodies marked them as different and disadvantaged. Attention to both the embodied and social impacts of hunger on children and their everyday relationships is key in formulating more sensitive, contextualised public health interventions. Despite ongoing equity initiatives, there is still a clear discrepancy in regards to access to higher education for potential students from socially disadvantaged backgrounds. This paper reports on an action research initiative that has developed a model of engaged outreach as an alternative approach to traditional university outreach. Engaged outreach uses the principles of higher education community engagement to develop stronger relationships between universities and their local communities for the purposes of increasing aspiration and access to higher education. The project was designed using a reflective, collaborative process with local Pacific Island immigrant communities living in an area of high social deprivation in southeast Queensland, Australia. Research progressed in three key stages, which together form the basis of the proposed model of engaged outreach. While it is acknowledged that the success of engaged outreach will depend on its implementation as a long-term strategy, preliminary results from this pilot project suggest that it demonstrates real potential to address this important but seemingly entrenched issue in Australian higher education. This paper explores the notion of sustainable regions by employing a case study of the Upper Hunter, Australia, a region that is a node in material and discursive networks for coal mining, wine making, and thoroughbred breeding. It defines sustainability as environmental sustainability, which is a basis for socio-economic development. New national and international economic and social forces have reshaped national geographies in general and the characteristics of cities in particular, resulting in a range of diverse social and spatial outcomes. These outcomes, which include greater differentiation across, within and between cities has become a feature of the economic and social forces associated with post-Fordist social structures. Taking localities across Australia's metropolitan regions, this paper develops a typology of advantage and disadvantage using a model-based approach with clustering of data represented by a parameterised Gaussian mixture model and confidence intervals of the means providing a measure of differences between the clusters. The year 2014 marked the abolition of the Baby Bonus and its replacement with a substantially reduced parenting payment. While often criticised as "middle-class welfare," or publicly denounced due to its purported misuse by disadvantaged mothers, this paper argues that the Baby Bonus provided valuable financial assistance to families experiencing high financial stress. To investigate young women's experience of receiving the Baby Bonus, 19 semistructured interviews were conducted with young mothers in Melbourne who had recently had babies. Many were experiencing financial stress and this payment provided much needed financial support for the basic costs arising from having a baby. Several mothers perceived work as a luxury that was out of reach due to high childcare costs relative to their earning capacity and therefore saw Paid Parental Leave as a further privilege unavailable to them. Our results suggest that while the concerns of policymakers to achieve the best use of scarce resources are critical, it should also be acknowledged that the policy change may have serious implications for many young mothers and may exacerbate disadvantage, and young mothers' sense of alienation, ultimately leading to greater inequalities. Objective:This paper presents a summary of otoacoustic emission (OAE) data collected as part of an Australian hearing health survey (Carter, 2011; Williams et al, 2014) designed to: (1) examine the relationship between audiological indicators and participant characteristics, and (2) extract audiological data suitable for reference use.Design:Cross-sectional cohort study. Distortion product (DP) OAE and transient evoked (TE) OAE measures.Study sample:Age range 11–35 years;N =1386 participants (2672 test ears).Results:Descriptive statistics for amplitude and signal-to-noise ratio (SNR) were calculated for 327 participants (589 test ears; age 13 to 32 years). DPOAE amplitudes down to the 25th percentile were > 0 dB SPL for test frequencies up to 6 kHz. TEOAE SNRs down to the 25th percentile were > 6 dB SPL up to 4 kHz.Summary:This dataset can be used as a clinical reference for similar populations, providing that the same test parameters are used.Conclusions:The clinical significance of OAE testing would be greater if agreed criteria were available. These data could be pooled with other datasets to build a substantial OAE database, similar to the existing international standards for pure-tone hearing threshold levels (HTLs) (ISO, 2000). Background: Alcohol disproportionately affects socially disadvantaged groups including Aboriginal and Torres Strait Islander Australians. Methods to assess alcohol intake for disadvantaged communities need to be able to capture variable or episodic drinking. The ability of a seven-day diary to capture typical consumption for a