We used a retrospective cohort study to measure the impact of caring for children on female Australian researchers. Our aim was to see whether caring for children was associated with reduced outputs and collaboration. Women were randomly selected for inclusion if they published a first author paper in one of three Australian journals during 2007 to 2015, women who did not publish during this time were not included. One-hundred and sixty women were approached and 95 (59%) completed a survey on their history of caring for children. Two key outcomes were the women's publication and citation counts, which were accessed from Scopus. We also examined the number of authors, affiliations and countries on their published papers, as a reduction in these numbers could indicate an impaired ability to collaborate. We examined the probability of being first or last author as a measure of esteem. There was a small increase in publication counts after the first child that was reversed after the second child. Average citations counts declined after children, particularly after the second child. There was some evidence of a reduced collaboration with overseas collaborators after the first child. The probability of being the last author increased after the second child. Three women were identified as statistically influential and all three had children and were in the top 10% of overall publications and citations. After removing these women the estimated changes in outcomes were noticeably different for most of the outcomes. The repeated presence of statistically influential women shows that it may be impossible to find an "average impact" of caring for children when considering research output. Background: Access to ongoing multidisciplinary healthcare services for children who have sustained a burn injury is critical for optimal recovery. This paper reports on barriers and facilitators to culturally safe and appropriate burn aftercare for Australia's First Nations children. The voices of First Nations families whose child had sustained a serious burn are central to this paper.Methods: Eighteen families, which consisted of 59 family members, of children younger than 16 years who had sustained a burn injury were asked to describe their own journey in accessing appropriate burn aftercare. Interviews were conducted in the families' homes using yarning (dialogue) and Dadirri (deep listening) as Indigenous research methods. Data was gathered in South Australia, the Northern Territory, Queensland and New South Wales, Australia. Using a cyclic process, transcripts and emerging themes were sent back to participants, and a collaborative approach was used to conduct the final analysis.Results: Lack of culturally safe communication between service providers and family members, in addition to institutionalised racism, were found to be the major barriers to accessing healthcare services. Distance to medical treatment also impacted children's access to aftercare. Involvement of First Nations Health Workers and/or Liaison Officers working with health providers, the child and family members, was found to be an important facilitator in reducing miscommunication and alleviating fear and anxiety in the children and families.Conclusion: There are significant barriers to access to aftercare following a serious burn including miscommunication, lack of cultural safety, distance to medical treatment and racism. However, these can be largely mitigated when First Nations families have input into the care received and the care needed for ongoing burn care to be effective and are supported by First Nations Health/Liaison Officers support. Violence in relationships is a common experience for a significant number of women. VicHealth (Australia) has noted that one of the underlying and contributing factors towards violence against women is their environment, citing 'faith-based institutions' such as churches as one such environment for many women. Indeed, international research shows that the language of religion is often used by women to explain abuse. Additionally, abused Christian women are more likely to remain in or return to unsafe relationships, citing religious beliefs to support avoidance of 'family break-ups' despite abuse. In contrast, however, churches can address domestic violence within a context of care, with emphasis on a theology of biblical equality. This paper examines how domestic violence may be supported by Christian language and belief, and suggests an 'alternate theology' concerning religious language in concepts of gender roles, sacrifice, submission, and suffering. It reviews current research on the connection between Christian religious language and domestic violence against women, to highlight the Christian church's role as a contributing factor to such abuse. Finally, the paper makes some suggestions on how religious language can, in contrast to perpetuating abuse through norms, sever the connections between domestic violence and religious language. Background: Middle ear disease (otitis media) is endemic among Aboriginal and Torres Strait Islander children in Australia and represents an important cause of hearing loss. The disease is the result of a mix of biological, environmental and host risk factors that interact in complex, non-linear ways along a dynamic continuum. As such, it is generally recognised that a holistic, systems approach is required to reverse the high rates of otitis media in Aboriginal and Torres Strait Islander children. The objective of this paper is to examine the alignment between efforts designed to address otitis media in Aboriginal and Torres Strait Islander children in Queensland, Australia and core concepts of systems thinking. This paper’s overall purpose is to identify which combination of activities, and at which level, hold the potential to facilitate systems changes to better support ear health among Aboriginal and Torres Strait Islander children. Methods: We began with a review of documents identified in consultation with stakeholders and an online search. In addition, key informants were invited to participate in an online survey and a face-to-face or phone interview. Qualitative interviews using a semi-structured interview guide were used to explore survey responses in more depth. We also undertook interviews at the community level to elicit a diverse range of views. Ideas, statements or activities reported in the documents and interviews as being performed under the Intervention Level Framework were identified using qualitative thematic and content analysis. A quantitative descriptive analysis was also undertaken, whereby data was extracted into an Excel spreadsheet and coded under the relevant strategic directions and performance indicators of the Framework. Subsequently, we coded activities against the five-level intervention framework developed by Malhi and colleagues, that is: 1) paradigm; 2) goals; 3) system structure; 4) feedback and delays; and 5) structural elements. Results: Overall, twenty documents were reviewed. We examined surveys and interviews with six key informants. Twenty-four individual and 3 group interviews were conducted across central and community level informants. One hundred and four items were coded from the 20 documents and 156 items from interview data. For both data sets, the majority of activities were coded at the structural elements level. The results suggested three key areas where further work is needed to drive sustained improvements: 1) build the governance structures needed for paradigm shift to achieve a multi-sectoral approach; 2) develop shared system level goals; 3) develop system-wide feedback processes. Conclusions: Sustained progress in improving ear health within Aboriginal and Torres Strait Islander children requires a holistic, system-wide approach. To advance such work, governance structures for multi-sectoral collaboration including the development of joint goals and monitoring and feedback are required. Introduction: There is a global trend towards place-based initiatives (PBIs) to break the cycle of disadvantage and promote positive child development. Co-location is a common element of these initiatives and is intended to deliver more coordinated services for families of young children. This paper examines how co-locating early childhood services (ECS) from health and education in Child and Family Centres (CFCs) has impacted collaboration between services. Methods: This ethnographic study included 130 participant observation sessions in ECS between April 2017 and December 2018 and semi-structured interviews with 45 early childhood service providers and 39 parents/carers with pre-school aged children. Results: Service providers based in CFCs reported that co-location of services was facilitating local cooperation and collaboration between services. However, insufficient information sharing between services, prioritising client contact over collaborative practice and limited shared professional development remained barriers to collaborative practice. For parents, co-location improved access to services, but they experienced services independently of each other. Discussion and Conclusion: Co-location of ECS in CFCs contributed to greater cooperation and collaboration between services. However, for the potential of CFCs to be fully realised there remains a need for governance that better integrates service policies, systems and processes that explicitly support collaborative practice. In terms of public health, childhood vaccination programs have benefits that far outweigh risks. However, some parents decide not to vaccinate their children. This paper explores the ways in which such parents talked about the perceived risks and benefits incurred by vaccinating (or not vaccinating) their children. Between 2013–2016 we undertook 29 in-depth interviews with non-vaccinating and/or ‘vaccine hesitant’ parents in Australia. Interviews were conducted in an open and non-judgmental manner, akin to empathic neutrality. Interviews focused on parents talking about the factors that shaped their decisions not to (or partially) vaccinate their children. All interviews were transcribed and analysed using both inductive and deductive processes. The main themes focus on parental perceptions of: 1. their capacity to reason; 2. their rejection of Western medical epistemology; and 3. their participation in labour intensive parenting practices (which we term salutogenic parenting). Parents engaged in an ongoing search for information about how best to parent their children (capacity to reason), which for many led to questioning/distrust of traditional scientific knowledge (rejection of Western medical epistemology). Salutogenic parenting spontaneously arose in interviews, whereby parents practised health promoting activities which they saw as boosting the natural immunity of their children and protecting them from illness (reducing or negating the perceived need for vaccinations). Salutogenic parenting practices included breastfeeding, eating organic and/or home-grown food, cooking from scratch to reduce preservative consumption and reducing exposure to toxins. We interpret our data as a ‘logic of care’, which is seen by parents as internally consistent, logically inter-related and inter-dependent. Whilst not necessarily sharing the parents’ reasoning, we argue that an understanding of their attitudes towards health and well-being is imperative for any efforts to engage with their vaccine refusal at a policy level. Background: The Australian Longitudinal Study on Male Health (Ten to Men) was established in 2011 to build the evidence base on male health to inform policy and program development.Methods: Ten to Men is a national longitudinal study with a stratified multi-stage cluster random sample design and oversampling in rural and regional areas. Household recruitment was conducted from October 2013 to July 2014. Males who were aged 10 to 55 years residing in private dwellings were eligible to participate. Data were collected via self-completion paper questionnaires (participants aged 15 to 55) and by computer-assisted personal interview (boys aged 10 to 14). Household and proxy health data for boys were collected from a parent via a self-completion paper-based questionnaire. Questions covered socio-demographics, health status, mental health and wellbeing, health behaviours, social determinants, and health knowledge and service use.Results: A cohort of 15,988 males aged between 10 and 55 years was recruited representing a response fraction of 35 %.Conclusion: Ten to Men is a unique resource for investigating male health and wellbeing. Background: Migrant families' transnational ties (i.e., connections to their countries of origin) may contribute to their hardships and/or may be a source of resiliency. A care approach that addresses these transnational ties may foster a positive identity and give coherence to experiences. We conducted an integrative review to determine what is known about transnational ties and the care of migrant families during pregnancy, postpartum and early childhood.Methods: We searched 15 databases to identify literature reporting on a health or social program, service, or care experience of migrant families during pregnancy up to age five in a Western country (i.e., Canada, US, Australia, New Zealand or a European country). Information regarding if and how the service/program/care considered transnational ties, and care-providers' perceptions of transnational ties, was extracted, analyzed and synthesized according to transnational 'ways of belonging' and 'ways of being'.Results: Over 34,000 records were screened; 69 articles were included. Care, programs and services examined included prenatal interventions (a mhealth app, courses, videos, and specialized antenatal care), doula support, maternity care, support groups, primary healthcare and psycho-social early intervention and early childhood programs. The results show that transnational ties in terms of 'ways of belonging' (cultural, religious and linguistic identity) are acknowledged and addressed in care, although important gaps remain. Regarding 'ways of being', including emotional, social, and economic ties with children and other family members, receipt of advice and support from family, and use of health services abroad, there is very little evidence that these are acknowledged and addressed by care-providers. Perceptions of 'ways of belonging' appear to be mixed, with some care-providers being open to and willing to adapt care to accommodate religious, cultural and linguistic differences, while others are not. How care-providers perceive the social, emotional and economic ties and/or the use of services back home, remains relatively unknown.Conclusion: Significant knowledge gaps remain regarding care-providers' perceptions of transnational 'ways of being' and whether and how they take them into account, which may affect their relationships with migrant families and/or the effectiveness of their interventions. The decision to remain in or leave the parental home represents the first housing career choice of young people. In this paper, we examine the parental home leaving outcomes for Australian birth cohorts in the twentieth century using recall questions contained in the Household, Income and Labour Dynamics in Australia Survey. The findings from survival analysis and hazard function modelling is that a continuous, gradual reduction in the age of first leaving the parental home in Australia is evident up to the 1947-51 birth cohort, followed by stability and then a rise in age of first leaving home for the most recent birth cohorts. Birth cohort effects remain significant even after other measured determinants of parental home leaving are taken into account. Background: Fathers can be a critical source of breastfeeding support for their partner, but little is known about what fathers would like to learn about breastfeeding. Partner's support and encouragement enhances mother's breastfeeding confidence and boost the capacity to address breastfeeding difficulties effectively. The aims of this study were to explore what fathers regard as important to learn around breastfeeding, and their current and preferred sources of information. Methods: A structured online survey was conducted, between September 2022 and November 2022, with fathers containing three sections: (1) sociodemographic variables; (2) perceived importance of 26 breastfeeding topics; and (3) sources of breastfeeding information. A convenience sample of expectant and current fathers aged 18 years or older, who were expecting a baby or had a child aged