Introduction: In Japan, even a general home visiting nurse (GHVN) lacking psychiatric experience and training can provide home nursing care to patients at risk of suicide. Little attention is given to nursing care for suicide by GHVNs who care for patients with physical illness. However, these patients with suicidal ideation for home care are surprisingly common. Aim/Question: We explored the experiences of GHVNs who cared for patients with suicidal ideation and focused on the nurses' perceptions and interactions. Method: We conducted semi‐structured interviews with 15 GHVNs. We discussed the nurses' experiences caring for patients with suicidal ideation, their perceptions during these interactions and their response. Results: General home visiting nurses were perplexed upon learning of the patient's suicidal ideation. They interacted with such patients by 'avoiding the core of the matter' while focusing on maintaining a relationship with them. Discussion: Not addressing the patient's suicidal thoughts will not only disable the suicide risk assessment but also lead to patient isolation. Suicide prevention care needs to consider the unique characteristics of home care. Implications for Practice: General home visiting nurses require training in suicide risk assessment and communication skills. Accessible Summary: What is known on the subject?: Children and adolescents' mental health problems, such as autism spectrum disorder, anxiety disorder and attention‐deficit hyperactivity disorder, are a global public health concern, and nurses require advanced expertise and skills to properly care for this population.There is a gap between the required competencies and the actual skills and knowledge of CAMHN practitioners. Previous studies suggest that educational interventions for nurses are necessary to enhance the quality of care for children and adolescents with mental health problems. However, the corresponding evaluation indicators have not been verified, making it difficult to determine the most effective methods. What this paper adds to existing knowledge?: The present study developed the two‐factor (direct care for children and their family members and approach to the care environment) Clinical Competency Assessment Scale in Child and Adolescent Mental Health Nursing (CCAS‐CAMHN).We demonstrated that the assessment scale was reliable and valid, based on its adequate internal consistency and temporal stability, the acceptable range of its model‐fit indexes, and its good concurrent and divergent validity. What are the implications for practice?: The newly developed scale is useful for assessing nurses' competency and could help them identify their difficulties in CAMHN.The scale could contribute to the development of effective educational interventions to enhance the quality of care for children and adolescents with mental health problems. Introduction: Caring for children and adolescents with mental health problems, such as autism spectrum disorder, anxiety disorder and attention‐deficit hyperactivity disorder, requires expertise and skills. A quantitative clinical competency measurement tool in child and adolescent mental health nursing (CAMHN) is needed to evaluate the indicators of advanced expertise. Aim: The aim of this study was to develop a clinical competency assessment scale in CAMHN and evaluate its psychometric properties. Method: Scale items were derived from previous studies and adjusted based on cognitive interviews with five CAMHN experts. In total, 505 nurses in CAMHN from 29 hospitals in Japan participated in the self‐administered survey. The scale's construct validity, criterion‐related validity, internal consistency and test–retest reliability were assessed. Results: A two‐factor (direct care for children and their family members and approach to the care environment) scale was constructed. A secondary structural model showed that a two‐factor model fits best. The total score was significantly and positively correlated with excellence in nursing practice, amount of clinical experience and mental status. The overall scale exhibited good validity and reliability. Discussion: The scale is reliable and valid for assessing CAMHN clinical competency. Purpose: Informed by health activism (Zoller, 2005), the purpose of this paper is to explore the communicative processes of organizations working with women migrants in countries of destination. In particular, it explored the definitions of and explanations for health of organizations, their solutions to disease and illness, as well as, the methods and tactics they use to communicate health. Design/methodology/approach: It employed qualitative approach specifically in-depth interviews with leaders or core members of not-for-profit and faith-based organizations working with Filipina migrants in Japan. Field notes from participant observations in formal meetings and informal gatherings were likewise used as data sources. Findings: While organizations also recognized physical and spiritual health, they placed strong emphasis on mental well-being. Other than translation service, pastoral care, and shelter, coordinating with other not-for-profit and faith-based organizations, international centers, and governments was solution for addressing illness and disease. Together with face-to-face, digital media were used as method and tactic to communicate within and outside organizations. It likewise found that the organizations included were inclusive such that they also worked with other Filipinos in Japan. Originality/value: This paper contributed to migration health literature by discussing the central role of organizations for mental well-being activism, favorable consequences of coordination among organizations to promote access to quality healthcare and information and dual characterization of digital media for organizing publics. Objective. On the basis of inheriting the classical aesthetic orientation of Japanese literature, contemporary Japanese literature changes the creative methods of traditional Japanese writers, strives to transcend the national limitations of Japanese literature, and strives to capture literary materials with a "modern feel." Combined with modern western literary techniques to express the "modern self," and integrating popular literature with pure literature, Japanese writers created a "healing" literature that is popular all over the world. There is some value in exploring the correlation between Japanese literature and the influence of psychotherapy. Method. In this paper, an equation diagnosis algorithm based on iterative learning for a class of linear discrete systems is studied, and the existing discrete iterative learning diagnostics are improved. We exchange the order of time domain and iterative domain to achieve the purpose of point-by-point equation diagnosis. On this basis, a virtual influence initial value estimation algorithm with a sliding window is proposed. The impact that has not been diagnosed is estimated and used as the initial value of the impact, so as to reduce the number of iterations and improve the efficiency of equation diagnosis. This paper takes the public and readers as different perspectives, based on the use and satisfaction theory and prospect theory, and uses interviews and questionnaires to study the communication effect of Japanese literature from the three dimensions of cognition, emotion, and behavior. The cognitive part includes the public's understanding and readers' understanding and reading; the emotional part includes the public's and readers' impressions and emotional attitudes; the behavioral part includes the public's reading situation and readers' behavior changes. Results/Discussion. In the survey of loyal readers, although readers' understanding of Japanese literature has improved compared with the general public, it is not high, and their liking and influence on behavior are slightly lower than the preset level. This paper believes that the existing problems of Japanese literature dissemination are mainly concentrated in three aspects: the market is somewhat biased, which narrows the cognitive scope of users; as a cross-cultural literary work, Japanese literature still makes readers feel that it exists; there is some sense of estrangement, which will affect their enjoyment in reading; as a niche interest, readers lack timely communication and sharing objects after reading, so it is difficult to enhance their sense of identity. Objective: To determine impact of undertaking an international educational program during a nurse's or midwife's pre-registration program on subsequent practice, focusing on how nurses and midwives were transformed personally through participation in such programs. Background: Participation in international educational programs has been reported to enhance nursing and midwifery students' personal and professional development, however long-term impacts remain unclear. This paper presents findings drawn from a larger grounded theory study. Study design and Methods: Charmaz's grounded theory methodology was used to elicit experiences from 13 general