This paper employs data from the 2021 UK census to initially explore sub-state (English, Welsh) national identities among minority ethnic groups. This shows that these identities remain much more exclusive of people in minority groups than is a British identity, and that this exclusion is particularly marked with respect to English identity. The analysis then builds on this observation using similar data to examine English identification among the White British ‘majority’ in a ‘superdiverse’ city – London. Attributes which are typically shared by London boroughs in which identification as English deviates most from the national average, and multi-variable analysis which considers the ethnic structure of the borough in which an individual lives alongside other key factors (age, education, social class) suggest differences in identification between people living in boroughs that are characterised by more established and extensive ethnic diversity and those in boroughs transitioning from a previously more homogeneous (white) ethnic structure. In exploring how the articulation of a specific national identity might relate to ethnically-diverse or ‘superdiverse’ contexts, the paper uniquely contributes to recent research which calls for a stronger focus on how people who do not belong to migrant-minority groups might respond to living in such contexts. A lack of regional data on the occupational structure in England and Wales during the interwar years has so far prevented extensive study of this time period. In the current paper, we fill this gap by reconstructing the occupational structure at the district level, based on a recently-digitized register for 1939 and by linking this dataset with the population censuses of 1911 and 1921. The resulting data reveals significant regional differences in the expansion of the tertiary sector, and the relative decline of agricultural and industrial activities. For industry, we find an increase in the level of geographical concentration during 1911–1921, to decline by 1939. The primary sector followed a similar pattern, whereas activities in the tertiary sector became less concentrated. [ABSTRACT FROM AUTHOR]

Non-stationary methods of flood frequency analysis are widespread in research but rarely implemented by practitioners. One reason may be that research papers on non-stationary statistical models tend to focus on model fitting rather than extracting the sort of results needed by designers and decision makers. It can be difficult to extract useful results from non-stationary models that include stochastic covariates for which the value in any future year is unknown. We explore the motivation for including such covariates, whether on their own or in addition to a covariate based on time. We set out a method for expressing the results of non-stationary models as an integrated flow estimate, which removes the dependence on the covariates. This can be defined either for a particular year or over a longer period of time. The methods are illustrated by application to a set of 375 river gauges across England and Wales. We find annual rainfall to be a useful covariate at many gauges, sometimes in conjunction with a time-based covariate. For estimating flood frequency in future conditions, we advocate exploring hybrid approaches that combine the best attributes of non-stationary statistical models and simulation models that can represent changes in climate and river catchments. Research frameworks for archaeology in the UK have a long history. Since the 1990s research frameworks have been developed in formal programmes initially driven by state heritage bodies. These were intended to facilitate better decision-making in development-driven archaeological projects, and to provide an interface between archaeologists. However the effectiveness of such frameworks is limited by a number of constraints. These include 'internal' boundaries created by historic environment professionals: chronological borders; professional borders; disciplinary borders; and borders limiting access and regulating control. There are also boundaries created by others, which include institutional and resourcing constraints as well as the geographical limits of modern administrative boundaries. This paper discusses these issues through the prism of the border region between England and Wales, a borderland zone with long histories of conflict and co-operation. Purpose -- The purpose of this paper is to examine the implications of recent local government reformfor political leadership in local authorities. In particular, it investigates the role played by local government officers in shaping and directing the work of politicians serving on overview and scrutiny committees. Design/methodology/approach -- Drawing on data from five case studies of local councils in England and Wales, the paper suggests that recent reform has done little to clarify decision making and leadership in local government. Findings -- The paper suggests that local councillors need to define their roles under the new system in order to reflect the changing requirements of their work. Originality/value -- The paper investigates the impact of the introduction of overview and scrutiny on the relations between councillors and local government officers. Research on leadership in local government is well established, but this literature needs updating in the light of recent reforms. The paper suggests a complex pattern of leadership in local democratic processes, and suggests that more work needs to be done to ensure that local councillors adapt to their new roles. Toxocara infection occurs through ingestion of parasite eggs excreted by dogs and cats, and can cause severe morbidity. The burden of disease in England and Wales is not well described, and the impact of public health campaigns conducted in the mid-1990s is uncertain. Purpose – The paper benchmarks compliance for 2010-2011 with the RICS Code of Practice for Commercial Service Charges 2006. Design/methodology/approach – Whether the proxy adopted is measured by floorspace or number of commercial office buildings, the sample size conforms to Kreycie and Morgan's determination for representative sample size. Data are generated directly from the original documents provided to commercial leaseholders to ensure authenticity and remove the need for third party reporting of said data. This guarantees the data are valid. Findings – The research discovers that compliance with the RICS Code of Practice for commercial service charges is poorly, if variably, implemented. This contrasts with claims by the professional body. Research limitations/implications – The work only concerns 17 corporate tenants operating principally in the financial services sector and data are drawn from the clients of one property services company. Content analysis is utilised in order to interpret the data and requires some subjective judgement by the researchers. The work only refers to multi-let office space in England and Wales. Originality/value – Data are original and the paper offers a unique benchmarking test. This contrasts markedly with the anecdotal evidence offered by the profession in defending their standards of practice and whilst the paper has limitations, it is the largest and most accurate study yet carried out in the field. Purpose - The purpose of this paper is to examine how Registered Intermediaries are used in the England and Wales to facilitate communication between vulnerable witnesses, victims and police investigators and criminal courts.Design/methodology/approach - The paper