one year or younger, living in Australia, and able to complete survey in English was recruited. Participants were recruited on Facebook advertisement. Results: A total of 174 fathers participated in the study, majority (75%) were aged 30–39 years, current dads (74%), and university educated (69%). The breastfeeding topics that fathers perceived as the most important/ important to learn about were how to work with their partner to overcome breastfeeding challenges, how fathers can be involved with their breastfed baby, the types of support fathers can provide to breastfeeding mothers, what to expect in the first week and the benefits of breastfeeding. The most preferred health professional sources of breastfeeding information were midwives, child and family nurses and doctors. Among non-health professional sources of support, mobile app, friends and family were most popular. Conclusion: Breastfeeding information to enhance fathers' knowledge and awareness of common breastfeeding challenges, and fathers' role in supporting their breastfeeding partner, appear to be (most) important for fathers. Background: Home visiting programs are implemented in high income countries to improve outcomes for families with young children. Significant resources are invested in such programs and high quality evaluations are important. In the context of research trials, implementation quality is often poorly reported and, when reported, is variable. This paper presents the quality of implementation of the right@home program, a sustained nurse home visiting intervention trialled in Australia, and delivered in a ‘real world’ context through usual child and family health services. right@home is structured around the core Maternal Early Childhood Sustained Home-visiting (MECSH) program, which is a salutogenic, child focused prevention model. Method: At each visit right@home practitioners completed a checklist detailing the client unique identifier, date of contact and activities undertaken. These checklists were collated to provide data on intervention dose, retention to program completion at child age 2 years, and visit content, which were compared with the program schedule. Quality of family-provider relationship was measured using the Session Rating Scale. Exploratory factor analysis was conducted to identify clusters of activities and allow qualitative assessment of concordance between program aims and program delivery. Results: Of 363 intervention families offered the program, 352 (97·0%) commenced the program and 304 (87·3%) completed the program to child age 2 years. 253 of 352 (71·9%) families who commenced the program received more than 75 percent of scheduled visits including at least one antenatal visit. Families rated the participant-practitioner relationship highly (mean 39.4/40). The factor analysis identified six antenatal and six postnatal components which were concordant with the program aims. Conclusions: The right@home program was delivered with higher adherence to program dose, schedule and content, and retention than usually reported in other home visiting research. Program compliance may have resulted from program design (visit schedule, dose, content and delivery flexibility) that was consistent with family aims. [ABSTRACT FROM AUTHOR]

Mental health can affect young people’s sense of wellbeing and life satisfaction, their ability to participate in employment and education, and their onward opportunities in life. This paper offers a rare opportunity to longitudinally examine mental health in a population-representative study of children aged 4–5 years to 14–15 years. Using data from the Longitudinal Study of Australian Children (LSAC), this study examined maternally-reported child mental health over a 10 year period, in order to understand their initial mental health status early in life and its change over time, as measured by the Strengths and Difficulties Questionnaire. Longitudinal models were fitted from ages 4–5 to 14–15 years. Results showed that child sex, maternal mental health, socio-economic status (family income, maternal education, neighbourhood disadvantage), maternal hostility, and child temperament (persistence, sociability, reactivity) are all independent contributors to child mental health at age 4. These effects largely persist over time, with the effects of maternal mental health increasing slightly over time. Persistence of these effects suggests the need for early intervention and supports. The independent contribution of these factors to child mental health suggests that multi-faceted approaches to child and maternal mental health are needed. [ABSTRACT FROM AUTHOR]

There has been growing interest in exploring factors that influence the success or otherwise of welfare policies in democratic countries, such as the interrelationships between the proposed policy and the context it will be introduced into, such as the sociodemographic characteristics of the population and the population's previous experience with welfare policies. However, there has been little exploration of factors that could influence general population support for long-term care for older people. The aim of this study was to investigate the general population's attitudes for determinants of high-quality aged care and different mechanisms for funding and any impact of individual characteristics on these. A representative sample of the Australian general population aged 18 years and over (N = 10,315, 52% female, 22% aged 65 years and over) drawn via quota sampling participated in the survey online. Participants were asked to rate the importance of a list of 10 determinants of quality care and their support for four models of funding, both using a five-point Likert scale. We identified consistently high expectations for long-term care services across the general population, especially among older people, females, those with a family member in care, and those living in rural or regional areas. In terms of how governments practically fund a high-quality long-term care system, we also identified broad support among the general population for both payment of a co-contribution towards the cost of care by older people using services and increased government funding for the system. Over 40% of participants said they would be willing to pay additional tax to improve access and quality of aged care services. While often neglected by governments in the past who assumed voter apathy on the topic, by comparison, our findings indicate that there is currently a strong appetite among the general population for improvements to the quality of care provided and that they are willing to consider changes to the funding model. Drawing on in-depth interviews conducted during 2002-03 for the Australian Family Formation Decisions (AFFD) Project, this paper probes the relationship formation experiences of 115 women, men and couples of family-forming age living in eastern Australia. Contemporary relationship formation is characterized by a mixture of ambivalence and resignation to having limited control over the process on the one hand ('if it happens it happens'), and urgency on the other, especially among women seeking to fulfil maternal ambitions in their thirties after prioritizing other things earlier in their adult lives. For most the process of partnering involves trial and error, with timing -- finding someone whose expectations of a relationship match one's own -- posing a major challenge. It gives rise to a common phenomenon, the 'too soon syndrome', where relationships with many positive attributes are abandoned because one party perceives the other as too keen to 'settle down', and/or himself or herself as not ready to do so. The paper also examines impediments to partnering, including traditional ones like shyness; negative trial-and-error experiences; the demands of study and career establishment; pursuit of agendas emphasizing travel and enjoyment; sole parenthood; and parental marriage breakdown. A framework is provided by Beck's (1992) concept of reflexive modernization, and his associated proposition that life has become highly individualized with an emphasis on creating 'do-it-yourself' biographies. Risk exposures and predictions of child development outcomes typically estimate the independent effects of individual exposures. As a rule though, children are not exposed piecemeal to individual or single risks but, rather, they are exposed to clusters of risk. Many of these clusters of risks are better thought of as comprising a developmental “circumstance” with a substantial duration, over which period, additional risk exposures also accumulate. In this paper we examined the distribution of 16 single risk exposures for low language ability using latent class analysis across a sample of approximately 4000 children from the Longitudinal Study of Australian Children. The best fitting model identified six distinct classes. 