Significance: Numerous optical imaging and spectroscopy techniques are used to study the tissue-optical properties; the majority of them are limited in information regarding the penetration depth. A simple, safe, easily applicable diagnostic technique is required to get deeper tissue information in a multilayer structure. Aim: A fiber-based diffuse reflectance (DR) technique is used to extract and quantify the bottom layer absorption coefficients in two-layer (2L) tissue-mimicking solid phantoms. We determine the Indian black ink concentrations in a deep-hidden layer that is sandwiched between agar and silicone-based phantom layers. Approach: A fiber-based DR experiment was performed to study the optical properties of the tissue at higher penetration depth, with different fiber core diameters and a constant numerical aperture (0.5 NA). The optimal core diameter of the fiber was chosen by measuring solid phantoms. In 2L phantoms, the thickness of the top layer was kept 5.5 mm with a constant absorption and reduced scattering coefficients (μa = 0.045 mm − 1 and μs ′ = 2.622 mm − 1), whereas the absorption coefficients of the bottom layers were varied from 0.014 to 0.037 mm − 1 keeping the μs ′ the same as the top layer. A unique crossover point (Cp) was found in the DR intensity profile against distance. We examined the slope before and after the Cp. These two slopes indicate the difference between the optical properties of the top and bottom layers. Our technique got further verification, as we successfully determined the Cp with different Indian black ink concentrations, placed at the junction between the agar and silicone-based phantom layers. Results: The DR measurements were applied to 2L phantoms. Two different slopes were found in 2L phantoms compared to the one-layer (optical properties equal to the top layer of 2L). We extracted the slopes before and after the Cp in the 2L phantoms. The calculated absorption coefficients before the Cp were 0.014 ± 0.0004, 0.022 ± 0.0003, 0.028 ± 0.0003, and 0.036 ± 0.0014 mm − 1, and the absorption coefficients after the Cp were 0.019 ± 0.0013, 0.013 ± 0.0004, 0.014 ± 0.0006, and 0.031 ± 0.0001 mm − 1, respectively. The calculated absorption coefficients before the Cp were in good agreement with the optical properties of the bottom layer. The calculated absorption coefficients after the Cp were not the same as the top layer. Our DR system successfully determines the crossover points 12.14 ± 0.11 and 11.73 ± 0.15 mm for 70% and 100% ink concentrations placed at the junction of the agar and silicone layers. Conclusions: In a 2L tissue structure, the Cp depends on the absorption coefficients of top and bottom layers and the thickness of the top layer. With the help of the Cp and the absorption coefficients, one can determine the thickness of the top layer or vice versa. The slope value before the Cp in the DR profile allowed us to determine the absorption properties of the bottom layer instead of having the average behavior of the 2L phantom in the far detection range (11.0 to 17.0 mm).

Significance: Despite its advantages in terms of safety, low cost, and portability, functional near-infrared spectroscopy applications can be challenging due to substantial signal contamination from hemodynamics in the extracerebral layer (ECL). Time-resolved near-infrared spectroscopy (tr NIRS) can improve sensitivity to brain activity but contamination from the ECL remains an issue. This study demonstrates how brain signal isolation can be further improved by applying regression analysis to tr data acquired at a single source–detector distance. Aim: To investigate if regression analysis can be applied to single-channel trNIRS data to further isolate the brain and reduce signal contamination from the ECL. Approach: Appropriate regressors for trNIRS were selected based on simulations, and performance was evaluated by applying the regression technique to oxygenation responses recording during hypercapnia and functional activation. Results: Compared to current methods of enhancing depth sensitivity for trNIRS (i.e., higher statistical moments and late gates), incorporating regression analysis using a signal sensitive to the ECL significantly improved the extraction of cerebral oxygenation signals. In addition, this study demonstrated that regression could be applied to trNIRS data from a single detector using the early arriving photons to capture hemodynamic changes in the ECL. Conclusion: Applying regression analysis to trNIRS metrics with different depth sensitivities improves the characterization of cerebral oxygenation signals.

For the validation of optical diagnostic technologies, experimental results need to be benchmarked against the gold standard. Currently, the gold standard for tissue characterization is assessment of hematoxylin and eosin (H&E)-stained sections by a pathologist. When processing tissue into H&E sections, the shape of the tissue deforms with respect to the initial shape when it was optically measured. We demonstrate the importance of accounting for these tissue deformations when correlating optical measurement with routinely acquired histopathology. We propose a method to register the tissue in the H&E sections to the optical measurements, which corrects for these tissue deformations. We compare the registered H&E sections to H&E sections that were registered with an algorithm that does not account for tissue deformations by evaluating both the shape and the composition of the tissue and using microcomputer tomography data as an independent measure. The proposed method, which did account for tissue deformations, was more accurate than the method that did not account for tissue deformations. These results emphasize the need for a registration method that accounts for tissue deformations, such as the method presented in this study, which can aid in validating optical techniques for clinical use. Significance: Oxygenation is one of the skin tissue physiological properties to follow for patient care management. Furthermore, long-term monitoring of such parameters is needed at the patient bed as well as outside the hospital. Diffuse reflectance spectroscopy has been widely used for this purpose. Aim: The aim of the study is to propose a low-cost system for the long-term measurement of skin physiological parameters in contact. Approach: We have developed a low-cost, wearable, CMOS-based device. We propose an original method for processing diffuse reflectance data to calculate the tissue oxygen saturation (StO2). Results: We tested the device for the assessment of tissue oxygenation during a first-in-human clinical trial that took place at the Grenoble University Hospital France. Conclusions: The results of this clinical trial show a good accordance between our sensor and commercial devices used a reference.

