Introduction Neoadjuvant chemotherapy (NAC) is the standard treatment for locally advanced breast cancer. However, some tumors will not respond to this treatment due to histological and molecular features. The protein EZH2 (enhancer of zest homolog 2) is a histone methyltransferase that is correlated with poorly differentiated breast carcinomas and aggressive tumor behavior. Purpose The present study evaluated the association between EZH2 expression and response to NAC, and its correlation with HER2 overexpression, estrogen and progesterone receptors (ER, PR) and Ki-67 proliferation index. Methods A total of 60 patients with locally advanced breast cancer treated with NAC were selected for this study. Twenty-three paraffin blocks had not enough material for tissue resection, and were not evaluated. A tissue microarray based in immunohistochemistry (IHC) analysis of EZH2 was performed for the remaining 37 specimens. Patients were divided into two groups based on response to NAC. Results EZH2 expression was significantly associated with markers of poor prognosis such as ER negativity (p = 0.001), PR negativity (p = 0.042) and high Ki-67 proliferation index (p = 0.002). High EZH2 expression was not correlated with the response to NAC. Conclusions Our data suggested that EZH2 protein expression may not correlate with the clinical response to NAC. Other studies with more patients are needed to confirm this observation.

The coumadin-induced skin necrosis is rare and occurs more frequently in the breasts, thighs and buttocks. We describe the first case of coumadin necrosis of the breast in Brazil in a 62-year-old patient.

OBJECTIVE: Conventional suturing methods of microvascular anastomosis are associated with various degrees of vascular wall damage that can lead to thrombosis and bypass occlusion. An experimental model of double end-to-side venous graft anastomosis on the common carotid artery was set up to compare conventional suturing methods with a low-power diode laser vascular welding technique. METHODS: The experiments were performed on 40 rabbits that underwent implantation of a 15-mm segment of jugular vein on the common carotid artery. The proximal end-to-side suture was performed by eight interrupted stitches; the distal suture, which was done using a laser welding technique, was supported by four stay sutures. The animals were evaluated after 2 days (n = 15), 9 days (n = 15), and 30 days (n = 10). The vascular segments were excised and subjected to histological, immunohistochemical, and ultrastructural evaluation. RESULTS: The average clamping time to perform both anastomoses was 35 minutes. At the end of the follow-up period, one case of complete occlusion was observed after 9 days and one case was observed after 30 days. Surgical observations and pathological evaluation indicated that adoption of the laser welding technique reduced operative time and bleeding. Histologically, a reduction of thrombosis, inflammation, myointimal hyperplasia, and dystrophic calcification was observed in laser-assisted anastomoses. A better preservation of the endothelium was also evident in laser-treated anastomoses. The observed differences were deemed statistically significant ( P= 0.05). CONCLUSION: Our study demonstrated the efficacy of diode laser welding in improving surgical techniques of high-flow bypass and in reducing the vascular wall damage observed with conventional methods.

La Regione Emilia Romagna ha avviato uno studio per definire linee guida ufficiali da utilizzare per il recupero delle cave in ambito fluviale in un tratto omogeneo del fiume Po, che attraversa le provincie di Piacenza, Parma e Reggio Emilia.In parallelo è stato avviato un progetto pilota nel quale le linee guida sono applicate alla riqualificazione ambientale di un sito di cava situato nel comune di Roccabianca (Parma).Si tratta di temi qualificativi perchè investono contemporaneamente problemi di ricerca e di recupero e ricostruzione di ambienti degradati per i quali mancano ancora conoscenze di base che consentano di individuare criteri e linee guida per una gestione sostenibile dell'attività estrattiva. Lo sviluppo delle linee guida è stato basato sui risultati di ricerche pregresse,svolte prevalentemente in alcuni laghi di cava e di zone umide perifluviali naturali situati nello stesso ambito fluviale. Ciò ha permesso di delineare alcune significative connessioni funzionali tra processi ecosistemici (produzione e decomposizione, ciclo dei nutrienti, successione delle biocenosi) in rapporto alla variabilità dei parametri morfometrici ed idrologici. In parallelo le conoscenze delle caratteristiche ecologiche nei tratti meno degradati del corso del fiume, ad esempio nei siti della Rete 2000, hanno costituito un primo riferimento e hanno fornito modelli per la riqualificazione naturalistica degli ambienti sia acquatici che terrestri. Le linee guida contengono una serie di criteri e indicazioni riferibili a 1) prescrizioni di sicurezza idraulica;2)morfologia della cuvetta lacustre,profondità di scavo,connettività idraulica col fiumee qualità delle acque;3)recupero e ricostruzione di habitat acquatici e terresti per la conservazione e lo sviluppo della diversità floristica e faunistica;4)ricostruzione delle connettività laterale tra alveo bagnato e piana golenale;5)avvia della realizzazione di una rete ecologica fluviale; 6)identificazione edi destinazioni d'uso ricreative, turistico e agricolo produttive in chiave di sostenibilità ambientale.