46% of children were in a Developmentally Enabled group, 20% were in a group typified as Working Poor families, 10% of children were in group typified as Overwhelmed group, 9% of children were in a group defined by Child Developmental Delay, 8% of children were in a group defined by Low Human Capital, and 7% of children were in a group defined by Resource Poor non-English Speaking background families. These groups had quantitatively and qualitatively distinct patterns of risk factors and showed different onward trajectories of receptive vocabulary. Our results demonstrate a range of multiple risk profiles in a population-representative sample of Australian children and highlight the mix of risk factors faced by children. Youth alcohol consumption has been steadily declining in Australia, as in other countries; fewer young people are drinking and the age of initiation is increasing. However, young people, their parents and others in their communities continue to believe that adolescent (excessive) drinking is the norm. This perception, and the concurrent misperception that the majority of parents are happy to provide their underage children with alcohol, creates a perceived culture of acceptance of youth alcohol consumption. Young people believe that it is accepted, and even expected, that they will drink; and parents perceive that not providing their adolescent children with alcohol will lead to social exclusion. There is evidence that shifting social norms can have an immediate and lasting effect adolescents’ (and adults’) alcohol related attitudes and behaviors. This paper reports on a novel, community based social marketing intervention designed to correct misperceptions of alcohol related social norms in an Australian community. The project utilized a social marketing approach, informed by the full complement of Andreasen’s social marketing benchmarking criteria, and concurrently targeted adolescents, parents of adolescents and the broader community. Using extensive formative research and multiple evaluation techniques, the study demonstrates that shifts in community social norms are possible and suggests that this approach could be used more widely to support the positive trends in youth alcohol consumption and parental supply. BACKGROUND Over the last few decades, Australian families have undergone profound changes, including fewer marriages, more divorces, and an increase in double-income families, resulting in a qualitative shift in understanding the family today. OBJECTIVE This paper investigates whether generational differences in family values and religious beliefs are at the core of changes to the family structure. METHODS Using the Household, Income and Labour Dynamics in Australia (HILDA) survey, we apply the Age-Period-Cohort Detrended (APCD) methodology to investigate generational differences in family values and religious beliefs. RESULTS Results show that changes in family values and religious beliefs are overwhelmingly generational. Cohorts born between 1946 and 1964 (commonly referred to as baby boomers) have significantly contributed to the revolutionary shift in family behaviours and attitudes. CONCLUSIONS The baby boomer generation has played a crucial role in supporting progressive views on marriage, children, gender roles, and religious beliefs. When compared to older and younger cohorts, baby boomers saw the largest shift in family behaviours and attitudes, having matured in a period of rapid economic prosperity and significant social change. The unique events that occurred during their formative years may have influenced these behaviours and attitudes, ultimately contributing to the qualitative shift in the understanding of family. This paper examines the impact of the demographic transition upon the potential supply of, and demand for, family support for the aged in Australia. Using census and survey information on population cohorts entering old age, comparisons are drawn concerning their surviving issue, household composition and family membership. Long-term changes in fertility are shown to have had only a small impact upon the supply of potential carers among relatives and, although the demographic transition has led to a more universal inclusion of old people in family networks, there have not been major changes through time in the proportions living in extended family households. Short-term changes, however, such as low fertility during the 1930s, have caused disordered cohort flow, with the result that current generations of the elderly are members of deprived cohorts in terms of their access to family support. Residents living in residential aged care facilities (RACFs) continue to experience medication-related harm. There is ongoing interest in expanding the role of pharmacists, including on-site pharmacists (OSPs), to help improve medication management in RACFs. The objectives of this mixed-methods study were to explore the extent and ways in which on-site pharmacists (OSPs) were normalised within RACFs as part of a complex intervention seeking to improve medication management. This study consisted of semistructured interviews informed by normalisation process theory (NPT) and a quantitative survey adapted from the normalisation measure development questionnaire (NoMAD) instrument which is underpinned by NPT. Semistructured interviews with prescribers, RACF managers, RACF nursing staff, OSPs, residents, and family members (n = 47) indicated that most participants supported OSPs within RACFs that having OSPs in RACFs made sense and was perceived as beneficial and that participants were invested in working with OSPs who often became part of routine practice, i.e., "normalised." Prescribers, RACF managers, and nursing staff (health care team members) completed the adapted survey, and their responses (n = 16) strongly complemented the positive qualitative findings. Overall, OSPs were positively appraised by health care team members as well as residents and family members and were generally considered to be normalised within their respective RACFs. This study explored the normalisation of OSPs within RACFs. From the perspective of residents, family members, health care team members, and OSPs, OSPs could become part of routine practice within Australian RACFs. The findings of this study also highlighted the value of using theory to guide the evaluation of a pharmacist intervention in RACFs and the utility of applying NPT in a new setting, Australian RACFs. Importantly, the findings of this study could help inform the future role of OSPs working and the rollout of OSPs within Australian RACFs. [ABSTRACT FROM AUTHOR]

