4. The Effect of Varenicline on Smoking Cessation in Hospitalized Patients: A Systematic Review and Meta-Analysis.

Abstract

Background: Varenicline tartrate is a new and selective agonist of the nicotinic acetylcholine receptor (nAChR). This systematic review and meta-analysis aimed to determine varenicline efficacy in smoking cessation among hospitalized patients. Methods: We looked through worldwide databases such as Web of Science, Embase, PubMed, Cochrane, and Scopus. Relevant pieces of research published on varenicline efficacy on smoking cessation among hospitalized patients were discovered using proper keywords. The data were analyzed using Stata software version 14 and a random-effects model meta-analysis. Findings: Nine studies were eligible to be included in this study, with a total sample size of 2131. Generally, the point abstinence rate was significantly greater in the varenicline group than in the placebo group at weeks 12 (odds ratio [OR] = 0.59; 95% CI: 053-0.65; P < 0.001), 24 (OR = 0.78; 95% CI: 0.72-0.84; P < 0.001), and 52 (OR = 0.86; 95% CI: 0.80-0.92; P < 0.001). Furthermore, the continuous abstinence rate for weeks 4 (OR = 0.70; 95% CI: 019-0.54; P = 0.000), 12 (OR = 0.26; 95% CI: 019-0.54; P < 0.001), 24 (OR = 0.32; 95% CI: 019-0.53; P < 0.001), and 52 (OR = 0.32; 95% CI: 019-0.54; P < 0.001) was significantly greater in the varenicline group than in the placebo group. Conclusion: According to the high efficacy of varenicline in both short- and long-term smoking settings and considering the importance of smoking cessation in high-risk hospitalized patients, varenicline consumption could be considered as a main smoking cessation strategy in these patients. Abstract

This study innovatively combines a set of methods to provide a new way to assess the economic potential of pumped hydro energy storage. It first provides a method based on geographic information systems to study the potential of pumped-hydro for different topologies. Second, using cost estimates for each identified site, cost-potential curves are derived. Finally, these curves are used for planning a fully renewable system to assess their impact on investment recommendations. Applications to Chile, Peru, and Bolivia show the usability of the methods. Over 450 pumped-hydro locations are identified, totaling around 20 TWh (1600 GW of installed capacity with 12 h of storage). These numbers exceed by 20-fold the projected daily energy demand of the corresponding countries. When taking into account investment costs, most locations are cheaper than current Li-ion batteries, but only some are expected to remain competitive. When using the resulting cost-potential curves to design a future energy system, the planning tool recommends about 1.6 and 5.0 times more pumped-hydro storage compared to using average values and literature values, respectively. Abstract

Carbon Monoxide (CO) poisoning is one of the most common causes of death in Iran. The present study investigated the indoor and outdoor CO concentrations at hospitals under the supervision of Mazandaran University of Medical Sciences in Sari, Iran. Indoor and outdoor measured CO were conducted over a period of one month during the 2012 winter season in 4 hospital kitchens using a direct measurement device specialized for CO. The average concentrations of CO gas through gas measuring stations were considered at three stations both in and out of the kitchens. The highest and the lowest average concentrations of outdoor CO in Fatemeh Zahra Hospital (about 2 ppm) and Zare Hospital (about 0.33 ppm) were measured respectively. Besides, the highest and the lowest average concentrations of indoor CO were detected in Imam Hospital (as 9.66 ppm) and BuAli Hospital (as7.66 ppm). The highest ratios of I/O (approximately 10 ppm) have been conducted between 8:00 and 9:00a.m. The lowest median concentrations of CO (approximately 8 ppm) between 11:00 and 12:00 a.m. have been measured. Concentrations of CO in all hospitals in this study were significant. This fact shows poorly ventilation of kitchen due to lack of ventilation design engineering. [ABSTRACT FROM AUTHOR]
