Abstract

Aims: Multidrug-resistant bacteria urinary tract infections (MDR-UTI) are a global concern, leading to increased morbidity, mortality and financial costs. The aim of this study was to determine the prevalence of MDR-UTI and establish the epidemiological profile of UTI due to different phenotypes of MDR-UTI. Methodology and results: This is a retrospective study of MDR-UTI assessed at the microbiology laboratory of Mohammed VI University Hospital Center of Oujda over 37 months from March 2016 to April 2019. The study included all requests for cytobacteriological examination of urine (CBEU) according to the instructions of the medical microbiology guidelines. An agar medium was utilized for bacterial culture experimentation. Antibiotic susceptibility testing was performed according to the European Committee on Antimicrobial Susceptibility Testing guidelines. CBEU in UTIs allowed the isolation of 1078 isolates. They were dominated by Gram-negative bacteria (87.65%; n=945), followed by Gram-positive bacteria for 12.34% (n=133). This included 181 (16.80%) multidrug-resistant strains, which were dominated by extended-spectrum beta-lactamase-producing Enterobacteriaceae (ESBL-E) (n=144; 79.55%). Young male adults exhibited a higher occurrence of isolated ESBL-E and carbapenem-resistant Acinetobacter baumannii (CRAB). As for the distribution by department, there was a near equality between the MDR isolates isolated from outpatients (n= 88; 49%) versus the inpatients (n= 93; 51%). Exactly 152 (84%) isolated MDR cases were observed in patients who did not have a urinary catheter, 157 (87%) and 141 (78%) cases of MDR of UTI were not associated with care and occurred in non-immunocompromised patients, respectively. Prior antibiotic exposure was observed in only 58 (32%) cases. Conclusion, significance and impact of study: This study confirmed the increasing prevalence of multidrug-resistant urinary tract infections in the community and hospitals. Abstract

The objectives of our work were to establish the epidemiological and bacteriological profile of female urinary tract infection at the Mohammed VI University Hospital of Oujda (Morocco), and then to study the drug resistance of the bacterial strains isolated. This is a retrospective study over 36 months including urine samples from patients hospitalized or consulting at the CHU Mohamed VI of Oujda (Morocco). Urines were processed according to the recommendations of the Medical Microbiology Reference (REMIC) and the EUCAST (European Committee on Antimicrobial Susceptibility Testing). We collected 12556 requests for CBEU from different departments. At the top of the list was the emergency department with a rate of 37% (n= 4666) followed by outpatient clinics (33.1%; n=4226). 5% (n=630) of the CBEU were positive. Escherichia coli (E. coli ) dominated the epidemiological profile with a rate of 72.50% (n=482). E. coli was resistant to penicillins in 69.50% (n=299) of cases, protected penicillins in 34.80% (n=149), third generation cephalosporins (C3G) in 9% (n=38), fluoroquinolones in 17.5% (n=73), Trimethoprim-Sulfamethoxazole in 46% (n=196) of cases and gentamicin in 12% (n=51) of cases. None of the strains were resistant to carbapenems. Awareness-raising on the proper use of antibiotics, issuing national recommendations for the treatment of urinary tract infections in order to standardize therapeutic regimens. Effective control of these infections requires a global prevention strategy that implies close collaboration between epidemiologists, clinicians, bacteriologists, hygienists and the health care team.

Abstract

Les infections à streptococcus pneumoniae sont le plus souvent respiratoires, ORL et méningées. Les infections ostéoarticulaires surviennent dans moins de 20% des cas chez l'adulte sain. Un ou plusieurs facteurs favorisants sont habituellement retrouvés. Nous rapportons une observation d’arthrite septique à pneumocoque du coude chez un adulte sans facteurs prédisposant. Il s’agit d’une patiente de 57 ans admise, fin octobre 2018, pour une tuméfaction douloureuse du coude droit évoluant depuis 10 jours sans facteurs déclenchant avec une rougeur et une chaleur locale. La patiente a bénéficié d’une échographie qui était en faveur d’un hygroma olécranien probablement surinfecté sans anomalies de structures ostéo-tendineuses. Un examen clinique plus étendu a été réalisé à la recherche d’autres foyers infectieux n’a rien décelé. La ponction articulaire, effectuée avant toute antibiothérapie, a révélé un liquide très purulent avec un taux de leucocytes à 900000 par mm3 dont 95% des PNN. La coloration de Gram d’une goutte de ce liquide a mis en évidence des cocci à Gram positif en diplocoque. Le bilan inflammatoire a révélé une protéine C réactive significativement élevé (CRP= 50mg/l) et une vitesse de sédimentation à 140 à la première heure. D’autres examens biologiques ont été réalisés, dont l’étude des sérologies virales étaient négatifs ce qui a exclu une immunodépression. Abstract

Background and Purpose: The presence of yeasts in the urine is not synonymous with urinary tract infection since it can result in simple colonization or contamination. Regarding this, it is required to further clarify the epidemiological profile of funguria. Accordingly, the present study was conducted to establish the epidemiology of funguria in the Mohammed VI Teaching Hospital of Oujda, Morocco. Materials and Methods: This retrospective study was conducted on all urine samples sent for cytobacteriological examination to a microbiology laboratory over a period of 28 months (i.e., from March 2016 to June 2018). After the removal of duplicates, the urine samples were treated according to the recommendations of the medical microbiology standards. Results: A total of 15,165 urine samples were collected. Urinary colonization accounted for 4.94% (n=749) of cases. The infections of the urinary tract accounted for 5.35% (n=811) of cases. Microbial isolates (n=1,669) in colonization and urinary tract infections were dominated by bacteria (93.47%, n=1,560). Furthermore, the yeasts accounted for 6.53% (n=109) of the isolates. Candida albicans was isolated from 56.88% (n=62) of funguria cases. The risk factors for funguria in our series were essentially old age, admission to intensive care unit, and broad-spectrum antibiotic therapy. Conclusion: The current level of knowledge about the clinical situations leading to funguria with the improvement and popularization of efficient identification techniques for yeasts other than C. albicans should redress the epidemiology of funguria. This should allow the knowledgeable societies to establish the rules of interpreting the cytobacteriological examination of the urine in case of funguria, as for bacteriuria.