predominantly Aboriginal sample has not been assessed. Objective: One aim of this paper was to examine agreement between a seven-day retrospective diary and 'usual' drinking assessed by a modified version of the Alcohol Use Disorders Identification Test question 3 (AUDIT-3m; two questions). Other aims were to describe drinking patterns as reported in the seven-day diary. Method: In 2012, consecutive adults attending an Aboriginal Community Controlled Health Service completed a cross-sectional health risk survey on a touch screen laptop ( n = 188). Alcohol consumption questions included the retrospective diary and AUDIT-3m. Agreement was assessed using weighted kappa analysis. Results: There was good agreement between the two measures of consumption; however, the AUDIT-3m questions identified more current drinkers. Respondents who were drinkers (54%) tended to consume large amounts per drinking occasion: almost half (46%) of diary completers reported nine or more standard drinks on at least one occasion in the last week. Conclusions: The seven-day diary did not adequately capture variability in alcohol consumption common among this sample. Although the AUDIT-3m appeared acceptable, alternative approaches to assess usual or risky alcohol consumption, such as asking about specific drinking occasions, or allowing participants to respond in non-standard drink sizes, also need to be considered for indigenous and other disadvantaged communities. This article is principally concerned with case management and rurality in the Australian context. While there is substantial literature exploring case management across disciplines and across contexts, there remains a substantial gap in knowledge in relation to the role played by rurality in case management practice in Australia. An emerging body of knowledge associated with human service delivery in rural Australia under the stewardship of authors such as Alston, Cheers, and Lonne, offers valuable links between rurality and human services and draws attention to the specificity of rural work. However, there remains a gap in knowledge about how rural specificity impacts on case management practice in the political, cultural, and socioecological context that makes up rural Australia. Thus, the aim of this paper is to reflect upon contemporary knowledge of rural case management in the light of current literature and emerging trends, and to provoke interest in this topic as an area for further discussion and research. Patients with mental health conditions commonly present in General Practice. Mental health curricula are broad. We do not know that trainees are exposed to the learning they require. This study aimed to establish the prevalence, characteristics and associations of GP trainees' management of mental health problems. This paper presents a cross-sectional analysis of the Registrar Clinical Encounters in Training (ReCEnT) study, an ongoing multisite cohort study of Australian GP trainees (registrars) documenting their clinical experiences over 60 consecutive consultations. Univariate and multivariable logistic regression analyses were conducted with outcome of the problem/diagnosis being a mental health condition. 1659 trainees provided data on 218,325 consultations and 340,453 problems/diagnoses. Mental health conditions were associated with patients being male, of Aboriginal or Torres Strait Islander or English-speaking background. Trainee characteristics were being more senior and having trained in Australia. Practice characteristics included being in low socioeconomic areas. Trainees sought less help for mental health concerns than they did for other problems. While early-career GPs see a broad range of mental health conditions, they may benefit from training to manage patients from cross-cultural contexts. This paper provides a thematic review of the literature on cancer in Aboriginal people in Australia, focusing on experiences in diagnosis, treatment and care as well as addressing sociocultural factors to guide the public health response to poorer treatment outcomes. A search of both medical and social scientific databases for journal articles published between 1995 and 2006 show that cancer incidence and possibly survival and mortality data are likely to be underestimated in Aboriginal people. Aboriginal people are more likely to die from cancer than non-Aboriginal Australians. There are significant differences between the cancer experiences of those living in the city and in rural or remote areas. There is also a relative absence of literature on cancer in Aboriginal men, who are likely to have particular needs during diagnosis, treatment and care. In drawing conclusions from these data, it can be seen that Aboriginal people with cancer have poorer outcomes than non-Aboriginal Australians, and there is a need for further research in the patterns of care and predictors of outcomes in Aboriginal men and women with cancer. Particular attention should be given to the different needs and experiences of Aboriginal people in urban or rural/remote areas. These findings indicate an urgent need to allocate additional resources to the treatment and care of Aboriginal people with cancer, in addition to screening interventions. There is also a continuing need to acknowledge cultural