Purpose: The benefits of laparoscopic approaches to treat colorectal cancer (CRC) and colorectal liver metastases (CRLM) separately are well established. However, there is no consensus about the optimal timing to approach the primary tumor and CRLM, whether simultaneously or staged. The objective of this review with practical reports is to discuss technical aspects required for patient selection to perform simultaneous laparoscopic approaches for CRC and CRLM. Methods: Literature review of oncological factors associated with patient selection for surgical treatment of CRLM and the use of laparoscopy in those cases, and report of technical aspects for simultaneous CRC and CRLM approaches. Results: Simultaneous laparoscopic resection has been successful in many series of selected patients, although it seems to be safer to perform minor and major liver resection with non-extended colorectal resections, and to avoid two high-risk procedures at the same time. Conclusions: Simultaneous CRC and CRLM resections seem to be safe when patients are carefully selected, also considering the risk of recurrence concerning oncologic outcomes. The pre-planning of simultaneous resection is mandatory to plan trocar positioning, procedure sequencing, and patient position.

In recent years the availability of high quality water is becoming a constraint in several countries. Agriculture represents the main world water user therefore, wastewater reuse in this area could increase the water availability for other needs. However conventional approach for wastewater treatment and reuse requires large and complex plants which include tertiary disinfection processes (i.e. NaClO, UV radiation). The aim of this research was to provide a compact scheme for treatment and reuse of municipal wastewater based on Sequencing Batch Biofilter Granular Reactors (SBBGR). Particular attention was dedicated to microbiological quality of water monitoring a wide group of indicators (Escherichia coli, Salmonella, Clostridium perfringens, Somatic coliphages, Giardia lamblia and Cryptosporidium parvum). The possibility of SBBGR enhancement with sand filtration was also evaluated. The SBBGR removed more than 90% of suspended solids and chemical oxygen demand (COD), and about 80% and 50% of total nitrogen and phosphorous respectively. SBBGR resulted effective also towards microbial indicators removing from 1 up to 4 log units of these microorganisms. The addition of sand filtration increased the disinfection efficiency of the system obtaining an additional removal of 1-2 log units. In conclusion, the biological treatment by SBBGR produced an effluent with properties already compatible with its agricultural reuse according to the WHO's quality criteria. Furthermore, combining SBBGR and sand filtration the plant effluent could even comply more restrictive reuse criteria.

Tra le tecnologie di recente introduzione impiegate per il trattamento di acque di rifiuto sia municipali che industriali, quelle che destano maggiore interesse sono basate sull'utilizzo di biomasse granulari aerobiche. In tali sistemi, la biomassa cresce sotto forma di granuli microbici densi e compatti che garantiscono, rispetto ai sistemi tradizionali, elevate concentrazioni di microrganismi in reattori a basse volumetrie e bassa produzione di fanghi di supero. In particolare, un innovativo sistema a granulazione aerobica di tipo discontinuo sequenziale definito SBBGR (Sequencing Batch Biofilter Granular Reactor), nel quale la biomassa si presenta come biofilm adeso sui corpi di riempimento e come aggregati microbici compatti (granuli), è attualmente oggetto di studio presso l'IRSA. Questo tipo di processo permette di trattare elevate portate di liquame in un volume molto ridotto ed è caratterizzato da concentrazioni di biomassa elevate (fino a 150 gTSS/Lbiomassa), ridotta produzione di fanghi di supero ed elevata flessibilità di esercizio. Essendo questa tecnologia molto giovane, molti aspetti legati alle cause ed ai meccanismi della granulazione aerobica necessitano ancora di approfondimenti. A tale scopo, il processo di granulazione aerobica è stato studiato su biomasse sviluppate in sistemi in scala di laboratorio alimentati con substrato sintetico. Unitamente al monitoraggio dei principali parametri di processo, la formazione degli aggregati microbici (sia biofilm che granuli) è stata analizzata mediante FISH (Fluorescence In Situ Hybridation), metodologia che permette l'identificazione e la quantificazione in situ dei microrganismi. L'analisi è stata effettuata su campioni di biofilm e su sezioni di granuli dello spessore di 15-20 ?m (ottenute mediante inclusione degli stessi in una resina sintetica, congelamento e sezionamento tramite criomicrotomo) allo scopo di valutare la struttura di tali aggregati. In combinazione con la FISH, sono state utilizzate tecniche tradizionali quali la colorazione Nilo Blue per valutare l'importanza dello stoccaggio del substrato carbonioso in tali sistemi e per verificare la presenza e funzione dell' EPS (polimeri extracellulari) la cui presenza si suppone sia importante per conferire ai granuli il loro grado di densità e compattezza.
