Despite receiving little attention in the scientific literature, earthquakes are known to affect sleep patterns indirectly and can contribute to the development or worsening of sleep disorders. Earthquake-caused disruptions, such as structural damage, displacement, and unstable living conditions, create uncomfortable and anxious sleeping environments. Psychological factors such as fear, stress, and anxiety can contribute to sleep disruption. Physical injuries from earthquakes can disrupt sleep due to pain and discomfort. Additionally, infrastructure disruptions such as power outages and utility damage create uncomfortable or unsafe sleeping conditions. This review presents a comprehensive and systematic overview of the relationship between earthquakes and sleep health.

Amaç: Bu çalışmanın amacı, hastaların polisomnografi (PSG) verileri ile PSG incelemesi sonrası sabah hastaların kendileri tarafından bildirilen verilerin birbiri ile uyumunu değerlendirmektir.Gereç ve Yöntemler: Çalışmaya Düzce Üniversitesi Tıp Fakültesi Göğüs Hastalıkları Uyku Polikliniği’ne başvurmuş ardışık 134 kişi dahil edildi. PSG ve uyku algısı ile ilgili sorular tüm katılımcılara aynı hekim tarafından yüz yüze görüşme ile uygulandı.Bulgular: Çalışmaya dahil edilen hastaların 90’ı (%67,2) erkek ve 44’ü (%32,8) kadın olup yaş ortalaması 47,3±12,6 yıl idi. Obstrüktif uyku apnesi (OUA) negatif olan hastalarda, hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku süresi ile PSG cihazı ile ölçülen uyku süresi arasında anlamlı bir korelasyon varken (p=0,042; r=0,301), OUA (+) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku süresi ile PSG cihazı ile ölçülen uyku süresi arasında korelasyon yoktu (p=0.269, r=0.125). Benzer şekilde, OUA (-) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku latansı ile PSG cihazı ile ölçülen uyku latansı arasında anlamlı bir korelasyon varken (p=0.026, r=0.352), OUA (+) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku latansı ile PSG cihazı ile ölçülen uyku latansı arasında korelasyon yoktu (p=0.060, r=0.223).Sonuç: OUA hastalarını değerlendirirken ve hastaların PSG sonrası ve öncesi tedavilerini açıklarken, hastaların algılarının bozulmuş olabileceği akılda tutulmalıdır. Bu nedenle, hasta ile görüşmelerde daha çok zaman ayırmak ve açıklamalarımızı daha açık ve anlaşılır bir şekilde yapmak gerektiğini düşünmekteyiz.

Objective: In this study, our objective was to evaluate mean platelet volume (MPV), an indirect marker of platelet activation, in patients with obstructive sleep apnea (OSA), and assess the effect of OSA treatment with continuous positive airway pressure (CPAP) on MPV. Materials and Methods: In this study, records of consecutive patients who underwent polysomnographic evaluation for OSA symptoms in the Sleep Disorders Laboratory during a one-year period were reviewed retrospectively. Patients who had both complete blood count and MPV measurements were included in the study. Results: A total of 158 patients, including 51 females (32.3%) and 107 males (67.7%), were included in the study. The mean age of the patients was 51±13 (min-18, max-82) years. OSA was detected in 74.1% (117/158) of the patients. It was determined that as the severity of OSA increased, hemoglobin and hematocrit values increased significantly. There was no significant difference in platelet count according to the presence and severity of OSA. The MPV was significantly lower in severe OSA cases compared to those without OSA and mild OSA cases. A negative correlation was observed between MPV and the apnea-hypopnea index, desaturation index, and the amount of oxygen saturation below 90% during sleep. There was no significant difference in median erythrocyte and thrombocyte counts, hematocrit percentage and hemoglobin values before and after treatment in OSA patients who used CPAP therapy. However, a significant decrease in MPV was observed after OSA treatment compared to pre-treatment. (p=0.021). Conclusions: The results of the study do not support an increase in MPV and hence platelet activation in severe OSA patients compared with those without OSA. However, the results suggest that one month of CPAP treatment reduces MPV and thus platelet activation in severe OSA patients. Objective: Obesity is one of the most important risk factors and also consequences of obstructive sleep apnea (OSA). Weight loss and positive airway pressure therapy are the main approaches in the treatment of OSA. In this study, it was aimed to evaluate the prevalence of OSA in obesity patients scheduled for bariatric surgery. Methods: In the University Hospital Obesity Center, 141 consecutive patients who were candidates for bariatric surgery in 2015 were clinically evaluated preoperatively for sleep-related respiratory disorders. Preoperative polysomnographic examination was recommended to all cases. Results: Of 141 bariatric surgery candidates with a mean age of 37±10 years, and 103 (73%) were female. The mean body mass index of the cases was 46.9±6.4 kg/m2. Among the major symptoms of OSA, snoring was present in 119 (84.4%), daytime sleepiness in 63 (44.7%) and witnessed apnea in 49 (34.8%) patients. OSA was detected because AHI>5/hour was found in 75.7% (84/111) of the patients who accepted the polysomnographic evaluation. 