differences in the health beliefs of Aboriginal people and to work in partnership with Aboriginal community controlled health organisations. This paper explores contrasting dimensions of the politics of recognition for second-generation immigrants by tracing the shift from adolescence to adulthood experienced by a group of young men of Arabic-speaking background. These young men were first interviewed in the 1990s when, as teenagers, they belonged to a group of friends that styled themselves as a "gang" as a way of negotiating the complex dynamics of friendship, ethnicity and masculinity and the desire for a sense of social power via respect gained through fear. Almost a decade later, these men still defined themselves in terms of questions of ethnicity, but voiced a different set of concerns. They had become "respectable" young adults - educated and employed. They sought respect through different strategies, which required that their reputation be convertible across different domains. These two moments in their life histories represent something of the contrasting logics of social inclusion and exclusion around the central themes of respect and respectability; the kinds of social recognition their embodied and cultural capital can confer. In developed economies, such as Australia, schooling is heavily impacted by neo-liberal and neo-conservative agendas. Policies suggest a homogeneity in students that fails to reflect regional contexts of inequality. For the new Australian Curriculum, which includes Health and Physical Education (AC: HPE), this logic prioritises consistency in content and standards for students no matter location or socio-economic circumstances. Little is known about the ‘lived’ realities of such aspirations as they relate to teaching students from disadvantaged regions. This paper reports on practitioner inquiry into a redesigned dance unit, as part of a broader investigation into the implementation of AC: HPE with disadvantaged students. We draw on literature around student engagement and Nancy Fraser’s theorisation of justice to explore the pedagogical redesign. We conclude in arguing that enhanced learning outcomes for disadvantaged students are dependent upon rich and contextualised pedagogical practices. [ABSTRACT FROM AUTHOR]

Purpose: The Australian English Communicative Development Inventory (OZI) is a 558-item parent report tool for assessing language development at 12–30 months. Here, we introduce the short form (OZI-SF), a 100-item, picture-supported, online instrument with substantially lower time and literacy demands. Method: In tool development (Study 1), 95 items were drawn from the OZI to match its item distribution by age of acquisition and semantic categories. Five items were added from four other semantic categories, plus 12 gestures and six games/routines. Simulations computed OZI-SF scores from existing long-form OZI norm data, and OZI and projected OZI-SF scores were correlated. In an independent norming sample (Study 2), parents (n = 230) completed the OZI-SF for their children aged 12–30 months. Child scores were analysed by age and sex. Result: OZI-SF and OZI scores correlate highly across age and language development levels. Vocabulary scores (receptive, expressive) correlate with age and the median for girls is higher until 24 months. By 24 months, 50% of the sample combine words "often". The median time to OZI-SF completion was 12 minutes. Conclusion: Fitted percentiles permit working guidelines for typical (median) performance and lower cut-offs for children who may be behind on age-based expectations and/or at risk for a communication difficulty. Social workers provide services in many cancer settings however there has been limited research into Australian oncology social work practice. This multisite study investigated the reasons those affected by a cancer diagnosis were referred to social workers and the types of intervention undertaken. Clinical data-mining methodology was used to complete a retrospective review of 250 patient medical records. Qualitative and descriptive analyses were undertaken with comparisons to existing classification systems. Findings of significance were that residential location, socio-economic status, gender, relationship status, responsibilities for dependent children, and other complex individual circumstances were key reasons for referral to social workers. Interventions included psychosocial assessment, financial management, counselling, service coordination, and advocacy across multiple systems. Complex circumstances reflecting systemic social inequalities led to higher levels of social worker engagement. The study outcomes indicate that social workers have a significant role in the provision of effective psychosocial cancer care. Poorer cancer outcomes are linked to socio-economic, cultural, and systemic disadvantage, all of which are domains of social work practice. Working across multiple systems, social workers provide a range of skilled interventions to patients, families, carers, and others in complex social situations exacerbated by a diagnosis of cancer. Improving availability and access to social workers will address many of the social circumstances that have a detrimental impact on those affected by cancer. [ABSTRACT FROM AUTHOR]

