1. Clinical, microbiological, and molecular characterization of pediatric invasive infections by Streptococcus pyogenes in Spain in a context of global outbreak

Abstract

ABSTRACTIn December 2022, an alert was published in the UK and other European countries reporting an unusual increase in the incidence of Streptococcus pyogenes infections. Our aim was to describe the clinical, microbiological, and molecular characteristics of group A Streptococcus invasive infections (iGAS) in children prospectively recruited in Spain (September 2022–March 2023), and compare invasive strains with strains causing mild infections. One hundred thirty isolates of S. pyogenes causing infection (102 iGAS and 28 mild infections) were included in the microbiological study: emm typing, antimicrobial susceptibility testing, and sequencing for core genome multilocus sequence typing (cgMLST), resistome, and virulome analysis. Clinical data were available from 93 cases and 21 controls. Pneumonia was the most frequent clinical syndrome (41/93; 44.1%), followed by deep tissue abscesses (23/93; 24.7%), and osteoarticular infections (11/93; 11.8%). Forty-six of 93 cases (49.5%) required admission to the pediatric intensive care unit. iGAS isolates mainly belonged to emm1 and emm12; emm12 predominated in 2022 but was surpassed by emm1 in 2023. Spread of M1UK sublineage (28/64 M1 isolates) was communicated for the first time in Spain, but it did not replace the still predominant sublineage M1global (36/64). Furthermore, a difference in emm types compared with the mild cases was observed with predominance of emm1, but also important representativeness of emm12 and emm89 isolates. Pneumonia, the most frequent and severe iGAS diagnosed, was associated with the speA gene, while the ssa superantigen was associated with milder cases. iGAS isolates were mainly susceptible to antimicrobials. cgMLST showed five major clusters: ST28-ST1357/emm1, ST36-ST425/emm12, ST242/emm12.37, ST39/emm4, and ST101-ST1295/emm89 isolates.IMPORTANCEGroup A Streptococcus (GAS) is a common bacterial pathogen in the pediatric population. In the last months of 2022, an unusual increase in GAS infections was detected in various countries. Certain strains were overrepresented, although the cause of this raise is not clear. In Spain, a significant increase in mild and severe cases was also observed; this study evaluates the clinical characteristics and the strains involved in both scenarios. Our study showed that the increase in incidence did not correlate with an increase in resistance or with an emm types shift. However, there seemed to be a rise in severity, partly related to a greater rate of pneumonia cases. These findings suggest a general increase in iGAS that highlights the need for surveillance. The introduction of whole genome sequencing in the diagnosis and surveillance of iGAS may improve the understanding of antibiotic resistance, virulence, and clones, facilitating its control and personalized treatment.

Abstract

Pleural drainage was differently performed in two similar neighboring hospitals (32.0 % vs. 58.2 %, p < 0.001), but the length of stay was shorter in the hospital using a more conservative approach (median 12 days vs. 18 days, p < 0.001). This result seemed unrelated to severity but associated with the shorter duration of intravenous treatment. This study adds to previous reports suggesting that pleural drainage is unnecessary in many cases; controlled studies are needed to determine which patients may actually benefit from its use.

Abstract

We conducted a multicenter clinical validity study of the Panbio coronavirus disease 2019 Antigen Rapid Test of nasopharyngeal samples in pediatric patients with coronavirus disease 2019-compatible symptoms of ≤5 days of evolution. Our study showed limited accuracy in nasopharyngeal antigen testing: overall sensitivity was 45.4%, and 99.8% of specificity, positive-predictive value was 92.5%.

