The turbulent times governments all over the world are facing include the transition towards the Fourth Industrial Revolution (4IR) in which the confluence of new technological developments result in major challenges for national as well as local governments. The confluence of the newly developed technologies is only possible if huge amounts of data can be stored and transmitted at high speed in, among others, hyperscale data centers (HDCs). This paper focuses on the complex relation that arises between local governments - on whose soil the HDCs are built - and the high tech companies planning and building the HDCs. This relationship is characterized by an unequal playing field with local governments being no match for the high tech industry. The latter sometimes have annual turnovers 1000 times higher than those of local governments and impose rather different demands on decision making processes than governments do. They impose restrictions on the decision-making process that result in processes running counter to principles of good governance and, if exposed, result in a deterioration of institutional trust among the population. This is illustrated through a case study in which a small municipality in the Netherlands was faced with an offer from Meta to make a huge investment in a hyperscale data center, provided the whole process towards the permits and the approval of the building would remain secretive. Shown is an imposed secrecy by the high tech firm and information revealed about the process afterward through the Freedom of Information Act disclosing many irregularities. This changed the existing trust in the municipality into complete distrust at the end of the process and eventually forced the High Tech company to withdraw. The relevance of this paper for this special issue is that the case study shows that the new developments towards the 4IR require capacitated, knowledgeable and decisive local governments, which momentarily are not even present in a developed country like the Netherlands. The degree of persistence of the real gross domestic product per capita, total factor productivity and labour productivity has been examined in a group of 23 developed and developing nations, as well as the overall Euro Area, by evaluating the order of integration of the macroeconomic series over the annual period from 1890 to 2019. As against the conventional use of using integer degrees of differentiation (i.e. 0 for stationary processes and 1 in case of unit roots), fractional values have been utilized. The empirical findings provide evidence for mean reversion in both total factor productivity and the real gross domestic product per capita in Chile, Germany, the Netherlands and New Zealand. The results further suggest that mean reversion only occurs in labour productivity of Australia. The non-linearity analysis shows that non-linearity is also present in the majority of the series. This paper explores an attempt to bring biotechnology more directly within the ambit of civil society in general and resource-poor farmers in particular. The Andhra Pradesh Netherlands Biotechnology Programme (APNLBP) was one of four country programmes initiated by Dutch aid in the early 1990s. It is coming to an end as an aid project next year. The broad objective of the programme was to contribute to poverty alleviation through biotechnologies but to do so in a rather unique way. Instead of having R&D laboratories develop a raft of new technologies and then ‘disseminating’ these to farmers, the emphasis was put on direct interaction with farmers and related stakeholder groups such as non-governmental organizations (NGOs). The paper describes the programme's inception and evolution, outlines key governance aspects and sets the analytical discussion within the context of modern ‘innovation systems’ discourse. Two aspects in particular are emphasized. The first is the degree of connectivity among the different stakeholders that are part of the system. It is now well recognized that it is the flow of information across stakeholder groups that often determines the degree of technological development that occurs, although clearly there are other factors also involved. This article examines PhD students' migration plans and strategies, their development over time, and the circumstances of their potential or real return within the changing life course context. The research is based on a longitudinal qualitative study conducted over six years (2012-17) among 21 international PhD students coming from developing countries, studying in the Netherlands. Results are discussed in the context of a cultural shift and the migration--development nexus. We argue that: (1) strong feelings of responsibility towards both their family and the development of their country of origin are leading factors shaping their migration strategies; (2) specialisation of their research project and its applicability in the home country also importantly influences their return potential; and (3) an apparent moral responsibility towards the home society and a willingness to help countries of migratory origin was growing stronger over the observation time (in harmony with post-materialist values the informants gained when studying in the Netherlands) along the progressing life course, which creates a broad potential for brain gain. Background: The incidence of human papillomavirus (HPV)-associated oropharyngeal