365. Changes in the diagnosis and management of diabetes in Mexico City between 1998-2004 and 2015-19

Abstract

Objective: To investigate the trends in diabetes prevalence, diagnosis, and management among Mexican adults who were participants in a long-term prospective study. Research design and methods: In 1998-2004, 159,755 adults from Mexico City were recruited into a prospective study and in 2015-2019 10,144 survivors were resurveyed. Diabetes was defined as self-reported diagnosis, glucose-lowering medication use, or HbA1c≥6.5%. Controlled diabetes was defined as HbA1c Results: 99,623 participants in 1998-2004 and 8986 participants in 2015-2019 were aged 45-84 years. Diabetes prevalence increased from 26% in 1998-2004 to 35% by 2015-2019. Of those with diabetes, the proportion previously-diagnosed increased from 76% to 89%, and glucose-lowering medication use among them increased from 80% to 94%. Median HbA1c among all with diabetes decreased from 8.2% to 7.3%, and the proportion of them with controlled diabetes increased from 16% to 37%. Use of blood pressure lowering medication among all with previously-diagnosed diabetes increased from 35% to 51% and their use of lipid-lowering therapy increased from 1% to 14%. The excess mortality risk associated with diabetes accounted for 34% of deaths at ages 35-74 years, of which 5% were attributable to controlled and 29% to inadequately controlled diabetes. Conclusions: Inadequately controlled diabetes is a leading cause of premature adult death in Mexico. Improvements in diabetes management have increased diagnosis and control, but substantial opportunities remain to improve treatment, particularly with lipid-lowering therapy.

Abstract

Parasites threaten human and animal health globally. It is estimated that more than 60% of people on planet Earth carry at least one parasite, many of them several different species. Unfortunately, parasite studies suffer from duplications and inconsistencies between different investigator groups. Hence, groups need to collaborate in an integrated manner in areas including parasite control, improved therapy strategies, diagnostic and surveillance tools, and public awareness. Parasite studies will be better served if there is coordinated management of field data and samples across multidisciplinary approach plans, among academic and non-academic organizations worldwide. In this paper we report the first 'Living organism-World Molecular Network', with the cooperation of 167 parasitologists from 88 countries on all continents. This integrative approach, the 'Sarcoptes-World Molecular Network', seeks to harmonize Sarcoptes epidemiology, diagnosis, treatment, and molecular studies from all over the world, with the aim of decreasing mite infestations in humans and animals. © 2011 International Society for Infectious Diseases.

367. Midterm fate of unifocalized major aortopulmonary collateral arteries in patients with retroesophageal major aortopulmonary collateral arteries.

Abstract

Pulmonary atresia with ventricular septal defect and major aortopulmonary collateral arteries (MAPCAs) is a relatively rare and complex form of congenital heart disease. Unifocalization of MAPCAs has been advocated by some groups for the treatment of this condition. The purpose of this study was to assess the midterm fate of unifocalized MAPCAs in a cohort of patients with retroesophageal MAPCAs. This was a retrospective review of 37 patients who underwent a unifocalization procedure. All patients in this study had 1 or more retroesophageal MAPCAs, and detailed mapping of the MAPCAs was made based on a combination of the cardiac catheterization and surgical findings. The 37 patients had a total of 166 MAPCAs, or 4.5 MAPCAs per patient. One hundred twenty-nine (78%) MAPCAs were unifocalized, whereas 37 (22%) were ligated because they were dual supply. Median follow-up was 69 months. At follow-up cardiac catheterization, evaluation of the 129 unifocalized MAPCAs demonstrated that 123 (95%) had antegrade flow, whereas 6 were occluded. For the 123 MAPCAs with antegrade flow, 97 (80%) were widely patent, whereas 26 were stenotic. Thirteen of the 37 patients have subsequently undergone reintervention on MAPCAs that were determined to be stenotic following unifocalization. Seven of these patients had mild disease and had complete resolution with balloon (n = 5) or surgical revision (n = 2). Six patients with moderate or severe disease underwent surgical revision with confirmed resolution in 4 of 6. The data demonstrate that the majority of unifocalized MAPCAs remain widely patent following unifocalization. However, one-quarter of unifocalized MAPCAs develop stenoses or occlusion. These results suggest the fate for most unifocalized MAPCAs is favorable but highlight the need for close vigilance. Midterm fate of unifocalized MAPCAs. Abstract

Bayesian networks (BNs) are graphical models that can combine knowledge with data to represent the causal probabilistic relationships between a set of variables and provide insight into the processes underlying disease progression, closely resembling clinical decision-making. This paper describes a BN causal model for the early diagnosis and prediction of endometriosis. The causal structure of the BN model is developed using an idioms-based approach and the model parameters are derived from the data reported in multiple medical observational studies and trials. The BN incorporates the impact of errors and omissions in reporting endometriosis, and the distinction between assumed and actual cases. Hence, it is also able to explain both flawed and counterintuitive results of observational studies.

Abstract

HEV is a low-cost, versatile, high-quality ventilator, which has been designed in response to the COVID-19 pandemic. The ventilator is intended to be used both in and out of hospital intensive care units, and for both invasive and non-invasive ventilation. The hard-ware can be complemented with an external turbine for use in regions where compressed air supplies are not reliably available. The standard modes provided include PC–A/C (Pressure Assist Control), PC–A/C–PRVC (Pressure Regulated Volume Control), PC-PSV (Pressure Support Ventilation) and CPAP (Continuous Positive Airway Pressure). HEV is designed to support remote training and post market surveillance via a web interface and data logging to complement the standard touch screen operation, making it suitable for a wide range of geographical deployment. The HEV design places emphasis on the quality of the pressure curves and the reactivity of the trigger,delivering a global performance which will be applicable to ventilator needs beyond the COVID-19 pandemic. This article describes the conceptual design and presents the prototype units together with their performance evaluation.