Most studies of children's development and parents' wellbeing have not dealt effectively with the complexity of multiple disadvantage. Traditional approaches have typically used a limited set of outcomes and predictors. Even studies utilizing multiple risk factors have often treated these as confounders, adjusting for their influence, while concentrating on a primary association of interest. Such strategies do not illuminate the real world essence of disadvantage, i.e. that adversities co-occur more than expected by chance and that multiple disadvantage is common. The main aim of the present paper is to address this neglected topic and develop summary measures of adversity using the 2004-2005 data from Wave 1 of the Longitudinal Study of Australian Children. Information was obtained from families of 5,107 babies (0-1 years) and families of 4,983 children (4-5 years). The prevalence of multiple disadvantage among families with young children and the degree to which summary adversity measures are associated with each other and with family and child outcomes is then estimated. Using factor analysis, 12 lower-order constructs and two higher-order components of adversity were developed, labelled (1) material and (2) psychosocial adversity. Findings show that the two component scores were more strongly associated with outcomes than were the more specific construct scores and that psychosocial adversity was somewhat more relevant to family wellbeing and child development than material adversity. Background: The family food environment is an important influence in the development of children's dietary habits. Research suggests that influences of current dietary behaviour and behaviour change may differ. The aims of this paper were to: (1) investigate the association between the food environment at baseline and change in children's saturated fat intake; and (2) to e×plore whether a change in the food environment was associated with a change in children's saturated fat intake. Method: Secondary analysis of a 12 week cluster randomised controlled trial in 133 4-13 year old children. Families were randomly allocated to parental education regarding changing to reduced-fat dairy foods or a comparison non-dietary behaviour. The interventions were family focused. Parents received education from a dietitian in 3×30minute sessions to facilitate behaviour change. Parents completed a comprehensive questionnaire capturing three domains of the food environment - Parent knowledge and attitudes; shaping practices; and behaviours and role modelling. Children's dietary intake was assessed via multiple 24-hour recalls at baseline and week 12. Changes in the family food environment and primary outcome (saturated fat) were calculated. Hierarchical linear regression models were performed to e×plore the association between baseline and change in food environment constructs and change in saturated fat intake. Standardised Beta are presented (p<0.05). Results: After adjustments for child and family demographics, higher levels of perceived food availability (β=-0.2) at baseline was associated with greater reduction in saturated fat intake, where as higher perceived responsibility (β=0.2), restriction (β=0.3) and pressure to eat (β=0.3) were associated with lesser change in saturated fat. An increase in nutrition knowledge (β=-0.2), perceived responsibility (β=-0.3) and restriction (β=-0.3) from baseline to week 12 were associated with greater reduction in saturated fat intake. Conclusions: The present study was one of the first to quantify changes in the family food environment, and identify a number of factors which were associated with a positive dietary change. Background: Improving the wellbeing of Indigenous populations is an international priority. Robust research conducted with Aboriginal and Torres Strait Islander peoples is key to developing programs and policies to improve health and wellbeing. This paper aims to quantify the extent of participation in a national longitudinal study of Aboriginal and Torres Strait Islander (Indigenous Australian) children, and to understand the reasons why caregivers participate in the study. Methods: This mixed methods study uses data from Wave 6 of Footprints in Time, the Longitudinal Study of Indigenous Children. We conducted descriptive analysis of quantitative variables to characterise the sample and retention rates. We applied conventional content analysis to 160 caregivers’ open-ended responses to the question, ‘Why do you stay in the study?’, identifying themes and overarching meta-themes. Results: The study has maintained a high retention rate, with 70.4% (n = 1239/1671) of the baseline sample participating in the study’s 6th wave. We identified seven themes related to why participants stay in the study: telling our story, community benefit, satisfaction, tracking Study Child’s progress, study processes, receiving study gifts, and valuing what the study stands for. These related to two meta-themes: reciprocity, and trust and connection. Caregivers reported that participation was associated with benefits for their family and community as well as for the study. They identified specific features of the Footprints in Time study design that built and maintained trust and connection between participants and the study. Conclusions: Our findings support the assertion that Aboriginal and Torres Strait Islander people want to be involved in research when it is done ‘the right way’. Footprints in Time has successfully recruited and retained the current-largest cohort of Aboriginal and Torres Strait Islander children in Australia through the use of participatory research methodologies, suggesting effective study implementation and processes. Participants indicated ongoing commitment to the study resulting from perceptions of reciprocity and development of trust in the study. Footprints in Time can serve as a successful model of Aboriginal and Torres Strait Islander health research, to promote good research practice and provides lessons for research with other Indigenous populations. With the increasing diversity of family situations, growing numbers of people, including children, have more than one home. In France, nearly 4% of inhabitants are likely to be counted twice in surveys; while in Australia, the proportion is less than 2%. Taking into account the possibility that a single individual could have multiple residences is necessary not only to avoid double counting, but also to accurately describe the family situations of adults and children. In this paper, we first estimate the proportion of people living in two dwellings and then describe the consequences of these two-home situations for basic estimates of family situations. Our analysis is based on two large-scale surveys: the 2004 EU-SILC in France and the 2001 HILDA in Australia. A striking result is that, in France, half of all children who are counted as living with their fathers are in fact dividing their time between the homes of separated parents. [ABSTRACT FROM AUTHOR]

Background: The way that health care systems in developing countries like India care for dying patients, has an impact on the expectations of such care for those who migrate to other countries faces. At the end of life, cultural issues may impact on the quality of life remaining and for that reason, it is important that particular cultural practices are understood. This paper describes a study that investigated the cultural issues of access to palliative care services for Indian migrants in Australia. Purpose of the Study: To investigate the experiences of the family members of terminally ill Indian migrants in Victoria, Australia. Objective of the Study: To explore the issues related to accessing palliative care services for Indian migrants; to identify the effectiveness of palliative care in supporting the patient and family and to recommend strategies for improving this care. Materials and Methods: A qualitative descriptive design was utilized. Up to 6 family members were selected for in-depth interviews in understanding cultural issues related to the palliative care services for a family member. Results: Analysis of the interviews revealed that families of Indian patients experience difficulties whilst receiving palliative care services, which fell into three main categories: Indian support systems, cultural issues, and caring experiences. Although each of these issues had a direct influence on the experience of terminal care that their family member received, cultural issues and support systems also influenced the caring experiences. Conclusion: Despite the successful implementation of palliative care services across Australia, there are still problems in accessing and receiving the services among minority and disadvantaged groups like various cultural groups. It was not until 1972 that governments in Australia began significantly funding childcare services. Since then, expenditure has steadily increased, but debate about the nature and extent of public involvement in childcare has not diminished. For example, Brennan (2002) has questioned the shift away from community-based providers to private providers and the quality of care given. Others have looked at the increase in childcare costs, the expansion of tax rebates for one-earner families, and the system of direct payments to institutional care providers. This paper, which is a policy forum, aims to address these issues.