cancer (OPC) is increasing in high income countries. HPV-associated OPC generally presents as an invasive disease, often with lymph node involvement, in relatively young patients with minimal or no history of smoking and alcohol consumption. Knowledge on HPV-associated OPC among primary care professionals is essential for disease recognition and early start of treatment. Aim: To examine the knowledge on HPV-associated OPC among GPs in the Netherlands. Design & setting: A cross-sectional postal survey among GPs in the Netherlands. Method: A 12-item questionnaire was sent to 900 randomly selected general practices. Outcome measures included awareness of the link between HPV and OPC, epidemiological trends, and patient characteristics. Data were statistically analysed for sex, years after graduation, and self-rated knowledge of OPC. Results: A total of 207 GPs participated in this study. Seventy-two per cent recognised HPV as a risk factor for OPC and 76.3% were aware of the increasing incidence rate of HPV-associated OPC. In contrast, 35.7% of participants knew that patients with HPV-associated OPC are more often male, and just over half (53.6%) of the participants were aware of the younger age of these patients. Conclusion: More than one-quarter of GPs in the Netherlands are unaware of HPV as a causative factor for OPC. Furthermore, there is a gap in knowledge on characteristics of patients with HPV-associated OPC. Further training on these topics could improve disease recognition and, ultimately, patient survival. Scholarship on the migration-development nexus has focused on the outcomes of remittances received by the inhabitants of countries in the Global South. This paper argues that this conceptualisation of remittances as one-way flows obscures the fact that remittances are part of reciprocal social relations. As such, they also entail flows of goods, money, and especially services from countries in the Global South to migrants, or what is called reverse remittances. The paper contributes to emerging literature on reverse remittances by broadening the conceptualisation of reverse remittances and presenting an analysis of their characteristics as well as those of receivers and providers. We find that most remittances from home communities to migrants are in the form of services rendered. These include childcare and helping with migrants' investments in housing and business. Furthermore, we elaborate on another type of reverse remittance overlooked in the literature: services conducted to help migrants obtain documents to regularise their stays in the host country. The type of reverse remittance received differs for documented and undocumented migrants, and the providers differ according to kin and non-kin relations. Depending on their significance, reverse remittances can be important in determining whether and how migration can lead to a betterment of people's lives in the Global South and are therefore relevant for the migration-development nexus, which has heretofore neglected their existence. The analysis is based on a simultaneous matched sample methodology conducted with 131 migrants in the Netherlands and their network members in Ghana, using both quantitative and qualitative analyses. Background: The "health workforce" crisis has led to an increased interest in health professional education, including MPH programs. Recently, it was questioned whether training of mid- to higher level cadres in public health prepared graduates with competencies to strengthen health systems in low- and middle-income countries. Measuring educational impact has been notoriously difficult; therefore, innovative methods for measuring the outcome and impact of MPH programs were sought. Impact was conceptualized as "impact on workplace" and "impact on society," which entailed studying how these competencies were enacted and to what effect within the context of the graduates' workplaces, as well as on societal health.Methods: This is part of a larger six-country mixed method study; in this paper, the focus is on the qualitative findings of two English language programs, one a distance MPH program offered from South Africa, the other a residential program in the Netherlands. Both offer MPH training to students from a diversity of countries. In-depth interviews were conducted with 10 graduates (per program), working in low- and middle-income health systems, their peers, and their supervisors.Results: Impact on the workplace was reported as considerable by graduates and peers as well as supervisors and included changes in management and leadership: promotion to a leadership position as well as expanded or revitalized management roles were reported by many participants. The development of leadership capacity was highly valued amongst many graduates, and this capacity was cited by a number of supervisors and peers. Wider impact in the workplace took the form of introducing workplace innovations such as setting up an AIDS and addiction research center and research involvement; teaching and training, advocacy, and community engagement were other ways in which graduates' influence reached a wider target grouping. Beyond the workplace, an intersectoral approach, national reach through policy advisory roles to Ministries of Health, policy development, and capacity building, was reported. Work conditions and context influenced conduciveness for innovation and the extent to which graduates