SARS-CoV-2, the etiological agent of COVID-19, caused a pandemic in 2020, which is only recently slowing down. The symptoms of COVID-19 range from cough to fever and pneumonia and may persist beyond the active state of the infection, in a condition called post-COVID syndrome. The aim of this paper is to review the relationship between COVID-19 and nutrition and to discuss to most up-to-date dietary supplements proposed for COVID-19 treatment and prevention. Nutrition and nutritional dysregulations, such as obesity and malnutrition, are prominent risk factors for severe COVID-19. These factors exert anti-inflammatory and proinflammatory effects on the immune system, thus exacerbating or reducing the immunological response against the virus. As for the nutritional habits, the Western diet induces a chronic inflammatory state, whereas the Mediterranean diet exerts anti-inflammatory effects and has been proposed for ameliorating COVID-19 evolution and symptoms. Several vaccines have been researched and commercialized for COVID-19 prevention, whereas several drugs, although clinically tested, have not shown promising effects. To compensate for the lack of treatment, several supplements have been recommended for preventing or ameliorating COVID-19 symptoms. Thus, it is critical to review the dietary supplements proposed for COVID-19 treatment. Supplements containing α-cyclodextrin and hydroxytyrosol exhibited promising effects in several clinical trials and reduced the severity of the outcomes and the duration of the infection. The aim of this study is to present a repository (Italian Recording for the Appropriate Management of Odontoiatric Patient, IRAMOP) for the registration of dental services performed on patients referred to five Italian universities, in relation to orthodontics, surgical dentistry, dental implantology, periodontal and endodontic specialties. MATERIALS AND METHODS. Individual clinical programmed pathways are hereby described, indicating procedures, protocols and associated pharmacotherapy. All operators following the recent clinical recommendations for dentistry, published by the Ministry of Health, will be able to record on a shared platform the data related to the provided services and the clinical response obtained in each individual treatment. Drug therapies, clinical and surgical procedures and used protocols will be recorded into a common database. RESULTS AND CONCLUSIONS. All collected and processed data will provide important insights into the correct approach to dentistry, including both therapeutic procedures and drug therapies. Post treatment assessments will be recorded by operators not only in relation to the clinical evaluation, but also according to patient's reports in terms of perceived pain measured by the Numeric Rating Scale (NRS). The aim of this study is to evaluate whether a transverse maxillary deficit can cause an alteration of vertebral development and therefore of the skeletal maturation comparing the cervical maturation stages index with the hand-wrist index.For the study were selected 200 patients aged 7-14 years, equally distributed by gender and divided into 100 study subjects with maxillary deficit and 100 controls without maxillary deficit. The skeletal maturation index (SM according to Fishman) was evaluated and compared with the hand-wrist x-rays and the cervical vertebrae maturation (CVM according to Hassel and Farmann).Forty-one per cent of the subjects in the test group show a discrepancy between CVM and SM. Among these 73% (30 subjects) present an advanced stage of CVM compared with the corresponding SM. Only 16% of the subjects in the control group show a discrepancy between CVM and SM. Among these 69% (11 subjects) appear in an advanced CVM stage.The analysis of the CVM stage in subjects with transverse maxillary deficit appears to be altered compared with the SM identified through a hand-wrist x-ray. In the case of individuals with transverse maxillary deficit it is advisable to use also a hand-wrist x-ray, thus not relying only on CVM for the evaluation of the skeletal growth stages.