Increasing inequality in Australian cities has created the need for a deeper understanding of the interaction between spatial segmentation and economic outcomes. This paper offers a preliminary economic analysis of Australian neighbourhood externalities in the context of increasing segmentation. Theoretically, neighbourhoods can affect youths' economic out- comes through their effects on school quality, information flows, job networks, and demonstration effects. We utilise a new survey of 171 year 12 students in ten Melbourne high schools to examine the impact of individual, family, and neighbourhood characteristics on the decision to attend post-secondary education. It is shown that there exist neighbourhood externalities that affect human capital investment decisions. Finally, we offer some tentative implications for public policy. [ABSTRACT FROM AUTHOR]

This article focuses on the Australian Institute on Family Studies' Housing Across the Life Course project. This paper set out to illustrate how the key concepts of a risk society might become manifest in contemporary Australian housing careers. This led to a consideration of whether entry to home purchase shows signs of increased differentiation and disconnectedness. Analysis reveals no evidence of increased differentiation; on the contrary, the age-related norm of entry to home ownership appears to be strengthening. That is, entry to home ownership by later cohorts is increasingly likely to have been attained within a narrower span of years than earlier cohorts. The data suggest that if individuals have not entered home ownership by their early thirties, they are increasingly unlikely to do so. With regard to disconnectedness, analysis found that entry to home ownership is becoming increasingly disconnected from other life course events. The paper has provided brief illustrations of the manifestation of risk society concepts and what they might mean for our understanding of family life.

This paper examines the availability and dynamics of family supports across a range of Australian families to determine whether distinctive patterns exist for families from non-English-speaking backgrounds. The first part of the paper provides an overview of cultural diversity in Australia, the general directions of immigration and family policies, and a summary of current research in family exchanges. The second part includes an analysis of data from the Australian Institute of Family Studies Australian Life Course Study on exchanges within a range of culturally diverse Australian families, followed by consideration of the extent to which families from non-English-speaking backgrounds reveal distinctive patterns of exchanges. Developments in two government policy areas--immigration and settlement policies, and family policies--are relevant to family exchanges of immigrant families. A policy trend in immigration and settlement services during the 1990s has been in the direction of reduced government expenditure and a more economically focused migration program. This has led to tighter management of the migration pro- gram, the introduction of cost recovery for migration services, and the elimination of services to some categories of immigrants.

Background: Family and domestic violence, encompassing diverse behaviours including physical, sexual, emotional and financial abuse, is endemic worldwide and has multiple adverse health and social consequences. Principal drivers include traditional gender values that disempower women. Changing these is a key prevention strategy. In Australia, high-quality national surveys provide data on public perspectives concerning family and domestic violence but may not capture community-level diversity. As part of a project for primary prevention family and domestic violence in outer regional Australia, our aims were to develop and administer a questionnaire-based survey suitable for the local community encompassing knowledge about, attitudes towards, and personal experiences of family and domestic violence, to describe and to investigate the theoretical (factor) structure and local socio-demographic predictors of responses, and to determine the extent to which the survey findings are locally distinctive. Methods: The online community survey for local residents (≥15 years), comprised items on respondents' sociodemographic characteristics plus questions abridged from pre-existing national instruments on knowledge about, attitudes towards, and personal experiences of family and domestic violence. Responses were rake-weighted to correct census-ascertained sample imbalance and investigated using exploratory factor analysis, with sociodemographic predictors determined using multiple linear regression and dominance analysis. Results: Among 914 respondents, males (27.0%), those from age-group extremes, and less-educated persons were underrepresented. Familiarity with diverse family and domestic violence behaviours was high among all subgroups. Poorer knowledge of the FDV behaviour continuum and attitudes supporting traditional gender roles and FDV were disproportionately evident among males, older respondents and those with lower education levels. Both the factor structure of extracted composite measures reflecting community perspectives and sociodemographic predictors of responses generally aligned with patterns evident in national data. Conclusions: Local reinforcement of existing nationwide findings on community understanding of and attitudes towards family and domestic violence provides salience for targeted interventions. In recent years, social network analysis has had its own distinctive theoretical and methodological underpinnings. This article proposes a novel method for the application of a fused mixed-methods approach and social network analyses that incorporates the critical aspects of numerous characteristics of children's social development in a statutory care setting. We collected data from children involved in the Mockingbird Family out-of-home care model, following its implementation in Australia. Our approach involves three steps: social-developmental network indicators, relational dynamics, and social-developmental network narrative. We contend that this approach has the potential to provide powerful data representation that facilitates the understanding of the complexities of children's social development, the links between different positions and roles of children, and their social network in the Mockingbird family. Using exemplars, we show the potential of the research method to unearth rich data for seeking to understand the system change important for strengthening children's safe and protective environments. Background: Consumption of ultra-processed foods is associated with increased risk of obesity and non-communicable diseases. Little is known about current patterns of ultra-processed foods intake in Australia. The aim of this study was to examine the amount and type of ultra-processed foods purchased by Australian households in 2019 and determine whether purchases differed by socio-economic status (SES). We also assessed whether purchases of ultra-processed foods changed between 2015 and 2019. Methods: We used grocery purchase data from a nationally representative consumer panel in Australia to assess packaged and unpackaged grocery purchases that were brought home between 2015 to 2019. Ultra-processed foods were identified according to the NOVA system, which classifies foods according to the nature, extent and purpose of industrial food processing. Purchases of ultra-processed foods were calculated per capita, using two outcomes: grams/day and percent of total energy. The top food categories contributing to purchases of ultra-processed foods in 2019 were identified, and differences in ultra-processed food purchases by SES (Index of Relative Social Advantage