24.3% (27/111) of the cases who underwent polysomnography had non OSA, 29.7% (33/111) mild OSA, 17.1% (19/111) moderate OSA, and 28.8% (32 /111), severe OSA was detected. AHI was positively correlated with age (p=0.003), neck and waist diameter (p<0.001), and negatively correlated with percent of the forced vital capacity (p<0.001). In polysomnographic controls performed an average of 9 months after bariatric surgery, an average of 41.2% improvement in AHI was observed, compared to an average of 22.8% decrease in BMI. Conclusions: OSA prevalence was found to be very high in bariatric surgery candidates. It was thought that polysomnographic examination should be performed before bariatric surgery not only in symptomatic cases but also in all cases. Bariatric surgery can help improve OSA. The aim of the study was to determine the cost of Coronavirus disease-19 (COVID-19) patients who were treated as outpatients and inpatients at Düzce University Health Application and Research Center (DUHARH) Chest Diseases Clinic before and after the pandemic from the perspective of the Social Security Institution (SSI).The study included 26.438 patients who applied to the Chest Diseases clinic in DUHARH before the COVID-19 pandemic (March 10 2019-March 10 2020) and after (March 11 2020-March 11 2021) and 2.971 patients who were hospitalized in the service. A sample was not selected in the research, and the entire universe was included in the study. The data obtained retrospectively were analyzed from bottom to top and through document analysis management. Frequency and percentage calculations, Spearman Correlation analysis, and Mann-Whitney U tests were used to evaluate the data.Before the COVID-19 pandemic, the average unit cost in the policlinic was 46.14 TL/patient ($8.14/patient), and the average unit cost was 64.69 TL/ patient ($9.23/patient) after the COVID-19 pandemic. The average cost of the pre-COVID-19 pandemic service was calculated as 1.139,64 TL/patient ($200/patient). After the COVID-19 pandemic, the average unit cost in the service was 2.136,27 TL/patient ($304.75/patient). A statistically significant difference in terms of costs was found between the two periods. It was determined that the costs of COVID-19 patients changed in terms of length of stay, age, and sex (p0.05).Even though the number of patients in the Chest Diseases clinic has decreased during the pandemic process, the costs have increased due to the high cost of COVID-19 patients and the patients needing advanced examination and treatment in this period. For this reason, patients need to apply to the relevant unit early.

16. The Role of Lung Cancer Symptom Screening in Smoking Cessation

- Abstract
Amaç: Bu çalışmada Düzce ilinde 30 yaş ve üzeri sigara içenlerde akciğer kanseri semptom taramasının sigara bırakmadaki rolü araştırıldı. Gereç ve Yöntemler: Randomize 700 hane seçilerek 30 yaş üzeri sigara anamnezi olan 500 olgu çalışmaya alındı. Olguların sigara alışkanlıkları ve akciğer kanseri semptomları bir anket formuyla yüz yüze görüşmeyle sorgulandı. Sigaranın akciğer kanseri için önemli bir risk faktörü olduğu vurgulandı. Halen aktif sigara içen 474 olgunun 353'üne 6 ay sonra telefonla sigara alışkanlıkları yeniden sorgulandı. Bulgular: Toplam 353 sigara içen olgunun 32’si (%9,1) kadın, 321’i (%90,9) erkekti. Yaş ortalaması 46±9 olan olguların, ortalama 36±24 paket-yıl sigara anamnezi vardı. 130'u (%36,8) sigarayı bırakmayı düşünüyordu ve 65'i (%18,4) sigarayı bırakmayı denemişti. 6 ay sonraki kontrolde 17 olgu (%4,8) sigarayı bırakmış, 59 olgu (%16) sigarayı azaltmış, 2 olgu (%0,6) ise artırmıştı. Sigarayı bırakanların hiçbiri ilk görüşmede bırakmayı düşündüğünü söyleyenler veya daha önce bırakmayı deneyenlerden değildi. Sigarayı bırakanların yaş ortalaması sigara içmeye devam edenlere göre daha yüksekti (p=0,044) ve sigarayı bırakanların daha uzun süredir sigara içme öyküleri vardı (p=0,001). Hemoptizi ve çomak parmak semptomlarının varlığı sigara bırakma oranlarını anlamlı düzeyde artırmaktaydı (sırasıyla p=0,048, p=0,013). Bu çalışmada akciğer kanseri semptom sorgulamasından 6 ay sonraki sigara bırakma oranı (%4,8), kendiliğinden sigarayı bırakma oranından (%1) anlamlı düzeyde yüksekti (p=0,001). Sonuç: Akciğer kanseri semptom taramasıyla sigara bırakma oranları artırılabilir. Sigara bırakma poliklinikleri yanında bu tip yaklaşımlar sigara mücadelesine katkıda bulunabilir.