Recent studies have reported nuclear delocalization of plakoglobin in acantholytic pemphigus vulgaris cells. The objective of this study was to evaluate the role of plakoglobin in the pathogenesis of acantholysis in pemphigus vulgaris (PV) and its relation with the urokinase-type plasminogen activator receptor (uPAR) expression.Plakoglobin and uPAR expressions were evaluated by immunohistochemistry in 22 cases of PV at various stages of the disease, and as controls in 18 specimens of skin/oral mucosa from healthy patients.Healthy skin/normal oral mucosa showed strong plakoglobin expression in the basal and spinous layers with prevalent cellular membrane distribution; the intensity of staining progressively decreased toward the superficial layers of the epithelium. In PV patients, a progressive displacement of the plakoglobin signal toward the nucleus was found in 18/22 of the cases. Healthy skin/normal oral mucosa showed low uPAR expression with prevalent cellular membrane distribution. In the PV patients, strong uPAR expression was present in the acantholytic cells in 16/22 of the cases. There was direct correlation (p < 0.05) between the uPAR expression and nuclear plakoglobin.The uPAR overexpression in acantholytic PV may be considered a direct consequence of plakoglobin abnormal distribution. Nuclear delocalization of plakoglobin, a direct consequence of plakoglobin-Dsg-3 dissociation induced by PV IgG, probably induces uPAR overexpression. This evidence suggests a central role for plakoglobin in PV pathogenesis because of its delocalization toward the nucleus, which is the probable cause of the uPAR gene expression.

Abnormal frenula may require treatment by frenulectomy. Several techniques are available, but using a laser for this purpose represents an innovative technology that may provide more efficient, more comfortable and more predictable treatment outcomes for both patient and surgeon. On this respect, Nd-YAP laser treatment is very useful allowing for excellent clinical outcomes with low morbidity. This case series reports on the use of Nd-YAP laser for a labial frenulectomy. Twenty-three patients were treated and afterward controlled. Laser treatment, above all Nd-YAP, appears to be the gold standard technique.

Juvenile Idiopathic Arthritis is a disease that can involve the temporomandibular joint unilaterally or bilaterally causing specific malocclusions.This paper reports a case of Juvenile Idiopathic Arthritis in an 8-year-old male with unilateral involvement of the temporomandibular joint, with remarkable asymmetry of the left mandible and reduced left facial height. The management of the temporomandibular damage consisted in the treatment with an activator in order to help mandibular ramus growth with a subsequent enhancement in occlusion, masticatory function and profile. This treatment has several advantages and permits to correct the typical malocclusion exhibited by these patients, to improve the clinical signs of the disease and to obtain stability over time.

Numerous in vitro studies have shown that composite materials, commonly used for restorations in conservative dentistry, and in orthodontics to anchor brackets to the tooth enamel, have cytotoxic and genotoxic effects. The study determined expression of p53, p63 and p16, biomarkers useful for predicting potential genotoxicity.p53, p63 and p16 expression was determined immunohistochemically in the gingival papillae of 99 patients (69 banded orthodontically for at least one year, brackets bonded to teeth with filled flowable composite resin, 30 without orthodontic banding as controls). The papillae samples were removed surgically and examined to evaluate morphological and biological alterations.In no case were morphological alterations visible by microscopy out of the 69 banded patients; four (5.80%) were positive for p53 and two for p63 expression in the basal and suprabasal layers (2.90%). One patient was positive for p16 (1.45%). No control case was positive for any of the biomarkers (0.00%).The significance of p53, p63 and p16 positivity, and whether these proteins may serve as biomarkers to predict the risk of developing oral lesions (dysplasia, oral cancer) is still unclear. Although details of the mechanisms leading to cell death, genotoxicity and cell-cycle delay are not fully understood, resin monomers may alter cell function in the oral cavity.

In this article the individual patient (IP) appliance is described. It consists of 250 options of bracket and band variations as the straight wire appliances. Increasing the bracket capabilities means using an increasing number of brackets, each with a specific design created for a treatment situation. The objective of IP appliance is to eliminate wire bending from orthodontic treatment and improve the treatment results. To manage this technique, a computer software is needed. Internet offers significant possibilities in managing each patient by patient basis. The clinician is required to make the diagnosis and treatment plan before ordering the appliance. Two clinical cases are described with the aim to present the advantages of this technique.