focuses on the need for early identification of the vulnerable person so that support measures can be put in place from the outset to assist them to provide their testimony.Findings - It is noted that real progress has been made by the introduction of legislation, specifically the Youth Justice and Criminal Evidence Act (1999), and the uptake by the police service of the subsequent special measures put in place. However, the criminal justice service cannot afford to be complacent as research demonstrates that the police and the courts need to be more effective in managing these issues.Originality/value - The paper recommends that support measures are widened to include witnesses and suspects being interviewed by the Independent Police Complaints Commission, HM Customs and Revenue, the Department of Health and the Department of Work and Pensions. Patterns of ethnic residential segregation within England and Wales have been linked to the idea that white people move away from, or avoid moving into, areas with large minority ethnic populations. Using analysis of the 2001 census, this paper explores the internal migration of the 'White British' ethnic group within England and Wales at the electoral ward level. It shows migration patterns are more complex than simply 'white flight' or 'white avoidance': areas of high minority ethnic concentrations with low inflows of the White British ethnic group are also areas of low outflows of the White British ethnic group; areas of high minority ethnic concentrations with high inflows of the White British ethnic group are also areas of high outflows of the White British ethnic group. Local area characteristics are stronger predictors of White British ethnic group mobility than whether the area is characterised by concentrations of minority ethnic groups. [ABSTRACT FROM AUTHOR]

The endangered Eurasian Otter (Lutra lutra) became a protected species in England and Wales in 1978. The gradual recovery of the species coincides with the rise in specimen fishing on stillwater fisheries and increased concerns about predation. Although otter-proof fencing has been identified as the most effective medium-long term solution, the lack of a formalised legal mechanism to remove otters from fenced fisheries compromised livelihoods and otter welfare. Recognising this, the UK Wild Otter Trust (UKWOT) successfully lobbied for a licence to humanely trap and remove them. This paper examines the initial media and stakeholder responses to Natural England introducing the CL36 'Class Licence' to live capture and transport Eurasian otters (Lutra lutra) that are trapped in fenced fisheries to prevent further damage. The paper analyses the UKWOT 'Interventions Spreadsheet' for the three years of the CL36 licence (November 2016 to June 2019) and the AT Fishery Management Advisors (FMA) 'Otter Log' (March 2017 to March 2019), to trace otter-related enquiries. Purpose - The aim of this paper is to examine the significance and the implications of efforts to institute workforce modernization within the police service in England and Wales. Design/methodology/approach - The approach taken uses an analysis of the modernization proposals advanced by Her Majesty's Inspectorate of Constabulary as its starting point. Findings - The development of workforce modernization in the police service would appear to have eroded the hitherto "reform-resistant" nature of policing, however political factors continue to impede reform. Research limitations/implications - Although more evidence concerning the scale and the outcomes of the reform process would be desirable, the main implication of this paper is that workforce modernization in the police is viable, but constrained by political factors. Originality/value - Empirically, the paper focuses on developments in a sector--the police service--that has been neglected by the existing literature on workforce modernization; theoretically, it demonstrates the important influence often exercised by political contingencies over public sector workforce reform. We present a model of 19th Century population change in England & Wales. The model highlights contrasting demographic and economic processes in rural and urban sectors as core explanations for rural to urban migration, stemming from labor surpluses in the former and labor shortages in the latter. This massive migration transformed the geographic distribution of the population, in tandem with the economic transformation caused by the Industrial Revolution. We argue that demographic literature on historical population processes in England & Wales in the 19th Century has paid insufficient attention to the role of internal migration. Current drought and water scarcity management in England and Wales is reactive rather than proactive and the array of instruments and measures focuses primarily on what to do when in a drought, but less on how to prevent a drought or to plan ahead for a drought. This paper presents the results of a workshop that was held in late 2016 with stakeholders and researchers from UK drought and water scarcity management – the environment ministry (Department for Environment, Food and Rural Affairs), the Environment Agency, water companies, the economic regulator and water consultancies using the explorative scenario technique. The workshop offered the opportunity for unconstrained blue-sky thinking about drought management options based on the question What can happen? and it is useful in cases where there is fairly good knowledge regarding how the system works at present, but one is interested in exploring the consequences of alternative developments in drought management. The workshop developed four scenarios on the topic of resilient drought and water scarcity management in England and Wales in 2065: (1) 'Accepting decline', (2) 'Rising to the challenge', (3) 'Enjoying their luck' and (4) 'Passive acceptance'. Given the centrality of regions in social movements, politics and public administration, here we aim to quantitatively study regional identity, cross-region communication and sentiment. This paper presents a new methodology to study social interaction within and between social-geographic regions, and then applies the methodology to a case study of England and Wales. We use a social network, built from geo-located Twitter data, to identify contiguous geographical regions with a shared social identity and then investigate patterns of communication within and between them. In contrast to other approaches (e.g. using phone call data records or online friendship networks), use of Twitter data provides message contents as well as social connections. This allows us to investigate not only the volume of communication between locations, but also the sentiment and vocabulary used in the messages. For example, our case study shows: a significant dialect difference between England and Wales; that regions tend to be more positive about themselves than about others, with the South being more ‘self-regarding’ than the North; and that people talk politics much more between regions than within. This study demonstrates how social media can be used to quantify regional identity and inter-region communications and sentiment, exposing these previously hard-to-observe geographic concepts to analysis. The view that vocational education and training is central to economic prosperity and social well‐being is one that is widely held by policy‐makers in many countries. Delivering this agenda requires skilled and professional teachers. Ensuring that vocational teachers are able to maintain and develop both