and Disadvantage) were assessed using survey-weighted linear regression. Changes in purchases of ultra-processed foods between 2015 to 2019 were examined overall and by SES using mixed linear models. Results: In 2019, the mean ± SD total grocery purchases made by Australian households was 881.1 ± 511.9 g/d per capita. Of this, 424.2 ± 319.0 g/d per capita was attributable to purchases of ultra-processed foods, which represented 56.4% of total energy purchased. The largest food categories contributing to total energy purchased included mass-produced, packaged breads (8.2% of total energy purchased), chocolate and sweets (5.7%), biscuits and crackers (5.7%) and ice-cream and edible ices (4.3%). In 2019, purchases of ultra-processed foods were significantly higher for the lowest SES households compared to all other SES quintiles (P < 0.001). There were no major changes in purchases of ultra-processed foods overall or by SES over the five-year period. Conclusions: Between 2015 and 2019, ultra-processed foods have consistently made up the majority of groceries purchased by Australians, particularly for the lowest SES households. In Australia, the decision to home educate is becoming increasingly popular (cf. Harding & Farrell, 2003; Townsend, 2012). In spite of its increasing popularity, the reasons home education is chosen by Australian families is under-researched (cf. Jackson & Allan, 2010). This paper reports on a case study that set out to explore the links between families that unschool and the parenting philosophies they follow. In-depth, qualitative interviews were conducted with a group of home education families in one of Australia's most populated cities. Data were analysed using Critical Discourse Analysis. The analysis revealed that there were links between the parents' beliefs about home education and their adherence to Attachment Parenting. [ABSTRACT FROM AUTHOR]

Objective: Improve documentation quality of end-of-life family meetings in a tertiary intensive care unit (ICU). Design: Before-and-after interventional quality improvement project between October 2018 and February 2020 utilising an electronic pro-forma record. Setting: Australian, University affiliated, mixed medical-surgical 22 bed adult ICU. Participants: Patients who were admitted to the ICU for active management and subsequently died during that ICU admission. We enrolled 50 patients who died before and 50 patients after the introduction of the electronic family meeting pro-forma record. Intervention: Through collaboration with ICU medical and nursing staff, End-of-life Special Interest Group and Clinical Documentation Committee we developed the ICU Family Meeting Discussion Note as an electronic pro-forma record with multiple key fields of entry. Main outcome measures: Patient records were examined for the presence of documented details around patient’s admission, family meetings and specific elements surrounding the patient’s death. Results: The introduction of a pro-forma record markedly improved the quality of documentation of end-of-life care related family meetings. Documentation increased in recording hospital admission date/time (6% vs 84%), meeting location (14% vs 70%), the reason patients were absent from the meeting (34% vs 72%), the Medical Treatment Decision Maker (MTDM) (10% vs 44%), the patient’s resuscitation status (22% vs 54%), and treatment options discussed (78% vs 94%) (p⩽0.005 for all). Conclusion: Introducing an electronic pro-forma record to facilitate family meeting documentation increased the frequency of important recorded information. Further studies are required to assess whether documentation quality improvements are sustainable and whether they affect patient- or relative-centred outcomes. [ABSTRACT FROM AUTHOR]

The transition to school is a time of change and expectation for children, families, and communities. It is also a time when a range of factors-both within and outside the family-influence educational experiences and outcomes. This paper reports the experiences and expectations of 24 Australian families as their children with special needs started school. Life for these families was complicated by their experiences of other factors described as complex support needs. Drawing on interview and case study data, we report issues and concerns, and examine the supports available for these families and their children across the transition to school. In analysing the data, we consider the ways in which children's special needs interact with the complex support needs of families, and consider implications of this interaction as families navigate access to support. We conclude that this interplay positions many families in ways that reinforce, rather than reduce, the difficulties encountered. [ABSTRACT FROM AUTHOR]

Over recent decades, Australia's highly progressive, individual-based taxation of families has been replaced by a system that tends towards joint taxation with an inverted U-shaped rate scale. The reform has been implemented by introducing family-income-targeted child payments (now Family Tax Benefit Part A) and by lowering tax rates on higher incomes. The new system has shifted the burden of taxation to two-earner families on low and average wages and, in particular, to working married mothers as second earners. Work life, family life and care are all being substantially re-shaped in Australia as in other post-industrial societies. Concern about work/life balance has increased as domestic gender relations slowly change, the breadwinner/homemaker model becomes less dominant and families undertake more demanding and flexible modes of employment and care. Using data from a qualitative study of nurses and their families this paper explores how families are managing the contemporary intersection of flexible family life and flexible labour. Family values emerged as an important mediating factor in the organisation of work and care. The couples valued parental care and time with children and used shift work to enable family life. We argue that just as the breadwinner model is being modified in Australia so too is the maternalist culture of care. This paper explores associations between social and policy context and how parents of young children allocate time to work and family, using data from the Australian Bureau of Statistics (ABS) Time Use Survey (TUS) 1997 and 2006. Over the period, Australia's economy was growing and unemployment was low. Political rhetoric supported 'traditional' family values, family tax policy favoured single income or 'modified male-breadwinner' households, and part time work was the most common 'family- friendly' workplace measure. Against this background, we investigate the market labour supply and intra-household time use of mothers and fathers in couple-headed households with at least one child aged 0 - 4 years (N=747 in 1997 and 626 in 2006). We identify associations between household labour supply and total (paid and unpaid) work, the way paid work, domestic labour and childcare is divided between mothers and fathers, and their subjective feelings of time pressure, at each time point. We find that by 2006, there was lower average maternal market work and a higher proportion of families with young children conformed to the one-and-a-half earner family form than in 1997. In the main there was increased total household work, increased gender specialisation in paid work and caring labour, and much higher subjective time pressure, especially for fathers and the relatively few mothers who were employed full time. This paper focuses on contemporary public perceptions of the challenges of combining paid work and raising a family, set against the backdrop of concerns about low fertility, structural population ageing and the composition of the future labour force. Australasian policy makers have responded to these