Background. The nature and the cell of origin of lesions variously described as inflammatory pseudotumour (IP), pseudosarcomatous proliferation or inflammatory myofibroblastic tumour (IMT) are still debated. Most cases behave as benign, reactive lesions, but rarely may have an aggressive course. We analysed the clinicopathological and molecular features of a series of IMTs of the respiratory tract, with the aim to verify if they represent a single entity or a heterogeneous group of lesions. Methods. Histological features of thirty-eight cases diagnosed as IMT or IP in various Institutions were reviewed. Immunohistochemistry was performed for the following antibodies: CAM 5.2, MNF116, CD34, CD21, CD35, betacatenin, CD68, HHV8 and 6, smooth muscle actin, desmin, ALK, ki67, IgG4. Molecular investigations included in situ hybridization for EBV and DNA sequencing for c-kit, cmet, EGFR, PDGFR alpha and beta. Results. Histologically, tumours were subdivided in two groups: type A (8 cases) included lesions with a predominance of inflammatory cells and no prominent spindle cell proliferation; type B (27 cases) included lesions composed of uniform proliferation of spindled cells, sometimes with myxoid features. Three cases had borderline features between type A and B and were classified as “type AB”. All cases were negative for cytokeratins, HHV8, HHV6 and CD21 and beta-catenin. Cases from all subtypes were variously positive with muscular markers, CD34, CD68 and IgG4. ALK antibody stained only 4 cases of B type. No cases showed mutations in the genes studied.Conclusions. Under the designation of IMT or IP are actually included different lesions. In the present series, we identify histologically two main groups: one which represents more likely true myofibroblastic tumoral proliferations, and a group which is represented by non-tumoural, reactive lesions. At moment, however, it appears that there are no immunohistochemical or molecular markers to distinguish the two types of lesions.

Il contributo analizza le risposte date dal campione di 2.346 associati alle organizzazioni di Terzo Settore in Italia (campione nazionale rappresentativo statisticamente) al fine di ricostruire gli orientamenti di valore che orientano l’agire dei membri delle Ops italiane. Già molti fatti e riflessioni sono emerse nei capitoli precedenti del volume e pertanto quanto si potrà leggere in queste pagine si inserisce in un quadro interpretativo più ampio e già noto. Il fine specifico di questo contributo è pertanto quello di trovare elementi che corroborino o, al contrario, respingano la tesi della rilevanza del «fattore religione» nell’associazionismo di Terzo Settore. Com’è noto, questa tesi è già stata discussa nel corso di ricerche che costituiscono i precedenti logici e cronologici della presente. In queste pagine si farà riferiemnto, pertanto, alla tesi di Donati, secondo cui il Cristianesimo sta alla base della società civile, in quanto promuove il rispetto e la considerazione verso la persona umana. L’apporto del pensiero cristiano ad una «semantica dopo-moderna del civile come fattore di civilizzazione» è dunque del tutto rilevante per la presente analisi [Donati in Id. e Colozzi, a cura di, 2001: 38-50]. Analogamente lo è la seguente tesi di Ivo Colozzi, che fra le tante variabili che intervengono nella formazione di un modello di “cultura civica”, le due più rilevanti sono l’orientamento alla religione e l’esperienza di appartenenza ad una associazione di privato sociale [Colozzi , in Donati e Id., a cura di, 2001: 43-44]. Rispetto a queste precedenti ricerche, la presente è caratterizzata dal fatto che il questionario è stato strutturato secondo la teoria multidimensionale della religiosità, formulata alla metà degli anni ’60 da Charles Y. Glock e da Rodney Stark [1966] e applicata, in tempi più recenti, anche in Italia [Martelli e coll. 19952; Pizzuti e al., a cura di, 1998]. In questo capitolo si analizzano congiuntamente due aspetti rilevanti del sistema simbolico-culturale: religione e comunicazione. L’esame congiunto di questi due aspetti, solitamente trascurato negli studi sociologici del nostro Paese, è motivato dalla convinzione che solo una prospettiva sinergica di maggiore collaborazione tra communication research e sociologia della religione potrà offrire un quadro attendibile dei mutamenti socio-culturali in atto