were able to have effect. Self-selection of graduates and their role in selecting peers and supervisors may have resulted in some bias, some graduates could not be traced, and social acceptability bias may have influenced findings.Conclusions: There was considerable impact at many levels; graduates were perceived to be able to contribute significantly to their workplaces and often had influence at the national level. Much of the impact described was in line with public health educational aims. The qualitative method study revealed more in-depth understanding of graduates' impact as well as their career pathways. Electric bicycles (e-bikes) represent one of the fastest growing segments of the transport market. Over 31 million e-bikes were sold in 2012. Research has followed this growth and this paper provides a synthesis of the most pertinent themes emerging over the past on the burgeoning topic of e-bikes. The focus istransportrather thanrecreationale-bike research, as well as the most critical research gaps requiring attention. China leads the world in e-bike sales, followed by the Netherlands and Germany. E-bikes can maintain speed with less effort. E-bikes are found to increase bicycle usage. E-bikes have the potential to displace conventional motorised (internal combustion) modes, but there are open questions about their role in displacing traditional bicycles. E-bikes have been shown to provide health benefits and an order of magnitude less carbon dioxide than a car travelling the same distance. Safety issues have emerged as a policy issue in several jurisdictions and e-bike numbers are now approaching levels in which adequate safety data are able to be collected. Research on e-bikes is still in its infancy. As e-bike usage continues to grow, so too will the need for further research, in order to provide the necessary data to inform policy-makers and industry. This paper analyses an innovative form of North–South cooperation– specifically between the Ministry of Development Cooperation of the Netherlands and Bolivia– to strengthen research capacity in the social sciences for development. The most notable innovation is that the donor approached the recipient country with an open agenda, allowing the Bolivian partners total autonomy to plan, implement, and manage their own programme, thus ensuring local ownership of the process in the South. The resulting programme– the Strategic Research Programme of Bolivia (PIEB)– was devised so as to promote research around an agenda built with wide participation of stakeholders. Research grants are allocated on a competitive basis, widely publicised, with clear guidelines and selection criteria. Research proposals are carried out by teams and have a strong component of training and involvement of research users. The achievements of the programme show the importance of the South retaining autonomy and ownership, provided the local leadership creates mechanisms for building trust and social accountability. They also demonstrate the importance of nurturing research excellence by creating quality control mechanisms. All elements taken together, PIEB may be seen as an example of institutional innovation in the Bolivian context. Notwithstanding the success and achievements presented above, building research capacity in the social sciences for development is an intensive, long-term, and continuous process. This paper situates climate resilience as a solution to urban flooding under racial capitalism by examining its implementation across Amsterdam, the Netherlands, and Dhaka, Bangladesh. The necessity for cities to enhance their climate resilience in the face of urban flooding has become a dominant refrain in the global governance of climate change. This is the case in both the Netherlands and Bangladesh, locations that are respectively and divergently framed as global "masters" of water management and needy "apprentices" requiring international guidance on flood control and economic development. I place these two sites in conversation with one another to break down constructed ontological and epistemological divides across the global North and global South, drawing attention to the relationship between contemporary urban climate resilience policies and broader histories of socio-ecological injustice under racial capitalism. I argue that urban climate resilience policies across Amsterdam and Dhaka renegotiate and reconfigure techniques, practices and ideologies of racial and class-based inequality in response to threats and insecurity posed by climate change within and across urban scales. Looking across Amsterdam and Dhaka, I demonstrate that climate resilience policies primarily aim to protect status quo forms of capital accumulation at the urban scale rather than enact comprehensive climate adaptation measures. As a result, the ineffective and inequitable nature of resilience continues to support highly racially uneven urban socio-natures across the global North and global South, enabled by racist imaginaries where communities in the global South – particularly the urban poor – are framed as being undeserving of environmental justice. The development of the environmental services trade is crucial to achieving climate goals and a green economic transition. Based on environmental services trade data from 2001 to 2019, this work uses the social network analysis (SNA) method to depict the structural features of global