issues with various initiatives aimed at assisting people who are raising families and engaging in paid work. The New Zealand Working for Families package, the Australian Family Tax Benefit package, and the two countries' parental leave and women-focused policies are compared, with a focus on cross-national similarities and differences. The most significant difference between the two nations is the lack of a paid parental leave scheme in Australia, but the family-friendly policies are similar in perpetuating tensions between ensuring both present and future labour supply. The policies are aimed at providing incentives to be in paid work when parents have children, but tend to reinforce the notion of work and family as separate spheres, and potentially contribute to social divisions between parents and non-parents by providing cash benefits to families with children. This analysis suggests that Australasian societies may be on the cusp of a more collective articulation of people's obligations to one another and the nation, including the collective consequences of personal lifestyle decisions such as choosing not to have children. Although employment levels in Australia are healthy when compared to those 20 years ago, the distribution of work across households has become more unequal. The present paper measures any polarisation of employment that has taken place in Australian households between 1982 and 2000/01. We find that employment has indeed become polarised across Australian households with most of the polarisation within-household types and not entirely reflecting changes in household size. Particularly worrying is the polarisation found in households with children. This is likely to have consequences for the well-being of future generations. [ABSTRACT FROM AUTHOR]

This article explores claims by Elisabeth Beck-Gemsheim that we are living in an era of post-familial families. The demographic evidence to support these claims is clearly evident in relation to changing patterns of family formation and dissolution, as well as the movement of married women into paid employment. But what is less evident is a decline in traditional patterns of gender stratification within families. This paper uses national data from Australia to examine the relationship between post-familial status, as indicated by marital status and employment and time spent on housework. The results show that gender is still a clear predictor of time spent on housework. However, there is evidence that gender inequality may be declining in non-traditional households.

The article focuses on the Ninth Australian Institute of Family Studies Conference, titled "Families Matter," which was held in Melbourne on 9-11 February 2005. The conference was opened by the Minister for Family and Community Services, Senator the Hon. Kay Patterson. The conference concluded with a panel discussion on the topic "Family policy in the Asia Pacific region." Early childhood development, early intervention strategies and the development of effective children's services continue to be enduring themes of Institute conferences. A number of papers discussed children's well-being in childcare. A number of sessions were dedicated to exploring the interaction between family life and community life. The participation of international delegates provides an important networking opportunity for Australian researchers and policy makers attending the Institute's Conference. Feedback from delegates attending the conference was very positive. During and immediately after the conference, research presented at the conference was reported in more than 800 media items.

Research into Australian families has been given an enormous boost with the recent release of the first wave of data from the longitudinal Household, Income and Labor Dynamics in Australia Survey. In the first survey of its type in Australia, the Household, Income and Labor Dynamics in Australia Survey will yield valuable insights into the dynamics of family relationships, parenting, housing arrangements, income, workforce involvement, and the health, well being and attitudes of young people and adults in Australia. This article examines the potential for the new survey to contribute to research on families in Australia. It summarizes the case for yet another survey; provides brief descriptions of the survey design and sample selection; outlines issues covered by the survey; and describes data collection procedures, wave one response rates, and characteristics of the sample. Background: The COVID-19 pandemic has placed considerable pressure on families, testing the quality of relationships and the strength of social support within and beyond the family network. However, little is known about the pre-pandemic factors that predict family relational resilience and social functioning during times of natural disaster or global crisis. Here we use data from one of Australia's longest running studies of social and emotional development to examine the nature and timing of possible relational and social support intervention aimed at preparing families for future adversities. Methods: Data were from the Australian Temperament Project Generation 3 (ATPG3) Study, a population representative three generation cohort study of families established in 1983. A subset of Generation 2 parents completed a COVID-19 specific survey in May-September 2020 (502 parents of 871 children; 60% mothers; 37–38 years). These participants had completed the Quality of Relationships Inventory to assess social support during young adulthood, at 23–24 years (2006) and 27–28 years (2010), before next generation conception. Participants had also completed the Maternity Social Support Scale 1 year postpartum for each child born across the ATPG3 assessment period (2012–2019). In 2020, during the height of the Australian lockdowns, participants rated the quality of their relationships with their partners, children and broader family and friends, in addition to social support within and extended beyond their family. Results: Pre-pandemic partner support was associated with partner relationship quality during the pandemic (β = 0.22). Pre-pandemic support from friends was associated with relationship quality with other family and friends during the pandemic (β = 0.12 – 0.18). Pre-pandemic support (from partner, family and friends) was consistently associated with social support within families during the pandemic (β = 0.11 – 0.21). Pre-pandemic support from friends was also associated with family support extended to others within their local community during the pandemic (β = 0.12 – 0.13). Conclusions: Strengthening supportive relationships during major life transitions, prior to the start of family life and in early parenthood, may have long-term and intergenerational benefits years into the future for both families and communities. Globally, injuries are the leading cause of death and represent the highest burden of ongoing disease amongst children 1-16 years of age. Increasingly, prevention programmes are recognising a growing need for intervention strategies that target children. The purpose of this study was to determine the efficacy of the SeeMore Safety Programme, designed to teach children (4-6 years of age) how to make conscious decisions about their own capabilities related to safety and how to manage risk. This retrospective study examined de-identified pre- and post-programme data from a sample of 1027 4 to 6-year-old pre-school children over the four-year period who participated in the SeeMore Safety Programme. Results show a significant improvement in each of the post-test scores and when compared to the pre-test scores (p < 0.001). Children from rural areas, as well as those from areas of greater disadvantage, also showed significant improvement in their pre- and post-test scores (p < 0.001).