nurses, two mental health nurses, three midwives and four dual qualified nurse/midwives across eight different countries. Data analysis led to the creation of three categories, with this paper reporting on the category of Developing as a Person. Findings: Participation in international educational programs can be transformative for nurses and midwives with long-lasting impacts, contributing positively to their personal growth and development. Discussion: The study findings underscore significant long-term impacts of international educational programs for nurses and midwives. These outcomes highlight the importance of incorporating international experiences into healthcare education. Conclusion: By providing opportunities for healthcare professionals to engage with diverse settings and populations, organisations and educational institutions can foster the development of well-rounded and globally competent practitioners. Implications for research, policy, and practice: The study's findings hold significant implications for research, policy, and practice in healthcare education. To deepen our understandings, additional longitudinal research across diverse countries is warranted. This paper examines members' practices involved in accounting for discrimination against their own group in interaction. Applying membership categorization analysis to interviews conducted with Chinese international students in Japan, I show that membership categories and category-based knowledge constitute crucial resources for target-of-discrimination group members' making discrimination intelligible and reasonable. Specifically, interviewee accounts for discrimination by bounding certain activities/traits to the membership category being discriminated against. Meanwhile, those activities/traits are displayed as sanctionable given the social relation the discriminating and the discriminated sides are embedded in. Consequently, discrimination becomes reasonable and understandable in its given context. Interviewer, drawing upon common-sense knowledge about social categories, regularly challenges interviewee's account by problematizing the association between the proposed activities/traits and the membership category and by contending the proposed sanctionable nature of those activities/traits. Interviewee can either persist through such challenges by further categorization work or modify their original account of discrimination. As such, target-of-discrimination group members' accounting for discrimination as reasonable and intelligible is collaboratively accomplished through participants' categorization work in interaction. Conceptually anchored on lived religion, this paper explores the meanings and experiences of health, illness, and healing among Filipino migrant women in Japan as they intersect with their religion. Likewise, it explores the functions and limitations of religion as migrant women face physical and mental health problems caused by work, marital status, and/or dislocation. Using biographical interviews and ethnography, this paper suggests that religion serves as a material and symbolic resource for making sense of health, illness and healing. As a material resource, it offered tangible, informational, and emotional support. It can however become limiting when personalised meanings and practices of religion frame illness based on morality, promote health misinformation, and delay healing and other health-seeking behaviours. Nonetheless, healing as perceived and experienced by Filipino migrant women involves lived religion in their complex meaning making and negotiated in terms of its physiological, spiritual and emotional effects. To include people with disabilities as equal citizens, CRPD (Convention on the Rights of Persons with Disabilities) promotes direct or supported decision-making by people with disabilities. However, involuntary psychiatry admission is considered in many countries to be necessary for people with psychosocial disabilities. To overcome the tension and implement CRPD, it is essential to understand the experiences and concerns of service users, family members, and medical professionals in each country. To understand the process and the factors that make psychiatrists decide involuntary psychiatric admission in Japan, and explore their attitudes toward direct or supported decision-making by people with psychosocial disabilities. Psychiatrists who had authorized involuntary admission and who were in charge of the service users were recruited at hospitals in Japan. The interviews were individual, peer to peer, and semi-structured. The interviews were audio-recorded, transcribed verbatim, and the analysis followed reflexive thematic analysis using NVIVO 12. Six psychiatrists (five designated psychiatrists and one psychiatric resident) participated in the study at two hospitals in urban Japan. The study found that the psychiatrists assessed symptoms, behaviors, and perceptions of the service users together with supports and wishes of their families. The psychiatrists decided on involuntary admission when they saw self-harm or violence, weak insights and judgment abilities, family's wishes, or when they wanted to avoid the service users leaving the hospital with incomplete treatment. The psychiatrists felt that the service users would not understand any explanations, which made their communications minimal. The psychiatrists thought it was hard to imagine a system other than the current involuntary admission mechanism. If it was to change, they felt the essential things were to avoid abuse, clarify who is responsible, make plans medically valid and feasible, and assess and plan through everyday life, not just in crisis. During a crisis, the psychiatrists were most careful about complying with the Mental Health Act and responded to the family's wish. The psychiatrists justified involuntary admission as they believed that people in a psychiatric crisis cannot decide or understand and need protection. Related protocols, laws, and expectations from family members shapes the values and practices of psychiatrists in Japan. The paper concludes with several recommendations to regard people with psychosocial disabilities as equal citizens, and promoting the aim of reducing or ending involuntary admission. Support needs for pregnancy-related anxiety among low-risk pregnant women remain unclear. This study aimed to clarify the kinds of support for anxiety that women seek during pregnancy in Japan. Data were collected in a semi-structured focus group interview involving five pregnant women who were not in specific risk groups, recruited from three facilities in Tokyo. We generated themes using inductive thematic analysis. This paper adhered to the consolidated criteria for reporting qualitative research. From the data on support needs for anxiety during pregnancy, three themes were derived: (1) seeking tailored professional support; (2) seeking continuous support within informal relationships; and (3) seeking others' success stories in the same situation. These three types of support gave participants a sense of reassurance or raised concern, depending on the situation. We proposed a model comprising the three derived themes using social cognitive theory. We discussed how these three types of support influenced pregnant women's self-efficacy, which is the core concept of the social cognitive theory. Our findings may help to plan theory-based research and effective interventions to provide support for women's anxiety during pregnancy using a population approach. The construction direction of Japanese teaching evaluation system based on hybrid teaching mode is to give full feedback to teaching process, diversify evaluation subjects and evaluation methods, and obtain comprehensive and objective evaluation results. Compared with most similar studies, more EEG data of teachers' emotional stress relief and healthy people in Japanese teaching process were collected, and a large number of features were extracted. An experiment of teachers' emotional stress relief recognition in Japanese teaching process based on EEG signal was designed. The feature selection algorithm was used to screen the EEG feature combinations of teachers' emotional stress relief and healthy subjects, and the classification experiment was carried out to verify the difference. Finally, this paper uses the feature selection algorithm of tree model and the random forest model classifier to establish the recognition model of teachers' emotional stress discharge in Japanese teaching process based on EEG signals and achieves the effect of more accurate recognition of teachers' emotional stress discharge in Japanese teaching process. First, the connotation, value, and influencing factors of teachers' emotion management ability are logically integrated and systematically expounded. Second, based on emotional psychology, emotional intelligence theory, psychotherapy theory, and management theory, teachers' emotional management ability is constructed according to the three-dimensional structure theory of intelligence and information processing theory. It is