their ‘craft’ skills and pedagogy, through continuing professional development (CPD), is, therefore, a growing area of policy concern. While research has revealed that models for the regulation and organisation of CPD for vocational teachers vary across countries, their relative effectiveness has rarely been considered. This paper seeks to address this gap by comparing the approaches currently being adopted in three countries – England, Wales and Norway. Focusing specifically on teachers of hairdressing, it explores the opportunities for CPD and considers the constraints on teachers’ ability to reflect and improve on their practice. The research draws mainly on interviews with teachers of hairdressing in two English and two Welsh further education colleges and three Norwegian upper‐secondary schools. The findings suggest that while hairdressing lecturers in England and Wales have more opportunities to keep up to date with their ‘craft’, those in Norway find it easier to access pedagogic education and inhabit an institutional environment that affords more time and space to share ideas and collectively improve their practice. The object of this paper is to assess how far developments in the labour market can help to explain the fluctuations in births which have been recorded in England and Wales between 1952 and 1980. We examine separately the period rate of women from each of the first four parities proceeding to another birth. Our analysis shows that different birth orders respond differently to economic variables, and that women of different ages but the same parity respond differently. We have found that growing real wages for both men and women tend to deter older parents from adding to existing families. During the early stages of family building, births are inhibited by labour markets favourable to women. But conditions in the labour market for men have the opposite effect on early breeding. This paper presents an analysis of levels of life expectancy and their patterns of change among six socio-economically differentiated sub-populations of England and Wales for the period 1851-1911. Differences in mortality levels among these sub-groups and their rates of change are analyzed with respect to three groups of explanatory variables, viz., environmental, stratification and demographic variables. Their relative importance for different periods is assessed and discussed. The findings show consistency with two previous studies, which have suggested that medical advances had little effect on the increase in life expectancy during the second half of the nineteenth century. The present paper supports the results of one study in that public health measures affected life expectancies earlier, while subsequently, the increase in standards of living was more important. Debates have persisted about the character of the East–West population flows that followed the EU expansions. Some of the discussions surround the extent to which the mobility has been temporal and hence how likely these migrants are to settle permanently or to stay for long periods in host countries. This paper enhances the understanding of such issues through examining survey data on 700 Polish recent migrants in seven English and Welsh towns and supplemented by an analysis of qualitative data. Three categories of migrants are initially identified on the basis of their intentions of stay in the UK. Multinomial logit models are then estimated to examine the characteristics of individuals in each category to establish the factors that influence migration strategies and changes in plans. Results indicate that although standard socio-economic characteristics tend to be insignificant, migration strategies and changes in intentions are affected by migrant's view of whether their job matches their expectations, the time of entry into the UK and remittances. Analysis of the qualitative information reinforces some of the key findings and also indicates the importance of migrants' ability to reference their experiences in two settings. This paper summarises the findings of a study on second trimester abortion in England and Wales in 2005. Second trimester abortions constitute a relatively small proportion of the total number of legal abortions performed in these countries yet attract quite substantial public, and particularly media, attention. Discussion of these abortions has, however, been conducted within a context of little understanding of the factors which explain why they happen. This paper starts with a brief introduction to the policy context for provision of second trimester abortion, and a summary of existing research in the area. It then presents the results of a survey of 883 women on their own reasons why they had abortions in the second trimester. The key concept is that of "delay" and reasons for delay in seeking or obtaining abortion at five stages in the pathway to abortion. No clear, single reason emerges. Amongst the main reasons identified are uncertainty about what to do if they were pregnant, not realising they were pregnant, experiencing bleeding which may have been confused with continuing to have periods, and changes in personal circumstances. The paper ends with a consideration of the implications of the results for education, policy development and service provision. Les avortements du deuxième trimestre représentent une proportion relativement modeste du nombre total d'avortements légaux pratiqués dans ces pays, mais ils attirent beaucoup l'attention du public et en particulier des médias. Le débat sur ces avortements s'est néanmoins déroulé dans un contexte où les facteurs qui les expliquent sont mal compris. L'article commence par une brève présentation du contexte politique pour la pratique de l'avortement du deuxième trimestre, et un résumé de la recherche dans ce domaine. Il expose ensuite les résultats d'une enquête ayant demandé à 883 femmes leurs raisons pour avoir avorté au deuxième trimestre. Le concept clé est celui de « retard » et des raisons du retard de la demande et de l'obtention de l'avortement à cinq étapes de la voie vers l'avortement. Aucune raison claire et unique n'est apparue. Después, se presentan los resultados de una encuesta entre 883 mujeres sobre sus propios motivos para tener abortos en el segundo trimestre. El concepto clave es el de "demora" y las razones para aplazar la búsqueda u obtención de servicios de aborto en cinco etapas en la ruta hacia el aborto. No surge ninguna razón clara y única. This paper investigates the perceptions of tourists and gatekeepers (such as tour operators and destination marketing organizations) on integrated rural tourism (IRT), noting their role in consuming and marketing the more recently acknowledged qualities of rurality, such as food processing, creativity and the arts, heritage and outdoor recreation. The focus is on the potential for IRT development in the England-Wales border region, an atypical tourist destination, peripheral in character and lacking a consolidated tradition of well-established destinations. Results from tourist questionnaires suggest that their overall experience in the region is positive, but one that can improve further with progress on the key dimensions of IRT. Findings also indicate that, while the national border presents a unique opportunity for cooperative branding and marketing efforts, only a few gatekeepers are capitalizing on the brand of the 'Marches' (the border) to promote the market and visual presence of the region, both nationally and internationally. Background: Around 6.5% of the population in the United