Significance: Near-infrared spectroscopy (NIRS) combined with diffuse correlation spectroscopy (DCS) provides a noninvasive approach for monitoring cerebral blood flow (CBF), oxygenation, and oxygen metabolism. However, these methods are vulnerable to signal contamination from the scalp. Our work evaluated methods of reducing the impact of this contamination using time-resolved (TR) NIRS and multidistance (MD) DCS. Aim: The magnitude of scalp contamination was evaluated by measuring the flow, oxygenation, and metabolic responses to a global hemodynamic challenge. Contamination was assessed by collecting data with and without impeding scalp blood flow. Approach: Experiments involved healthy participants. A pneumatic tourniquet was used to cause scalp ischemia, as confirmed by contrast-enhanced NIRS, and a computerized gas system to generate a hypercapnic challenge. Results: Comparing responses acquired with and without the tourniquet demonstrated that the TR-NIRS technique could reduce scalp contributions in hemodynamic signals up to 4 times (rSD=3 cm) and 6 times (rSD=4 cm). Similarly, blood flow responses from the scalp and brain could be separated by analyzing MD DCS data with a multilayer model. Using these techniques, there was no change in metabolism during hypercapnia, as expected, despite large increases in CBF and oxygenation. Conclusion: NIRS/DCS can accurately monitor CBF and metabolism with the appropriate enhancement to depth sensitivity, highlighting the potential of these techniques for neuromonitoring.

Significance: The practicality of optical methods detecting tissue optical contrast (absorption, elastic and inelastic scattering, fluorescence) for surgical guidance is limited by interferences from blood pooling and the resulting partial or complete inability to interrogate cortex and blood vessels. Aim: A multispectral diffuse reflectance technique was developed for intraoperative brain imaging of hemodynamic activity to automatically discriminate blood vessels, cortex, and bleeding at the brain surface. Approach: A manual segmentation of blood pooling, cortex, and vessels allowed the identification of a frequency range in hemoglobin concentration variations associated with high optical signal in blood vessels and cortex but not in bleeding. Reflectance spectra were then used to automatically segment areas with and without hemodynamic activity as well as to discriminate blood from cortical areas. Results: The frequency range associated with low-frequency hemodynamics and respiratory rate (0.03 to 0.3 Hz) exhibits the largest differences in signal amplitudes for bleeding, blood vessels, and cortex. A segmentation technique based on simulated reflectance spectra initially allowed discrimination of blood (bleeding and vessels) from cortical tissue. Then, a threshold applied to the low-frequency components from deoxyhemoglobin allowed the segmentation of bleeding from vessels. A study on the minimum acquisition time needed to discriminate all three components determined that ∼25 s was necessary to detect changes in the low-frequency range. Other frequency ranges such as heartbeat (1 to 1.7 Hz) can be used to reduce the acquisition time to few seconds but would necessitate optimizing instrumentation to ensure larger signal-to-noise ratios are achieved. Conclusions: A method based on multispectral reflectance signals and low-frequency hemoglobin concentration changes can be used to distinguish bleeding, blood vessels, and cortex. This could be integrated into fiber optic probes to enhance signal specificity by providing users an indication of whether measurements are corrupted by blood pooling, an important confounding factor in biomedical optics applied to surgery.

Blast-induced traumatic brain injury has been a recent major concern in neurotraumatology. However, its pathophysiology and mechanism are not understood partly due to insufficient information on the brain pathophysiology during/immediately after shock wave exposure. We transcranially applied a laser-induced shock wave (LISW, ∼19 Pa·s) to the left frontal region in a rat and performed multispectral imaging of the ipsilateral cortex through a cranial window (n=4). For the spectral data obtained, we conducted multiple regression analysis aided by Monte Carlo simulation to evaluate vascular diameters, regional hemoglobin concentration (rCHb), tissue oxygen saturation (StO2), oxygen extraction fraction, and light-scattering signals as a signature of cortical spreading depolarization (CSD). Immediately after LISW exposure, rCHb and StO2 were significantly decreased with distinct venular constriction. CSD was then generated and was accompanied by distinct hyperemia/hyperoxemia. This was followed by oligemia with arteriolar constriction, but it soon recovered (within ∼20 min). However, severe hypoxemia was persistently observed during the post-CSD period (∼1 h). These observations indicate that inadequate oxygen supply and/or excessive oxygen consumption continued even after blood supply was restored in the cortex. Such a hypoxemic state and/or a hypermetabolic state might be associated with brain damage caused by a shock wave.

The determination of aluminum hydrochloride was performed at a wavelength of 615 nm, by measuring consumption of the purple colorimetric reagent Alizarin S, due to reaction with aluminum. The linear range was from 10.0 to 125.0 mg L−1 and could be described by the equation: AR = 0.4479 − 0.002543 CAl (R = 0.999).