constructed from three dimensions: object dimension (teachers themselves, students, and students), content dimension (positive and negative emotions), and operational dimension (emotional perception, emotional understanding, emotional expression, and emotional regulation). Objectives: Hikikomori – an almost complete withdrawal from social interaction first seen in Japan – is becoming an emerging psychological syndrome worldwide. The mental health community in Japan has focused on hikikomori since the 1990s. Hikikomori was initially considered a culture-bound trait, unique to Japan; however, it has become an international concern, and cases have been reported even outside of Japan. While home visiting support for people with hikikomori has gained popularity, an effective solution remains elusive. This paper describes the process involved in effective home visiting support provided by experienced workers to help people with hikikomori. Method: Semi-structured interviews were conducted with 21 home-visiting support workers in Japan. Collected data were analysed using a constant comparative method based on Grounded Theory. Results: 'Supporting them in finding their own way to participate in society' was identified as the core category. This core category was substantiated by following three interrelated stages: preparing the involved surroundings for reaching out to a person, maintaining constant communication and expanding the range of activities and relationships. The process encouraged people with hikikomori to enhance their social connections and improved their motivation for social participation. People with hikikomori experiencing despair and isolation began to find pleasure in social participation through the three stages of support provided by home-visiting workers. Conclusion: Our study suggests that Home visiting support for people with hikikomori can be enhanced by these three interrelated methods which can help individuals integrate into society and connect with others. Aim: This study investigates aspects of the socio‐culturally influenced Japanese primiparas' transition to motherhood. Design: A qualitative descriptive study approach with interviews of Japanese primiparas was used. Methods: Individual interviews with 21 Japanese primiparas were conducted, with a focus on their transition to motherhood and familiarization with the maternal role. The interviews were analysed, and categories and subcategories were established. This paper was developed using the COREQ guidelines for reporting qualitative studies. Results: Seven major categories with 26 subcategories were identified in the transition to motherhood. The seven categories are listed as follows: "confusion with first childcare experience," "suffering related to childcare," "clinging to the image of an ideal mother," "internal conflict while comparing oneself to other mothers," "undertaking childcare by disengaging from one's stereotype," "realization of becoming a mother" and "changing relationship with surrounding people." Hierarchy and power characterize health‐care relationships around the world, constituting a barrier to assertive communication and a risk to patient safety. This issue is more problematic and complex in countries such as Japan, where deep‐seated cultural values related to hierarchy and power persist. The current paper is the second of two that present the findings from a study exploring Japanese nurses' views and experiences of how cultural values impact assertive communication for health‐care professionals. We conducted semistructured interviews with 23 registered nurses, following which data were analyzed using directed content analysis. Two overarching themes emerged from the analysis: hierarchy/power and collectivism. In the present study, we focus on cultural values related to hierarchy and power, including differences in professional status, gender imbalance, seniority/generation gap, bullying, and humility/modesty. The findings from our research provide meaningful insights into how Japanese cultural values influence and constrain nurses' communication and speaking up behaviors, and can be used to inform educational programs designed to teach assertiveness skills. At present, Japanese researchers conducting social surveys encounter several difficulties such as restricted access to official registers and a decrease in current response rates. These researchers may expect these difficulties to be solved by the survey techniques or methods developed outside Japan, such as computer-assisted interviews (CAI) and Internet surveys. It is unclear whether the numerous technical differences between survey methodologies developed in Japan and elsewhere imply the backward-ness of Japan or cultural differences. This paper discusses the potential of various recent survey methods, focusing on the culture and paradigms in Japan and the UnitedStates. First, this paper presents an overview of survey techniques, such as computer-assisted personal interviewing, Internet surveys, and mixed modes. Second, it points out the fact that the existence of the registers and different survey concepts might have created the different survey paradigms. In addition, reference is made to the linguisticdifferences between the US and Japan. The results indicate that survey research is more dispersed in Japan than in the US and that survey research adapted to the uniqueness of Japanese culture is not accumulated sufficiently, although such an accumulation is required. Finally, it is stated that not only surveys, but also the concept of methodology, is not very popular in Japan. Background: Sleep disorders have an enormous impact on occupational health and are counterproductive from an economic perspective. However, the processes of causing sleep disorders from psychosocial aspects have not yet been known. The purpose of this study was to describe how sleep disorders develop among workers with respect to different psychosocial conditions. Methods: A conventional qualitative content analysis was conducted with a semi-structured interview among twenty-seven workers (14 males and 13 females) who were diagnosed with sleep disorders or had a self-reported history of sleep difficulties. Study participants were recruited from a specialized clinic and communities using snowball sampling. This paper adhered to the Standards for Reporting Qualitative Research (SRQR) checklist. Results: The results showed that there were four steps involved in the sleep disorders development process. Firstly, participants with sleep disorders developed 'early warning signs' with 11 categories of triggers; secondly, 'aggravating factors' on top of these early warning signs; thirdly, workers tried to 'cope with' their sleep disorders in the ways they thought would be effective. Finally, when coping failed to improve the quality of sleep, it led to the onset of sleep disorders. Conclusion: The development of sleep disorders and triggers of psychosocial factors were revealed. An occupational health nurse can bring these findings in practice for preventing worker's sleep disorders. [ABSTRACT FROM AUTHOR]

Nurses' beliefs about food and nutrition influence the care and advice they give patients, their families, nurses and others, but these beliefs have not been extensively researched before. This paper presents findings from the qualitative phase of a large q-methodology study that involved both quantitative and qualitative methods. The phase of the study reported here utilized a qualitative descriptive approach regarding a range of beliefs, and in-depth interviews with 240 participants who comprised 30 academics and 30 clinical nurses each from China, Thailand, Japan and Australia. Content analysis was employed to analyse the extracted data regarding their beliefs about nutrition and nutritional supplements, and the sources of these beliefs. Findings and resultant discussion are reported about 17 specific nutritional beliefs. We concluded that many nurses in all the surveyed countries had some false and scientifically unsupported beliefs about nutrition, derived primarily from the media or personal experience. Study findings speak to the need for nurses to critically examine the sources of information they use in their practice and teaching, as well as a need for research to be reported responsibly and accurately. The review of the presented evidence about nutrition will assist nurses in their clinical and teaching practice, and hopefully inspire them to use evidence-based practice in future. Aims and objectives: To clarify beliefs linked with depression among older adults with depression in Japan. Background: As a result of global population ageing, caring for older adults with depression has become an important issue worldwide. In this paper, the concept of "beliefs" pertains to phenomena that lead to distress about disease and can be alleviated through talk therapy. While previous studies focused on illness beliefs in people with mental illness and depression, no studies have yet focused on such beliefs among older adults with depression. Design: Qualitative, narrative‐research‐based