environmental services trade networks, empirically testing the influencing mechanism of network evolution based on the quadratic assignment procedure (QAP) model. The results indicate that the global environmental services trade is now in a rebound stable period. The market of the environmental services trade is becoming increasingly diversified, the accessibility and convenience of trade are constantly being enhanced, and there is an obvious core–edge structure in the network. Belgium, Italy, and the Netherlands are at the center of the global environmental services trade network; Greece, as the most prominent trade catch-up country, has gradually become an important "bridge" and "hub." Climate change, geographical distance, and population size are the key factors affecting the global trade network of environmental services; environmental regulation, economic distance, and green technology distance have no significant impact on the development of the global environmental services trade, and language difference is no longer an obstacle to the growth of this trade. The results indicate that countries should develop in-depth transnational trade cooperation in the field of environmental services and do so with a more open attitude. Building with Nature (BwN), is a concept that has been applied in various projects around the world in the broader context of nature based solutions for climate resilience and adaptation. Most of these projects are regional or local in nature and are implemented on a smaller scale. Larger scale projects are more rare. The aim of this article is to highlight a number of large‐scale projects that have been completed or are in the developing stage. The focus will be on coastal regions, from temperate locations in the Netherlands and from developing countries in the tropics including Bangladesh. The value of implementing the principle of BwN for a sustainable future is underlined and described. Certain aspects of governance are highlighted to point out the differences and challenges, but also similarities, of carrying out such projects in different parts of the world. The importance acquired by private cars as the leading travel mode in most advanced countries has drawn attention to concerns related to pro-environmental travel behaviour. Indeed, the car has brought great benefits to society, albeit causing a whole lot of environmental and socio-economic consequences. In this perspective, we exploit Eurobarometer data on the attitudes of Europeans towards urban mobility to investigate the main motivations of citizens' public transport use frequency. Ordered logistic regressions are estimated by country (Germany, Italy, and the Netherlands) and by gender. Our results suggest the key role played by a comprehensive set of socio-demographic, economic, and environmental aspects in determining urban travel behaviour. Moreover, our investigation brings to light some relevant cross-country and cross-gender commonalities and differences. The provided evidence may give policymakers a better knowledge of travel behaviour, useful for designing new interventions for environmentally-sustainable travelling. [ABSTRACT FROM AUTHOR]

The COVID-19 pandemic presents Northern-based development organisations with unprecedented difficulties. They are challenged in fundraising opportunities in their home countries and in finding ways to continue their work in the Global South. As the first study to present a systematic mixed method, cross-country study of small-scale, voluntary development organisations in four different European countries, this study provides insight into the role of these private development initiatives (PDIs) in the COVID-19 crisis and sheds light on the differential impact of the crisis on these organisations. Whereas most PDIs are involved in long(er)-term development interventions, the COVID-19 crisis was for most organisations their first experience of emergency aid. Overall, we see strong resilience among PDIs and also find that the organisations which relied more exclusively on traditional methods of fundraising (offline) received a greater funding hit than organisations—often with more younger members—that had already moved to online fundraising. This article introduces the notion of 'accumulated homelessness' to account for the repeated loss and lack of home experienced by many migrants in Europe today. Through the lens of home and homelessness, we argue that the debate on protracted displacement—often applied only to developing countries—should be extended to Europe. Going beyond the idea of shelterlessness, we consider homelessness as a multidimensional and multiscalar condition which encompasses material and emotional aspects. By analysing two life histories of Eritrean men living in Italy and in the Netherlands, we examine the set of structural, social, and individual conditions leading them to lose home in different places and times as well as their attempts—such as finding accommodation, establishing a community organisation, or moving onwards—to reconstruct home. The Australian government has announced major reforms with the move to a primary maternity care model. The direction of the reforms remains contentious; with the Australian Medical Association warning that the introduction of non-medically led services will compromise current high standards in maternity services and threaten the safety of mothers and babies. The