Öğretmenlerin örgütsel destek, psikolojik sermaye ve iş doyumu algı düzeylerinin ortaya konması; değişkenlerin birbiri ile olan ilişkilerinin yönü ve düzeyinin belirlenmesi ve psikolojik sermayenin algılanan örgütsel destek ile iş doyumu arasındaki aracılık rolünün incelenmesi bu araştırmanın amaçlarını teşkil etmektedir. Giresun ilinde görev yapan 1668 lise öğretmeni araştırma evrenini teşkil ederken hesaplamalar neticesinde örneklem sayısı 475 öğretmen olarak belirlenmiştir. İlişkisel tarama modeli şeklinde desenlenmiş olan bu araştırmada nicel veri toplama teknikleri kullanılmıştır. Veriler üç farklı ölçek vasıtasıyla bir araya getirilmiştir. Ölçek puanlarına ait aritmetik ortalama neticesinde katılımcıların algılanan örgütsel destek, psikolojik sermaye ve iş doyumu düzeylerinin yüksek olduğu görülmektedir. Korelasyon analizi sonucuna göre öğretmenlerin örgütsel destek algı düzeyi ile psikolojik sermaye düzeyi arasında pozitif yönde düşük seviyede; örgütsel destek algı düzeyi ile iş doyumu düzeyi arasında pozitif yönde orta seviyede; psikolojik sermaye düzeyi ile iş doyumu düzeyi arasında pozitif yönde orta seviyede bir ilişki bulunmaktadır. Bununla birlikte yapılan regresyon analizi de psikolojik sermayenin algılanan örgütsel destek ve iş doyumu ilişkisinde kısmi aracı etkiye sahip olduğunu göstermektedir.

Aim: Although coronavirus disease 2019 (COVID-19) is a complex disease threatening global public health and leading to a pandemic, the contributions of the pro-oxidant/oxidant imbalance involved in the pathogenesis of the disease have not been adequately studied. Therefore, the aim of this study is to evaluate the effects of smoking on oxidative stress and antioxidant defense system biomarkers in COVID-19 patients.Material and Methods: Participants were divided into 4 groups in the study: (Control group) 45 healthy individuals without a diagnosis of COVID-19 and not smoking, (Group 2) 45 individuals without a diagnosis of COVID-19 and smoking, (Group 3) 45 patients with a diagnosis of COVID-19 and not smoking, (Group 4) 45 patients with a diagnosis of COVID-19 and smoking 45 patients. D-dimer, nitric oxide (NO), glutathione reductase (GR), superoxide dismutase (SOD), catalase (CAT), malondialdehyde (MDA), total antioxidant levels (TAS) and total oxidant levels (TOS) in the serum of the participants were analyzed using commercial kits according to the ELISA method.Results: There is a significant increase in D-dimer levels in people diagnosed with COVID-19 (p, Amaç: Koronavirüs hastalığı 2019 (COVID-19), küresel halk sağlığını tehdit eden ve bir pandemiye yol açan karmaşık bir hastalık olmasına rağmen, hastalığın patogenezinde yer alan pro-oksidan/oksidan dengesizliğinin katkıları yeterince araştırılmamıştır. Obstructive sleep apnea syndrome (OSAS) is characterized by repetitive episodes of upper airway obstruction which occurs during sleep and usually associated with a reduction in oxygen saturation, and it is one of the common health problems with significant mortality and morbidity. It is very well-known that OSAS alters the serum levels of several hormones by the effect of hypoxia on central neurotransmitters that causes changes in the hypothalamic-pituitary-adrenal axis (HPA) in relation with sleep fragmentation. Growth hormone secretion (GH) is affected by several factors such as age, sex, obesity, body composition and sleep quality. Also, the oxygen content of the inhaled air affects the secretion of GH. On the other hand, intermittent hypoxemia increases GH secretion. In this study, we aimed to examine the serum level of GH in patients with OSAS.