method. Methods: Observation and 1–5 narrative interviews lasting 60–90 min were conducted with each of 19 older adults with depression in a Japanese psychiatric ward. The resulting narratives were thematically analysed to derive relevant themes and subthemes. Reporting of this research adheres to COREQ guidelines. Results: Four themes and twelve subthemes were revealed. Depression‐linked beliefs among older adults with depression were (1) "guilt and regret," (2) "pessimism," (3) "futility of treatment" and (4) "desire to be needed by loved ones and society." Conclusion: Depression‐linked beliefs among older adults with depression constituted spiritual pain reflecting character tendencies, including experiences of loss and developmental challenges related to ageing as well as subjects' natural diligence and consideration for others. Relevance to clinical practice: (1) Helping nurses better understand distress experienced by older adults with depression, who are prone to feel guilt and regret when reflecting on the past and to alleviate distress through dialogue and providing positive affirmation for patients. (2) To understand and assuage distress over experiences of loss in old age. (3) To help older adults with depression form a more relaxed attitude towards life, so that even if they have experienced loss as a result of old age, they can maintain their self‐esteem and existential sense of self‐worth. This paper contributes methodological discussions on collecting and analysing international tourists' affects and emotions. The method of data collection used is electronic mail interviews, in some instances corroborated by participant observation in the field, whereas the methods of data analysis employed are affective mapping and emotionality of texts. Such methods of analysis capture the on-flow and contingency of the tourist's experience in post-disaster places. E-mail interviews are becoming an increasingly widespread method of data collection. In post-disaster contexts, the potential of email interviews allows researchers to conduct fieldwork even when they cannot obtain face-to-face interviews, or in cases in which participants feel more at ease answering from their home and can take their time to respond. Twelve semi-structured e-mail interviews have been conducted in 2016 with international tourists who visited the Tohoku region, Japan, hit in 2011 by a triple disaster: an earthquake, a tsunami, and a nuclear meltdown at the Fukushima nuclear power plant. To explore and better understand participants' affective and emotional experiences, we exemplify affective mapping and emotionality of texts drawing on tools like linguistic features of e-mails, imagework, short stories and anecdotes. Aim: To identify insights for supporting people at risk of suicide in the community based on primary healthcare workers' experiences. Design: Qualitative descriptive design. The data were subjected to qualitative descriptive analysis. Results: The analysis revealed eleven subthemes, clustered under the following three themes: (1) fostering 'opportunities for discovery' for people at risk of suicide by encompassing strategies such as increasing access to services and promoting a holistic understanding of the life cycle; (2) fostering 'meaningful connections' by searching for channels to reach the heart and taking approaches from different objectives; and (3) engaging in 'risk management', by understanding and coping with real‐life struggles and implementing timely crisis intervention. Conclusion: Not only direct care targeting suicide prevention but also holistic care that includes atypical perspectives such as 'approaches from different objectives' may provide important insights for supporting people with suicide risk. Implications for Practice: Beyond the limitations of existing suicide risk screening methods, this study highlights the importance of a holistic approach that sees people at risk of suicide in the context of their daily lives and aims to alleviate the difficulties in their lives that have brought them to the point of considering suicide. Impact: By describing support focused on the difficulties that people at risk of suicide face in their daily lives, this study identified insights for supporting people at risk of suicide in communities where screening and intervention are difficult. These findings will contribute to the relief of people at risk of suicide by stimulating suicide prevention measures in primary healthcare settings. Subjective cognitive complaints (SCCs) are a crucial modifiable risk factor for dementia. There is increasing interest in the association between SCC and sleep disturbance; however, the effects of sleep disturbance on SCC development among community-dwelling elderly individuals in Japan remain unclear. We aimed to cross-sectionally investigate the association between SCC and sleep disturbance, with adjustment for multiple factors related to cognitive decline, among 241 community-dwelling elderly persons without cognitive impairment. The measures were SCCs (Kihon Checklist-Cognitive Function, KCL-CF), sleep disturbance (Japanese version of the Athens Insomnia Scale, AIS-J), general cognitive function (Mini-Mental State Examination), and depressive symptoms (five-item version of the Geriatric Depression Scale [GDS-5]). The following data were collected: sex, age, educational history, whether the participants had visited a medical institution for diseases (hypertension, diabetes, hyperlipidemia, heart disease), and the presence/absence of established risk factors (hearing loss, history of head injury, drinking habits, smoking habits, social isolation, and physical inactivity and activity). Based on the KCL-CF, 96 and 145 participants were considered to have and lack SCCs, respectively. On logistic regression analysis, the AIS-J score and smoking history were significantly associated with SCCs. Our findings suggest that sleep disturbance is associated with SCC development among community-dwelling elderly people in Japan. Introduction: With advancements in communication technologies and internet connectivity, avatar robots for children who cannot attend school in person due to illness or disabilities have become more widespread. Introducing these technologies to the classroom aims to offer possibilities of social and educational inclusion. While implementation is still at an experimental level, several of these avatars have already been introduced as a marketable service. However, various obstacles impede widespread acceptance. Methods: In our explorative qualitative case study we conducted semistructured interviews with eight individuals involved in the implementation of the avatar robots AV1 in Germany and eleven participants involved with implementing OriHime in Japan. We analyzed and compared implementation processes, application areas, access and eligibility, and the potential and limitations of avatars at schools. Results: We identified structural similarities and differences in both countries. In the German cases the target is defined as temporary use for children who cannot attend school in person because of childhood illness, with the clear goal of returning to school. Whereas in Japan OriHime is also implemented for children with physical or developmental disabilities, or who cannot attend school in person for other reasons. Discussion: Our study suggests that avatar technologies bear high potential for children to stay socially and educationally connected. Yet, structures need establishing that grant equal access to avatar technologies. These include educational board regulations, budgets for funding avatar technologies and making them accessible to the public, and privacy protection standards that are adequate, yet do not create implementation hurdles that are too high. Furthermore, guidelines or training sessions on technical, educational and psychosocial aspects of including avatar technologies in the classroom for teachers are important for successful implementation. Since our Japanese cases suggest that expanding the area of application beyond childhood illness is promising, further research on the benefits for different groups is needed. [ABSTRACT FROM AUTHOR]

