Abstract

The foramen spinosum is an easily identifiable landmark in microsurgery of the middle cranial fossa, and knowledge of the variations in its relationship to the surrounding neurovascular structures is important when operating in this area. We studied the anatomical relationship of the foramen spinosum to the foramen ovale, the mandibular branch of the trigeminal nerve, the greater superficial petrosal nerve, and the petrous part of the internal carotid artery in 12 cadaver heads. We also tried to define an external landmark for early identification of the location of the foramen spinosum in ten dry skulls. We found considerable variations in the anatomy around the foramen spinosum. This knowledge may improve the identification and preservation of the neurovascular structures when using approaches to the middle cranial fossa

30. The microsurgical anatomy of the suboccipital vertebral artery and its surrounding structures

Abstract

The suboccipital area contains the anatomic complex of the vertebral artery, its periarterial autonomic neural plexus, its branches, and the adjacent spinal nerves, all of which are cushioned in the venous compartment and covered by 3 layers of muscles. Until more recently, study of the microsurgical anatomy of this area has been neglected. Its thorough understanding provides, however, the foundation for surgical treatment of numerous pathologic entities in this area. Significant advances in understanding of the microsurgical anatomy have resulted in improved outcomes in the treatment of these entities, which now have become a routine. We review the microsurgical anatomy of the suboccipital area with its surgical and certain functional implications.

31. Total petrosectomy approach for lesions of the skull base

Abstract

Technical advances have significantly enhanced the ability of the neurosurgeon to address pathology once considered inoperative. Implementation of various skull-base approaches provides routes of access to areas previously described as inaccessible. The total petrosectomy provides wide exposure to the petrous bone, petroclival region, superior clivus and incisura, middle fossa, and cavernous sinus. A thorough understanding of temporal bone anatomy is essential in this approach. This article provides an overview of relative indications of the total petrosectomy approach, its limitations, postoperative complications, and the description of the approach.

Abstract

Traditionally, the approach to the perimescencephalic and petroclival region has been challenging to the neurosurgeon. Approaches to this region have been associated with multiple complications including excessive temporal lobe and cerebellum retraction, hearing loss, facial nerve injury, and venous infarction. The petrosal approach is intended to provide direct access to the petroclival-perimesencephalic region while minimizing the above neuro-otologic complications. The approach relies on a retrolabyrinthine presigmoid window using a single temporo-suboccipital bone flap combined with a mastoidectomy and tentorial splitting, while preserving the major venous sinuses.

Abstract

The cranio-orbito-zygomatic approach has been in evolution since the frontal approach was first introduced. The addition of both the orbital and zygomatic osteotomies has expanded the limits of neurosurgery to include orbital, craniofacial, and infratemporal pathology. The goal of any skull base approach is to shorten the operative working distance and reduce retraction of the brain while improving exposure. Utilizing the benefits afforded by the cranio-orbito-zygomatic approach requires a thorough understanding of the extradural anatomy of the anterior and middle fossae, including the temporal bone, the craniofacial skeleton, and the cavernous sinus.