Lesbian women and gay men are at greater risk of post-traumatic stress disorder (PTSD) than heterosexual people, however few studies have examined PTSD in older lesbian women and gay men. This study examined predictors of having ever been diagnosed with PTSD, as well as relationships to current quality of life, among 756 lesbian women and gay men aged 60 years and older in Australia. Participants were surveyed on their sociodemographic characteristics, experiences of sexual orientation discrimination over their lifetime, whether they had ever been diagnosed with PTSD, whether they were currently receiving treatment for PTSD, and their current quality of life. After adjusting for sociodemographic variables, participants who reported having a PTSD diagnosis (11.2%) had significantly more frequent experiences of discrimination over their lifetime and were significantly less likely to currently be in a relationship. Older lesbian women were significantly more likely than older gay men to report ever having had a PTSD diagnosis. Additionally, having ever been diagnosed with PTSD significantly predicted current poorer quality of life. These findings suggest that a history of PTSD among older lesbian women and gay men is linked to experiences of discrimination and other factors, with associated links to current quality of life. The aim of the present study was to characterize sedentary time (ST) and sedentary bouts among young children, across a typical week and by day type (childcare days, non-childcare days and weekends), over a 12-month period. ST % (ST/accelerometer wear time*100) and number of sedentary bouts/hours were assessed using accelerometers at two time-points 12-months apart in 153 toddlers and 74 pre-schoolers. Differences between baseline and follow-up according to the whole week and the different day types were calculated using linear mixed models with adjustment for age, sex, socio-economic status and zBMI. During the weekend, toddlers had more 20–30 min/h sedentary bouts at follow-up than at baseline (MD: 0.03). In pre-schoolers, ST % (MD:-8.4), the number of 1–4 min/h (MD:-1.45), 5–9 min/h (MD: −0.46) and 10–19 min/h sedentary bouts (MD:-0.11) were lower at baseline, compared to follow-up, on non-childcare days; as were the number of 5–9 min/h sedentary bouts on weekends (MD:0.40). P < 0.05 for all. This study found that longer sedentary bouts (20–30 min/h) tended to decrease among toddlers and pre-schoolers, whereas ST and shorter sedentary bouts increased over 12-months, particularly among pre-schoolers on non-childcare weekdays. Non-childcare days, and home environment may be targeted for intervention to reduce ST and sedentary bouts among pre-schoolers. [ABSTRACT FROM AUTHOR]

We undertook a qualitative study that explored exercise participation and exercise promotion in the multiple sclerosis (MS) community who live in regional or remote areas of Australia. By simultaneously gathering views from persons with MS, carers, healthcare providers and healthcare managers we aimed to gather unique perspectives which represented views from across socio-ecological levels of MS healthcare. We used interpretive description methodology, and conducted semi-structured interviews or focus groups with people with MS (n = 28), carers (n = 8), healthcare providers (n = 12) and managers/supervisors of MS healthcare systems (n = 16). Data were analysed using thematic analysis. We identified three themes with 10 subthemes. The first theme was "Factors associated with exercise engagement" for the people with MS, from individual, interpersonal, organisational and community/public policy perspectives. The second theme was "Factors influencing the MS community's promotion of exercise" focusing on carers, healthcare providers and healthcare systems. The third theme was "Motivators to increase exercise promotion" which should be delivered by the MS community across varying socio-ecological levels of healthcare to encourage exercise participation. We identified new evidence on the factors which influence the MS community's promotion of exercise and we now better understand that training on exercise should be provided to the wider MS community, and exercise services should be considered locally and perhaps delivered via teleheath. Cohesive healthcare campaigns, and clinical guidelines based on empirical evidence should be established for symptom management in MS with a focus on the role of exercise. Symptom management strategies should consider the whole MS community, including patients, carers, healthcare professional and healthcare co-ordinators. Internal factors (e.g., emotion and motivation) and broader factors (e.g., funding and location) must be considered when designing exercise interventions in persons with MS. [ABSTRACT FROM AUTHOR]