Background: Advance care planning (ACP) is a process in which adults engage in an ongoing dialogue about future medical treatment and care. Though ACP is recommended to improve the quality of end-of-life care, the details of the dialogue experience in ACP are unknown. Objective: To explore participants' experiences of dialogue in an ACP educational program that encouraged them to discuss the value of a way of life. Research design: This qualitative descriptive study used the focus group interview method. Data were analyzed using qualitative content analysis. Participants and research context: A dialogue-based ACP educational program was conducted in four regions in Japan for local citizens to discuss the value of their way of life. A total of 66 individuals (mean age = 55.5 ± 17.2 years; 50 women and 16 men) participated in focus group interviews. Ethical considerations: This study was approved by the Ethical Review Committee of Tokyo Women's Medical University (no. 4723) and Kyoto University (no. R2099). Findings: Five main themes were extracted: discussing one's thoughts with others in a considerate manner, reflecting on one's way of life through others, feeling a sense of connection with others through storytelling, realizing the difficulties of talking about "what if" topics, and turning one's eyes toward the future through the dialogue. These themes were interrelated and illustrated the complexity of the experience of discussing values. Conclusions: The results suggest that dialogue in ACP is useful in clarifying values. They also indicated the need for dealing with the ethical challenges of discussing value and the importance of caring for the interlocutor to have a safe dialogue. In the ACP process, safety in dialogue may improve readiness in ACP, and health practitioners who support ACP need to address the ethical challenges entailing dialogue about values. Social anxiety disorder (SAD) is an excessive fear of social situations that can lead to serious functional impairment. The prevalence of SAD has increased over the past four decades, making it one of the most prevalent psychiatric disorders in Japan. Therefore, more effective interventions are needed to treat this disorder. False safety behavior elimination therapy (F-SET), which eliminates safety behavior (i.e., strategies that reduce anxiety), was administered in our study to six Japanese patients with SAD. This is the first adaptation of F-SET into Japanese. Patients were diagnosed using the Mini-International Neuropsychiatric Interview. The Liebowitz Social Anxiety Scale was used as the primary outcome measure to evaluate the change in symptoms. All patients received five weekly 60-minute F-SET sessions. Participants who completed the treatment exhibited a substantial decrease in SAD symptoms, providing preliminary support for the adapted treatment's efficacy. We have discussed the cultural differences in SAD and the adaptation of F-SET. What is already known about this topic: (1) Transdiagnostic group CBT is an effective treatment for anxiety disorders. (2) Few studies have examined the effectiveness of transdiagnostic group CBT in naturalistic settings. (3) Few studies have examined the effectiveness of treatments as implemented by clinicians in routine clinical practice. What this paper adds: (1) Eight to ten sessions of transdiagnostic group CBT developed by practising clinicians seemed sufficient in improving anxiety and depressive symptoms, daily functioning, and quality of life in patients from an outpatient anxiety disorders clinic in a hospital setting. (2) Patients that completed treatment reported significantly more impairment in daily functioning before treatment than participants who dropped out. Purpose: Though shopkeepers in small enterprises (SSEs) are pioneers in rethinking retirement, working ages and the lifestyles of an aging society, some pertinent health issues remain unexplored. Using narratives of shopkeepers in small, rural enterprises, the purpose of this paper is to investigate the ways of thinking about health maintenance. Design/methodology/approach: This study employed an ethnographic approach. In total, 20 shopkeepers and residents in a rural shopping street in Tohoku, in northeast Japan, were interviewed between June 2016 and May 2017. These interviews were analysed inductively. Findings: The ways shopkeepers think about health maintenance are expressed through an interaction with their personal history. Shopkeepers and customers used narratives to arrive at novel methods to re-think health maintenance. Through data analysis, the cultural theme of maintaining vitality through long-lasting self-struggle supported by cooperation and familiar membership was identified. This cultural theme was related to three sub-themes: struggle stimulated by flexibility and responsibility; cooperation by devoting personal capital; and compensation through family support. Practical implications: There is a growing focus on the important role that SSEs play in rural health. The results of this study will enable rural nurses to approach and develop health via communal interaction. In addition, the support of the family through cooperation at home should be enhanced. Originality/value: There exists the potential for shopkeepers to develop a care strategy for small enterprise-related work. Purpose Though several work-related mental health training initiatives have been implemented in Japan, the effectiveness of such approaches remains unclear. Consequently, some Japanese corporations prefer using interventions such as neuro-linguistic programming (NLP) to improve employee mental health and wellbeing. This language-based development methodology has been the subject of debate in terms of the quality of the underlying empirical evidence. However, a perspective missing from this debate is an evidence-based understanding of the first-hand experiences of employees that have undertaken NLP training. The purpose of this paper is to inform this debate by conducting a rigorous qualitative examination of the experiences of Japanese senior managers who had recently received training in NLP.Design/methodology/approach Semi-structured interviews attended by 11 Japanese NLP master practitioners were analysed using thematic analysis.Findings Four themes emerged from the data set: improving work-related mental health, NLP fosters a better understanding of the mind, NLP helps to reframe perspectives relating to work and mental health, and challenges of NLP training.Originality/value While managers found NLP training skills such as reframing and neuro-logical levels useful to their managerial practice and mental health more generally, they raised concerns about NLP's reputation as well as the utility of some of the techniques employed in NLP. Culture influences the way health‐care professionals communicate with each other and their ability to relate to colleagues in an assertive manner. Cultural barriers can also make it difficult for nurses to speak up even when they have concerns about patient safety. An understanding of the potential impact of cultural factors is therefore needed when developing assertiveness communication training programs. This paper presents the findings from a study that explored Japanese nurses' perceptions of how culture and values impact assertive communication in health care. Semistructured interviews with 23 registered nurses were undertaken, and data were analyzed using directed content analysis. Two major themes were identified: collectivism and hierarchy/power. In the present study, we discuss the cultural values related to collectivism that included four categories of "wa" (harmony), "uchi to soto" (inside and outside), implicit communication/ambiguity, and "nemawashi" (groundwork). The findings highlight the impact of culture on nurses' assertive communication behaviors and can be used to inform the design of culturally‐appropriate assertiveness communication training programs for Japanese nurses working both within their own country or internationally. Davey is a Purpose -- The purpose of this paper is to discuss the implementation and evaluation of a cognitive behavioral therapy (CBT) training course for clinicians in Chiba, the sixth-largest province in Japan. Design/methodology/approach -- Individual CBT for obsessive-compulsive disorder, bulimia nervosa, or social anxiety disorder was delivered by trainees of the Chiba CBT training course in a single study design. Findings -- The results demonstrated that individual CBT delivered by trainees led to statistically significant reductions in symptom severity for all three disorders. Feedback from the trainees indicated that the training course achieved its aims. Research limitations/implications -- Barriers to the dissemination of CBT in Japan such as opportunities for training and possible solutions are discussed. Objectives: In this paper and prospective study, we examine the number of healthy behaviors and the incidence of disability in community-dwelling older adults aged 65 years and older. Methods: Participants (N = 4483) were residents of Obu, Japan who were asked about regular exercise, smoking status, and sleep duration. Demographic variables, history of disease, physical function, and cognitive function were measured as confounders. Information about disability was obtained from the Obu City Office. Results: At 24 months after baseline assessment, 165 participants (3.7%) were certified as having disability. Participants with 2 healthy behaviors had a 1.61-fold increased risk of disability (95% CI: 1.08 -2.42) compared with those with 3 healthy behaviors; those with one or no healthy behaviors had a 2.01-fold risk (95% CI: 1.26-3.19) even though adjusting for confounders. Conclusions: The number of healthy behaviors was associated with the incidence of disability, with the hazard ratios increasing progressively as the number of healthy behaviors decreased. Introduction: Mental health is a serious concern among novice school nurses in