Introduction: Fatigue is considered a frequent and characteristic feature of sarcoidosis. This study was designed to determine the prevalence of fatigue in patients with sarcoidosis and to determine its potential clinical correlations in relation to symptom severity. Methods: A total of 56 sarcoidosis patients were included. Data on patient demographics, anthropometrics, disease characteristics, pulmonary function tests, 6-min walking distance (6MWD), blood biochemistry and hemogram findings were retrieved from hospital records. Psychometric instruments involved fatigue assessment scale (FAS), Beck Depression Inventory (BDI) and Short Form-36 (SF-36) for health-related QOL (HRQOL). The medium- and long-term effects of COVID-19 infection on pulmonary function are still unknown. The present study aimed to investigate the pulmonary functions in healthcare professionals who had persistent complaints after contracting COVID-19 and returning to work.The study included COVID-19-infected healthcare professionals from the Düzce University Medical Faculty Hospital who volunteered to participate. Medical histories, medical records, pulmonary function tests, the diffusing capacity of the lungs for carbon monoxide (DLCO) test, and the 6-minute walk test (6MWT) were used to collect data from all participants.The study included 53 healthcare professionals, with an average age of 38 ± 10 years (min: 24 years and max: 71 years), including 29 female (54.7%) and 24 male (45.3%) participants. Of the participants, 22.6% were smokers, 35.8% (19 individuals) had comorbidities, and 17% (9 individuals) were hospitalized. The mean length of stay was 9 ± 4 days (mean ± standard deviation). The most prevalent symptoms were weakness (88.7%), muscle aches (67.9%), inability to smell/taste (60.4%), headache (54.7%), fever (45.3%), cough (41.5%), and shortness of breath (37.7%). The mean time to return to work after a positive polymerase chain reaction (PCR) test for COVID-19 was 18 ± 13 days. The average time among post-disease pulmonary function, 6MW, and DLCO tests was 89 ± 36 days (min: 15 and max: 205). The DLCO level decreased in 39.6% (21) of the participants. Female participants had a significantly higher rate of decreased DLCO levels than male participants (25% vs. 55.2%, P = .026). DLCO levels were significantly higher in participants with long-term persistent complaints (P = .043). The later the time to return to work, the lower the DLCO value (r = -0.290 and P = .035). The 6MWT distance was positively correlated with hemoglobin and lymphocyte levels at the time of the disease onset and negatively correlated with D-dimer levels. The most prevalent symptoms during the control visits were shortness of breath/effort dyspnea (24.6%), weakness (9.5%), and muscle aches (7.6%).Significant persistent complaints (47.2%) and low DLCO levels (39.6%) were observed in healthcare professionals during control visits at a mean time of 3 months after the COVID-19 infection. Symptoms and spirometry measurements, including DLCO, may be helpful in the follow-up of healthcare professionals who contracted COVID-19. Further comprehensive studies with long-term follow-up periods are required.