Objective: This study utilises Kleinman's theory of explanatory models of health and illness to explore the experience of chronic hepatitis B (CHB) among Vietnamese people living in Australia. It examines how these explanatory models are formed and shaped by the broader community, and the extent to which this influences understandings and responses to CHB. Design: This study is based on semi-structured interviews with 22 Vietnamese people with CHB in Melbourne, Australia. The individual interviews ranged from 30 minutes to 1.5 hours in length, and were electronically recorded, translated where necessary and transcribed verbatim. Transcripts were thematically coded using NVivo 10, with coding themes guided by categories identified in Kleinman's explanatory models framework. Results: Fundamental to most participants' narratives was the profound impact of cultural, social and economic environments on their understandings and responses to CHB. Regardless of socio-demographic background, most participants juxtaposed biomedical elements of CHB with their own existing humoral-based health belief system. In the context of a chronic asymptomatic condition that, for the most part, does not require pharmaceutical treatment, a humoral-based health belief system provided a familiar conceptual framework from which participants could immediately respond and take control of their infection. This was observed through changes in diet and lifestyle, and the use of traditional herbal medicine in an attempt to 'cure' or halt the progression of their infection. Conclusions: By speaking to people living with CHB directly, it became clear that there is a disjuncture between what is commonly assumed by the biomedical model of CHB and what is understood by individuals with the infection. The public health burden of CHB will continue unless the healthcare system, including public health policies, deliver a hepatitis B model of care that is responsive to the needs and expectations of priority populations. Objectives: Though several studies have focused on risk factors associated with excess weight gain, little is known about the extent to which socio-cultural beliefs about body sizes may contribute to risk of excess weight gain, especially in non-Western migrant communities. Drawing on socio-cultural and attribution theories, this study mainly explored socio-cultural beliefs about an ideal body size among Australian residents who were born in sub-Saharan Africa (SSA). Implications of body size beliefs for risk of excess weight gain after immigration have also been discussed. Design: Employing a qualitative design, 24 in-depth interviews were conducted with Australian residents who were born in SSA. Thematic content analysis was undertaken to ensure that participants' experiences and views were clearly captured. Results: According to the participants, a moderately large body size is idealised in the SSA community and post-migration weight gain is commonly regarded as evidence of well-being. While desirability of a moderately large body size was noted by some participants, others were concerned about health risks (e.g. high blood pressure) associated with excess weight gain. Moreover, body size ideals seemed to be different for men and women in the SSA community and these ideals were mainly promoted by family and friends. Participants reported that women with very slim (skinny) body sizes are often regarded as persons suffering from health problems, whereas those with 'plumpy' body types are often considered beautiful. Participants also noted that men are expected to look well-built and muscular while those with big bellies are often seen as financially rich. Conclusions: Participants' interpretation of post-migration weight gain as evidence of well-being calls for urgent intervention as risk of excess weight gain appear to be high in this immigrant group. Two characteristics of millennial behaviour motivate the present study – an increasing concern for various life issues, and an increasing level of social media usage. In the present study, we focused on millennials' societal concerns, financial concerns and social exclusion concerns and examined how these concerns influence social media behaviour. Adopting the compensatory consumer behaviour framework, we proposed that millennials' social media behaviour can be understood as a form of compensatory consumption, that is, a coping mechanism for dealing with major life concerns. Data collected from a commercial survey of Australian millennials supported most of our hypotheses. We observed that social exclusion concerns directly influenced social media behaviour, whereas societal and financial concerns influence social media behaviour indirectly through influencing social exclusion concerns. Our proposed model explained more than half of the variance in social exclusion concerns, and more than one-third of the variance in social media behaviour. [ABSTRACT FROM AUTHOR]

There have always been independent private psychology practitioners in Australia, yet in the past payment of their services was largely by a user-pays model. The introduction of Medicare Benefits for patients, under the Enhanced Primary Care program in 1999, and Better Access in Mental Health Care in 2006, along with Government-funded mental health initiatives such as Better Outcomes in Mental Health Care introduced in 2001, has provided an alternative funding model for independent private psychological services. Introduction of these and other Government-funded programs has raised questions about the responsiveness of the psychology workforce to meet the changing demands for psychological services created by these reforms. This study aimed to profile the characteristics of 3,587 independent private psychologists