purpose of this paper is to conduct a critical review of the literature to determine whether there is convincing evidence to support the safety of non-medically led models of primary maternity care. Twenty-two non-randomised international studies were included representing midwifery-led care, birth centre care and home birth. Comparative outcome measurements included: perinatal mortality; perinatal morbidity; rates of medical intervention in labour; and antenatal and intrapartum referral and transfer rates. Findings support those of the three Cochrane reviews, that there is sufficient international evidence to support the conclusion of no difference in outcomes associated with low risk women in midwifery-led, birth centre and home birth models compared with standard hospital or obstetric care. These findings are limited to services involving qualified midwives working within rigorous exclusion, assessment and referral guidelines, limiting the number of urgent intrapartum transfers that come with increased risk of perinatal mortality. What is known about the topic? Systematic reviews of maternal and perinatal outcomes associated with midwifery-led care when compared to conventional intrapartum hospital care concluded that these non-medically led models of care are associated with several benefits for low risk women and their babies with no identified adverse effects. The finding of no difference in outcomes associated with midwifery-led, birth centre and home birth compared with standard hospital or obstetric care is limited to international studies involving women in the care of qualified midwives working within rigorous guidelines for practice involving inter-professionally agreed exclusion, assessment and referral criteria. Midwives caring for women in non-medically led models are urged to be vigilant to the need for early detection and prompt action in the event of unforseen complications to avoid an over emphasis on normality. It is relatively easy to obtain a survey of the tuberculosis situation in the geographically demarcated countries of Europe. On the other hand it is impossible to acquire reliable and representative information on the majority of developing countries. Also the concept of a developing country cannot be accurately defined: the geographical, ethnic and cultural conditions predominating there may be diametrically different. In addition the paper is intended not only to take into account the present tuberculosis situation, but also to say something on the future development of tuberculosis in Europe and the developing countries during the next few years.

This article discusses incoherence between tax and development policies, a relatively new area in the debate on policy coherence for development, using a case study of the Netherlands. Dutch business entities play a key role in tax avoidance structures of multinational corporations. We argue that the Dutch tax regime facilitates the avoidance of substantial amounts of tax revenues in developing countries when compared to the Dutch aid budget. As domestic tax revenues are an important source of financing for development, this suggests that the Dutch tax policy is incompatible with the Dutch policy on development co-operation. Two decades of awareness-raising and dialogues have clearly spelled out the water-related challenges the world is facing. Still, not enough concrete improvements have been realized on key issues, such as the coverage of potable water and sanitation services, and an adequate balance between water for food and ecosystems. Addressing these problems remains a continuous challenge that can only be confronted with an integrated approach, comprising knowledge generation, permanent communication between very different stakeholders, adequate capacity-building and continuous learning, and innovative technology development. With a view to the Fourth World Water Forum in Mexico, this contribution reviews such issues, drawing lessons from experience in a program on Water for Food and Ecosystems (funded by Netherlands Partners for Water initiative) and related on-going work in this area carried out by Wageningen University and Research Centre working closely with partner institutions inside and outside the Netherlands. The debate is structured around three key dimensions (i) balancing human and natural values of water, (ii) multiple stakeholder involvement, (iii) technological innovation. Visualized in the Triangle of Sustainability, the study recognizes the advances made in integrating political processes such as multi-stakeholder negotiation in water resource management as a way of achieving a better balance between the values of water, but warn against neglecting the potential for technical advances that can help increasing the size of the ‘cake’ as a whole. Background: Stillbirth, the death of a baby before birth, is associated with significant psychological and social consequences that can be mitigated by respectful and supportive bereavement care. The absence of high‐level evidence to support the broad scope of perinatal bereavement practices means that offering a range of options identified as valued by parents has become an important indicator of care quality. This study aimed to describe bereavement care practices offered to parents across different high‐income and middle‐income countries. Methods: An online survey of parents of stillborn babies was conducted between