Abstract

To evaluate the nutritional, clinical and immunological status of children at HIV diagnosis in the continental region of Equatorial Guinea.Children18 years diagnosed with HIV between 2009 and 2017 were included. Clinical, immunological and nutritional data were collected. Weight-for-height, weight-for-age and height-for-age Z-scores were calculated using WHO Child Growth Standards. The population was assessed in two equal periods (2009-2013 and 2014-2017) from the time of diagnosis.A total of 213 children were diagnosed with HIV (49.3% males), median age 3.8 years (IQR: 1.5-8.2). A total of 121 cases (56.8%) were at WHO clinical stage III, and 53 (24.9%) were at WHO clinical stage IV. CDC immunological stage II was diagnosed in 51/130 (39.2%) and CDC stage III in 44/130 (33.8%). About 56.2% of children were underweight (weight-for-age-2 SD); 20.1% moderately and 36.1% severely so. About 27.6% of children were wasted (weight-for-height-2 SD); 11.9% moderately and 15.7% severely so. About 56.3% of children were stunted (height-for-age-2 SD); 20.7% moderately and 35.6% severely so. The prevalence of wasting was higher in children ≤5 years than in children5 years (36.4% vs. 19.1%, P = 0.026). In the second period, the prevalence of moderate-severe immunodeficiency decreased (87.2% to 67.0%, P = 0.018), without significant differences in the other nutritional or clinical data. Severe underweight was a risk factor for moderate-severe immunodeficiency (aOR: 4 [95% CI: 1.4-11.4], P = 0.010).We highlight a high proportion of malnutrition at the time of HIV diagnosis in Guinea´s paediatric population. Early diagnosis of HIV infection is a priority, achievable by training Guinea´s physicians to suspect HIV early, introducing HIV molecular diagnostic techniques and ensuring intensive nutritional treatment.Evaluer le statut nutritionnel, clinique et immunologique des enfants au moment du diagnostic du VIH dans la région continentale de la Guinée équatoriale. MÉTHODES: Les enfants de moins de 18 ans diagnostiqués avec le VIH entre 2009 et 2017 ont été inclus. Les données cliniques, immunologiques et nutritionnelles ont été collectées. Les scores Z du poids pour la taille, du poids pour l'âge et de la taille pour l'âge ont été calculés en utilisant les normes de croissance de l'enfant de l'OMS. La population a été évaluée en deux périodes égales (2009-2013 et 2014-2017) à partir du moment du diagnostic. RÉSULTATS: 213 enfants ont été diagnostiqués avec le VIH (49,3% de sexe masculin), âge médian de 3,8 ans (IQR: 1,5-8,2). 121 cas (56,8%) étaient au stade clinique III de l’OMS et 53 (24,9%) étaient au stade clinique IV de l’OMS. Le stade immunologique II du CDC a été diagnostiqué chez 51/130 (39,2%) et le stade III du CDC chez 44/130 (33,8%). 56,2% des enfants avaient un poids insuffisant (poids pour âge-2 écart type [ET]); 20,1% modérément et 36,1% sévèrement. 27,6% des enfants étaient émaciés (poids pour taille-2 ET); 11,9% modérément et 15,7% sévèrement. 56,3% des enfants avaient un retard de croissance (taille pour l'âge-2 ET); 20,7% modérément et 35,6% sévèrement. La prévalence de l'émaciation était plus élevée chez les enfants ≤5 ans que chez les enfants5 ans (36,4% contre 19,1%; P = 0,026). Dans la deuxième période, la prévalence de l'immunodéficience modérée à sévère a diminué (87,2% à 67,0% ; P = 0,018), sans différences significatives dans les autres données nutritionnelles ou cliniques. L’insuffisance pondérale sévère était un facteur de risque d'immunodéficience modérée à sévère (aOR: 4 [IC95%: 1,4-11,4] ; P = 0,010).Nous mettons en évidence ici une forte proportion de malnutrition au moment du diagnostic du VIH dans la population pédiatrique guinéenne. Le diagnostic précoce de l'infection par le VIH est une priorité, réalisable en formant les médecins guinéens à identifier le VIH tôt, en introduisant des techniques de diagnostic moléculaire du VIH et en assurant un traitement nutritionnel intensif.

7. The Association of HIV-1 Neutralization in Aviremic Children and Adults with Time to ART Initiation and CD4+/CD8+ Ratios

Abstract

Broadly neutralizing antibodies (bnAbs) bind and neutralize diverse HIV isolates and demonstrate protective effects in primate models and humans against specific isolates. To develop an effective HIV vaccine, it is widely believed that inducing these antibodies is crucial. However, the high somatic hypermutation in bnAbs and the limited affinity of HIV Env proteins for bnAb germline precursors suggest that extended antigen exposure is necessary for their production. Consequently, HIV vaccine research is exploring complex sequential vaccination strategies to guide the immune response through maturation stages. In this context, the exploration of the factors linked to the generation of these antibodies across diverse age groups becomes critical. In this study, we assessed the anti-HIV-1 neutralization potency and breadth in 108 aviremic adults and 109 aviremic children under 15 years of age who were receiving ART. We used a previously described minipanel of recombinant viruses and investigated the factors associated with neutralization in these individuals. We identified individuals in both groups who were capable of neutralizing viruses from three different subtypes, with greater cross-neutralization observed in the adult group (49.0% vs. 9.2%). In both groups, we observed an inverse association between neutralization breadth and the CD4+/CD8+ ratio, as well as a direct association with the time to ART initiation. However, we found no association with time post-infection, cumulative ART duration, or CD8+ cell levels. The present study demonstrates that children receiving antiretroviral therapy generate broadly neutralizing responses to HIV-1, albeit with lower magnitude compared to adults. We also observed that neutralization breadth is associated with CD4+/CD8+ levels and time to treatment initiation in both children and adults living with HIV-1. Our interpretation of these results is that a delay in ART initiation could have prolonged the antigenic stimulation associated with viral replication and thus facilitate the capacity to elicit long-lasting broadly neutralizing responses. These results corroborate prior findings that show that HIV-1-neutralizing responses can persist for years, even at low antigen levels, implying an HIV-1 vaccine may induce lasting neutralizing antibody response.