Japan. Numerous novice school nurses with various health problems experience high stress levels, affecting their mental health. They may be experiencing reality shock, a known risk factor for burnout and turnover, or its factor, the "reality gap." Objective: This study aimed to examine the hypothetical model by which the reality gap affects the mental health of novice school nurses. Methods: This cross-sectional study conducted a self-administered questionnaire survey, and responses were obtained from 102 novice school nurses. A path model was created based on a hypothetical model. Results: After several revisions on the hypothesis model, a modified path model was generated (CFI =.98, TLI =.93, RMSEA =.06). The path model showed that reality gaps negatively affected work engagement, work engagement positively affected intention to stay, and intention to stay positively affected mental health. Reality gap was also directly negatively associated with mental health. Having various support networks reduced the effects of reality gaps on mental health. Additionally, social support in the workplace boosted work engagement. Conclusion: Unique from other studies, this study contributed to understanding the effects of reality gaps on mental health. School managers and administrators should establish a support system that enables school nurses to receive various supports from within and outside the school. Background: Care of older adults requires comprehensive management and control of systemic diseases, which can be effectively managed by family physicians. Complicated medical conditions in older patients admitted to orthopedic departments (orthopedic patients) necessitate interprofessional collaboration. Nutrition is one of the essential components of management involved in improving the systemic condition of older patients. Nutrition support teams play an important role in nutrition management and can be supported by family physicians. However, the role of family physicians in nutrition support teams is not well documented. This study aimed to investigate the role of family physicians in supporting nutrition management in orthopedic patients. Methods: This qualitative study was conducted between January and June 2023 using constructivist grounded theory methodology. Eight family medicine physicians, three orthopedic surgeons, two nurses, two pharmacists, four rehabilitation therapists, four nutritionists, and one laboratory technician working in Japanese rural hospitals participated in the research. Data collection was performed through ethnography and semi-structured interviews. The analysis was performed iteratively during the study. Results: Using a grounded theory approach, four theories were developed regarding family physicians' role in providing nutrition support to orthopedic patients: hierarchical and relational limitation, delay of onset and detection of the need for geriatric care in orthopedic patients, providing effective family medicine in hospitals, and comprehensive management through the nutrition support team. Conclusions: The inclusion of family physicians in nutrition support teams can help with early detection of the rapid deterioration of orthopedic patients' conditions, and comprehensive management can be provided by nutrition support teams. In rural primary care settings, family physicians play a vital role in providing geriatric care in community hospitals in collaboration with specialists. Introduction: The development of cultural sensitivity is essential for health care professionals but there are few tools to measure this quality in Japan. The purpose of this study was to develop a scale of cultural sensitivity for Japanese health care professionals and examine its reliability and validity. Method: A draft scale was created through conceptual analysis and a questionnaire was completed by 515 health care professionals and 1,322 college students. Exploratory and confirmatory factor analyses were used to determine suitable scale items and examine model fitness. Results: The four-factor 18-item scale showed acceptable model fitness. Cronbach's α coefficient exceeded.90 and correlation coefficients for criterion-related validity were over.29. Construct validity was confirmed by the significantly higher score of the cross-culturally experienced groups. The intraclass correlation coefficient was.642 (professionals) and.722 (students). Discussion: This reliable and valid scale for Japanese health professionals and college students may be used to evaluate training programs to increase their cultural sensitivity. This paper proposes Cognitive Chrono-Ethnography (CCE), a new study methodology for understanding people's in situ behavior selections in daily life. People select their next behavior to maximize their satisfaction for a given behavioral needs. They appropriately coordinate available cognitive resources to make the best decisions by using their knowledge of past experiences and by processing input from the environment and individual intrinsic state. When a study field is specified, CCE starts by defining critical parameters for understanding people's behavior by considering the nature of behavior selection processes in the field in question, and then designing ethnographical field observations by taking into account the fact that their results will be described in terms of the specified critical parameters. The participants' behavior is recorded, followed by a series of structured retrospective interviews for the purpose of describing their present behavior and obtaining their history of behavioral development. Analysis of the interview results aid in developing models of present behavior selections and their chronological changes in the past. These models serve as defining future needs of persons who follow the same developing paths with a certain amount of delay, e.g., a few years of delay. Purpose – Few works have dealt with the impact of a corporation's country of origin on the perception of the qualities and behaviour expected of its leaders. This paper seeks to focus on these aspects with special attention to the specific nature of the Japanese leadership style. The choice of Japan was conditioned neither by the author's admiration for this country, nor by his admiration for their unusual determination to attain perfection, but mainly by the position that Japanese corporations occupy in the global economy. The main objective of the work is to analyze the nature of the vision creation process. Design/methodology/approach – The research project incorporated surveys of and interviews with 2,000 of the world's largest corporations (the Forbes Global 2000 database (2006 version)). Findings – Japanese corporations' leaders more frequently opt for giving their subordinates a free hand in shaping their own visions than leaders of the other researched corporations. However, in nearly all the cases it was mentioned that the proposed assumptions must be coherent with the vision envisaged by top management. Originality/value – The paper attempts to examine the phenomenon of leadership from the perspective of corporate business. This paper examines how Japanese multinational companies manage corporate social responsibility (CSR). It considers how the concept has come to be framed within Japanese business, which is increasingly globalized and internationally focused, yet continues to exhibit strong cultural specificities. The discussion is based on interviews with managers who deal with CSR issues and strategy on a day-to-day basis from 13 multinational companies. In looking at how CSR practice has been adopted and adapted by Japanese corporations, we can begin to see what implications arise from the fact that CSR is a Western-led concept, so opening up critical questions about the future development and evolution of CSR practice within a global context. In being exposed to the concept of CSR as practiced vigilantly in western countries, Japanese multinational company managers have certainly come to re-evaluate aspects of business likely to need rectifying (with potential concerns being gender inequalities, discrepancies in employee conditions, and issues over human rights and supply chains). Japan can be thought to be lagging behind in its understanding and adoption of CSR, in part because corporations do not necessarily state their policies as formally as might be expected. Yet, by analyzing more deeply the kinds of responses gained from CSR managers in Japan (and by placing their remarks within a broader context of Japanese culture and business practices) a far more subtle and revealing picture becomes apparent, not least a more complex picture of the local/global interaction of the frames of reference of corporate responsibility. Background: Domestication is an important and contested concept. Insects are used as food worldwide, and while some have been described as domesticated and even 'semi-domesticated', the assumptions and implications of this designation are not clear. The purpose of this paper is to explore these aspects of insect domestication, and broader debates in domestication