who provide services to clients under these schemes by analysing their responses to the Australian Psychology Workforce Survey. Of the 44% of psychologists completing the survey who indicated that they had a Medicare Provider Number, only 61% were in private practice as their main job. The remainder conducted services for Medicare-funded clients as part of a private practice in a second job. Purpose: This study aimed to understand the knowledge, attitudes and beliefs of early childhood health and education professionals on bilingualism and heritage language maintenance for children with language delay, and how their interactions with caregivers may influence the decision-making process to maintain or not maintain speaking heritage languages with children. Method: A qualitative methodology was used to explore the research aims. Seven early childhood health and education professionals with experience working with bilingual families participated in in-depth interviews. Data were analysed using a grounded theory approach and a symbolic interactionism theoretical framework. Result: Understandings of bilingualism and language development were strongly influenced by socially and culturally contextualised factors, such as the linguistic context in Australia. Barriers and facilitators to supporting bilingualism and helping caregivers make informed choices included: unique child and family characteristics, the level of trust in professional advice, and the level of professional agency to support knowledge and practice. Conclusion: Findings highlighted the need to focus on the processes involved in supporting caregivers to make informed decisions about bilingualism and heritage language maintenance for children with language delay. This study explores the experience of leaders and teachers in two Australian primary schools who were seeking more effective ways to respond to students with challenging behaviours and improve their classroom climates. The schools implemented a form of critical reflection as a strategy to understand their experiences, beliefs and practices within the social and cultural context of a school. School leaders and teachers adopted the Reflective Circle Education Model, a group process of reflective circles to explore their reactions to specific school experiences in order to generate new practices that might bring classroom change. After six reflective circles, staff were interviewed and they reported increased mutual support and validation from peers, and a deeper understanding of the values and assumptions that influenced their actions and reactions. This study describes physical activity levels and factors associated with physical activity at the end of stroke rehabilitation. Primary stroke survivors were assessed at completion of physical rehabilitation. Outcomes included physical activity (e.g. step count, moderate-vigorous physical activity duration) and sedentary time measured with the Sensewear Armband, gait speed, and cognition. The number of participants meeting physical activity recommendations was calculated. Differences in physical activity were examined between household ambulators (gait speed <0.4 m/s), limited community ambulators (0.4–0.8 m/s), and unlimited community ambulators (>0.8 m/s). The influence of age, cognition, and gait speed on physical activity was determined by multiple regression. Seventy-nine stroke survivors participated. Twenty-one participants achieved 30 min/day of moderate-vigorous physical activity accumulated in 10 min bouts. Unlimited community ambulators took more steps/day (median 4975 vs. 469 limited, 355 household, p < 0.001), had higher moderate-vigorous physical activity (median 74 min/day vs. 22 limited, 31 household, p < 0.001) and lower sedentary time (mean 1105 vs. 1239 limited, 1232 household minutes/day, p < 0.001). Age, gait speed, and cognition predicted 21.3% of the variance in moderate-vigorous physical activity (p = 0.001); adding employment status to the model predicted 57.3% of the variance in step count (p < 0.001). Physical activity is low following stroke and should be a target for treatment, particularly in those with gait speeds ≤0.8 m/s. Seventy-three percent of stroke survivors performed ≥30 minutes of moderate to vigorous physical activity throughout the day. Twenty-seven percent of stroke survivors accumulated ≥30 minutes of moderate to vigorous physical activity in 10 minute bouts. Despite relatively good physical ability, daily step count was low in this sample. Those with gait speeds ≤0.8 m/s had lower physical activity levels and higher sedentary time. This study examined patterns of rearrest in a sample of Australian Aboriginal and/or Torres Strait Islander and non-Indigenous youth who have sexually harmed, and the association between these patterns and personal and environmental characteristics present at the first (i.e., onset) sexual offence. Participants included 111 (n = 67 non-Indigenous; n = 44 Aboriginal and/or Torres Strait Islander) youth who were reviewed at five fixed-time intervals over a 10-year period. Bivariate analyses (i.e., Chi-square and t-tests) and Analysis of Variance indicated that generally, a higher proportion of Aboriginal and/or Torres Strait Islander youth were rearrested for violent and