December 2014 and February 2015. Frequencies of nine practices were compared between high‐income and middle‐income countries. Differences in proportions of reported practices and their associated odds ratios were calculated to compare high‐income and middle‐income countries. Results: Over three thousand parents (3041) with a self‐reported stillbirth in the preceding five years from 40 countries responded. Fifteen countries had atleast 40 responses. Significant differences in the prevalence of offering nine bereavement care practices were reported by women in high‐income countries (HICs) compared with women in middle‐income countries (MICs). All nine practices were reported to occur significantly more frequently by women in HICs, including opportunity to see and hold their baby (OR = 4.8, 95% CI 4.0‐5.9). The widespread occurrence of all nine practices was reported only for The Netherlands. Conclusions: Bereavement care after stillbirth varies between countries. Future research should look at why these differences occur, their impact on parents, and whether differences should be addressed, particularly how to support effective communication, decision‐making, and follow‐up care. Recent high-profile scandals raise concerns about how development cooperation is represented. This article examines how the subject gets in the media, examining the tone of voice and framing in newspaper articles and NGO advertisements in the Netherlands. It reveals a remarkable difference between newspaper articles and opinion pieces. Regular reports are characterised by, a neutral to slightly positive tone. In contrast, opinion pieces are predominantly negative. The article identifies possible explanations for the critical tone of opinion pieces. It finds that NGOs' own advertisements may contribute to negative opinion pieces, by problematising the situation in developing countries while rarely demonstrating their impacts achieved. Introduction: The radiochemical purity (RCP) of technetium-99m labelled radiopharmaceuticals (RP) is important to ensure optimal scintigraphic image quality. In low-income settings, it may not be possible to use compendial analytical methods or expensive equipment for radiochemical purity analysis. All radiochemical analysis methods should however be validated against compendial or otherwise proven methods. To ensure the efficacy of RP prepared at Yaoundé General Hospital (YGH) Cameroon, this study cross-validated a cost-effective routine chromatographic method using a simple survey meter technique. A GMP-compliant method used at the University Medical Center Groningen (UMCG), the Netherlands was used as the comparator. Methods: Sestamibi, HMDP and DMSA kits currently used at YGH were reconstituted at UMCG with about 2000 MBq of freshly eluted sodium pertechnetate as described by the manufacturer, and spiked with eluate of the same generator to obtain a range of impurity concentrations. Samples of technetium-99m RP were spotted on 1 × 10 cm iTLC-SG strips and developed in appropriate mobile phases. Each strip was first scanned on the chromatogram-scanner used at the UMCG (standard method), and immediately thereafter the strip was cut in two pieces and radioactivity from each portion was counted with a small survey meter from YGH. The percentage RCP for each TLC strip was calculated using both counting methods. Internationally recommended validation parameters and acceptance criteria were used. Student's paired t-test or ANOVA were used with 'no significant difference' designated at a 95% confidence-interval (P ≥ 0.05). Linearity of the survey meter was determined for Tc-99m. Readings obtained with the survey meter were also plotted against the scanner results. Results and discussion: The proposed method proved to be accurate (CV of mean RCP < 2), precise (RSD < 2%), linear (slope close to 1, r2 ≥ 0.99) within the RCP range of approximately 80% to 100%, and robust (P > 0.05). LOD and LOQ were determined for the survey meter. Specificity depends on chemical separation. As we were validating the suitability of a method to quantify radioactivity, specificity was not included in the validation parameters. Conclusion: The proposed method compared well with the standard method and is suitable as a reliable low cost method for limited resource settings. [ABSTRACT FROM AUTHOR]

Results from medical research from high-income countries may not apply to low- and middle-income countries. Some expatriate physicians combine clinical duties with research. We present global health research conducted by Dutch medical doctors in Global Health and Tropical Medicine in low- and middle-income countries and explore the value of their research. We included all research conducted in the last 30 years by medical doctors in Global Health and Tropical Medicine in a low- and middle-income country, resulting in a PhD thesis. Articles and co-authors were found through Medline. More than half of the 18 identified PhD theses concerned maternal health and obstetrics, and the majority of the research was conducted in low-income countries, mostly in rural hospitals. Background: At the turn of the 90s, studies showed that health research contributed little to health and development in low- and middle-income countries because it was oriented towards international priorities and dominated by researchers from the North. A new approach to North-South collaboration was required that would support