Kingdom has been diagnosed with diabetes. It is associated with several long-term consequences and higher hospitalization rates. Aim: To examine the profile of hospital admissions related to diabetes mellitus and the prescription rates of antidiabetic medications in England and Wales. Method: This is an ecological study that was conducted for the period between April 1999 and April 2020 using publicly available hospitalisation data in England and Wales. Hospital admission data for patients of all ages was extracted from Hospital Episode Statistics in England and the Patient Episode Database for Wales. The difference between admission rates in 1999 and 2020, as well as the difference between diabetes mellitus medication prescription rates in 2004 and 2020, were assessed using the Pearson Chi-squared test. A Poisson regression model with robust variance estimation was used to examine the trend in hospital admissions. Results: A total of 1,757,892 diabetes mellitus hospital admissions were recorded in England and Wales during the duration of the study. The hospital admission rate for diabetes mellitus increased by 15.2%. This increase was concomitant with an increase in the antidiabetic medication prescribing rate of 105.9% between 2004 and 2020. Males and those in the age group of 15–59 years had a higher rate of hospital admission. The most common causes of admissions were type 1 diabetes mellitus related complications, which accounted for 47.1% of all admissions. Conclusion: This research gives an in-depth overview of the hospitalization profile in England and Wales during the previous two decades. In England and Wales, people with all types of diabetes and related problems have been hospitalized at a high rate over the past 20 years. Male gender and middle age were significant determinants in influencing admission rates. Diabetes mellitus type 1 complications were the leading cause of hospitalizations. We advocate establishing preventative and educational campaigns to promote the best standards of care for individuals with diabetes in order to lower the risk of diabetes-related complications. Objective: To investigate the trends in hospital admissions and medication prescriptions related to asthma and chronic obstructive pulmonary disease (COPD) in England and Wales. Methods: An ecological study was conducted between April 1999 and April 2020 using data extracted from the hospital episode statistics database in England and the patient episode database for Wales. The Office of National Statistics mid-year population estimates for 1999 through 2020 were collected, and medication prescription data for 2004–2020 were extracted from the prescription cost analysis database. Results: The total annual number of COPD and asthma hospital admissions for various causes increased by 82.2%, from 210,525 in 1999 to 383,652 in 2020, representing a 59.1% increase in hospital admission rate (from 403.77 in 1999 to 642.42 per 100,000 persons in 2020, p < 0.05). Chronic obstructive pulmonary disease with acute lower respiratory infection accounted for 38.7% of hospital admissions. Around 34.7% of all hospital admissions involved patients aged 75 and older. Around 53.8% of all COPD and asthma hospital admissions were attributable to females. The annual number of prescriptions dispensed for COPD and asthma medications increased by 42.2%. Conclusions: Throughout the study period, hospital admissions due to chronic obstructive pulmonary disease and asthma, as well as medication prescriptions, increased dramatically among all age groups. Hospitalization rates were higher for women. Further observational and epidemiological research is required to identify the factors contributing to increased hospitalization rates. Background. The sex ratio at birth (male live births divided by total live births) may be a sentinel health indicator. Stressful events reduce this ratio 3-5 months later by increasing male fetal loss. This ratio can also change 9 months after major population events that are linked to an increase or decrease in the frequency of sexual intercourse at the population level, with the ratio either rising or falling respectively after the event. We postulated that the COVID-19 pandemic may have affected the ratio in England and Wales. Methods. Publicly available, monthly live birth data for England and Wales was obtained from the Office for National Statistics up to December 2020. Using time series analysis, the sex ratio at birth for 2020 (global COVID-19 onset) was predicted using data from 2012-2019. Observed and predicted values were compared. Results. From 2012-2020 there were 3,133,915 male and 2,974,115 female live births (ratio 0.5131). Three months after COVID-19 was declared pandemic (March 2020), there was a significant fall in the sex ratio at birth to 0.5100 in June 2020 which was below the 95% prediction interval of 0.5102-0.5179. Nine months after the pandemic declaration, (December 2020), there was a significant rise to 0.5171 (95% prediction interval 0.5085-0.5162). However, December 2020 had the lowest number of live births of any month from 2012-2020. Conclusions. Given that June 2020 falls within the crucial window when population stressors are known to affect the sex ratio at birth, these findings imply that the start of the COVID-19 pandemic caused population stress with notable effects on those who were already pregnant by causing a disproportionate loss of male fetuses. The finding of a higher sex ratio at birth in December 2020, i.e., 9 months after COVID-19 was declared a pandemic, could have resulted from the lockdown restrictions that initially spurred more sexual activity in a subset of the population in March 2020. If migration between 1851 and 1911 is to be analysed using individual‐level census data, it must be proven that individuals' place of birth was reliably recorded. However, existing studies have primarily been concerned with establishing only the level of absolute error, rather than how such errors were introduced. Through assessing the determinants of inaccuracy in the 1851–1911 censuses, this article finds that while birthplaces were generally reported accurately, the errors which did exist were not only the fault of the individual in question, but also of the householder whose responsibility it was to fill in the census form, and of those charged with transcribing the original, handwritten schedules into a machine‐readable format. By demonstrating that individuals' ability to identify their place of birth was a function of their age and the distance they migrated from their place of birth, rather than either their intelligence or familiarity with their place of birth per se, shows that individuals' ability to identify their birthplace accurately was a function of the relevance it held in their lives. Consequently, this article argues that individuals' ability to identify their place of birth reflected its role as a component of their identity, becoming less relevant the further from it they became, but increasingly relevant as they aged. This paper investigates the link between cannabis depenalisation and crime using individual-level panel data for England and Wales from 2003 to 2006. We exploit the declassification of cannabis in the UK in 2004 as a natural experiment. Specifically, we use the fact that the declassification changed expected punishments differently in various age groups due to thresholds in British criminal law and employ a difference-in-differences type design using data from the longitudinal version of the Offending, Crime and Justice Survey. This