studies, through the case of edible wasps in central rural Japan. Methods: Both authors conducted ethnographic fieldwork with communities in central rural Japan. Fieldwork comprised participant observation, semi-structured interviews, quantitative surveys and a review of resources including the personal and public records of wasp collectors. Results: The practice of keeping wasps in hive boxes has historical roots and has changed significantly within living memory. Current attempts to further develop the practice involve collectors' great efforts to keep new queens during their hibernation. Collectors have also tried, still without success, to keep wasps living within a human-made enclosure for their entire life cycle. These and other practices are costly in both time and money for collectors, who emphasise enjoyment as their primary motivation. At the same time, they also engage in practices such as pesticide use that they recognise as damaging to wasp ecology. Conclusions: These practices can be understood to some extent in domesticatory terms, and in terms of care. We develop a framework for understanding domesticatory practices of insect care, discuss how this case contributes to ongoing debates within domestication studies, and recommend further research to be pursued. The stigmatization of individuals with disabilities is a widespread social justice issue. This paper introduces a study of disability, stigmatization and self for children with disabilities and their typically-developing peers. It is the first of two companion papers. It examines the problem of stigmatization from the perspectives of experienced elementary school educators practicing in diverse cultural contexts. We conducted cross-cultural analyses of individual, audio recorded interviews with 26 Japanese, 43 Korean, 16 Taiwanese and 18 US educators, including school social workers. Educators from all four cultural groups described disability and stigma as challenging children's development of self: children with disabilities may experience the self as isolated and inadequate, and typically-developing peers may experience the self as lacking culturally expected values of empathy and respect. Educators' understandings of children's experiences also were culturally nuanced. Educators variously described children with disabilities as experiencing a sense of not belonging [Japan], loss of motivation [South Korea], too much shame [Taiwan], and low self-esteem [US]. They variously described typically-developing children as challenged to show empathy [Japan], include children with disabilities in their peer group [South Korea], develop benevolence [Taiwan], and show respect for individual differences [US]. We contextualize educators' perceptions within their specific sociocultural-historical contexts. Our second paper will focus on solutions; specifically, educators' perspectives on how to minimize stigmatization and support the development of self for children with disabilities and their typically-developing peers. Aim We elucidated patients' challenges immediately after hospital discharge following acute psychiatric inpatient care to clarify how to improve inpatient care and post-discharge follow-ups. Method This study utilized a qualitative descriptive study design and incorporated patient interviews. Participants comprised 18 patients who experienced involuntary admission following a diagnosis of schizophrenia spectrum disorder. Inductive qualitative content analysis was used to create codes and categories from interview transcripts. Findings The core category of post-discharge challenges that emerged was 'separating life as an inpatient from community life'. This comprised two subcategories: 'dissatisfaction with the inpatient care received' and 'lack of abilities to coordinate lifestyle following discharge'. Discussion Patients should be able to disclose their feelings about their experiences in inpatient care settings and the current challenges in community care settings openly. Implications for practice Advocate (e.g. peer staff) should be appointed in acute psychiatric inpatient care settings to reflect patients' feelings and individual needs to transitional care without bias to inpatient and community care providers. INTRODUCTION This study aimed to develop a scale to measure prenatal smoking cessation counseling for Japanese public health nurses (PHNs). METHODS A cross-sectional study was conducted via an anonymous, self-administered questionnaire. The sample included 1933 PHNs working in 424 municipal health centers nationwide, which were randomly selected. We created the draft scale based on semi-structured interviews, previous studies, and preliminary survey. Additionally, we conducted back translation for English version of the draft scale to be applicable in English countries. The analytic strategy consisted of item analysis, exploratory factor analysis, and differentiation by 'known groups'. RESULTS A total of 550 responses (28.5%) were included in the analysis. Most of the respondents were female (98.2%) and the mean age was 37.5±9.37 years. In the exploratory factor analysis, two factors were extracted and the factor loadings for all items were greater than 0.40. The first factor with eleven items was named as 'basic counseling' and the second factor with seven items was named as 'advanced counseling'. The Cronbach's alpha of the scale was 0.918, and the cumulative contribution was 44.908%. Multiple comparisons by experience years working as a PHN revealed significant differences in the scale and two factors. Background: The integration of stakeholders is crucial in developing smart living technologies to support the autonomy of elderly populations. Despite the clear benefits of these technologies, there remains a significant gap in comprehensive research. Methods: This study presents the viewpoints of 19 stakeholders from Europe and Japan, focusing on the sustainability of smart living solutions for Active and Healthy Ageing (AHA). Data were gathered through qualitative semi-structured interviews and analysed using a Framework Analysis approach. Results: Analysis of the interviews revealed six key sustainability categories: addressing the unmet needs of older adults, functionalities of the smart living coach, integration within organizations, identified barriers, financial considerations, and the social role of the smart living coach. Conclusions: This research underscores the importance of evaluating user needs through the involvement of various stakeholders, including the elderly, their caregivers, professionals, technicians, service providers, and government bodies. Background: Although more than half of the habitual smokers recognize that they want to quit smoking cigarettes, approximately half have failed to quit and experienced distress relapse; therefore, there is an urgent need to focus on these populations. When chronic behavior occurs, it is necessary to view the behavior in the context of the entire life of the person involved, considering the history of the person. In this study, we aimed to describe experiences with smoking from the onset to the present and the need for smoking cessation among habitual smokers in Japan and to explore efforts to address them. Methods: Semi-structured interviews that lasted for 55–90 min were conducted with the cooperation of 16 habitual smokers who smoked cigarettes daily. The content of the interviews included demographic characteristics, experiences with smoking from the onset to the present, whether they have attempted to quit and related experiences, and their thoughts on smoking. Interviews were transcribed verbatim and analyzed qualitatively. The Medical Research Ethics Review Committee of Jikei University approved this study (approval number: 33–384(11008)). Results: The participants were aged 26–59 years (mean ± SD: 40.8 ± 8.9 years) and included 10 men and 6 women. The participants started smoking between age 13 and 24 years. The highest number of cigarettes smoked in the participants' lives ranged from 10 to 80 daily, and 12 participants had attempted to quit smoking so far without success. Regarding experiences with smoking from the onset to the present, four themes of "expand one's world," "unconscious attachment," "attempts and failures," and "losing oneself" were extracted. Regarding the need for smoking cessation, four themes of "empowerment from experts," "peer interaction," "social commitment," and "recovery of confidence" were extracted. Conclusion: To support smoking cessation from the perspective of habitual smokers, in addition to improvements through the existing approaches, it is important to recover their confidence using ongoing activities in peer groups according to the target background and support from experts incorporating visual assessments of lung function, along with multiple short-term goals. Aim: To examine how supporters working at after‐school daycare centres, who are involved in the lives of children with profound intellectual and multiple disabilities in the community, pay attention to the sensory characteristics of these children