Introduction: The repetitive hypoxemia can induce oxidative stress mechanisms. Oxidative stress increases serum ADMA levels in patients with OSAS.Aim: To investigate the relationship between serum levels of ADMA and OSAS severity.Methods: 330 patient’s records, who underwent overnight polysomnography (PSG) were retrospectively examined. The patients who had a abnormal serum glucose, lipid, thyroid, renal and liver function tests levels, had excluded. 72 patients had participating criteria. Patients were categorized into five groups according to apnea/hipopnea index (AHI). Measurement of ADMA levels was performed by ELISA kit. Results: The mean age was 44,19 ± 14,24 years. 31,9 % of the patients were female. Patients in normal group, severe, medium, mild and positional–REM dependent OSAS group were 26,4%, 25%, 6,9 %, 16,7% and 25%; respectively. Objective: The aim of this present study was to investigate the eating attitudes ofsarcoidosis patients.Methods: 50 patients with sarcoidosis and 45 healthy individuals without chronicdisease were included to study. All participants evaluated for metabolic syndrome(MetS) according to National Cholesterol Education Program's Adult Treatment PanelIII (NCEP-ATP III) criteria. Eating Attitude Test (EAT) and Beck DepressionInventory (BDI) were applied to all participants. All participants evaluated formetabolic syndrome (MetS). OBJECTIVE: Differential diagnosis of idiopathic pulmonary fibrosis (IPF) is important among fibrotic interstitial lung diseases (ILD). This study aimed to evaluate the rate of IPF in patients with fibrotic ILD and to determine the clinical-laboratory features of patients with and without IPF that would provide the differential diagnosis of IPF. MATERIAL AND METHODS: The study included the patients with the usual interstitial pneumonia (UIP) pattern or possible UIP pattern on thorax high-resolution computed tomography, and/or UIP pattern, probable UIP or possible UIP pattern at lung biopsy according to the 2011 ATS/ERS/JRS/ALAT guidelines. Demographics and clinical and radiological data of the patients were recorded. All data recorded by researchers was evaluated by radiology and the clinical decision board. RESULTS: A total of 336 patients (253 men, 83 women, age 65.8±9.0 years) were evaluated. Of the patients with sufficient data for diagnosis (n=300), the diagnosis was IPF in 121 (40.3%), unclassified idiopathic interstitial pneumonia in 50 (16.7%), combined pulmonary fibrosis and emphysema (CPFE) in 40 (13.3%), and lung involvement of connective tissue disease (CTD) in 16 (5.3%). When 29 patients with definite IPF features were added to the patients with CPFE, the total number of IPF patients reached 150 (50%). Öğretmenlerin örgütsel destek, psikolojik sermaye ve iş doyumu algı düzeylerinin ortaya konması; değişkenlerin birbiri ile olan ilişkilerinin yönü ve düzeyinin belirlenmesi ve psikolojik sermayenin algılanan örgütsel destek ile iş doyumu arasındaki aracılık rolünün incelenmesi bu araştırmanın amaçlarını teşkil etmektedir. Giresun ilinde görev yapan 1668 lise öğretmeni araştırma evrenini teşkil ederken hesaplamalar neticesinde örneklem sayısı 475 öğretmen olarak belirlenmiştir. İlişkisel tarama modeli şeklinde desenlenmiş olan bu araştırmada nicel veri toplama teknikleri kullanılmıştır. Veriler üç farklı ölçek vasıtasıyla bir araya getirilmiştir. Ölçek puanlarına ait aritmetik ortalama neticesinde katılımcıların algılanan örgütsel destek, psikolojik sermaye ve iş doyumu düzeylerinin yüksek olduğu görülmektedir. Korelasyon analizi sonucuna göre öğretmenlerin örgütsel destek algı düzeyi ile psikolojik sermaye düzeyi arasında pozitif yönde düşük seviyede; örgütsel destek algı düzeyi ile iş doyumu düzeyi arasında pozitif yönde orta seviyede; psikolojik sermaye düzeyi ile iş doyumu düzeyi arasında pozitif yönde orta seviyede bir ilişki bulunmaktadır. Bununla birlikte yapılan regresyon analizi de psikolojik sermayenin algılanan örgütsel destek ve iş doyumu ilişkisinde kısmi aracı etkiye sahip olduğunu göstermektedir., The aims of the current research are to reveal teachers' perception levels of organizational support, psychological capital, and job satisfaction, determine the direction and level of the correlation between the variables, and examine the mediating role of psychological capital between perceived organizational support and job satisfaction. While 