paper critically discusses the challenges mental health nurses face in trying to achieve a balance between fulfilling biomedical and social roles. We suggest that dilemmas exist for nurses in attempting to combine both approaches in their practice. We present a specific example of these as occasioned by the advent of the approved mental health professional role in England and Wales. This statutory role requires the adoption of an independent social perspective as a counterbalance to the biomedical perspective brought by psychiatrists. Using the idea of occupational jurisdictions we discuss how nurses embarking on this new role are effectively crossing into territories previously occupied by the profession of social work. We also reveal the tensions for nurses who fulfil the approved mental health professional role whilst simultaneously carrying out work in other areas which demands a more overtly biomedical approach. We review critical accounts of the validity of bio-psycho-social models and concerns about maintaining positive therapeutic alliances alongside making applications for compulsory detention, assessment and treatment. We argue that the new role may become part of the professional project of mental health nursing, but also present challenges in helping redefine nursing's identity and practice. Background: Identifying trends of hospital admissions for respiratory diseases is crucial for public health and research to guide future clinical improvements for better outcomes. This study aims to define the trends of respiratory disease-related hospital admissions (RRHA) in England and Wales between 1999 and 2019.Methods: An ecological study was conducted using hospital admission data taken from the Hospital Episode Statistics database in England and the Patient Episode Database for Wales. Hospital admissions data for respiratory diseases were extracted for the period between April 1999 and March 2019. The trend in hospital admissions was assessed using a Poisson model.Results: Hospital admission rate increased by 104.7% [from 1535.05 (95% CI 1531.71-1538.38) in 1999 to 3142.83 (95% CI 3138.39-3147.26) in 2019 per 100,000 persons, trend test, p < 0.01]. The most common causes were influenza and pneumonia, chronic lower respiratory diseases, other acute lower respiratory infections, which accounted for 26.6%, 26.4%, and 14.9%, respectively. The age group 75 years and above accounted for 34.1% of the total number of hospital admissions. Males contributed to 50.5% of the total number of hospital admissions. Hospital admission rate in females increased by 119.8% [from 1442.18 (95% CI 1437.66-1446.70) in 1999 to 3169.38 (95% CI 3163.11-3175.64) in 2019 per 100,000 persons, trend test, p < 0.001]. It reflects on problems of definition and on the limited availability of data concerning the origins and geographical distribution of greens. The first clinical case of persistent HEV infection in England was reported in 2009. We describe the demography, virology and outcomes of patients identified with persistent HEV infection in England and Wales between 2009 and 2017. A series of 94 patients with persistent HEV infection, defined by HEV viraemia of more than 12 weeks, was identified through routine reference laboratory testing. Virology, serology and clinical data were recorded through an approved PHE Enhanced Surveillance System. Sixty‐six cases (70.2%) were transplant recipients, 16 (17.0%) had an underlying haematological malignancy without stem cell transplantation, six (6.4%) had advanced HIV infection, five (5.3%) were otherwise immunosuppressed, and one patient (1.1%) had no identified immunosuppression. Retrospective analysis of 46 patients demonstrated a median 38 weeks of viraemia before diagnostic HEV testing. At initial diagnosis, 16 patients (17.0%) had no detectable anti‐HEV serological response. Of 65 patients treated with ribavirin monotherapy, 11 (16.9%) suffered virological relapse despite undetectable RNA in plasma or stool at treatment cessation. Persistent HEV infection remains a rare diagnosis, but we demonstrate that a broad range of immunocompromised patients are susceptible. Both lack of awareness and the pauci‐symptomatic nature of persistent HEV infection likely contribute to significant delays in diagnosis. Diagnosis should rely on molecular testing since anti‐HEV serology is insufficient to exclude persistent HEV infection. Finally, despite treatment with ribavirin, relapses occur even after cessation of detectable faecal shedding of HEV RNA, further emphasising the requirement to demonstrate sustained virological responses to treatment. Background: COVID-19 poses one of the most profound public health crises for a hundred years. As of mid-May 2020, across the world, almost 300,000 deaths and over 4 million confirmed cases were registered. Reaching over 30,000 deaths by early May, the UK had the highest number of recorded deaths in Europe, second in the world only to the USA. Hospitalization and death from COVID-19 have been linked to demographic and socioeconomic variation. Since this varies strongly by location, there is an urgent need to analyse the mismatch between health care demand and supply at the local level. As lockdown measures ease, reinfection may vary by area, necessitating a real-time tool for local and regional authorities to anticipate demand.Methods: Combining census estimates and hospital capacity data from ONS and NHS at the Administrative Region, Ceremonial County (CC), Clinical Commissioning Group (CCG) and Lower Layer Super Output Area (LSOA) level from England and Wales, we calculate the number of individuals at risk of COVID-19 hospitalization. Combining multiple sources, we produce geospatial risk maps on an online dashboard that dynamically illustrate how the pre-crisis health system capacity matches local variations in hospitalization risk related to age, social deprivation, population density and ethnicity, also adjusting for the overall infection rate and hospital capacity.Results: By providing fine-grained estimates of expected hospitalization, we identify areas that face higher disproportionate health care burdens due to COVID-19, with respect to pre-crisis levels of hospital bed capacity. Including additional risks beyond age-composition of the area such as social deprivation, race/ethnic composition and population density offers a further nuanced identification of areas with disproportionate health care demands.Conclusions: Areas face disproportionate risks for COVID-19 hospitalization pressures due to their socioeconomic differences and the demographic composition of their populations. Our flexible online dashboard allows policy-makers and health officials to monitor and evaluate potential health care demand at a granular level as the infection rate and hospital capacity changes throughout the course of this pandemic. The outbreak of the COVID-19 pandemic has resulted in various public health responses around the globe. Due to the devolved powers of the United Kingdom, the response has been centralized but simultaneously greatly differing across England, Wales, Scotland, and Northern Ireland. The following article examines the governmental responses to the outbreak, the public health measures taken, data collection and statistics, protective equipment and bed capacity, the society's response, and lastly, the easing of the lockdown restrictions. In terms of the governmental response, the COVID-19 pandemic was initially met with less urgency and social distancing, along with the development of herd immunity, were first mentioned. As