demand-driven and locally led research in the South. The aim of this study was to analyse the development and functioning of a programme for demand-driven and locally led research in Ghana that was supported by a North-South collaboration.Methods: For this mixed-method case study, we combined document analysis, key informant interviews and observation of programme events.Results: The development of the research programme started with constructing a sponsorship constellation in the Netherlands. After highlighting the problems with traditional research collaboration, an advisory council formulated a vision for a more equal and effective approach to North-South collaboration. Together with Ghanaian partners, this vision was turned into a proposal for a Ghanaian-led programme for demand-driven and locally led research, which was funded by the Netherlands government. Research priority setting showed that the Ghanaian research needs were very different from the priorities of foreign funders and researchers. After a slow start, the number of locally submitted proposals increased from 13 in 2001 to 94 in 2005, revealing the existence of a substantial, but partly latent reservoir of research capacity. In total, 79 studies were funded. An impact evaluation showed that the results of the majority of the studies were used to contribute to action. Despite its success, the research programme came to an end in 2008 after the sponsorship constellation in the Netherlands fell apart.Conclusion: Our study shows that realising a programme for demand-driven and locally led research in the South provides an effective approach to North-South collaboration in which results are used and local capacities and institutions are strengthened. Purpose: The purpose of this study was to elucidate why neglected clubfeet still exists despite the availability of a highly (cost)-effective treatment-the Ponseti treatment.Methods: A qualitative and partly quantitative study with semi-structured interviews was conducted in four countries: the Netherlands, South Africa, Argentina and Indonesia with both caregivers, mostly parents, of children with clubfoot and practitioners treating clubfoot. The topics discussed with the caregivers (n = 51) were the conceptions of the cause of clubfeet, received information, accessibility, financial aspects, and social stigma. With the practitioners (n = 11) the focus of the interviews was the treatment protocol and finance.Results: Several barriers towards the start of the treatment were highlighted. At all places treatment was financed by the government, insurance or charity. Nevertheless, the cost of transport and missed working days formed a barrier, although there is a large difference between and within countries. Poverty, long travel duration, and beliefs of supernatural causes for the clubfoot result in delay in the start of treatment.Conclusion: These are problems we need to address when making effective treatment available for every child to diminish the burden of neglected clubfoot; especially accessible clinics in rural areas can be a good alternative to highly specialized hospitals in large cities. We as a community should try to find and overcome the barriers to treat these patients, because we have a relatively easy and highly cost-effective treatment option which can be given by trained non-physicians supervised by an interested medical doctor. Hottola introduced ‘culture confusion’ as a new theoretical model to overcome the inadequacies of Oberg’s ‘culture shock’. This article proposed culture unrest as a new conceptualization whereby culture shock is negotiated at home before the overseas trip begins. In the past years, the growing number of immigrants in some countries has transformed culture shock into culture unrest, defined as the context where two cultures live together, but at a level of acceptance that has developed from a state of interest or curiosity to apathy, annoyance or even antagonism. Based on a background of ‘multicultural drama’, the study examined the perceptions of native Dutch respondents about the Moroccan community resident in the Netherlands and assessed their intentions to visit Morocco in the future. The transfer of knowledge in an ecotourism project is never a one-way affair. An approach connected to bottomup development is the submersion into another culture, while creating a new organizational structure. For co-creation, patterns that are often latent, such as leadership roles, the association with business, or even the color of education can be revealed by carefully facilitated brainstorms or workshops. Especially in countries with a different hierarchical structure, such as Indonesia compared to Holland, a careful analysis is needed before starting cooperation. Although a case is only a temporary view on a situation and not a guarantee for a truly sustainable system, the bottom-up approach tested has interesting starting points for an ecotourism system. The article focuses on controlling nosocomial infections in countries with limited resources. It states the prevalence of nosocomial infections is generally higher in developing countries with limited resources than industrialized countries. It explains the challenges in controlling nosocomial infections by comparing Turkey, which has limited resources and the Netherlands, which has reasonable resources, with hospitals, staffing and hygiene being the major factors.