1668 high school teachers working in Giresun province constituted the study population, the sample size was found to be 475 teachers as a result of the calculations. Quantitative data collection techniques were employed in the present research, designed as a relational survey design. The data were brought together using three different scales. The arithmetic mean of the scale scores demonstrates that the participants' perceived organizational support, psychological capital, and job satisfaction levels are high. In accordance with the correlation analysis results, there is a low positive correlation between the teachers' perceived organizational support level and the psychological capital level, there is a moderate positive correlation between the perceived organizational support level and job satisfaction, and there is a moderate positive correlation between the psychological capital level and job satisfaction level. Furthermore, the regression analysis shows that psychological capital has a partial mediating effect on the correlation between perceived organizational support and job satisfaction

As the chest diseases clinic of DüzceMedical School, we were one of the departments that took an active role in the COVID-19 pandemic. Chest diseases department; It was one of the main branches in the differential diagnosis, treatment and follow-up of COVID-19 cases. In this process, our assistants, who were our hands and arms, were our fighters who were at the forefront of the pandemic process. During the pandemic process, we worked every day with an order consisting of a faculty member and two assistants. We continued our scientific meetingand assistant trainings online so that our training activities are not interrupted. On the one hand, we continued our scientific activities by participating in the studies, planning new research, and writing projects. As a result, when we look back, we have completed this difficult process by further enhancing our sense of solidarity that exists as a department in the pandemic process we experienced for the first time in our lives., Düzce Tıp Fakültesi göğüs hastalıkları kliniği olarak COVID-19 pandemisinde aktif rol alan bölümlerden biriydik. Göğüs hastalıkları bölümü; COVID-19 olgularının ayırıcı tanı, tedavi ve takibinde yer alan ana branşlardandı. Bu süreçte elimiz, kolumuz olan asistanlarımız pandemi sürecinin en ön cephesinde yer alan savaşçılarımızdı. Pandemi sürecinde her gün bir öğretim üyesi ve iki asistandan oluşan bir düzenle çalıştık. Eğitim faaliyetlerimizin de aksamaması adına bilimsel toplantı ve asistan eğitimlerimize online olarak devam ettik. Bir taraftan da bilimsel aktivitelerimize merkeze çalışmalara katılarak, yeni araştırmalar planlayarak, projeler yazarak devam ettik. Amaç: Bu çalışmanın amacı, hastaların polisomnografi (PSG) verileri ile PSG incelemesi sonrası sabah hastaların kendileri tarafından bildirilen verilerin birbiri ile uyumunu değerlendirmektir.Gereç ve Yöntemler: Çalışmaya Düzce Üniversitesi Tıp Fakültesi Göğüs Hastalıkları Uyku Polikliniği’ne başvurmuş ardışık 134 kişi dahil edildi. PSG ve uyku algısı ile ilgili sorular tüm katılımcılara aynı hekim tarafından yüz yüze görüşme ile uygulandı.Bulgular: Çalışmaya dahil edilen hastaların 90’ı (%67,2) erkek ve 44’ü (%32,8) kadın olup yaş ortalaması 47,3±12,6 yıl idi. Obstrüktif uyku apnesi (OUA) negatif olan hastalarda, hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku süresi ile PSG cihazı ile ölçülen uyku süresi arasında anlamlı bir korelasyon varken (p=0,042; r=0,301), OUA (+) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku süresi ile PSG cihazı ile ölçülen uyku süresi arasında korelasyon yoktu (p=0.269, r=0.125). Benzer şekilde, OUA (-) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku latansı ile PSG cihazı ile ölçülen uyku latansı arasında anlamlı bir korelasyon varken (p=0.026, r=0.352), OUA (+) olan hastalarda hastaların sabah uyandıktan sonra kendileri tarafından bildirilen uyku latansı ile PSG cihazı ile ölçülen uyku latansı arasında korelasyon yoktu (p=0.060, r=0.223).Sonuç: OUA hastalarını değerlendirirken ve hastaların PSG sonrası ve öncesi tedavilerini açıklarken, hastaların algılarının bozulmuş olabileceği akılda tutulmalıdır. Bu nedenle, hasta ile görüşmelerde daha çok zaman ayırmak ve açıklamalarımızı daha açık ve anlaşılır bir şekilde yapmak gerektiğini düşünmekteyiz.