the virus continued to spread, the government started imposing stricter measures and a lockdown was implemented. Tests were conducted using a five pillar typology. The collection of information, particularly on COVID-19 associated deaths, varied across the United Kingdom and among the governmental organizations due to differing definitions. In term of hospital bed availability, the rate of hospitalizations was the highest from late March to early April of 2020. Temporary hospitals were constructed, however, they mostly went unused. The United Kingdom society was generally compliant in adapting to the lockdown and trust in the government rose. Nonetheless, as the lockdown progressed, trust in the government began to fall. After several months, the rate of infection decreased and the lockdown in the United Kingdom was lifted in accordance with 'Our plan to rebuild: The United Kingdom Government's COVID-19 recovery strategy'. Background: In October 2017, the United Kingdom Joint Committee on Vaccination and Immunisation (JCVI) recommended removal of one primary dose of the 13-valent pneumococcal conjugate vaccine (PCV13) from the existing 2+1 schedule (2, 4, 12 months). We conducted a mathematical modelling study to investigate the potential impact of a 1+1 (3, 12 month) schedule on invasive pneumococcal disease (IPD) and pneumococcal community-acquired pneumonia (CAP). Our results and those from a 1+1 immunogenicity study formed the key evidence reviewed by JCVI.Methods and Findings: We developed age-structured, dynamic, deterministic models of pneumococcal transmission in England and Wales to describe the impact on IPD of 7-valent PCV (PCV7; introduced in 2006) and PCV13 (introduced in 2010). Key transmission and vaccine parameters were estimated by fitting to carriage data from 2001/2002 and post-PCV IPD data to 2015, using vaccine coverage, mixing patterns between ages, and population data. We considered various models to investigate potential reasons for the rapid increase in non-PCV13 (non-vaccine serotype [NVT]) IPD cases since 2014. After searching a large parameter space, 500 parameter sets were identified with a likelihood statistically close to the maximum and these used to predict future cases (median, prediction range from 500 parameter sets). Our findings indicated that the emergence of individual NVTs with higher virulence resulting from ongoing replacement was likely responsible; the NVT increase was predicted to plateau from 2020. Long-term simulation results suggest that changing to a 1+1 schedule would have little overall impact, as the small increase in vaccine-type IPD would be offset by a reduction in NVT IPD. Our results were robust to changes in vaccine assumptions in a sensitivity analysis. Under the base case scenario, a change to a 1+1 schedule in 2018 was predicted to produce 31 (6, 76) additional IPD cases over five years and 83 (-10, 242) additional pneumococcal-CAP cases, with together 8 (-2, 24) additional deaths, none in children under 15 years. Long-term continuation with the 2+1 schedule, or changing to a 1+1, was predicted to sustain current reductions in IPD cases in under-64-year-olds, but cases in 65+-year-olds would continue to increase because of the effects of an aging population. Limitations of our model include difficulty in fitting to past trends in NVT IPD in some age groups and inherent uncertainty about future NVT behaviour, sparse data for defining the mixing matrix in 65+-year-olds, and the methodological challenge of defining uncertainty on predictions.Conclusions: Our findings suggest that, with the current mature status of the PCV programme in England and Wales, removing one primary dose in the first year of life would have little impact on IPD or pneumococcal CAP cases or associated deaths at any age. A reduction in the number of priming doses would improve programmatic efficiency and facilitate the introduction of new vaccines by reducing the number of coadministered vaccines given at 2 and 4 months of age in the current UK schedule. Background: Scotland has disproportionately high rates of suicide compared with England. An analysis of trends may help reveal whether rates appear driven more by birth cohort, period or age. A 'birth cohort effect' for England & Wales has been previously reported by Gunnell et al. (B J Psych 182:164-70, 2003). This study replicates this analysis for Scotland, makes comparisons between the countries, and provides information on 'vulnerable' cohorts.Methods: Suicide and corresponding general population data were obtained from the National Records of Scotland, 1950 to 2014. Age and gender specific mortality rates were estimated. Age, period and cohort patterns were explored graphically by trend analysis.Results: A pattern was found whereby successive male birth cohorts born after 1940 experienced higher suicide rates, in increasingly younger age groups, echoing findings reported for England & Wales. Young men (aged 20-39) were found to have a marked and statistically significant increase in suicide between those in the 1960 and 1965 birth cohorts. The 1965 cohort peaked in suicide rate aged 35-39, and the subsequent 1970 cohort peaked even younger, aged 25-29; it is possible that these 1965 and 1970 cohorts are at greater mass vulnerability to suicide than earlier cohorts. This was reflected in data for England & Wales, but to a lesser extent. Suicide rates associated with male birth cohorts subsequent to 1975 were less severe, and not statistically significantly different from earlier cohorts, suggestive of an amelioration of any possible influential 'cohort' effect. Scottish female suicide rates for all age groups converged and stabilised over time. Women have not been as affected as men, with less variation in patterns by different birth cohorts and with a much less convincing corresponding pattern suggestive of a 'cohort' effect.Conclusions: Trend analysis is useful in identifying 'vulnerable' cohorts, providing opportunities to develop suicide prevention strategies addressing these cohorts as they age. This comparative study between Wales and England was undertaken to better understand what influences or drives the professed aims for outdoor provision of early years teachers; specifically the extent to which professed aims reflect the research-based literature common to both countries, and/or statutory curricular, which differs in each country. The research gathered quantitative and qualitative data through an online survey. Participants were teachers of children aged four to five years working in the respective country's University partnership schools. Partnership schools are those who work with the University to train teachers. The findings suggestWelsh teachers aim and plan to use their outdoor spaces explicitly for curriculum-related learning more so than their English counterparts who appear not to identify such specific curriculum-related learning outcomes but to emphasise personal/social/dispositional aspects of development for young children when outside. This research indicates how the divergence of education-related policy and curriculum appears to have impacted upon the way practitioners express their aims for outdoor learning in England and Wales. The values underpinning the relative curricular documentation appear to emerge in the intended practice of early years teachers in both countries. We evaluated clinical Shiga toxin-producing Escherichia coli O157 infections in England and Wales during 1983-2012 to describe changes in microbiological and surveillance methods. A strain replacement