non-sexual/non-violent offences. No differences were found for sexual offences. Overall, those who were rearrested for sexual/violent offences scored higher on personal behavioural controls at onset; whereas, non-sexual/non-violent rearrests were associated with environmental vulnerabilities (i.e., adverse social conditions, population stressors). Ensuring sufficient and adequately maintained housing in Indigenous Australian communities remains an ongoing policy challenge for government, with major implications for the health of Indigenous Australians. This study sought to characterise the current status of housing conditions experienced by Indigenous Australians, with special reference to the Northern Territory. The assessment examined a range of indicators relating to crowding, dwelling condition, 'health hardware', and provision of maintenance and repairs. While acknowledging data deficiencies and inconsistencies, the analysis produced mixed results. There was evidence of a reduction in crowding but little observable improvement in the provision of maintenance and repairs. Some housing-related health outcomes have shown improvement, though these have tended to coincide with mass treatment campaigns. Achieving the goal of healthy homes – and ultimately closing the gap on Indigenous disadvantage – requires further investment in new houses that are appropriately designed and constructed, alongside an increased emphasis on cyclical maintenance. Physical activity (PA) tends to decline during late childhood and adolescence. In Australia, this decline has been shown to particularly occur non-organized PA (e.g., active play, informal sport). This study aimed to explore predictors of non-organized PA at 13y; and decline between 11y and 13y. Data were sourced from the Longitudinal Study of Australian Children. Non-organized PA was measured using time-use diaries. Analyses included Cragg hurdle and binary logistic regressions. The likelihood of non-organized PA at 13y was predicted by PA enjoyment (OR = 1.36, p = 0.007), number of siblings (OR = 1.11, p < 0.001), sex (females) (OR = 0.66, p < 0.001) and home computer use at 11y (OR = 0.98, p = 0.002) (n = 3,193). Females also engaged in less non-organized PA at 13y (β = −77.77, p = 0.048) (n = 3,193). Significant predictors of decline in non-organized PA were socioeconomic position (OR = 0.92, p = 0.047) and sugary drink consumption at 11y (OR = 1.06, p = 0.033) (n = 3,222). Strategies to promote non-organized PA may include targeting females and those of lower socioeconomic position, and focusing on types of PA that may replace the perceived benefits of computer use. Emerging adulthood is a neglected phase of the life course in health research. Health problems and risk behaviors at this time of life can have long-term consequences for health. The 2016 Lancet Commission on Adolescent Health and Wellbeing reported that the influence of socioeconomic factors was under-researched among adolescents and young adults. Moreover, the influence of socioeconomic factors on health has been little researched specifically in emerging adult men. We aimed to investigate associations between socioeconomic disadvantage and mental health, suicidal behavior, and substance use in young adult Australian men. Logistic regression was used to examine the association between Year 12 (high school) completion and area disadvantage on mental health, suicidal behavior, and substance use in 2,281 young men age 18–25 participating in the Australian Longitudinal Study on Male Health (Ten to Men). In unadjusted analysis both Year 12 non-completion and area disadvantage were associated with multiple adverse outcomes. In adjusted analysis Year 12 non-completion, but not area disadvantage, was associated with poorer mental health, increased odds of suicidal behavior, and substance use. Retaining young men in high school and developing health-promotion strategies targeted at those who do exit education early could both improve young men's mental health and reduce suicidal behavior and substance use in emerging adulthood. Background: The COVID-19 pandemic shows variable dynamics in WHO Regions, with lowest disease burden in the Western-Pacific Region. While China has been able to rapidly eliminate transmission of SARS-CoV-2, Germany – as well as most of Europe and the Americas – is struggling with high numbers of cases and deaths. Objective: We analyse COVID-19 epidemiology and control strategies in China and in Germany, two countries which have chosen profoundly different approaches to deal with the epidemic. Methods: In this narrative review, we searched the literature from 1 December 2019, to 4 December 2020. Results: China and several neighbours (e.g. Australia, Japan, South Korea, New Zealand, Thailand) have achieved COVID-19 elimination or sustained low case numbers. This can be attributed to: (1) experience with previous coronavirus outbreaks; (2) classification of SARS-CoV-2 in the highest risk category and consequent early employment of aggressive control measures; (3) mandatory isolation of cases and contacts in institutions; (4) broad employment of modern contact tracking technology; (5) travel restrictions to prevent SARS-CoV-2 re-importation; (6) cohesive communities with varying levels of social control.