Aim: In this study, the role of lung cancer symptom screening in smokers aged 30 and over in Duzce city was investigated.Materials and Methods: 500 cases over 30 years of age with a smoking history randomly selected from 700 households were included in the study. The smoking habits and symptoms of lung cancer cases were evaluated by a questionnaire with a face to face manner. It was emphasized that smoking is an important risk factor for lung cancer. Smoking habits were re-questioned by phone in 353 of 474 active smokers 6 months later.Results: Of the 353 smokers, 32 (9.1%) were female and 321 (90.9%) were male. It consisted of cases with a mean age of 46±9 and an average of 36±24 pack/year smoking history. 130 cases (%36.8) were intended to smoking cessation and 65 (%18,4) had tried smoking cessation. In the 6 months follow-up 17 (%4.8) of them had given up smoking, 59 (%16) of them had decreased, and 2 (%0,6) of them had increased it. The cases who had given up smoking were not the ones who had an intention to smoking cessation and had tried to quit at the first interview. Those who quit smoking had a higher mean age (p=0.044) and had a longer smoking history (p=0.001). The presence of hemoptysis and clubbing symptoms significantly increased smoking cessation rates (p=0.048, p=0.013, respectively). In this study, the quit rate after 6 months of lung cancer symptom questioning (4.8%) was significantly higher than the spontaneous smoking cessation rate (1%) (p=0.001).Conclusion: Smoking cessation rates can be increased by screening for lung cancer symptoms. Along with smoking cessation outpatient clinics, such approaches can contribute to the fight against smoking., Amaç: Bu çalışmada Düzce ilinde 30 yaş ve üzeri sigara içenlerde akciğer kanseri semptom taramasının sigara bırakmadaki rolü araştırıldı.Gereç ve Yöntemler: Randomize 700 hane seçilerek 30 yaş üzeri sigara anamnezi olan 500 olgu çalışmaya alındı. Olguların sigara alışkanlıkları ve akciğer kanseri semptomları bir anket formuyla yüz yüze görüşmeyle sorgulandı. Sigaranın akciğer kanseri için önemli bir risk faktörü olduğu vurgulandı. Halen aktif sigara içen 474 olgunun 353'üne 6 ay sonra telefonla sigara alışkanlıkları yeniden sorgulandı.Bulgular: Toplam 353 sigara içen olgunun 32’si (%9,1) kadın, 321’i (%90,9) erkekti. Yaş ortalaması 46±9 olan olguların, ortalama 36±24 paket-yıl sigara anamnezi vardı. 130'u (%36,8) sigarayı bırakmayı düşünüyordu ve 65'i (%18,4) sigarayı bırakmayı denemişti. 6 ay sonraki kontrolde 17 olgu (%4,8) sigarayı bırakmış, 59 olgu (%16) sigarayı azaltmış, 2 olgu (%0,6) ise artırmıştı. Sigarayı bırakanların yaş ortalaması sigara içmeye devam edenlere göre daha yüksekti (p=0,044) ve sigarayı bırakanların daha uzun süredir sigara içme öyküleri vardı (p=0,001). Hemoptizi ve çomak parmak semptomlarının varlığı sigara bırakma oranlarını anlamlı düzeyde artırmaktaydı (sırasıyla p=0,048, p=0,013). Bu çalışmada akciğer kanseri semptom sorgulamasından 6 ay sonraki sigara bırakma oranı (%4,8), kendiliğinden sigarayı bırakma oranından (%1) anlamlı düzeyde yüksekti (p=0,001).Sonuç: Akciğer kanseri semptom taramasıyla sigara bırakma oranları artırılabilir. Sigara bırakma poliklinikleri yanında bu tip yaklaşımlar sigara mücadelesine katkıda bulunabilir.

This study aimed to evaluate the frequency of obstructive sleep apnea (OSA) in patients with sarcoidosis and related clinical factors.Consecutive patients diagnosed with sarcoidosis in our clinic were evaluated for OSA risk during sleep using the Epworth Sleepiness Scale, Stanford Sleepiness Scale, Pittsburgh Sleep Quality Index, Berlin questionnaire, STOP and STOP-BANG questionnaires, and polysomnography (PSG).A total of 60 sarcoidosis patients (mean age: 50 ± 11 years, 45 (75%) women) were included in the study. Polysomnography was performed in 54 cases and revealed the diagnosis of OSA in 70% (38/54) of the patients. The mean age was higher in patients with sarcoidosis and OSA (54 ± 11 vs. 47 ± 13, p = 0.041) and body mass index values were significantly higher as well (31.9 ± 4.4 vs, 29.0 ± 4.6 kg/mHigh rates of OSA were detected in sarcoidosis, increasing with the advanced disease stage. The findings suggest that patients with sarcoidosis and advanced age, obesity, steroid treatment, and involvement of lung parenchyma (stages 2 and 3) should be evaluated for OSA risk. Further investigations are needed to establish the potential causes of the high prevalence of OSA in sarcoidosis.