event was captured; phage type (PT) 2 decreased to account for just 3% of cases by 2012, whereas PT8 and PT21/28 strains concurrently emerged, constituting almost two thirds of cases by 2012. Despite interventions to control and reduce transmission, incidence remained constant. However, sources of infection changed over time; outbreaks caused by contaminated meat and milk declined, suggesting that interventions aimed at reducing meat cross-contamination were effective. Petting farm and school and nursery outbreaks increased, suggesting the emergence of other modes of transmission and potentially contributing to the sustained incidence over time. Studies assessing interventions and consideration of policies and guidance should be undertaken to reduce Shiga toxin-producing E. coli O157 infections in England and Wales in line with the latest epidemiologic findings. The judicial approach to making an adoption order where birth parents do not give their consent suddenly changed after a UK Supreme Court judgement in mid-2013, which examined the position in the light of the Human Rights Act 1998 - Re B [2013] UKSC 33. This article argues that this case did not set a higher threshold for such an order to be made but confirmed existing domestic and European human rights law. However, the unintended consequences of the judgement were an opportunity for the judiciary to take issue with government policy to increase the rate of adoption and with recent family justice reforms, which have constrained judicial oversight of care proceedings. An unprecedented rate of contested litigation, based on perceived shortcomings in social work assessments and judgements in the lower courts, has caused uncertainty and delays in planning for children, with a slowing in the rate of children being adopted, although with little evidence of any associated benefits for birth parents. ABSTRACT Coastal dunes are valuable ecological reserves providing a range of ecosystem services from coastal flood defence to recreational amenity. An important feature of dune systems is the slack floors, areas of high ecological value that are closely linked to the prevailing hydrological system in the dunes. Dune slacks are sensitive environments that are impacted by external changes, such as rainfall patterns and foreshore erosion as well as conservation management. Understanding the effects of these influences upon the water balance in a dune system is fundamental to the management of the slacks. Occurrence of seasonal flood depths, area and timing and how far the water table recedes below ground in summer are critical to the flora and fauna that the slacks will support. Change in the hydrological regime directly impacts the ecosystem. Experience is drawn from four sites in Wales and west coast England: Ainsdale on the Sefton Coast, Newborough Warren in North Wales, Whiteford Burrows in South Wales and Braunton Burrows in North Devon. Correctional institutions which are characterised by authoritarian organisation may control access to health care services through filtering requests through correctional staff, or conversely by using staff to identify and facilitate inmate medical care. We investigated the relationship between inmate-assessed prison social climate and satisfaction with health care in over 4,800 male and female inmates in 49 English/Welsh correctional institutions (ranging from high to low security). A single eleven-item dimension of health care satisfaction was identified in the Measuring the Quality of Prison Life (MQPL) instrument. Multiple regression of the dimensions of prison climate as measured by the MQPL on health care satisfaction indicated that the scales 'Relationships with staff', 'Safety', 'Feedback and care', 'Fairness' and 'Care for vulnerable', predicted 30% of variance in health care satisfaction. Qualitative data on health care issues from a parallel study of twelve English/Welsh local prisons were used to explicate the quantitative findings. Data suggest that positive prison climates facilitated interactions between correctional and health care staff and prisoners, while in negative climates correctional staff acted as a filter or barrier between inmates and health services. Further, data suggested that health care staff themselves may be influenced by prison climate. These findings implicate health care provision and access as an integral part of prison climate, and suggest that inmates' judgments on access to, and satisfaction with, prison health services are significantly associated with general, non-health related prison climate measures. This article focuses on the second-generation Irish in Britain, and presents findings of the relationship between particular social characteristics and predictions of the likelihood of second-generation Irish selecting 'White Irish' or 'White British' in the 2001 Census in England and Wales. Using a combination of new quantitative data and earlier (unpublished) qualitative evidence, it analyses the complexity underlying the public claiming of a British or Irish identity in the Census and argues that it is not possible to predict that individuals with the closest attachments to Ireland will necessarily select the 'White Irish' category nor that those who select 'White British' inevitably have weaker ties. The ONS Longitudinal Survey data presented here reveal that age, gender, marital status, educational qualifications, upward social mobility and number of Irish-born parents are significant social characteristics increasing the likelihood of particular selections of census category. The article discusses the form of the ethnic question and its impact on response patterns, proposed revisions for the 2011 Census, and the usefulness or otherwise of census categories as a lens for examining second-generation identification. As a 'total institution' as well as a tool of government policy in limiting and promoting human mobility, prisons serve important migration-related functions in contemporary society. The social structure of prisons as well as their roles in incapacitating potential migrants and deterring migration contribute to mental health problems amongst the incarcerated. A major consequence of high prevalence of mental illness in prisons is suicide. Irrespective of complex intertwined factors such as harsh penal regime and the psychological impact of arrest an incarceration which increase the risk of prison suicides, mentally ill prisoners are significantly more likely to commit suicide during incarceration and within the first year following release from custody. The author examines the contributions of prison mental health promotion policies and programs in England and Wales to preventing prison suicides, and concludes that Britain's recent initiatives in this sector have a strong potential to sustainably reduce prison suicides as well as improve prisoners' mental health. Changes in hepatitis C virus (HCV) prevalence from 1992 to 2006 were examined by using 24,311 records from unlinked anonymous surveillance of injecting drug users in England and Wales. Bayesian logistic regression was used to estimate annual prevalence, accounting for changing recruitment patterns (age, gender, injecting duration, geographic region, interactions) and the sensitivity and specificity of different oral fluid testing devices. After controlling for these differences, the authors found that the adjusted HCV prevalence decreased from 70% (95% credible interval: 62, 78) in 1992 to 47% (95% credible interval: 43, 51) in 1998 before rising again to 53% (95% credible interval: 48, 58) in 