and provide support. Design: A qualitative descriptive design. Methods: Data were collected through semi‐structured interviews with 20 supporters in after‐school daycare centres. Interview transcripts were analysed via qualitative content analysis. Results: The participants' years of involvement in supporting children with profound intellectual and multiple disabilities ranged from 0.5 to 40 years, with an average of 9.8 years. Data were classified into 68 subcategories, 11 categories and three themes: understanding sensory characteristics and devising support, systematic support and challenges supporting the children. Supporters dealt with physical complications and cooperated with other caregivers to understand and respond to children's sensory characteristics. Difficulties dealing with sensory characteristics, challenges due to the supporters' own characteristics and challenges with the facility's infrastructure were identified. The findings could guide sensory characteristics considerations and support systems in after‐school daycare facilities for children with profound intellectual and multiple disabilities. Background and aims: Research on the psychological impact of the coronavirus disease 2019 pandemic has highlighted its negative and positive effects on children with autism spectrum disorder and their families. However, little is known about the neutral effects that remain the same, even in particular circumstances, and how children with autism spectrum disorder and their parents perceive each other. We explored how children with autism spectrum disorder and their mothers perceived and experienced the pandemic in Japan. Methods: A mixed-methods design was employed. Thirteen children with autism spectrum disorder and 12 mothers participated. Data were collected through online semi-structured interviews and analyzed using thematic analysis. Similarities and differences in perceptions were compared. Results: The results revealed six broad themes and 27 categories. Regarding neutral effects, some mothers reported no substantial impact because there were no changes in their jobs or other dramatic life changes. In addition, some children were not affected because they had had no social contact before the pandemic or because their lives had not changed dramatically. Regarding the perceptions of children/mothers, most expressed that they/their children enjoyed spending time with their families. At home, mothers made various efforts to interact with their children. However, mothers and children differed in their perceptions, such as regarding the emergence of anxiety about conducting school events and the resolution of study-related concerns. Conclusions: There were negative, neutral, and positive effects on both children with autism spectrum disorder and their mothers; specifically, they were striving to move forward to overcome the problems posed by the pandemic. Both parties tapped into their resilience by enhancing family interactions, such as cooking together or discussing children's interests. Background: Family members can support advance care planning conversations. However, how family involvement in advance care planning operates to achieve goal-concordant care remains unclear. Aim: To explore how family involvement impacts the process of advance care planning for advanced cancer patients and their family members to achieve goal-concordant care in Japan. Design: Qualitative study incorporating semi-structured in-depth interviews with thematic analysis informed by Family Systems Theory. Setting/participants: Medical oncology departments at two tertiary hospitals in Japan. A purposive sample of 13 advanced cancer patients, 10 family members and 9 healthcare professionals who cared for them. Results: Twenty-five interviews were conducted, comprising 7 dyads of patients and their family members and 18 individual interviews. Four themes were identified: characteristics of patients and family members and their views on illness and advance care planning; family context and communication; interactions with healthcare professionals and societal and cultural influences; and family members' acceptance, preparation and confidence. Family involvement was observed as being variable at an individual level and also across generations. Family members provided patients with the instrumental and emotional support that facilitated the advance care planning process. Family involvement enabled family members to better prepare for realising patients' wishes. It increased family members' confidence in surrogate decision-making. Conclusions: Two mechanisms of how family involvement may enable goal-concordant care were identified: family members' support provision and their preparation for realising patients' wishes. Healthcare professionals should assess family's readiness to engage in advance care planning, and the time required to prepare them for the process. [ABSTRACT FROM AUTHOR]

Background: Dental caries is one of the most common chronic diseases worldwide, affecting lifelong as well as children. Therefore, it is important to clarify factors related to early childhood caries (ECC) in a younger population in terms of caries prevention. However, the prevalence of ECC is low in developed countries in the twenty-first century and a large-scale survey is needed to clarify the risk factors. Furthermore, earlier tooth eruption is not taken into consideration in most studies of ECC, even though it may be a factor of ECC. The present study investigated the prevalence and risk factors of dental caries in children aged 18 months in a core city of Japan. Methods: Findings from a total of 7351 children aged 18 months were analyzed. Anthropometric measurements of height and weight, as well as an oral examination and a microbiological caries-risk test, were performed. Additionally, a structured interview sheet was provided to the parents or guardians. Findings of dental caries at 18 months of age were evaluated using a logistic regression model. Results: Of the enrolled children, 1.2% had experienced dental caries. Multivariable logistic regression analysis results indicated a significant association with dental caries at 18 months of age for the following factors: second child (OR = 1.78; 95% CI:1.08–2.93, P < 0.05), third and later child (OR = 2.08; 95% CI:1.12–3.89, P < 0.05), 12 or fewer erupted teeth (OR = 0.47; 95% CI:0.24–0.96, P < 0.05), 17 or more erupted teeth (OR = 4.37; 95% CI:1.63–11.7, P < 0.01), Cariostat score (+ + +) (OR = 3.99; 95% CI:1.29–12.31, P < 0.05), daily eating before bed (OR = 2.62; 95% CI: 1.55–4.45, P < 0.001), three or more snacks per day (OR = 2.03; 95% CI:1.15–3.58, P < 0.05), and breastfeeding (OR = 3.30; 95% CI:2.00–5.44, P < 0.001). Conclusions: These results suggest that the number of erupted teeth, as well as birth order, eating habits, and breastfeeding, are significant factors in dental caries occurrence at 18 months of age. The 19th century roots of social work in social justice movements within immigrant communities continue to thrive in contemporary social work. Yet relatively little attention has focused on the challenges faced by Asian immigrants, currently the second largest immigrant group in the U.S. Indeed, Asians in the U.S. have long been stereotyped as a "model minority," perpetuating the myth that Asian children do not need special attention when acculturating to U.S. schools. Yet parents report obstacles to their children's acculturation, including racism. As part of a larger ethnography, this study examines how Japanese immigrant and temporary resident parents understand their children's acculturation to the U.S. We conducted in-depth, individual interviews with 14 Japanese immigrant and temporary resident parents of school-aged children. They discussed acculturation challenges centered on differences in the Japanese and U.S. cultural self, and how they modified their socialization practices to support their children's acculturation. Rather than employing Japanese child rearing practices that implicitly guide children by shaping their environment, parents shifted to explicit efforts to ensure their children's development of Japanese cultural selves in the U.S. Such practices, however, may result in children losing a sense of independence and autonomy important to both U.S. and Japanese cultural selves. These experiences of Japanese parents challenge the stereotype of Asians as a model minority. We discuss social work implications for culturally appropriate support for acculturation. Through the lens of Institutional Entrepreneurship, this paper discusses how governments use the levers of power afforded through business and welfare systems to affect change in the organisational management of older workers. It does so using national stakeholder interviews in two contrasting economies: the United Kingdom and Japan. Both governments have taken a ‘light-touch’ approach to work and retirement. However, the highly institutionalised Japanese system affords the government greater leverage than that of the liberal UK system in changing employer practices at the workplace level.