- Abstract
Objective: differential diagnosis of idiopathic pulmonary fibrosis (IPF) is important among fibrotic interstitial lung diseases (ILD). This study aimed to evaluate the rate of IPF in patients with fibrotic ILD and to determine the clinical-laboratory features of patients with and without IPF that would provide the differential diagnosis of IPF. Material and methods: the study included the patients with the usual interstitial pneumonia (UIP) pattern or possible UIP pattern on thorax high-resolution computed tomography, and/or UIP pattern, probable UIP or possible UIP pattern at lung biopsy according to the 2011 ATS/ERSARS/ALAT guidelines. Demographics and clinical and radiological data of the patients were recorded. All data recorded by researchers was evaluated by radiology and the clinical decision board. Results: a total of 336 patients (253 men, 83 women, age 65.8 +/- 9.0 years) were evaluated. Of the patients with sufficient data for diag-nosis (n=300), the diagnosis was IPF in 121 (40.3%), unclassified idiopathic interstitial pneumonia in 50 (16.7%), combined pulmonary fibrosis and emphysema (CPFE) in 40 (13.3%), and lung involvement of connective tissue disease (CTD) in 16 (5.3%). When 29 patients with definite IPF features were added to the patients with CPFE, the total number of IPF patients reached 150 (50%). Rate of male sex (p<0.001), smoking history (p<0.001), and the presence of clubbing (p=0.001) were significantly high in patients with IPE None of the women <50 years and none of the men <50 years of age without a smoking history were diagnosed with IPE Presence of at least 1 of the symptoms suggestive of CTD, erythrocyte sedimentation rate (ESR), and antinuclear antibody (FANA) positivity rates were significantly higher in the non-IPF group (p<0.001, p=0.029, p=0.009, respectively). Aim: To investigate the incidence of bronchiectasis supposed to be made by the external compression of hiatal hernia (HH) to bronchi. Materials and Methods: The thorax computed tomography (CT) scans of patients which were carried out in Duzce University Hospital between February 2014 and August 2015 were retrospectively evaluated. The repeated scans in the same patient were excluded. Results: A total of 4388 patients were included in the study. A total of 98 HH cases were detected of which 58 (59.2%) were female. The mean age was 73.30 +/- 9.14 (45-90). The rate of HH according to small, moderate and large size was 45 (45.9%), 9 (9.2%), 44 (44.9%), respectively. The rate of hiatal hernia accompanied by bronchiectasis were similar in both males and females (P = 0.078). The prevalence of bronchiectasis was significantly high in large hernias with 81.4% rate (P = 0.009). Bronchiectasis rate was 12.343 times (OR: 12.343, 95% CI: 1.479-103.027, P = 0.009) higher in the large HH group compared to small and moderate HH groups. Hiatal hernia accompanied by bronchiectasis was 88.1% anatomically near to HH. Conclusions: Thus, hiatal hernia may cause bronchiectasis due to external compression rather than lymphadenopathy or the tumor as an etiology of bronchiectasis and should be considered in the differential diagnosis. As the chest diseases clinic of Duzce Medical School, we were one of the departments that took an active role in the COVID-19 pandemic. Chest diseases department; It was one of the main branches in the differential diagnosis, treatment and follow-up of COVID-19 cases. In this process, our assistants, who were our hands and arms, were our fighters who were at the forefront of the pandemic process. During the pandemic process, we worked every day with an order consisting of a faculty member and two assistants. We continued our scientific meeting and assistant trainings online so that our training activities are not interrupted. On the one hand, we continued our scientific activities by participating in the studies, planning new research, and writing projects. As a result, when we look back, we have completed this difficult process by further enhancing our sense of solidarity that exists as a department in the pandemic process we experienced for the first time in our lives. Aim. Previous studies have shown a higher sensitization rate to hazelnut in processing workers but no relation was found between the respiratory symptoms in workplace and hazelnut sensitization. Material and Method. To evaluate the association between the hazelnut sensitization and workplace-related respiratory complaints, hazelnut processing workers had undergone a questionnaire included work-related respiratory symptoms, smoking history, pulmonary function testing, and measurement of serum IgE antibodies against hazelnut. Results. This study consisted of 88 hazelnut processing workers (79 females and 9 males), aged 14–59 years (Mean ± SD: years). The mean working duration was months (min: 1–max: 180). Specific IgE against hazelnut allergens was positive in 14 of cases (17.1%). There was no significant difference between the cases with and without specific IgE against hazelnut allergens regarding respiratory symptoms, history of allergy, smoking status and spirometric values. Conclusion. 17.1% of the hazelnut processing workers were seropositive against hazelnut. Being sensitized to hazelnut was not found to be associated with work-related respiratory symptoms in this study. Further studies are needed in hazelnut workers respiratory health to search topics other than asthma.