2006. Women injecting drug users had a higher HCV risk than did men (odds ratio = 1.50, 95% credible interval: 1.31, 1.73). Two regions (London and North West) had a markedly higher HCV prevalence than did the rest of England and Wales. Among individuals who had injected for less than 1 year, the adjusted HCV prevalence in 2006 was higher than that in 1992 (28% vs. 19%, respectively). HCV infection can be prevented. Conservative adherence to the sovereignty of the Crown-in-Parliament resulted in the merging of English with British consciousness. During the 1990s, England's political nationalism expressed itself as a defence of Britishness. This defence of Britishness prevented a political English nationalism cohering at a time when political nationalisms had matured in Scotland and Wales. The modern study of fossil crinoids began with J. S. Miller who, in 1821, described specimens from southern England, nearby Wales and other regions, and named several common Early Carboniferous genera. Later, in 1950–60, James Wright monographed all known Early Carboniferous crinoids from the British Isles. In spite of such previous scrutiny, we recognize here two new genera among species already described: Glamorganocrinus gen. nov. (type species: Ophiurocrinus gowerensis Wright, 1960) from South Wales and Mendipocrinus gen. nov. (type species: Poteriocrinus latifrons Austin and Austin, 1847) from southern England. These new genera increase the number of advanced cladid genera in the Ivorian Substage of the Tournaisian in western Europe to 18, and the total number of crinoid genera to 36. A review of species assigned to Mespilocrinus has led to the recognition of M. granulifer De Koninck and LeHon, 1854 as a nomen dubium. A new species of Mespilocrinus, M. wrighti sp. nov., is described from the Ivorian of South Wales; this is the most highly derived species of the genus, as based on a phylogenetic analysis including ten species and 13 characters, with Pycnosaccus as the outgroup. A single, well-ordered tree resulted from this analysis. Interpretation of this tree suggests that the centre of evolution for Mespilocrinus was North America, where three species appeared during the Kinderhookian (early Tournaisian), rapidly achieving morphological disparity within the genus. This radiation event was part of the overall explosive radiation of crinoids following the Late Devonian mass extinction event when crinoid diversity was at a global minimum during the Frasnian. The article presents a study that examines the 1959 and 1983 Mental Health Acts in England and Wales. One issue that is being tackled in this study is the consent to treatment which was not mentioned in the 1959 legislation that assumes that a detained patient could be treated against his will. Furthermore, this study evaluates the issues in England and Wales who advocated and enabled this important change to come about, using examples from the clinical practice of electroconvulsive therapy.

Objectives: To assess the quality of care provided to women undergoing induced abortion. Methods: The design was a single round audit questionnaire aimed at the providers of abortion services throughout England and Wales (NHS hospitals, clinics under NHS, agency agreements and private units. The levels of care were assessed against agreed audit criteria. Results: Responses were received from 240 (74%) of the 324 units providing abortion care in England and Wales. These units undertook 80% of the 176 000 termination procedures. Generally standards of care were appropriate, but there were clear areas for improvement including choice of method, infection screening, and delays in referral. A number of unnecessary investigations were still evident, including cross-matching, while lower doses and alternative drug regimens might reduce costs. Only half of the units audited procedures and complications. Conclusions: This audit was carried out prior to the publication of the RCOG evidence-based guideline and a further round of audit to assess the impact of the guideline should now be considered. Alongside the introduction of a plan-led system in England and Wales, environmental appraisal of development plans has rapidly become a common activity among local authorities. This paper describes a reviewof 71 appraisals with the purpose of examining the practical application of the government's suggested method. It was found that while the basic steps were being undertaken, more complex stages in the process are being neglected. It is concluded that the guidance is generally sound, although some changes would be advantageous. Increasing numbers of one-person households are an important featureof European population geography. This paper examines the increase of one-person households during the 1980s in England and Wales and France set within the broader context of household change in Europe as awhole. The sources used are national censuses and, for England and Wales, the Samples of Anonymised Records and the ONS Longitudinal Study. One-person households vary geographically by age, with younger one-person households found especially in larger urban areas; older one-person households have a more varied distribution, although rural and traditional retirement areas have particular concentrations. Increases in numbers are a result of both compositional changes in the population (increasing numbers of divorced and never-marrieds as well as increasing numbers of elderly) and also an increasing propensity to live alone, especially among younger age groups. While more younger men than women live alone, the latter are catching up. In general, living alone is associated with higher social class, especially for women, who show much larger social class contrasts with all women than do the living-alone men with all men. Traditionally, privately-rented accommodation has been the most important tenure arrangement of one-person households, but has been replaced by owner occupation. The study adds to our understanding of the changing nature of living alone and emphasises its geographical significance; there are variations withineach country and while there are many similarities between the two countries, they also continue to show contrasts. Public participation is central to the IWRM discourse and often associated with claims of improved environmental policy outputs and their implementation. Whilst the involvement of nonstate actors in environmental decision-making has attracted scholarly attention from various angles, our knowledge is scant as to the forces that drive organisational reform towards participatory governance. This article sets out to contribute to this largely neglected research area and explores conditions under which policy-makers would be willing to attend towards more participative water governance. Its ambition is twofold: first, to explore the conditions under which public officials attempt to institutionalise more participatory modes of water governance. To this end, I analyse the implementation of the Directive's active involvement provision in England and Wales. For many decades, water management in England and Wales had a reputation for being a technocratic exercise. In the past 15 years, however, the Environment Agency has made considerable efforts to lay the foundation for enhanced stakeholder participation. Second, with reference to the case of England and Wales, this study contributes to understanding the difficulties that reformers may meet when it comes to building support within an organisation and to implementing reforms towards participatory governance.