The Indicator for Coastal Eutrophication Potential (ICEP) for river nutrient export of nitrogen, phosphorus, and silica at the global scale was first calculated from available measurement data. Positive values of ICEP indicate an excess of nitrogen and phosphorus over silica and generally coincide with eutrophication. The sign of ICEP based on measured nutrient fluxes was in good agreement with the corresponding one calculated from the Global-NEWS models for more than 5000 watersheds in the world. Calculated ICEP for the year 2050 based on Global NEWS data for the four Millennium Ecosystem Assessment scenarios show increasing values particularly in developing countries. For further evaluation of the ICEP at the outlet of the rivers of the world based on measurements, there is a need for additional determination silica fluxes and concentrations, which are scarcely documented. The article offers the author's insights on the International Communications and Third World Participation seminar organized by the Instituto Latinoamericano de Estudios Transnaeionales (ILET) which was held in Amsterdam, Netherlands in September 1977. The author says that the seminar has been considered as a follow-up of the debate on a draft-Resolution on Third World participation in international communications. He mentions that the different opinions of the delegates during the 19th General Conference of Unesco in Nairobi, Kenya in October-November 1976 has emanated from basic ideological differences between the main politico-social systems in the world. He adds that the outcome of the meetings must be attributed on the juridical framework of international communications.

Clinical criteria/Family history-based BRCA testing misses a large proportion of BRCA carriers who can benefit from screening/prevention. We estimate the cost-effectiveness of population-based BRCA testing in general population women across different countries/health systems. A Markov model comparing the lifetime costs and effects of BRCA1/BRCA2 testing all general population women ≥30 years compared with clinical criteria/FH-based testing. Separate analyses are undertaken for the UK/USA/Netherlands (high-income countries/HIC), China/Brazil (upper–middle income countries/UMIC) and India (low–middle income countries/LMIC) using both health system/payer and societal perspectives. BRCA carriers undergo appropriate screening/prevention interventions to reduce breast cancer (BC) and ovarian cancer (OC) risk. Outcomes include OC, BC, and additional heart disease deaths and incremental cost-effectiveness ratio (ICER)/quality-adjusted life year (QALY). Probabilistic/one-way sensitivity analyses evaluate model uncertainty. For the base case, from a societal perspective, we found that population-based BRCA testing is cost-saving in HIC (UK-ICER = $−5639/QALY; USA-ICER = $−4018/QALY; Netherlands-ICER = $−11,433/QALY), and it appears cost-effective in UMIC (China-ICER = $18,066/QALY; Brazil-ICER = $13,579/QALY), but it is not cost-effective in LMIC (India-ICER = $23,031/QALY). From a payer perspective, population-based BRCA testing is highly cost-effective in HIC (UK-ICER = $21,191/QALY, USA-ICER = $16,552/QALY, Netherlands-ICER = $25,215/QALY), and it is cost-effective in UMIC (China-ICER = $23,485/QALY, Brazil−ICER = $20,995/QALY), but it is not cost-effective in LMIC (India-ICER = $32,217/QALY). BRCA testing costs below $172/test (ICER = $19,685/QALY), which makes it cost-effective (from a societal perspective) for LMIC/India. Population-based BRCA testing can prevent an additional 2319 to 2666 BC and 327 to 449 OC cases per million women than the current clinical strategy. Findings suggest that population-based BRCA testing for countries evaluated is extremely cost-effective across HIC/UMIC health systems, is cost-saving for HIC health systems from a societal perspective, and can prevent tens of thousands more BC/OC cases.
